Abstract:Objective To explore the efficacy and safety of small-incision untethering surgery in the treatment of tethered cord syndrome(TCS)in children. Methods We retrospectively collected the clinical data of TCS patients who underwent merely untethering surgery at Department of Neurosurgery, Children's Hospital, Zhejiang University School of Medicine from May 2015 to May 2018. A total of 72 cases were included and all required transection of merely terminal filament. Among them, there were 33 patients undergoing small-incision operation(small-incision group)and 39 undergoing traditional surgery(traditional group). Comparative analysis of intraoperative incision size, postoperative complications, length of stay and postoperative treatment effect were implemented in both groups. Results All 72 patients underwent surgery successfully. None of the children in the small incision group needed to have the incision extended. In small-incision and tradition groups, the length of the incision was(2. 5 ±0. 3)cm and(4. 8 ±1. 9)cm respectively, and the length of stay was(4. 4 ±1. 8)days and(6. 7 ±2. 6)days respectively. Both differences were statistically significant(P＜0. 01). In traditional group, 5 cases(12. 8％)had surgery-related complications including 2 cases of wound infection, 1 case of cerebrospinal fluid leakage and 2 cases of pain requiring medication. Only 1 case of pain(3. 0％, 1/33)required medication in small-incision group. The effective rates in traditional and small-incision groups were 92. 3％(36/39)and 93. 9％(31/33)respectively at 6 months post surgery, and the difference was not statistically significant(P>0. 05). At the end of follow-up, the total effective rate in traditional group(>6-41 months)was 89. 7％(35/39), while that in small-incision group(>6-27 months)was 93. 9％(31/33). There was no statistically difference between them(P>0. 05). Conclusion The small-incision untethering for children with TCS who need merely transection of terminal filament seems as effective as traditional large-incision surgery. It has the advantages of small trauma and short length of stay.
[Classification and Microsurgical Treatment of Primary Tethered Cord Syndrome in Adults].

Abstract:To summarize the clinical manifestation, classification, and experience of surgical treatment of primary tethered cord syndrome (TCS) in adults.The authors retrospectively analyzed a series of 171 adult patients with primary TCS who were surgically treated under microscope from March 2007 to October 2019. There were 61 males and 110 females whose ages were 18-65 years, with an average age of (39.02±11.81) years. Clinically, the patients presented with various neurological symptoms and signs including lower back and legs pain, reflex changes, sensory disturbances, muscle weakness, and sphincter problems. They were divided into 5 types by clinical manifestations and neuro-imaging features: (1) filum terminale traction in 69 cases, (2) split cord malformation in 21 cases, (3) myelomeningocele in 20 cases, (4) lipomyelomeningocele in 36 cases, and (5) dermal sinus traction in 25 cases. All the patients underwent microsurgery to untether the spinal cord. The patients kept prone position 7 days postoperatively. The Kirollos grading was used to evaluate the outcome of intraoperative untethering. The visual analogue scale (VAS) was used to evaluate the pain, the score of critical muscle strength was used to evaluate the lower extremity motor function, and the Japanese Orthopaedic Association (JOA) sphincter function score was used to evaluate the bladder function.All of the 171 patients were treated with microsurgery to release the adhesion and cut off the filum terminalis. 61 cases of them received resection of the lesions according to the etiology. All the tethered spinal cord reached Kirollos grade Ⅰ untethering and the dural sac was reconstructed. Other than 5 patients had cerebrospinal fluid leakage and incision laceration and underwent re-suture, there was no surgical complication. The local pain was relieved, the lower limbs weakness or bowel and bladder dysfunction gradually recovered postoperatively. The period of follow-up ranged from 6 months to 12.5 years with an average of (5.62±2.31) years. The neurological function was improved in 153 cases and stable in 18 cases. There was no recurrence of tethered cord be found during the follow-up period.The primary TCS in adulthood could be classified into 5 types by clinical manifestations and neuro-imaging features and surgical treatment should be undertaken in regard to the classifications including dissection and resection of the lesion detethering the spinal cord and reconstruction of the dura sac under microscope. The outcome of surgical treatment is satisfactory.
Effect Analysis of Surgical Treatment in Tethered Cord Syndrome

