Serum uric acid level in chronic liver disease and its correlation with Child-Pugh score in a tertiary care hospital from South India

Abstract:Background: Chronic liver disease (CLD) is one of the important causes of morbidity and mortality in our country, and since the damage to the liver is irreversible, we have to look for many severity markers or predictors for the prognosis of the patient. In this study, we have tried to correlate the level of serum uric acid (UA) with the severity of CLD presented as a Child-Pugh score. Methods: A cross-sectional observational study was conducted at Vijayanagar Institute of Medical Science (VIMS), Ballari, Karnataka, from October 2015 to June 2017 in the Department of General Medicine. Fifty patients diagnosed with CLD, aged between 18 and 65 years, of either gender, were enrolled in the study. Serum UA levels were measured, and liver function and coagulation parameters were assessed. A statistical analysis was performed to evaluate the association between serum UA levels, liver function test, and coagulation parameters. Results: In our study, the mean serum UA level was 6.52 mg/dl and was raised in patients with CLD in correlation to its severity. Alcoholic liver disease (ALD) was the most common etiology for CLD (80%) followed by hepatitis B (Hep B) virus infection (12%) and hepatitis C (Hep C) virus infection (6%). Serum UA levels increased as the Child-Turcotte-Pugh (CTP) score increased. The mean UA level in CTP class C was 8.29 mg/dl. Various parameters such as serum aspartate aminotransferase (AST), alanine transaminase (ALT), alkaline phosphatase, total bilirubin, international normalized ratio (INR), calcium, and albumin were significantly associated with serum UA levels in CLD patients. Conclusion: The correlation between rising blood UA levels and the Child-Pugh score shows that UA estimate may be a valid and affordable indicator for assessing the extent of liver cirrhosis in CLD.
Effect of Serum SAAG Ratio on the Outcome of Diagnosed Patients with (SBP) Spontaneous Bacterial Peritonitis in Comparison to their CTP & Meld Classification

Abstract:Objective: To investigate the serum albumin levels among hospitalized patients of chronic Liver disease (CLD) having SBP in Asian Institute of Medical Sciences Hospital Hyderabad, detecting early can potentially reduce its outcome of In-Hospital Mortality. Methodology: The descriptive case series study was conducted from January 2019 to June 2019 at Gastroenterology ward of Asian Institute of Medical Sciences, Hyderabad. All cases of CLD having spontaneous bacterial peritonitis and either of gender were enrolled in the study. Cirrhosis stages were assessed by CTP (Child-Turcot-Pugh) score and MELD (Model for End-stage Liver Disease) score. Serum levels of albumin were taken under the supervision of well-trained staff nurse(s) by following all guideline protocols and taking care of every risk factor and error for minimizing the bias results. The data was analyzed using SPSS version 20. Results: Mean age of the patients was 47.56±13.069 years. sampled serum albumin levels were found less than 2.0mg/dl have much higher in-hospital mortality rate compared to those having serum albumin levels more than 3.0mg/dl. There were also significant differences in outcome (in-hospital mortality) seen among the gender groups: Male vs. female; in-hospital death 42/165 vs. 19/58 (p-0.283). Hospital mortality was significantly high in CTP-C patients compared to CTP-B patients (59/185 vs. 02/38, (p-0.001). Early detection of high-risk patients is critical for better prognosis. Conclusion: It was observed that, there is a high prevalence of cirrhosis and its complications and a significant relationship of in-hospital mortality among patients of SBP and their SAAG (serum Albumin ratios), which clearly signifies that those patients who have serum Albumin ratio lesser than 2.0mg/dl have higher in-hospital mortality as compared to those have serum Albumin ratio lesser than 2.1-3.1mg/dl and more than 3.0mg/dl. Clinical significance of multiple biomarker detection of ascites

