Abstract:Second-generation antipsychotics (SGAs) are widely used in treating schizophrenia and related disorders, also other mental disorders. However, the efficacy and safety of SGAs for treating other mental disorders is unclear. A systematic literature search for randomized, placebo-controlled trials of 11 SGAs for treating 18 mental disorders apart from schizophrenia were carried out from database inception to April 3, 2022. The primary outcome was the mean change in the total score for different mental disorders. The secondary outcome was the odds ratio of response, remission rates and risk ratio (RR) of adverse events (AEs). A total of 181 studies (N = 65480) were included. All SGAs showed significant effects in treating other mental disorders compared with placebo, except autistic disorder and dementia. Aripiprazole is the most effective treatment for bipolar mania [effect size = -0.90, 95% CI: -1.59, -0.21] and Tourette's disorder [effect size = -0.80, 95% CI: -1.14, -0.45] , olanzapine for bipolar depression [effect size = -0.86, 95% CI: -1.32, -0.39] and post-traumatic stress disorder [effect size = -0.98, 95% CI: -1.55, -0.41], lurasidone for depression [effect size = -0.66, 95% CI: -0.82, -0.50] , quetiapine for anxiety [effect size = -1.20, 95% CI: -1.96, -0.43] , sleep disorders [effect size = -1.2, 95% CI: -1.97, -0.58] , and delirium [effect size = -0.36, 95% CI: -0.70, -0.03] , and risperidone for obsessive-compulsive disorder [effect size = -2.37, 95% CI: -3.25, -1.49] , respectively. For safety, AE items for each SGAs was different. Interestingly, we found that some AEs of OLZ, QTP, RIS and PALI have significant palliative effects on some symptoms. Significant differences in the efficacy and safety of different SGAs for treatment of other mental disorders should be considered for choosing the drug and for the balance between efficacy and tolerability for the specific patient.

Pharmacological Treatment of Neuropsychiatric Symptoms of Dementia: a Network Meta-Analysis.

Abstract:Background Pharmacological treatments are very common to be used for alleviating neuropsychiatric symptoms (NPS) in dementia. However, decision on drug selection is still a matter of controversy. Aims To summarise the comparative efficacy and acceptability of currently available monotherapy drug regimens for reducing NPS in dementia. Method We searched PubMed, MEDLINE, EMBASE and Cochrane Central Register of Controlled Trials between inception and 26 December 2022 without language restrictions; and reference lists scanned from selected studies and systematic reviews. Double-blind randomised controlled trials were identified from electronic databases for reporting NPS outcomes in people with dementia. Primary outcomes were efficacy and acceptability. Confidence in the evidence was assessed using Confidence in Network Meta-Analysis (CINeMA). Results We included 59 trials (15,781 participants; mean age, 76.6 years) and 15 different drugs in quantitative syntheses. Risperidone (standardised mean difference [SMD] -0.20, 95% credible interval [CrI] -0.40 to -0.10) and galantamine (-0.20, -0.39 to -0.02) were more effective than placebo in short-term treatment (median duration: 12 weeks). Galantamine (odds ratio [OR] 1.95, 95% CrI 1.38-2.94) and rivastigmine (1.87, 1.24-2.99) were associated with more dropouts than placebo, and some active drugs. Most of the results were rated as low or very low according to CINeMA. Conclusions Despite the scarcity of high-quality evidence, risperidone is probably the best pharmacological option to consider for alleviating NPS in people with dementia in short-term treatment when considering the risk-benefit profile of drugs.

