Abstract:We present a case of a life-threatening almost complete airway obstruction resulting from poorly differentiated thyroid carcinoma in a 48-year-old male. Airway obstruction may lead to unexpected mortality by suffocation and patients with poorly differentiated thyroid carcinoma usually have a fast deterioration and fatal outcome. In the case presented, we describe a safe and effective treatment strategy. Assisted by femoro-femoral cardiopulmonary bypass oxygenation, a tracheal stent was implanted successfully. Following surgery there were no complications, and chemoradiotherapy resulted in the relief of obstructing symptoms and improved the quality of life of the patient. This case indicates that femoro-femoral cardiopulmonary bypass provides adequate oxygen support to undergo further management and that tracheal stent implant is an effective emergent measure to relieve severe airway obstruction in patients with poorly differentiated thyroid carcinoma.
Systematic sequential therapy for ex vivo liver resection and autotransplantation: A case report and review of literature

Abstract:BACKGROUND Perihilar cholangiocarcinoma (pCCA) is a highly malignant tumor arising from the biliary tree. Radical surgery is the only treatment offering a chance of long-term survival. However, limited by the tumor's anatomic location and peri-vascular invasion, most patients lose the chance for curative treatment. Therefore, more methods to increase the resectability of tumors as well as to improve outcomes are needed.CASE SUMMARY A 68-year-old female patient had a hepatic hilar mass without obvious symptoms. Laboratory results showed hepatitis B positivity. Magnetic resonance imaging indicated that the mass (maximum diameter: 41 mm) invaded the left and right branches of the main portal vein, as well as the middle, left and right hepatic veins; enlarged lymph nodes were also detected in the hilum. The patient was diagnosed with pCCA, and the clinical stage was determined to be T4N1M0 (stage IIIC). Considering the tumor's anatomic location and vascular invasion, systematic conversion therapy followed by ex vivo liver resection and autotransplantation (ELRA) was determined as personalized treatment for this patient. Our original systemic sequential therapeutic strategy (lenvatinib and tislelizumab in combination with gemcitabine and cisplatin) was successfully adopted as conversion therapy because she achieved partial response after three cycles of treatment, without severe toxicity. ELRA, anastomotic reconstruction of the middle hepatic vein, right hepatic vein, root of portal vein, inferior vena cava and right hepatic artery, and lymph node dissection were performed at one month after systemic therapy. Pathological and immunohistochemical examination confirmed the diagnosis of pCCA with lymph node metastasis. Although the middle hepatic vein was partially obstructed four months later, hepatic vein stent implantation successfully addressed this problem. The patient has survived for 22 mo after the diagnosis, with no evidence of recurrence or metastasis.CONCLUSION An effective therapeutic strategy for conversion therapy greatly increases the feasibility and efficiency of ELRA.
Surgical techniques and perioperative management of lung transplantation:a report of 3 cases

Abstract:Objective To investigate the management points to note and effective program during perioperative period of lung transplant surgery.Methods Three cases of lung transplantation were carried out in the First Affiliated Hospital,College of Medicine,Zhejiang University from August 2011 to June 2012.The patients were a 55-year-old male with idiopathic pulmonary fibrosis(case 1),a 71-year-old female with double emphysema associated with chronic bronchitis(case 2),and a 56year-old male with bilateral bronchiectasis(case 3).The surgical approaches were lung transplantation(case 1) and sequential bilateral single-lung transplantation(case 2 and 3).Auxiliary extracorporeal membrane oxygenation was used during the surgery in case 1 and 3.The lung grafts were harvested en bloc with the hearts and then were separated and pruned in vitro before transplantation.After transplantation,measures for lung water control,immunosuppression,and anti-infection were carried out.Results The 3 patients received successful surgeries,and the operative time was 143,316,and 336 min,respectively.The cold ischemia time was 140 min,left 182 min and right 288 min,and right 245 min and left 377 min,respectively.The blood loss during the operation was 2850,3200,and 1300 mL,respectively.Two patients(case 1 and 3) were pulled out trachea cannula extubation within 24 h and then were moved into general ward in 2 weeks,and were discharged from hospital in 5 weeks.By now,they have been alive in satisfactory condition for 10 and 5 months.Case 2,however,was with serious infection after operation and failed to get rid of the ventilator and died of infection encephalopathy on postoperative day 82.Surgical complications and graft rejection did not occur in all 3 patients.Conclusion Comprehensive effective surgical techniques and perioperative management are the key to successful lung transplantation for the treatment of end-stage lung diseases.
Traumatic laryngotracheal stenosis treated by hyoid-sternohyoid osseomuscular flap combined with xenogenic acellular dermal matrix: A case report and literature review.