Abstract:Objective:To observe the postoperative function improvement in patients with tethered cord syndrome.Method:112 patients with TCS defined by MRI were operated,and 72 of them were followed-up for at least two years.The patients underwent extradural release,lipomatomy,scar removal,intradural release of adhesive nerve root and spinal cord and transection of terminal filament.Result:After operation,more than 60 percent of patients who were younger than 10 years old improved in sensation,movement,sphincter function,and other complications.However,less than 40 percent of patients older than 10 years improved.Kendall grade correlation analysis shows positive relation between age of operation and function improvement (P0 01).Conclusion:The most reliable diagnosis of TCS can be acquired from MRI.The operation should be performed as early as possible after confirmation of diagnosis.The younger,the better.To avoid injury,microsurgical technique and electrophysiological monitor should be applied.
Long-term outcome following surgical treatment of posttraumatic tethered cord syndrome: a retrospective population-based cohort study

Abstract:Abstract Study design Retrospective population-based cohort study. Objective To investigate the long-term outcome following surgery for posttraumatic spinal cord tethering (PSCT). Setting Publicly funded tertiary care center. Methods Patients surgically treated for PSCT between 2005–2020 were identified and included. No patients were excluded or lost to follow-up. Medical records and imaging data were retrospectively reviewed. Results Seventeen patients were included. Median age was 52 (23–69) years and 7 (41%) were female. PSCT was diagnosed at a median of 5.0 (0.6–27) years after the initial trauma. Motor deficit was the most common neurological manifestation (71%), followed by sensory deficit (53%), spasticity (53%), pain (41%) and gait disturbance (24%). Median follow-up time was 5.1 (0.7–13) years. Fifteen patients (88%) showed satisfactory results following untethering, defined as improvement or halted progression of one or more of the presenting symptoms. Treatment goals were met for motor symptoms in 92%, sensory loss in 100%, spasticity in 100%, gait disturbance in 100% and pain in 86%. Statistically, a significant improvement in motor deficit ( p = 0.031) and syrinx decrease ( p = 0.004) was also seen. A postoperative complication occurred in four patients: three cases of cerebrospinal fluid leakage and one postoperative hematoma. Two patients showed a negative surgical outcome: 1 with increased neck pain and 1 with left arm weakness following the postoperative hematoma. Conclusion Surgical treatment of PSCT results in improved neurological function or halted neurological deterioration in the vast majority of patients.

Abstract:BACKGROUND: Tethered cord syndrome, is a disease characterized by neurological, gastrointestinal, musculoskeletal, and urinary problems related to spinal cord traction. It may be present alone, although it is commonly a part of undetected spinal dysraphism. A tethered cord release open surgery is the current standard of care management for TCS. TCR is frequently used in pediatric patients, particularly because the earlier intervention is frequently advised for better results. OBJECTIVE: The aim of this retrospective study was to investigates the outcomes and perform audit of the tethered Cord Syndrome release in pediatric population. METHODOLOGY: The records of total of 37 patients who underwent surgical release for tethered cord syndrome from January 2015 to December 2022 were evaluated at Ali Institute of Neurosciences, Irfan General Hospital, Peshawar. Patient demographics, clinical presentations, and outcomes were analyzed. Data were analyzed using descriptive statistics by using SPSS software version 26. RESULTS: Mean age of the participants was 10.5 years. Majority were females 22(59.46%) as compared to males 15 (40.54%). Clinical manifestations encompassed a spectrum of symptoms, including leg weakness (75.7%), foot deformity (48.6%), and trophic ulcers (40.5%). 40.5% of patients experienced a significant enhancement in motor function. CONCLUSION: The results of our study concluded that release of tethered cord syndrome is an effective and safe surgical procedure associated with improved clinical outcomes in terms of symptomatic relief and improvements in motor and sensory functions. Abstract:Objective To explore the operative treatment in the management of cervical spinal cord injury without fracture and dislocation, and the timing of the operation. Methods Twenty-four cases with cervical spinal cord injury without fracture and dislocation were retrospectively analyzed and divided into A group(≤7 days)and B group(7 days) according to the time of surgery . JOA(Japanse Orthopaedic Association) scores at injury and during follow-up were recorded to make a comparison between A group and B group before and after operation. Results The spinal cord function recovered at some extent in those who had been given an operation. The spinal cord functional recovery of A group was better than B group. Conclusions The operative treatment promises better outcome . The shorter the lapse between the injury and operation, the better the effect. As long as the choice of surgical approach and the procedure is correct, the spinal cord function can acquire an optimal recovery both through the anterior and posterior approaches.
Microsurgery for Tethered Cord Syndrome in Children

