Abstract:Statement of problem Previous studies have examined the maxillary esthetic zone for immediate implant placement, but these studies had small sample sizes and did not investigate multiple parameters. Purpose The purpose of this cross-sectional study was to use cone beam computed tomography (CBCT) to evaluate the sagittal root position (SRP), alveolar bone concavity, labial bone perforation (LBP), and tooth-ridge angle in the maxillary anterior teeth region. Material and methods A total of 352 CBCT images of patients (180 men and 172 women) who visited the dental hospital between 2018 and 2023 were analyzed. The sample included 2112 anterior maxillary teeth. SRP, alveolar bone concavity, tooth ridge angle, and LBP were assessed using virtual implant simulation. The data were analyzed by using chi-squared and kappa tests to analyze distributions, independence, and dependence, t tests to assess sex and tooth-specific differences, and the Spearman correlation test to explore potential correlations (α=.05). Results The CBCT images revealed that the majority of SRP were in Class I (85.4%), with smaller percentages in Class II (12.5%) and Class IV (2.3%); Class III was the least common (1%). The canine exhibited the most significant alveolar bone concavity angle average (149.14 ±6.35 degrees), followed by the lateral incisor (142.32 ±8.7 degrees). The canines had the highest occurrence of LBP (63.1%) ( P <.01). Women had a higher frequency of labial bone perforation and deeper alveolar bone concavities than men ( P <.01). Conclusions The findings of this study indicate that careful assessment and planning are necessary for immediate implant placement in the maxillary anterior region. The significant occurrence of LBP highlights the importance of thorough planning and evaluation to avoid surgical mishaps and complications which may necessitate bone grafting and result in added costs and time. Special attention should be given to women and the canine area.

Cortical demyelination in PML and MS

Abstract:Objective: To characterize pathologic changes in the cerebral cortex of patients with multiple sclerosis (MS) and progressive multifocal leukoencephalopathy (PML). Methods: Autopsy brain tissue was obtained from 13 patients with PML, 4 patients with MS, 2 patients with HIV encephalopathy, and 1 subject without neurologic pathology. Immunohistochemistry for myelin proteins, inflammatory cells, and neurofilaments was performed to evaluate the distribution of cortical lesions, their inflammatory activity, and neuritic pathology. Confocal microscopy was applied to examine pathologic changes in neurites in PML cortex. Results: Leukocortical, intracortical, and subpial patterns of cortical demyelination were represented in MS brain tissue. In PML brain tissue intracortical and leukocortical but not subpial lesions were observed. Cortical lesions in PML and MS contained fewer inflammatory cells than demyelinated areas in the white matter. Neuritic pathology in cortical PML lesions was represented by dystrophic and transected neurites. Pathologic modifications in neuritic processes in PML were more evident in highly inflamed white matter than in gray matter areas of demyelination, reminiscent of previous reports of neuritic pathology in MS. JC virus-infected cells were associated with PML white matter, leukocortical and intracortical lesions. Conclusions: Cortical pathology represents a distinct feature of progressive multifocal leukoencephalopathy. Similarities and differences with regard to multiple sclerosis cortical pathology were noted and may be informative regarding the pathogenesis of both disorders. The Two-Dimensional Size of Peri-Implant Soft Tissue in the Anterior Maxilla and Some Relevance: A 1- to 7-Year Cross-Sectional Study.