Abstract:Objective The treatment of laryngotracheal stenosis is a major therapeutic challenge. Various treatments include observation, medical management, and surgical management. The most effective surgical management is resection and reconstruction. To the authors' knowledge, no reports have described the use of xenogenic acellular dermal matrix (ADM) for laryngotracheal stenosis. Methods A 27-year-old man presented with hemoptysis of the neck due to a traffic accident. Emergency orotracheal intubation was performed. Tracheostomy was then performed under local anesthesia. Computed tomography revealed fractures of the right thyroid cartilage and posterior arc of the cricoid cartilage and stenosis of the subglottis and first and second tracheal rings. We used a composite hyoid-sternohyoid osseomuscular flap with xenogenic ADM and a straight silicone tube as a lumen stent to reconstruct the laryngotracheal stenosis. Results Surgical recovery was uneventful. The tracheotomy opening was changed to a metal tube 5 days postoperatively. Four months postoperatively, the silicone tube was endoscopically removed under local anesthesia. The patient was decannulated 20 days later. The patient satisfied with his voice, respiration, and deglutition at the 16-month postoperative follow-up. Conclusion The use of ADM for laryngotracheal stenosis may reduce the growth of granulation tissues and promote the repair process.

Abstract:To summarize the emergency nursing experience of a patient with massive hemoptysis after lung transplantation undergoing rigid bronchoscopy assisted by extracorporeal membrane oxygenation. For patients with massive hemoptysis without symptoms,a comprehensive emergency plan was adopted,including safe and quick transport,extracorporeal membrane oxygenation care,bedside rigid bronchoscopy management,and monitoring of patient bleeding and thrombosis. After 12 days of active treatment and care,the patient recovered and was discharged from the hospital.
[Perioperative care in lung transplantation: report of 18 cases]

Abstract:Objective: To evaluate monitoring and cares in the intensive care unit (ICU) after lung transplantation. Methods: From September 2002 to April 2005, there were 18 cases of lung transplant recipients, who had suffered from emphysema (9 cases), pulmonary fibrosis (5 cases), silicosis (1 cases), bronchiectasis (1 case), pulmonary vascular leiomyoma (1 case), ventricular septal defect and Eisenmenger's syndrome (1 case), respectively. Operative procedures included 9 cases with left lung transplantation, 5 right lung transplantation, 1 case right single lung transplantation and ventricular septal defect repair, and 3 cases bilateral lung transplantations. All the patients received mechanical ventilation, immunosuppressive agents, antibacterial prophylaxis, and prevention of reperfusion injury in the ICU after operation. Preoperatively, echocardiography (ECHO), artery blood gas,and oxygenation index (partial pressure of oxygen in artery/fraction of inspired oxygen, PaO(2)/FiO(2)) were observed. Results: The average weaning time from the ventilator was (7.39+/-4.89) days. The average ICU stay time was (9.72+/-8.32) days. The systolic pulmonary artery pressure (Ppa, syst) was monitored with Swan-Ganz catheterization 1 week post transplant, and it was found to have decreased significantly from (48.94+/-14.45) mm Hg (1 mm Hg=0.133 kPa) to (39.59+/-7.45) mm Hg (P<0.05). At the same time, oxygenation index was improved from (263.89+/-82.09) mm Hg to (345.56+/-92.18) mm Hg (P<0.05), partial pressure of carbon dioxide in artery (PaCO(2)) was decreased from (63.29+/-22.56) mm Hg to (38.37+/-9.19) mm Hg (P<0.05). In hospital mortality (HM) was 16.7% (3/18 cases), and an early death was due to severe infection on the 30 th postoperative day in 1 patient and acute rejection on the 15 th postoperative day in another patient, and the other patient died due to pulmonary vein embolism on the 36 th day. Fifteen patients recovered quickly and discharged from the hospital. One patient was followed up for 32 months. Conclusion: Lung transplantation remains the only hope for many patients with end stage pulmonary disease. It is important that the lung transplant team possesses a working knowledge of the treatment of common complications, the time of these complications mostly likely to occur and how best to treat them when they do arise, to ensure long-term survival and success.
Surgical Procedures in Five Heart Transplant Patients

