Abstract:AIM To report the safety, efficacy, and accuracy of small-incision lenticule extraction (SMILE) or femtosecond-assisted laser in situ keratomileusis (FS-LASIK) for the correction of myopia or myopic astigmatism in patients with deep corneal opacity denoted by anterior segment optical coherence tomography (AS-OCT). METHODS Four patients with monocular corneal opacity (3 due to mechanical injury, 1 due to a firecracker wound) were recruited and treated with refractive surgery (3 for SMILE, 1 for FS-LASIK combined with limbal relaxing incision (LRI). Preoperative ocular manifestations, surgical details, postoperative visual outcomes, corneal opacity parameters, and corneal topography were analyzed. RESULTS Preoperatively, spherical diopter ranged from -3.0 D to -4.75 D with cylinder ranging from -0.75 to -5.0 D, and corrected distance visual acuity (CDVA) ranging from 20/25 to 20/20. One eye's corneal opacity was located in the central zone and three were in the mid-peripheral optical zone. Three patients underwent uneventful SMILE in both eyes, whilst one patient underwent FS-LASIK for high astigmatism in both eyes and LRI in the right eye. CDVA of the eye with corneal opacity ranged from 20/22 to 20/20 one to six weeks postoperatively. Two patients achieved better CDVA and no patients lost Snellen lines. The postoperative diopter was within ±0.75 D for all eyes. Significant edema existed above the corneal opacity in one eye and dissipated soon. No eccentric corneal topography or morphological anomaly of the corneal cap or flap was observed. CONCLUSION The cases demonstrate that SMILE or FS-LASIK is safe and effective to treat myopic astigmatism combined with deep corneal opacity lesions after comprehensive preoperative evaluation and appropriate candidate selection. FS-LASIK combined with LRI is also sufficient for correcting high astigmatism due to corneal scarring.
Long-term observation of monovision surgery using femtosecond laser- assisted laser in situ keratomileusis for myopia correction in early presbyopia

Abstract:Abstract Purpose: This study aimed to investigate the long-term safety and efficacy of femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in patients with myopia with presbyopia. Methods: This case series study included 30 eyes of 15 patients who underwent monovision designed FS-AISK for myopia correction with presbyopia (male/female, 8/7; age, 49.20±7.05 years; spherical equivalent [SE], –6.97±1.82 D). Parameters including SE; uncorrected distal visual acuity (VA); corrected distal VA; intraocular pressure; presbyopic add power; VA of dominant eyes (D-eye), non-dominant (nD-eye) eyes, and both eyes (Bi) at 0.4 m, 0.8 m, and 5 m; wavefront aberration; and contrast sensitivity (CS) were recorded. Follow-up was performed for 2.5 years. Differences were considered statistically significant at p<0.05. Results: The safety indicesand efficacy indices were 1.08±0.19 and 0.77±0.29 at 2.5 years postoperatively. The VAs (logMAR) of D-eyes, nD-eyes, and Bi at 5.0 m, 0.8 m, and 0.4 m were –0.02±0.08, 0.26±0.19 (D-eyes vs. nD-eyes, p<0.01), and –0.29±0.06; 0.05±0.14, 0.02±0.11 (D-eyes vs. nD-eyes, p>0.05), and –0.03±0.07; and 0.21±0.17, 0.01±0.07 (D-eyes vs. nD-eyes, p<0.001), and –0.02±0.07, respectively. The binocular VA was >20/25 (Snellen lines) at three distances in 100% of the subjects. Between the D-eyes and nD-eyes in CS, a minor but significant difference was observed at 1.5–12.0 cpd. Conclusions: FS-LASIK is safe and practical for the correction of myopes with presbyopia, with long-term efficacy at near and far distances. Compared with D-eyes, the CS in nD-eyes may be impaired at low and moderate spatial frequencies.
Early Visual Outcomes and Optical Quality after Femtosecond Laser Small-Incision Lenticule Extraction for Myopia and Myopic Astigmatism Correction of Over-10 Dioptres

