Abstract:Lung cancer (LC) is the leading cause of cancer-related death. The sensitive and non-invasive diagnostic tools in the early stage are still poor. We present a pilot study on the early diagnosis of LC by detecting markers in exhaled breath condensate (EBC). EBC samples were collected from 105 patients with LC and 56 healthy controls. We applied chemiluminescence immunoassay to detect CEA (carcinoembryonic antigen), SCC (squamous cell carcinoma) antigen and NSE (neuron specific enolase) in EBC and serum. Concentrations of markers were compared between independent groups and subgroups. A significantly higher concentration level of each marker was found in patients with LC than healthy controls. The areas under curve of receiver operating characteristic (ROC) curves were 0.800, 0.771, 0.659, 0.679, 0.636 and 0.626 for EBC-CEA, serum-CEA, EBC-SCC, serum-SCC, EBC-NSE and serum-NSE, respectively. Markers in EBC had a higher positive rate (PR) and were more specific to histologic types than markers in serum. In addition, multivariate analysis was performed to evaluate the association of presenting markers with the stages of non-small cell lung cancer (NSCLC). EBC-CEA showed the best predictive characteristic (p < 0.006) of early-NSCLC. Our study suggested that tumor markers in EBC may have a better diagnostic performance for LC than those in serum. With further investigation on the combination of markers in EBC, detection of EBC could probably be a novel and non-invasive method to detect NSCLC earlier.
P-STAT3 and IL-17 in Tumor Tissues Enhances the Prognostic Value of CEA and CA125 in Patients with Lung Adenocarcinoma

Abstract:Aim: The present study aimed to examine the capability of p- signal transducer and activator of transcription (STAT)3 and interleukin-17 (IL-17), along with two known tumor markers carcinoembryonic antigen (CEA) and carbohydrate antigen 125 (CA125), for disease prognosis. Moreover, the associations among biomarkers and clinicopathological parameters were evaluated to uncover the potential mechanisms responsible for their correlations with lung adenocarcinoma (LAD) prognosis. Methods: Five LAD-related parameters were used in the study: CEA, CA125, STAT3, p-STAT3, and IL-17. Spearman and chi-square correlation tests were used to explore the relationships between some clinicopathological variables and parameter expression levels and the associations among these five parameters. Results: The disease-specific survival decreased with the positive expression of CEA, CA125, p-STAT3, and IL-17, with no significant difference in the expression level of STAT3. Combinations of p-STAT3 and IL-17, CEA and p-STAT3, CEA and IL-17, CA125 and p-STAT3, and CA125 and IL-17 had higher predictive values in LAD prognosis. The correlation analyses indicated the synergic activities of STAT3, p-STAT3, and IL-17 and the coordinated expression of CEA, CA125, p-STAT3, and IL-17. The tumor-node-metastasis (TNM) stage significantly correlated with the levels of CA125 and p-STAT3. Conclusions: Elevated levels of CEA, CA125, p-STAT3, and IL-17 alone and/or combinations of p-STAT3 and IL-17, CEA and p-STAT3, CEA and IL-17, CA125 and p-STAT3, and CA125 and IL-17 were recommended as the prognostic predictors of unfavorable clinical outcomes in patients with postoperative LAD. Also, p-STAT3 and IL-17 combined with CA125 and CEA helped in predicting the overall survival of patients with LAD and informing the TNM stage.
Combining High-Resolution CT Parameters and Inflammatory Markers to Predict Spread Through Air Spaces in Lung Cancer

Abstract:OBJECTIVE:To explore the predictive value of high-resolution computed tomography (CT) parameters and inflammatory markers for spread through air spaces (STAS) in lung cancer patients. METHODS:A retrospective analysis was conducted on 72 lung cancer patients with STAS and 128 STAS-negative patients treated during the same period. Differences in high-resolution CT indicators and inflammatory markers between the two groups were assessed. Binary logistic regression was used to analyze the relationship between these indicators and STAS positivity. Receiver operating characteristic (ROC) curve analysis was performed to assess the predictive efficacy of these indicators for STAS positivity. RESULTS:Patients in the STAS-positive group exhibited a higher prevalence of leaf signs, pleural traction signs, and blurred tumor-lung boundaries than the STAS-negative group (P<0.05). Additionally, the STAS-positive group had elevated levels of the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), interleukin-6 (IL-6), and C-reactive protein (CRP), alongside a lower lymphocyte-to-monocyte ratio (LMR) (P<0.05). The combined predictive model incorporating pleural traction sign, LMR, NLR, PLR, SII, IL-6, and CRP yielded an area under the curve (AUC) of 0.977, with a sensitivity of 94.4% and a specificity of 90.8%. CONCLUSION:The integration of high-resolution CT parameters with inflammatory markers demonstrates significant value in predicting STAS positivity in lung cancer patients, with the combined predictive model showing superior performance.