Abstract:Objective To study the levels and clinical significance of cytokines,C-reactive protein(CRP),carcinoembryonic antigen(CEA),lactate dehydrogenase(LDH) in ascites from patients with spontaneous bacterial peritonitis(SBP) complicated liver cirrhosis,malignant tumors,or kidney disease.Methods In 69 patients(26 with SBP,31 with malignant tumors,12 with kidney disease) registered to First Affiliated Hospital of Zhejiang University Medical College,the levels of IL-6,TNF-α and VEGF were measured by enzyme-linked immunosorbent assay(ELISA),and CRP,CEA,LDH and other biomarkers were detected by Abbott Ci8200 automatic analyzer.Results The levels of TNF-α,IL-6 and CRP in SBP ascites were significantly higher than those in nephrogenic or malignant ascites(P<0.01 or P<0.05).Compared with SBP and nephrogenic ascites group,the levels of VEGF,CEA and LDH in patients with malignant tumors were significantly increased(all P<0.01).The level of CRP in malignant ascites was significantly higher than that in nephrogenic ascites(P<0.01).The sensitivity and specificity were 90.3% and 76.3%,respectively,for combined detection of VEGF,CEA and LDH in diagnosis of malignant ascites;96.2% and 72.1% for combined detection of TNF-α,IL-6 and CRP in diagnosis of SBP ascites.Conclusion The elevated levels of TNF-α,IL-6 and CRP in ascites might favor identification of SBP.Measurement of VEGF,CEA and LDH in ascites may contribute to differential diagnosis between benign and malignant ascites.
Significance of Serum Procalcitonin and C-Reactive Protein in the Diagnosis and Prediction of Spontaneous Bacterial Peritonitis in Decompensated Chronic Liver Disease.

Abstract:The role of serum procalcitonin (PCT) and C-reactive protein (CRP) levels in the diagnosis of spontaneous bacterial peritonitis (SBP) with decompensated chronic liver disease (CLD) has been a subject of debate. The purpose of this cross-sectional, observational study was to evaluate the significance of CRP and PCT for the diagnosis and prediction of SBP in decompensated CLD patients. Fifty patients with ascites due to decompensated CLD were enrolled conveniently from the department of Gastrointestinal, Hepatobiliary and Pancreatic disorders (GHPD), BIRDEM General Hospital, Bangladesh from July 2019 to July 2020. Of these decompensated CLD patients with SBP were enrolled as the case group and without SBP as control group. Diagnostic and predictive value of PCT and CRP were calculated using the different statistical analysis. Among 50 patients, SBP was diagnosed in 9 patients (18.0%). The ROC analysis results yielded that the optimum cut off value for PCT was 0.67ng/ml and sensitivity, specificity, positive predictive value, negative predictive value, accuracy, AUC were 88.9%, 90.2%, 66.6%, 97.3, 90%, 0.947 respectively. On the contrary the optimum cut off value for CRP was 57.4mg/L and sensitivity, specificity, positive predictive value, negative predictive value, accuracy, AUC were 77.8%, 85.4%, 53.8%, 94.5%, 84%, 0.859 respectively. Our results indicate that the value of serum PCT and CRP were reliable to diagnose SBP in ascites due to decompensated CLD. Serum PCT and CRP level measurements may provide an early good diagnostic test for SBP in decompensated CLD patients.

Correlation of Serum Liver Fibrosis Markers with Severity of Liver Dysfunction in Liver Cirrhosis: a Retrospective Cross-Sectional Study.

Abstract:Hyaluronic acid (HA), laminin (LN), amino-terminal pro-peptide of type III pro-collagen (PIIINP), and collagen IV (CIV) are four major serum markers of liver fibrosis. This retrospective cross-sectional study aimed to evaluate the correlations of the four serum markers with the severity of liver dysfunction in cirrhotic patients. Between January 2013 and June 2014, a total of 228 patients with a clinical diagnosis with liver cirrhosis and without malignancy underwent the tests of HA, LN, PIIINP, and CIV levels. Laboratory data were collected. Child-Pugh and model for the end-stage of liver diseases (MELD) scores were calculated. Of them, 32%, 40%, and 18% had Child-Pugh class A, B, and C, respectively. MELD score was 7.58±0.50. HA (coefficient r: 0.1612, P=0.0203), LN (coefficient r: 0.2445, P=0.0004), and CIV (coefficient r: 0.2361, P=0.0006) levels significantly correlated with Child-Pugh score, but not PIIINP level. Additionally, LN (coefficient r: 0.2588, P=0.0002) and CIV (coefficient r: 0.1795, P=0.0108) levels significantly correlated with MELD score, but not HA or PIIINP level. In conclusions, HA, LN, and CIV levels might be positively associated with the severity of liver dysfunction in cirrhotic patients. However, given a relatively weak correlation between them, our findings should be cautiously interpreted and further validated.

