Abstract:Diabetes, particularly Type 2 Diabetes Mellitus (T 2 DM), is a chronic metabolic disorder with high and increasing global prevalence, characterized by insulin resistance and inadequate insulin secretion. Despite advancements in novel drug delivery systems, widespread and systematic treatment of advanced glycation end products (AGEs) remains challenging due to issues like drug toxicity, low water solubility, and uncontrolled release. Thus, developing nanoplatforms with controlled release capabilities has become a major research focus. Due to its excellent biocompatibility and drug delivery properties, chitosan has attracted considerable attention as a typical biopolymer. In this study, we designed and synthesized an intelligent fluorescence-pH sensitive nanopolymer material using chitosan. We loaded drug 1 and chromium phthalocyanine (CrPc) into folic acid-conjugated carboxymethyl chitosan (FA-CMCS) nanocarriers, forming FA-CMCS@1-CrPc. Comprehensive characterization of FA-CMCS@1-CrPc was conducted using Fourier transform infrared spectroscopy (FTIR), scanning electron microscopy (SEM), transmission electron microscopy (TEM), thermal gravimetric analysis (TGA), and gas adsorption analysis (BET). The results indicate that the nanomaterial was successfully synthesized and exhibits excellent specific surface area, biocompatibility, and fluorescence response. Further research revealed that FA-CMCS@1-CrPc not only achieved controlled drug release but also could regulate drug release by adjusting pH. Additionally, due to its strong fluorescence performance, the nanomaterial demonstrated higher detection sensitivity, especially for monitoring the release of 5% trace drugs. An in vitro model of insulin-resistant cells was established to evaluate the effects of the drug delivery system on glucose degradation and AGE-RAGE regulation, providing a foundation for the development of new T 2 DM drugs.

Development and Characterization of a Glimepiride-Loaded Gelatin-Coated Mesoporous Hollow Silica Nanoparticle Formulation and Evaluation of Its Hypoglycemic Effect on Type-2 Diabetes Model Rats

Abstract:In this study, we prepared gelatin-coated mesoporous hollow silica nanospheres (GSN) as a drug carrier to improve the water solubility and regulate the release rate of glimepiride (GLM). GLM was loaded into GSN by an absorption method, and drug-loaded samples (GLM-GSN) were characterized by differential scanning calorimeter (DSC) and X-ray diffraction (XRD). Cellular uptake and in vivo intestinal uptake experiments were performed in rats. In addition, the studies of in-vitro drug dissolution, pharmacokinetics, and pharmacodynamic experiments also were performed. GLM-GSN showed excellent drug loading (39.7% ± 0.7%) and sustained GLM release. The state of GLM in GSN was amorphous according to DSC and XRD results. Cellular uptake and in vivo intestinal uptake experiments indicated that GSN could be effectively absorbed, and an MTT experiment demonstrated that GSN had good biocompatibility. Furthermore, the GLM-GSN had a higher bioavailability in pharmacokinetics experiments and a prominent hypoglycemic effect on type-2 diabetes model rats in pharmacodynamic experiments. This study clearly shows that GSN is a promising platform for delivering GLM for the treatment of type-2 diabetes.