Abstract:Background: Serum markers, such as Krebs von den Lungen-6 (KL-6), are reported to reflect the severity, progression and prognosis of idiopathic pulmonary fibrosis (IPF). We aimed to evaluate whether tumor markers could improve the diagnosis of connective tissue diseases (CTDs) and predict CTD-related interstitial lung diseases (ILDs) (CTD–ILD); and whether there was a significant difference between ILD patients with CTD and without CTD (non-CTD–ILD). Methods: In this study, 146 patients with ILD only, 61 with CTD–ILD, 102 with lung cancer (LC), 111 with COPD, and 46 with CTD were enrolled. Serum levels of several tumor markers were collected. Results: Compared with COPD, the serum levels of CEA, NSE, CYFRA21-1, CA19-9 and CA125 were higher, but the level of SCC was lower; meanwhile, there were significant differences in NSE, SCC, CYFRA21-1 and CA125 between CTD–ILD and non-CTD–ILD groups. Conclusions: It appears that tumor-associated antigens may be the predictive factors for CTD–ILD that could be used to identify CTD from ILD before it is detected on autoimmune symptoms or auto-antibodies. However, further studies are needed to validate our findings and determine whether serum tumor markers have prognostic value or predict progressive CTD–ILD.
B-360 Usefulness of the Ratios of CYFRA21-1 and P53 Autoantibody Immune Complexes to Their Free Antigens in Lung Cancer

Abstract:Background Autoantibodies against specific cancer-associated antigens are produced in the early stages of tumorigenesis. In lung cancer, the possibility of autoantibodies as diagnostic biomarkers has been proposed. This study was to evaluate the usefulness of novel DNA-guided sandwich immunoassay detecting CYFRA21-1 and p53 autoantibody immune complexes and their free antigens in diagnosis of lung cancer. Methods In total, 100 patients with lung cancer and 119 healthy controls were analyzed with 9G testTM Lung/Cancer (Biometrix Technology, South Korea) measuring plasma autoantibody immune complexes (CYFRA21-1 with anti-CYFRA21-1 autoantibody immune complex (CIC), p53 with anti-p53 autoantibody immune complex (PIC)) and their free antigens (CYFRA 21-1 and p53). The ratios of CIC to CYFRA21-1 (CIC/CYFRA21-1) and PIC to p53 (PIC/p53), and the multiplication of the two ratios, named as LC index were calculated. Results The levels of CIC and PIC were significantly increased in lung cancer compared with those in healthy control (P = 0.0062 and P = 0.0026, respectively), while free antigens didn’t distinguish between lung cancer and healthy control. The ratios CIC/CYFRA21-1 and PIC/p53 were significantly higher in lung cancer than those in healthy control (all, P < 0.0001). Diagnostic performances expressed as area under the curve (AUC) were higher in the ratios of autoantibody immune complexes to each free antigen (PIC/p53 and CIC/CYFRA21-1) than autoantibody immune complexes (PIC and CIC) (AUC; 0.887, 0.847 vs 0.618, 0.608, respectively). The most excellent diagnostic performance was proven with LC index at diagnostic cut-off 3.60, which showed 81% sensitivity and 95% specificity with 0.945 AUC (P < 0.001). It was more sensitive to early lung cancer than to advanced lung cancer (sensitivity 87.5% vs 72.7%). Conclusion The ratios of CIC to CYFRA21-1 and PIC to p53 are useful biomarkers in lung cancer diagnosis and the combination of the ratios, LC index could be complementary tool for lung cancer screening with radiological tests.
Combined detection of serum IL-6 and CEA contributes to the diagnosis of lung adenocarcinoma in situ