Abstract:OBJECTIVE: The relation between excessive prolactin and endometriosis-related infertility is debatable. Anovulation or defective luteal phase occurs frequently due to hyperprolactinemia in subfertile women. In this investigation, we evaluated the association between serum prolactin levels and the severity of endometriosis.METHODS: This retrospective cohort study carried out at the Babol Infertility Research Center looked into the baseline serum prolactin levels of 114 infertile women with endometriosis and compared them to the levels seen in 101 infertile women without endometriosis (controls). Statistical analysis included independent t-test, chi-square, Welch test and ROC curve analysis.RESULTS: Infertile women with endometriosis had significantly higher serum prolactin levels than infertile women without endometriosis (p=0.003). A significant difference was detected between controls and individuals with endometriosis stages III/IV (p-value=0.009). Prolactin was found to have diagnostic value to detect endometriosis stages III/IV vs. stages I/II in AUC=0.65, 95% CI (0.55, 0.76). Prolactin values with a cut off set at 20.08 ng/mL had a sensitivity of 0.74 and specificity of 0.54 in detecting disease stages III/IV vs. I/II. The prognostic capability of prolactin in detecting endometriosis in cases vs. controls by ROC curve analysis had an AUC=+0.67, 95% CI (0.60, 0.74). Prolactin values with a cut off set at 17.5 ng/mL had a sensitivity of 0.64 and specificity of 0.63 in segregating subjects with and without endometriosis.CONCLUSION: Higher prolactin levels were observed in infertile women with more severe endometriosis when compared to infertile women without endometriosis. Prolactin levels act as a probable prognostic biomarker to detect endometriosis stages III/IV vs. I/II and segregate infertile women with endometriosis from subjects without endometriosis.

Abstract:Background: Liver cirrhosis is one of the most common causes of morbidity and mortality. The availability of liver transplant has stressed on the need for accurate prognostication. Various scoring systems have been developed for the same and studies have been conducted to find the correlation of various biochemical parameters with these.Methods: This is a cross sectional study conducted on 100 patients with stigmata of liver cell failure on clinical examination and substantiated by imaging. Serum Uric acid and other biochemical parameters were determined. Child Turcotte Pugh Score, Model for End Stage Liver Disease (MELD) score, United Kingdom Model for End Stage Liver Disease (UKELD) score was calculated and the correlation obtained.Results: The study showed significant, positive correlation between uric acid level and CTP, MELD and UKELD score. The study also showed the positive correlation of serum uric acid with various biochemical parameters such as total bilirubin, Prothrombin time/ International Normalized Ratio (PT/INR) and serum creatinine and negative correlation with serum albumin, with a significant p value. The mean serum uric acid was found to be 4.79(4.79± 2.0)Conclusions: The study showed a correlation between serum uric acid and the various available scoring systems such as CTP score, MELD and UKELD score. Hence serum uric acid can be used as an alternative prognostic parameter in predicting the severity and prognosis of cirrhosis of liver.
Correlation between both Child Pugh Score and Spontaneous Bacterial Peritonitis with Plasma D-Dimer Level in Cirrhotic Patients

