Abstract:About 65.5 % of the Indians live in rural areas while availability of healthcare is biased towards urban setups. Medicine in undergraduates is taught in ivory towers of medical colleges, a paradigm shift in teaching bedside medicine from these ivory towers to community level is required. Hence a new competency-based medical education for undergraduate course was adopted with family adoption program as a vital component, which provides an opportunity for undergraduates to get sensitised to problems and social dynamics at grass root level. We propose an integrated holistic health team model to implement family adoption program so as to address needs of community and build consensus amongst graduate students from various health disciplines to work in integrated team approach and result in enhanced learning. A five pronged strategy is implemented by forming an integrated team comprising of students from medical, aental, ayurveda, and physiotherapy & nursing streams. The institutions that have undergraduate program for any two to five of these disciplines can adopt proposed approach. Expected outcomes of utilising this model is to create holistic health team thereby inculcating in budding graduates team spirit, mutual cooperation & respect for other disciplines of health sciences and thereby holistic healthcare to community.

Abstract:In the field of delivery of medical education, social accountability envisages the responsibility of medical and healthcare institutions to meet the health-related needs of the communities in the catchment areas. The medical colleges that have integrated social accountability in their curriculum work with a goal to produce not only knowledgeable and skilled medical graduates but also those who are committed to the well-being of the entire community. The process of implementation of social accountability in medical education can have its own share of challenges. There is an immense need to plan and implement potential solutions for each of the identified challenges to ensure maximum benefit to the students, teachers, and community at large. In conclusion, social accountability in medical education has been associated with multiple benefits to all the involved stakeholders. This calls for the need to identify the challenges that can play their part in the successful implementation in medical schools and employment of appropriate measures to overcome them and move thereby bringing about a decline in health disparity.
Providing Adaptation Solutions to the Problems Faced by Adoptive Families

Abstract:Background: Adoption is frequently viewed as a way to complete the family because of the infertility that some families experience and the desire for kids and teenagers, especially orphans, to have a loving family. Aims: This work intends to identify and propose adaptation solutions to address the psychological problems faced by adoptive families. By doing so, it is hoped that the mental health and overall well-being of individuals and society as a whole will be enhanced. Materials and methods: In the first stage of this study, the grounded theory approach based on Strauss and Corbin's methodology was used to interview representatives of adoptive families in Iran in 2022 and identify the problems they experience. In the second stage, the same approach and methodology were used to interview representatives of experts. In the second stage, the results of the first stage were presented to a sample of purposefully selected experts, who subsequently proposed solutions for the families to adapt to their problems. Data analysis was conducted using MAXQDA 2020 software (VERBI Software, Berlin, Germany). Ethical requirements were followed at every stage of the study. Results: Four problem categories were identified in the first step of the study: legal-psychological problems, problems related to the growth of the child in an environment of social harm, attitude and worldview, and the resilience of the adoptive couple. In the second step of the study, four groups of solutions for psychological adjustment were extracted: explaining the issue of what, how, and why of adoption (correct identification of the problems faced by adoptive families, clarification of concepts and demystification, the need for research to address other gaps in the knowledge infrastructure, and the importance of critical thinking education), operational solutions (supervising the adoption database to become more efficient, unitization of adoption national institutes in each province, selection of officials based on both factors of professional and complete mental health, alignment of the provisions and other related legal matters between the involved institutions, and utilization of media influence are recommended, interactional solutions for family resilience (cognitive-therapeutic interactions for mental health based on flexibility, raising the family's social capacity through active counseling, improving problem-solving skills, fostering family self-efficacy, creating purposefulness, and appropriate beliefs to predict a bright future), attitude-changing solutions (redefining social norms and facilitating the adoption of desired attitudes by employing social psychology principles, exploiting the power of media and influential figures, employing techniques derived from the psychology of learning, establishing conducive conditions within the framework of individuals' cognitive dissonance to modify their attitudes, and employing persuasion strategies). Conclusion: Legislators and law enforcers, adoptive families, psychologists and consultants working in this field, and physicians specializing in infertility treatment can benefit from the findings of this study.