Abstract:Psychotic symptoms of dementia are highly prevalent and lead to poor medical outcomes and substantial dysfunction. To date, which drug to use remains controversial without a summary of all direct or indirect comparisons of pharmacotherapy. Therefore, we conducted a systematic review with pairwise and network meta-analysis to examine efficacy and tolerability outcomes of pharmacological treatments in dementia patients. MEDLINE, Cochrane Library, EMBASE, and PubMed were searched systematically up to August 31, 2020. We included trials of cholinesterase inhibitors (ChEIs), memantine, antipsychotics, antidepressants, and mood stabilizers, with final approval from the U.S. Food and Drug Administration. We ranked the comparative effects of all drugs against placebo with surface under the cumulative ranking (SUCRA) probabilities. This analysis is based on 34 trials, which included 10,415 patients randomly assigned to 15 commonly used drug regimens. Donepezil (standardized mean difference [SMD] -0.30, 95% credible interval [CrI] -0.50 to -0.12; SUCRA, 0.85), memantine (SMD -0.20, 95%CrI -0.34 to -0.07; SUCRA, 0.68) and aripiprazole (SMD -0.17, 95% CrI -0.32 to -0.02; SUCRA, 0.62) showed greater benefit than placebo, and with relatively good tolerability in network meta-analyses. Risperidone was also found to be more efficacious than placebo (SMD -0.16, 95% CrI -0.28 to -0.05; SUCRA, 0.60), but with poor tolerability (odds ratios [OR] 1.50, 95% CrI 1.06-2.26). Donepezil, memantine, haloperidol, aripiprazole and risperidone were more efficacious than quetiapine (SMDs ranged from -0.36 to -0.22). Besides, donepezil, memantine and mirtazapine were more efficacious than sertraline (SMDs ranged from -0.47 to -0.36). Most of the results were rated as "low" to "very low". Several effective treatment choices for psychotic symptoms are available across drug classes. Donepezil, memantine and aripiprazole are probably the appropriate options to consider when a pharmacological treatment is indicated. Given the limitations of the meta-analytic approach and the low methodological quality of the majority of studies, our results should be cautiously interpreted.

Abstract:Objectives: Olanzapine and risperidone have emerged as the most widely used drugs as short-term prescription in the treatment of behavioral disturbances in dementia. The present systematic review and meta-analysis was hence performed to investigate the effectiveness and safety profile of olanzapine and risperidone in the treatment of behavioral and psychological symptoms of dementia (BPSD), aiming to provide updated suggestion for clinical physicians and caregivers. Design: Prospective controlled clinical studies were included, of which available data was extracted. Outcomes of BEHAVE-AD scores with the variation of grades, specific behaviors variables, as well as safety signals were pooled for the analysis by odds rates and weighted mean differences, respectively. Data sources: Medline, Embase, Cochrane Library, China National Knowledge Infrastructure (CNKI), and WanFang. Eligibility criteria: Prospective, controlled clinical studies, conducted to compare the effectiveness and safety profile of olanzapine and risperidone in the treatment of BPSD. Data extraction and synthesis: Interested data including baseline characteristics and necessary outcomes from the included studies were extracted independently by 2 investigators. BEHAVE-AD scale was adopted to assess the efficacy in the present study. All behaviors were evaluated at the time of the initiation of the treatment, as well as the completion of drugs courses. Adverse events were assessed with the criteria of Treatment Emergent Symptom Scale, or Coding Symbols for a Thesaurus of Adverse Reaction Terms dictionary. Weighted mean difference was used for the pooled analysis. Results: A total of 2427 participants were included in the present meta-analysis. Comparative OR on response rate, and remarkable response rate between olanzapine and risperidone was 0.65 (95% CI: 0.51-0.84; P = .0008), and 0.62 (95% CI: 0.50-0.78; P < .0001), respectively. There were statistical differences observed by olanzapine on the improvement of variables including delusions (WMD, -1.83, 95% CI, -3.20, -0.47), and nighttime behavior disturbances (WMD, -1.99, 95% CI, -3.60, -0.38) when compared to risperidone. Conclusion: Our results suggested that olanzapine might be statistically superior to risperidone on the reduction of BPSD of Alzheimer's disease, especially in the relief of delusions and nighttime behavior disturbances. In addition, olanzapine was shown statistically lower risks of agitation, sleep disturbance, and extrapyramidal signs.
Early Diffusion Weighted MRI as a Negative Predictor for Disabling Stroke After ABCD2 Score Risk Categorization in Transient Ischemic Attack Patients