Abstract:Aim This study measured the two-dimensional size of soft and hard tissues and analysed some relevance between them. Material and methods Ninety-six maxillary anterior implants with a follow-up time 1-7 years postoperatively were evaluated. We superimposed the CBCT data of 72 patients with the optical scan data, followed by the measurements of the thickness and the height of soft and bone tissues. The shoulder of the implant was the reference point for all vertical measurements. Results At the level of implant shoulder, the mid-buccal mean thickness of soft tissue was 1.83 +/- 0.76 mm, and on the palatal side, it was 4.00 +/- 1.22 mm. The mean height of mid-buccal and mid-palatal gingiva was 4.16 +/- 1.07 mm and 4.27 +/- 1.07 mm. The buccal and palatal vertical bone wall peak was 0.41 +/- 0.73 mm and 0.22 +/- 0.57 mm coronal to the implant shoulder, and the marginal bone loss was -0.28 +/- 0.76 mm and -0.84 +/- 1.41 mm. Correlation analysis showed that the soft tissue thickness was negatively correlated with the bone thickness at 0 mm on the buccal side and at 0/2/4 mm on the palatal side apical towards from implant shoulder. The gingival height was significantly positively correlated with bone peak height and marginal bone height. Conclusion Soft and hard tissues were basically negatively correlated in the horizontal direction. Sites with little marginal bone loss or high bone peak positively correlated with higher soft tissue levels.
Evaluation of Maxillary Alveolar Ridge Morphology and Residual Bone for Implant Placement by Cone Beam Computed Tomography (CBCT)

Abstract:The aim of this study was to assess the morphology and dimension of maxillary alveolar ridge at anterior and posterior regions using cone beam computed tomography (CBCT) prior to the placement of immediate implants. CBCT scans of 192 patients were evaluated. Measurements were taken at the cross sectional views in the maxillary central incisor, lateral incisor, canine incisor, first and second premolars and first and second molars regions. Alveolar ridge width and height, buccal plate thickness, buccal undercut, inter-root septum height, and root penetration to sinus floor were measured. Independent T-test was used for statistical analysis. The mean value of alveolar height in the maxillary incisors were higher than the posterior teeth with central incisors having the highest value of 19.23 ± 8.74 mm. At anterior maxilla, the lateral incisor with the mean value of 1.07 ± 0.64 mm exhibits the highest buccal undercut and the thinnest ridge width. At the site of posterior regions premolars had higher buccal undercut than molars. Canine with the mean value of 0.19 ± 0.31 mm thickness at the mid-alveolar of buccal plate, had the thinnest plate among other maxillary incisors. Sinus penetration and inter-root septum height were higher in premolar teeth than molars. The mean value of ridge width at all levels was significantly different between men and women, except at the level of crest in central incisor. CBCT evaluation presents accurate measurements for ridge assessment prior to immediate implant insertion. The recommended appropriate size of implant platform would be 5 mm in the incisor and almost 6 mm in premolar region.
Assessment of Posterior Maxillary Alveolar Bone for Immediate Implant Placement: A Quantitative and Qualitative Analysis

Abstract:Objectives: The aims of this study were to evaluate posterior maxillary alveolar bone dimensions and to compare these dimensions in males and females. Materials and methods: The sample consisted of 102 cone beam computed tomography (CBCT) images for 62 male patients (mean age 29.92 ± 9.04 years) and 40 female patients (mean age 29.70 ± 9.54 years). Four distances and three densities were measured; a multivariate analysis of variance and Mann-Whitney's U test were applied to compare the differences between sexes. Results: For the first maxillary molar, there were significant differences between males and females in terms of coronal width (13.95 ± 1.31 and 13.22 ± 1.159 mm, respectively) and middle width (14.28 ± 1.43 and 13.57 ± 1.478 mm, respectively). However, no significant difference was found regarding height (7.93 ± 3.8 mm for both) or apical width (14.68 ± 2 mm for both). Regarding the second maxillary molar, significant differences between males and females were found in terms of coronal width (14.66 ± 1.63 and 13.54 ± 1.512 mm, respectively), middle width (14.35 ± 1.825 and 13.25 ± 1.52 mm, respectively), and height (7.29 ± 3.00 and 8.66 ± 3.16 mm, respectively), whereas the gender dimorphism regarding apical width had borderline significance (14.09 ± 1.731 mm; p = 0.048). No significant differences were found regarding density. Conclusion: The minimum average alveolar bone height for the second maxillary molar region was 7.29 ± 30 mm with significant gender dimorphism. Therefore, CBCT scans should be recommended prior to immediate implant placement.