Abstract:Background Effective discrimination of lung adenocarcinoma (LUAD) in situ (AIS) from benign pulmonary nodules (BPN) is critical for the early diagnosis of AIS. Our pilot study in a small cohort of 90 serum samples has shown that serum interleukin 6 (IL-6) detection can distinguish AIS from BPN and health controls (HC). In this study, we intend to comprehensively define the diagnostic value of individual and combined detection of serum IL-6 related to the traditional tumor markers carcinoembryonic antigen (CEA) and cytokeratin 19 fragment (CYFRA21-1) for AIS. Methods The diagnostic performance of serum IL-6 along with CEA and CYFRA21-1 were evaluated in a large cohort of 300 serum samples by a chemiluminescence immunoassay and an electrochemiluminescence immunoassay. A training set comprised of 65 AIS, 65 BPN, and 65 HC samples was used to develop the predictive model for AIS. Data obtained from an independent validation set was applied to evaluate and validate the predictive model. Results In the training set, the levels of serum IL-6 and CEA in the AIS group were significantly higher than those in the BPN/HC group ( P < 0.05). There was no significant difference in serum CYFRA21-1 levels between the AIS group and the BPN/HC group ( P > 0.05). Serum IL-6 and CEA levels for AIS patients showed an area under the curve (AUC) of 0.622 with 23.1% sensitivity at 90.7% specificity, and an AUC of 0.672 with 24.6% sensitivity at 97.6% specificity, respectively. The combination of serum IL-6 and CEA presented an AUC of 0.739, with 60.0% sensitivity at 95.4% specificity. The combination of serum IL-6 and CEA showed an AUC of 0.767 for AIS patients, with 57.1% sensitivity at 91.4% specificity in the validation set. Conclusions IL-6 shows potential as a prospective serum biomarker for the diagnosis of AIS, and the combination of serum IL-6 with CEA may contribute to increased accuracy in AIS diagnosis. However, it is worth noting that further research is still necessary to validate and optimize the diagnostic efficacy of these biomarkers and to address potential sensitivity limitations.

Predictive value of community-acquired pneumonia severity scores on lung cancer patients with immune checkpoint inhibitor-associated pneumonitis: a retrospective analysis

Abstract:Introduction: The immune checkpoint inhibitor-associated pneumonitis (CIP) is a potentially fatal type of immune-related adverse events. There is urgent need for objective and consistent assessment tools for evaluating the severity of CIP. Several severity scores of community-acquired pneumonia (CAP) might meet the need. Methods: A retrospective study was conducted to explore preliminarily if several CAP severity scores including Pneumonia Severity Index (PSI), CURB65, CRB65 and SMART-COP, could predict the mortality in lung cancer patients with CIP. Results: A total number of 53 lung patients with CIP were included, which were divided into two groups according to each severity score respectively. Patients with higher severity score were more likely to have severe CIP, higher C-reactive protein and D-dimer, and receive corticosteroids treatment. Mortality was significantly higher in the high CURB65 group than in the low CURB65 group (30-day: 28.6% vs 2.6%; 90-day: 35.7% vs 7.7%; 180-day:42.9% vs 18.00%), as well as in the high SMART-COP group compared to the low SMART-COP group (30-day: 27.8% vs 0; 90-day: 27.8% vs 8.6%; 180-day:38.9% vs 17.1%). The PSI, SMART-COP and CURB65 were moderately correlated with American Society of Clinical Oncology (ASCO) grade of CIP respectively. Conclusion: Both CURB65 and SMART-COP accurately stratified the risk of mortality in lung cancer patients with CIP. They might complement the ASCO grade in the assessment and prediction of mortality in these population.
[Diagnostic Value of Serum Tumor Markers in Differentiating Malignant from Benign Solitary Pulmonary Nodules].

Abstract:OBJECTIVE:To investigate the diagnostic value of serum tumor marker in solitary pulmonary nodules (SPN). METHODS:In the study, 175 cases of SPN postoperatively diagnosed by pathology between Jan. 2011 and Jan. 2013 in Peking University First Hospital were selected, including 125 cases of lung cancer and 50 cases of benign lesions. The levels of serum carcino-embryonic antigen (CEA), squamous cell carcinoma (SCC), neuron-specific enolase (NSE) and cytokerantin-19-fragment (CYFRA21-1) were detected by electrochemical luminescence immunoassay. SPSS 11.5 software package was used for statistical analysis. RESULTS:In the malignant SPN group, CEA, SCC and CYFRA21-1 levels were significantly higher than in the benign group (P<0.05). The positive rates of CEA, SCC, CYFRA21-1 and combined detection of the four serum tumor markers in the malignant SPN group was significantly higher than in the benign group (P<0.05). ROC curves showed that the under-curve area of CEA, NSE, SCC and CYFRA21-1 was 0.633 ± 0.045, 0.494 ± 0.047, 0.664 ± 0.042 and 0.711 ± 0.041, respectively. The combination of CEA, SCC, NSE and CYFRA21-1 showed the highest sensitivity (52.0%) and better specificity (76.0%) for diagnosis of lung cancer. There were no statistical differences in the positive rates of tumor markers between the malignant SPN group and benign group in the smoking patients (P>0.05). CONCLUSION:CEA, SCC and CYFRA21-1 have higher positive rates in the malignant SPN patients, suggesting a certain value in the early diagnosis of malignant SPN.

