Abstract:Acromioclavicular joint (AC) injuries are common in the young athletic population. There is general agreement that grade I and grade II separations are best managed nonoperatively, whereas grades IV to VI warrant surgical stabilization. In contrast, considerable controversy exists surrounding the best method of treatment for grade III AC separations. The trend in recent literature is toward the initial nonoperative management of these injuries. However, consideration of other factors such as type of sport, timing of injury relative to athletic season, or the throwing demands in an injured dominant arm may play a role in the decision to treat grade III injuries operatively or nonoperatively. We offer a protocol algorithm for treating grade III AC separations for managing this controversial injury.
Efficacy and Safety of Benralizumab for Eosinophilic Asthma: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Abstract:Context: Benralizumab is a humanized monoclonal antibody that targets the alpha chain of the IL-5 receptor (IL-5R alpha) and is currently being assessed in clinical trials for asthma control. Objective: Our systematic review and meta-analysis intends to evaluate the therapeutic efficacy and safety of benralizumab in patients with eosinophilic asthma. Data sources and extraction: Literature searches of PubMed, Embase, and the Cochrane Library were performed to identify randomized controlled trials of benralizumab and clinic outcomes in asthmatics. Results: In total, 7 articles with 2,321 subjects met our inclusion criteria. From this pooled analysis, we found that benralizumab significantly reduces exacerbations (RR: 0.63, 95% CI: 0.52-0.76, p < 0.00001; l(2) = 52%, p = 0.06) compared to placebo in eosinophilic asthma. There was no statistical trend for improvement in forced expiratory volume in 1 second or asthma control indices such as Quality of Life Assessment (AQLQ) and Asthma Control Questionnaire score in benralizumab-treated patients. In addition, safety data indicated that benralizumab administration resulted no increasing incidence of adverse events and was well tolerated (RR: 1.00, 95% CI: 0.95-1.05, p = 0.96; l(2) = 40%, p = 0.13). Conclusion: These results demonstrate the efficacy and safety of benralizumab for asthma patients with severe or uncontrolled symptoms and elevated eosinophils and provide support for benralizumab as an ideal option to treat asthma in this patient population.

The Role of Benralizumab in Eosinophilic Immune Dysfunctions: A Case Report-Based Literature Review

Abstract:In the past years, the knowledge of eosinophils playing a primary pathophysiologic role in several associated conditions has led to the development of biologics targeting therapies aiming at normalizing the immune response, reducing chronic inflammation, and preventing tissue damage. To better illustrate the potential relationship between different eosinophilic immune dysfunctions and the effects of biological therapies in this scenario, here, we present a case of a 63-year-old male first referred to our department in 2018 with a diagnosis of asthma, polyposis, and rhinosinusitis and presenting a suspicion of nonsteroidal anti-inflammatory drugs’ allergy. He also had a past medical history of eosinophilic gastroenteritis/duodenitis (eosinophilia counts >50 cells/high-power field HPF). The use of multiple courses of corticosteroid therapy failed to completely control these conditions. In October 2019, after starting benralizumab (an antibody directed against the alpha chain of the IL-5 cytokine receptor) as add-on treatment for severe eosinophilic asthma, important clinical improvements were reported both on the respiratory (no asthma exacerbations) and gastrointestinal systems (eosinophilia count 0 cells/HPF). Patients’ quality of life also increased. Since June 2020, systemic corticosteroid therapy was reduced without worsening of gastrointestinal symptoms or eosinophilic inflammation. This case warns of the importance of early recognition and appropriate individualized treatment of eosinophilic immune dysfunctions and suggests the conduction of further larger studies on the use of benralizumab in gastrointestinal syndromes aiming at better understanding its relying mechanisms of action in the intestinal mucosa.