Abstract:Rare earth (RE) materials, which are excited in the ultraviolet and emit in the visible light spectrum, are widely used as phosphors for lamps and displays. In the 1960's, researchers reported an abnormal emission phenomenon where photons emitted from a RE element carried more energy than those absorbed, owing to the sequential energy transfer between two RE ions--Yb(3+)-sensitized Er(3+) or Tm(3+)--in the solid state. After further study, researchers named this abnormal emission phenomenon upconversion (UC) emission. More recent approaches take advantage of solution-based synthesis, which allows creation of homogenous RE nanoparticles (NPs) with controlled size and structure that are capable of UC emission. Such nanoparticles are useful for many applications, especially in biology. For these applications, researchers seek small NPs with high upconversion emission intensity. These UCNPs have the potential to have multicolor and tunable emissions via various activators. A vast potential for future development remains by developing molecular antennas and energy transfer within RE ions. We expect UCNPs with optimized spectra behavior to meet the increasing demand of potential applications in bioimaging, biological detection, and light conversion. This Account focuses on efforts to control the size and modulate the spectra of UCNPs. We first review efforts in size control. One method is careful control of the synthesis conditions to manipulate particle nucleation and growth, but more recently researchers have learned that the doping conditions can affect the size of UCNPs. In addition, constructing homogeneous core/shell structures can control nanoparticle size by adjusting the shell thickness. After reviewing size control, we consider how diverse applications impose different requirements on excitation and/or emission photons and review recent developments on tuning of UC spectral profiles, especially the extension of excitation/emission wavelengths and the adjustment and purification of emission colors. We describe strategies that employ various dopants and others that build rationally designed nanostructures and nanocomposites to meet these goals. As the understanding of the energy transfer in the UC process has improved, core/shell structures have been proved useful for simultaneous tuning of excitation and emission wavelengths. Finally, we present a number of typical examples to highlight the upconverted emission in various applications, including imaging, detection, and sensing. We believe that with deeper understanding of emission phenomena and the ability to tune spectral profiles, UCNPs could play an important role in light conversion studies and applications.
Evaluation of mandibular first molar teeth in the context of immediate implant placement using Cone Beam Computed Tomography

Abstract:Objective: During the immediate dental implant (IMI) procedure in the mandibular posterior region, some limitations caused by anatomical structures that may affect the success of implant treatment and increase the risk of complications may be encountered. Socket size, distance from root apices to the inferior alveolar canal (IAC), and lingual concavity are some of the critical conditions. This study aimed to examine the alveolar bone of mandibular first molars using cone beam computed tomography (CBCT) and to evaluate its prevalence on a subpopulation basis. Material and Methods: A total of 153 mandibular first molar teeth in 100 patients who met the evaluation criteria were evaluated for cross-sectional classification of the alveolar bone, distance from root apices to IAC, and socket size using CBCT scans. Results: In this study, which included 42 females and 58 males, the age range was 19-70 years (38.13±13.74 years). Of the 153 mandibular first molars analyzed, 53.6% were on the left, while 46.4% were on the right. The distances from the apices of the roots to the IAC were the least in females (p<0.05) and U-type ridges. It was also found that this distance was positively correlated with age. The mean crestal socket width measured in the current study was suitable for choosing a dental implant with an appropriate diameter for IMI surgery. Conclusion: Cross-sectional analysis of the relevant regions before surgery is important for IMI placement. This will allow clinicians to take precautions against possible complications.
Estimating the occurrence of labial bone perforation and implantation into the maxillary sinus maxillary premolars based on the morphology of maxillary premolars: A clinical study