Abstract:Background and Purpose— The prognostic value early diffusion-weighted magnetic resonance imaging (DWMRI) adds in the setting of transient ischemic attack (TIA), after risk stratification by a clinical score, is unclear. The purpose of this study is to evaluate, after ABCD2 score risk categorization in admitted TIA patients, whether negative DWMRI performed within 24 hours of symptom onset improves on the identification of patients at low risk for experiencing a disabling stroke within 90 days. Methods— At 15 North Carolina hospitals, we enrolled a prospective nonconsecutive sample of admitted TIA patients. We excluded patients not undergoing a DWMRI within 24 hours of admission and patients for whom a dichotomized (≤ or >3) ABCD2 score could not be calculated. We conducted a medical record review to determine disabling ischemic stroke outcomes within 90 days. Results— Over 35 months, 944 TIA patients met inclusion criteria, of whom 4% (n=41) had a disabling ischemic stroke within 90 days. In analyses stratified by low versus moderate/high ABCD2 score, the combination of a low risk ABCD2 score and a negative early DWMRI had excellent sensitivity (100%, 95% CI 34 to 100) for identifying low-risk patients. In patients classified as moderate to high risk, a negative early DWMRI predicted a low risk of disabling ischemic stroke within 90 days (sensitivity 92%, 95% CI 80 to 97; NLR 0.11, 95% CI 0.04 to 0.32). Conclusions— After risk stratification by the ABCD2 score, early DWMRI enhances the prediction of a low risk for disabling ischemic stroke within 90 days. Further study is warranted in a large, consecutive TIA population of early DWMRI as a sensitive negative predictor for disabling stroke within 90 days.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Introduction</h3><p>We investigated the efficacy and acceptability of pharmacotherapy for dementia with Lewy bodies (DLB) while simultaneously considering the neuropsychiatric symptoms (NPS), cognitive function, motor symptoms, and acceptability.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Electronic databases were searched from inception through June 5, 2019, for randomized controlled trials (RCTs) and open-label trials (OLTs) in patients with DLB. We performed a pairwise conventional meta-analysis (PWMA) and network meta-analysis (NMA) within a frequentist framework. The main outcomes were mean change scores in NPS, general cognition, motor symptoms and acceptability. The effect sizes and odds ratios with 95% confidence intervals (CIs) were calculated. This study was registered with PROSPERO (CRD42018096996).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>In total, we included 29 studies (9 RCTs and 20 OLTs). In the NMA with 9 RCTs, both high- (mean difference [MD] 2.00, 95% CIs, 0.69 to 3.31) and low-dose (1.86, 0.58 to 3.15) donepezil were associated with a greater cognitive improvement than placebo. High-dose zonisamide was associated with greater motor symptom improvement ( -4.10, -7.03 to -1.17]). No medications reached statistical significance regarding improving neuropsychiatric symptoms or developing intolerable adverse effects as compared to placebo. In the second NMA, with 29 studies as an exploratory analysis, aripiprazole and yokukansan may be effective for neuropsychiatric symptoms, while levodopa may be associated with cognitive impairment.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>We report the most comprehensive evidence for the selection of pharmacotherapy for treating different clusters of DLB-related symptoms. Due to the limited availability of RCTs on DLB, more well-conducted RCTs are needed for MMA to warrant clinical efficacy in the future.</p>

Clinical Practice Guidelines for the Management of Behavioral and Psychological Symptoms of Dementia: A Systematic Review With AGREE II

