Abstract:Gait impairments are among the most common hallmarks of Parkinson's disease (PD), usually appearing in the early stage and becoming a major cause of disability with disease progression. Accurate assessment of gait features is critical to personalized rehabilitation for patients with PD, yet difficult to be routinely carried out as clinical diagnosis using rating scales relies heavily on clinical experience. Moreover, the popular rating scales cannot ensure fine quantification of gait impairments for patients with mild symptoms. Developing quantitative assessment methods that can be used in natural and home-based environments is highly demanded. In this study, we address the challenges by developing an automated video-based Parkinsonian gait assessment method using a novel skeleton-silhouette fusion convolution network. In addition, seven network-derived supplementary features, including critical aspects of gait impairment (gait velocity, arm swing, etc.), are extracted to provide continuous measures enhancing low-resolution clinical rating scales. Evaluation experiments were conducted on a dataset collected with 54 patients with early PD and 26 healthy controls. The results show that the proposed method accurately predicted the patients' unified Parkinson's disease rating scale (UPDRS) gait scores (71.25% match on clinical assessment) and discriminated between PD patients and healthy subjects with a sensitivity of 92.6%. Moreover, three proposed supplementary features (i.e., arm swing amplitude, gait velocity, and neck forward bending angle) turned out to be effective gait dysfunction indicators with Spearman correlation coefficients of 0.78, 0.73, and 0.43 matching the rating scores, respectively. Since the proposed system requires only two smartphones, it holds a significant benefit for home-based quantitative assessment of PD, especially for detecting early-stage PD. F- rthermore, the proposed supplementary features can enable high-resolution assessments of PD for providing subject-specific accurate treatments.

A Clinically Interpretable Computer-Vision Based Method for Quantifying Gait in Parkinson's Disease

Abstract:Gait is a core motor function and is impaired in numerous neurological diseases, including Parkinson's disease (PD). Treatment changes in PD are frequently driven by gait assessments in the clinic, commonly rated as part of the Movement Disorder Society (MDS) Unified PD Rating Scale (UPDRS) assessment (item 3.10). We proposed and evaluated a novel approach for estimating severity of gait impairment in Parkinson's disease using a computer vision-based methodology. The system we developed can be used to obtain an estimate for a rating to catch potential errors, or to gain an initial rating in the absence of a trained clinician-for example, during remote home assessments. Videos (n=729) were collected as part of routine MDS-UPDRS gait assessments of Parkinson's patients, and a deep learning library was used to extract body key-point coordinates for each frame. Data were recorded at five clinical sites using commercially available mobile phones or tablets, and had an associated severity rating from a trained clinician. Six features were calculated from time-series signals of the extracted key-points. These features characterized key aspects of the movement including speed (step frequency, estimated using a novel Gamma-Poisson Bayesian model), arm swing, postural control and smoothness (or roughness) of movement. An ordinal random forest classification model (with one class for each of the possible ratings) was trained and evaluated using 10-fold cross validation. Step frequency point estimates from the Bayesian model were highly correlated with manually labelled step frequencies of 606 video clips showing patients walking towards or away from the camera (Pearson's r=0.80, p<0.001). Our classifier achieved a balanced accuracy of 50% (chance = 25%). Estimated UPDRS ratings were within one of the clinicians' ratings in 95% of cases. There was a significant correlation between clinician labels and model estimates (Spearman's ρ=0.52, p<0.001). We show how the interpretability of the feature values could be used by clinicians to support their decision-making and provide insight into the model's objective UPDRS rating estimation. The severity of gait impairment in Parkinson's disease can be estimated using a single patient video, recorded using a consumer mobile device and within standard clinical settings; i.e., videos were recorded in various hospital hallways and offices rather than gait laboratories. This approach can support clinicians during routine assessments by providing an objective rating (or second opinion), and has the potential to be used for remote home assessments, which would allow for more frequent monitoring.