Abstract:Background: Empagliflozin (EMPA) is an SGLT2 inhibitor, a new class of anti-diabetic medication, indicated for treating type-2 diabetes. Its low permeability, poor solubility and bioavailability limits its use in management of diabetes. The study was aimed to formulate EMPA loaded polymeric micelles (PMs) to overcome these obstacles in oral absorption. Methodology: In silico studies-molecular docking, molecular dynamic simulation (MDS), and quantum chemical calculation were employed to study the interaction of EMPA with different polymers. EMPA loaded TPGS polymeric micelles (EMPA-TPGS-PMs) were formulated by direct dissolution method and characterized in terms of surface morphology, entrapment, particle size, in vitro drug release, and in vitro cytotoxicity (HEK293 cells). In vivo pharmacokinetic and pharmacodynamic studies were also performed. Results: The results suggested a good interaction between TPGS and EMPA with lowest binding energy compared to other polymers. Further MDS results and DFT calculations validated the stable binding of the complex hence TPGS was selected for further wet lab experiments. The EMPA-TPGS complex displayed lower value of Total energy (T.E.) than its individual components, indicating the overall stability of the complex while, the energy band gap (∆E) value lied between the two individual molecules, signifying the better electron transfer between HOMO and LUMO of the complex. Based on the solubility, entrapment and cytotoxicity studies, 5% TPGS was selected for formulating drug loaded micelles. EMPA-TPGS5-PMs presented a size of 9.008 ± 1.25 nm, Polydispersity index (PDI) of 0.254 ± 0.100, a controlled release behaviour upto 24 h. SEM and AFM images of the nanoformulation suggested spherical particles whereas, DSC, and PXRD studies confirmed the loss of crystallinity of EMPA. A 3.12-folds higher AUC and a greater reduction in blood glucose levels was exhibited by EMPA-TPGS5-PMs in comparison to EMPA-SUSP in mice model. Conclusion: EMPA-TPGS-PMs has exhibited better bio absorption and therapeutic effectiveness in diabetes treatment. This improved performance would open the possibility of dose reduction, reduced dosing frequency & dose-related side effects, improving pharmaco-economics and thereby improved overall compliance to the patient. However, this translation from bench to bedside would necessitate studies in higher animals and human volunteers.

Abstract:Journal of Bioactive and Compatible Polymers, Ahead of Print. Marine beds are an untapped resource of bioactive materials which can be explored for drug delivery applications. In the present study, a hydrogel was developed with an optimal concentration of sodium alginate-chitosan core polyelectrolytic complex loaded with anti-diabetic drug metformin and coated with ĸ-Carrageenan as an efficient oral drug delivery vehicle. The formulation was optimized by changing parameters such as concentration of polymers, amount of cross-linker and the type and amount of coating material. The prepared hydrogels were characterized for their structural integrity using instrumental techniques such as FTIR, XRD, DSC, and SEM while the physical properties were assessed by evaluating its thickness, UV barrier ability and swelling degree. In vitro study demonstrated the influence of presence and type of coating material affecting drug delivery process. The study suggested that coating with 3% ĸ-Carrageenan (A19) was found most suitable for oral drug delivery since it could resist diffusion of drug in the stomach (pH 1.2) so that maximum drug could reach the intestine (pH 7.4) for absorption. Metformin loaded hydrogel (A20) released ~49% drug in the simulated gastric fluid (pH 1.2). In the simulated intestinal fluid (pH 7.4) both the hydrogel exhibited a sustained release pattern lasting for more than 4 h. Investigation of drug release kinetics using different mathematical models showed that Higuchi model was the best fit release model with R2 ⩾ 0.973. The results indicated that the prepared hydrogels could be potential drug delivery vehicle toward intestine as well as for extended release to colon targeted drug delivery.

Nano-encapsulation and characterizations of glimepiride drug with chitosan nanoparticles and its in vitro drug release kinetics and antidiabetic activity

Abstract:The objective of the present study was to encapsulate the glimepiride (GLM) drug with biodegradable chitosan nanoparticles (CS NPs) by using the emulsification-solvent evaporation method to control and provide better treatment options for those suffering from type 2 diabetes mellitus. The encapsulation efficiency of encapsulated GLM–CS NPs was found to be 91.4 %, indicating that the CS NPs were able to effectively encapsulate with the GLM drug. The XRD analysis showed characteristic peaks at 2 θ values of 16.25 ° , 22.99 ° and 35.01 ° , which indicated an amorphous nature. The HR-TEM showed an average size of 22.33 nm, were spherical in shape, and some were clusters in nature. The AFM showed a spherical shape with a size of 25.15 nm and a height of 59.1 nm. The particle size analysis showed an average size of 184.9 d.nm, and the Zeta potential form was -8.46 mV. The in vitro drug release study indicated sustained release over a period of 32 hours and also displayed outstanding inhibitory potential against α- amylase and α- glucosidase activities with IC 50 values of 281.64 and 283.69 μg/mL, respectively, and standard acarbose showed 252.50 and 252.71 μg/mL, respectively. We believed that sustained release of encapsulated GLM–CS NPs could control diabetes by blocking the enzymes that break down carbohydrates into simple sugars, thus preventing them from being absorbed into the bloodstream. It could offer a controlled and prolonged release of the drug, which help to decrease the regularity of drug administration and potentially lead to improved patient compliance and better control of plasma glucose levels.

