Abstract:Aim: To determine the circulatory procalcitonin levels, lipid profile and uric acid levels in patients presenting with preeclampsia and to correlate these with clinical severity of the disease. Methodology: This double blinded case-control study was conducted for two years in department of Obstetrics and Gynecology in collaboration with the department of biochemistry, Government Institute of Medical Sciences, Greater Noida, UP. This study included 80 subjects out of 40 were in group I (Control) and other 40 were in group II (PE patients) admitted in the hospital in third trimester of pregnancy. General demographic and clinical data such as age, gestational week, number of gestation, and BMI were recorded, and the results of routine blood cell counts, PCT and other indicators were also obtained. The PE patients were women younger than 40 years old who met the diagnostic criteria for PE in the guidelines for diagnosis and treatment of hypertensive diseases in pregnancy-2014. The healthy pregnant women were between 24 and 40 years old, with a single fetus, normal fetal development, normal liver and kidney function, and without eclampsia, gestational diabetes mellitus, fetal growth restriction, or history of other adverse pregnancy conditions.

Abstract:<p>This meta‐analysis aims to compare serum uric acid levels among preeclamptic and healthy pregnant women across the various trimesters and provide a summary of the effect size of this biomarker in predicting adverse pregnancy outcomes. MEDLINE, Scopus, CENTRAL, Clinicaltrials.gov, and Google Scholar databases were systematically searched from inception. Observational studies were held eligible if they reported serum uric acid among preeclamptic and healthy pregnant women. Meta‐analysis was conducted regarding uric acid concentration, diagnostic accuracy, and association with perinatal outcomes. The credibility of evidence was appraised using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) framework. The analysis included 196 studies, comprising 39 540 women. Preeclampsia was associated with significantly elevated uric acid levels during the 1st (mean difference [MD]: 0.21 mg/dL, 95% confidence intervals [CI]: 0.06‐0.35) trimester, 2nd (MD: 1.41 mg/dL, 95% CI: 0.78‐2.05) trimester, and 3rd (MD: 2.26 mg/dL, 95% CI: 2.12‐2.40) trimester. Higher uric acid was estimated for severe preeclampsia, eclampsia, and hemolysis, elevated liver enzymes, low platelet syndrome. The sensitivity for adverse perinatal outcome prediction ranged from 67.3% to 82.7% and the specificity from 47.7% to 70.7%. In conclusion, it is suggested that serum uric acid levels are increased in preeclampsia and can be used to predict disease severity and pregnancy complications. Future prospective studies should verify these outcomes, assess the optimal cutoffs, and incorporate uric acid to combined predicting models.</p>