Abstract:PurposeTo investigate early visual and refractive outcomes, corneal stability and optical quality after femtosecond laser small-incision lenticule extraction (SMILE) for treating myopia and myopic astigmatism over -10D. MethodsThirty eyes (30 patients) with myopia and myopic astigmatism of over -10D were treated with VisuMax((R)) femtosecond laser (version 3.0; Carl Zeiss Meditec AG, Jena, Germany). Six months postoperative safety, efficacy and predictability were evaluated. Corneal Scheimpflug topography was measured preoperatively, 1day, 3months and 6months postoperatively. Wavefront aberrations were measured preoperatively, 3months and 6months postoperatively. ResultsSix months postoperatively, LogMAR uncorrected and corrected distance visual acuity (CDVA) were -0.0130.086 and -0.073 +/- 0.069, respectively. 73% (97%) of eyes were within 0.5 (1) D of target refraction. No eyes lost CDVA, 43% (13 eyes) gained one line and 7% (two eyes) gained two lines. Mean corneal back curvature (KMB) and posterior central elevation (PCE) did not change significantly comparing preoperative and 6months postoperative data (p=0.91 and 0.77, respectively). Comparing 1day with 6months postoperative data, central corneal thickness (CCT), mean corneal front curvature (KMF), KMB and PCE did not change significantly (p=0.27, 0.07, 0.52, 0.71, respectively). Total higher-order aberration (HOA), spherical aberration and coma increased significantly (p<0.01) but trefoil remained stable (p=0.49). ConclusionOur results indicate that SMILE can correct myopia and myopic astigmatism of over -10D predictably. No early ectasia was observed. Long-term changes in visual quality and corneal stability require further investigation.
Long-term Visual Quality after Small Incision Lenticule Extraction (SMILE) and Laser Assisted Subepithelial Keratomileusis (LASEK) for Low Myopia

Abstract:Abstract Background Few studies have reported the visual outcomes of small-incision lenticule extraction (SMILE) and laser-assisted subepithelial keratomileusis (LASEK) for myopia correction. This study aims to compare the visual quality and corneal wavefront aberrations after SMILE and LASEK for low-myopia correction. Methods In this prospective study, we included 29 eyes of 29 patients who received SMILE and 23 eyes of 23 patients who received LASEK between June 2018 and January 2019. The following measurements were assessed: uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, corneal wavefront aberrations, and subjective visual quality. All patients were followed up for two years. Results All procedures were uneventful. An efficacy index of 1.19 ± 0.17 was established in the SMILE group and 1.23 ± 0.20 in the LASEK group. No eyes lost more than two lines of CDVA. We found that 93% (27/29) of the treated eyes in the SMILE group and 91% (21/23) in the LASEK group had spherical equivalent (SE) within ± 0.25D. The increases in the total corneal spherical aberration and the corneal front spherical aberration were lower in the SMILE group than in the LASEK group (P < 0.01). In contrast, the increases in the total corneal vertical coma and the corneal front vertical coma in the SMILE group were greater than those in the LASEK group (P < 0.01). Conclusion Both SMILE and LASEK have good safety, stability, and patient-reported satisfaction for low myopia. SMILE induced less corneal spherical aberration but greater vertical coma than LASEK.

Abstract:Purpose To compare the efficacy and visual outcomes after femtosecond laser small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in correcting high myopia. Methods This prospective, randomized study included patients who underwent SMILE or FS-LASIK for the correction of high myopia [the sum of spherical and cylindrical error from -10.00 to -14.00 dioptres (D)]. Preoperative, 1-month (P1m), 3-months (P3m) and 6-months (P6m) postoperative outcomes were analysed and compared between the two procedures. Results Ninety-six right eyes of 96 patients (SMILE: n = 51, FS-LASIK: n = 45) were included. Both the (Attempted - achieved) sphere and the (attempted - achieved) spherical equivalent (SEQ) were greater in the FS-LASIK group at all three postoperative time-points (p < 0.001). The postoperative vector means of astigmatism were smaller in the SMILE group than in the FS-LASIK group. The differences between the optical zone of tissue removal (ROZ) during surgery and the postoperative functional optical zone (FOZ; ROZ-FOZ) were smaller in the SMILE group than in the FS-LASIK group. No significant difference was found between the two procedures in the delta-root mean square (RMS) of aberrations, except for delta-spherical aberration (SA) at P3m. Conclusions Both SMILE and FS-LASIK are effective in correcting high myopia. SMILE resulted in less under-correction, less regression, a smaller decrease in the FOZ and a smaller increase in SA when compared to FS-LASIK, resulting in better visual outcomes with SMILE.