Abstract:Objective To appraise the value of tumor markers TPA, NSE, CA15 3 and CEA in the diagnosis of lung Methods The levels of TPA, NSE, CA15 3 and CEA were measured by ELISA in serum of 36 patients with lung cancer and 28 with benign lung diseases. Results The levels of TPA, NSE, CA15 3 and CEA in cancer patients were significantly higher than that of benign ones ( t=1.992-3.271, P 0.05, 0.01). The rise of TPA was most obviously in squamous cell carcinoma, while adenocarcinoma as well as small cell lung cancer(SCLC) the next. NSE increased in more obvious in SCLC. The increase of CA15 3 was higher in adenocarcinoma , then spuamous cell one and SCLC . The specificity of TPA, NSE, CA15 3 and CEA was 89%, 96%, 93% and 100%; the sensitivity was 71.3%, 27.3%, 58.8% and 53.7%, respectively. Conclusion TPA, CA15 3 and CEA have important diagnostic value for NSCLC, but NSE, CA15 3, CEA for SCLC.A combination of multiple tumor markers may increase the diagnostic specificity and sensitivity of lung cancer.
Lack of Efficacy of Combined Carbohydrate Antigen Markers for Lung Cancer Diagnosis

Abstract:Background: Lung cancer (LC) is top-ranked in cancer incidence and is the leading cause of cancer death globally. Combining serum biomarkers can improve the accuracy of LC diagnosis. The identification of the best potential combination of traditional tumor markers is essential for LC diagnosis. Patients and Methods. Blood samples were collected from 132 LC cases and 118 benign lung disease (BLD) controls. The expression levels of ten serum tumor markers (CYFR21, CEA, NSE, SCC, CA15-3, CA 19-9, CA 125, CA50, CA242, and CA724) were assayed, and that the expression in the levels of tumor markers were evaluated, isolated, and combined in different patients. The performance of the biomarkers was analyzed by receiver operating characteristic (ROC) analyses, and the difference between combinations of biomarkers was compared by Chi-square (χ 2) tests. Results: As single markers, CYFR21 and CEA showed good diagnostic efficacy for nonsmall cell lung cancer (NSCLC) patients, while NSE and CEA were the most sensitive in the diagnosis of small cell lung cancer (SCLC). The area under the curve (AUC) value was 0.854 for the panel of four biomarkers (CYFR21, CEA, NSE, and SCC), 0.875 for the panel of six biomarkers (CYFR21, CEA, NSE, SCC, CA125, and CA15-3), and 0.884 for the panel of ten markers (CYFR21, CEA, NSE, SCC, CA125, CA15-3, CA19-9, CA50, CA242, and CA724). With a higher sensitivity and negative predictive value (NPV), the diagnostic accuracy of the three panels was better than that of any single biomarker, but there were no statistically significant differences among them (all P values > 0.05). However, the panel of six carbohydrate antigen (CA) biomarkers (CA125, CA15-3, CA19-9, CA50, CA242, and CA724) showed a lower diagnostic value (AUC: 0.776, sensitivity: 59.8%, specificity: 73.0%, and NPV: 60.4%) than the three panels (P value < 0.05). The performance was similar even when analyzed individually by LC subtypes. Conclusion: The biomarkers isolated are elevated for different types of lung cancer, and the panel of CYFR21, CEA, NSE, and SCC seems to be a promising serum biomarker for the diagnosis of lung cancer, while the combination with carbohydrate antigen markers does not improve the diagnostic efficacy.
[Significance of Combined Determination of CK19mRNA, Carcinoembryanic Antigen, Neuron-Specific Enolase, and Tissue Polypeptide Antigen in Peripheral Blood of Patients with Lung Cancer].