Abstract:Annett (2001) has made an in-depth critique of subjectivity in science, concentrating not only on those rating scales which are extensively used in ergonomics, but also the very philosophy of subjectivism itself. The present authors take this critique and use it to reappraise their own interpretation of mental workload, a key concept popularly assessed by subjective measurement. Having recently proposed a definition of mental workload (Young & Stanton, 2001), the present commentary considers deeper problems of subjectivity in the interpretation, perception, and measurement of mental workload. It is realised that there are fundamental problems inherent with such a subjective concept, particularly concerned with the dependence on context and lack of an absolute metric. However, it is also noted that the very point of ergonomics is to address contextual influences on human performance. Therefore, in the absence of a complete paradigm shift in ergonomics, the discipline will have to either accept or find ways of coping with the problems of subjectivity

Abstract:Parkinson’s disease (PD) is a neurodegenerative disease of the central nervous system that results from the degeneration of dopaminergic neurons in the substantia nigra. Abnormal gait begins in the early stage and becomes severe as the disease progresses; therefore, the assessment of gait becomes an important issue in evaluating the progression of PD and the effectiveness of treatment. To provide a clinically useful gait assessment in environments with budget and space limitations, such as a small clinic or home, we propose and develop a portable method utilizing the monocular image sequences of walking to track and analyze a Parkinsonian gait pattern. In addition, a centroid tracking algorithm is developed and used here to enhance the method of quantifying kinematic gait parameters of PD in different states. Twelve healthy subjects and twelve mild patients with PD participate in this study. This method requires one digital video camera and subjects with two joint markers attached on the fibula head and the lateral malleolus of the leg. All subjects walk with a natural pace in front of a video camera during the trials. Results of our study demonstrate the stride length and walking velocity significantly decrease in PD without drug compared to PD with drug in both proposed method and simultaneous gait assessment performed by GAITRite® system. In gait initiation, step length and swing velocity also decrease in PD without drug compared to both PD with drug and controls. Our results showed high correlation in gait parameters between the two methods and prove the reliability of the proposed method. With the proposed method, quantitative measurement and analysis of Parkinsonian gait could be inexpensive to implement, portable within a small clinic or home, easy to administer, and simple to interpret. Although this study is assessed Parkinsonian gait, the proposed method has the potential to help clinicians and researchers assess the gait of patients with other neuromuscular diseases, such as traumatic brain injury and stroke patients.

Abstract:Parkinson's disease (PD) is a progressive neurological disorder primarily affecting motor function resulting in tremor at rest, rigidity, bradykinesia, and postural instability. The physical severity of PD impairments can be quantified through the Movement Disorder Society Unified Parkinson's Disease Rating Scale (MDS-UPDRS), a widely used clinical rating scale. Accurate and quantitative assessment of disease progression is critical to developing a treatment that slows or stops further advancement of the disease. Prior work has mainly focused on dopamine transport neuroimaging for diagnosis or costly and intrusive wearables evaluating motor impairments. For the first time, we propose a computer vision-based model that observes non-intrusive video recordings of individuals, extracts their 3D body skeletons, tracks them through time, and classifies the movements according to the MDS-UPDRS gait scores. Experimental results show that our proposed method performs significantly better than chance and competing methods with an F1-score of 0.83 and a balanced accuracy of 81%. This is the first benchmark for classifying PD patients based on MDS-UPDRS gait severity and could be an objective biomarker for disease severity. Our work demonstrates how computer-assisted technologies can be used to non-intrusively monitor patients and their motor impairments. What the Trained Eye Cannot See: Quantitative Kinematics and Machine Learning Detect Movement Deficits in Early-Stage Parkinson's Disease from Videos