Abstract:Curcumin and epigallocatechin gallate have the disadvantage of low aqueous solubility and first-pass metabolism, resulting in limited bioavailability. This work aimed to enhance oral bioavailability by forming gastric pH-stable shellac nanoparticles containing curcumin and epigallocatechin gallate using locust bean gum by anti-solvent precipitation (CESL-NP). The nanoparticles were characterized by their particle size, morphology, zeta potential, gastric pH stability, release profile, drug loading, and entrapment efficiency. The findings showed that a network of hydrolyzed shellac, locust bean gum, curcumin, and epigallocatechin gallate successfully entrapped individual particles inside a complex system. The morphological investigation of the CESL-NP formulation using FESEM, TEM, and AFM revealed the presence of spherical particles. FTIR, DSC, and XRD analysis revealed that curcumin and epigallocatechin gallate were amorphous due to their bond interactions with the matrix. Streptozotocin-treated mice, upon treatment with CESL-NP, showed kidney and pancreatic improvements with normalized kidney hypertrophy index and histopathology, maintained biochemical parameters, increased beta cell count, and a 38.68-fold higher blood glucose level inhibition were observed when compared to free-(CUR + EGCG). This research affirms that the shellac-locust bean gum complex shows potential for the sustained oral delivery of curcumin and epigallocatechin gallate, specifically for treating diabetic nephropathy.

Abstract:Introduction: Glyburide is a drug for the treatment of diabetes mellitus and has a potential effect on Alzheimer's disease. It is also a BCS Class 2 drug with low solubility and low permeability. Developing a nanosuspension formulation and increasing the solubility and dissolution rate of glyburide is required to overcome this challenge. Methods: Thus, the goal of this work was to create glyburide nanosuspensions by ball milling and homogenizing glyburide to increase its solubility and rate of dissolution. To achieve this, the nanosuspension formulation was optimized using a central composite design. Zeta potential, particle size distribution and solubility were selected by way of dependent variables, and ball milling time, homogenization cycles, and Pluronic F-127/glyburide ratio were chosen as independent variables. Glyburide nanosuspensions were obtained with a particle size of 244.6 ± 2.685 nm. In vitro release and solubility studies were conducted following optimization. Results: The saturation solubility of glyburide was nearly doubled as a result of the nanocrystal formation. Xray diffraction (XRD), scanning electron microscopy (SEM), differential scanning calorimetry (DSC), and Fourier-transform infrared spectroscopy (FT-IR) were used to assess the nanosuspension. SEM images confirmed that the nanocrystal formation process was successful. Glyburide and the excipients have no incompatibilities, their physical states have not changed, and the preparation method has not affected the stability of glyburide, according to DCS, XRD, and FT-IR analyses. Conclusion: These studies indicated that a combination of ball milling and homogenization techniques significantly enhanced the solubility of glyburide and its release from the formulation. Consequently, this approach can be applied to formulations characterized by low absorption and limited bioavailability.
A polyphenol-metal nanoparticle platform for tunable release of liraglutide to improve blood glycemic control and reduce cardiovascular complications in a mouse model of type II diabetes