Abstract:Background: High-quality clinical practice guidelines (CPGs) are important for the effective treatment of behavioral and psychological symptoms of dementia (BPSD). However, recommendations provided by different quality guidelines may lead to varied clinical practice outcomes. Objective: To assess the quality of available CPGs for the management of BPSD and summarize the best recommendations for treating BPSD. Methods: This was a systematic review of CPGs for the management of BPSD with data obtained from electronic databases and evaluated using the Appraisal of Guidelines for Research and Evaluation II instrument, consisting of six domains: "Scope and purpose", "Stakeholder involvement", "Rigor of development", "Clarity of presentation", "Applicability", and "Editorial independence". The criteria for high-quality guidelines were set as: the score of high-quality guidelines in the "Rigor of development" domain should be ≥60% and as well as a score of >60% in at least three other domains. High-quality guidelines were selected for recommendation extraction, and the final recommendations were formed in combination with the latest meta-analysis and randomized clinical-trial results. Results: In term of median scores in each domain for the six included CPGs, "Scope and purpose" (87.5%) scored better than all others, whereas "Applicability" (46.5%) was the domain with the lowest score. Four CPGs (2015 APA, 2018 NICE, 2018 CANADA, 2020 EAN) met the criteria of high-quality guidelines and were used to extract recommendations. From these four CPGs, nine specific recommendations related to the management of BPSD were summarized, of which seven were related to pharmacological treatment and two to non-pharmacological treatment. These recommendations covered the applicability of antipsychotic drugs, medication recommendations, withdrawal times, and several suitable non-pharmacological therapies. Conclusion: The quality of CPGs for the management of BPSD requires improvement, especially for the "Applicability" domain. For psychotic-like symptoms in dementia, the use of antipsychotics should be based on the individual's risk-benefit ratio, and the use of atypical antipsychotics seems to be a better choice. Abstract:A wide variety of atypical antipsychotic drugs (risperidone, olanzapine, quetiapine, aripiprazole, ziprasidone and clozapine) are widely used in the management of neuropsychiatric symptoms, which are commonly seen in dementia, but results from randomised controlled trials (RCTs) on the efficacy and safety of these agents are conflicting. We aimed to quantify the efficacy and safety of atypical antipsychotic drugs on neuropsychiatric symptoms in dementia patients.PubMed, EMBASE, the Cochrane Controlled Trials Register and the Cochrane Database of Systematic Reviews for reports published before August 2014 were searched for eligible randomized controlled trials of atypical antipsychotic drugs therapy in patients with psychotic symptoms of dementia. Two reviewers independently assessed the quality of the trials and extracted information.Overall, 23 relevant RCTs with 5,819 participants were identified. This meta-analysis demonstrated a significant efficacy of atypical antipsychotics on psychiatric symptoms and cognitive functions compared to placebo. In the meta-analysis, the weighted mean differences (WMDs) in change scores for psychiatric symptoms were in favor of aripiprazole (-4.4, 95% confidence interval (CI) - 7.04 to -1.77) and risperidone (-1.48, 95% CI -2.35 to -0.61) compared to placebo. In cognitive effects, WMDs in change scores for the Clinical Global Impression-Change (CGI-C) were in favor of aripiprazole, risperidone, olanzapine and quetiapine which ranged from a -0.30 points mean difference (95% CI:-0.59 to -0.01) in the aripiprazole trials to -0.43 (95% CI:-0.62 to -0.25) in the risperidone group. Patients receiving atypical antipsychotics showed no difference in risk for injuries or falls (P > 0.05), significantly higher risks (P < 0.05) for somnolence, urinary tract infection, edema and abnormal gait. However, there was no significant evidence for death reported.Aripiprazole and risperidone are able to improve psychiatric symptoms and slow decline in cognition function at average 12 weeks in patients with neuropsychiatric symptoms of dementia. However, high adverse events may offset the efficacy of atypical antipsychotics in dementia.

Efficacy of Brexpiprazole in Patients with Agitation Associated with Dementia Due to Alzheimer's Disease Analyzed by Patient Characteristics: A Post Hoc Pooled Analysis of Randomized, Fixed-Dose, Placebo-Controlled Trials