Abstract:Statement of problem: Previous studies have classified the sagittal root position of the maxillary anterior teeth and measured buccal plate thickness to aid treatment planning. A thin labial wall and buccal concavity may cause buccal perforation, dehiscence, or both in maxillary premolars. However, data on the restoration-driven principle to classify the maxillary premolar region are lacking. Purpose: The purpose of this clinical study was to investigate the occurrence of labial bone perforation and implantation into the maxillary sinus between various tooth-alveolar classifications with respect to the crown axis in maxillary premolars. Material and methods: Cone beam computed tomography images of 399 participants (1596 teeth) were analyzed to determine the probability of labial bone perforation and implantation into the maxillary sinus when associated with variables that included tooth position and tooth-alveolar classification. Results: The morphology in the maxillary premolars was classified as straight, oblique, or boot-shaped. The first premolars were 62.3% straight, 37.0% oblique, and 0.8% boot-shaped, and labial bone perforation occurred in 4.2% (21 of 497) of the straight, 54.2% (160 of 295) of the oblique, and 83.3% (5 of 6) of the boot-shaped first premolars when the virtual implant was 3.5×10 mm. When the virtual tapered implant was 4.3×10 mm, labial bone perforation occurred in 8.5% (42 of 497) of the straight, 68.5% (202 of 295) of the oblique, and 83.3% (5 of 6) of the boot-shaped first premolars. The second premolars were 92.4% straight, 7.5% oblique, and 0.1% boot-shaped, and labial bone perforation occurred in 0.5% (4 of 737) of the straight, 33.3% (20 of 60) of the oblique, and 0% (0 of 1) of the boot-shaped, respectively, when the virtual tapered implant was 3.5×10 mm; and labial bone perforation occurred in 1.3% (10/737) of the straight, 53.3% (32/60) of the oblique, and 100% (1/1) of the boot-shaped second premolars when the virtual tapered implant was 4.3×10 mm. Conclusions: When an implant is placed in the long axis of a maxillary premolar, the tooth position and tooth-alveolar classification should be considered when assessing the risk of labial bone perforation. Attention should be paid to the implant direction, diameter, and length in the oblique and boot-shaped maxillary premolars.
Risk Assessment of Nasal Cavity Perforation in the Maxilla: A Virtual Implant Placement Study Using Cone Beam Computed Tomography

Abstract:Implant surgery in individuals with atrophic maxilla presents challenges, particularly concerning nasal cavity complications such as perforations, implant migration, and airway obstruction. While panoramic radiographs offer diagnostic convenience, their two-dimensional nature limits the evaluation of anatomical structures. Cone beam computed tomography (CBCT) provides a three-dimensional assessment, enhancing surgical planning accuracy and potentially reducing complications. With the aim of understanding the measurement differences between panoramic radiographs and CBCT images, this retrospective study examined CBCT images of patients with severely atrophic maxilla taken between September 2021 and December 2023 at the Ahmet Keleşoğlu Faculty of Dentistry. Virtual implants were placed in various tooth regions using OnDemand3D software. The incidence of nasal cavity perforations and vertical bone height differences between panoramic radiographs and CBCT images were evaluated. For this purpose, vertical bone length measurements in panoramic and CBCT images were compared for the virtual implant placement areas. Statistical analyses, including t-tests and ANOVA, were performed to determine significant differences among quantitative measurements, and a chi square test with Bonferroni corrected z-tests were used for possible associations between ratios. CBCT data from 59 patients, totaling 1888 virtual implants, revealed significant differences in implant depths among tooth regions (F = 9.880, p < 0.001). Canine regions showed higher perforation risks, especially with 12 mm and 14 mm implants. Panoramic radiographs often overestimated vertical bone height in canine and first premolar regions compared to CBCT measurements, which could lead to increased perforation risks. Radiographic evaluations using CBCT prior to implant surgery in atrophic maxilla cases are crucial to prevent nasal cavity complications. Panoramic radiographs may inadequately represent three-dimensional anatomy, underscoring the importance of CBCT for accurate implant placement and surgical planning. Further studies should consider varying implant sizes and brands to generalize findings.

Factors influencing the sinus membrane thickness in edentulous regions: a cone-beam computed tomography study