Abstract:Introduction While mentoring students during regular medical education has a long-standing tradition in many developed countries' medical schools, it has yet to become a standard practice in the majority of medical institutions, especially in the developing world, such as India. In institutions where mentoring programs are sparsely implemented, there is a lack of data regarding their assessment. Methodology This qualitative study involved two groups of students - nine undergraduate medical students (five male and four female) and 10 undergraduate medical students (six male and four female) who had at least three years of experience in the existing mentorship program at a tertiary care teaching hospital. We conducted two focused group discussions (FGDs) with these two groups of students using a guide, with FGDs lasting 45 and 50 minutes, respectively. We recorded the audio and it was transcripted to text. Thematic analysis of the transcripts from the 2 FGDs was conducted using Atlasti (Version 7.1.8) software to assess perceptions of the mentorship program. Results The content analysis of the discussions revealed two broad themes, namely "Current Functioning of the Programme" and "Suggestions for Improvement." These themes were further divided into multiple domains and subdomains, providing a comprehensive overview of the study's findings. Although there is a consensus among students that the mentorship program is essential, the current operational framework still has limited confidence due to biases, fears, and misinformation among the students. Conclusion The ongoing medical curriculum imparts a vast amount of scientific knowledge within a limited timeframe, with practical application occurring primarily in the last three years of the academic curriculum and minimal emphasis on ethical practice, professionalism, effective communication, handling urgent health situations, and interacting with family members, underscores the genuine need for a structured mentorship curriculum for undergraduate medical students. To enhance the program's effectiveness, the active involvement of undergraduate students must address their specific needs.

Abstract:Abstract Background Intra-regional cultural and linguistic differences are common in low- and middle-income countries. To sensitise undergraduate medical students to the social and contextual determinants of health to achieve the ‘health for all’ goal, these countries must focus on innovative teaching methods. The early introduction of a Community Orientation Program (COP) as a Community-based Medical Education (CBME) method could be a game changing strategy. In this paper the methods, evaluation, and implication of the COP in an Indian setting are described. Methods The curriculum of the COP was developed based on the analysis, design, development, implementation, and evaluation (ADDIE) model for educational intervention. In this learner-centric and supervised educational program, the key aim was to focus on developing students’ communication skills, observation power and enhancing their motivation for learning through collaborative learning. To meet the objectives of the COP, a situated learning model under the constructivism theory was adopted. Results Between 2016 and 2019, 557 students were trained through the COP by visiting more than 1300 households in ten villages. To supplement the students’ observations in the community, more than 150 small group discussions, a health education programme for the community and summary presentations were conducted. The students’ feedback indicated the need to improve the clinical examinations demonstration quality and increase the number of instruments for clinical examinations. More than 80% of students felt that the program would assist them to improve their communication skills, their understanding of the various socio-demographic factors associated with the common diseases, and it will enable them to respect the local culture during their clinical practice. Conclusions Early initiation of the COP as a CBME method in the undergraduate medical curriculum in an Indian setting has shown promising results. Further evidence is required to adopt such a program routinely for under-graduate medical teaching in the low- and middle- income settings.

Abstract:Background Maintaining the quality of teaching across India is a challenge. Teachers are equally responsible for patient care and administration. The importance of training medical teachers under the various faculty development programs (FDPs) is well accepted. A mechanism to evaluate the competencies acquired by medical teachers after attending FDPs becomes equally important. In the present study, we evaluate the impact of the various FDPs on medical teachers. Methodology A cross-sectional study was conducted for one year among 50 medical teachers attending FDPs. Ethical clearance was taken from the Institutional Ethical Committee. For quantitative data collection, the questionnaire was validated by the Scientific Approval Committee of the Institute. The study questionnaire was filled in by the participants just before and three months after attending FDPs. For qualitative data collection, in-depth Interviews (IDIs) using the Kirkpatrick model of learning evaluation were conducted. Descriptive statistics were presented as frequencies and percentages. The thematic areas of self-efficacy and teaching competency before and after FDPs were tested using the chi-square test. P-values <0.05 were considered significant. Results There was a significant increase in self-efficacy (300 vs. 426, p = <0.0001) and teaching competency (456 vs. 608, p = <0.0001) in the domains of teaching difficult students and motivating students for innovative projects. Improvement in communication skills and ability to engage the students were noteworthy in teaching competency. IDIs revealed that FDPs are essential for the efficient delivery of the competency-based medical curriculum. Conclusions FDPs play a key role in bringing about significant improvement in generating self-efficacy and teaching competencies among medical teachers. FDPs may be incorporated into the postgraduate medical curriculum itself.