Abstract:Introduction Brexpiprazole is an efficacious and well-tolerated treatment for agitation associated with dementia due to Alzheimer's disease. To assist with treatment decision-making, it is valuable to explore treatment effects in different patient subgroups. These post hoc subgroup analyses explored the efficacy of brexpiprazole in patients with agitation associated with dementia due to Alzheimer's disease according to dementia severity, co-occurring neuropsychiatric symptoms, concomitant use of treatments for dementia or psychiatric conditions, and care setting. Methods This was a post hoc analysis of efficacy data from two 12-week, Phase 3, randomized, placebo-controlled trials of fixed-dose brexpiprazole in patients with agitation associated with dementia due to Alzheimer's disease (NCT01862640, NCT03548584). Data were pooled for brexpiprazole 2 or 3 mg/day treatment arms, and separately for placebo arms. Change in Cohen-Mansfield Agitation Inventory (CMAI) Total score from baseline to Week 12 (primary endpoint in each trial) was analyzed using a mixed model for repeated measures. Least squares mean change from baseline was compared between brexpiprazole and placebo arms in each subgroup. Responder analyses were also conducted, based on a meaningful within-patient change threshold of ≥20-point reduction from baseline to Week 12 in CMAI Total score. Baseline dementia severity was classed as mild/moderate (Mini Mental State Examination score ≥13) or severe (score ≤12). Presence of baseline co-occurring neuropsychiatric symptoms was defined as reported history of each of 11 neuropsychiatric symptoms associated with Alzheimer's disease at the screening visit. Concomitant use of treatments for dementia or psychiatric conditions was defined as use of memantine, cholinesterase inhibitor, or antidepressant during the trials, or a history of benzodiazepine or anxiolytic use prior to the trials. Setting was defined as community-based or long-term care facility. Results The analysis included 610 patients: 363 who received brexpiprazole 2 or 3 mg/day and 247 who received placebo. The least squares mean change in CMAI Total score at Week 12 was larger with brexpiprazole than placebo in subgroups with mild/moderate dementia and severe dementia. Similarly, brexpiprazole was associated with a larger change from baseline to Week 12 in CMAI Total score than placebo in all subgroups with and without co-occurring neuropsychiatric symptoms, and in all subgroups with and without concomitant use of treatments for dementia or psychiatric conditions. Brexpiprazole also showed a larger change in CMAI Total score at Week 12 than placebo in the subgroups based in the community and in long-term care facilities. Overall, a greater proportion of patients treated with brexpiprazole achieved meaningful response compared to placebo, across different studied subgroups. These analyses are limited by small sample sizes for certain subgroups. Conclusions In patients with agitation associated with dementia due to Alzheimer's disease, the effect of brexpiprazole on symptoms of agitation was consistent across different patient subgroups. Brexpiprazole showed larger improvement than placebo, and higher response rates based on a meaningful within-patient change threshold, regardless of dementia severity, presence or absence of co-occurring neuropsychiatric symptoms, use of concomitant treatments for dementia or psychiatric conditions, and care setting.

Abstract:Annals of Pharmacotherapy, Ahead of Print. Background:Alzheimer's disease (AD) is a neurodegenerative condition leading to memory loss, cognitive impairment, and neuropsychiatric symptoms. Second-generation antipsychotics (SGAs) are commonly used to manage these neuropsychiatric symptoms, but their safety profile in patients with AD requires further investigation.Objective:The objective was to evaluate the safety of SGAs in patients with AD by analyzing adverse drug reactions (ADRs) using the US Food and Drug Administration Adverse Event Reporting System (FAERS) database.Methods:This study conducted a comprehensive analysis of FAERS data from 2014 to 2023, focusing on ADRs in patients with AD treated with SGAs such as risperidone, quetiapine, olanzapine, clozapine, and aripiprazole. Descriptive, disproportionality, time, and dose analysis were performed using the Bayesian confidence propagation neural network, Weibull, and physiologically based pharmacokinetic model.Results:Out of 1289 patients with AD treated with SGAs, the most common ADRs involved the nervous system, gastrointestinal system, and cardiac disorders. Disproportionality analysis identified significant positive signals in cardiac, renal, and vascular systems. Quetiapine, risperidone, and olanzapine showed more positive signals compared with clozapine and aripiprazole. Time analysis indicated that cardiovascular ADRs occurred randomly, whereas renal ADRs increased with prolonged use. Dose analysis suggested that small doses of SGAs did not elevate the risk of multiple cardiac, renal, or vascular ADRs.Conclusion and Relevance:The study underscores the importance of monitoring for ADRs, particularly in the cardiac and renal systems, when using SGAs in patients with AD. Future research incorporating more comprehensive clinical data is warranted to support safe and rational drug utilization.

