Abstract:Rectal leiomyosarcoma is a rare neoplasm, and no standard therapeutic strategy has been established, other than surgical resection. The prognosis of advanced leiomyosarcoma in the rectum is poor. We describe a case of a 47-year-old man with a locally advanced rectal leiomyosarcoma who presented with anal bleeding and lower abdominal pain. After discussions by a multidisciplinary team (MDT), the patient received preoperative pelvic short-course radiotherapy followed by laparoscopic-assisted abdominoperineal resection. Adjuvant chemotherapy with doxorubicin and ifosfamide was administered postoperatively. Local recurrence was detected 13 months after the resection. The patient received chemotherapy with gemcitabine and docetaxel followed by radioactive particle implantation. Thereafter, apatinib was administered to gain systemic control. The patient died 24 months after the diagnosis. This case report draws attention to treatment with multidisciplinary therapy, as discussed by MDT, for rare locally advanced rectal leiomyosarcoma.

SRUS uncommonly accompanied by serrated lesions should at least be considered by pathologists and clinicians.

Management of Primary Female Urethral Adenocarcinoma: Two Rare Case Reports and Literature Review

Abstract:Primary urethral adenocarcinoma in females is an extremely rare malignancy with unclear origin and only a few retrospective cases have been reported. The controversy continues to exist over the origin of primary urethral adenocarcinoma from periurethral glands (which include the Skene’s glands), urethritis glandularis or intestinal metaplasia. Herein, we report one case of a 49-year-old female with distal urethral adenocarcinoma who presented with obstructive voiding. Abdominal and pelvic CT scans and chest radiology were unremarkable. Biopsy of the mass confirmed urethral adenocarcinoma. The patient underwent partial ureterectomy and was disease-free at the 2-years follow-up period. We also present another extremely rare case of primary urethral adenocarcinoma with mucinous features in a 58-year-old female who initially complained of external urethral orifice itching with painless urethral bleeding and was treated with local excision. The patient has not received any neoadjuvant or adjuvant therapy, and experienced tumor recurrence, inguinal lymph nodes metastasis, and even local iliopsoas metastasis during over 10-years follow-up. In conclusion, our current study emphasizes the importance of imaging studies and biopsy in making an accurate preoperative diagnosis of this rare disease, and further highlights the role of multimodal therapy. A combination of radiotherapy, chemotherapy and surgery is recommended for the optimal local and distant disease control. Moreover, better medical compliance and regular follow-up are required in these patients.
Mucinous Prostate Cancer Shows Similar Prognosis To Typical Prostate Acinar Carcinoma: A Large Population-Based And Propensity Score-Matched Study

Abstract:Background: Mucinous prostate cancer (PCa) is an extremely rare form of prostate malignancy. To date, the limited knowledge of its biology and outcomes stems from mostly small, single institution experiences. We analyzed the Surveillance, Epidemiology, and End Results (SEER) database to explore the incidence and treatment of mucinous PCa together with its prognostic factors to gain relatively large and consolidated insights. Methods: Age-adjusted incidence (AAI) rates were evaluated over time. Propensity score matching (PSM) and Kaplan-Meier analyses were used to compare the prognosis between mucinous PCa and typical prostate acinar adenocarcinoma. We assessed cancer-specific survival (CSS) and overall survival (OS) after patient stratification according to summary stage and treatment choice. Cox hazards regression analysis was performed to determine independent predictors of CSS and OS. Results: The AAI in 2016 was 0.24 per million. Patients with mucinous PCa had similar CSS and OS to matched individuals with typical prostate acinar adenocarcinoma. In terms of treatment, 65.3% of mucinous PCa patients underwent surgery, and 23.9% received radiation therapy. Patients who underwent surgery had longer survival (CSS, p = 0.012; OS, p < 0.001), and patients who received radiation therapy had similar survival to those who did not receive radiation therapy (CSS, p = 0.794; OS, p = 0.097). A multivariate Cox analysis for CSS and OS showed that older age (CSS: HR: 4.982, p = 0.001; OS: HR: 4.258, p < 0.001) and distant stage (CSS: HR: 40.224, p < 0.001; OS: HR: 9.866, p < 0.001) were independent prognostic factors for mucinous PCa patients. Conclusions: Mucinous PCa has an extremely low AAI. Analysis of its outcomes indicates that it is not a more malignant tumor as previously suspected. Mucinous PCa shows a similar prognosis to typical prostate acinar carcinoma. Surgery was associated with prolonged survival. An older age at diagnosis and distant stage was associated with poor survival.