Abstract:PURPOSE:The aim of this study was to compare visual outcomes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for high myopia.METHODS:In this prospective, comparative study, a total of 52 eyes of 34 consecutive highly myopic patients with spherical equivalent within the range of -8.00 to -10.00 diopters were recruited. Twenty-three eyes of 16 patients were in the FS-LASIK group and 29 eyes of 18 patients were in the SMILE group. Visual outcomes and wavefront aberrations were analyzed preoperatively and 6 months postoperatively.RESULTS:At the postoperative 6-month visit, 96.6% in the SMILE group and 91.3% in the FS-LASIK group achieved unchanged or better corrected distance visual acuity (CDVA). As for uncorrected distance visual acuity (UDVA), 96.6% in the SMILE group and 95.7% in the FS-LASIK group achieved UDVA of 20/20 or better. As for wavefront aberrations, high-order aberrations (HOAs) and spherical aberrations increased significantly after surgery in both groups relative to corresponding preoperative values (p < 0.001), and vertical coma increased after SMILE (p < 0.001). No statistically significant differences in changes of HOAs (p =0.90), spherical aberrations (p = 0.07), horizontal coma (p = 0.56), coma (p =0.08), horizontal trefoil (p =0.19), vertical trefoil (p = 0.90), and trefoil (p = 0.45) were detected between the 2 groups, except for vertical coma (p < 0.01).CONCLUSIONS:SMILE is as effective as FS-LASIK in correcting high myopia, but attention should be paid to the induction of vertical coma in highly myopic patients following a SMILE procedure.

Abstract:Background This study compares the clinical outcomes of femtosecond laser small-incision lenticule extraction (SMILE) for the correction of myopia and myopic astigmatism greater than − 10 D, and − 10 D or less respectively. Methods 60 eyes/patients were equally selected into group 1 (myopia and myopic astigmatism of − 10 D or less) and group 2 (myopia and myopic astigmatism of over − 10 D), both of which were treated with SMILE. Visual and refractive outcomes, corneal higher-order aberrations, and Bowman’s layer micro-distortions were evaluated preoperatively, 3 months, and 6 months postoperatively. Results LogMAR corrected distance visual acuity (CDVA) of group 1 and group 2 was − 0.069 ± 0.047 and − 0.053 ± 0.073 6 months postoperatively ( P = 0.48). 100% eyes in group 1 and 97% in group 2 were within 1 D of targeted correction ( P = 0.45). Meanwhile, 100% eyes in group 1 and 97% in group 2 had an uncorrected distance visual acuity of 20/25 or better ( P = 0.20). Changes in corneal higher-order aberrations root mean square, coma, and trefoil were similar between the two groups but spherical aberration was higher in group 2 ( P < 0.01). Micro-distortions were observed in 53% in group 1 and 77% in group 2. More micro-distortions were observed in group 2 (3.40 ± 2.66) than in group 1 (2.07 ± 2.29) ( P = 0.041). The total number of micro-distortions was not correlated with postoperative CDVA ( P = 0.77). Conclusions Visual outcomes showed similar results of SMILE for myopic correction of > − 10 D and ≤ − 10 D. Refractive outcomes showed slightly under-correction in higher myopic eyes. Higher myopic treatment tends to induce more spherical aberrations. Micro-distortions had no impact in visual and refractive outcomes.
Refractive Outcomes Comparing Small-Incision Lenticule Extraction And Femtosecond Laser-Assisted Laser In Situ Keratomileusis For High Myopia

