Abstract:Particularly in the field of computer vision, Deep Learning (DL) models have demonstrated superior performance over classical Machine Learning (ML) at recognizing patterns in complicated and high-dimensional data. Recent years have seen a significant increase in interest in the use of DL for automated categorization and early diagnosis of Alzheimer's disease (AD) through 2D/3D MRI images. This paper studied different algorithms by using ML, DL models and MRI images to detect the multiple stages of AD such as "Very-Mild-Demented," "Mild-Demented," "Moderate-Demented," and "No-Demented." Even though, the DL models with pre-processing steps have outperformed with regard to performance, there are still certain difficulties to select hyper-parameters. This article presents a thorough analysis and assessment of the most recent algorithms for early AD detection and classification that exploit cutting-edge DL approaches. Moreover, it describes the up-to-date databases frequently employed for performance assessment of different AD disease. For the researcher's support, the difficulties and possible improvements of classical and deep learning methods are talked about in-depth. Last but not least, problems with research gaps are described as well as ways to fix them. In this study, we looked at how benchmarks for GPUs and CPUs affect DL algorithms. To help other professionals, the limitations and future directions are also discussed. It concludes by discussing the prospects and difficulties for systems rely on DL for classifying AD.

Abstract:Background: Dementia is a general term for several progressive neurodegenerative disorders including Alzheimer's disease. Timely and accurate detection is crucial for early intervention. Advancements in artificial intelligence present significant potential for using machine learning to aid in early detection. Objective: Summarize the state-of-the-art machine learning-based approaches for dementia prediction, focusing on non-invasive methods, as the burden on the patients is lower. Specifically, the analysis of gait and speech performance can offer insights into cognitive health through clinically cost-effective screening methods. Methods: A systematic literature review was conducted following the PRISMA protocol (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). The search was performed on three electronic databases (Scopus, Web of Science, and PubMed) to identify the relevant studies published between 2017 to 2022. A total of 40 papers were selected for review. Results: The most common machine learning methods employed were support vector machine followed by deep learning. Studies suggested the use of multimodal approaches as they can provide comprehensive and better prediction performance. Deep learning application in gait studies is still in the early stages as few studies have applied it. Moreover, including features of whole body movement contribute to better classification accuracy. Regarding speech studies, the combination of different parameters (acoustic, linguistic, cognitive testing) produced better results. Conclusions: The review highlights the potential of machine learning, particularly non-invasive approaches, in the early prediction of dementia. The comparable prediction accuracies of manual and automatic speech analysis indicate an imminent fully automated approach for dementia detection.

Abstract:Alzheimers Disease AD is an acute neuro disease that degenerates the brain cells and thus leads to memory loss progressively. It is a fatal brain disease that mostly affects the elderly. It steers the decline of cognitive and biological functions of the brain and shrinks the brain successively, which in turn is known as Atrophy. For an accurate diagnosis of Alzheimers disease, cutting edge methods like machine learning are essential. Recently, machine learning has gained a lot of attention and popularity in the medical industry. As the illness progresses, those with Alzheimers have a far more difficult time doing even the most basic tasks, and in the worst case, their brain completely stops functioning. A persons likelihood of having early-stage Alzheimers disease may be determined using the ML method. In this analysis, papers on Alzheimers disease diagnosis based on deep learning techniques and reinforcement learning between 2008 and 2023 found in google scholar were studied. Sixty relevant papers obtained after the search was considered for this study. These papers were analysed based on the biomarkers of AD and the machine-learning techniques used. The analysis shows that deep learning methods have an immense ability to extract features and classify AD with good accuracy. The DRL methods have not been used much in the field of image processing. The comparison results of deep learning and reinforcement learning illustrate that the scope of Deep Reinforcement Learning DRL in dementia detection needs to be explored.

Abstract:Dementia, a chronic and progressive cognitive declination of brain function caused by disease or impairment, is becoming more prevalent due to the aging population. A major challenge in dementia is achieving accurate and timely diagnosis. In recent years, neuroimaging with computer-aided algorithms have made remarkable advances in addressing this challenge. The success of these approaches is mostly attributed to the application of machine learning techniques for neuroimaging. We present a comprehensive survey of automated diagnostic approaches for dementia using medical image analysis and machine learning algorithms published in the recent years. Based on the rigorous review of the existing works, we have found that, while most of the studies focused on Alzheimer's disease, recent research has demonstrated reasonable performance in the identification of other types of dementia remains a major challenge. Multimodal imaging analysis deep learning approaches have shown promising results in the diagnosis of these other types of dementia. The main contributions of this review paper are as follows: 1. Based on the detailed analysis of the existing literature, this paper discusses neuroimaging procedures for dementia diagnosis, and 2. It systematically explains the most recent machine learning techniques and, in particular, deep learning approaches for early detection of dementia.