Abstract:Introduction Women's sexual and reproductive health depends on their autonomy to decide, freedom to choose free from coercion, right to education and information, right to dignity, and equity in access to sexual and reproductive health services. These services, especially regarding family planning and abortion services, must be strengthened in the Indian medical schools. Hence, implementation research with baseline and endline comparison was conducted to strengthen reproductive rights and evidence-based practices related to family planning and abortion care services in these schools. This study presents the findings of baseline assessment. Objective To determine the status of reproductive rights and evidence-based family planning and abortion services in Indian medical schools Methods A concurrent mixed methods study design was used to conduct the baseline assessment in nine medical schools in Rajasthan, Gujarat, and Union territory in India from October 2018 to June 2019. Reproductive health services provided to 104 women during counseling for family planning (N=85) and abortion (N=19) were observed quantitatively using a predesigned, pre-tested checklist. In-depth interviews of 33 faculty members from the Department of Obstetrics and Gynecology were conducted to qualitatively explore their beliefs and practices regarding a rights-based approach to providing reproductive healthcare services. The Capability, Opportunity, and Motivation model of behaviors (COM-B) was used to qualitatively identify barriers and facilitators of reproductive rights while providing family planning and abortion services in medical schools. Results Family planning counseling services with a cafeteria approach were observed in 69.4% of cases. Among these, 7.1% were provided with couple counseling services. Auditory privacy could be provided to 22.2% of the family planning counseling sessions. The choice of three or more contraceptives was offered to 76.5% nullipara, 54.2% para one, and 75.6% multipara women. The average duration of counseling was 7.4 minutes. Counseling on family planning and abortion was provided to 31.6% of clients. For 36.8% of clients, sterilization/IUD insertion was kept as a pre-condition for abortion. Medical schools lacked space, staffing, and logistics to provide rights-based reproductive health services. Social barriers like the poor socio-economic status of the clients, lack of education and awareness of women's rights, less autonomy in decision-making, and social and gender bias compromised women's reproductive rights. As per the COM-B model, the reproductive rights-based approach facilitators included well-informed faculty regarding dignified and respectful care, ensuring total privacy and confidentiality. However, infrastructure gaps, limited space, high workload, insufficient human resources, and lack of standard operating procedures often compromise privacy and confidentiality during counseling. Conclusions Right-based reproductive services around family planning counseling and abortion services were delivered partially despite the medical schools' trained faculty. This was mainly due to limited workspace, high workload, less autonomy, and lower awareness of reproductive rights among women. Disclosure No.

Abstract:The Family Community-based Assistance Resourcing and Education Program (FCP) is a nurse home visiting program that was introduced in Queensland two decades ago to redress health inequalities for infants from families experiencing specific social stressors. Locally adapted versions of this home visiting program are still in use, but have not been evaluated. This study examined child health nurse perceptions of the adapted FCP in one regional Queensland health service. A qualitative descriptive exploratory study using two focus groups (conducted May 2019) with Child Health Nurses who delivered the FCP was conducted. Transcripts of digital recordings were analysed using Braun and Clarke's (2006) six-step framework for guided thematic analysis. A total of 16 Child Health Nurses participated in the study, with a mean of 10 years' experience with the program. Data analysis generated 12 themes organised under three domains: 'Establishing the relationship with families', 'What works in practice' and 'We could do it better'. Participants cited flexibility, expert input and in-home delivery as key program benefits. However, narrow eligibility criteria, poor screening for perinatal anxiety and resourcing constraints were identified as limitations. This study is the first to measure Child Health Nurses' perceptions of an adapted FCP. It sheds light on their 'practice wisdom', including the program's ability to meet the needs of families with social vulnerabilities. The study supports prior calls for home visiting programs to be evaluated against clearly stated program intentions. Participant insights have been shared to inform practice and program implementation both locally and as part of Queensland's First 2000 Days health service delivery reform agenda.