Abstract:BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a multisystemic disease characterized by eosinophilic tissue inflammation. Benralizumab, an anti-IL-5 receptor (anti-IL-5R) monoclonal antibody, induces rapid depletion of eosinophils; its longer-term effect in EGPA is unknown.OBJECTIVE: To assess the real-world effectiveness and clinical remission rates of anti-IL-5R therapy in EGPA.METHODS: We performed a retrospective cohort analysis of patients with EGPA, who commenced treatment with benralizumab. Clinical remission, assessed at 1 year and 2 years after the initiation of benralizumab, was defined as an absence of active vasculitis (Birmingham Vasculitis Activity Score of 0) and an oral corticosteroid (OCS) dose of ≤4 mg/d of prednisolone. "Super-responders" were defined as patients in remission and free of any significant relapses (asthma or extrapulmonary) over the preceding 12 months. The corticosteroid-sparing capacity of benralizumab, patient-reported outcome measures, and characteristics associated with clinical remission and super-responder status were also analyzed.RESULTS: A total of 70 patients completed at least 1 year of treatment with benralizumab, of whom 53 completed 2 years. Of 70 patients, 47 (67.1%) met the definition for clinical remission at 1 year, with a similar proportion in remission at 2 years. Excluding asthma-related relapses, 61 of 70 (87.1%) patients were relapse free at 1 year, and of the 53 who completed 2 years, 45 (84.9%) were relapse free. A total of 67.9% of patients no longer needed any OCS for disease control. No significant difference was seen between antineutrophilic cytoplasmic antibody (ANCA)-positive and ANCA-negative subgroups.CONCLUSIONS: In this real-world setting of patients with EGPA, treatment with benralizumab was well tolerated and resulted in corticosteroid-free clinical remission for the majority of patients.

Appendiceal mucocele due to mucinous adenoma diagnosed by computed tomography colonography

Abstract:An asymptomatic 50-year-old man underwent a low-dose computed tomography colonography (CTC) for screening purpose. The CTC examination was negative for colonic mass or polyps, but axial and coronal two-dimensional CTC images (Fig. 1) showed a large elongated low-attenuation mass in the expected region of the appendix, in line with an appendiceal mucocele. Three-dimensional CTC endoluminal images (Fig. 2) showed the mass bulging into the cecal lumen. The patient underwent appendectomy with partial right hemicolectomy and is in good health. Pathology confirmed an appendiceal mucocele from mucinous adenoma, and in particular the histological examination revealed a low-grade mucinous appendiceal neoplasm with negative regional lymph nodes and with no signs of mucinous peritoneal carcinomatosis. This case shows the crucial role of CTC in the diagnosis of an unknown submucosal disease [1], potentially malignant if not early diagnosed [2,3]; that diagnosis would not have been made if the patient had had an optical colonoscopy or a double contrast barium enema instead of CTC, since both tests fail to evaluate extraluminal areas.

Abstract:Background: Chronic obstructive pulmonary disease (COPD) predominantly represents a neutrophilic inflammatory phenotype, however 1/3 of the cases are eosinophilic. Benralizumab is an afucosylated humanised monoclonal antibody that targets the IL-5 receptor α subunit, leading to rapid and near-complete eosinophil depletion via antibody-dependent cellular cytotoxicity. We investigated the relationship of COPD airway microbiome in response to benralizumab targeted immune modulation. Methods: Sputum samples from 94 COPD patients enrolled to the GALATHEA trial (NCT02138916) and randomized to receive placebo (33), benralizumab at 100mg (29) or 30mg (32) over 52 weeks, were analysed at baseline, week 24 and at end-of-treatment (week 56). Sputum microbiota taxonomic profiles and diversity indices, generated from paired-end Illumina sequencing targeting the 16sRNA gene, were used to perform the airway microbiome comparisons. Linear mixed model analysis was applied to assess the eosinophil-related immune marker changes. Results: Participants treated with 100 and 30 mg benralizumab respectively, showed a significant reduction from baseline in blood and sputum eosinophil counts (Blood p = 1.2E-10 & p = 8.8E-10; Sputum p=0.03 & p = 0.004) and eosinophil-derived mediators (ECP p < 3E-09 & p < 2E-08; EDN p < 8E- 12 & p < 2E-09). However, no significant changes in the composition or diversity of the sputum microbiome was observed. Conclusions: In this study, the airway microbiome was unaffected by targeted reduction of eosinophilic inflammation with benralizumab treatment.