Abstract:Purpose: To compare the long-term refractive effects of smallincision lenticule extraction (SMILE) and femtosecond laserassisted laser in situ keratomileusis (FS-LASIK) on the correction of high myopia and astigmatism.Setting: The Eye and ENT Hospital of Fudan University, Shanghai, China.Design: Retrospective case series.Methods: Patients had SMILE or FS-LASIK for high myopia. Subgroup analyses of high myopia (-6.00 to -8.75 diopters [D]) and extremely high myopia (>=-9.00 D) were performed. The main outcome measure was refractive predictability, compared between SMILE and FS-LASIK groups. Secondary outcomes included efficacy, safety, and residual astigmatism.Results: This study included 121 patients (121 eyes: 75 in the SMILE group and 46 in the FS-LASIK group). No differences were found in terms of refractive predictability between SMILE and FS-LASIK in eyes with highmyopia: 56% vs 58.7% achieved +/- 0.50 D of attempted correction (P =.771) and 81.3% vs 76.1% achieved +/- 1.00 D of attempted correction (P =.489). Efficacy indices of the SMILE and FS-LASIK groups were 1.02 +/- 0.24 and 1.03 +/- 0.24 (P =.742), respectively; safety indices were 1.23 +/- 0.22 and 1.20 +/- 0.24 (P =.324), respectively. Logarithmof the minimumangle of resolution uncorrected distance visual acuity and spherical equivalent in the high myopia subgroup were better than in the extremely high myopia subgroup after both SMILE and FS-LASIK (P <.01).Conclusions: SMILE and FS-LASIK were both effective in correcting high myopia and myopic astigmatism. Comparison of Posterior Corneal Elevation after SMILE and FS-LASIK in Correcting Myopia over −9.0 Diopters

Abstract:Background: To compare the changes in posterior corneal elevation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in correcting myopia over -9 diopters (D). Methods: In this prospective comparative study, 82 eyes of 82 patients scheduled for refractive correction were recruited. Eyes were randomly assigned to the SMILE group (45 eyes, -10.43 +/- 0.92 D) or FS-LASIK group (37 eyes, -10.97 +/- 1.37 D). The posterior corneal surface was measured using a Scheimpflug camera (Pentacam, Oculus, Germany) preoperatively and at 1 day, 1 month, and 6 months after surgery. Posterior corneal elevation in the central point and central 4-mm area, and in various optical zones above the best-fit sphere, was analyzed. A P value of less than 0.05 was considered statistically significant. Results: All surgeries were completed successfully. The safety index and efficacy index were 1.20 and 1.00, respectively, in the SMILE group, and was 1.10 and 0.90, respectively, in the FS-LASIK group. No significant difference existed in all analyzed data before and at 6 months after surgery in both the SMILE group and the FS-LASIK group. Changes in posterior corneal elevation after FS-LASIK were greater than after SMILE, with no statistical significance (P >= 0.07). In the SMILE group, residual bed thickness was found to be moderately negatively correlated with changes in the elevation in the central area (P <= 0.045); whereas it was positively correlated in the peripheral area (P=0.002). Conclusions: SMILE and FS-LASIK presented stable posterior corneal surface in correction of myopia over -9.0 D at the follow-up visit of 6 months.

Abstract:Aims To compare long-term clinical outcomes following small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) for myopia and myopic astigmatism correction. Methods In this retrospective study, we enrolled a total of 101 patients (101 eyes) who underwent SMILE or FS-LASIK 3 years prior. Measured parameters included uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction and corneal wavefront aberrations. Results No significant differences in patient characteristics were found between the two groups. At the 3-year follow-up, UDVA was better than or equal to 20/20 in 90% and 85% (p=0.540) of the eyes; the efficacy indexes were 1.05±0.19 and 1.01±0.21 in the SMILE and FS-LASIK groups, respectively (p=0.352). Safety indexes were 1.19±0.17 and 1.15±0.20 in the SMILE and FS-LASIK groups, respectively (p=0.307). Eighty per cent and 65% of eyes were within ±0.50 D of the attempted spherical equivalent correction after SMILE and FS-LASIK, respectively (p=0.164). Vector analysis revealed no significant differences in astigmatic correction between the two groups (p>0.05). Surgically induced spherical aberration was higher in the FS-LASIK group than in the SMILE group (p<0.001). Conclusion Long-term follow-up analysis suggested that both SMILE and FS-LASIK were safe and equally effective for myopic and astigmatic correction.

Visual Outcomes and Optical Quality After Femtosecond Laser Small Incision Lenticule Extraction: An 18-Month Prospective Study.