Abstract:Background: Cirrhosis is a chronic liver disease in which diffuse destruction and regeneration of hepatic parenchymal cells have occurred. D-dimer is regarded as a diagnostic marker for Portal vein thrombosis in liver cirrhosis. Objective: To correlate plasma D-dimer levels as a hemostatic parameter with Child Turcotte Pugh (CTP) score of post hepatitis cirrhosis patients and occurrence of spontaneous bacterial peritonitis (SBP). Patients and methods: This was a case-control study that included 84 patients with hepatitis C-related cirrhosis who were admitted to Al-Ahrar Teaching Hospital in the period from November 2020 to June 2021. 28 apparently healthy subjects were included as control group. Results: There was a significantly higher level of D-dimer in each cirrhotic group; child A, child B and child C with median of 1.05 mg/l, 2 mg/l and 3 mg/l respectively compared to healthy control group with a median of 0.09 mg/l. The best cutoff of D dimer in prediction of Child C was ≥ 2.45 mg/l with area under curve of 0.986, sensitivity of 96.4%, specificity of 94%, positive predictive value (PPV) of 84.4%, negative predictive value of 98.8 % and accuracy of 94.6% (p < 0.001). There was a statistically significant association between SBP and D dimer, which was significantly higher among those with SBP.
Serum Prolactin Levels and Mortality in Adults Without Prolactinoma: A Meta-Analysis

Abstract:Abstract Context Prolactin (PRL) is a highly versatile, multifunctional hormone synthesized and secreted by lactotroph cells of the anterior pituitary. Its metabolic role has been extensively studied even in normoprolactinemic populations. Recently, a wealth of observational data have outlined the potential prognostic value of PRL in various clinical settings. Objective This systematic review aims to systematically evaluate and quantitatively synthesize the association between serum PRL levels and risk of mortality in adults without prolactinoma. Methods A systematic literature search was conducted up to June 10, 2023, to identify studies reporting the association of serum PRL levels with clinical outcomes of adults without prolactinoma. A random-effects meta-analysis was conducted to quantify the adjusted hazard ratios [(a)HRs] for all-cause and cardiovascular death (CVD) during follow-up. Results Twenty-eight studies were deemed eligible reporting the outcomes of adults without prolactinoma, in whom serum PRL levels were measured for risk-stratification. Fourteen studies reported appropriate data for meta-analysis encompassing a total of 23 596 individuals. Each unit of PRL increase was independently associated with increased risk of all-cause (pooled aHR = 1.17 [1.08-1.27]; I2 = 48%) and CV mortality (pooled aHR = 1.54 [1.14-2.09]; I2 = 89%). Individuals belonging to the highest PRL category had significantly higher risk for all-cause (pooled aHR = 1.81 [1.43-2.30]; I2 = 65%) and CV (pooled aHR = 1.59 [1.04-2.42]; I2 = 82%) mortality compared to their lowest-PRL category counterparts. The association between PRL levels and in-hospital death did not reach statistical significance. Conclusion PRL levels seem to be an independent predictor for mortality. Further validation is warranted before its role as a risk-stratification tool can be delineated in clinical practice.

Diagnostic value of PCT in patients with liver cirrhosis complicated with spontaneous bacterial peritonitis