Abstract:Objective To investigate the therapeutic efficacy of microsurgery on tethered cord syndrome(TCS) in children.Methods 35 cases of TCS in children were analyzed retrospectively.The average age was 2.2 years old.TCS was accompanied by spina bifida in all the cases,by myelomeningocele in 20 cases,by myelomeningocele and lipoma in 13,by teratoma in one and by dermoid cyst in one.Microsurgery was performed to disassemble the adhesive spinal cord and nerve fibers,and to cut off the tethered filum terminale.Results The total effective rate was 80.0% for the patients with paraesthesia in the perineal area,and 77.1% for the patients with urinary dysfunction.The behavioral disturbance was improved in most of patients while combined with orthopedic operations.Conclusion It is crucial to disassemble the medullary cone and cauda equina nerve,and to cut off the tethered filum terminale in the microsurgery for TCS in children.Most symptoms of TCS in children can be improved significantly by microsurgery combined with orthopedic operations.

Abstract:BACKGROUND: Tethered cord syndrome (TCS) secondary to split cord malformation (SCM) is rare in adulthood. There is as yet no consensus about the optimal treatment method for adult patients with SCMs and degenerative spine diseases such as lumbar stenosis, spondylolisthesis and ossification of the ligamentum flavum (OLF). The tethered cord poses a great challenge to the decompression and fusion procedures for the intraoperative stretching of the spinal cord, which might lead to deteriorated neural deficits. Here, we report on a case to add our treatment experience to the medical literature.CASE SUMMARY: We treated a 67-year-old female patient with type II SCM suffering from lumbar disc herniation, degenerative lumbar spondylolisthesis and thoracic OLF. The patient underwent thoracolumbar spinal fusion and decompression surgery for severe lower back pain, extensive left lower limb muscle weakness and intermittent claudication. After the thoracolumbar surgery, without stretching the tethered cord, the patient achieved complete relief of pain and lower extremity weakness at final follow-up.CONCLUSION: For adult patients with underlying TCS secondary to SCM coupled with thoracic OLF and lumbar spondylolisthesis, a thoracolumbar fusion surgery could be safe and effective with the tethered cord untreated. It is critical to design individualized surgical protocols to reduce the stretch of the low-lying spinal cord.

Tethered cord syndrome from pediatric and adult perspectives: a comprehensive systematic review of 6135 cases

Abstract:Objective: This study aimed to investigate the differences in clinical features, diagnostic examination, treatment, and pathological results between adult-onset and pediatric-onset tethered cord syndrome (TCS). Methods: The authors searched the PubMed, Embase, and Cochrane Library databases through January 2023 for reports on TCS, extracting information on clinical features, imaging data, treatment modalities, prognosis, and pathological research results. A total of 6135 cases from 246 articles were included in the analysis. This review was conducted in accordance with the 2020 PRISMA guidelines and registered on PROSPERO. Results: The most common adult clinical manifestations were pain, urinary symptoms, and numbness; in children, they were urinary symptoms, skin lesions, bowel symptoms, and unspecific motor deficits. Surgical treatment was the primary approach for both adults and children, with a higher clinical improvement rate observed in adults. However, adults also had a higher rate of surgical complications than children. TCS pathological studies have not yet identified the differences between adults and children, and the pathogenesis of adult-onset TCS requires further investigation. Conclusions: Adult-onset and pediatric-onset TCS exhibit certain differences in clinical characteristics, diagnostic examinations, and treatments. Abstract:Context: Tethered cord syndrome is associated with motor and sensory deficits. Objective: Our objective was to summarize evidence regarding the diagnosis, prophylactic surgery, symptomatic treatment, and repeat surgery of tethered spinal cord in a systematic review (CRD42023461296). Data sources and study selection: We searched 13 databases, reference-mined reviews, and contacted authors to identify diagnostic accuracy studies and treatment studies published until March 2024. Data extraction: One reviewer abstracted data, and a content expert checked the data for accuracy. We assessed the risk of bias, strength of evidence (SoE), and applicability. Results: The evidence base includes 103 controlled studies, many with risk of bias and applicability concerns, and 355 case series providing additional clinical information. We found moderate SoE for MRI diagnosing tethered spinal cord, with medium to high diagnostic sensitivity and specificity. A small number of prophylactic surgery studies suggested motor function benefits and stability of neurologic status over time, but also complications such as surgical site infection (low SoE). A larger body of evidence documents treatments for symptomatic patients; studies revealed improvement in neurologic status after surgical detethering (low SoE), but also postoperative complications such as cerebrospinal fluid leakage (moderate SoE). A small body of evidence exists for retethering treatment (low or insufficient SoE for all outcomes). Limitations: There was insufficient evidence for key outcomes (eg, over- or undertreatment, clinical impact of diagnostic modalities, ambulation, quality of life). Conclusions: This comprehensive overview informs difficult clinical decisions that parents and their children with tethered spinal cords, as well as their health care providers, face.
Pre-and postoperative changes of urodynamic characteristics of patients with tethered cord syndrome