Abstract:PURPOSE: To investigate the long-term visual outcomes and optical quality after femtosecond laser small incision lenticule extraction (SMILE) for moderate to high myopia correction. METHODS: Fifty-four patients (37 women, 17 men) who underwent SMILE were enrolled in this prospective clinical study. Patient ages ranged from 18 to 40 years, with a mean spherical equivalent of -6.50 +/- 1.64 diopters. Data including refractive parameters, retinal image quality, intraocular scattering, and aberrations were collected at 6 and 18 months after surgery. Patient satisfaction was also evaluated. RESULTS: At 18 months after surgery, both the mean safety index and the efficacy index were 1.23 +/- 0.23 (range: 0.80 to 1.50); there was no significant difference with indices measured at 6 months (P > .05). No patient lost two or more lines of corrected distance visual acuity. Total higher-order aberrations (HOAs) and coma significantly increased after SMILE (P < .05), whereas total spherical aberrations and trefoil changed only slightly. The mean modulation transfer function cutoff frequency was 36.66 +/- 8.54 cycles per degree (cpd) before surgery and 37.81 +/- 6.89 cpd at 18 months postoperatively; the mean objective scatter index was 0.62 +/- 0.33 before surgery and 0.71 +/- 0.38 at 18 months postoperatively. No significant difference was found between the three time points (P > .05). Mean patient satisfaction was 9.31 +/- 0.64 (with a maximum score of 10). CONCLUSIONS: SMILE showed good safety, efficacy, and stability in correcting moderate to high myopia, and patients were highly satisfied. HOAs increased after SMILE, mainly due to the increase of coma, whereas retinal image quality and intraocular scattering barely changed.

Abstract:PURPOSE:To compare the objective and subjective quality of vision after femtosecond laser-assisted small incision lenticule extraction (SMILE) and laser-assisted subepithelial keratomileusis (LASEK) for mild to moderate myopia.METHODS:This prospective, comparative study included 65 eyes of 33 patients in the SMILE group, with a mean spherical equivalent (SE) of -4.16 ± 0.82 diopters, and 50 eyes of 25 patients in the LASEK group, with a mean SE of -3.81 ± 0.97 diopters. Visual acuity, corneal topography, contrast sensitivity, and wavefront aberrations were recorded preoperatively and compared with postoperative measurements. A quality of vision questionnaire was scored and analyzed 3 months postoperatively.RESULTS:Three months postoperatively, the SMILE group had fewer (P < .05) higher-order aberrations (HOAs) (0.390 ± 0.175 µm), including spherical aberration (SA) (0.262 ± 0.242 µm), than the LASEK group (HOAs = 0.479 ± 0.148 µm, SA = 0.576 ± 0.287 µm, trefoil = 0.465 ± 0.248 µm). There was no significant difference in the amount of coma and trefoil between the two groups after surgery. Analysis of the scores of glare and halos in the quality of vision questionnaire revealed that SMILE (night glare = 1.25 ± 1.22, halos = 0.97 ± 1.12) provided a better quality of vision (P < .05) than LASEK (night glare = 2.32 ± 1.99, halos = 1.96 ± 1.77). The two groups demonstrated no significant difference in contrast sensitivity 3 months postoperatively. No vision-threatening complications were noted at any stage in either group.CONCLUSIONS:Both SMILE and LASEK are safe and effective surgical procedures in the correction of myopia. SMILE has a lower induction rate of HOAs and a higher degree of patient satisfaction than LASEK at 3 months postoperatively.
Small Incision Lenticule Extraction (SMILE) Versus Femtosecond Laser-Assisted in Situ Keratomileusis (FS-LASIK) for Myopia: A Systematic Review and Meta-Analysis