Abstract:Background: Benralizumab is effective in the treatment of eosinophilic asthma and is being investigated for the treatment of other eosinophil-associated diseases. Reports on the use of benralizumab for the treatment of eosinophilic granulomatosis with polyangiitis (EGPA) are limited to case reports and small case series. Methods: We conducted a multicentre, retrospective study including EGPA patients treated with off-label benralizumab. The primary endpoint was the rate of complete response defined as no disease activity (Birmingham Vasculitis Activity Score=0) and a prednisone dose ≤4 mg/day. Partial response was defined as no disease activity and a prednisone dose ≥4 mg/day. Results: Sixty-eight patients were included, including 31 (46%) who had previously received mepolizumab. The use of benralizumab was warranted by uncontrolled asthma in 54 (81%), persistent ear, nose and throat (ENT) manifestations in 27 (40%) and persistent glucocorticoids (GCs) use in 48 (74%) patients. Median (IQR) follow-up after starting benralizumab was 23 (9-34) months. Thirty-three patients (49%) achieved a complete response, 24 (36%) achieved a partial response and 10 (15%) did not respond. Among the 57 patients who initially responded, 10 (18%) eventually required further line treatments. GCs were discontinued in 23 patients (38%). Prior mepolizumab use was associated with a higher rate of primary failure (26.7% vs 5.4%, p=0.034) and less frequent GCs discontinuation (14.8% vs 55.9%, p=0.001). Vasculitis flares occurred in 7 patients (11%) and were associated with histological evidence of vasculitis and/or antineutrophil cytoplasmic antibodies positivity at benralizumab initiation (p=0.004). Conclusions: Benralizumab appears to be an effective treatment for refractory asthma or ENT manifestations in EGPA and allows GC-sparing. However, its efficacy was lower after prior failure of mepolizumab.

Abstract:BACKGROUND: Eosinophilic gastritis and duodenitis are characterized by gastrointestinal mucosal eosinophilia, chronic symptoms, impaired quality of life, and a lack of adequate treatments. Mast-cell activity may contribute to the pathogenesis of the conditions. AK002 (lirentelimab) is an anti-Siglec-8 antibody that depletes eosinophils and inhibits mast cells and that has shown potential in animal models as a treatment for eosinophilic gastritis and duodenitis.METHODS: In this phase 2 trial, we randomly assigned adults who had symptomatic eosinophilic gastritis, eosinophilic duodenitis, or both conditions in a 1:1:1 ratio to receive four monthly infusions of low-dose AK002, high-dose AK002, or placebo. The primary end point was the change in gastrointestinal eosinophil count from baseline to 2 weeks after the final dose; to maximize statistical power, we evaluated this end point in the placebo group as compared with the combined AK002 group. Secondary end points were treatment response (>30% reduction in total symptom score and >75% reduction in gastrointestinal eosinophil count) and the change in total symptom score.RESULTS: Of the 65 patients who underwent randomization, 43 were assigned to receive AK002 and 22 were assigned to receive placebo. The mean percentage change in gastrointestinal eosinophil count was -86% in the combined AK002 group, as compared with 9% in the placebo group (least-squares mean difference, -98 percentage points; 95% confidence interval [CI], -121 to -76; P<0.001). Treatment response occurred in 63% of the patients who received AK002 and in 5% of the patients who received placebo (difference, 58 percentage points; 95% CI, 36 to 74; P<0.001). The mean change in total symptom score was -48% with AK002 and -22% with placebo (least-squares mean difference, -26 percentage points; 95% CI, -44 to -9; P = 0.004). Adverse events associated with AK002 were similar to those with placebo, with the exception of higher percentages of patients having mild-to-moderate infusion-related reactions with AK002 (60% in the combined AK002 group and 23% in the placebo group).CONCLUSIONS: In patients with eosinophilic gastritis or duodenitis, AK002 reduced gastrointestinal eosinophils and symptoms. Abstract:Introduction Asthma associated with eosinophilic granulomatosis with polyangiitis (EGPA) is often severe and corticosteroid-dependent, leading to significant morbidity. Mepolizumab and benralizumab are humanized monoclonal antibodies targeting interleukin 5 (IL-5) and its receptor, respectively. They have been shown to be effective in steroid-sparing in patients with severe eosinophilic asthma. Objective Our aim was to evaluate the efficacy and safety of mepolizumab and benralizumab prescribed for severe asthma in patients with EGPA under “real-world” conditions. Methods This was a retrospective analysis of patients with EGPA and persistent asthma who received either mepolizumab 100 or 300 mg administered every 4 weeks, or benralizumab 30 mg administered every 4 weeks for the initial 3 injections and followed by an injection every 8 weeks thereafter, whilst combined with oral glucocorticoids. The follow-up every 6 ± 3 months included an assessment of clinical manifestations, pulmonary function tests and eosinophil cell count. The primary outcome was the proportion of patients at 12 months receiving a daily oral dose of prednisone or equivalent of 4 mg or less with a BVAS of 0. Results Twenty-six patients were included. After 12 months of treatment with mepolizumab or benralizumab, 32% of patients met the primary outcome and were receiving less than 4 mg of prednisone per day with a BVAS of 0. The median dose of prednisone was 10 mg per day at baseline, 9 mg at 6 months, and 5 mg at 12 months ( p ≤ 0.01). At 12 months, 23% of patients were weaned off corticosteroids, while an increase or no change in dose was observed in 27% of patients. The median eosinophil count was significantly reduced from 365 cells/mm 3 to 55 cells/mm 3 at 6 months and 70 cells/mm 3 at 12 months, respectively. No significant change was observed in FEV1. After 12 months of treatment, 14% of patients had had an average of 1 exacerbation of asthma, compared with 52% of patients before baseline. The tolerability profile was favorable. Conclusion In this real-world study in patients with severe asthma and a history of EGPA asthma, mepolizumab and benralizumab had a significant steroid-sparing effect and reduced asthma exacerbation, but no significant effect on lung function.