Abstract:Objective To evaluate the pre-and postoperative changes of urodynamic characteristics in patients with tethered cord syndrome(TCS).Methods A series of 21 patients with TCS surgically treated in our institute were prospectively analyzed from 2006 to 2007.The preoperative and postoperative(2 weeks) urodynamic indexes included bladder volume at the first desire to void,bladder compliance,bladder leak point pressure,maximum flow rate,residual volume,detrusor reflexia were measured and evaluated.Results There were 19 patients (91%) presented with abnormal urodynamic results.Among those 19 cases,14 patients demonstrated with lower motor nerve bladder and 5 higher motor nerve bladder.There were 13 patients with normal bladder compliance,4 lower compliance and 2 higher compliance.There were 3 patients presented with normal detrusor reflexia,8 detrusor areflexia,5 hyperreflexia and 3 patients with hyporeflexia.There was significant improvement of the postoperative bladder residual volume(P＜0.05).Otherwise,there was no significant difference in terms of the volume at the first desire to void,bladder compliance,detrusor reflexia,maximum flow rate and bladder leak point pressure.Conclusion Urodynamic study is an effective method to evaluate neuropathic characteristics of lower urinary tract symptom in patients with TCS.Microsurgical treatment could prevent the symptoms from deterioration.The urodynamic study also could be used to evaluate the symptoms and prognosis of patients with TCS.
New Spinal Shortening Technique for Tethered Cord Syndrome: A Technical Note

Abstract:Background and Objectives: To present a new spinal shortening technique for tethered cord syndrome. Tethered cord syndrome (TCS) is a debilitating condition leading to progressive neurological decline. Surgical detethering for TCS is the gold standard of treatment. However, symptomatic retethering of TCS has been reported in 5%-50% of patients after initial release. To solve this problem, posterior spinal shortening osteotomy has been reported. This technique has risks of massive blood loss and neurological deterioration. The authors hereby report a new safe spinal shortening technique for tethered cord syndrome. Materials and Methods: A 31-year-old man with gait disturbance was referred to our hospital. After the delivery of treatment, he underwent surgical untethering of the spinal cord in another hospital. He had hyperreflexia of the Achilles tendon reflex and bilateral muscle weakness of the legs (MMT 3-4). He also had urinary and bowel incontinence, and total sensory loss below L5. An anteroposterior lumbar radiogram indicated partial laminectomy of L3 and L4. Lumbar MRI showed retethering of spinal cord. Results: The patient underwent a new spinal shortening technique for tethered cord syndrome under the guidance of O-arm navigation. First, from the anterior approach, disectomy from T12 to L3 was performed. Second, from the posterior approach, Ponte osteotomy was performed from T12 to L3, shortening the spinal column by 15 mm. The patient was successfully treated surgically. Postoperative lumbar MRI showed that the tension of the spinal cord was released. Manual muscle testing results and the sensory function of the left leg had recovered almost fully upon final follow-up at one year. Conclusions: A retethered spinal cord after initial untethering is difficult to treat. This new spinal shortening technique can represent another good option to release the tension of the spinal cord.