Abstract:Liraglutide is a GLP-1 receptor agonist recently approved for Type-II diabetes (T2D) treatment with superior hypoglycemic effect while also improving cardiovascular function for the patients. However, its application has been limited by its short half-life (~13 h), which requires daily injections to maintain effective drug concentrations in blood, thus increasing the risk of poor patient compliance and complications. In this study, we developed a ternary liraglutide/tannic acid (TA)/Al3+ nanoparticle system based on hydrogen bond formation between liraglutide and TA and stabilized by complex coordination interaction between TA and Al3+. This ternary nanoparticle formulation offers sustained release of liraglutide for >8 days by optimizing the concentration of TA during nanoparticle assembly. A flash nanocomplexation (FNC) process was adopted to confer homogeneous mixing of the three components and control the assembly kinetics, thus enabling efficient encapsulation, a tunable drug release profile, improved nanoparticle size uniformity, and a high degree of colloidal stability. Upon a single intraperitoneal (i.p.) administration, the optimized formulation effectively lowered the high blood glucose level in a T2D db/db mice model to the normal range (8-10 mmol/L) within 6 h, maintained it for 60 more hours, and kept it lower than the original level for >6 days. In a 30-day treatment study, the nanoparticle formulation with a dosage frequency of once every 5 days exhibited similar or better control of blood sugar level (20% reduction in HbA1c) and weight control than daily injection of free liraglutide at the same treatment dose. The extended glycemic control led to distinctive improvements on reducing cardiomyopathy, including inhibition in lipo-toxicity by decreasing 40% of triglyceride, 30% of diacylglycerol and 50% of PKC level in the heart, as well as ameliorating oxidative stress and cell apoptosis activities through positive regulation on superoxidase, malondialdehyde, caspase-3 and Bax. This nanoparticle system demonstrates improved therapeutic potential owing to its long-acting glycemic control with improved cardiovascular function and reduced tissue toxicity in multiple organs.
Dually-crosslinked Degradable Polyionic Micelles for Sustained Glucose-responsive Insulin Release

Abstract:The glucose-sensitive delivery system holds great promise as a therapeutic approach for high-incidence diabetes due to its ability to release insulin whenever elevated glycemia. However, it still faces the instability problem under hyperglycemic environment, which leads to the short time of sustained insulin release. Herein, we designed dually-crosslinked insulin polyionic micelles (DCM@insulin) based on triblock polymers of o-glycol and phenylboronic acid-functionalized poly(ethylene glycol)-poly(dimethylamino carbonate)-poly (dimethylamino-trimethylene carbonate) (mPEG-P(AC-co-MPD)-PDMAC and mPEG-P(AC-co-MAPBA)-PDMATC, respectively) for sustained glucose-responsive insulin release. DCM@insulin with phenylboronic acid ester structure (first crosslinking structure) enhanced glycemic responsiveness by regulating insulin release under hyperglycemic environment. Additionally, the UV-crosslinking structure (second crosslinking structure) by the residual double bonds in AC units endowed DCM@insulin with the ability to effectively protect the loaded insulin against protease degradation and avoid burst release under multiple insulin release. The in vivo findings demonstrated that DCM@insulin effectively maintained glycemic levels (BGLs) within the normal range for 6 h in comparison to single-crosslinked micelles (SCM@insulin). Therefore, the glucose-responsive and dually-crosslinked polyionic micelles system exhibits potential as a viable option for treatment of diabetes.

Formulation, Statistical Optimization, and In Vivo Pharmacodynamics of Cydonia oblonga Mucilage/Alginate Mucoadhesive Microspheres for the Delivery of Metformin HCl