Abstract:Objective To investigate the diagnostic value of blood procalcitonin (PCT) and ascites PCT in patients with cirrhosis and spontaneous bacterial peritonitis (SBP). Methods From March 2016 to March 2018, 127 patients with cirrhotic ascites in the Third People's Hospital of Yangquan were selected in the research, 35 cases combined with SBP were selected as observation group, and 92 cases without SBP were selected as control group.The ascites white blood cells (WBC), neutrophils (PMN), PCT, blood WBC and PCT were detected and compared.The diagnostic efficiency of plasma PCT and ascites PCT in patients with cirrhosis combined with SBP was analyzed. Results The ascites levels of PCT, WBC, PMN, plasma PCT and WBC in the observation group were (1.15±0.38)ng/mL, (0.85±0.22)×109/L, (0.29±0.11)×109/L, (0.95±0.24)ng/mL, (11.83±3.21)×109/L, respectively, which in the control group were (0.60±0.26)ng/mL, (0.18±0.08)×109/L, (0.05±0.02)×109/L, (0.24±0.08)ng/mL, (6.21±2.05)×109/L, respectively, the differences were statistically significant (t=4.254, 5.382, 7.325, 5.638, 4.992, all P<0.05). The plasma PCT was positively correlated with the ascites PMN, the ascites PCT and the ascites WBC (r=0.658, 0.624, 0.598, all P<0.05), and the ascites PCT was positively correlated with the ascites PMN and the ascites WBC (r=0.523, 0.693, all P<0.05). The area under the ROC curve of plasma PCT and ascites PCT in the diagnosis of cirrhosis combined with SBP was 0.901 and 0.864, respectively, with the best thresholds of 0.92ng/mL and 1.01ng/mL, respectively.The sensitivity(89.6%) and specificity(87.2%) of plasma PCT in diagnosis of cirrhosis combined with SBP were higher than those of ascites PCT (85.7% and 82.9%). Conclusion In the diagnosis of cirrhosis combined with SBP, plasma PCT and ascites PCT have high diagnostic potency, and plasma PCT has higher diagnostic value. Abstract:Background & Aims: The aim of the study was to evaluate serum c-reactive protein (CRP), ascitic procalcitonin (PCT) and monocyte chemotactic protein-1 (MCP-1) in the diagnosis of spontaneous bacterial peritonitis (SBP) in cirrhotic patients. Methods: A cross-sectional analytic study that included 199 patients with decompensated cirrhosis (101 with SBP and 98 without SBP). Patients were classified according to Child-Pugh criteria. Ascitic PCT and MCP-1 were measured by enzyme-linked immunosorbent assay. Serum CRP, liver and renal functions were assessed. Results: Three markers are significantly elevated in SBP patients than those without ascites. Using the ROC curve at AUC 0.883 and a cut-off value of >186 ng/ml, the diagnostic performance of ascitic MCP-1 level was higher than CRP (AUC 0.562) and ascitic fluid procalcitonin (AUC 0.751) in the diagnosis of SBP. The sensitivity and specificity were 86.15% and 79.59% at the cutoff of 186 ng/ml for MCP-1, 65.4 and 75.5 at ≥ 1 ng/ml for PCT, and 52.5 and 64.3, respectively for at 11.2 mg/dl CRP. Conclusion: Ascitic MCP-1 has a better diagnostic value with higher sensitivity and specificity in diagnosis SBP compared to CRP and procalcitonin which has higher diagnostic accuracy than CRP. Further studies with a large number will be necessary to evaluate the usefulness of these markers in diagnosis, follow-up and relation to morbidity and mortality of SBP patients.
A Study of Spontaneous Bacterial Peritonitis in Cirrhosis of Liver with Ascites at a Tertiary Care Hospital in North India: Prevalence, Clinical, and Microbiological Profile

Abstract:Abstract Background and Objectives: In patients with chronic liver disease and ascites, spontaneous bacterial peritonitis (SBP) is a common complication. This can progress gradually and subtly or go undiagnosed until signs start to show. Early diagnosis is essential for effective treatment and the prevention of subsequent episodes because the mortality rate following a single episode varies from 20% to 40%. The goal was to study the incidence, clinical profile, and microbiological characteristics of SBP and its variants in people with liver cirrhosis and ascites. Materials and Methods: This observational study was done on 104 cirrhotic patients with ascites admitted at a tertiary care hospital. Basic demographics, symptoms, and clinical signs of patients were recorded. Diagnostic paracentesis was done aseptically and immediately sent for ascitic fluid cytology, biochemistry, and culture. Bacterial examination and antibiotic sensitivity tests were carried out by standard microbiological techniques. Results: Out of a total of 104 cirrhotic patients, 28 (26.9%) were diagnosed with SBP. Alcohol was the most common etiological factor, followed by hepatitis C infection. Among the SBP patients, the most common presentation was culture-negative neutrocytic ascites (71.4%), followed by classical SBP (17.8%), then mono bacterial non neutrocytic bacterascites MNB (10.7%). Among the symptoms, the most common was abdominal distension, followed by icterus, pedal edema, and abdominal pain. Majority of the patients belonged to Child–Pugh’s Grade C. Of 28 cases of SBP, 8 were culture positive. Gram-negative isolates were predominant (75%) and Escherichia coli were the most common isolate. Interpretation and Conclusion: The prevalence of SBP in cirrhotic patients was 26.9%, and Gram-negative isolates were predominant. Ascitic fluid culture and susceptibility testing can lead to accurate diagnosis of SBP and can guide for treatment as resistance to antibiotics is common.
The serum-ascites vitamin D gradient: a diagnostic marker for spontaneous bacterial peritonitis in Egyptian cirrhotic patients