Abstract:Background The disorder of serum uric acid metabolism during pregnancy is associated with adverse pregnancy outcomes,while few studies have analyzed and compared the correlation of serum uric acid and serum uric acid/creatinine ratio levels with adverse pregnancy outcomes.Objective To explore the correlation of serum uric acid and serum uric acid/creatinine ratio levels with adverse pregnancy outcomes in late pregnancy.Methods A total of 743 pregnant women with singleton live births who were routinely examined and delivered at Nanjing Drum Tower Hospital from 2015 to 2022 were selected.Based on the occurrence of adverse pregnancy outcomes,pregnant women were divided into normal group(344 cases)and adverse outcome group(399 cases).Serum uric acid and serum uric acid/creatinine ratio were divided into three levels by quartiles,respectively,including Q1(serum uric acid<257 μmol/L),Q2(serum uric acid 257-359 μmol/L),Q3(serum uric acid>359 μmol/L)and q1(serum uric acid/creatinine ratio<5.88),q2(serum uric acid/creatinine ratio 5.88-7.94),q3(serum uric acid/creatinine ratio>7.94).According to the median age of pregnant women,they were divided into the age subgroup<30 years old(341 cases)and age subgroup≥30 years old(402 cases).Based on previous pregnancies and deliveries,they were divided into the primiparous subgroup(539 cases)and multiparous subgroup(194 cases).The correlation of serum uric acid and serum uric acid/creatinine ratio levels with adverse pregnancy outcomes was analyzed by using multivariate Logistic regression.Results Pregnant women in adverse outcomes group were older with higher levels of BMI,serum uric acid,serum uric acid/creatinine ratio and triglycerides than those in the normal group(P<0.05).After adjustment for confounders,the multivariate Logistic regression results of the effect of serum uric acid and serum uric acid/creatinine ratio on adverse pregnancy outcomes showed that,compared to Q1 serum uric acid level,the risk of preeclampsia(AOR=4.41,95%CI=2.16-8.99)and intrauterine growth restriction(AOR=3.59,95%CI=1.08-11.96)increased at Q3 serum uric acid level(P<0.05);compared to q1 serum uric acid/creatinine ratio level,the risk of preeclampsia(AOR=2.33,95%CI=1.13-4.79;AOR=3.56,95%CI=1.68-7.56)increased at q2 and q3 serum uric acid/creatinine ratio levels.Compared to q1 serum uric acid/creatinine ratio level,the risk of preterm labor(AOR=2.76,95%CI=1.33-5.71)and intrauterine growth restriction(AOR=5.15,95%CI=1.39-19.14)increased,while the risk of macrosomia(AOR=0.43,95%CI=0.19-0.98)and large for gestational age(AOR=0.38,95%CI=0.15-0.96)decreased(P<0.05).The results of the effect of serum uric acid and serum uric acid/creatinine ratio on preeclampsia and preterm labor in different age subgroups showed that,compared to Q1 serum uric acid level,the risk of preeclampsia increased in both age subgroups at Q3 serum uric acid level(P<0.05);compared to q1 serum uric acid/creatinine ratio level,the risk of preeclampsia increased among women aged≥30 years at q2 and q3 serum uric acid/creatinine ratio level(P<0.05).The results of the effect of serum uric acid and serum uric acid/creatinine ratio on preeclampsia and preterm labor in different pregnancies and deliveries subgroups showed that compared to Q1 serum uric acid level,the risk of preeclampsia increased at Q3 serum uric acid level among primiparous women(P<0.05);compared to q1 serum uric acid/creatinine ratio level,the risk of preeclampsia increased at q2 and q3 serum uric acid/creatinine ratio level among primiparous women,as well as the risk of preterm birth at q3 serum uric acid/creatinine ratio level among primiparous women(P<0.05).Conclusion Elevated levels of serum uric acid and serum uric acid/creatinine ratio were associated with the risk of preeclampsia and intrauterine growth restriction,in which preeclampsia mainly occurred in pregnant women aged≥30 years or primiparous women.The risk of preterm labor was increased at high levels of serum uric acid/creatinine ratio,primarily in primiparous women.Serum uric acid/creatinine ratio predicted more adverse pregnancy outcomes than serum uric acid.

Abstract:Objectives: Study was designed to find out the Correlation of serum uric acid with renal function parameters in Preeclampsia. Study Design: Cross Sectional study. Setting: Sir Ganga Ram Hospital Lahore. Period: July 2016 to July 2017. Material & Methods: Level of serum uric acid, serum creatinine and blood urea of 40 Preeclamptic women and 30 gestation-matched normotensive controls were estimated. Their Demographic and clinical characteristics were noted. The blood sample was analyzed for biochemical parameters, blood urea, serum uric acid, serum creatinine and urinary protein. Result: Mean age and gestational age of women was 25 weeks with BMI 29 Kg/m2. Level of serum uric acid and blood urea and serum creatinine were increased, but significant difference only observed with serum uric acid and blood urea with marked proteinuria. An inverse relationship of serum uric acid with urinary protein was observed. A direct relationship, of serum uric acid with serum creatinine was observed. Conclusion: it is concluded that estimation of parameters of renal function of preeclamptic women are important along with hyperuicaemia.