Abstract:Introduction: Machine learning (ML) has been extremely successful in identifying key features from high-dimensional datasets and executing complicated tasks with human expert levels of accuracy or greater. Methods: We summarize and critically evaluate current applications of ML in dementia research and highlight directions for future research. Results: We present an overview of ML algorithms most frequently used in dementia research and highlight future opportunities for the use of ML in clinical practice, experimental medicine, and clinical trials. We discuss issues of reproducibility, replicability and interpretability and how these impact the clinical applicability of dementia research. Finally, we give examples of how state-of-the-art methods, such as transfer learning, multi-task learning, and reinforcement learning, may be applied to overcome these issues and aid the translation of research to clinical practice in the future. Discussion: ML-based models hold great promise to advance our understanding of the underlying causes and pathological mechanisms of dementia.

Abstract:Alzheimer’s disease (AD), the most familiar type of dementia, is a severe concern in modern healthcare. Around 5.5 million people aged 65 and above have AD, and it is the sixth leading cause of mortality in the US. AD is an irreversible, degenerative brain disorder characterized by a loss of cognitive function and has no proven cure. Deep learning techniques have gained popularity in recent years, particularly in the domains of natural language processing and computer vision. Since 2014, these techniques have begun to achieve substantial consideration in AD diagnosis research, and the number of papers published in this arena is rising drastically. Deep learning techniques have been reported to be more accurate for AD diagnosis in comparison to conventional machine learning models. Motivated to explore the potential of deep learning in AD diagnosis, this study reviews the current state-of-the-art in AD diagnosis using deep learning. We summarize the most recent trends and findings using a thorough literature review. The study also explores the different biomarkers and datasets for AD diagnosis. Even though deep learning has shown promise in AD diagnosis, there are still several challenges that need to be addressed.

Fusion of Double-layered Patella Using a Single Small Fragment Partially Threaded Screw: A Case Report

Abstract:Background The double-layered patella (DLP) was first described by Büttner in 1925 and is now considered pathognomonic for multiple epiphyseal dysplasia. In this study, we describe the case of an 11-year-old girl with multiple epiphyseal dysplasia and right-sided DLP. Methods The patient presented with intermittent unilateral anterior knee pain and swelling. There was a reproducible high-pitched click with every step. Imaging showed a DLP in good alignment. Surgical management comprised open debridement and fixation of the 2 patellar layers using one 3.5 mm (removed-) partially threaded AO small fragment screw. Results Standard radiographs of the right knee, 3 years postoperatively, showed sound union of the 2 patellae. The patient now plays sports, and recent examination showed a full range of movement with no instability, effusion, or tenderness. Conclusions We conclude that surgical fusion of the patellar fragments in DLP using a single small fragment partially threaded screw provides excellent outcome at 3 years follow-up. Clinical Relevance This case report shows a comparatively simple and economical surgical solution to the problem of DLP.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disorder that causes memory degradation and cognitive function impairment in elderly people. The irreversible and devastating cognitive decline brings large burdens on patients and society. So far, there is no effective treatment that can cure AD, but the process of early-stage AD can slow down. Early and accurate detection is critical for treatment. In recent years, deep-learning-based approaches have achieved great success in Alzheimer's disease diagnosis. The main objective of this paper is to review some popular conventional machine learning methods used for the classification and prediction of AD using Magnetic Resonance Imaging (MRI). The methods reviewed in this paper include support vector machine (SVM), random forest (RF), convolutional neural network (CNN), autoencoder, deep learning, and transformer. This paper also reviews pervasively used feature extractors and different types of input forms of convolutional neural network. At last, this review discusses challenges such as class imbalance and data leakage. It also discusses the trade-offs and suggestions about pre-processing techniques, deep learning, conventional machine learning methods, new techniques, and input type selection.