Abstract:In recent years, attention has shifted toward the utilization of natural polymers for encapsulation and sustained release of health-hazardous drugs. The purpose of this work is to define and assess the sustained delivery potential and mucoadhesive potential of a Cydonia oblonga mucilage (COM) and sodium alginate (Na-Alg)-constituting polymeric delivery carrier of antidiabetic drugs with a specific end goal to retain metformin HCl in the stomach while expanding the drug's bioavailability. Metformin HCl was encapsulated in mucoadhesive microspheres by an ionic gelation method. Polymers with different combinations were tried, and the resulting mucoadhesive COM/Na-Alg microspheres were assessed for particle size (mm) PS/Y1, drug encapsulation efficiency DEE (%)/Y2, and in vitro percentage cumulative drug release R12h/Y3 using Drug Design Expert software version 10. The response surface methodology by a 32-central composite design predicted optimal synthesis parameters for the microspheres to be 295 mg for COM and 219 mg for Na-Alg. An optimized formulation was prepared under these conditions and used to evaluate the micrometric properties, morphology and structural characteristics, swelling behavior, in vitro drug release, and kinetics. Acute toxicity studies were carried out on blank COM/Na-Alg microspheres to deem them safe for in vivo studies. The DEE (%) was calculated to be 85.8 ± 1.67, whereas scanning electron microscopy (SEM) showed a coarse surface with characteristic wrinkles and cracks with an optical microscopic particle size of 0.96 ± 2.45. The ex vivo tests showed great mucoadhesive properties and good swelling behavior with pH-responsive drug release and a significant reduction in in vivo blood glucose levels. The results advocated the use of optimized microspheres to enhance the bioactivity with a possible dose reduction, making it less symptomatic, reducing the expense of the treatment, and subsequently facilitating better patient compliance.
The Ras Signaling Inhibitor LOX-PP Interacts with Hsp70 and c-Raf To Reduce Erk Activation and Transformed Phenotype of Breast Cancer Cells

Abstract:ABSTRACT The lysyl oxidase gene (LOX) inhibits Ras signaling in transformed fibroblasts and breast cancer cells. Its activity was mapped to the 162-amino-acid propeptide domain (LOX-PP) of the lysyl oxidase precursor protein. LOX-PP inhibits Erk signaling, motility, and tumor formation in a breast cancer xenograft model; however, its mechanism of action is largely unknown. Here, a copurification-mass spectrometry approach was taken using ectopically expressed LOX-PP in HEK293T cells and the heat shock/chaperone protein Hsp70 identified. Hsp70 interaction with LOX-PP was confirmed using coimmunoprecipitation of intracellularly and bacterially expressed and endogenous proteins. The interaction was mapped to the Hsp70 peptide-binding domain and to LOX-PP amino acids 26 to 100. LOX-PP association reduced Hsp70 chaperone activities of protein refolding and survival after heat shock. LOX-PP interacted with the Hsp70 chaperoned protein c-Raf. With the use of ectopic expression of LOX-PP wild-type and deletion proteins, small interfering RNA (siRNA) knockdown, and Lox−/− mouse embryo fibroblasts, LOX-PP interaction with c-Raf was shown to decrease downstream activation of MEK and NF-κB, migration, and anchorage-independent growth and reduce its mitochondrial localization. Thus, the interaction of LOX-PP with Hsp70 and c-Raf inhibits a critical intermediate in Ras-induced MEK signaling and plays an important role in the function of this tumor suppressor.
Ternary Blend of Chitosan/Polyvinyl Alcohol/Carboxymethylcellulose as an Efficient System for the Release Studies of Metformin Hydrochloride

Abstract:The release of metformin from the ternary blend of chitosan, polyvinyl alcohol, and carboxymethylcellulose is discussed in the study. The antidiabetic activity of the developed beads is assessed. The hydrogel beads made of chitosan/polyvinyl alcohol/carboxymethylcellulose are an excellent vehicle for the antidiabetic drug metformin. Chitosan (CS) has attracted substantial attention as a sustainable biopolymer in drug delivery applications, in recent years, because of its nontoxicity, biocompatibility, and accessibility. This study develops chitosan/polyvinyl alcohol/carboxymethylcellulose (CS/PVA/CMC) ternary hydrogel with excellent efficacy as a potential delivery vehicle for the antidiabetic drug metformin hydrochloride. The incorporation of carboxymethylcellulose (CMC) in the blend remarkably improves the swelling properties of the hydrogel. Metformin hydrochloride is loaded in the CS/PVA/CMC ternary blend by ultrasonication method. The prepared hydrogel beads are characterized by, Fourier‐transform infrared spectroscopy (FT‐IR), Scanning electron microscopy (SEM), and X‐ray diffraction (XRD) techniques and confirm the presence of the drug in the beads. The loading and release of metformin from the CS/PVA/CMC hydrogel beads are followed by UV–visible spectroscopy at 233 nm. Different formulations are prepared by varying the compositions of chitosan and carboxymethylcellulose. α‐Glucosidase inhibitory studies reveal that metformin loaded CS/PVA/CMC hydrogel beads show excellent antidiabetic activity. The maximum value of inhibitory activity observed is 89%. Cytotoxic analysis proves that the beads prepared are nontoxic.