Abstract:Purpose: This study evaluated the association between maternal serum uric acid-to-creatinine ratio (SUA/SCr) in the first trimester and adverse maternal and neonatal outcomes. Methods: A prospective birth cohort study was conducted between 2018 and 2021. Logistic regression models and restricted cubic splines were utilized to estimate the associations between the SUA/SCr ratio and feto-maternal pregnancy outcomes. Women were stratified according to maternal age and pre-pregnancy body mass index. Results: This study included 33,030 pregnant women with live singleton pregnancies. The overall prevalence of gestational diabetes mellitus (GDM), pregnancy-induced hypertension (PIH), cesarean delivery, preterm birth, large-for-gestational age (LGA), small-for-gestational age, and low Apgar scores were 15.18%, 7.96%, 37.62%, 4.93%, 9.39%, 4.79% and 0.28%, respectively. The highest quartile of SUA/SCr was associated with the highest risk of GDM (odds ratio [OR] 2.14, 95% CI 1.93-2.36), PIH (OR 1.79, 95% CI 1.58-2.04), cesarean delivery (OR 1.24, 95% CI 1.16-1.33), and preterm birth (OR 1.30, 95% CI 1.12-1.51). The associations between SUA/SCr with adverse pregnancy outcomes showed linear relationships except for GDM (P < 0.001 for all, P < 0.001 for non-linearity). Subgroup analyses revealed that the associations between the SUA/SCr ratio and the risks of PIH and LGA were significantly stronger in younger pregnant women (P = 0.033 and 0.035, respectively). Conclusion: Maternal SUA/SCr levels were associated positively with the risk of adverse pregnancy outcomes. Timely monitoring of SUA and SCr levels during early pregnancy may help reduce the risk of adverse pregnancy outcomes and provide a basis for interventions.

Abstract:BACKGROUND: It is well-known that estimation of 24-h urine protein and spot urine protein/creatinine (P/C) ratio are commonly performed investigations to assess proteinuria in preeclamptic women. Serum uric acid has been shown to correlate well with disease severity in preeclampsia.MATERIALS AND METHODS: A total of 24-h urine protein estimation, spot urine P/C ratio, and serum uric acid measurements were carried out in 75 pregnant preeclamptic women and the correlation between these investigations, as also the association between proteinuria and hyperuricemia with adverse fetomaternal outcomes were studied.RESULTS: Pearson's correlation test showed a positive correlation between 24-h urine protein and spot urine P/C ratio. A statistically significant and direct correlation was also found between serum uric acid and spot urine P/C ratio, while there was no statistically significant difference between proteinuria and hyperuricemia with respect to the various fetal and maternal outcome parameters studied.CONCLUSION: In the present study, we found a moderate correlation between 24-h urine protein and spot urine P/C ratio. There was a moderate correlation between spot urine P/C ratio and uric acid, while there was no statistical significance of the association between proteinuria and uric acid with fetomaternal outcomes in preeclampsia.

Abstract:Chronic inhibition of the renin angiotensin system prevents increased BP and renal injury in N(G)-nitro-L-arginine methyl ester (L-NAME) hypertension. However, a relationship between plasma renin activity and the protective effect of chronic angiotensin II (Ang II) blockade has not been established. With this background, this study was undertaken to evaluate how the chronic administration of deoxycortisone acetate (DOCA) modifies the effects of losartan on BP, renal injury, and other variables in L-NAME hypertensive rats. The following groups were used: Control, DOCA, L-NAME, L-NAME + losartan, L-NAME + DOCA, and L-NAME + DOCA + losartan. Tail systolic BP was measured twice a week. After 4-wk evolution, mean arterial pressure and metabolic, morphologic, and renal variables were measured. The final mean arterial pressure values were 116 +/- 6 mmHg for control, 107 +/- 2 mmHg for DOCA, 151 +/- 5 mmHg for L-NAME, 123 +/- 2 mmHg for L-NAME + losartan, 170 +/- 3 mmHg for L-NAME + DOCA, and 171 +/- 5.5 mmHg for L-NAME + DOCA + losartan. Losartan prevented microalbuminuria, hyaline arteriopathy, and glomerulosclerosis of L-NAME hypertension but was ineffective in L-NAME + DOCA-treated rats. Plasma protein was significantly reduced in the L-NAME + DOCA group when compared with control and L-NAME groups, whereas no significant differences were observed in the other groups. Plasma renin activity was suppressed in the DOCA (0.55 +/- 0.2) and L-NAME + DOCA (0.60 +/- 10.2) groups but unsuppressed in the L-NAME + DOCA + losartan group (5.8 +/- 1). The conclusion is that DOCA blocks the preventive effect of losartan on the increased BP and renal injury of L-NAME hypertension, which suggests that DOCA transforms L-NAME hypertension into an Ang II-independent model of hypertension. These data also suggest that losartan prevents L-NAME hypertension by blocking the activity of systemic Ang II.
Study of C-reactive Protein Levels in Hypertensive Disorders of Pregnancy