Abstract:Alzheimer's disease is the most prevalent form of dementia, which is a gradual condition that begins with mild memory loss and progresses to difficulties communicating and responding to the environment. Recent advancements in neuroimaging techniques have resulted in large-scale multimodal neuroimaging data, leading to an increased interest in using deep learning for the early diagnosis and automated classification of Alzheimer's disease. This study uses machine learning (ML) methods to determine the severity level of Alzheimer's disease using MRI images, where the dataset consists of four levels of severity. A hybrid of 12 feature extraction methods is used to diagnose Alzheimer's disease severity, and six traditional machine learning methods are applied, including decision tree, K-nearest neighbor, linear discrimination analysis, Naïve Bayes, support vector machine, and ensemble learning methods. During training, optimization is performed to obtain the best solution for each classifier. Additionally, a CNN model is trained using a machine learning system algorithm to identify specific patterns. The accuracy of the Naïve Bayes, Support Vector Machines, K-nearest neighbor, Linear discrimination classifier, Decision tree, Ensembled learning, and presented CNN architecture are 67.5%, 72.3%, 74.5%, 65.6%, 62.4%, 73.8% and, 95.3%, respectively. Based on the results, the presented CNN approach outperforms other traditional machine learning methods to find Alzheimer severity.

Abstract:Background: Alzheimer's disease (AD) is the most prevalent cause of dementia, characterized by a steady decline in mental, behavioral, and social abilities and impairs a person's capacity for independent functioning. It is a fatal neurodegenerative disease primarily affecting older adults. Objectives: The purpose of this literature review is to investigate various AD detection techniques, datasets, input modalities, algorithms, libraries, and performance evaluation metrics used to determine which model or strategy may provide superior performance. Method: The initial search yielded 807 papers, but only 100 research articles were chosen after applying the inclusion-exclusion criteria. This SLR analyzed research items published between January 2019 and December 2022. The ACM, Elsevier, IEEE Xplore Digital Library, PubMed, Springer and Taylor & Francis were systematically searched. The current study considers articles that used Magnetic Resonance Imaging (MRI), Positron Emission Tomography (PET), APOe4 genotype, Diffusion Tensor Imaging (DTI) and Cerebrospinal Fluid (CSF) biomarkers. The study was performed following Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. Conclusion: According to the literature survey, most studies (n = 76) used the DL strategy. The datasets used by studies were primarily derived from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database. The majority of studies (n = 73) used single-modality neuroimaging data, while the remaining used multi-modal input data. In a multi-modality approach, the combination of MRI and PET scans is commonly preferred. Also, Regarding the algorithm used, Convolution Neural Network (CNN) showed the highest accuracy, 100 %, in classifying AD vs. CN subjects whereas the SVM was the most common ML algorithm, with a maximum accuracy of 99.82 %.

Abstract:Background Early identification of Alzheimer’s disease or mild cognitive impairment can help guide direct prevention and supportive treatments, improve outcomes, and reduce medical costs. Existing advanced diagnostic tools are mostly based on neuroimaging and suffer from certain problems in cost, reliability, repeatability, accessibility, ease of use, and clinical integration. To address these problems, we developed, evaluated, and implemented an early diagnostic tool using machine learning and non-imaging factors. Methods and results A total of 654 participants aged 65 or older from the Nursing Home in Hangzhou, China were identified. Information collected from these patients includes dementia status and 70 demographic, cognitive, socioeconomic, and clinical features. Logistic regression, support vector machine (SVM), neural network, random forest, extreme gradient boosting (XGBoost), least absolute shrinkage and selection operator (LASSO), and best subset models were trained, tuned, and internally validated using a novel double cross validation algorithm and multiple evaluation metrics. The trained models were also compared and externally validated using a separate dataset with 1,100 participants from four communities in Zhejiang Province, China. The model with the best performance was then identified and implemented online with a friendly user interface. For the nursing dataset, the top three models are the neural network (AUROC = 0.9435), XGBoost (AUROC = 0.9398), and SVM with the polynomial kernel (AUROC = 0.9213). With the community dataset, the best three models are the random forest (AUROC = 0.9259), SVM with linear kernel (AUROC = 0.9282), and SVM with polynomial kernel (AUROC = 0.9213). The F1 scores and area under the precision-recall curve showed that the SVMs, neural network, and random forest were robust on the unbalanced community dataset. Overall the SVM with the polynomial kernel was found to be the best model. The LASSO and best subset models identified 17 features most relevant to dementia prediction, mostly from cognitive test results and socioeconomic characteristics. Conclusion Our non-imaging-based diagnostic tool can effectively predict dementia outcomes. The tool can be conveniently incorporated into clinical practice. Its online implementation allows zero barriers to its use, which enhances the disease’s diagnosis, improves the quality of care, and reduces costs.