Abstract:Background Solitary rectal ulcer syndrome (SRUS) is a rare disorder often misdiagnosed as a malignant ulcer. Histopathological features of SRUS are characteristic and pathognomonic; nevertheless, the endoscopic and clinical presentations may be confusing. The aim of the present study was to assess the clinical findings, surgical treatment, and outcomes in patients who suffer from SRUS.Materials and methods A retrospective chart review was undertaken, from January 1989 to May 2005 for all patients who were diagnosed with SRUS. Data recorded included: patient's age, gender, clinical presentation, past surgical history, diagnostic and preoperative workup, operative procedure, complications, and outcomes.Results During the study period, 23 patients were diagnosed with SRUS. Seven patients received only medical treatment, and in three patients, the ulcer healed after medical treatment. Sixteen patients underwent surgical treatment. In four patients, the symptoms persisted after surgery. Two patients presented with postoperative rectal bleeding requiring surgical intervention. Three patients developed late postoperative sexual dysfunction. One patient continued suffering from rectal pain after a colostomy was constructed. Median follow-up was 14 (range 2-84) months.Conclusion The results of this study show clearly that every patient with SRUS must be assessed individually. Initial treatment should include conservative measures. In patients with refractory symptoms, surgical treatment should be considered. Results of anterior resection and protocolectomy are satisfactory for solitary rectal ulcer.
Retroperitoneal Primary Mucinous Adenocarcinoma: A Case Report

Abstract:Retroperitoneal primary mucinous adenocarcinoma (RPMA) is extremely rare and the histogenesis of this tumor remains unclear. There is no consensus on the appropriate treatment for RPMA. Surgical resection is standard for the treatment of RPMA, whereas the benefits of chemotherapy with regard to this tumor remain to be established. This case report concerns a 21-year-old female individual with RPMA. The patient initially presented with chronic lower back pain and weight loss. Additionally, carcinoembryonic antigen (CEA) levels were found to be elevated. A computed tomography scan revealed a mass in the abdominal cavity. Consequently, laparotomy was performed, which revealed a well-defined tumor in the right retroperitoneum. Chemotherapy as a monotherapy was not considered as a viable treatment option. Therefore, the patient was initially administered a combined treatement of oxaliplatin and 5-fluorouracil. This treatment was then changed to paclitaxel and 5-fluorouracil. Findings showed a decrease in the CEA serum levels, indicating that this combination treatment may be efficacious in the treatment of RPMA since local recurrence following surgical resection was well controlled with chemotherapy.

Abstract:Solitary rectal ulcer syndrome (SRUS) is a chronic, benign rectal ulcer disease. Because of its various clinical manifestations, it is easily misdiagnosed as rectal cancer or inflammatory bowel disease. A case of solitary rectal ulcer syndrome in Peking University First Hospital was reproted. The diagnosis and treatment of this patient is helpful to improve the understanding of this disease and to emphasize the significance of long-term follow-up and pathological biopsy.
Case Report: Colon malignant tumor caused by retroperitoneal small round cell undifferentiated sarcoma

