Abstract:BACKGROUND:Delirium in patients in intensive care units (ICUs) is an acute disturbance and fluctuation of cognition and consciousness. Though increasing age has been found to be related to ICU delirium, there is limited evidence of the effect of age on delirium outcomes. The aim of this study is to investigate the relationship between age categories and outcomes among ICU delirium patients.METHODS:Data were extracted from the electronic ICU (eICU) Collaborative Research Database with records from 3,931 patients with delirium. Patients were classified into non-aged (<65 years), young-old (65-74 years), middle-old (75-84 years), and very-old (≥85 years) groups. A Cox regression model was built to examine the role of age in death in ICU and in hospital after controlling covariates.RESULTS:The sample included 1,667 (42.4%) non-aged, 891 (22.7%) young-old, 848 (21.6%) middle-old, and 525 (13.3%) very-old patients. The ICU mortality rate was 8.3% and the hospital mortality rate was 15.4%. Compared with the non-aged group, the elderly patients (≥65 yeras) had higher mortality at ICU discharge (χ2 =13.726, P=0.001) and hospital discharge (χ 2=56.347, P<0.001). The Cox regression analysis showed that age was an independent risk factor for death at ICU discharge (hazard ratio [HR]=1.502, 1.675, 1.840, 95% confidence interval [CI] 1.138-1.983, 1.250-2.244, 1.260-2.687; P=0.004, 0.001, 0.002 for the young-, middle- and very-old group, respectively) as well as death at hospital discharge (HR=1.801, 2.036, 2.642, 95% CI 1.454-2.230, 1.638-2.530, 2.047-3.409; all P<0.001).CONCLUSIONS:The risks of death in the ICU and hospital increase with age among delirious patients.
Diagnosis and treatment of craniocerebral injury in elderly people

Abstract:【Objective】 To summarize clinical features and treatment experience of craniocerebral injury in elderly people.【Methods】 The clinical features and treatment of 56 elderly patients with craniocerebral injury were retrospectively analyzed.There were 46 patients undergoing surgical treatment,which includes craniotomy evacuation of hematoma(n =26),hematoma aspiration plus decompressed craniectomy(n =15),Puncture drainage(n =5).The other 10 patients did not receive surgery.Postoperative side effects as pulmonary infection,gastrointestinal hemorrhage and malnutrition were prevented and treated.【Results】Patients were followed up for 3 months.Outcomes were analyzed using Glasgow Outcome Scale(GOS): 8 cases of good recovery,15 cases of moderate disability,7 cases of severe disability,8 cases of vegetative stage and 18 cases of death.The mortality was 32.1%.【Conclusion】 The individualized treatment of craniocerebral injury in elderly people,as well as prevention and control of side effects,is the key point to improve the survival rate.There are a lot of Systemic Complication,high mortality and bad prognosis in the Aged with TBI.Early diagnosis and correctly treated the complication were a key to enhance curative effects on the Aged with TBI.

Acute Confusional Migraines: A Case Report

Abstract:Introduction: Acute confusional migraine (ACM) is a rare variant of migraine that is benign and self-resolving but difficult to diagnose. Without known causative pathophysiology and a lack of recognition in the International Classification of Headache Disorders (ICHD-3), ACM offers a puzzling clinical presentation. There currently is no standardized treatment for ACM, but with a growing anecdotal dataset there is the opportunity to formally recognize and establish protocols to improve patient care and outcomes. Case report: A 14-year-old female presented to the emergency department (ED) with acute onset of confusion, vision changes, right-sided weakness, and dysarthria one hour prior to arrival. A stroke workup at the initial ED offered no pertinent findings. The patient was transferred to a pediatric specialty ED where all symptoms, aside from numbness and a mild headache, resolved during transfer. After administration of a migraine cocktail at the pediatric specialty ED, all remaining symptoms completely resolved. The patient was discharged home from the ED the same evening with outpatient follow-up. Conclusion: This case presents the difficulty of diagnosing and treating ACM prior to its self-resolution. It highlights the need for formal recognition of the condition by the ICHD-3. In doing so, greater recognition will promote more research, awareness, and establishment of a standardized treatment for ACM.