Abstract:BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a vasculitis characterized by eosinophilic inflammation. Benralizumab, a monoclonal antibody against the interleukin-5α receptor expressed on eosinophils, may be an option for treating EGPA.METHODS: We conducted a multicenter, double-blind, phase 3, randomized, active-controlled noninferiority trial to evaluate the efficacy and safety of benralizumab as compared with mepolizumab. Adults with relapsing or refractory EGPA who were receiving standard care were randomly assigned in a 1:1 ratio to receive benralizumab (30 mg) or mepolizumab (300 mg) subcutaneously every 4 weeks for 52 weeks. The primary end point was remission at weeks 36 and 48 (prespecified noninferiority margin, -25 percentage points). Secondary end points included the accrued duration of remission, time to first relapse, oral glucocorticoid use, eosinophil count, and safety.RESULTS: A total of 140 patients underwent randomization (70 assigned to each group). The adjusted percentage of patients with remission at weeks 36 and 48 was 59% in the benralizumab group and 56% in the mepolizumab group (difference, 3 percentage points; 95% confidence interval [CI], -13 to 18; P = 0.73 for superiority), showing noninferiority but not superiority of benralizumab to mepolizumab. The accrued duration of remission and the time to first relapse were similar in the two groups. Complete withdrawal of oral glucocorticoids during weeks 48 through 52 was achieved in 41% of the patients who received benralizumab and 26% of those who received mepolizumab. The mean (±SD) blood eosinophil count at baseline was 306.0±225.0 per microliter in the benralizumab group and 384.9±563.6 per microliter in the mepolizumab group, decreasing to 32.4±40.8 and 71.8±54.4 per microliter, respectively, at week 52. Adverse events were reported in 90% of the patients in the benralizumab group and 96% of those in the mepolizumab group; serious adverse events were reported in 6% and 13%, respectively.CONCLUSIONS: Benralizumab was noninferior to mepolizumab for the induction of remission in patients with relapsing or refractory EGPA. Abstract:Introduction: EGPA is an inflammatory disorder featuring asthma, eosinophilia, and vasculitis. This exploratory post-hoc analysis assessed whether patients with EGPA from the MANDARA study can achieve asthma remission. Methods: MANDARA was a Phase 3, randomised, 52-week double-blind study (NCT04157348) of benralizumab 1x30 mg (n=70) vs mepolizumab 3x100 mg (n=70) SC Q4W in patients with EGPA. All patients had asthma, eosinophilia and oral glucocorticoid (OGC) dose ≥7.5 mg/day ± immunosuppressants. Asthma remission (3-component) was defined as no asthma relapse during the 52-week period, OGC=0 mg/day and an Asthma Control Questionnaire (ACQ-6) score of –10%). Results: Patients had a mean (SD) age of 52.3 (14.1) years and 60.0% were women. The adjusted 3-component asthma remission rate was 33.6% and 22.3% for the benralizumab and mepolizumab groups respectively (difference: 11.25%; 95% CI: –3.19, 25.70; p=0.127; Figure). More benralizumab- than mepolizumab-treated patients achieved the 4-component remission (28.0% vs 12.4%; difference: 15.64%; 95% CI: 2.96, 28.32; p=0.016). Conclusion: Asthma remission is achievable in patients with EGPA after treatment with benralizumab or mepolizumab.