Abstract:PURPOSE:The goal of this study was to compare small incision lenticule extraction (SMILE) with femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for treating myopia. METHODS:The CENTRAL, EMBASE, PubMed databases and a Chinese database (SinoMed) were searched in May of 2016. Twelve studies with 1,076 eyes, which included three randomized controlled trials (RCTs) and nine cohorts, met our inclusion criteria. The overall quality of evidence was evaluated using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) working group framework. Data were extracted and analysed at three to six months postoperatively. Primary outcome measures included a loss of one or more lines of best spectacle corrected visual acuity (BSCVA), uncorrected visual acuity (UCVA) of 20/20 or better, mean logMAR UCVA, postoperative mean spherical equivalent (SE) and postoperative refraction within ±1.0 D of the target refraction. Secondary outcome measures included ocular surface disease index (OSDI), tear breakup time (TBUT) and Schirmer's 1 test (S1T) as dry eye parameters, along with corneal sensitivity. RESULTS:The overall quality of evidence was considered to be low to very low. Pooled results revealed no significant differences between the two groups with regard to a loss of one or more lines in the BSCVA (OR 1.71; 95% CI: 0.81, 3.63; P = 0.16), UCVA of 20/20 or better (OR 0.71; 95% CI: 0.44, 1.15; P = 0.16), logMAR UCVA (MD 0.00; 95% CI: -0.03, 0.04; P = 0.87), postoperative refractive SE (MD -0.00; 95% CI: -0.05, 0.05; P = 0.97) or postoperative refraction within ±1.0 D of the target refraction (OR 0.78; 95% CI: 0.22, 2.77; P = 0.70) within six months postoperatively. The pooled analysis also indicated that the FS-LASIK group suffered more severely from dry eye symptoms (OSDI; MD -6.68; 95% CI: -11.76, -2.00; P = 0.006) and lower corneal sensitivity (MD 12.40; 95% CI: 10.23, 14.56; P < 0.00001) at six months postoperatively. CONCLUSIONS:In conclusion, both FS-LASIK and SMILE are safe, effective and predictable surgical options for treating myopia. However, dry eye symptoms and loss of corneal sensitivity may occur less frequently after SMILE than after FS-LASIK.

Abstract:Purpose: To investigate differences in objective and subjective visual quality 12 months following Q value-guided (Custom-Q) femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) versus small incision lenticule extraction (SMILE) for correction of low-to-moderate myopia and compensate for age-related accommodation deficiency. Methods: Clinical data of 45 patients were analyzed, of whom 23 were treated with FS-LASIK and 22 with SMILE. At the 12-month follow-up, the distance, intermediate, and near visual acuities; objective and subjective refractions; Q factor; corneal higher-order aberrations (HOAs); defocus curve; contrast sensitivity; stereopsis and a subjective visual quality questionnaire were evaluated. Results: At 12 months postoperatively, the two groups had comparable binocular uncorrected distance and intermediate visual acuities (UDVA, UINA). A slightly better uncorrected near visual acuity (UNVA) was observed in the FS-LASIK group (P < 0.05), but patients reported equivalent near vision satisfaction and spectacle independence. Total HOAs and coma increased significantly in all treated eyes (P < 0.05). The Q value and spherical aberration (SA) became more positive, but the shifts were significant only in the SMILE group and the dominant eyes of the FS-LASIK group (P < 0.05). Binocular contrast sensitivity was better in the FS-LASIK group. No significant differences in visual disturbances were found between groups. Conclusion: For myopic patients with mild age-related accommodation deficiency, both FS-LASIK with Custom-Q protocol and SMILE could provide good efficacy, safety, predictability, and high patient satisfaction. The Custom-Q protocol could better preserve the original prolate shape of the cornea and achieve superior visual quality compared to SMILE. Key messages: What is known Both femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) and small-incision lenticule extraction (SMILE) have been widely applied in young myopic patients, with comparable efficacy and safety. Studies evaluating the clinical outcomes of FS-LASIK and SMILE in treating myopia with age-related accommodation insufficiency are lacking. What is new For myopic patients with mild age-related accommodation deficiency, both FS-LASIK with Custom-Q protocol and SMILE could provide good efficacy, safety, predictability, and high patient satisfaction. The Custom-Q protocol could better preserve the original prolate shape of the cornea and achieve superior visual quality compared to SMILE.
Early visual function outcomes of topography-guided FS-LASIK and SMILE in treatment of myopia and myopic astigmatism