Abstract:Background: During implant treatment in the maxillary molar area, maxillary sinus floor augmentation is often performed to ameliorate the reduced alveolar bone height attributable to bone remodeling and pneumatization-induced expansion of the maxillary sinus. However, this augmentation may cause complications such as misplaced implants, artery damage, and maxillary sinus mucosal perforation; infections like maxillary sinusitis; and postsurgical complications such as bone graft leakage and postoperative nasal hemorrhaging. To reduce the complications during maxillary sinus floor augmentation and postoperative infections, we performed retrospective investigations of various systemic and local factors that influence pre-operative sinus mucosal thickness (SMT) by using cone-beam computed tomography (CBCT). Subjects included patients who underwent maxillary sinus floor augmentation in an edentulous maxillary molar area with a lateral approach. Pre-operative SMT, existing bone mass, and nasal septum deviation were measured using CBCT images. Relationships between SMT and the following influencing factors were investigated: (1) age, (2) sex, (3) systemic disease, (4) smoking, (5) period after tooth extraction, (6) reason for tooth extraction, (7) residual alveolar bone height (RBH), (8) sinus septa, and (9) nasal septum deviation. Correlations were also investigated for age and RBH (p < 0.05). Results: We assessed 35 patients (40 sinuses; 11 male, 24 female). The average patient age was 58.90 ± 9.0 years (males, 57.9 ± 7.7 years; females, 59.9 ± 9.4 years; age range, 41-79 years). The average SMT was 1.09 ± 1.30 mm, incidence of SMT > 2 mm was 25.0%, incidence of SMT < 0.8 mm was 50.0%, and the average RBH was 2.14 ± 1.02 mm. The factors that influenced SMT included sex (p = 0.0078), period after tooth extraction (p = 0.0075), reason for tooth extraction (p = 0.020), sinus septa (p = 0.0076), and nasal septum deviation (p = 0.038). Conclusions: Factors associated with higher SMT included male sex, interval following tooth extraction < 6 months, periapical lesions, sinus septa, and nasal septum deviation. Factors associated with SMT > 2 mm were sex and reason for tooth extraction, while factors associated with SMT < 0.8 mm were time following tooth extraction and nasal septum deviation. Despite the limitations of this study, these preoperative evaluations may be of utmost importance for safely conducting maxillary sinus floor augmentation.
Three-dimensional alveolar bone assessment of mandibular molars for immediate implant placement: a virtual implant placement study

Abstract:Background To elucidate the anatomical features of the mandibular molar region to allow safe immediate implant placement. Methods Cone-beam computed tomography images of 150 patients (600 teeth) were reviewed retrospectively. The virtual implants were placed in the mandibular first and second molar region. The anatomic structures of the mandible and inter-radicular septum were both categorized into three types. The relationship between implant and inferior alveolar nerve (IAN), and the horizontal distance from the implant surface to the bone wall were analyzed. Variables were compared using a student’s t-test, or Mann–Whitney U test. Results Type U (39.0%) and type S (56.0%) were the most common in the first molar, while type U (67.7%) and type M (54.7%) had the highest prevalence rate in the second molar. The mean distance from the level where the virtual implant was completely surrounded by bone to IAN was 7.06 mm. The mean horizontal widths from the implant to the mesial and distal socket wall were 1.59 mm and 1.89 mm. The widths of the inter-radicular septum and the distances from implant to the buccal and lingual plate on different sections were significantly associated with tooth position ( P < .05). Conclusions In the first molar region, the implant is suggested to be placed in the center of the inter-radicular septum, while in the second molar region, the mesial root socket could be considered. Immediate implant placement in the mandibular second molar sockets shows a high risk of IAN injury, lingual perforation, and inadequate primary stability.
Immediate implant placement in the posterior mandible: A cone beam computed tomography study

Abstract:Objective: To determine the longest upright and tilted implant that can be placed in posterior mandibular teeth (second premolars [2PM], first molars [1M], and second molars [2M]) in case of immediate implant placement (IIP), respecting a safe distance from the inferior alveolar canal (IAC) and the submandibular fossa, using cone beam computed tomography (CBCT) scans. Method and materials: In CBCT scans of 118 subjects, the following were measured: distance from the tooth apex to the IAC (DTC), lateromedial width of the mandible, classification of mandibular cross-sectional morphology, upright and tilted implant placement simulation, tilting angle that allowed placement of the longest implant. Results: The DTC has shown to be greater for 1M, shorter for 2PM, and even shorter for 2M. Between 70% and 88% of all 2PM, 1M, and 2M present less than 6 mm of DTC. The cross-sectional morphology of the mandible had a tendency to be classified as "undercut" in more posterior regions. For both 4.0- and 5.0-mm implants, more posterior regions resulted in a greater difference in length between the upright and tilted implants. The tilting angle was greater for the more posterior regions. Most of the relationships between age/sex and the morphometric parameters were weak. Conclusion: Lingual concavity is a common clinical finding at the posterior mandibular region, and its occurrence varies according to the tooth type. The great majority of the 2PM, 1M, and 2M present limited available bone between the root apex and the IAC, which stresses the need for careful attention when IIP is planned in this area.
Assessment of Buccal and Palatal Alveolar Bone Thickness in Maxillary Anterior Teeth on Cone Beam Computed Tomography