Abstract:BACKGROUND & OBJECTIVE:Cellular keratin, an indicator for cancer cell in circulation gains increasing attention. Although CK19mRNA as a gene marker used to identify micro-metastasis is of relatively high practical value, it lacks specificity and sensitivity in diagnosis of cancer. The aim of this study is to evaluate whether the combined detection of CK19mRNA with other tumor markers, such as carcinoembryanic antigen(CEA), neuron-specific Enolase(NSE), tissue polypeptide antigen(TPA), would improve the diagnosis of lung cancer. METHODS:The expression level of CK19mRNA in peripheral blood mononucleated cells (PBMCS) was determined by RT-PCR, The levels of CEA, NSE, and TPA in serum were determined by enzyme-linked immunosorbent assay (ELISA) and time-resolved fluorometry(TRF) in 91 patients with lung cancer, 33 with benign lung diseases (BLD) and 50 healthy controls. The data was analyzed by t test and chi 2 test. RESULTS:The positive rates of CK19mRNA, CEA, NSE, and TPA in lung cancer group (49.5%, 65.9%, 67.0%, and 83.5%, respectively) were remarkably higher than in BLD group(24.2%, 9.0%, 15.2%, and 9.0%, respectively) and in healthy control group (10.0%, 4.0%, 8.0%, and 6.0%, respectively) (P < 0.01). The positive rate of CK19mRNA was not statistically different in various histological subtypes (P > 0.05). Although the positive rate of CK19mRNA appeared to be associated with the clinical stage (stage I + II: 44.4%, stage III: 48.4%, stag IV: 54.2%), there was no statistical significance among the three groups (P > 0.05). The serum level (microgram/L)/positive rate of CEA(3.5/27.8%, 5.1/35.5%, and 8.5/58.3%, respectively), NSE (12.5/33.3%, 15.3/41.9%, and 24.7/58.3%, respectively), and TPA (1.1/44.4%, 1.8/58.1%, and 3.6/66.7%, respectively) in stage I + II, III, and IV appeared to be associated with the clinical stages of lung cancer, but no statistical significance among the three groups (P > 0.05). Combined detection of three or four tumor markers yielded significant higher effective accuracy in diagnosis of lung cancer than that of any of two tumor markers or single tumor marker (P < 0.05). In addition, the results showed that TPA, CEA, and NSE were relatively specific for squamous cell carcinoma (76.7%), adenocarcinomas (83.3%), and small-cell lung cancer (79.2%), respectively. CONCLUSION:The combined measurement of CK19mRNA with CEA, NSE, and TPA can increase diagnosis rate of lung cancer, can also provide potent diagnosis basis for treatment.

Abstract:The value of serum tumor biomarkers used for lung cancer diagnosis is still controversial in clinical practice. This study aimed to further dissect and evaluate the clinical value of serum progastrin-releasing peptide (ProGRP), neuron-specific enolase (NSE), squamous cell carcinoma antigen (SCC-Ag), carcinoembryonic antigen (CEA), cytokeratin-19 fragment (CYFRA21-1) together with a potential new biomarker, the human epididymis protein 4 (HE4) for lung cancer diagnosis, in a large cohort of a Chinese population. Ostensibly healthy individuals, as well as those with benign non-cancerous diseases, benign tumors, lung cancers, and other types of malignancies, were enrolled in the study. Serum ProGRP, NSE, SCC-Ag, CEA, CYFRA21-1, and HE4 were analyzed using the chemiluminescence immunoassay. Data were analyzed utilizing the SPSS and GraphPad Prism software. Detailed dissection of the diagnostic characteristics of serum 6 biomarkers on lung cancer was performed. All 6 biomarkers showed capabilities in characterizing lung cancer from other diseases. ProGRP and NSE were highly specific to small cell lung cancer (SCLC); SCC-Ag was a fair biomarker for NSCLC, specifically SCC histotype; CEA showed specificity to SCLC, followed by NSCLC; CYFRA21-1 was a good biomarker for both SCLC and NSCLC; HE4 showed high specificity to SCLC. For NSCLC characterization, CYFRA21-1+HE4+CEA was the best combinatory pattern in the terms of diagnostic performance (AUC=0.8110). The best combinatory analysis for SCLC was ProGRP+NSE+HE4 (AUC=0.9282). Patients with advanced stage, larger tumor, males, and age 50 or older had higher serum biomarkers levels than those with early stage, smaller tumor, females, and age under 50. Six biomarkers had capabilities in characterizing lung cancer with high or fair diagnostic performance. HE4 is a potential biomarker for both SCLC and NSCLC diagnosis, which merits further investigation.
The Value of Combination Analysis of Tumor Biomarkers for Early Differentiating Diagnosis of Lung Cancer and Pulmonary Tuberculosis