Abstract:Background Evaluation of disease severity in Parkinson's disease (PD) relies on motor symptoms quantification. However, during early-stage PD, these symptoms are subtle and difficult to quantify by experts, which might result in delayed diagnosis and suboptimal disease management. Objective To evaluate the use of videos and machine learning (ML) for automatic quantification of motor symptoms in early-stage PD. Methods We analyzed videos of three movement tasks—Finger Tapping, Hand Movement, and Leg Agility— from 26 aged-matched healthy controls and 31 early-stage PD patients. Utilizing ML algorithms for pose estimation we extracted kinematic features from these videos and trained three classification models based on left and right-side movements, and right/left symmetry. The models were trained to differentiate healthy controls from early-stage PD from videos. Results Combining left side, right side, and symmetry features resulted in a PD detection accuracy of 79 % from Finger Tap videos, 75 % from Hand Movement videos, 79 % from Leg Agility videos, and 86 % when combining the three tasks using a soft voting approach. In contrast, the classification accuracy varied between 40 % and 72 % when the movement side or symmetry were not considered. Conclusions Our methodology effectively differentiated between early-stage PD and healthy controls using videos of standardized motor tasks by integrating kinematic analyses of left-side, right-side, and bilateral symmetry movements. These results demonstrate that ML can detect movement deficits in early-stage PD from videos. This technology is easy-to-use, highly scalable, and has the potential to improve the management and quantification of motor symptoms in early-stage PD.

Assessing gait dysfunction severity in Parkinson's Disease using 2-Stream Spatial-Temporal Neural Network

Abstract:Parkinson's Disease (PD), a neurodegenerative disorder, significantly impacts the quality of life for millions of people worldwide. PD primarily impacts dopaminergic neurons in the brain's substantia nigra, resulting in dopamine deficiency and gait impairments such as bradykinesia and rigidity. Currently, several well-established tools, such as the Movement Disorder Society-Unified Parkinson's Disease Rating Scale (MDS-UPDRS) and Hoehn and Yahr (H&Y) Scale, are used for evaluating gait dysfunction in PD. While insightful, these methods are subjective, time-consuming, and often ineffective in early-stage diagnosis. Other methods using specialized sensors and equipment to measure movement disorders are cumbersome and expensive, limiting their accessibility. This study introduces a hierarchical approach to evaluating gait dysfunction in PD through videos. The novel 2-Stream Spatial-Temporal Neural Network (2S-STNN) leverages the spatial-temporal features from the skeleton and silhouette streams for PD classification. This approach achieves an accuracy rate of 89% and outperforms other state-of-the-art models. The study also employs saliency values to highlight critical body regions that significantly influence model decisions and are severely affected by the disease. For a more detailed analysis, the study investigates 21 specific gait attributes for a nuanced quantification of gait disorders. Parameters such as walking pace, step length, and neck forward angle are found to be strongly correlated with PD gait severity categories. This approach offers a comprehensive and convenient solution for PD management in clinical settings, enabling patients to receive a more precise evaluation and monitoring of their gait impairments.
An Optimized-Skeleton-Based Parkinsonian Gait Auxiliary Diagnosis Method with Both Monitoring Indicators and Assisted Ratings.

Abstract:Abnormal gait is one of the indispensable diagnostic sources of Parkinson’s disease (PD) diagnosis, typically presenting as small shuffling steps and gait bradykinesia. However, its diagnostic accuracy is lower due to the subjective judgments of doctors. To assist in improving the accuracy and reducing the subjectivity of the doctors, we propose an optimized-skeleton-based Parkinsonian gait auxiliary diagnosis method with both monitoring indicators and assisted ratings. By inputting a patient gait video captured from the side, our PD symptom-applicable pose trajectory model will extract a more precise and stable 2D skeleton sequence of patients. Next, the sequence will be used to calculate our proposed five monitoring indicators: gait frequency, ankle speed, whole speed, ankle angle speed, ankle acceleration, and previous work indicators: arm swing angle, leg angle, two feet x-axis distance to record the patient’s gait details at every moment. The extracted gait frequency will then be input into a random forest model to obtain the gait rating. Lastly, doctors can make more accurate judgments by referring to our objective monitoring indicators and assisted ratings. Experimental results show that our monitored indicators improve the doctors’ diagnosis accuracy by 16%, the skeleton speed and acceleration error of our optimized-skeleton extraction method achieve 4.18 cm/s and 5.71 cm/s <sup xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">2</sup> , and our random forest model has reached a classification accuracy of 95.8%.