Abstract:Background Improving social determinants of health, such as access to nutritious food, is crucial for achieving health equity. Nutrition insecurity, especially during pregnancy and postpartum, can lead to poor maternal and birth outcomes. Food is medicine (FIM) programs, which integrate food into the healthcare system to prevent or manage disease, have the potential to improve nutrition insecurity, but research about perinatal FIM programs is limited. Objective The purpose of this study was to explore 1) perceptions of public health impacts of perinatal food is medicine programs from the perspectives of both program implementers and program supporters and 2) implementation strategies used to enhance program adoption, implementation, and maintenance. Design Qualitative data were collected through semi-structured interviews. The interview guide was based on the reach, effectiveness, adoption, implementation, maintenance (RE-AIM) framework. Participants /setting: Program implementers (n=16) and program supporters (n=20) were recruited across the United States through purposive sampling in 2022-2023. Analysis Data were analyzed using deductive thematic analysis and an iterative feedback loop with the project partner. Results Interviews were completed with program implementers and program supporters and generated meaning units (N=1942) that were coded into themes aligned with each RE-AIM dimension. Perinatal FIM programs reached multiple priority populations who were mainly recruited through healthcare systems. Effectiveness measures typically included nutrition patterns and practices, as well as return on investment. Motivations for adopting programs primarily included partnerships and connections, financing, and policies/laws. Program components varied and were adapted to meet participants and setting needs. Policy, evidence, funding, and partnerships could lead to program maintenance. Implementation strategies applied by the program supporters included financial strategies and infrastructure changes. Conclusions There is a need to identify the core functions and adaptable forms of perinatal FIM programs, which could lead to identification of standard evaluation metrics. This could result in greater uptake by potential delivery agents, increased funding and policy support, and enhanced benefits for perinatal population experiencing health disparities.

Abstract:Background: The milieu of maternal health and family planning services always presents challenges in Indian communities. The situation is relatively more challenging in the rural parts of the country. The challenges are a mix of complex issues encompassing the health system and community challenges. Objectives: The basic objective of this study was to understand the challenges encountered by different cadres of health workers in providing maternity management and family planning services. Settings and Design: Different cadres of health care providers, that is, obstetrics and gynecology (O and G) specialists, medical officers (MOs), rural health practitioners (RHPs), and nurses working at the district and block levels were selected conveniently with the permission of State health administration.Materials and Methods: A qualitative pilot study was conducted in two districts of Assam. Participants were interviewed in-depth with the help of a semistructured interview schedule, which lasted for 26 days during May–July 2014. All the quantitative as well as qualitative data were entered and analyzed using Microsoft Excel. Results: It is evident from our observation that the set of challenges varies with the service provider as well as the category of services in maternity management and family planning. Lack of medicine, lack of manpower, and community dynamics are perceived to be the greatest challenges in providing services among three or six cadres of health care providers. In addition, poor patient compliance, lack of infrastructure, caseload, and inadequate training are also perceived to be major challenges. Conclusion: Many of these challenges are in fact obvious and articulated by many researchers, but it is what needs to be done to overcome these problems that is the most important issue.
A cross-sectional survey on medical education needs of general practitioners and family medicine: Delhi, Himachal Pradesh and Tamil Nadu, India

Abstract:Background: In India, most physicians pursue a specialist's degree resulting in a dearth of general practitioners (GPs). To provide primary care across populations and support universal healthcare coverage (UHC), there is a need to develop a core educational foundation for generalists and family medicine in undergraduate and post-graduate training. Methodology: A cross-sectional survey was conducted as a part of a medical educational needs assessment (ENA) with a focus on family medicine. Respondents included practicing physicians, residents and medical students in Himachal Pradesh, Delhi and Tamil Nadu. Descriptive and bi-variate data analysis (Pearson's Chi square, independent t-tests and analysis of variance [ANOVA]) was performed to summarise data and determine significant differences between demographic groups of respondents. Results: Three hundred and sixty-one surveys were completed. From which, 80.7% (284) of respondents felt that family medicine would be beneficial/very beneficial to the Indian health system. Respondents were split over whether family medicine programmes should be integrated within the existing bachelor of medicine and bachelor of surgery (MBBS) programmes (149/42.5%) or created as a separate post-graduate level specialty (131/37.3%). Overall, 84.2% (292) and 85.4% (294) agreed/strongly agreed that family medicine would benefit specialists and decrease the health disparities. Challenges include lack of information about family medicine and patients' use of specialists for primary healthcare needs. Conclusions: There was a positive response to expanding education for generalists and development of family medicine as a specialty in India. Mechanisms to support policies and programmes need to be further explored to ensure successful implementation across the country. Interest in skills-based courses can be an opportunity to provide GP and family medicine training while broader system-level changes are considered.