Abstract:Objective: Distinguishing early lung cancer from pulmonary tuberculosis is difficult. Biomarkers have been applied to tumor diagnoses widely. However, the ability for tumor biomarkers to uniquely identify either lung cancer or pulmonary tuberculosis remains controversial. Methods: The retrospective analysis of patients hospitalized with suspected pathological tissue mass in their thoracic cage, found via imaging, was conducted. The levels of tumor biomarkers CEA, NSE, CYFRA21-1, Pro-GRP, and SCC-Ag were measured and compared in patients with defined lung cancer (N=235) and pulmonary tuberculosis (N=224), respectively. Results: In the study, Serum CEA, NSE, CYFRA21-1, Pro-GRP, and SCC-Ag levels were significantly higher in the lung cancer group than in the pulmonary tuberculosis group (P<0.001). The combined detection of CEA, CYFRA21-1, and NSE was used to diagnose lung cancer with a specificity of 89.9%, and a sensitivity of 94.9%. The detection's accuracy was higher (AUC=0.972) than five tumor biomarkers alone or combined. Conclusion: The combination of CEA, CYFRA21-1, and NSE possesses better values for identifying lung cancer patients who are at a high risk of being misdiagnosed for pulmonary tuberculosis.
Use of tumor markers in distinguishing lung adenocarcinoma-associated malignant pleural effusion from tuberculous pleural effusion

Abstract:Background: The distinction between lung adenocarcinoma-associated malignant pleural effusion (MPE) and tuberculous pleural effusion (TPE) continues to pose a challenge. This study sought to assess the supplementary value of tumor markers in enabling a differential diagnosis. Methods: Data concerning tumor markers, which included carcinoembryonic antigen (CEA), cancer antigen 125 (CA125), cancer antigen 153 (CA153), cancer antigen 724 (CA724), neuron-specific enolase (NSE), cytokeratin19 fragment (Cyfra21-1), and squamous cell carcinoma antigen (SCCA), in both serum and pleural effusion samples, were retrospectively compiled from lung adenocarcinoma-associated MPE and TPE patients. A comparative analysis of tumor marker concentrations between the two groups was performed to assess diagnostic utility, followed by a multiple logistic regression to control for confounding variables. Results: While gender, serum CA125 and SCCA, and pleural effusion SCCA manifested comparability between the groups, distinctions were noted in patient age and the concentration of other tumor markers in serum and pleural effusion, which were notably elevated in the MPE group. Multiple logistic regression demonstrated a positive association between the risk of lung adenocarcinoma-associated MPE and levels of CEA and CA153 in serum and pleural effusion, as well as Cyfra21-1 in serum (P < 0.05). The odds ratio for CEA surpassed that of CA153 and Cyfra21-1. Conclusions: CEA and CA153 in serum and pleural effusion, and Cyfra21-1 in serum emerge as biomarkers possessing supplementary diagnostic value in distinguishing lung adenocarcinoma-associated MPE from TPE. The diagnostic efficacy of CEA is superior to CA153 and Cyfra21-1. Conversely, the utility of CA125, CA724, NSE, and SCCA appears constrained.