Abstract:Aim: To compare the quality outcomes of vision at early phase after topography-guided femtosecond laser-assisted in situ keratomileusis (FS-LASIK-CV) and small incision lenticule extraction (SMILE) in treatment of myopia and myopic astigmatism. Methods: Retrospective comparative analysis of 49 patients that underwent FS-LASIK (n=23) or SMILE (n=26) procedure for myopia and myopic astigmatism between April and September in 2019. Pre- and postoperative uncorrected visual acuity (UCVA), spherical equivalent refraction (SEQ), cylindrical refraction, contrast sensitivity function (CSF), and corneal higher-order aberrations (HOAs) were evaluated. Independent t-test was used for inter-group comparison, while repeated measures ANOVA was used to analyze changes at different time points. Results: In both groups, 100% of the eyes obtained a UCVA of 20/20 or better at 1wk, 1, and 3mo postoperatively. At 1d and 3mo postoperatively, UCVA was better in FS-LASIK-CV group than in SMILE group. At 1wk postoperatively, SEQ was lower in SMILE group than in FS-LASIK-CV group (P=0.006). At 3mo postoperatively, the SEQ reached target refraction in both groups. The residual astigmatism was reduced in both groups without intergroup difference (P>0.05). At 3mo postoperatively, the spherical aberration and coma under 6 mm pupil size were higher than preoperative levels in both groups (P<0.05). However, the increase in the corneal HOAs in the FS-LASIK-CV group was less than the SMILE group (P<0.05). At 3mo postoperatively, the logCS were better than preoperative levels under scotopic conditions without glare and scotopic conditions with glare in both groups (P<0.05). At 1 and 3mo postoperatively, under scotopic conditions without glare and scotopic conditions with glare, FS-LASIK-CV group showed more improvement in logCS at two spatial frequencies (12.0 c/d and 18.0 c/d; P<0.05). Conclusion: Both FS-LASIK-CV and SMILE demonstrate to be safe, effective, and predictable in treatment of myopia and myopic astigmatism. Early postoperative improvement in UCVA and CSF at high spatial frequency under scotopic conditions were better after FS-LASIK-CV than SMILE.

Abstract:PURPOSE: To compare the functional optical zone (FOZ) and visual quality after small-incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in correcting high myopia.METHODS: Ninety-two eyes of 46 high myopic patients with the same programmed optical zone (POZ) received SMILE in one eye and FS-LASIK in the contralateral eye. FOZ was calculated using a refractive power method. The decentration, visual outcomes, wavefront aberrations, contrast sensitivity, and quality of vision (QoV) questionnaire were analyzed at 6 months postoperatively.RESULTS: The postoperative visual and refractive outcomes were comparable between SMILE and FS-LASIK. The FOZ for SMILE (5.62 ± 0.31 mm) was larger than for FS-LASIK (5.35 ± 0.28 mm; P < 0.001). Moreover, the total decentration for SMILE (0.29 ± 0.14 mm) was greater than in FS-LASIK (0.22 ± 0.11 mm; P < 0.001). The induced change in spherical aberration was less for SMILE than for FS-LASIK (P < 0.001). There was better contrast sensitivity under the mesopic condition with glare for SMILE than for FS-LASIK (P = 0.024). However, no significant difference was found in QoV scores between the two groups.CONCLUSIONS: SMILE created a larger FOZ and greater decentration than FS-LASIK when the same POZ was designed in high myopia. Objective and subjective visual symptoms were comparable between SMILE and FS-LASIK.TRANSLATIONAL RELEVANCE: The differences in FOZ and decentration between SMILE and FS-LASIK have little effect on vision outcomes. Surgeons should consider the FOZ and decentration in surgical options in high myopia.

Abstract:Objective To investigate the clinical efficacy,safety,predictability and stability of femtosecond laser small incision lenticule extraction (SMILE) for the treatment of myopia.Methods In this prospective observational study,94 patients (170 eyes) with myopia underwent SMILE surgeries.The average age of the patients was 26.8±4.7 years (range 18 to 44 years).The mean preoperative spherical equivalent (SE) was-6.20±1.37 diopters (D),from-3.25 to-9.13 D.The median myopic astigmatism was-0.75 (0.75) D,up to-3.25 D.The mean corneal thickness was 554.7±27.5 μm (range 484 to 600 μm).Routine ophthalmic examinations were performed.The patients were followed from 6 months to 2 years with uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),manifest refraction,intraocular pressure,corneal topography,and slit lamp examinations.The results were analyzed with a paired t test and linear regression analysis.Results All procedures were completed successfully.At the long-term follow-up examination,97％ of eyes had a UCVA of 1.0 or better and 100％ of eyes had a UCVA of 0.8 or better.BCVA was equal to or better than the preoperative BCVA in 100％ of eyes.35％ gained one line and 10％ gained two lines of BCVA.The mean postoperative SE was-0.01±0.45 D and 87％ and 100％ were within ±0.50 and ±1.00 D of the attempted correction.The root mean square (RMS) values of total higher order aberrations (HOA),spherical aberrations and coma increased by a significant amount after surgery:0.15±0.12 μm,0.04±0.11 μm,0.14±0.14 μm (t=-6.861,-2.078,-5.672,P＜0.05).Conclusion The SMILE procedure is safe,effective,predictable and the results were stable for treating myopia.HOA,spherical and coma aberrations increased postoperatively but the change in spherical aberrations was small.