Abstract:An insulin delivery system that self-regulates blood sugar levels, mimicking the human pancreas, can improve hyperglycaemia. At present, a glucose-responsive insulin delivery system combining AAPBA with long-acting slow release biomaterials has been developed. However, the safety of sustained-release materials and the challenges of preventing diabetic complications remain. In this study, we developed a novel polymer slow release material using a plant extract-p-hydroxyphenylethyl anisate (HPA). After block copolymerisation with AAPBA, the prepared nanoparticles had good pH sensitivity, glucose sensitivity, insulin loading rate and stability under physiological conditions and had high biocompatibility. The analysis of streptozotocin-induced diabetic nephropathy (DN) mouse model showed that the insulin-loaded injection of nanoparticles stably regulated the blood glucose levels of DN mice within 48 h. Importantly, with the degradation of the slow release material HPA <i>in vivo</i>, the renal function improved, the inflammatory response reduced, and antioxidation levels in DN mice improved. This new type of nanoparticles provides a new idea for hypoglycaemic nano-drug delivery system and may have potential in the prevention and treatment of diabetic complications.

Abstract:Diabetes mellitus is a chronic metabolic disorder marked by elevated blood sugar levels, leading to organ dysfunction. Curcumin, derived from turmeric, exhibits promise in managing type II diabetes. Amphipathic polymer prodrugs were synthesized by conjugating curcumin with chitosan through succinic anhydride. Nanomicelles, formed via dialysis of amphipathic polymer prodrug, were spherical with an average hydrodynamic size of 57 nm. In vitro release studies revealed 97% curcumin release at pH 5 in 7 days. A 21-day experiment on diabetic mice compared nano micelles, standard drugs, and free curcumin’s impact on fasting blood glucose. The study showcased gradual, controlled curcumin release from nano micelles, suggesting their potential in type II diabetes treatment.

Abstract:AIM:To investigate the possibility of poly(lactic-co-glycolic acid) as a carrier for the delivery of macromolecular.METHODS:Insulin-loaded poly (lactic-co-glycolic acid) nanoparticles (INS-PLGA-NPs) was prepared by a double-emulsion solvent evaporation method. The size distribution was examined by photo-correlation spectrometry. The entrapment efficiency was determined by HPLC and important factors that affected the entrapment efficiency were investigated. The loading mechanism of different size nanoparticles was assayed by radioimmunoassay (RIA). INS-PLGA-NPs release behavior in vitro was carried out under sink condition. After oral administration of the nanoparticles to alloxan-induced diabetic rats, its glucose level was determined by glucose oxidize method and the oral pharmacological bioavailability in contrast to s.c. of insulin solution was calculated according to the area over the curve.RESULTS:The INS-PLGA-NPs was prepared with poloxamer 188 as a emulsifier, the mean diameter was 149.6 nm and the polydispersity index was decreased to 0.09. While the entrapment efficiency was increased to 42.8%. Most of the insulin loaded was adsorbed on the surface of the nanoparticles. The release behavior in vitro showed an initial burst effect followed by a slower rate stage. After oral administration of 10 u.kg-1 INS-PLGA-NPs, the plasma glucose level decreased significantly after 4 h (P < 0.05), 10 h later the glucose level decreased to the lowest (52.4% +/- 10.2%, P < 0.01) and the relative pharmacological bioavailability is (10.3 +/- 0.8)%.CONCLUSION:PLGA-NPs might be used as a new oral carrier for protein drug delivery systems in the future.
Mesoporous silica and alumina nanoparticles to improve drug delivery of pioglitazone on diabetic type 1 nephropathy in rats