Abstract:The detection of serum biomarkers can aid in the diagnosis of lung cancer. In recent years, an increasing number of lung cancer markers have been identified, and these markers have been reported to have varying diagnostic values. A method to compare the diagnostic value of different combinations of biomarkers needs to be established to identify the best combination. In this study, automatic chemiluminescence analyzers were employed to detect the serum concentrations of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), cytokeratin 19 fragment (CY211), neuron-specific enolase (NSE), and squamous cell carcinoma antigen (SCC) in 780 healthy subjects, 650 patients with pneumonia, and 633 patients with lung cancer. Receiver operating characteristic (ROC) curve and logistic regression analyses were also used to evaluate the diagnostic value of single and multiple markers of lung cancer. The sensitivities of the five markers alone were lower than 65% for lung cancer screening in healthy subjects and pneumonia patients. SCC was of little value in screening lung cancer. After combining two or more markers, the areas under the curves (AUCs) did not increase with the increase in the number of markers. For healthy subjects, the best marker for lung cancer screening was the combination CEA + CA125, and the positive cutoff range was 0.577 CEA + 0.035 CA125 > 2.084. Additionally, for patients with pneumonia, the best screening markers displayed differences in terms of sex but not age. The best screening marker for male patients with pneumonia was the combination CEA + CY211 with a positive cutoff range of 0.008 CEA + 0.068 CY211 > 0.237, while that for female patients with pneumonia was CEA > 2.73 ng/mL, which could be regarded as positive. These results showed that a two-marker combination is more suitable than a multimarker combination for the serological screening of tumors. Combined ROC curve and logistic regression analyses are effective for identifying the best markers for lung cancer screening.

Abstract:Tumor markers are beneficial for the diagnosis and therapy monitoring of lung cancer. However, the value of tumor markers in lung cancer histological diagnosis is unknown. In this study, we analyzed the serum levels of six tumor markers (CEA, CYFRA21-1, SCC, NSE, ProGRP, and CA125) in 2097 suspected patients with lung cancer and determined whether the combination of the tumor markers was useful for histological diagnosis of lung cancer. We found that CYFRA21-1 was the most sensitive marker in NSCLC. ProGRP showed a better clinical performance than that of NSE in discriminating between SCLC and NSCLC. The serum level of CYFRA21-1 or SCC was significantly higher in squamous carcinoma ( p < 0.05 ), and the levels of ProGRP and NSE were significantly higher in SCLC ( p < 0.05 ). According to the criteria established, SCLC and NSCLC were discriminated with sensitivity of 87.12 and 62.63% and specificity of 64.61 and 99.5%, respectively. The sensitivity and specificity in the differentiation of adenocarcinoma and squamous carcinoma were 68.1 and 81.63% and 70.73 and 65.93%, with NPV of 46.03 and 68.97% and PPV of 85.82 and 79.47%, respectively. Our results suggested the combination of six tumor markers could discriminate the histological types of lung cancer.

Abstract:Background: To investigate the value of carcinoembryonic antigen (CEA), cytokeratin 19 fragment (CYFRA21-1), squamous cell carcinoma antigen (SCC-Ag), and gastrin-releasing peptide (pro-GRP) in the differential diagnosis of lung cancer. Methods: We enrolled 120 patients with malignant lung cancer who were treated at our hospital between June 2018 to June 2020. A further 58 patients with benign lung tumors and 60 healthy volunteers were also enrolled. Serum levels of CEA, CYFRA21-1, SCC-Ag, and pro-GRP were determined and compared across different populations, different pathological types, and different TNM stages. An ROC curve was drawn to evaluate the value of the four indicators when combined for the diagnosis of lung cancer. Results: The levels of CEA, CYFRA21-1, SCC-Ag, and pro-GRP in the malignant group were significantly higher than those in the benign and healthy groups (P<0.05). CEA in adenocarcinoma was significantly higher than that in squamous cell carcinoma (SCC) and small cell carcinoma (P>0.05), and CYFRA21-1 in non-small cell carcinoma was significantly higher than that in small cell carcinoma (P<0.05). Pro-GRP in small cell carcinoma was significantly higher than that in non-small cell carcinoma (P<0.05), and the SCC-Ag level in SCC was significantly higher than that in small cell carcinoma and adenocarcinoma (P<0.05). There was no statistically significant difference in CEA among various pathological types (P>0.05). However, there were significant differences in the levels of CEA and pro-GRP in different TMN stages (P<0.05), and in the levels of CEA and pro-GRP in different TMN stages (P<0.05) where from stage I to stage IV CEA, pro -GRP levels increased. There was a significant difference in CYFRA21-1 levels in stages I-III (P<0.05), and in stages III and IV, although there was no statistically significant difference in SCC-Ag in different stages (P>0.05). The area under the curve (AUC) for the combined diagnosis of lung cancer with the four markers was 0.9250 (95% CI: 0.8866-0.9634), the sensitivity was 93.29%, and the specificity was 84.32%. Conclusions: Joint inspection of CEA, CYFRA21-1, SCC-Ag, and pro-GRP levels has certain clinical value for the differential diagnosis of lung cancer.