Abstract:ObjectiveTo summarize the surgical outcomes and clinical experience of surgical disease for patients having undergone orthotopic heart transplantation.MethodsFive cardiac transplant recipients requiring surgical management due to other surgical diseases including acute cholecystitis in 2 patients,acute appendicitis,bilateral mammary hypertrophy and lung tumor in 1 patients,respectively.The mean age of the entire group at the time of reoperation was 44.6 years(14-60 years) and the average time of operative procedures after transplant was 16.4 months(4-37 months).Four patients were treated with triple immunosuppression,including cyclosporine A(CsA) or tacrolimus,mycophenolate mofetil and corticosteroids,respectively.One patient received double-therapy of CsA and mycophenolate mofetil with early withdrawal of corticosteroids.All the acute cholecystitis and appendicitis patients underwent open cholecystectomy and appendectomy emergently.Reduction mammaplasty was performed on the bilateral mammary hypertrophy patients.For the lung tumor patient,right upper lobectomy and nodes excision were undertaken radically after the sample proved to be malignancy by the thoracoscopy.Closely surveillance at concentration of CsA or FK506 was performed continueously in order to adjust the effective blood concentration in a steady way by which acute rejection can be avoided.The pathways which pathogen organisms invading the human body were controlled strictly as well as intension on anti-infection treatment during perioperative period.ResultsFour patients discharged to home within 2 weeks.Only one patient needed further treatment in Digestive Department after emergent cholecystectomy due to gastric retention.And she was discharged after 66 days.No acute rejections or operative complications such as severe infection or bleeding were found during the perioperative period.The average length of stay was 21.3 days(8-66 days).During the follow-up from 1 month to 13 months,there was no relapse or allograft disfunction performed on any patients.All of them enjoy quality lives.ConclusionsMore attention should be paid to regular follow-up after transplantation,by which the emergent surgical diseases can be diagnosed and treated earlier and more effectively.Cardiac transplant recipients who subsequently require surgical intervention do quite well overall.Most of them can obtain excellent surgical outcomes.

Abstract:Objective To summarize the experience during early stage of postoperative monitoring and treatment for the patients follwing lung transplantation in surgical intensive care unit,and to improve the patient's outcome of lung transplantation.Methods A retrospective analysis of 9 cases of orthotopic lung transplantation admitted to surgical intensive care unit (SICU) of Beijing Anzhen Hospital from May 2005 to May 2011 was made.Results There 5 male patients and 4 female patients with the age 21 -67 (46.1 ± 14.1 ) years old in present group,and among them 6 patients survived (survival rate:66.7％ ).The etiological indication of admission were idiopathic pulmonary fibrosis (n =4),pulmonary lymphangioleiomyomatosis ( n =3 ),and bronehiectasis ( n =2).Bilateral sequential lung transplantation ( n =5 ),single left lung transplantation ( n =1 ) and single rigbt lung transplantation ( n =3) were performed.The postoperative duration of mechanical ventilation,stay duration in SICU and in hospital were 2 -32 d,3 -42 d,and 3 -60 d respeetively.Postoperative complication taking place after surgery were acute rejection in 2 cases and pneumonia 4 cases (3 cases were treated with extracorporeal membrane oxygenator).Postoperative in hospital death was 3 cases of 9 resulting from severe intection (n =2) and cireulation failure ( n =1 ).Conclusion Well-planned postoperative management stroategy in SICU plays an important role in outcome control after lung transplantation,including monitor and respiratory and cardiacvascular management,reasonable implementation of vasoactive medications and diuretics,protective mechanical ventilation strategy,reducing the time of ventilation.