Functional decline and delirium which are common to the elderly during hospitalization, leads to cognitive impairment and poor health outcomes. Steps taken for its prevention is usually not considered the top priority by the cardiologist. The present study was conducted to identify cognitive decline among elderly patients who developed ACS during hospitalization for noncardiac illness and their outcome. Materials and Methods: Three hundred and ten elderly patients above 60 years of age with ACS were included from June 26, 2020 to October 13, 2020. Subjects were divided into those admitted primarily due to an ACS (Group I, n = 94) and those developing ACS following admission for noncardiac illness (Group II, n = 216). Co-morbidities, medications, investigations, management, clinical outcome, and Montreal Cognitive Assessment scale were compared between the two groups at the time of admission, after 30 days and after 6 months. Results: Majority of the subjects were admitted due to acute kidney injury (27.1%) in Group II and had a non-ST elevation ACS (90.2%). Optimum management was given to a lesser extent due to the clinical condition of these patients. Poor clinical outcome, cognitive impairment during hospitalization and cognitive decline during follow-up was more in Group II. Conclusion: Clinicians must be vigilant for the development of cognitive impairment and cognitive decline when an elderly patient is admitted to the hospital, as early detection and optimum management provides better clinical and cognitive outcome.

Abstract:The present review aims to highlight this intricate syndrome, regarding diagnosis, pathophysiology, etiology, prevention, and management in elderly people. The diagnosis of delirium is based on clinical observations, cognitive assessment, physical, and neurological examination. Clinically, delirium occurs in hyperactive, hypoactive, or mixed forms, based on psychomotor behavior. As an acute confusional state, it is characterized by a rapid onset of symptoms, fluctuating course and an altered level of consciousness, global disturbance of cognition or perceptual abnormalities, and evidence of a physical cause. Although pathophysiological mechanisms of delirium remain unclear, current evidence suggests that disruption of neurotransmission, inflammation, or acute stress responses might all contribute to the development of this ailment. It usually occurs as a result of a complex interaction of multiple risk factors, such as cognitive impairment/dementia and current medical or surgical disorder. Despite all of the above, delirium is frequently under-recognized and often misdiagnosed by health professionals. In particular, this happens due to its fluctuating nature, its overlap with dementia and the scarcity of routine formal cognitive assessment in general hospitals. It is also associated with multiple adverse outcomes that have been well documented, such as increased hospital stay, function/cognitive decline, institutionalization and mortality. In this context, the early identification of delirium is essential. Timely and optimal management of people with delirium should be performed with identification of any possible underlying causes, dealing with a suitable care environment and improving education of health professionals. All these can be important factors, which contribute to a decrease in adverse outcomes associated with delirium.

Abstract:This case-based discussion focuses on the clinical presentation and diagnostic assessment of a uniquely challenging group of elderly patients: those with symptoms of mild memory problems. Such patients present a challenge to clinicians because of flux in our understandings of normal, age-related cognitive changes; of cognitive changes due to neurodegenerative illnesses; and of the relationships between depression and cognitive impairment. In addition, symptoms of memory problems may be reported by an observer rather than by the patient. These challenges warrant stepwise evaluation of elderly patients who present with symptoms of memory loss.