Abstract:The rapid global aging trend has led to an increase in dementia cases, including Alzheimer's disease, underscoring the urgent need for early and accurate diagnostic methods. Traditional diagnostic techniques, such as cognitive tests, neuroimaging, and biomarker analysis, face significant limitations in sensitivity, accessibility, and cost, particularly in the early stages. This study explores the potential of machine learning (ML) as a transformative approach to enhance early dementia detection by leveraging ML models to analyze and integrate complex multimodal datasets, including cognitive assessments, neuroimaging, and genetic information. A comprehensive review of existing literature was conducted to evaluate various ML models, including supervised learning, deep learning, and advanced techniques such as ensemble learning and transformer models, assessing their accuracy, interpretability, and potential for clinical integration. The findings indicate that while ML models show significant promise in improving diagnostic precision and enabling earlier interventions, challenges remain in their generalizability, interpretability, and ethical deployment. This research concludes by outlining future directions aimed at enhancing the clinical utility of ML models in dementia detection, emphasizing interdisciplinary collaboration and ethically sound frameworks to improve early detection and intervention strategies for Alzheimer's disease and other forms of dementia.

Abstract:INTRODUCTION: Advanced machine learning methods might help to identify dementia risk from neuroimaging, but their accuracy to date is unclear. METHODS: We systematically reviewed the literature, 2006 to late 2016, for machine learning studies differentiating healthy ageing through to dementia of various types, assessing study quality, and comparing accuracy at different disease boundaries. RESULTS: Of 111 relevant studies, most assessed Alzheimer's disease (AD) vs healthy controls, used ADNI data, support vector machines and only T1-weighted sequences. Accuracy was highest for differentiating AD from healthy controls, and poor for differentiating healthy controls vs MCI vs AD, or MCI converters vs non-converters. Accuracy increased using combined data types, but not by data source, sample size or machine learning method. DISCUSSION: Machine learning does not differentiate clinically-relevant disease categories yet. More diverse datasets, combinations of different types of data, and close clinical integration of machine learning would help to advance the field.

Abstract:part of the much larger topic of free choice as a principle in the organization and delivery of health care services. I focus my attention on the choice of a source of care by clients. I narrow the scope of my inquiry even further by putting aside, at least for now, the closely related issue of free access to clients by practitioners, including the reasons for wanting to place restrictions on that access. Even though, in these ways, I have greatly simplified my task, I hope to be able to show the remarkably complex ramifications of the principle of free choice even within this restricted domain. For though I can hardly expect to say anything new about the principle itself, I hope that I shall be able to demonstrate the many, and often conflicting, considerations that flow from the principle of free choice when it becomes operational. Far from remaining a softly comfortable abstraction to which one can safely pay lip service, free choice now becomes a hard (and sometimes intractable) reality that the administrator must learn to handle. My method in this exposition will be to look at the consequences of free choice from several different perspectives that I shall identify

Abstract:Dementia, a prevalent neurodegenerative condition, is a major manifestation of Alzheimer's disease (AD). As the condition progresses from mild to severe, it significantly impairs the individual's ability to perform daily tasks independently, necessitating the need for timely and accurate AD classification. Machine learning or deep learning models have emerged as effective tools for this purpose. In this study, we suggested an approach for classifying the four stages of dementia using RF, SVM, and CNN algorithms, augmented with watershed segmentation for feature extraction from MRI images. Our results reveal that SVM with watershed features achieves an impressive accuracy of 96.25%, surpassing other classification methods. The ADNI dataset is utilized to evaluate the effectiveness of our method, and we observed that the inclusion of watershed segmentation contributes to the enhanced performance of the models.

Abstract:Objective Alzheimer disease (AD) is the most common cause of dementia, a syndrome characterized by cognitive impairment severe enough to interfere with activities of daily life. We aimed to conduct a systematic literature review (SLR) of studies that applied machine learning (ML) methods to clinical data derived from electronic health records in order to model risk for progression of AD dementia. Materials and Methods: We searched for articles published between January 1, 2010, and May 31, 2020, in PubMed, Scopus, ScienceDirect, IEEE Explore Digital Library, Association for Computing Machinery Digital Library, and arXiv. We used predefined criteria to select relevant articles and summarized them according to key components of ML analysis such as data characteristics, computational algorithms, and research focus. Results: There has been a considerable rise over the past 5 years in the number of research papers using ML-based analysis for AD dementia modeling. We reviewed 64 relevant articles in our SLR. The results suggest that majority of existing research has focused on predicting progression of AD dementia using publicly available datasets containing both neuroimaging and clinical data (neurobehavioral status exam scores, patient demographics, neuroimaging data, and laboratory test values). Discussion: Identifying individuals at risk for progression of AD dementia could potentially help to personalize disease management to plan future care. Clinical data consisting of both structured data tables and clinical notes can be effectively used in ML-based approaches to model risk for AD dementia progression. Data sharing and reproducibility of results can enhance the impact, adaptation, and generalizability of this research.