Abstract:Introduction: Evaluating the dimensions of buccal and palatal alveolar bone in the anterior maxillary teeth is very important in patients who are candidates for immediate implant replacement. The aim of this study was Assessment of buccal and palatal alveolar bone thickness in maxillary anterior teeth on cone beam computed tomography (CBCT). Materials & Methods: In this descriptive-analytical study, 78 CBCT images of patients referred to Radiology Department of Khorasgan Dental Faculty were evaluated. The collected data included the measurement of the bone thickness of the alveolar buccal and palatal anterior maxillary teeth at three locations of the crest, the middle region of the root and apex with the Sidexis software. Then, the measurements were compared between males and females. The data were analyzed by T-test statistical methods. Results: The mean thickness of the buccal alveolar bone in the crest region and the middle region of the central, lateral and canine teeth was less than 1 mm. The mean thickness of the buccal alveolar bone at the apex site of the central, lateral and canine teeth was more than one mm. The mean thickness of the palatal alveolar bone in the crest region of the central, lateral and canine teeth was less than one millimeters. The mean thickness of the palatal alveolar bone in the middle region of the root and apex was more than one mm. There was no statistically significant difference between the sex and thickness of the buccal alveolar bone, except at the apex of the central and lateral teeth (p value > 0.05). Between sex and thickness of palatal alveolar bone, in the crest of lateral and canine teeth and in the middle root and apex area of all three central, lateral and canine teeth in men were significantly more than women (p value < 0.05). Conclusion: The buccal alveolar bone was present in most of the points examined at the crest and mid-root site, and the palatal alveolar bone was at the crest site at most checked points with a thickness of less than 1 mm.
Preoperative Cone Beam Computed Topography Assessment of Maxillary Sinus Variations in Dental Implant Patients

Abstract:This study aimed to evaluate the pathological factors and anatomical variations in the maxillary sinus in patients undergoing dental implant treatment using cone beam computed tomography (CBCT). CBCT, as a key imaging technique in dentistry, offers high-resolution images to assess bone morphology and quality, crucial for preoperative dental implant planning. Material and methods: The study included a cohort of 200 patients recommended for CBCT as part of their preoperative evaluation. The methodology involved detailed CBCT image analysis to identify and document various anatomical variations due to pneumatization, exostosis, hypoplasia, polyps, cysts, foreign bodies, and anthroliths within the maxillary sinus. Results: Pneumatization was the most common variation, present in 77.5% of subjects. Polypoid lesions were found in 17.5% of patients, with a higher prevalence in younger age groups (57.1% in ages 20-35). Cysts and polyps affected 17.5% of subjects, predominantly males (65.7%). Anthroliths were observed in a minimal percentage (2%), and foreign bodies were found in 1.5% of the patients. Positive correlations were observed between the patient's age and both mucosal thickness and polypoid lesions and between the patient's gender and bone thickening (p-values < 0.05). Conclusions: The study concluded that CBCT is essential in the preoperative assessment of the maxillary sinus in dental implant candidates due to its superior imaging capabilities, allowing for the identification of critical anatomical variations and pathologies. This thorough evaluation is imperative to ensure the success of implant placement and to mitigate potential complications. However, further research with larger, more diverse populations is recommended to confirm these findings.