Abstract:Background: Gait disorder is a prominent motor symptom in Parkinson's disease (PD), objective and quantitative assessment of gait is essential for diagnosing and treating PD, particularly in its early stage. Methods: This study utilized a non-contact gait assessment system to investigate gait characteristics between individuals with PD and healthy controls, with a focus on early-stage PD. Additionally, we trained two machine learning models to differentiate early-stage PD patients from controls and to predict MDS-UPDRS III score. Results: Early-stage PD patients demonstrated reduced stride length, decreased gait speed, slower stride and swing speeds, extended turning time, and reduced cadence compared to controls. Our model, after an integrated analysis of gait parameters, accurately identified early-stage PD patients. Moreover, the model indicated that gait parameters could predict the MDS-UPDRS III score using a machine learning regression approach. Conclusion: The non-contact gait assessment system facilitates the objective and quantitative evaluation of gait disorder in PD patients, effectively distinguishing those in the early stage from healthy individuals. The system holds significant potential for the early detection of PD. It also harnesses gait parameters for a reasoned prediction of the MDS-UPDRS III score, thereby quantifying disease severity. Overall, gait assessment is a valuable method for the early identification and ongoing monitoring of PD.

Abstract:Abstract Background Parkinson’s disease (PD) and essential tremor (ET) are movement disorders that can have similar clinical characteristics including tremor and gait difficulty. These disorders can be misdiagnosed leading to delay in appropriate treatment. The aim of the study was to determine whether balance and gait variables obtained with wearable inertial motion sensors can be utilized to differentiate between PD and ET using machine learning. Additionally, we compared classification performances of several machine learning models. Methods This retrospective study included balance and gait variables collected during the instrumented stand and walk test from people with PD ( n = 524) and with ET ( n = 43). Performance of several machine learning techniques including neural networks, support vector machine, k-nearest neighbor, decision tree, random forest, and gradient boosting, were compared with a dummy model or logistic regression using F1-scores. Results Machine learning models classified PD and ET based on balance and gait characteristics better than the dummy model (F1-score = 0.48) or logistic regression (F1-score = 0.53). The highest F1-score was 0.61 of neural network, followed by 0.59 of gradient boosting, 0.56 of random forest, 0.55 of support vector machine, 0.53 of decision tree, and 0.49 of k-nearest neighbor. Conclusions This study demonstrated the utility of machine learning models to classify different movement disorders based on balance and gait characteristics collected from wearable sensors. Future studies using a well-balanced data set are needed to confirm the potential clinical utility of machine learning models to discern between PD and ET.

Abstract:This paper develops neural network models that can recognize Parkinson’s disease (PD) at its early stage. PD is a common neurodegenerative disorder that presents with progressive slow movement, tremor, limb rigidity, and gait alterations, including stooped posture, shuffling steps, festination, freezing of gait, and falling. Early detection of PD enables timely initiation of therapeutic management that decreases morbidity. However, correct recognition of PD, especially in early-stage disease, is challenging because the aging population, which has a high PD prevalence, also commonly exhibits progressive gait slowness due to other disorders, such as joint osteoarthritis or sarcopenia. Therefore, developing a reliable and objective method is crucial for differentiating PD gait characteristics from those of the normal elderly. The aim of this study was to develop neural network models that could use the participants’ motion data during walking to identify PD. We recruited 32 drug-naïve PD patients with variable disease severity and 16 age/sex-matched healthy controls, and we measured their motions using inertial measurement unit (IMU) sensors. The IMU data were used to develop neural network models that could identify patients with advanced-stage PD with an average accuracy of 92.72% in validation processes. The models also differentiated patients with early-stage PD from normal elderly subjects with an accuracy of 99.67%. Another independent group of participants recruited to test the developed models confirmed the successful discrimination of PD-affected from healthy elderly, as well as patients at different severity stages. Our results provide support for early diagnosis and disease severity monitoring in patients with PD.