Abstract:Introduction Patients undergoing electroconvulsive treatment (ECT) may display an acute confusional state, often characterized by transient disorientation, inattention, memory and cognitive deficits. Objectives In this retrospective medical chart naturalistic study, we sought the determine whether white mater lesions and brain atrophy associate with the emergence of confusion during ECT treatment and preliminary results are presented herein Methods Medical charts of 24 consecutive inpatients with depression admitted to a psychogeriatric ward and subjected to bilateral frontotemporal ECT were examined retrospectively for patient and clinical characteristics. Mini-Mental State Examination (MMSE) and Geriatric Depression Scale (GDS) scores at admission and hospital discharge were retrospectively collected. Available brain Magnetic Resonance Imaging (MRI) scans were graded for lesions (white matter hyperintensities, WMH), parietal, temporal and global brain atrophy Results In this pilot study of mostly elderly patients, 50% displayed signs of confusion. All patients improved substantially, as indicated by MMSE and GDS scores, irrespectively of whether they experienced transient confusion during ECT. Preliminary results indicate that WMH are unrelated to the emergence of confusion. Instead, brain atrophy, and in particular temporal lobe and mostly frontal lobe atrophy associated with confusion Conclusions In our sample of elderly inpatients with depression subjected to bilateral ECT, preliminary results of this pilot study indicate that brain atrophy, as evidenced by MRI scans, appears as a predictor of post-ECT confusion. Moreover, the Pasquier scale, and specifically the scale sub-scores regarding brain atrophy in the frontal and temporal sulci, could prove useful in helping the clinician estimate the probability of ECT-related confusion during ECT treatment Disclosure of Interest None Declared

ADHD in Older Adults: Diagnostic Confusion and Treatment Hesitancy

Abstract:Attention Deficit Hyperactivity Disorder is an unrecognized disorder in patients over the age of 50. The evaluation of cognitive symptoms in older adults rarely considers ADHD in the differential diagnoses. His presentation will review the prevalence of ADHD in adults and its persistence over the age of 50. I will discuss the clinical presentation and subjective symptoms of these patients. I'll review ADHD symptom checklist that complements the clinical evaluation. Because of age, there are multiple factors contributing to cognitive complaints that will be discussed for a differential diagnosis. Beyond the differential diagnoses, there is a necessity to assess for other concurrent psychiatric and medical disorders that may contribute to cognitive changes. The enumeration of concurrency facilitates a diagnostic prioritization of psychiatric disorders in order to develop a thoughtful pharmacologic and treatment sequence. This sequencing will be discussed along with the medications used to treat ADHD in older adults. Because polypharmacy is often present in older adults, medical illnesses are an important consideration when prescribing ADHD medications. Drug interaction, both dynamic and kinetic, will be reviewed to highlight safe prescription practice. For older women, peri-menopause may present with declining cognition secondary to declining estrogen. This presentation will discuss diagnostic distinguish factors for ADHD in peri-menopausal women. Unfortunately, there is a paucity of clinical research from which clinicians can receive guidance. I intend to review this subject in my presentation.

Abstract: Objective To investigate the clinical features of elderly in-patients with delirium in the psychiatric consultations at general hospitals. Methods A total of 364 elderly in-patients (over 65 years old) from general hospitals who were diagnosed as delirium during the psychiatric consultation by our department during 2008 to 2011 were subjected in this study. Detailed clinical data of all subjects were retrospectively surveyed by self-made questionnaire for consultation reasons, primary physical diseases, and drug treatment after consultation, and then summarized and analyzed. Results The most common reasons for psychiatric consultation were disturbance of consciousness (55.2%) and verbal or behavioral confusion (24.6%). Medical conditions of these subjects were mainly neurological diseases (25.0%), post-operation (24.7%), cardiovascular diseases (22.5%) and respiratory diseases (19.2%). Neuroleptics were administered to 83.5%of the subjects, and they were olanzapine (46.2%), haloperidol (29.7%), quetiapine (5.2%), risperidone (3.8%) and benzodiazepines (3.8%). During the four years, the proportion of olanzapine was increased from 35.3% in 2008 to 56.9% in 2011 (P<0.01), while that of haloperidol was decreased from 38.2% in 2008 to 19.3% in 2011 (P<0.01). Conclusion Elderly in-patients are at high-risk of delirium, especially for those with cerebral infarction, infection or after surgery. More attention should be paid to these elderly patients.