Abstract:Background The most prevalent infectious consequence of liver cirrhosis is spontaneous bacterial peritonitis (SBP). Ascitic fluid polymorph nuclear neutrophils (PMN) counts of more than 250 cells/μL are used as the gold standard for diagnosing SBP. Patients with decompensated cirrhosis also had significantly decreased serum 25-OH vitamin D levels compared with controls. Aim Our aim is to investigate the role of (serum ascites vitamin D gradient) as a diagnostic factor for diagnosis of SBP. Patients and method This cross-sectional study included 80 cirrhotic ascitic patients from Egypt who were treated at the hospitals affiliated with the Tropical Medicine and Infectious Diseases Department at Tanta University. Group 1: 40 ascitic patients with SBP. Group 2: 40 ascitic patients without SBP History taking, clinical examination, laboratory analysis, and imaging studies were applied to every patient. Results Child-Pugh score was significant in both groups as child C was significantly higher in group 1 than group 2. Total leucocyte count and differential neutrophil were significantly higher in group 1 than group 2 while child B was higher in group 2 than group 1. Differential lymphocyte was significantly lower in group 1 than group 2. SADG was significantly lower in group 1 than group 2. SADG ratio can significantly predict SBP ( P = 0.036 and AUC = 0.632) at cut-off less than or equal to −25.8 with 37.50% sensitivity, 95.00% specificity, 88.2% positive predictive value (PPV), and 60.3% negative predictive value (NPV). Conclusion Patients with cirrhotic ascites had decreased SADG and higher SBP. In these cases, it might be used as a diagnostic and prognostic marker for SBP. Patients with decompensated cirrhosis, independent of SBP, have an increased prevalence of vitamin D insufficiency.
Implant–bone interface healing and adaptation in resurfacing hip replacement

Abstract:Hip resurfacing demonstrates good survivorship as a treatment for young patients with osteoarthritis, but occasional implant loosening failures occur. On the femoral side there is radiographic evidence suggesting that the implant stem bears load, which is thought to lead to proximal stress shielding and adaptive bone remodelling. Previous attempts aimed at reproducing clinically observed bone adaptations in response to the implant have not recreated the full set of common radiographic changes, so a modified bone adaptation algorithm was developed in an attempt to replicate more closely the effects of the prosthesis on the host bone. The algorithm features combined implant–bone interface healing and continuum bone remodelling. It was observed that remodelling simulations that accounted for progressive gap filling at the implant–bone interface predicted the closest periprosthetic bone density changes to clinical X-rays and DEXA data. This model may contribute to improved understanding of clinical failure mechanisms with traditional hip resurfacing designs and enable more detailed pre-clinical analysis of new designs.
Thyroid Profile in Liver Cirrhosis Patients-Correlation with Disease Severity and Recommendations for Clinical Practice: A Cross-sectional Observational Study

Abstract:Introduction: Liver diseases are increasingly recognized as significant public health concerns in India, prompting investigations into novel approaches for assessing disease severity and prognosis. Recognizing the potential utility of thyroid hormone levels in these assessments, we conducted an observational cross-sectional study at our tertiary care hospital. Our study included 89 patients aged 12 years and above, admitted to medicine wards with ultrasound-diagnosed liver cirrhosis, excluding pregnant women and those on thyroid-altering medications. Observations: Our findings revealed a male-to-female ratio of 4.23:1, with the majority of patients falling within the 40-60 age-group, averaging 46.93 years. Notably, 87.6% of patients exhibited thyroid abnormalities, primarily low free T3 (FT3) syndrome and subclinical hypothyroidism. Classifying patients according to Child-Pugh (CP) score, 2.2% were CP class A, 22.5% were CP class B, and the remaining 75.3% were CP class C. Across all CP classes, low FT3 syndrome was prevalent, particularly in CP class C. Correlations between thyroid hormone levels and liver disease severity, assessed via CP and model for end-stage liver disease (MELD) scoring systems, were observed. Specifically, FT3 levels demonstrated a negative correlation with liver disease severity (p = 0.001), while no significant correlations were found for free T4 (FT4) and thyroid-stimulating hormone (TSH) levels. Based on our findings, we recommend routine thyroid function testing for all liver cirrhosis patients, irrespective of disease severity, to facilitate early detection and intervention. However, our study had limitations, including a small sample size and a precision error of 10% due to resource constraints for thyroid function testing. Moreover, reliance solely on ultrasound for liver cirrhosis diagnosis may lead to missed diagnoses, highlighting the need for complementary noninvasive tests such as FibroScan and aspartate aminotransferase to platelet ratio index (APRI) scores. Conclusion: Our study underscores the importance of considering thyroid function in the management of liver cirrhosis patients and provides valuable insights for enhancing clinical practice in this context.
The Association Between Prolactin Levels and Cognitive Function in Female Patients With Severe Mental Disorders