Abstract:Background Current diagnosis and monitoring of Parkinson's disease (PD) is based on subjective clinical assessments. Objective measures of motor functioning could support clinical acumen. Computer vision (CV) technology is a promising contactless technique but requires further validation. Aim To investigate the performance of CV analysis of clinic-based videos of finger-tapping. Our goals were (i) to distinguish PD from healthy controls (HC), when compared to human raters, (ii) to measure the severity of bradykinesia, and (iii) detect ON/OFF medication state. Methods Videos of thirty-one persons with PD and forty-nine HC were collected during clinical outpatient visits. Videos were analysed using CV to produce speed, amplitude, rhythm and composite bradykinesia measures. All videos were independently rated by three raters using the Movement Disorder Society Unified Parkinson's Disease Rating Scale (MDS-UPDRS) and Modified Bradykinesia Rating Scale (MBRS). Twenty video pairs were conducted in ON and OFF states. Classification accuracy for PD/HC state and ON/OFF state were measured using the Area under Receiver Operating characteristic curve and a confusion matrix. CV and clinical measures were correlated using Spearman coefficients. Results CV classified disease state with higher accuracy than clinical raters (91 % sensitivity; 97 % specificity). CV measures of bradykinesia correlated significantly with clinical ratings: R = 0.740 for MDS-UPDRS, 0.715 for MBRS speed, 0.714 for amplitude and 0.504 for rhythm. CV classified ON/OFF state as accurately as clinical raters. Discussion CV can provide a valid, objective and contactless bradykinesia assessment based on clinically collected videos, which offers promise as a new clinical outcome, including in remote settings.

Abstract:Gait analysis plays an important role in maintaining the well-being of human mobility and health care, and is a valuable tool for obtaining quantitative information on motor deficits in Parkinson's disease (PD). In this paper, we propose a method to classify (diagnose) patients with PD and healthy control subjects using gait analysis via deterministic learning theory. The classification approach consists of two phases: a training phase and a classification phase. In the training phase, gait characteristics represented by the gait dynamics are derived from the vertical ground reaction forces under the usual and self-selected paces of the subjects. The gait dynamics underlying gait patterns of healthy controls and PD patients are locally accurately approximated by radial basis function (RBF) neural networks. The obtained knowledge of approximated gait dynamics is stored in constant RBF networks. The gait patterns of healthy controls and PD patients constitute a training set. In the classification phase, a bank of dynamical estimators is constructed for all the training gait patterns. Prior knowledge of gait dynamics represented by the constant RBF networks is embedded in the estimators. By comparing the set of estimators with a test gait pattern of a certain PD patient to be classified (diagnosed), a set of classification errors are generated. The average L1 norms of the errors are taken as the classification measure between the dynamics of the training gait patterns and the dynamics of the test PD gait pattern according to the smallest error principle. When the gait patterns of 93 PD patients and 73 healthy controls are classified with five-fold cross-validation method, the accuracy, sensitivity and specificity of the results are 96.39%, 96.77% and 95.89%, respectively. Based on the results, it may be claimed that the features and the classifiers used in the present study could effectively separate the gait patterns between the groups of PD patients and healthy controls.