Abstract:Eosinophilic gastrointestinal disorders (EGIDs) are characterized by GI tract dysfunction (dysphagia, vomiting, gastro-esophageal reflux, abdominal pain, diarrhea, etc.) and findings of increased tissue eosinophils on biopsy. These disorders can affect any segment of the GI tract, and the best characterized is eosinophilic esophagitis (EoE). Originally described as a distinct entity in the 1990s, EoE is now recognized worldwide with defined clinical, endoscopic, and histologic diagnostic criteria as well as widely accepted guidelines for management [ 1 ]. Treatment options include food elimination diets, which focus on removal of the triggering food(s), or use of medications, to induce disease remission. There are generally three categories of medications utilized: proton-pump inhibitors (PPIs), swallowed, topical corticosteroids (TCS), and biologics. In EoE, PPIs function as anti-inflammatory agents by decreasing the expression of Th2 (allergic) cytokines and improving the disrupted barrier function of the esophagus. Swallowed, topical corticosteroids and many biologics function in a similar way: downregulation of Th2 cytokines, repair of esophageal barrier function, and prevention of tissue remodeling. In trials of PPIs, between 20% and 50% of patients demonstrate clinical and histologic improvement with high-dose monotherapy and the majority maintain this with long-term use [ 2 ]. PPIs are positioned among the first-line treatments for EoE, although they are not approved by the FDA in the United States for this indication. TCS have undergone the most extensive evaluation of any therapeutic for EoE. There are currently two formulations utilized in the United States: budesonide, which is mixed into a 'slurry,' and fluticasone propionate, which is administered as an aerosol through an MDI device. Both formulations are swallowed, providing a topical effect in the esophagus and each is effective for both induction of clinical and histological remission of EoE as well as long-term maintenance therapy [ 3 ]. Biologics have garnered much recent attention, likely due to their success in a variety of other disorders including atopic diseases such as asthma and atopic dermatitis.