Abstract:Abstract Background Pleural effusion (PE) can be divided into benign pleural effusion (BPE) and malignant pleural effusion (MPE). There is no consensus on the identification of lung cancer-associated MPE using the optimal cut-off levels from five common tumor biomarkers (CEA, CYFRA 21-1, CA125, SCC-Ag, and NSE). Therefore, we aimed to find indicators for the auxiliary diagnosis of lung cancer-associated MPE by analyzing and then validating the optimal threshold levels of these biomarkers in pleural fluid (PF) and serum, as well as the PF/serum ratio. Patients and method The study has two sets of patients, i.e. the training set and the test set. In the training set, 348 patients with PE, between January 1, 2016 and December 31, 2017, were divided into BPE and MPE based on the cytological diagnosis. Subsequently, the optimal cut-off levels of tumor biomarkers were analyzed. In the test set, the diagnostic compliance rate was verified with 271 patients with PE from January 1, 2018 to July 31, 2019 to evaluate the auxiliary diagnostic value of the aforementioned indicators. Result In the training set, PF CEA at the cut-off value of 5.23 ng/ml was the most effective indicator for MPE compared with other tumor biomarkers (all p < 0.001). In the test set, PF CEA at the cut-off value of 5.23 ng/ml showed the highest sensitivity, specificity and accuracy, positive and negative predictive value among other tumor biomarkers, which were 99.0%, 69.1%, 91.6%, 90.7%, and 95.9%, respectively. Conclusion PF CEA at the cut-off level of 5.23 ng/ml was the most effective indicator for identifying lung cancer-associated MPE among the five common tumor biomarkers.

Abstract:Objectives: In this study, we established a serum protein biomarker panel (consisting of Pro-SFTPB, CA125, Cyfra21-1, and CEA) and evaluated the feasibility and performance for the auxiliary diagnosis of lung cancer in the Chinese population.Materials and Methods: The current study was a single-center study based on the Chinese population and performed in two cohorts (training cohort and validation cohort). Serum concentrations of Pro-SFTPB, CA125, Cyfra21-1, and CEA were measured by a bead-based flow fluorescence immunoassay. The discrimination performance of the model was assessed using sensitivity, specificity, and the area under the receiver operating characteristic (ROC) curve (AUC).Results: For the biomarker panel model, the AUC was 0.88 (95% CI, 0.85-0.91) in the training cohort and 0.90 (95% CI, 0.86-0.92) in the validation data cohort, which was significantly greater than the AUC of each biomarker alone. For the nodule risk model, the AUC was improved to 0.96 (95% CI, 0.94-0.98) in the training cohort and 0.95 (95% CI, 0.93-0.97) in the validation cohort. In addition, the biomarker panel model yielded an AUC of 0.78 (95% CI, 0.74-0.81) for stage I & II lung cancer, better than the performance of individual biomarker alone.Conclusions: It was demonstrated that 4-protein biomarker panel had a significant performance in identifying lung cancer patients from healthy controls, especially combining with the nodule size. Specifically, it yielded excellent discrimination for identifying early-stage lung cancer patients than individual biomarker alone. A future large-scale study is underway to further define the clinical application of this method for the early diagnosis of lung cancer among Chinese populations.

Abstract:Objective To explore the significance of tumor markers in bronchoalveolar lavage fluid for diagnosis of primary lung cancer.Methods 86 patients with primary lung cancer (lung cancer group) and 63 patients with benign pulmonary diseases (benign disease group) were allocated for this study.The levels of CEA, CYFRA21-1, NSE and SCC-Ag of BALF were determined by electrochemiluminescent immunoassay.All these data were analyzed.Results The levels of four tumor markers in lung cancer group were higher than those of benign disease group (P<0.05).The positive rate of CEA in lung adenocarcinoma group, CYFRA21-1 in non-small cell lung carcinoma group, NSE in small cell lung carcinoma group and SCC-Ag in lung squamous cell carcinoma group were significantly different from other kinds of pathological types.The sensitivity of combination detection of these four tumor markers was 86.05%, the veracity was 77.18%,which was significantly higher than that of detection by single marker.Conclusion The four tumor markers may be applied for diagnosis of primary lung cancer.At the same time, it should be valuable for determination of clinical stage, histological type and follow-up studies of lung cancer.The combined detection of tumor markers in BALF would significantly enhance the sensitivity and accuracy of diagnosis for primary lung cancer.