Abstract:Introduction: Dementia patients often experience behavioral and psychological symptoms (BPSD), which severely affect their quality of life and activities of daily living. Non-pharmacological interventions are effective in treating BPSD, according to multiple clinical trials and systematic reviews. However, the optimal non-pharmacological treatment remains controversial. Therefore, the study aims to evaluate and compare multiple non-pharmacological methods for treating BPSD in order to identify the optimal non-pharmacological intervention. Objective: This study aims to perform a systematic review and network meta-analysis of evidence on non-pharmacological interventions in the treatment of BPSD, which may potentially guide future research and clinical decisions. Methods: In order to select potentially relevant randomized controlled trials (RCTs), 10 academic databases and 3 clinical trial registries will be systematically searched from inception until the 1 October 2022. Two researchers will independently extract information from eligible articles. The primary outcome is the severity of BPSD. Herein, Pairwise and Bayesian network meta-analyses will be conducted utilizing STATA 15.0 and ADDIS 1.16.8. Evidence quality will be assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE). Results: Results from this study will be published in peer-reviewed journals or conference reports. Discussion: In this study, we aim to comparatively assess the efficacy of various non-pharmacological treatments for BPSD. Abstract:Introduction Agitation symptoms are prevalent clinical manifestations of dementia due to Alzheimer's disease and are among the most difficult and stressful aspects of the disease for patients and caregivers. The presence of agitation symptoms in patients with dementia due to Alzheimer's disease is linked to a greater negative impact on caregivers. This post hoc analysis explored changes in neuropsychiatric symptoms (including agitation) and impact on caregivers, in patients with agitation associated with dementia due to Alzheimer's disease, treated with brexpiprazole. Methods This is a pooled post hoc analysis of two 12-week, Phase 3, randomized, placebo-controlled trials of fixed-dose brexpiprazole in patients with agitation associated with dementia due to Alzheimer's disease (NCT01862640; NCT03548584). In each study, the Neuropsychiatric Inventory (NPI) Nursing Home version (NPI-NH; institutionalized patients), or the neuropsychiatric assessment for non-institutionalized patients based on the NPI-NH, was assessed. The NPI is evaluated via caregiver interview and comprises 12 domains (neuropsychiatric symptoms), which are scored from best to worst on frequency (1 to 4), severity (1 to 3), and occupational disruptiveness (institutionalized)/caregiver distress (non-institutionalized) (0 to 5). The present analysis evaluated pooled data from the two fixed-dose trials (data for institutionalized and non-institutionalized patients) for the following outcomes: NPI total (sum of frequency x severity for the 12 domains), NPI agitation/aggression domain, NPI occupational disruptiveness/caregiver distress (total, and in the agitation/aggression domain), NPI-4A subscale (agitation/aggression, irritability/lability, aberrant motor behavior, and anxiety domains), and NPI-4D subscale (agitation/aggression, irritability/lability, aberrant motor behavior, and disinhibition domains). Change from baseline was analyzed using a mixed model for repeated measures. A hierarchical clustering analysis of the 12 NPI domains and NPI occupational disruptiveness/caregiver distress was also carried out. Results A total of 610 patients were included in the present analysis, who received either brexpiprazole 2 or 3 mg/day (n=363) or placebo (n=247). Brexpiprazole demonstrated greater reductions versus placebo on all NPI outcome scores at Week 12: NPI total (p=0.0003), NPI agitation/aggression domain (p=0.0083), total NPI occupational disruptiveness/caregiver distress (p=0.0006), NPI occupational disruptiveness/caregiver distress in the agitation/aggression domain (p=0.0011), NPI-4A subscale (p<0.0001), and NPI-4D subscale (p=0.0003). For some NPI outcomes, greater reductions were observed with brexpiprazole versus placebo as early as Week 6 (p<0.05). In the clustering analysis, six NPI domains (agitation/aggression, irritability/lability, aberrant motor behavior, disinhibition, anxiety, and sleep/nighttime) were clustered with NPI occupational disruptiveness/caregiver distress. The first five of these six domains (which correspond to the well-established NPI-4A and NPI-4D subscales), remained clustered with NPI occupational disruptiveness/caregiver distress after sensitivity analyses by care setting. Conclusions In patients with agitation associated with dementia due to Alzheimer's disease, agitation-related symptoms were associated with impact on caregivers. In this analysis, brexpiprazole was associated with reductions in neuropsychiatric symptoms and reduced impact on caregivers compared with placebo.