Abstract:Introduction Globally, one of the main causes of maternal and perinatal deaths is hypertensive disorders of pregnancy (HDP). Preeclampsia is a pregnancy-specific syndrome characterized by high blood pressure and proteinuria after 20-week gestation. Women who develop preeclampsia are at increased risk for the development of many systemic complications. Materials and methods A cross-sectional study was conducted in the Department of Biochemistry, SHKM, Nuh, Haryana in collaboration with the Department of Obstetrics & Gynecology. 200 study subjects; Group 1: cases - 100 HDP, Group 2: controls - 100 age-matched normotensive pregnant women. The specimen of 3 mL venous blood was collected under aseptic precautions. The data was evaluated using statistical evaluation tools. Result Out of 200 subjects, mean C-reactive protein (CRP) levels were normal in three cases and 35 controls. Minor elevation of CRP was observed in 11 cases and 29 controls (very significant). Moderate elevation of CRP was observed in 61 cases and 36 controls (highly significant). Marked elevation of CRP was observed in 25 cases. Mean CRP levels increased from preeclampsia without severe features (3.9 mg/dL), preeclampsia with severe features (5.2 mg/dL), and eclampsia (12.7 mg/dL) when compared with the respective control group (1.1 mg/dL, 1.16 mg/dL, and 1.2 mg/dL). Conclusion There is a highly significant association between CRP levels and HDP. Additionally, one useful indicator of the severity of preeclampsia is CRP, a measure of systemic inflammation. Elevated CRP levels in mild PE can be used as an indicator of the progress of the disease and early prevention can be done.

Several potential origins for increased UA levels include abnormal renal function, increased tissue breakdown, and increased activity of the enzyme Xanthine Oxidase (XO). The aim of the study was to determine serum levels of UA and XO not only in maternal serum, but also in umbilical vein (UV) and umbilical artery (UA) and explore their possible role in PE development.

Abstract:BACKGROUND: Serum uric acid levels are elevated in polycystic ovary syndrome, however, the relationship between serum uric acid level and reproductive outcomes in women with polycystic ovary syndrome remains unclear.OBJECTIVE: This study aimed to investigate the association between serum uric acid level and the reproductive outcomes in women with polycystic ovary syndrome undergoing in vitro fertilization or intra-cytoplasmic sperm injection embryo transfer cycles.STUDY DESIGN: This was a retrospective cohort study performed at a university-affiliated reproductive medicine center. A total of 1903 women with polycystic ovary syndrome undergoing their first in vitro fertilization or intracytoplasmic sperm injection embryo transfer cycles between January 2010 and January 2021 were initially included. The trends for repro-ductive outcomes in polycystic ovary syndrome across quartiles of serum uric acid levels were assessed. A logistic regression analysis was per-formed to obtain the odds ratios for in vitro fertilization outcomes based on the quartiles of serum uric acid with or without adjusting for potential confounding variables. Using generalized additive models, serum uric acid was further treated as its original continuous property to visualize its nonlinear relationship with in vitro fertilization outcomes. The live birth rate was the main outcome.RESULTS: After exclusions, a total of 883 women with polycystic ovary syndrome with their first fresh-embryo transfer cycles were included. In quartiles of serum uric acid levels, there was a significant decreasing trend in the live birth rate from the lowest quartile (Q1: 61.8%) to the highest (Q4: 45.9%) (Ptrend=.002). The percentage of low birthweight increased from Q1 (22.3%) to Q4 (31.7%) (Ptrend=.049). Compared with those in Q1, women in Q4 showed a significant lower probability of live birth and clinical pregnancy and a higher risk for low birthweight (all P <.05). Both the unadjusted and adjusted generalized additive models indicated that as the serum uric acid level increased, the probability of clinical pregnancy and the live birth rate exhibited an overall decreasing profile, and the risk for low birthweight showed an increasing profile.CONCLUSION: An elevated serum uric acid level is associated with decreased probabilities of live birth and clinical pregnancy and an increased risk for low birthweight in women with polycystic ovary syn-drome. However, these associations may be confounded by other factors and more well-designed studies are needed to confirm these findings in the future.
Serum urea acid and urea nitrogen levels are risk factors for maternal and fetal outcomes of pregnancy: a retrospective cohort study