Abstract:We report on engineering risk factors associated with clinician- applied forces during vaginal delivery of newborns. Specifically, we present and interpret data from a series of experiments using force-sensing devices on 29 randomly selected vaginal births, including two shoulder dystocia deliveries and one birth injury. The results indicate that clinician-applied peak forces are typically about 47 N for routine deliveries, 69 N for difficult deliveries, and 100 N for a shoulder dystocia delivery (P<.01). The time required to deliver fetal shoulders doubles for nonroutine deliveries (P<.01). In addition, impulse and rate of application of force distinguish between routine and nonroutine deliveries (P<.03). We conclude that, if properly perceived, force, force rate, and the duration of force are objective parameters that can be used in recognizing and managing shoulder dystocia and in predicting thresholds for birth injury

Abstract:Background: The objective of this paper is to anatomically describe the bone morphology in the maxillary and mandibular tooth areas, which might help in planning post-extraction implants. Methods: CBCT images (Planmeca ProMax 3D) of 403 teeth (208 upper teeth and 195 lower teeth) were obtained from 49 patients referred to the Dental School of Seville from January to December 2014. The thickness of the facial wall was measured at the crest, point A, 4 mm below, point B, and at the apex, point C. The second parameter was the angle formed between the dental axis and the axis of the basal bone. Results: A total of 403 teeth were measured. In the maxilla, 89.4% of incisors, 93.94% of canines, 78% of premolars and 70.5% of molars had a buccal bone wall thickness less than the ideal 2 mm. In the mandible, 73.5% of incisors, 49% of canines, 64% of premolars and 53% of molars had < 1 mm buccal bone thickness as measured at point B. The mean angulation in the maxilla was 11.67 ± 6.37° for incisors, 16.88 ± 7.93° for canines, 13.93 ± 8.6° for premolars, and 9.89 ± 4.8° for molars. In the mandible, the mean values were 10.63 ± 8.76° for incisors, 10.98 ± 7.36° for canines, 10.54 ± 5.82° for premolars and 16.19 ± 11.22° for molars. Conclusions: The high incidence of a buccal wall thickness of less than 2 mm in over 80% of the assessed sites indicates the need for additional regeneration procedures, and several locations may also require custom abutments to solve the angulation problems for screw-retained crowns.

Abstract:OBJECTIVES: This study aims to determine if there are enough buccal alveolar bone thickness to perform an immediate dental implant placement in anterior and posterior maxillary teeth.MATERIAL AND METHODS: A total of 1463 teeth were examined, from 202 cone-beam computed tomography scans with voxel sizes of 0.15 mm. On each tooth, the following measures were determined: the alveolar bone thickness in two locations; the vertical distance between the buccal alveolar crest and cementoenamel junction; the angle between the tooth's long axis and the alveolar bone axial inclination in the sagittal plane.RESULTS: In the most coronal location of maxillary teeth, the thickness of alveolar bone was lower than 0.6 (SD 0.6) mm in 50% of the teeth, and in the middle of the root the bone thickness was, on average, 0.96 (SD 0.6) mm. For the same maxillary teeth, the vertical distance between the buccal alveolar crest and cementoenamel junction and the angulation measured were, on average, 3.6 (SD 1.2) mm and 12.1º (SD 1.4º), respectively.CONCLUSIONS: The present study revealed that in most cases the thickness of buccal alveolar bone was less than 1 mm. Consequently, in such cases, immediate dental implant placement operation is not recommended, or should be combined with bone regeneration techniques.
Anatomical Factors Affecting the Complexity of Maxillary Sinus Augmentation in Saudi Patients: A Cone Beam Computed Tomography (CBCT) Study