Confusion and Hallucination in a Geriatric Patient. Pitfalls of a Rare Differential: Case Report of an Anti-LGI1-Encephalitis

Abstract:Background: Confusion and hallucinations in geriatric patients are frequent symptoms and typically associated with delirium, late-life psychosis or dementia syndromes. A far rarer but well-established differential in patients with rapid cognitive deterioration, acute psychosis, abnormal movements and seizures is autoimmune encephalitis. Exemplified by our case we highlight clinical and economic problems arising in management of geriatric patients with cognitive decline and psychotic symptoms. Case presentation: A 77-year-old female caucasian patient with an unremarkable medical history was hospitalized after a fall in association with diarrhea and hyponatremia. Upon adequate therapy, disorientation and troubled short-term memory persisted. Within a week the patient developed visual hallucinations. Basic blood and urine samples and imaging (cranial computed tomography and magnetic resonance imaging) were unremarkable. With progressive cognitive decline, amnestic impairment, word finding difficulty and general apathy, psychiatric and neurologic expertise was introduced. Advanced diagnostics did not resolve a final diagnosis; an electroencephalogram showed unspecific generalized slowing. Extended clinical observation revealed visual hallucinations and faciobrachial dystonic seizures. A treatment with anticonvulsants was initiated. Cerebrospinal fluid ultimately tested positive for voltage-gated potassium channel LGl1 (leucine-rich-inactivated-1) antibodies confirming diagnosis of autoimmune anti-LGI1 encephalitis. Immediate immunotherapy (high-dose glucocorticoids and administration of intravenous immunoglobulin G) led to a rapid improvement of the patient's condition. After immunotherapy was tapered, the patient had one relapse and completely recovered with reintroduction of glucocorticoids and initiation of therapy with rituximab. Conclusion: Rapidly progressive dementia in geriatric patients demands a structured and multidisciplinary diagnostic approach. Accurate management and financially supportable care is a major issue in rare diseases such as anti-LGI1-encephalitis. Education and awareness about autoimmune encephalitis of all physicians treating a geriatric population is important in order to involve expertise and establish treatment within reasonable time.
Impact of Frailty and Delirium Among Older Adults Admitted With Acute Decompensated Heart Failure

Abstract:Background: The presence of frailty or delirium among patients hospitalized for acute decompensated heart failure (ADHF) is associated with increased mortality and prolonged hospital stay. Objectives: The purpose of this study was to assess the combined effect of frailty and delirium on in-hospital mortality and disposition at discharge among older adults hospitalized with ADHF. Methods: We conducted a retrospective observational study using Nationwide Inpatient Sample data from the Agency for Healthcare Research and Quality from 2016 to 2018. Patients aged 65 years or older with a diagnosis of ADHF (both with preserved and reduced left ventricular ejection fraction) were included. For analysis, we conducted a multivariable logistic regression analysis to determine OR for in-hospital mortality or nonhome discharge from delirium and frailty. Results: A total of 3,577,433 weighted number of hospitalizations with ADHF were included. Delirium, moderate frailty risk, and high frailty risk increased the OR for in-hospital mortality (3.74; 95% CI: 3.70-3.78, 4.02; 95% CI: 3.96-4.09, and 8.63; 95% CI: 8.47-8.78, respectively) and nonhome discharge (4.21; 95% CI: 4.18-4.25, 2.95; 95% CI: 2.94-2.97, and 8.86; 95% CI: 8.78-8.94, respectively). When the combination of delirium and frailty was assessed, compared to those without delirium and with low frailty risk, the OR of mortality among those with delirium and high frailty risk was the highest at 12.18 (95% CI: 11.89-12.48). For nonhome discharge, the OR was the highest among those with delirium and high frailty risk at 14.01 (95% CI: 13.77-14.26). Conclusions: Frailty and delirium, independently and in combination, led to higher odds of in-hospital mortality and nonhome disposition at discharge among patients hospitalized with ADHF.