Abstract:Background: In recent years, results on the association between serum uric acid (UA) and pregnancy outcomes have been inconsistent, and the association between urea nitrogen (UN) and adverse pregnancy outcomes in normal pregnant women has not been reported. Thus, we examined the association of UA and UN levels during gestation with the risk of adverse pregnancy outcomes in a relatively large population. Methods: A total of 1602 singleton mothers from Union Shenzhen Hospital of Huazhong University of Science and Technology at January 2015 to December 2018 were included. Both UA and UN levels were collected and measured during the second (16-18th week) and third (28-30th week) trimesters of gestation respectively. Statistical analysis was performed using multivariate logistic regression. Results: After adjustment, the highest quartile of UA in the third trimester increased the risk of premature rupture of membranes (PROM) and small for gestational age infants (SGA) by 48% (odds ratio [OR]: 1.48, 95% confidence interval [CI]: 1.04-2.10) and 99% (95% CI: 1.01-3.89) compared to those in the lowest quartile. The adjusted OR (95% CI) in the highest quartile of UN for the risk of SGA was 2.18 (95% CI: 1.16-4.13) and 2.29 (95% CI: 1.20-4.36) in the second and third trimester, respectively. In the second trimester, when UA and UN levels were both in the highest quartile, the adjusted OR (95% CI) for the risk of SGA was 2.51 (95% CI: 1.23-5.10). In the third trimester, when the group 1 (both indicators are in the first quartile) was compared, the adjusted ORs (95% CI) for the risk of SGA were 1.98 (95% CI: 1.22-3.23) and 2.31 (95% CI: 1.16-4.61) for group 2 (UA or UN is in the second or third quartile) and group 3 (both indicators are in the fourth quartile), respectively. Conclusions: Higher UA and UN levels increased the risk of maternal and fetal outcomes. The simultaneous elevation of UA and UN levels was a high-risk factors for the development of SGA, regardless of whether they were in the second or third trimester.
High serum uric acid level is a mortality risk factor in peritoneal dialysis patients: a retrospective cohort study

Abstract:Background The results remain controversial with regards to the impact of serum uric acid on clinical outcomes from peritoneal dialysis population. The aim of our study was to investigate the influence of serum uric acid levels on mortality in peritoneal dialysis patients. Methods Data on 9405 peritoneal dialysis patients from the Zhejiang Renal Data system were retrospectively analyzed. All demographic and laboratory data were recorded at baseline. The study cohort was divided into quintiles according to baseline uric acid level (mg/dL): Q1 (< 6.06), Q2 (6.06–6.67), Q3 (6.68–7.27) (reference), Q4 (7.28–8.03), and Q5 (≥8.04). Hazards ratio (HR) of all-cause and cardiovascular mortality was calculated. Results Mean serum uric acid was 7.07 ± 1.25 mg/dL. During a median follow-up of 29.4 (range, 3.0 to 115.4) months, 1226 (13.0%) patients died, of which 515 (5.5%) died of cardiovascular events. The Kaplan-Meier survival curves showed that patients in the middle uric acid quintile (Q3: 6.68–7.27) exhibited the highest patient and cardiovascular survival rates (log-rank test P < 0.05). Multivariate Cox regression analysis showed that, using Q3 as the reference, in the fully adjusted model, a higher uric acid level (Q4: 7.28–8.03, and Q5: ≥8.04) was significantly associated with higher all-cause mortality (Model 3; Q4: HR, 1.335, 95% CI, 1.073 to 1.662, P = 0.009; Q5: HR, 1.482, 95% CI, 1.187 to 1.849, P = 0.001), but not with cardiovascular mortality. The adverse effect of higher uric acid level (≥7.28 mg/dL) on all-cause mortality was more prominent in groups such as male, hypoalbuminemia, normal weight, non-diabetes mellitus at baseline rather than in their counterparts respectively. Conclusions A higher uric acid level was an independent risk factor for all-cause mortality in peritoneal dialysis patients.