Abstract:Lung transplantation (LTx) has become an effective therapy method for carefully selected patients with end-stage lung disease. With the improvement of organ harvesting technology, organ preservation and surgical technology, the success rate of surgery has been significantly improved. Whlie the lung is the only entity transplanted which connects with the outside world, there are virous factors that associated with the post-operative complications. Primary graft dysfunction, infection and acute rejection are major complication in the early stage after surgery. Standardized and precise management after operation, especially in the intensive care unit (ICU) is important to prevent post-operative complications of LTx and improve the outcome. Abstract:Acute-on-chronic liver failure (ACLF) is a syndrome in which acute liver failure with extrahepatic organ failure occurs on chronic liver disease. Recently, liver transplantation is the only effective treatment for ACLF. There is still room for discussion on the optimal surgery timing for ACLF, perioperative infection prevention and control, and maintenance of nutrition and organ function. The Transplantation Immunology Committee of Branch of Organ Transplantation Physician of Chinese Medical Doctor Association and Enhanced Recovery of Liver Transplantation Group of Enhanced Recovery after Surgery Committee of Chinese Research Hospital Association invited relevant experts to discuss the perioperative management of ACLF liver transplantation in areas including surgery timing, organ protection, nutritional support, infection prevention and control, rehabilitation exercises, regulation of the internal environment, etc. An expert consensus was developed as reference for clinicians. Effects of Solid Organ Transplantation on the Risk of Developing Thyroid Cancer: a Systematic Review and Meta-Analysis

Abstract:Background: Solid organ transplantation (SOT), which is the best remedy for end-stage organ failure, is accompanied by the risk of developing a postoperative malignant tumor. To date, assessments of the changes in the increased risk of thyroid cancer (TC) after SOT remain controversial. This study sought to reevaluate the risk of TC after SOT based on the latest literature. Our findings could improve the early diagnosis of tumors and the overall prognosis of patients after SOT. Methods: A computerized search of four major English-language databases (i.e., PubMed, EMBASE, Cochrane Library, and Web of Science) was performed to retrieve cohort studies on the risk of developing TC after SOT. The standardized incidence ratio (SIR) was used as the pooled-effect size and expressed as the 95% confidence interval (CI). Results: In total, 20 cohort studies, comprising 362,079 patients who underwent SOT, were included in the meta-analysis. We found that the patients all had an increased risk of developing TC after the transplantation of different solid organs, including the kidney, heart, lung, and liver (P<0.05), and patients had the highest risk of developing TC after kidney transplantation (SIR =5.28, 95% CI: 4.03-6.92, P<0.01). Conclusions: Patients have an increased risk of developing TC after SOT. Aggressive and regular tumor screenings after SOT for early detection and timely treatment may improve patient prognosis.
[Orthotopic Heart Transplantation: Clinical Experience on 56 Patients].

Abstract:OBJECTIVE:To summarize the clinical experience on 56 patients undergoing orthotopic heart transplantation.METHODS:Between May 2000 and December 2003 56 patients, 47 with cardiomyopathy, 2 with end-stage valvular heart disease, 2 with end-stage ischemic heart disease, 2 with primary malignant cardiac tumor, 1 with complicated congenital heart disease, 1 with muscular dystrophy cardiomyopathy, and 1 with refractory malignant ventricular arrhythmias, underwent orthotopic heart transplantation in the Transplantation Center of Fudan University. The operative procedures included 19 conventional Stanford orthotopic cardiac transplantation in 19 cases and bicaval anastomotic cardiac transplantation in 37 cases. Postoperatively, the patients were prescribed with cyclosporine A + corticosteroids + MMF or FK506 + corticosteroids + MMF as anti-rejection therapy.RESULTS:One patient undergoing his fifth operation died of bleeding 3 days after operation. All survivors were followed-up for 12.4 months on average. Five patients died within 1 year postoperatively with a one-year survival rate of 91%. One patient died of allograft vasculopathy 38 months after operation. Other patients enjoyed heart function recovering to class I-II (NYHA).CONCLUSION:With proper recipient selection, excellent donor heart conservation, bicaval anastomotic technique, and efficient postoperative surveillance and treatment, heart transplantion may produce satisfying mid-term results.
Critical care issues following orthotopic liver transplantation