Abstract:Parkinson’s disease is a neurodegenerative disease that affects more than 6.1 million people worldwide. In the clinical routine, the main tool to diagnose and monitor disease progression is based on motor impairments, such as postural instability, bradykinesia, tremor, among others. Besides, new biomarkers based on motion patterns have emerged to describe disease findings. Nonetheless, this motor characterization has low sensitivity, especially at early stages, and is largely expert-dependent, because protocols are mainly based on visual observations. However, most of these analyses require complex and some invasive systems that additionally only bring global information of complete recordings. This work introduces a multimodal approach that integrates gait and eye motion videos to quantify and predict patient stage on-the-fly. This method starts by computing dense apparent velocity maps that represent the local displacement of the person seen from the gait in a sagittal plane and as micro-movements during the fixation experiment. Then, each frame is described as a covariance descriptor of deep feature activation maps computed over the motion field at each video time. Then, the covariance video manifold is mapped to a recurrent LSTM network to learn higher non-local dependencies and quantify a motion descriptor. Also, an end-to-end scheme allows to lately fuse both modalities (gait and fixational eye) to obtain a more sensitive Parkinson disease descriptor. In a study with 25 subjects, the proposed approach reaches an average F1-score of 0.83 with an average recall of 0.78. In a temporal prediction analysis, the approach reports major correlations with the disease considering swing phase.
Differentiation of speech in Parkinson's disease and spinocerebellar degeneration using deep neural networks

Abstract:Introduction: Assessing dysarthria features in patients with neurodegenerative diseases helps diagnose underlying pathologies. Although deep neural network (DNN) techniques have been widely adopted in various audio processing tasks, few studies have tested whether DNNs can help differentiate neurodegenerative diseases using patients' speech data. This study evaluated whether a DNN model using a transformer architecture could differentiate patients with Parkinson's disease (PD) from patients with spinocerebellar degeneration (SCD) using speech data. Methods: Speech data were obtained from 251 and 101 patients with PD and SCD, respectively, while they read a passage. We fine-tuned a pre-trained DNN model using log-mel spectrograms generated from speech data. The DNN model was trained to predict whether the input spectrogram was generated from patients with PD or SCD. We used fivefold cross-validation to evaluate the predictive performance using the area under the receiver operating characteristic curve (AUC) and accuracy, sensitivity, and specificity. Results: Average ± standard deviation of the AUC, accuracy, sensitivity, and specificity of the trained model for the fivefold cross-validation were 0.93 ± 0.04, 0.87 ± 0.03, 0.83 ± 0.05, and 0.89 ± 0.05, respectively. Conclusion: The DNN model can differentiate speech data of patients with PD from that of patients with SCD with relatively high accuracy and AUC. The proposed method can be used as a non-invasive, easy-to-perform screening method to differentiate PD from SCD using patient speech and is expected to be applied to telemedicine.

Abstract:Overlapping phenotypic features between Early Onset Ataxia (EOA) and Developmental Coordination Disorder (DCD) can complicate the clinical distinction of these disorders. Clinical rating scales are a common way to quantify movement disorders but in children these scales also rely on the observer's assessment and interpretation. Despite the introduction of inertial measurement units for objective and more precise evaluation, special hardware is still required, restricting their widespread application. Gait video recordings of movement disorder patients are frequently captured in routine clinical settings, but there is presently no suitable quantitative analysis method for these recordings. Owing to advancements in computer vision technology, deep learning pose estimation techniques may soon be ready for convenient and low-cost clinical usage. This study presents a framework based on 2D video recording in the coronal plane and pose estimation for the quantitative assessment of gait in movement disorders. To allow the calculation of distance-based features, seven different methods to normalize 2D skeleton keypoint data derived from pose estimation using deep neural networks applied to freehand video recording of gait were evaluated. In our experiments, 15 children (five EOA, five DCD and five healthy controls) were asked to walk naturally while being videotaped by a single camera in 1280 × 720 resolution at 25 frames per second. The high likelihood of the prediction of keypoint locations (mean = 0.889, standard deviation = 0.02) demonstrates the potential for distance-based features derived from routine video recordings to assist in the clinical evaluation of movement in EOA and DCD. By comparison of mean absolute angle error and mean variance of distance, the normalization methods using the Euclidean (2D) distance of left shoulder and right hip, or the average distance from left shoulder to right hip and from right shoulder to left hip were found to better perform for deriving distance-based features and further quantitative assessment of movement disorders.