Abstract:Small round cell undifferentiated sarcoma is a rare and highly invasive group of malignant bone and soft tissue tumors, often associated with a high misdiagnosis rate. The patient in this case was a 34-year-old male who presented with a two-month history of abdominal pain that worsened over the past two weeks. Elevated levels of tumor markers CA19-9 and CA72-4 were observed. Imaging revealed a substantial, well-vascularized mass in the lower left abdomen, located in the posterior abdominal cavity, invading the descending colon and the root of the small mesentery, and infiltrating the serous layer. The lesion was extensively resected without any postoperative complications. Microscopic examination indicated a combination of mucinous adenocarcinoma (approximately 30%) and small round cell undifferentiated sarcoma (approximately 70%). The patient was followed up for six months, and one month after surgery, a recurrence of the tumor was observed in the left paracolonic sulcus area, with metastases to the abdominal wall, peritoneum, and medial iliac muscles. Chemotherapy and targeted therapy were administered, and the patient currently survives with the presence of tumors. Small round cell undifferentiated sarcoma is an uncommon and highly invasive tumor, and clinical surgeons need to raise their awareness and realize to the maximum extent possible that this disease can be described through a multi-modal combination of immunohistochemistry and genetic test to improve diagnostic accuracy and reduce missed diagnoses. Further research in the field of biology is necessary to explore targeted drugs specifically suitable for this disease.
Primary adenosquamous carcinoma of the duodenum: A case report with literature review

Abstract:Adenosquamous carcinoma (ASC) of the duodenum is a malignant tumor that combines adenocarcinoma and squamous cell carcinoma components. Case reports of ASC are rare, and our understanding of the complex clinicopathological features and treatment strategies for the disease remains incomplete. The current study presents a case of duodenal ASC and discusses the available literature on this rare condition. A 55‐year‐old male patient presented with a history of jaundice and fever, with pruritus and clay‐colored stools. Preoperative investigations suggested an increase in transaminase, bilirubin and tumor markers. Enhanced magnetic resonance imaging of the abdomen revealed a mass in the descending duodenum and another in the pancreatic region, without distant metastases. An endoscopy of the upper gastrointestinal tract revealed an infiltrative growth mass invading the duodenal papilla in the descending part of the duodenum. The patient underwent a Whipple procedure. Histopathological and immunohistochemical examinations confirmed duodenal ASC. Although an R0 resection was performed, the patient developed hepatic metastases 2 months after surgery and succumbed 6 months post‐surgery. This case underscores the aggressive nature of duodenal ASC and the need for further studies to develop effective treatment interventions.

Abstract:Colorectal cancer (CRC) is the third most common cancer in the world. Other than adenocarcinomas, exceptional tumors of the colon and rectum represent a neglected clinical issue due to their rarity. Signet ring cell carcinoma (SRCC) is a rare subtype of CRC and has an extremely poor prognosis due to its advanced stage at diagnosis. Here we report a rare case of colorectal SRCC manifested as recurrent intestinal obstruction with a negative colonoscopy. Finally, he was diagnosed with signet ring cell carcinoma of the colon by postoperative pathology. It emphasized the special feature of intramural tumor growth without penetrating the mucosa in SRCC, which requires timely surgical intervention to avoid delay in diagnosis and treatment.

Abstract:Background Breast and vulvar metastases from rectal signet ring cell carcinoma (SRCC) represent a rare and obscure clinical entity associated with poor survival. Managing patients with metastatic rectal SRCC is extremely challenging due to the absence of high-quality evidence. Case presentation A 26-year-old woman presented with progressively worsening anal pain, constipation, and hematochezia for approximately two years. Following the diagnosis of locally advanced rectal cancer ( c T 3 N 0-1 M 0 ), she received neoadjuvant chemotherapy with modified FOLFOX6 regimen and underwent laparoscopic abdominoperineal resection. Metastases to the breast and vulva developed during postoperative chemotherapy. Genetic testing revealed RAS/BRAF wild-type and microsatellite instability (MSI)-low status. Though sequential administration of irinotecan plus tegafur and tegafur plus raltitrexed-based chemotherapy in combination with bevacizumab, the disease progressed rapidly. Sadly, the patient passed away 15 months after initial diagnosis due to rapidly progressive disease. Conclusion Rectal SRCC is associated with younger on-set, aggressive behaviors, and worse survival outcomes. Due to poor cohesiveness, SRCC tends to develop metastases. A patient’s medical history and immunohistochemical staining (such as CK20, CK7, and CDX-2) can aid in identifying the tumor origin of breast and vulvar metastases. Mutations and signaling pathways predominant in the tumorigenesis of SRCC remains unveiled. There is poor effect of conventional chemotherapies, targeted and immunotherapies for colorectal adenocarcinoma on SRCC, so novel therapies are needed to treat this patient population.