Abstract:Introduction: Artificial intelligence (AI) and neuroimaging offer new opportunities for diagnosis and prognosis of dementia. Methods: We systematically reviewed studies reporting AI for neuroimaging in diagnosis and/or prognosis of cognitive neurodegenerative diseases. Results: A total of 255 studies were identified. Most studies relied on the Alzheimer's Disease Neuroimaging Initiative dataset. Algorithmic classifiers were the most commonly used AI method (48%) and discriminative models performed best for differentiating Alzheimer's disease from controls. The accuracy of algorithms varied with the patient cohort, imaging modalities, and stratifiers used. Few studies performed validation in an independent cohort. Discussion: The literature has several methodological limitations including lack of sufficient algorithm development descriptions and standard definitions. We make recommendations to improve model validation including addressing key clinical questions, providing sufficient description of AI methods and validating findings in independent datasets. Collaborative approaches between experts in AI and medicine will help achieve the promising potential of AI tools in practice. Highlights: There has been a rapid expansion in the use of machine learning for diagnosis and prognosis in neurodegenerative disease Most studies (71%) relied on the Alzheimer's Disease Neuroimaging Initiative (ADNI) dataset with no other individual dataset used more than five times There has been a recent rise in the use of more complex discriminative models (e.g., neural networks) that performed better than other classifiers for classification of AD vs healthy controls We make recommendations to address methodological considerations, addressing key clinical questions, and validation We also make recommendations for the field more broadly to standardize outcome measures, address gaps in the literature, and monitor sources of bias.
Breaking barriers: a statistical and machine learning-based hybrid system for predicting dementia

Abstract:Introduction: Dementia is a condition (a collection of related signs and symptoms) that causes a continuing deterioration in cognitive function, and millions of people are impacted by dementia every year as the world population continues to rise. Conventional approaches for determining dementia rely primarily on clinical examinations, analyzing medical records, and administering cognitive and neuropsychological testing. However, these methods are time-consuming and costly in terms of treatment. Therefore, this study aims to present a noninvasive method for the early prediction of dementia so that preventive steps should be taken to avoid dementia. Methods: We developed a hybrid diagnostic system based on statistical and machine learning (ML) methods that used patient electronic health records to predict dementia. The dataset used for this study was obtained from the Swedish National Study on Aging and Care (SNAC), with a sample size of 43040 and 75 features. The newly constructed diagnostic extracts a subset of useful features from the dataset through a statistical method (F-score). For the classification, we developed an ensemble voting classifier based on five different ML models: decision tree (DT), naive Bayes (NB), logistic regression (LR), support vector machines (SVM), and random forest (RF). To address the problem of ML model overfitting, we used a cross-validation approach to evaluate the performance of the proposed diagnostic system. Various assessment measures, such as accuracy, sensitivity, specificity, receiver operating characteristic (ROC) curve, and Matthew's correlation coefficient (MCC), were used to thoroughly validate the devised diagnostic system's efficiency. Results: According to the experimental results, the proposed diagnostic method achieved the best accuracy of 98.25%, as well as sensitivity of 97.44%, specificity of 95.744%, and MCC of 0.7535. Discussion: The effectiveness of the proposed diagnostic approach is compared to various cutting-edge feature selection techniques and baseline ML models. From experimental results, it is evident that the proposed diagnostic system outperformed the prior feature selection strategies and baseline ML models regarding accuracy.

Abstract:Abstract Worldwide, there are nearly 10 million new cases of dementia annually, of which Alzheimer’s disease (AD) is the most common. New measures are needed to improve the diagnosis of individuals with cognitive impairment due to various etiologies. Here, we report a deep learning framework that accomplishes multiple diagnostic steps in successive fashion to identify persons with normal cognition (NC), mild cognitive impairment (MCI), AD, and non-AD dementias (nADD). We demonstrate a range of models capable of accepting flexible combinations of routinely collected clinical information, including demographics, medical history, neuropsychological testing, neuroimaging, and functional assessments. We then show that these frameworks compare favorably with the diagnostic accuracy of practicing neurologists and neuroradiologists. Lastly, we apply interpretability methods in computer vision to show that disease-specific patterns detected by our models track distinct patterns of degenerative changes throughout the brain and correspond closely with the presence of neuropathological lesions on autopsy. Our work demonstrates methodologies for validating computational predictions with established standards of medical diagnosis.