Abstract:Aim: This study aims to evaluate the anatomical variations of the maxillary sinus and determine the difficulty score of maxillary sinus augmentation (MSA) in Saudi patients seeking dental implant rehabilitation of the posterior maxilla using cone beam computed tomography (CBCT). Methodology: CBCT records of dental patients seeking treatment at a University Dental Teaching Hospital between 2019 and 2023 were retrospectively analyzed. Measurements were obtained from CBCT images, including membrane thickness, sinus width, buccal bone thickness, presence of maxillary sinus septa, residual alveolar ridge height, angle of the buccolingual sinus wall, and the presence of the alveolar antral artery (AAA). The difficulty score for MSA was determined based on these anatomical factors. Results: A total of 107 maxillary sinuses in 86 subjects were evaluated. The average membrane thickness was 2.23 mm, with males showing significantly higher thickness than females. Sinus septa were found in 54 (50.5%) sinuses, with 18 (17%) of sinuses having interfering septa. Twenty-three (21.5%) sinuses had a residual alveolar ridge height of less than 4 mm. The average angle of the buccolingual sinus wall was 79.39°, indicating a high prevalence of wide-shaped sinuses. The average sinus width was 14.09 mm, with 55 sinuses (51.4%) less than 15 mm. The average buccal bone thickness was 1.07 mm, in 29 (27%) sinuses, the thickness was more than 2 mm. AAA was visualized in 60 (56%) of sinuses, with 45 (42%) of sinuses having AAA interfering with the MSA window. Conclusions: In this study, most sinuses were classified as simple or moderate difficulty, with higher membrane thickness, presence of septa, and AAA being the significant risk factors for complications. These findings provide valuable insights for implant surgeons in Saudi patients seeking dental implant rehabilitation of the posterior maxilla, enabling them to anticipate and minimize potential complications during MSA procedures.

Abstract:Objective: The objective of this study was to evaluate the prevalence and measure the depth of the mandibular lingual concavities, using Cone-Beam Computed Tomography (CBCT) scans. This was done in an attempt to avoid perforation of the lingual cortex during dental implant insertion. Methodology: CBCT scans of 174 patients requiring dental implants were used in this study. The scans included 86 females and 88 males. On each scan, the depth of the lingual concavity was measured at the first and second molar areas. These measurements were then tabulated and statistically analyzed to compare the prevalence and depth of the lingual concavity at the first and second molar areas, on the right and left sides, in males and females, and in dentate and edentulous cases. Results: There was a statistically significant difference between the mean depth of the lingual concavity in males and females, as it was higher in males (4.85mm) than in females (4.09mm). The mean depth of the concavity was greater at the second molar than at the first molar in both males and females. The concavities were also found to be deeper on the left side of the mandible in both genders. Conclusion: In the examined Egyptian population, the concavity depth was greater in males than in females and greater in the second molar area, on the left side of the mandible. Therefore, CBCT imaging is crucial for accurate preoperative examination in dental implant cases to avoid complications such as lingual perforations.
Analysis of Facial Bone Wall Dimensions and Sagittal Root Position in the Maxillary Esthetic Zone: a Retrospective Study Using Cone Beam Computed Tomography.

Abstract:PURPOSE:To determine the thickness of the facial bone wall and the sagittal angulation between the long axis of the teeth and the long axis of the associated alveolar bone in the maxillary esthetic zone using cone beam computed tomography (CBCT).MATERIALS AND METHODS:A retrospective radiographic study of CBCT images was conducted in 300 patients. The distance between the cementoenamel junction (CEJ) and the facial bone crest, the thickness of the facial bone wall (4 mm apical to the CEJ and midroot), and the sagittal angle between the long axis of teeth and the long axis of the respective alveolar bone were measured. Descriptive statistics and frequency analyses were performed.RESULTS:The sample included 133 men and 167 women (mean age, 36.9 years; range, 18 to 60 years). The distance between the CEJ and the facial bone crest varied between 0.1 and 4 mm and showed an overall tendency to increase with age. Almost 80% of anterior teeth and 40% of premolars exhibited a thin facial bone wall (< 1 mm), and nearly 30% of sites had a bone wall thinner than 0.5 mm. The sagittal angle at approximately 80% of central incisor and second premolar sites was < 20 degrees, but the angle at more than 40% of canine sites was ≥ 30 degrees. There was a significant decrease in facial bone wall thickness from premolars to anterior teeth.CONCLUSION:The facial bone wall in most maxillary anterior teeth was very thin. There was a substantial sagittal angulation between the long axes of teeth and those of their respective alveolar bone in most esthetic zone positions. CBCT analyses of the facial bone wall and the sagittal angle are recommended to ensure the most appropriate dental implant treatment approach.