Abstract:Background. Schools are important settings for increasing reach and uptake of adolescent mental health interventions. There is limited consensus on the focus and content of school-based mental health services (SBMHSs), particularly in low-resource settings. This study elicited the views of diverse stakeholders in two urban settings in India about their priorities and preferences for SBMHSs. Methods. We completed semi-structured interviews and focus group discussions with adolescents ( n = 191), parents ( n = 9), teachers ( n = 78), school counsellors ( n = 15), clinical psychologists/psychiatrists ( n = 7) in two urban sites in India (Delhi and Goa). Qualitative data were obtained on prioritized outcomes, preferred content and delivery methods, and indicated barriers. Results. All stakeholders indicated the need for and acceptability of SBMHSs. Adolescents prioritized resolution of life problems and exhibited a preference for practical guidance. Parents and teachers emphasized functional outcomes and preferred to be involved in interventions. In contrast, adolescents' favored limited involvement from parents and teachers, was related to widespread concerns about confidentiality. Face-to-face counselling was deemed to be the most acceptable delivery format; self-help was less frequently endorsed but was relatively more acceptable if blended with guidance or delivered using digital technology. Structured sensitization was recommended to promote adolescent's engagement. Providers endorsed a stepped care approach to address different levels of mental health need among adolescents. Conclusion. SBMHSs are desired by adolescents and adult stakeholders in this setting where few such services exist. Sensitization activities are required to support implementation. School counsellors have an important role in identifying and treating adolescents with different levels of mental health needs, and a suite of interventions is needed to target these needs effectively and efficiently.

Abstract:Background: Competency-based medical education (CBME) curriculum in India has introduced many new concepts like a foundation course, early clinical exposure, and self-directed learning (SDL). Sometimes SDL simply means self-study. Self-directed learning as defined by Knowles is a process in which individuals take the initiative with or without the help of others in diagnosing their learning needs, setting their own learning goals, identifying appropriate learning resources, and selecting appropriate learning strategies. SDL is seen as a prerequisite for life-long learners, especially medical graduates. We found poor uptake of SDL sessions in terms of learning and attendance by students. To develop and assess the effect of the SDL module in Community Medicine for Phase -3 MBBS students.Materials and Methods: The study design was a program development and evaluation design. The program development consists of free listing and Nominal Group Technique (NGT). The evaluation design consists of a formative assessment, an end-of-module assessment, and feedback from undergraduate students, postgraduates, and faculties. Data collection procedure: SDL module was developed, agreed and implemented among undergraduates of Phase - 3 MBBS students.Results: Free listing was conducted among undergraduate students who had completed the phase 3 MBBS examination and Nominal Group Technique was conducted among the faculties (n = 7) and Postgraduates of the Department of Community Medicine (n = 2) to explore the appropriate topics for SDL in Community Medicine. The topic with the highest ranking and which was finalized for preparation of the SDL module was "National Health Programme". Three fourth 118 (75%) of the students scored ≥50% at the end of the module assessment. Manual content analysis for the feedback was categorized into three themes such as facilitating factors, challenges, and solutions.Conclusions: Effective implementation and assessment of SDL sessions are one of the new concepts in the CBME curriculum.

Abstract:Introduction: This article explores the potential challenges in implementing a national-level standardized curriculum for physician associate (PA) programs in India, an initiative under the directions of the National Commission for Allied and Healthcare Professionals Act of 2021. Methods: Initiated in August 2023, a multistage process began with emailing a draft curriculum for review to key stakeholders, followed by a virtual meeting on September 3. Key stakeholders, including faculty from various universities, participated in focus group discussions (FGDs) at the International Association of Physician Associate Educators and the Indian Association of Physician Assistants annual conference on September 21, 2023, in Gandhinagar. Results: Four major themes emerged: (1) faculty development, (2) faculty recruitment, (3) alignment with the National Educational Policy, and (4) the necessity of master's degree programs for PAs. Challenges identified included shortage of qualified faculty, uncompetitive salaries for academic roles, and low enrollment in master's programs. Recommendations included increasing financial compensation for teaching PAs, developing more PA educators through additional master's programs, encouraging hospitals to mandate clinical and teaching roles for recruits, and introducing pedagogical training for PAs interested in teaching. Discussion: Implementing a standardized curriculum for PAs faces significant challenges, especially in faculty recruitment and retention. Engaging a broader range of stakeholders is essential for the successful rollout of the curriculum and for meeting India's evolving health care needs.