Abstract:Objective: Cognition impairments are considered as a fundamental characteristic of severe mental disorders (SMD). Recent studies suggest that hyperprolactinemia may exert a detrimental influence on cognitive performance in patients with SMD. The objective of this study was to investigate the correlation between serum prolactin levels and cognitive function in female individuals diagnosed with SMD. Methods: We conducted a study on 294 patients with SMD and 195 healthy controls, aged between 14 to 55 years old. Cognitive function was assessed using the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS), while prolactin levels were measured in serum. Descriptive analysis and comparative analysis were performed to compare cognitive function and prolactin levels between groups, and linear regression models were used to explore the relationship between prolactin and cognitive function. Results: Compared to the healthy control, individuals with SMD exhibited significantly higher levels of prolactin, while scoring lower on RBANS total and every index scores. Furthermore, a negative association between prolactin levels and cognitive function (RBANS total index score, attention, and delayed memory) was observed in SMD patients. Importantly, this inverse correlation between prolactin and cognition function (RBANS total index score, total scale score, and attention) persisted in patients who were not taking medications that could potentially influence serum prolactin levels. Conclusion: Our study reveals a significant correlation between elevated prolactin levels and cognitive impairment in female patients with SMD, underscoring the importance of monitoring prolactin levels in order to prevent cognitive deterioration among female SMD patients.
Serum prolactin levels in psoriasis and its association with disease activity: A case-control study

Abstract:BACKGROUND: Psoriasis is a T-cell-mediated autoimmune chronic skin disorder in which an environmental factor, perhaps a viral antigen, induces T cells to produce cytokines. These cytokines stimulate keratinocyte proliferation and production of antigenic adhesion molecules in the dermal blood vessels. Several mediators and hormones have been implicated in keratinocyte hyperproliferation and among these hormones, prolactin (PRL) has been found to have an effect on epithelial cells, lymphocytes and keratinocytes, thus an effect on the etiopathogenesis of psoriasis.AIM: The present study was designed to compare serum PRL levels in psoriatic patients with a control group.SETTINGS AND DESIGN: This study was a hospital-based case control study, conducted in the department of Dermatology, STD and Leprosy, SMHS Hospital (Associated teaching hospital of Government Medical College Srinagar) over a period of 1 year, from September 2012 to 2013.MATERIALS AND METHODS: The present study included 60 patients of psoriasis (42 males and 18 females) and 60 controls matched for age and sex. Serum PRL levels of patients and controls were measured by ECLIA and inferences were drawn.STATISTICAL ANALYSIS USED: Statistical significance of the results was carried out by the Chi-square test and the independent samples t-test. Statistical significance was determined at a level of P < 0.05.RESULTS: Serum PRL levels were significantly increased in patients as compared to the control group (P value: 0.002). There was a positive correlation between pretreatment serum PRL levels and PASI score (r value: 0.379; P value: 0.003). An insignificant association was found between the pretreatment PRL level and serum PRL level after treatment (P value: 0.22). Also, a negative correlation between the duration of psoriasis and serum PRL was seen (r value: -0.008; P value: 0.954).CONCLUSION: PRL may have a role to play in the etiopathogenesis of psoriasis. However, further studies with large sample size should be carried out so as to validate this hypothesis.