Abstract:Abstract Background: Eosinophil-related cutaneous adverse events (ercAEs) are common toxicities of PI3K inhibitors, antibody drug conjugates (ADCs), checkpoint inhibitors (CPIs), and targeted cancer therapies. While systemic corticosteroids are effective, their use is limited by additional toxicities and high recurrence. Benralizumab is an anti-IL-5Rα monoclonal antibody that induces eosinophil depletion via antibody-dependent cellular cytotoxicity, and is FDA-approved for severe eosinophilic asthma but has not been studied for ercAEs. Methods: We conducted a single-center, single-arm, Phase 2 trial (NCT04552288) of benralizumab in cancer patients with Grade 2/3 ercAEs from PI3K inhibitors, ADCs, CPIs or targeted therapies. Patients received benralizumab 30 mg subcutaneously every 4 weeks for 3 doses, then every 8 weeks for 3 doses. The primary endpoint was clinical response, defined as reduction in ercAE Grade to 0/1 at Week 4. Secondary endpoints included changes in quality of life, rash body surface area (BSA), cytokines and eosinophil biomarkers, as well as adverse events (AEs). Results: Forty-seven patients were enrolled, predominantly with breast (55.3%), bladder (10.6%), melanoma (10.6%), or kidney (6.4%) cancer. Baseline ercAEs were Grade 2 (48.9%) and Grade 3 (51.1%), most often from PI3K inhibitors (46.8%), CPIs (21.3%), tyrosine kinase inhibitors (8.5%) or ADCs (8.5%). Overall, 32 of 42 evaluable patients (76.2%) had clinical response by Week 4, with decreased ercAE grade (mean 2.4 to 0.9, p<0.0001), improved quality of life, reduced rash BSA, decreased peripheral and skin eosinophils (all p<0.0001), and cytokine changes. Clinical response by Week 4 was seen in all patients with ercAEs from alpelisib (PI3K inhibitor; n=18) or enfortumab vedotin (ADC; n=4); p-value for both: p<0.05. Mild-to-moderate mucositis, diarrhea, and xerosis were the most common AEs, all unrelated to benralizumab. Conclusions: Benralizumab demonstrated favorable safety and efficacy for Grade 2/3 ercAEs from cancer therapies, especially alpelisib and enfortumab vedotin. Abstract:Abstract Background Patients with ulcerative colitis (UC) are often treated with biological therapies or small molecules. Knowledge about the impact of these therapies on the intestinal and peripheral blood immune cell composition is scarce. Therefore, we investigated how advanced therapies modulate immune cell distribution in UC patients. Methods We included 30 UC patients (53% male, median age 42 years) who started a biological or small molecule. Before the first drug administration, mucosal colonic biopsies and a peripheral blood sample were obtained. At the end of induction, colonic biopsies and peripheral blood were sampled again. Patients starting adalimumab (n=2), infliximab (n=3), vedolizumab (n=11), ustekinumab (n=6), ozanimod (n=2) and the JAK inhibitors filgotinib (n=3) and tofacitinib (n=3) were included. Endoscopic improvement was defined as a Mayo endoscopic subscore of 0-1 at the end of induction. From the biopsies, a single-cell suspension was made. Intestinal and circulating immune cells were characterized via flow cytometry. Statistical analysis was performed using a paired t-test. Results Independent of the mechanism of action (MOA), patients responding to therapy showed a decrease of colonic granulocytes (neutrophils (p<0.0001) (Figure 1A), basophils (p<0.0001) (Figure 1B) and eosinophils (p=0.008) (Figure 1C)), active eosinophils (p=0.002) (Figure 1D)), B cells (p=0.05) (Figure 1E), regulatory T cells (p<0.0001) (Figure 1F) and T helper (Th) 2 cells (p=0.02) (Figure 1G), balanced with an increase of Th1 cells (p=0.03) (Figure 1H). In peripheral blood, eosinophils increased in patients not responding to therapy (p=0.05) (Figure 1I). Furthermore, we observed that only patients starting vedolizumab (n=11) showed a decrease in colonic eosinophils (p=0.02) (Figure 1J), active eosinophils (p=0.002) (Figure 1K), B cells (p=0.03) (Figure 1L) and T cells (p=0.004) (Figure 1M). Considering only non-vedolizumab patients (n=19), we did not observe this effect. Conclusion UC patients responding to advanced therapies showed a different intestinal immune cell distribution compared to non-responders, regardless of MOA. Vedolizumab therapy furthermore decreased several mucosal immune cell subsets that migrate to the gut through α4β7-MAdCAM-1 binding. While the effect of vedolizumab on B cells and T cells was previously described, we have now potentially identified an additional eosinophil-reducing effect in the colon.

Abstract:This meta‐analysis of eight studies evaluates the efficacy and safety of benralizumab in patients with eosinophilic granulomatosis polyangiitis (EGPA). Findings demonstrate a significant reduction in oral corticosteroid (OCS) usage with a mean reduction of 8.25 mg/day, achieved remission in 56.8% of patients, and immunosuppressants withdrawal in 28.1%, over a mean follow‐up of about 1 year. Adverse events were mild, with only one discontinuation. Benralizumab is effective and well‐tolerated in managing EGPA. Objective Eosinophilic granulomatous polyangiitis (EGPA) is a rare autoimmune disease characterized by multisystemic inflammation, with eosinophils playing a central role in its pathogenesis. Traditional management relies heavily on corticosteroids and immunosuppressants, which are associated with significant side effects. The emergence of biologic agents, such as benralizumab, offers targeted therapeutic options by inhibiting the interleukin‐5 receptor α, thereby reducing eosinophilic inflammation. Methods This systematic review and meta‐analysis comprehensively evaluate the efficacy and safety of benralizumab in EGPA patients, focusing on its ability to reduce oral corticosteroid (OCS) use, facilitate remission and spare immunosuppressants. We searched MEDLINE, LILACS and ISI Web of Science databases for relevant studies up to July 2024. Results Eight studies, including both randomized controlled trials (RCTs) and observational studies, were included in the meta‐analysis, involving a total of 396 EGPA patients. The pooled analysis demonstrated a significant reduction in OCS dose, with an overall estimated effect of −8.25 mg/day (95% CI, −9.39 to −7.10). Complete remission was achieved in 56.8% of patients, and immunosuppressants were reduced or discontinued in 28.1% of cases. Adverse events (AEs) were reported in 21.9% of patients, with only one discontinuation due to an AE. Conclusion These findings provide robust evidence supporting the use of benralizumab as an effective and well‐tolerated treatment option for EGPA, significantly reducing OCS requirements and offering promising remission rates. Future research should focus on larger, multicentre RCTs to confirm these findings and further elucidate the long‐term benefits and safety profile of benralizumab in EGPA.