Abstract:Introduction: Recognition of tethered cord syndrome (TCS) in children is important because as the child grows, the spinal cord is stretched, leading to ischemia and subsequent neurological deficits, including bowel or bladder dysfunction, back and leg pain, or lower extremity weakness. Imaging findings raising concerns for tethering include presence of a fatty and/or thickened filum or a conus medullaris located caudal to L2. Objectives: The objective of this study was to describe the symptomatic presentation of TCS in our institution, detect demographic and diagnostic predictors of signs and symptoms, assess changes in symptoms over time, and examine whether demographic and diagnostic variables affect changes in symptoms over time. Methods: Using a retrospective chart review from patients who underwent detethering at our institution between April 1, 2015 and March 31, 2019, we report on the signs and symptoms of patients undergoing detethering surgery at presentation and examine possible demographic and diagnostic predictors of those symptoms and changes in symptoms over time. Logistic regression analyses were used to determine whether symptoms at presentation were related to demographic and diagnostic predictors and to assess a change in symptoms over time. Results: A total of 273 patients underwent detethering and were analyzed. Of these, 144 (53%) were <5 years of age, 151 (55%) were male, 233 (85%) had a fatty filum, 179 (66%) had a thickened filum, and 106 (39%) had a low-lying conus. Patients <5 years of age were less likely to have urological, gastrointestinal, and neurological or orthopedic symptoms; patients with thickened fila (i.e., greater than 2 mm in diameter regardless of fat-infiltration) were less likely to have urological symptoms; and patients with low-lying coni were less likely to have gastrointestinal symptoms. Patients exhibiting symptoms at presentation had reduced rates of symptoms at the follow-up, but a reduction in rates of symptoms over time was unrelated to demographic or diagnostic variables. Surprisingly, 123 (45%) patients presented with intractable constipation. Conclusion: In the population studied, several presenting symptoms, particularly constipation, were commonly reported. Children <5 years old were less likely to manifest clinically evident neurological/orthopedic, urological, and gastrointestinal symptoms than the older cohorts. Patients were less likely to report symptoms at both their first and second postoperative visit compared to presentation.

Microsurgical management of spina bifida complicated with tethered cord syndrome in adults

Abstract:【Objective】To investigate the method and results of microsurgical management of spina bifida com-plicated with tethered cord syndrome in adults. 【Method】Clinical manifestations, pathological changes, microsur-gical method and results of 21 adult patients with spina bifida complicated by tethered cord syndrome treated in ourdepartment from September 1998 to June 2004, were analyzed retrospectively. 【Results】14 cases (67%) of patientsshowed improvement postoperatively, 13 cases were followed up, in followed up period (0.5 to 2 years), 38% of casesgot further improvement. No serious complications were observed. 【Conclusion】Microsurgical techniques could sig-nificantly improve the outcomes of spina bifida complicated with tethered cord syndrome in adults, and positive man-agement is needed for adult patients, and the earlier the treatment, the better the outcomes.
[Operative complications in tethered cord syndrome and their management].

Abstract:OBJECTIVES:To find out the common complications induced by the operation on patients with tethered cord syndrome (TCS) and to discuss the mechanism and the treatment of these complications. METHODS:During 1993 and 2001, the spinal cords of 87 patients with TCS were explored and released operatively. The operative complications were analyzed retrospectively and the results of prevention and treatment of these complications were evaluated. RESULTS:The operative complications in patients with TCS included headache (43.7%), hemorrhage (31.0%), lumbago (21.8%), retethering (18.4%), lumbar instability (12.7%), neural injury (8.1%) and CSF leakage (4.6%). CONCLUSIONS:To reduce operative complications, the pathology of TCS should be investigated thoroughly before operation. The techniques of microsurgery and electrophysiology should be used properly and some complications were reversible after treatment. Post-operation rehabilitation training should be stressed.
Disparities in Indications and Outcomes Reporting for Spinal Column Shortening for Tethered Cord Syndrome