Abstract:Primary mucinous lesions of the urinary system are extremely rare. We describe two cases of primary mucinous lesions of the urothelial tract. One case is of mucinous metaplasia in the bladder of a 40-year-old man presenting with frequent urination, urgency, and gross hematuria. The other case is of mucinous adenocarcinoma in the pelvis of an otherwise healthy 67-year-old man with left nephrolithiasis. The histological images of the two cases demonstrate a spectrum from benign mucinous metaplasia to malignant mucinous adenocarcinoma, and suggest that mucinous metaplasia in urothelial tract may be the precancerous lesion of mucinous adenocarcinoma.

Abstract:We study the conductivity sigma(T) of interacting electrons in a low-dimensional disordered system at low temperature T. For weak interactions, the weak-localization regime crosses over with lowering T into a dephasing-induced "power-law hopping." As T is further decreased, the Anderson localization in Fock space crucially affects sigma(T), inducing a transition at T = T(c), so that sigma(T < T(c)) = 0. The critical behavior of sigma(T) above T(c) is ln sigma(T) proportional to -(T - T(c))(-1/2). The mechanism of transport in the critical regime is many-particle transitions between distant states in Fock space.

Abstract:Background: Squamous cell carcinoma (SCC) of the sigmoid colon is an exceedingly rare subtype of colorectal cancer (CRC) associated with chronic inflammatory conditions. Due to its variable clinical presentation ranging from subclinical to fully symptomatic and limited available information, it poses a diagnostic challenge. We aim to provide a review of the current literature and raise awareness about the importance of a thorough clinical analysis for an early diagnosis. Case presentation: We describe the case of a 59-year-old Peruvian woman with a medical history of diverticular disease and irritable bowel syndrome. The patient presented with nonspecific symptoms such as abdominal discomfort, constipation, and bloating. Diagnostic tests and biopsy revealed a rare case of squamous cell carcinoma of the sigmoid colon. The patient underwent surgical resection and adjuvant chemotherapy. Conclusion: Despite the rarity of this type of cancer in the colon, the patient's clinical course highlights the importance of considering it as a potential diagnosis in patients with nonspecific symptoms and a history of gastrointestinal disorders. Surgical treatment followed by radiotherapy is the preferred management. Factors such as lack of postoperative complications and the stage of the neoplasia can augur a positive. Prognosis: A prompt diagnosis is crucial, as detecting a neoplasia in its early stages can make surgery more effective.
Multiple sarcomatoid carcinomas in the small intestine with perforation: A case report and literature review