Abstract:AimsHigher serum uric acid (UA) has been associated with increased risk of Type 2 diabetes mellitus. This cohort study examined whether there are any associations between serum UA in early pregnancy and the subsequent risk of gestational diabetes mellitus (GDM).MethodsThis cohort study was conducted in Shanghai, China, and included 85,609 pregnant women. Generalised additive models were used to estimate the associations of serum UA with risk of GDM.ResultsThe prevalence of GDM was 14.0% (11,960/85,609). Non-linear associations between serum UA and GDM risk were observed and these associations varied by gestational ages. Only elevated serum UA levels at 13–18 weeks gestation was associated with substantially increased risk of GDM. Analysis by UA quintiles at 13–18 weeks gestation showed the odds ratios for GDM were 1.11 (95%CI, 1.03–1.20) for the second, 1.27 (95%CI, 1.17–1.37) for the third, 1.37 (95%CI, 1.27–1.48) for the fourth and 1.70 (95%CI, 1.58–1.84) for the fifth quintile of serum UA in comparison with the first quintile. Stratified analysis showed the associations of serum UA with GDM were stronger among pregnant women aged 35 years or older.ConclusionWe found higher serum UA at 13–18 gestational weeks was a risk factor for GDM. Our findings provide new evidence for the role of serum UA in the prevention and early intervention of GDM, and highlighted the need for monitoring serum UA at 13–18 gestational weeks.

Abstract:A systematic review and meta‐analysis of cohort studies including 105,380 participants was conducted to investigate the relationship between uric acid levels during pregnancy and the risk of gestational diabetes. Our results showed a positive association between uric acid levels and the risk of gestational diabetes. Measuring uric acid levels before 20 weeks of gestation can potentially predict gestational diabetes, especially in younger women. Aims/Introduction Although the association between uric acid levels and adverse pregnancy outcomes has been investigated, the effects of higher uric acid levels on the risk of gestational diabetes mellitus (GDM) have yet to be established. Therefore, this systematic review and meta‐analysis aimed to investigate the relationship between uric acid levels during pregnancy and the risk of GDM. Materials and Methods PubMed/Medline, Scopus and Web of Science databases were searched up to April 2022 for relevant observational studies. Early Prediction of Hypertensive Diseases of Pregnancy by Using Combined Screening Methods in a Rural Population