Abstract:Study Design. Systematic review. Objective. To identify commonly reported indications and outcomes in spinal column shortening (SCS) procedures. Summary of Background Data. SCS is a surgical procedure used in patients with tethered cord syndrome (TCS)—characterized by abnormal attachment of neural components to surrounding tissues—to shorten the vertebral column, release tension on the spinal cord/neural elements, and alleviate associated symptoms. Methods. PubMed and EMBASE searches captured SCS literature published between 1950 and 2023. Prospective/retrospective cohort studies and case series were included without age limit or required follow-up period. Review articles without new patient presentations, meta-analyses, systematic reviews, conference abstracts, and letters were excluded. Studies included adult and pediatric patients. Results. The 29 identified studies represented 278 patients (age 5–76 y). In 24.1% of studies, patients underwent primary TCS intervention via SCS. In 41.4% of studies, patients underwent SCS after failed previous primary detethering (24.1% of studies were mixed and 10.3% were unspecified). The most commonly reported non-genitourinary/bowel surgical indications were back pain (55.2%), lower-extremity pain (48.3%), lower-extremity weakness (48.3%), lower-extremity numbness (34.5%), and lower-extremity motor dysfunction (34.5%). Genitourinary/bowel symptoms were most often described as nonspecific bladder dysfunction (58.6%), bladder incontinence (34.5%), and bowel dysfunction (31.0%). After SCS, non-genitourinary/bowel outcomes included lower-extremity pain (44.8%), back pain (31.0%), and lower-extremity sensory and motor function (both 31.0%). Bladder dysfunction (79.3%), bowel dysfunction (34.5%), and bladder incontinence (13.8%) were commonly reported genitourinary/bowel outcomes. In total, 40 presenting surgical indication categories and 33 unique outcome measures were reported across studies. Seventeen of the 278 patients (6.1%) experienced a complication. Conclusion. The SCS surgical literature displays variability in operative indications and postoperative outcomes. The lack of common reporting mechanisms impedes higher-level analysis. A standardized outcomes measurement tool, encompassing both patient-reported outcome measures and objective metrics, is necessary. Level of Evidence. Level 4

Abstract:Objective To summarize the clinical manifestations, imaging characteristics and experience of surgical treatment of spinal cord terminal syringomyelia with tethered cord syndrome (TCS). Methods and Results Clinical data of 10 patients with spinal cord syringomyelia combined with TCS surgically treated under microscope from January 1999 to March 2014 in our hospital were retrospectively analyzed. There were 3 males and 7 females with average age of 15.06 years old (ranged from 2 to 35 years old). The course of disease ranged from 3 months to 20 years (average 42.17 months). Among those patients, one patient presented hydromyelia, 8 patients suffered from meningeal cyst within the sacral canal, and one patient were concurrent with sacral dermal sinus. The weakness of lower extremities, especially distal limbs, was the main clinical manifestation. Five patients were accompanied with bowel and bladder dysfunction and 5 patients with sensory disturbance below the level of syringomyelia, especially hypesthesia. Preoperative MRI showed conus medullaris disappeared at the end of spinal cord, and there was fluid signal in the lower spinal cord with hypo-intensity signal in T1WI and hyper-intensity signal in T2WI without enhancement. All patients underwent surgical procedures. Under microscope, filum terminale was cut off, drainage was performed, meningeal cyst within the sacral canal was removed, and tethered cord was released. The success rate of operations was 100%. The duration of surgery ranged from 1.52 to 3.07 h (average 2.15 h), with average intraoperative blood loss 220 ml (ranged from 100 to 410 ml). The tethering filum had been totally resected and histological examination showed typical filum tissue in all cases. No operative complication was found. Visual Analogue Scale (VAS) score was decreased, and the lower limbs weakness as well as bowel and bladder dysfunction was gradually relieved after operation. The period of follow-up was ranged from 6 months to 14.50 years (average 6.10 years). All patients presented neurological intactness (McCormick gradeⅠ), recovery of conus medullaris, rising of the end of spinal cord, no recurrence of syringomyelia with favorable alignment of spine during follow- up period. Conclusions Terminal syringomyelia in the spinal cord with TCS is rarely occurred and characterized by chronic dysfunction of spinal cord and nerve. The manifestation in MRI imaging is characterized by low level of spinal cord, disappearing of conus medullaris, long T1 and long T2 signal of filum terminale and spinal cord, and no enhancement of capsule wall. The surgical treatment includes resection of filum terminale, drainage of the cerebrospinal fluid (CSF) in syringomyelia, and de-tethering of the spinal cord under microscope. Analysis on clinical manifestation between adolescent and adult patients with tethered cord syndrome