Abstract:Background and purpose: Diabetic nephropathy leads to end-stage renal disease. The present study aimed to evaluate the prophylactic effect of pioglitazone-loaded mesoporous silica and alumina scaffold on renal function and the underlying mechanisms in streptozotocin-induced diabetic rats. Experimental approach: The mesoporous nanoparticles were synthesized by chemical methods from tetraethylorthosilicate and aluminum isopropoxide and characterized by Fourier transform infrared spectroscopy, X-ray diffraction, and scanning electron microscopy. The soaking method was applied to load pioglitazone into the mesoporous silica and alumina. Subsequently, the most capable formulation was evaluated for lipid profile, blood glucose, renal function biomarkers, malondialdehyde, and kidney histopathological changes in diabetic rats. Findings/results: Pioglitazone loaded in the mesoporous included a superior release of about 80%. No interaction was observed in Fourier transform infrared spectroscopy and X-ray diffraction was shown crystalline. Scanning electron microscopy showed the size of the nanometer in the range of 100 - 300 nm. Mesoporous silica containing the drug significantly decreased urinary parameters, triglycerides, low-density lipoprotein, blood urea nitrogen, blood glucose, malondialdehyde, and creatinine. In addition, it showed increased high-density lipoprotein, significantly. The renal histopathological changes indicated improvement compared with the untreated diabetic group. Conclusion and implications: It was concluded that the mesoporous was potent to serve as a promising drug carrier and a platform aimed at the delivery of poorly water-soluble drugs for improving oral bioavailability. Furthermore, it has the potential to provide a beneficial effect on the changes in diabetic parameters.

Abstract:The objective of the present study was to develop a pH-sensitive drug delivery system by using polymethylmethacrylate-grafted gellan gum (PMMA-g-GG). PMMA-g-GG was synthesized by free radical polymerization reaction by using redox initiator ceric ammonium nitrate (CAN), and a series of graft copolymers were prepared with varying concentrations of methylmethacrylate (MMA) and CAN. Grafting parameters such as the percentage and efficiency of grafting were calculated, and the effect of monomer as well as initiator concentration was studied on the grafting yield. Optimization was done by one optimal response surface methodology. The batch with a better percentage grafting and grafting efficiency was selected and characterized by elemental analysis (CHN), FT-IR, DSC, PXRD, <sup>1</sup>H-NMR, and SEM. Furthermore, acute oral toxicity study and histopathological analysis suggested non-toxic and biocompatible nature of the grafted gum. Metformin hydrochloride pellets were prepared using PMMA-g-GG, characterized in detail, and assessed for biocompatibility and efficacy. PMMA-g-GG-based formulation (M4) exhibited a pH-sensitive as well as sustained release of the drug over the period of 12 h and the release profile followed Peppas model. In vivo efficacy studies indicated a promising antidiabetic potential of the prepared formulation. Thus, PMMA-g-GG-based formulations can be implicated as novel drug delivery systems for facilitated antidiabetic therapy in the near future.

Abstract:Background: Gestational diabetes mellitus (GDM) poses significant risks during pregnancy for both mother and fetus. Adherence to oral antidiabetic medications, like glibenclamide (GB), can be challenging, necessitating novel drug delivery methods. Nanostructured lipid carriers (NLC) offer a promising approach by efficiently permeating the skin due to their small size and lipid-based composition. Objective: This study aimed to develop and evaluate transdermal patches loaded with glibenclamideloaded NLCs to treat GDM. Methods: Glibenclamide NLCs were prepared using hot homogenization with ultrasonication and melt dispersion method. A central composite design was utilized to optimize the formulations. Transdermal patches containing optimized NLCs were developed using HPMC K 100 and Eudragit L polymers. The patches were evaluated for various parameters, and their pharmacokinetic and pharmacodynamic studies were carried out to assess their safety and efficacy. Results: Optimized NLCs efficiently permeated rat skin. Cell viability studies indicated the nontoxicity of the formulations. NLC-loaded transdermal patches (F2 and F7) showed drug release of 1098 μg/cm2 and 1001.83 μg/cm2 in 24 hours, with a 2.5-fold higher flux and permeation coefficient than the GB patch. Pharmacokinetic analysis revealed Tmax of 8 and 10 hours and Cmax of 7127 ng/ml and 7960 ng/ml for F2 and F7, respectively, ensuring sustained drug action. AUC0-α was 625681 ng/ml*h and 363625 ng/ml*h for F2 and F7, respectively, indicating improved bioavailability. Conclusion: Transdermal patches incorporating NLCs hold promise for enhancing glibenclamide's therapeutic efficacy in GDM treatment. Improved skin permeation, sustained drug release, and enhanced bioavailability make NLC-based transdermal patches a potential alternative with better patient compliance.