Abstract:Orthotopic liver transplantation (OLT) remains a formidable undertaking. A multidisciplinary approach to pre-operative optimization and intra- and postoperative care of patients undergoing OLT increases the chance of a successful outcome. Although there have been moves towards avoidance of Intensive Care Unit (ICU) admission for "routine" OLT recipients, critical care practitioners continue to play a key role in liver transplant programs in the MELD era. Use of protocolized care delivery and innovative ICU therapeutic interventions have streamlined the pre-operative optimization and perioperative care of OLT recipients. The postoperative course is significantly influenced by the patient's pre-operative status, the intraoperative course and the function of the liver graft. In addition to discussion of general ICU concepts such as the use of prognostic scoring systems and protocolization of care, this review will use an organ-system based approach to describe the postoperative ICU care of OLT recipients. We discuss hemodynamic management, ventilator weaning, optimization of sedation and analgesia, and the investigation and management of renal and metabolic abnormalities. In addition, we examine postoperative complications including hemorrhage, central nervous system pathology and graft dysfunction. The review concludes with a discussion of the additional challenges practitioners face when dealing with living donor liver transplantation and donation after cardiac death.

Abstract:Familial amyloid polyneuropathy, an autosomal-dominant disease due to mutations in the transthyretin gene, often affects the heart and liver, and is treated best with a combined heart–liver transplantation (CHLT). Although it remains an uncommonly performed procedure, the number of patients undergoing CHLT is increasing. Because of the complexity associated with dual pathophysiology, CHLT poses an extraordinary challenge for anesthesia management. Either both heart and liver transplantation are performed on cardiopulmonary bypass (CPB); or heart transplantation is performed on CPB, followed by liver transplantation with venovenous bypass. Recent reports suggested that liver transplantation can be performed without bypass using the inferior vena cava-sparing technique. However, both bypass and caval sparing technique have their own complications. Here, we present the anesthesia management in a case of sequential heart–liver transplantation using a routine caval cross-clamp technique without venovenous bypass. A 48-year-old man complaining of chest tightness, chest pain, and shortness of breath was diagnosed with amyloid cardiomyopathy. Cardiac ultrasonography revealed thickening of ventricular walls and left ventricular systolic insufficiency (ejection fraction decreased from 46% to ∼20% in 6 months), which was refractory to medical therapy. Symptoms occurred repeatedly. Therefore, CHLT was planned. Heart transplantation was performed smoothly under general anesthesia and standard CPB. His heart functioned well with dobutamine and epinephrine infusion. Subsequently, the patient was weaned from CPB. Liver transplantation was planned using the piggyback procedure with the caval sparing technique. However, upon caval clamping, unexpected blood loss occurred. Clamping of the caval was tested followed by cross-clamping. Norepinephrine, epinephrine, and dobutamine were administered. After the hepatic vein was anastomosed, the clamp was released and nitroglycerin was administered. Hemodynamics was stable, and the patient was discharged after 37 days of hospitalization. The case indicates that CHLT could be performed using caval clamp without venovenous bypass in selected patients.
Nursing care of a child with severe interstitial pneumonia complicated by acute rejection after lung transplantation: a case report