Abstract:Objective According to analysis on difference clinical manifestation between adolescent and adult patients with tethered cord syndrome,the process from generation to progression in the disease was inquired into.According to evaluation on nervous disorders from the disease,clinical characters and prognosis significance for the disease were judged and mastered.Methods 142 patients with tethered cord syndrome were grouped into adolescent(less than 14-year-old) and adult(more than 15-year-old) in our study.The average internal period from initial symptoms to operation was 6 years.The disorder of urine,deform of lower legs and foots,weakness of bilateral lower extremities,sensation dysfunction were compared with between two groups.The X-ray,MRI scan,urine dynamics,remained urine of bladder,and monitor of electrophysiology on time were compared with between preoperative and postoperative,and were evaluated long-time follow-up.Results In adolescent group,there are 16(59.3%) cases obtained improvement after operation in 27 cases suffered urinary dysfunction.The deform of legs and foots was interrupted in 22(43.1%) cases from 29 suffered the progressing deformation.The muscle strength was improved in 31(81.5%) cases postoperative from 38 cases suffered legs weakness.The improvement of sensation was obtained in 39(82.9%) cases from 47 cases suffered sensation dysfunction.In adult group,there are 52(62.7%) cases obtained improvement after operation in 83 cases suffered urinary dysfunction.The muscle strength was improved in 56(72.7%) cases postoperative from 77 cases suffered legs weakness.The improvement of sensation was obtained in 43(76.9%) cases from 56 cases suffered sensation dysfunction.Conclusions In urine dysfunction,the improving rate in adult was higher than in adolescent.In muscle strength,deform of legs and foots,and sensation dysfunction,the improving rate in adolescent was higher than in adult.In additional,intra-operative electrophysiological monitoring prevented the nervous from injury,the satisfied outcome and prognosis were obtained by open operation.

Abstract:Purpose: Tethered cord syndrome (TCS) is characterized by abnormal attachment of the spinal cord neural elements to surrounding tissues. The most common symptoms include pain, motor or sensory dysfunction, and urologic deficits. Although TCS is common in children, there is a significant heterogeneity in outcomes reporting. We systematically reviewed surgical indications and postoperative outcomes to assess the need for a grading/classification system. Methods: PubMed and EMBASE searches identified pediatric TCS literature published between 1950 and 2023. Studies reporting surgical interventions, ≥ 6-month follow-up, and ≥ 5 patients were included. Results: Fifty-five studies representing 3798 patients were included. The most commonly reported non-urologic symptoms were nonspecific lower-extremity motor disturbances (36.4% of studies), lower-extremity/back pain (32.7%), nonspecific lower-extremity sensory disturbances (29.1%), gait abnormalities (29.1%), and nonspecific bowel dysfunction/fecal incontinence (25.5%). Urologic symptoms were most commonly reported as nonspecific complaints (40.0%). After detethering surgery, retethering was the most widely reported non-urologic outcome (40.0%), followed by other nonspecific findings: motor deficits (32.7%), lower-extremity/back/perianal pain (18.2%), gait/ambulation function (18.2%), sensory deficits (12.7%), and bowel deficits/fecal incontinence (12.7%). Commonly reported urologic outcomes included nonspecific bladder/urinary deficits (27.3%), bladder capacity (20.0%), bladder compliance (18.2%), urinary incontinence/enuresis/neurogenic bladder (18.2%), and nonspecific urodynamics/urodynamics score change (16.4%). Conclusion: TCS surgical literature is highly variable regarding surgical indications and reporting of postsurgical outcomes. The lack of common data elements and consistent quantitative measures inhibits higher-level analysis. The development and validation of a standardized outcomes measurement tool-ideally encompassing both patient-reported outcome and objective measures-would significantly benefit future TCS research and surgical management.

Abstract:Tethered cord syndrome is a condition in which the spinal cord is tethered by pathological structures such as a tight filum terminale, intradural lipomas with or without a connecting extradural component, intradural fibrous adhesions, diastematomyelia, and neural placode adhesions following closure of a myelomeningocele.It usually occurs in childhood and adolescence as the spine grows in length, but it can also develop in adulthood. Symptoms of tethered cord syndrome are slowly progressive and varied. Incorrect diagnosis and inappropriate treatment may be provided if the physician lacks knowledge and understanding of this disease.This chapter aims to describe the pathophysiology, syndromes, diagnostic imaging, surgical treatment, and prognosis of tethered cord syndrome to enhance the understanding of this condition.