Development and optimization of sitagliptin and dapagliflozin loaded oral self-nanoemulsifying formulation against type 2 diabetes mellitus

Abstract:Control of hyperglycemia and prevention of glucose reabsorption (glucotoxicity) are important objectives in the management of type 2 diabetes. This study deals with an oral combined dosage form design for two anti-diabetic drugs, sitagliptin and dapagliflozin using self-nanoemulsifying drug delivery systems (SNEDDS). The SNEDDS were developed using naturally obtained bioactive medium-chain/long-chain triglycerides oil, mixed glycerides and nonionic surfactants, and droplet size was measured followed by the test for antioxidant activities. Equilibrium solubility and dynamic dispersion experiments were conducted to achieve the maximum drug loading. The in vitro digestion, in vivo bioavailability, and anti-diabetic effects were studied to compare the representative SNEDDS with marketed product Dapazin®. The representative SNEDDS containing black seed oil showed excellent self-emulsification performance with transparent appearance. Characterization of the SNEDDS showed nanodroplets of around 50-66.57 nm in size (confirmed by TEM analysis), in addition to the high drug loading capacity without causing any precipitation in the gastro-intestinal tract. The SNEDDS provided higher antioxidant activity compared to the pure drugs. The in vivo pharmacokinetic parameters of SNEDDS showed significant increase in C max (1.99 ± 0.21 µg mL-1), AUC (17.94 ± 1.25 µg mL-1), and oral absorption (2-fold) of dapagliflozin compared to the commercial product in the rat model. The anti-diabetic studies showed the significant inhibition of glucose level in treated diabetic mice by SNEDDS combined dose compared to the single drug therapy. The combined dose of sitagliptin-dapagliflozin using SNEDDS could be a potential oral pharmaceutical product for the improved treatment of type 2 diabetes mellitus.
Optimisation and evaluation of Gymnema sylvestre extract loaded polymeric nanoparticles for enhancement of in vivo efficacy and reduction of toxicity

Abstract:Aim: This work studies the development and evaluation of Gymnema sylvestre (GYM) extract loaded sustained release polymeric nanoparticles (PNPs) for enhanced bioavailability and reduced nephrotoxicity. The current therapy is associated with the drawbacks of addiction and repeated administration. Method: The sustained release PNPs were developed and evaluated for toxicity. PNPs of GYM were prepared by double emulsion solvent evaporation technique utilising Taguchi model and evaluated for physicochemical properties (particle size, zeta potential, entrapment efficiency), in vitro drug release, compatibility, and stability. Further, the bioavailability and in vivo nephrotoxicity studies in diabetic rat model were also carried out. Result: The developed optimised nanoparticles were 205.7 ± 1.20 nm in size, -40.68 mV zeta potential, compatible, and stable in nature with improved entrapment efficiency (67.1 ± 0.2%) and sustained release. Moreover, nanoparticles were found to lower the blood glucose level in single as well as multiple doses. Results of in vivo study indicated that GYM-NPs increased the phosphorylase activity and thus enhanced insulin secretion. Furthermore, the nanoparticles were free from toxicity, which was confirmed by the estimation of kidney biomarker. Conclusion: The nanoparticles increased the bioavailability of GYM extract and have a great potential for the treatment of diabetes in reduced dose, and so these can be potential candidates for treating diabetes.