Abstract:Background: Acute rejection after transplantation occurs as a result of complex immune responses following the detection of the major histocompatibility complex of the donors in recipients. As one of the risk factors of chronic rejection, acute rejection can directly lead to death. Therefore, early prevention and monitoring of transplant patients is critical. Pediatric acute rejection after lung transplantation is relatively rare compared to adults, and it remains a considerable challenge since information on rare primary disease complicated by acute rejection after lung transplantation in children is extremely limited, with only a single case series reported in the literature.Case Description: Here, we present a case of a 10-year-old female diagnosed with severe interstitial pneumonia, pulmonary heart disease and severe malnutrition. The patient underwent double-lung transplantation under general anesthesia. Through monitoring and management of immunosuppressants, prevention and control of infection, dynamic body fluid management, personalized nutritional support, psychological care and rehabilitation exercises, the patient achieved recovery and was safely discharged after 21 days.Conclusions: Characteristics of acute rejection after lung transplantation in children include rapid onset and progression of respiratory distress, significant difficulty in nursing and frustration in communication. Anti-infection, anti-rejection, and symptomatic measures in the acute phase are critical in controlling disease progression and improving prognosis.

Abstract:We report a case of severe primary graft dysfunction (PGD) and refractory hypoxemia after bilateral lung transplantation in our center. The effect of conventional therapy of the patient was inadequate, and the patient was unable to be weaned off extracorporeal membrane oxygenation (ECMO). Employing electrical impedance tomography (EIT) monitoring technology, we implemented a series of interventions including prone position, inhaled nitric oxide (iNO) therapy, tracheotomy and other treatment methods. After undergoing a rigorous treatment process, the patient was successfully transitioned out of intensive care unit (ICU) on the 24th day after operation. In conclusion, the utilization of EIT for visual respiratory management, in conjunction with a multifaceted therapeutic approach, substantially contributed to the improved prognosis of lung transplant recipients.

Abstract:BACKGROUND: Prolonged invasive respiratory support and extracorporeal membrane oxygenation (ECMO) in patients requiring urgent lung transplantation (ULTx) present significant challenges to clinical practice due to severe underlying diseases and complex conditions. The aim of the study was to report the clinical outcomes of patients who received ULTx and followed the perioperative rehabilitation protocol implemented in a lung transplant center.METHODS: A retrospective analysis was conducted in ULTx patients who required preoperative invasive mechanical ventilation (IMV) and ECMO between January 2018 and January 2023. Data were retrieved from electronic medical records at our lung transplant center.RESULTS: Fourteen patients (mean age 57.43±10.97 years; 12 males, 2 females) underwent ULTx with bridging ECMO and IMV. The mean body mass index was 23.94±3.33 kg/m², and the mean Acute Physiology and Chronic Health Evaluation (APACHE) II score was 21.50±3.96. The Nutritional Risk Screening 2002 (NRS 2002) scores were ≥3. ULTx was performed after an 8.5-day waiting period (interquartile interval [IQR] 5.0-26.5 d). Following the surgeries, the average lengths of ECMO and IMV were 1.0 (IQR 1.0-2.0) d and 5.0 (IQR 3.0-7.3) d, respectively. The total length of hospital stay was 60.1±30.8 d, with an average intensive care unit stay of 38.3±22.9 d and post-operative hospitalization stay of 45.8±26.1 d. Two patients died within 30 d after ULTx, with a 30-day survival rate of 85.71%.CONCLUSION: Patients receiving ULTx showed an acceptable short-term survival rate, validating the practicality and safety of the treatment protocols implemented in our center.

[A Case Report of Perioperative Management for Liver Transplantation in a Patient with Multiple Old Myocardial Infarction].

Abstract:A 60-years-old (85 kg, 178 cm) coronary arterial disease patient having had acute myocardial infarction for several times received liver transplantation successfully. He had a previous episode of acute myocardial infarction associated with entire obstruction of right coronary 6 years ago, and was inserted a bracket then. One year ago the patient got chest pain again, and was diagnosed as inferior wall myocardial infarction. Then he received expectant treatment in internal medical department for several days. A 774HF75 PAC catheter (Edwards Lifescience, USA) was inserted into an internal jugular vein, and cardiac output, right ventricular end diastolic volume index, right ventricular ejection fraction, stroke volume index, system vascular resistance, pulmonary vascular resistance, left ventricular-stroke work index and right ventricular-stroke work index were calculated. During the operation, cardiovascular medications such as dopamine, norepinephrine, dobutamine were infused and adjusted by steps carefully. Electrolytes and acid-base balance were maintained normal. With these hemodynamic parameters, BGA and systemic management, the anesthesia was managed safely and successfully.
Conservative management of a rare case of post thyroidectomy tracheal injury with coagulation abnormalities