Abstract:Rationale: Sarcomatoid carcinoma of the small intestine is an exceedingly rare and aggressive malignancy, often diagnosed at advanced stages with a poor prognosis. This study documents a detailed case of sarcomatoid carcinoma of the small intestine, highlighting the diagnostic challenges and treatment approaches, underscored by a comprehensive review of related literature. Given the rarity of this condition, our report aims to enrich the existing diagnostic and treatment frameworks for this malignancy, emphasizing the necessity for early detection and intervention strategies. By presenting this case in conjunction with a literature review, we seek to shed light on the elusive nature of sarcomatoid carcinoma in the small intestine and propose avenues for improving patient outcomes. Patient concerns: Case presentation A 61-year-old male patient initially presented with recurrent abdominal pain and gastrointestinal symptoms. Initial abdominal computed tomography (CT) scans and gastrointestinal endoscopy revealed only inflammatory and hyperplastic changes in the duodenum and jejunum, with a diagnosis of intestinal obstruction. Two years later, due to gastrointestinal perforation, the patient was hospitalized again. Diagnoses: CT scans and other examinations revealed small intestinal lesions. Four small intestinal lesions were surgically removed, and pathology and immunohistochemistry confirmed sarcomatoid carcinoma of the small intestine. A short time later, enhanced CT scans revealed metastatic lesions in the hepatic portal and adrenal glands. Interventions: After surgery, the gastrointestinal function gradually recovered, and the patient was discharged from the hospital on a semiliquid diet. No further treatment such as radiotherapy or chemotherapy was administered postoperatively. Outcomes: Five months after the surgery, the patient died due to brain metastasis. Lessons: The study outcomes reveal the aggressive nature of sarcomatoid carcinoma of the small intestine, characterized by rapid progression and poor prognosis despite surgical interventions. The patient condition rapidly deteriorated, leading to metastasis and death within 5 months postsurgery. These findings underscore the critical need for early detection and possibly innovative treatment approaches to improve survival rates. This case also highlights the potential for gastrointestinal sarcomatoid carcinoma to metastasize to distant organs, including the brain, suggesting a propensity for hematogenous spread.

A rare multiple primary sarcomatoid carcinoma (SCA) of small intestine harboring driver gene mutations: a case report and a literature review

Abstract:Primary sarcomatoid carcinoma (SCA) is a type of rare tumor consisting of both malignant epithelial and mesenchymal components. Only 32 cases of SCA of the small bowel have been reported in the literature to date. Due to its rarity and complexity, this cancer has not been genetically studied and its diagnosis and treatment remain difficult. Here we report a 54-year-old male underwent emergency surgical resection in the small intestine due to severe obstruction and was diagnosed with multiple SCA based on postoperative pathological examination. Over 100 polypoid tumors scattered along his whole jejunum and proximal ileum. Chemotherapy (IFO+Epirubicin) was performed after surgery while the patient died two months after the surgery due to severe malnutrition. Whole-exome sequencing was performed for the tumor tissue with normal tissue as the control. Important cancer-related gene mutations, including KRAS (c.37G>T, p.G13C), TP53 (c.871A>T, p.K291*), EGFR (c.1351C>T, p.R451C), and CDKN2A (c.104_138del, p.G35fs), were found among 286 nonsynonymous somatic mutations (SNV and Indel). Copy-number amplified genes mainly gathered in chromosome 6, 7, 16 and 20. Mutation clustering analysis showed that main genetic abnormalities included DNA methylation, DNA alkylation, cellular homeostasis, and shared similarities with melanoma, glioma, prostate cancer, bladder cancer, non-small cell lung cancer, and pancreatic cancer. In summary, the genomic features of the small intestine SCA were explored at whole-exome level for the first time, and over 200 somatic mutations were identified in the tumor tissue. Key tumor driver gene mutations were revealed, as well as several aberrant functional pathways. These results contribute to further understanding of the pathogenesis and molecular mechanism of this rare tumor.

We present a patient who came to the hospital with worsening constipation then subsequently found to have invasive SCC of the rectum.

Abstract:A 53-years-old man has a dysentery since two weeks. The blood test shows Coombs-positive hemolytic anemia and inflammation. Autoimmune hemolytic anemia (AIHA) is treated with corticosteroid. A colonoscopy reveals an ulcerative colitis. The evolution of the patient is complicated by a spontaneous digestive perforation treated by total proctocolectomy. After this intervention, there is a resolution of the AIHA and the patient is gradually weaned from corticosteroids. AIHA is a rare extra-intestinal manifestation of inflammatory bowel disease essentially ulcerative colitis. Identification of this cause of secondary AIHA is important for the therapeutic strategy. However treatment is nonspecific and based on low levels of evidence.