Abstract:Introduction: The most frequent medical issue during pregnancy is hypertension, which can complicate up to 10% to 15% of pregnancies worldwide. An estimated 14% of all maternal fatalities worldwide are thought to be caused by hypertensive disease of pregnancy, one of the main causes of maternal and fetal morbidity and mortality. Despite the fact that maternal mortality is substantially lower in high-income countries than in low- and middle-income countries, hypertension is still one of the leading causes of maternal death globally. Maternal mortality associated with hypertension fluctuated between 0.08 and 0.42 per 100,000 births between 2009 and 2015. In India, the estimated overall pooled prevalence of HDP was determined to be one out of 11 women, or 11% (95% CI, 5%-17%). Despite various government programs, there is still a high prevalence of hypertension, which calls for stakeholders and healthcare professionals to focus on providing both therapeutic and preventive care. The best solution is to concentrate more on the early detection of pregnancy-related hypertension and to guarantee its universal application so that proper care can be carried out to prevent maternal and fetal morbidity. Aim: To estimate the predictive value of the combination of maternal characteristics, i.e., mean arterial pressure (MAP), biophysical evaluation (uterine artery Doppler), and biochemical markers (pregnancy-associated plasma protein A (PAPP-A)), in the first trimester of pregnancy for hypertensive diseases of pregnancy. Methodology: It was a prospective observational study of longitudinal variety that took over 18 months in a tertiary care rural hospital. The number of women admitted to the hospital for labor care during 2019 was 5261. A total of 513 were diagnosed with hypertensive illnesses during pregnancy. At a prevalence rate of 10%, we calculated a sample size of 350 to achieve a sensitivity of 85% with an absolute error of 12.5% at a 95% CI. Maternal histories, such as age, education, socio-economic status, gravidity, and BMI, were taken along with three parameters, i.e., MAP, which was significant above 90 mmHg, uterine artery Doppler, which was taken significant above 1.69, and serum PAPP-A, which was significant at less than 0.69 ml/IU. Observation and results: We have found that the following are associated with the prediction of hypertension: among the maternal characteristics are advanced age >35 years, presence of body edema, and urine proteins along with MAP, uterine artery pulsatility index (UtA-PI), and PAPP-A are significant. The predictive accuracy of the combination of MAP, UtA-PI, and PAPP-A is also significant. We also found that there is a significant increase in cesarean sections and NICU admissions in hypertensive patients. Conclusion: A combination of screening parameters, including MAP, UtA-PI, and PAPP-A, to predict early hypertensive disease of pregnancy is developed and tested.
Uric acid as a mediator in the correlation between white blood cells and preeclampsia severity: a retrospective cohort study

Abstract:This study aimed to analyze the independent risk factors for predicting preeclampsia severity and explore its underlying mechanism. Clinical data of patients with preeclampsia were collected from the Medical Information Mart for Intensive Care (MIMIC)-IV database. Univariate and multivariate analyses were employed to assess the significant factors associated with preeclampsia severity. Additionally, we performed multivariate logistic regression analysis and mediation analysis to investigate the potential regulatory path. Based on inclusion and exclusion criteria, 731 participants were enrolled: severe preeclampsia (n = 381) and mild to moderate preeclampsia (n = 350). Age, white blood cells (WBC), platelet, creatinine, albumin, uric acid, aspartate aminotransferase, alanine aminotransferase, international normalized ratio, and prothrombin time were significantly related to preeclampsia severity. Besides, hospital length of stay was significantly higher in the severe group. Notably, age and uric acid were independent predictors for preeclampsia severity. Further, WBC and creatinine were significantly associated with uric acid. Finally, the mediation analysis showed that uric acid was a mediator of the relationship between WBC and preeclampsia severity. In conclusion, WBC might affect preeclampsia severity and progression via the mediation of uric acid. This study might provide novel insight into preventing preeclampsia development.

Abstract:Background: Pregnancy is a physiological condition in which the maternal environment undergoes many changes. Serum uric acid (UA) levels have been used for the early diagnosis of preeclampsia, predictor of reduced birth weight and fetal outcome. UA is also expressed in saliva, and collection of saliva sample is a noninvasive method which will be more acceptable by the patients. Aims and objectives: The present study aimed to estimate and compare serum and salivary UA levels in age-matched healthy nonpregnant and healthy pregnant women at different trimesters longitudinally. Methodology: Forty female participants with age ranging between 20 and 38 years comprised the study population. The study group consisted of 20 healthy nonpregnant women (controls) and an equal number of confirmed cases of healthy pregnant women in the first trimester (cases). The cases were followed in their second and third trimesters for the sample collection. Results: Both serum and salivary UA levels were significantly reduced in the first trimester of pregnancy than the controls. In the second and third trimesters, the values of serum and salivary UA levels gradually increased and gained values similar to that of nonpregnant women. Salivary UA levels showed a highly significant positive correlation with serum levels in both controls and cases. Conclusion: Salivary estimation, being a noninvasive procedure, is easily accepted by the patients and carries minimal risk of exposure to the blood-borne pathogens compared to serum estimation. Our findings warrant the use of saliva instead of blood for UA estimation.