Abstract:Parkinson's Disease (PD) is a common neurodegenerative disease which impacts millions of people around the world. In clinical treatments, freezing of gait (FoG) is used as the typical symptom to assess PD patients' condition. Currently, the assessment of FoG is usually performed through live observation or video analysis by doctors. Considering the aging societies, such a manual inspection based approach may cause serious burdens on the healthcare systems. In this study, we propose a pure video-based method to automatically detect the shuffling step, which is the most indistinguishable type of FoG. Firstly, the RGB silhouettes which only contain legs and feet are fed into the feature extraction module to obtain multi-level features. 3D convolutions are used to aggregate both temporal and spatial information. Then the multi-level features are aggregated by the feature fusion. Skip connections are implemented to reserve information of high resolution and period-wise horizontal pyramid pooling is utilized to fuse both global context and local features. To validate the efficacy of our method, a dataset containing 268 normal gait samples and 362 shuffling step samples is built, on which our method achieves an average detection accuracy of 90.8%. Besides shuffling step detection, we demonstrate that our method can also assess the severity of walking abnormity. Our proposal facilitates a more frequent assessment of FoG with less manpower and lower cost, leading to more accurate monitoring of the patients' condition.

Abstract:Gait impairments are among the most common and disabling symptoms of Parkinson's disease and worsen as the disease progresses. Early detection and diagnosis of subtype-specific gait deficits, as well as progression monitoring, can help to implement effective and preventive personalized treatment for PD patients. Yet, the gait features have not been fully studied in PD and its motor subtypes. To characterize comprehensive and objective gait alterations and to identify the potential gait biomarkers for early diagnosis, subtype differentiation, and disease severity monitoring. We analyzed gait parameters related to upper/lower limbs, trunk and lumbar, and postural transitions from 24 tremor-dominant (TD) and 20 postural instability gait difficulty (PIGD) dominant PD patients who were in early stage and 39 matched healthy controls (HC) during the Timed Up and Go test using wearable sensors. Results show: (1) Both TD and PIGD groups showed restricted backswing range in bilateral lower extremities and more affected side (MAS) arm, reduced trunk and lumbar rotation range in the coronal plane, and low turning efficiency. The receiver operating characteristic (ROC) analysis revealed these objective gait features had high discriminative value in distinguishing both PD subtypes from the HC with the area under the curve (AUC) values of 0.7~0.9 ( p < 0.01). (2) Subtle but measurable gait differences existed between TD and PIGD patients before the onset of clinically apparent gait impairment. (3) Specific gait parameters were significantly associated with disease severity in TD and PIGD subtypes. Objective gait biomarkers based on wearable sensors may facilitate timely and personalized gait treatments in PD subtypes through early diagnosis, subtype differentiation, and disease severity monitoring.

Automatic Health Problem Detection from Gait Videos Using Deep Neural Networks

Abstract:The aim of this study is developing an automatic system for detection of gait-related health problems using Deep Neural Networks (DNNs). The proposed system takes a video of patients as the input and estimates their 3D body pose using a DNN based method. Our code is publicly available at <a class="link-external link-https" href="https://github.com/rmehrizi/multi-view-pose-estimation" rel="external noopener nofollow">this https URL</a>. The resulting 3D body pose time series are then analyzed in a classifier, which classifies input gait videos into four different groups including Healthy, with Parkinsons disease, Post Stroke patient, and with orthopedic problems. The proposed system removes the requirement of complex and heavy equipment and large laboratory space, and makes the system practical for home use. Moreover, it does not need domain knowledge for feature engineering since it is capable of extracting semantic and high level features from the input data. The experimental results showed the classification accuracy of 56% to 96% for different groups. Furthermore, only 1 out of 25 healthy subjects were misclassified (False positive), and only 1 out of 70 patients were classified as a healthy subject (False negative). This study presents a starting point toward a powerful tool for automatic classification of gait disorders and can be used as a basis for future applications of Deep Learning in clinical gait analysis. Since the system uses digital cameras as the only required equipment, it can be employed in domestic environment of patients and elderly people for consistent gait monitoring and early detection of gait alterations.