Abstract:Introduction Eosinophilic esophagitis (EoE) is a chronic, inflammatory, antigen-driven disease of the esophagus. Tissue EoE pathology has previously been extensively characterized by novel transcriptomics and proteomic platforms, however the majority of surface marker determination and screening has been performed in blood due to mucosal tissue size limitations. While eosinophils, CD4 + T cells, mast cells and natural killer (NK) T cells were previously investigated in the context of EoE, an accurate picture of the composition of peripheral blood mononuclear cells (PBMC) and their activation is missing. Methods In this study, we aimed to comprehensively analyze the composition of peripheral blood mononuclear cells and their activation using surface marker measurements with multicolor flow cytometry simultaneously in both blood and mucosal tissue of patients with active EoE, inactive EoE, patients with gastroesophageal reflux disease (GERD) and controls. Moreover, we set out to validate our data in co-cultures of PBMC with human primary esophageal epithelial cells and in a novel inducible mouse model of eosinophilic esophagitis, characterized by extensive IL-33 secretion in the esophagus. Results Our results indicate that specific PBMC populations are enriched, and that they alter their surface expression of activation markers in mucosal tissue of active EoE. In particular, we observed upregulation of the immunomodulatory molecule CD38 on CD4 + T cells and on myeloid cells in biopsies of active EoE. Moreover, we observed significant upregulation of PD-1 on CD4 + and myeloid cells, which was even more prominent after corticosteroid treatment. With co-culture experiments we could demonstrate that direct cell contact is needed for PD-1 upregulation on CD4 + T cells. Finally, we validated our findings of PD-1 and CD38 upregulation in an inducible mouse model of EoE. Discussion Herein we show significant alterations in the PBMC activation profile of patients with active EoE in comparison to inactive EoE, GERD and controls, which could have potential implications for treatment. To our knowledge, this study is the first of its kind expanding the multi-color flow cytometry approach in different patient groups using in vitro and in vivo translational models.

Abstract:Mepolizumab and Benralizumab represent two anti-eosinophilic biologic strategies (A-EBS) for the treatment of severe uncontrolled eosinophilic asthma. Bronchial hyperreactivity (BH) is a hallmark of asthma, whereas A-EBS impact on BH is not fully clarified. We aimed to evaluate the change in BH, estimated through bronchodilator response (BDR), after 1 year of A-EBS in severe asthmatics. 27 patients (Mepolizumab n=16 and Benralizumab n=11) were included in this prospective cohort study. Lung function parameters were collected at baseline and 20 minutes after intake of salbutamol 400 mcg, both at the beginning (T0) and after one year of an A-EBS (T12). Blood Eosinophil Count (BEC), Fraction Exhaled Nitric Oxide (FeNO) and Asthma Control Test (ACT) were also recorded. Data were expressed as median (IQR) and analyzed with the Mann-Whitney test. Statistical significance was set at a p-value<0.05. No statistical significance was found for the adjusted FEV1% change between T0 and T12 [0.09% (0.16-0.02) vs 0.1% (0.19-0.05); p=0.336]. ACT was 16 (18.5-12.5) at T0 vs 21 (24-19) at T12; p<0.0001. BEC was 1040/μL (1035-410) at T0 vs 65/μL (132.5-0) at T12; p<0.0001. FeNO was 40 ppb (67.5-17) at T0 vs 26 ppb (62-18.5) at T12; p=0.441. We found no significant change in BHR after one year of A-EBS, although a significant improvement in ACT and BEC reduction. Interestingly, FeNO values, despite lowered, were not significantly reduced, showing a persisting share of airway inflammation. In light of these data, we hypothize that BHR might be related to inflammatory mechanisms linked to additional cytokines and barrier alarmins, beyond IL-5.