Abstract:Background: Tracheal injury is a rare but potentially serious acute complication of endotracheal intubation. Very few cases of tracheal injury associated with coagulation abnormalities have been reported in the literature. We present a rare case of a patient presenting with tracheal injury in combination with coagulation abnormalities following thyroidectomy. Case presentation: A 58-year-old woman with a history of postoperative chemotherapy for breast cancer, gastric polyps, multiple colonic polyps, esophageal papillary adenomas, and thyroid adenomas presented with dyspnea following 10 ml hemoptysis on the third day after thyroidectomy; she was admitted to the intensive care unit and underwent tracheal intubation for maintaining the airway. Subsequent bronchoscopy revealed a nodular red neoplasm 5-cm from the carina in the trachea obstructing part of the lumen, with a small amount of fresh hemorrhage on the surface. Tracheal injury was considered the preliminary diagnosis. Fiberoptic bronchoscope guided tracheal intubation helped prevent rupture of the tumor, and the cannula was properly inflated to arrest the bleeding while blocking the lower part of the trachea. An emergency surgical evacuation of the cervical hematoma was performed for managing postoperative bleeding. The patient demonstrated persistent pancytopenia despite frequent transfusions. Laboratory examination results revealed abnormal coagulation parameters, anemia, and hepatic dysfunction. Following a multidisciplinary team discussion, pituitrin for hemostasis, tranexamic acid for strengthening hemostasis treatment, and nutritional support and anti-infection treatment were initiated. Endotracheal tube cuff inflation was performed to compress the bleeding site. Complete resolution of the subcutaneous hematoma was observed nine days after the tracheal injury; bronchoscopy revealed residual ecchymosis in the airway hematoma with no evidence of obstruction. Conclusion: Conservative management of tracheal injury limited to the mucosa or submucosa without significant amount of active bleeding using endotracheal intubation is considered a practical and effective approach. Successful management was ensured by appropriate clinical suspicion, early multidisciplinary team discussion, and prompt diagnosis and interventions.
Successful treatment of gastric cancer 10 years after heart transplantation: A case report

Abstract:Background: While survival rates among cardiac allograft recipients have improved, there has been a rise in post-transplant malignancies, with gastric cancer being less commonly reported. This study presented a successful treatment of gastric cancer in an individual 10 years after undergoing a heart transplant. Case presentation: A 66-year-old Chinese man presented to the gastrointestinal clinic with a complaint of diagnosis of gastric cancer for 4 months and treated with neoadjuvant therapy for 1 month. He has undergone orthotopic heart transplantation 10 years earlier due to a myocardial infarction. Physical examination and laboratory tests did not reveal any significant abnormalities. Abdominal contrast-enhanced computed tomography (CT) imaging indicated a gastric mass near the greater curvature, with gastroscopy suggesting a carcinoma at the esophagogastric junction, Siewert III. An echocardiogram indicated left atrial enlargement with mild mitral and tricuspid regurgitation. The diagnosis suggested that his gastric cancer at the esophagogastric junction was a consequence of long-term immunosuppressive therapy. A multidisciplinary team (MDT) consultation recommended a proximal radical gastrectomy. Postoperatively, the patient received 4 cycles of adjuvant chemotherapy with XELOX combined with Herceptin, initiated a month after surgery. During the 1-year follow-up, the patient showed commendable recovery, with no signs of tumor recurrence or metastasis. Conclusion: This case underscores the potential risk of malignancy from immunosuppressive agents in transplant recipients. The successful management of this complex scenario underscores the indispensable role of an MDT approach in treating such unique and challenging cases.