Abstract:The effect of the polarization state on electro-optic coupling is studied by using the wave coupling theory of the linear electro-optic effect. The numerical results show that the polarization state obviously influences the electro-optic coupling. The conditions for realizing perfect coupling are emphasized. As an application of perfect coupling, a novel polarization rotator, which can rotate the polarization of a light beam with an arbitrary angle but keep the output intensity unchanged, is presented.

Abstract:Background Competency-Based Medical Education (CBME) is now mandated by many graduate and undergraduate accreditation standards. Evaluating CBME is essential for quantifying its impact, finding supporting evidence for the efforts invested in accreditation processes, and determining future steps. The Ambulatory Healthcare Services (AHS) family medicine residency program has been accredited by the Accreditation Council of Graduate Medical Education-International (ACGME-I) since 2013. This study aims to report the Abu Dhabi program’s experience in implementing CBME and accreditation. Objectives Compare the two residents’ cohorts’ performance pre-and post-ACGME-I accreditation. Study the bi-annually reported milestones as a graduating residents’ performance prognostic tool. Methods All residents in the program from 2008 to 2019 were included. They are called Cohort one—the intake from 2008 to 2012, before the ACGME accreditation, and Cohort two—the intake from 2013 to 2019, after the ACGME accreditation, with the milestones used. The mandatory annual in-training exam was used as an indication of the change in competency between the two cohorts. Among Cohort two ACGME-I, the biannual milestones data were studied to find the correlation between residents’ early and graduating milestones. Results A total of 112 residents were included: 36 in Cohort one and 76 in Cohort two. In Cohort one, before the ACGME accreditation, no significant associations were identified between residents’ graduation in-training exam and their early performance indicators, while in Cohort two, there were significant correlations between almost all performance metrics. Early milestones are correlated with the graduation in-training exam score. Linear regression confirmed this relationship after controlling the residents’ undergraduate Grade Point Average (GPA). Competency development continues to improve even after residents complete training at Post Graduate Year, PGY4, as residents’ achievement in PGY5 continues to improve. Conclusion Improved achievement of residents after the introduction of the ACGME-I accreditation is evident. Additionally, the correlation between the graduation in-training exam and graduation milestones, with earlier milestones, suggests a possible use of early milestones in predicting outcomes.

Abstract:Background: Family Integrated Care (FICare) integrates parents as partners in neonatal intensive care unit care. Our team adapted and implemented this approach in a Ugandan unit for hospitalized neonates. Purpose: This qualitative descriptive study examined the perceptions of mothers and healthcare professionals (HCPs) of the benefits and challenges of this new approach to care. Methods: Fifty-one mothers of hospitalized neonates born weighing greater than 2000 g participated in the program. They were taught to assess neonate danger signs, feeding, and weight. After discharge, a subsample (n = 15) participated in focus groups to explore benefits and challenges of their participation in care. Interviews with 8 HCPs were also conducted for the same purpose. Transcripts from focus groups and interviews were analyzed using inductive content analysis to describe the benefits and challenges from the perspectives of mothers and HCPs. Results: For mothers a benefit was decreased stress. Both mothers and HCPs reported that the knowledge and skills mothers acquired were a benefit as was their ability to apply these to the care of their neonate. Improved relations between mothers and HCPs were described, characterized by greater exchange of information and HCPs’ attentiveness to mothers’ assessments. Mothers felt ready for discharge and used their knowledge at home. HCPs noted a decrease in their workload. Challenges included the need for mothers to overcome fears about performing the tasks, their own well-being and literacy skills, and access to equipment. Implications for Practice: Mothers’ participation in their neonates’ care can have benefits for them and their neonate.