Abstract:Background: Interleukin-5 (IL-5) inhibitors represent novel therapies for eosinophilic granulomatosis with polyangiitis (EGPA). This study assessed the effectiveness and safety of the IL-5 receptor inhibitor benralizumab in a European cohort of patients with EGPA. Methods: This retrospective cohort study included patients with EGPA from 28 European referral centres of the European EGPA Study Group across six countries (Italy, France, UK, Russia, Spain, and Switzerland) who received benralizumab as any line of treatment between Jan 1, 2019, and Sep 30, 2022. We assessed the rates of complete response, defined as no disease activity (Birmingham Vasculitis Activity Score [BVAS] of 0) and a prednisone dose of up to 4 mg/day, in contrast to partial response, defined as a BVAS of 0 and a prednisone dose greater than 4 mg/day. Active disease manifestations, pulmonary function, variation in glucocorticoid dose, and safety outcomes were also assessed over a 12-month follow-up. Findings: 121 patients with relapsing-refractory EGPA treated with benralizumab at the dose approved for eosinophilic asthma were included (64 [53%] women and 57 [47%] men; median age at the time of beginning benralizumab treatment 54·1 years [IQR 44·2-62·2]). Complete response was reported in 15 (12·4%, 95% CI 7·1-19·6) of 121 patients at month 3, 25 (28·7%, 19·5-39·4) of 87 patients at month 6, and 32 (46·4%, 34·3-58·8) of 69 patients at month 12; partial response was observed in an additional 43 (35·5%, 27·0-44·8) patients at month 3, 23 (26·4%, 17·6-37·0) at month 6, and 13 (18·8%, 10·4-30·1) at month 12. BVAS dropped from 3·0 (IQR 2·0-8·0) at baseline to 0·0 (0·0-2·0) at months 3 and 6, and to 0·0 (0·0-1·0) at month 12. The proportion of patients with systemic manifestations, active peripheral neurological disease, ear, nose, and throat involvement, and pulmonary involvement decreased, with an improvement in lung function tests. Six patients relapsed after having a complete response. The oral prednisone (or equivalent) dose decreased from 10·0 mg/day (5·0-12·5) at baseline to 5·0 mg/day (3·6-8·5) at month 3 (p<0·01), to 5·0 mg/day (2·5-6·3) at month 6, and to 2·5 mg/day (0·0-5·0) at month 12 (p<0·0001). 19 (16%) of 121 patients had adverse events and 16 (13%) discontinued benralizumab. Interpretation: These data suggest that benralizumab could be an effective treatment for EGPA in real-life clinical practice. Further clinical trials are required to confirm the efficacy of benralizumab in patients with a higher baseline disease activity. Funding: None.

Abstract:BackgroundEosinophilic gastroenteritis (EGE) is a rare, chronic inflammatory condition of the gastrointestinal tract. Little is known about its natural history and treatment outcomes. The aims of our analysis were to describe clinical presentation, response to current medical treatments, and to evaluate the response of refractory EGE to anti-integrin therapy.MethodsPatients with confirmed diagnosis of EGE fulfilling the diagnostic criteria: (1) the presence of gastrointestinal symptoms, (2) dense eosinophilic infiltration of the gastrointestinal mucosa, and (3) exclusion of other conditions leading to gastrointestinal eosinophilia were included in this analysis. In patients non-responding to corticosteroids and/or anti-TNF treatment the integrin blocker vedolizumab was used.ResultsEGE patients (n = 22) were predominantly female (63%) with a median age at diagnosis of 41.5 years. The most frequent symptoms were abdominal pain (100%), diarrhea (59%), nausea/vomiting (36%), and bloating (27%). No pathognomonic endoscopic alterations were found. Eosinophilic infiltration was observed in the majority of patients in more than one segment. Patients were treated with systemic steroids, topical, and enteral release steroids in 21/22 (95%) patients, proton pump inhibitors in 7/22 (32%), TNFα inhibitors in 3/22 (14%), and vedolizumab in 4/22 (18%) patients. In 3/4 of steroid-refractory patients vedolizumab induced a clinical and histological improvement.ConclusionsThe combination of highly variable clinical presentation, subtle endoscopic abnormalities, and involvement of several GI segments undermines the difficulty to diagnose EGE and the need for structured biopsy sampling. Corticosteroids were efficient in the majority of patients to induce remission. Response to the integrin blocker vedolizumab suggests further assessment in refractory cases.