Abstract:PURPOSE: To perform a comparative clinical analysis of the safety, efficacy, and predictability of two surgical procedures (ie, small incision lenticule extraction [SMILE] and femtosecond laser-assisted LASIK [FS-LASIK]) to correct myopia. METHODS: Sixty eyes of 31 patients with a mean spherical equivalent of −5.13 ± 1.75 diopters underwent myopia correction with the SMILE procedure. Fifty-one eyes of 27 patients with a mean spherical equivalent of −5.58 ± 2.41 diopters were treated with the FS-LASIK procedure. Postoperative uncorrected and corrected distance visual acuity, manifest refraction, and higher-order aberrations were analyzed statistically at 1 and 3 months postoperatively. RESULTS: No statistically significant differences were found at 1 and 3 months in parameters that included the percentage of eyes with an uncorrected distance visual acuity of 20/20 or better ( P = .556, .920) and mean spherical equivalent refraction ( P = .055, .335). At 1 month, 4 SMILE-treated eyes and 1 FS-LASIK–treated eye lost one or more line of visual acuity ( P = .214, chi-square test). At 3 months, 2 SMILE-treated eyes lost one or more line of visual acuity, whereas all FS-LASIK–treated eyes had an unchanged or corrected distance visual acuity. Higher-order aberrations and spherical aberration were significantly lower in the SMILE group than the FS-LASIK group at 1 ( P = .007, .000) and 3 ( P = .006, .000) months of follow-up. CONCLUSIONS: SMILE and FS-LASIK are safe, effective, and predictable surgical procedures to treat myopia. Clinical outcomes after small-incision lenticule extraction versus femtosecond laser-assisted LASIK for high myopia: A meta-analysis

Abstract:Aim To compare postoperative clinical outcomes of high myopia after small-incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK). Methods From March 2018 to July 2020, PubMed, MEDLINE, Embase, the Cochrane Library, and several Chinese databases were comprehensively searched. The studies meeting the criteria were selected and included; the data were extracted by 2 independent authors. The clinical outcome parameters were analyzed with RevMan 5.3. Results This meta-analysis included twelve studies involving 766 patients (1400 eyes: 748 receiving SMILE and 652 receiving FS-LASIK). Pooled results revealed no significant differences in the following outcomes: the logarithm of the mean angle of resolution (logMAR) of postoperative uncorrected distance visual acuity (weighted mean difference (WMD) = -0.01, 95% confidence interval (CI): -0.02 to 0.00, I 2 = 0%, P = 0.07 at 1 mo; WMD = -0.00, 95% CI: -0.01 to 0.01, I 2 = 0%, P = 0.83 at 3 mo; WMD = -0.00, 95% CI: -0.01 to 0.00, I 2 = 32%, P = 0.33 in the long term), and the postoperative mean refractive spherical equivalent (WMD = -0.03, 95% CI: -0.09 to 0.03, I 2 = 13%, P = 0.30). However, the SMILE group had significantly better postoperative corrected distance visual acuity (CDVA) than the FS-LASIK group (WMD = -0.04, 95% CI, -0.05 to -0.02, I 2 = 0%, P<0.00001). In the long term, postoperative total higher-order aberration (WMD = -0.09, 95% CI: -0.10 to -0.07, I 2 = 7%, P<0.00001) and postoperative spherical aberration (WMD = -0.15, 95% CI: -0.19 to -0.11, I 2 = 29%, P<0.00001) were lower in the SMILE group than in the FS-LASIK group; a significant difference was also found in postoperative coma (WMD = -0.05, 95% CI: -0.06 to -0.03, I 2 = 30%, P<0.00001). Conclusion For patients with high myopia, both SMILE and FS-LASIK are safe, efficacious and predictable. However, the SMILE group demonstrated advantages over the FS-LASIK group in terms of postoperative CDVA, while SMILE induced less aberration than FS-LASIK. It remains to be seen whether SMILE can provide better visual quality than FS-LASIK; further comparative studies focused on high myopia are necessary.