Abstract:BackgroundChronic HBV infection is a major cause of Cirrhosis and an important risk factor to develop hepatocellular carcinoma. The study is conducted to find out the changes in the lipid metabolism of HBV-cirrhosis patients.MethodsIn the present study, serum lipid profiles of patients with HBV-cirrhosis were assessed by utilizing micro-lab and gas chromatography, while risk factors for transmission of HBV-cirrhosis studied through the standard questionnaire.ResultsThe epidemiological and etiological risk factors strongly associated with HBV-cirrhosis patients compared to controls, included as family history, shave from the barber, blood transfusion (without proper screening), mutual sharing of household contents, positive surgery history, and dental treatment. The HBV-cirrhosis patients have significantly lower level (p < 0.001) of lipid profile including total cholesterol (96.65 mg/dl), TAG (82.85 mg/dl), VLDL-C (16.57 mg/dl), LDL-C (68.27 mg/dl), HDL-C (27 mg/dl) and total lipid (424.76 mg/dl) in comparison to controls, indicating hypolipidemia in patients. The MELD score indicated mild prognostic values of the hepatic function for the study group. The result of total fatty acid composition of HBV-cirrhotic patients with comparison of control subjects reveals that palmitic (24.54 g/100 g) and palmitoleic acid (4.65 g/100 g) were significantly (p < 0.05) higher whereas eicosatrienoic (0.09 g/100 g), arachidonic (3.57 g/100 g), linoleic (22.75 g/100 g) and α-linolenic acid (0.12 g/100 g) were significantly lower. Marker for stearoyl-CoA desaturase (SCD = ∆9-desaturase) activity i.e. palmitoleic: palmitic (0.2) and oleic: stearic acid (1.5) ratios, originated higher in HBV-cirrhotic patients, while PUFA: SFA (0.6) was lower in HBV-cirrhosis patients as compared with control subjects. The serum SFA and MUFA were increased while PUFA were reduced in both total and free form.ConclusionPresent study concluded that hypolipidemia observed in HBV-cirrhosis patients, MELD were found to be independent predictors of survival and alteration in fatty acid composition, possibly due to impairment in fatty acid metabolism by enzymatic elongation and desaturation.

Abstract:BACKGROUND/AIM: This study intends to determine the correlation of a patient's hepatic venous pressure gradient (HVPG) measurement with six factors: Child-Turcotte-Pugh (CTP) score, model for end-stage liver disease (MELD) score, presence of ascites, size of varices, presence of variceal bleeding, and an etiology of cirrhosis. The study also aims to identify the predictors of higher HVPG measurements that can indirectly affect the prognosis of cirrhotic patients.PATIENTS AND METHODS: Thirty patients diagnosed with cirrhosis were enrolled prospectively and each patient's HVPG level was measured by the transjugular catheterization of the right or middle hepatic vein. The wedged hepatic venous pressure (WHVP) and free hepatic venous pressure (FHVP) were measured using a 7F balloon catheter. The HVPG level was calculated as the difference between the WHVP and FHVP measurements.RESULTS: The mean HVPG level was higher in alcoholic than in nonalcoholic cirrhosis (19.5 ± 7.3 vs 15.2 ± 4.5 mm Hg, P = 0.13). The mean HVPG was also higher in bleeders compared with nonbleeders (18.5 ± 5.3 vs 10.7 ± 3.1 mmHg, P = 0.001). Patients with varices had a higher mean HVPG level than those without varices (17.4 ± 5.8 vs 11.7 ± 3.9 mmHg, P = 0.04). The difference among the three categories of varices (small, large, and no varices) was statistically significant (P = 0.03). In addition, the mean HVPG level was higher in patients with ascites than in those without ascites (18.7 ± 4.7 vs 11 ± 5.3 mmHg, P = 0.002), and it was significantly higher in patients in CTP class C (21.8 ± 5.5 mmHg) as compared with those in CTP class B (16.9 ± 2.9 mmHg) and CTP class A (10.5 ± 4.1 mmHg; P ≤ 0.001).CONCLUSION: HVPG levels were significantly higher in patients in CTP class C as compared with those in CTP classes A and B, thereby indicating that an HVPG measurement correlates with severity of liver disease. A high HVPG level signifies more severe liver disease and can predict the major complications of cirrhosis.