Abstract:Abstract Purpose To examine differences in demographics, medical history, injury characteristics, and multidomain clinical assessment outcomes between different age groups of older adults following concussion. Method This prospective study included participants aged 50+ years with a diagnosed concussion (<12 months) who presented to a specialty concussion clinic. Participants completed a clinical intake/interview (e.g., demographics) and a multidomain clinical assessment comprising symptoms (Clinical Profile Screen [CP-Screen]), cognitive (Repeatable Battery for the Assessment of Neuropsychological Status [RBANS]), psychological health (Patient Health Questionnaire-9 [PHQ-9], Generalized Anxiety Disoder-7 [GAD-7]), vestibular/ocular (Vestibular/Ocular Motor Screen [VOMS]), and quality of life (Neurological Quality of Life [Neuro-QoL]) at an initial visit. Older adults aged 50–59 and 60+ years were compared using independent samples t-tests, X2 tests, and analyses of covariance. Results 76 participants (M = 59.6¬ ± 6.3 years, 55.3% female) were included, 41 (53.9%) aged 50–59 and 35 (46.1%) aged 60+ years. The 50–59-year group had a greater proportion of females (68.3% vs. 40.0%, p = 0.01) and a history of anxiety (39.0% vs. 17.1%, p = 0.036). The 50–59-year group had worse Neuro-QoL (F[1, 72] = 5.1, p = 0.03, ηp2 = 0.10), RBANS recall (F[1, 72] = 7.1, p < 0.01, ηp2 = 0.09), RBANS fluency (F[1, 72] = 5.0, p = 0.03, ηp2 = 0.06), and RBANS list recognition (F[1, 72] = 4.1, p = 0.04, ηp2 = 0.06), controlling for sex and history of anxiety. Conclusions This study demonstrated that adults aged 50–59 years were more likely to be female and have a history of anxiety compared to adults 60+ years. Interestingly, older adults 50–59 years demonstrated greater perceived difficulties in their cognitive abilities post-concussion, which was reflected in impairments on cognitive testing compared to adults 60+ years.

Abstract:The aging of the population has increased the demand for healthcare resources. The number of patients aged 80 years and older admitted to the intensive care unit (ICU) increased during the past decade, as has the intensity of care for such patients. Yet, many physicians remain reluctant to admit the oldest, arguing a "squandering" of societal resources, that ICU care could be deleterious, or that ICU care may not actually be what the patient or family wants in this instance. Other ICU physicians are strong advocates for admission of a selected elderly population. These discrepant opinions may partly be explained by the current lack of validated criteria to select accurately the patients (of any age) who will benefit most from ICU hospitalization. This review describes the epidemiology of the elderly aged 80 years and older admitted in the ICU, their long-term outcomes, and to discuss some of the solutions to cope with the burden of an aging population receiving acute care hospitalization.

Abstract:Objectives: Although delirium is often investigated, little is known about the outcomes of patients having acute neuropsychological changes at a single time point without fulfilling the criteria of full delirium. Our aim was to determine point prevalence, predictors, and long-term outcomes of delirium and acute neuropsychological changes in patients aged 60 years and older across different departments of a university hospital with general inpatient care. Design: Prospective observational study. Setting: University hospital excluding psychiatric wards. Participants: At baseline, 669 patients were assessed, and follow-ups occurred at months 6, 12, 18, and 36. Measurements: Measurements were obtained using the Confusion Assessment Method (CAM), comprehensive geriatric assessment, health-related quality of life, functional state (month 6), and mortality rates (months 6, 12, 18, and 36). Subjects were classified into (1) patients with delirium according to the CAM, (2) patients with only two positive CAM items (2-CAM state), and (3) patients without delirium. Results: Delirium was present in 10.8% and the 2-CAM state in an additional 12.7% of patients. Highest prevalence of delirium was observed in medical and surgical intensive care units and neurosurgical wards. Cognitive restrictions, restricted mobility, electrolyte imbalance, the number of medications per day, any fixations, and the presence of a urinary catheter predicted the presence of delirium and 2-CAM-state. The mean Karnofsky Performance Score and EuroQol-5D were comparable between delirium and the 2-CAM state after 6 months. The 6-, 12-, 18-, and 36-month mortality rates of patients with delirium and the 2-CAM state were comparable. The nurses' evaluation of distinct patients showed high specificity (89%) but low sensitivity (53%) for the detection of delirium in wide-awake patients. Conclusion: Patients with an acute change or fluctuation in mental status or inattention with one additional CAM symptom (ie, disorganized thinking or an altered level of consciousness) have a similar risk for a lower quality of life and death as patients with delirium.