Abstract:Introduction Agitation is a prevalent manifestation of Alzheimer's dementia, which can co-present with psychosis symptoms. Presence of either symptom is associated with accelerated disease progression. This analysis explored the efficacy and safety of brexpiprazole in patients with agitation associated with dementia due to Alzheimer's disease, based on presence or absence of baseline psychosis. Methods This is a post hoc analysis of Phase 3, placebo-controlled trials of brexpiprazole in patients with agitation associated with Alzheimer's dementia (NCT01862640 [fixed-dose], NCT01922258 [flexible-dose], NCT03548584 [fixed-dose]). In each study, the primary endpoint was the change in Cohen-Mansfield Agitation Inventory (CMAI) Total score from baseline to Week 12. Presence of baseline psychosis was defined as a Neuropsychiatric Inventory delusion item and/or hallucination item score ≥4. Efficacy data were pooled from the two fixed-dose studies (brexpiprazole 2 or 3 mg/day compared with placebo). Safety data were pooled from all three studies (brexpiprazole 0.5 - 3 mg/day compared with placebo). Results In the fixed-dose studies (pooled), mean CMAI Total scores at baseline were 82.4 (brexpiprazole 2 or 3 mg/day) and 84.1 (placebo) in patients with baseline psychosis (n=142), and 75.1 and 73.0, respectively, in patients without baseline psychosis (n=465). The mean change in CMAI Total score at Week 12 was -25.1 (brexpiprazole 2 or 3 mg/day) versus -19.1 (placebo) in the subgroup with baseline psychosis (p=0.043), and -20.5 versus -16.3, respectively, in the subgroup without baseline psychosis (p=0.0021). Safety was analyzed using data from 238 patients with baseline psychosis (brexpiprazole 0.5 - 3 mg/day, n=153; placebo, n=85) and 802 patients without baseline psychosis (brexpiprazole 0.5 - 3 mg/day, n=500; placebo, n=302). The incidence of treatment-emergent adverse events (TEAEs), serious TEAEs, and severe TEAEs was generally similar between treatments in each subgroup. The incidence of deaths in patients with baseline psychosis was 1.3% (brexpiprazole 0.5 - 3 mg/day) and 0.0% (placebo), and in patients without baseline psychosis was 0.8% (brexpiprazole 0.5 - 3 mg/day) and 0.3% (placebo). The causes of death were varied, and reflective of the types of events that would be expected in the underlying population. All deaths were considered to be unrelated to study treatment. In patients who received brexpiprazole 0.5 - 3 mg/day and in patients who received placebo, the incidence of falls, sedation, somnolence, TEAEs for cardiovascular events, and TEAEs for cerebrovascular events was <5% in subgroups with and without baseline psychosis. Conclusions In patients with agitation associated with Alzheimer's dementia, with or without baseline psychosis, brexpiprazole was associated with improvement in symptoms of agitation compared with placebo and had a similar safety profile.

Abstract:Background Neuropsychiatric symptoms (NPS) of dementia are a common issue in dementia patients which can lead to poor medical and functional outcomes. Pharmacological interventions are its treatment of choice. However, whether to use pharmacological treatments in this population and which drug should be preferred remain controversial. We therefore aimed to compare and rank pharmacological interventions for NPS according to their efficacy and acceptability profiles by quantifying information from randomized controlled trials (RCTs). Methods We will include all RCTs reported as double-blind and comparing one active drug with another or with placebo that compare cholinesterase inhibitors (ChEIs), N-methyl-D-aspartic acid (NMDA) receptor modulators, antipsychotics, antidepressants, and mood stabilisers. Studies will be retrieved by searching electronic databases, including Cochrane Central Register of Controlled Trials, PubMed, MEDLINE, Clinicaltrial.govs, EMBASE, and with no date or language restrictions. The primary outcomes were efficacy (change in overall symptoms) and acceptability (all-cause discontinuation). The network meta-analysis (NMA) will be conducted in R software within a Bayesian framework. The quality of evidence will be evaluated using the Cochrane risk of bias tool, and the GRADE approach. We will conduct subgroup analyses to assess the robustness of our findings. Results The results of this study will be published in a peer-reviewed journal. Conclusions This systematic review will synthesize the available evidence on the comparative efficacy of different pharmacological approaches in the management of overall NPS, agitation, psychosis, apathy and depressive symptoms in dementia patients. The results of the present NMA will influence evidence-based treatment decisions for clinicians.
Long-term Metabolic Side Effects of Second-Generation Antipsychotics in Chinese Patients with Schizophrenia: A Within-Subject Approach with modelling of dosage effects

Abstract: Second-generation antipsychotics (SGAs) are commonly used to treat schizophrenia (SCZ), but SGAs may differ in the severity of side effects. Previous observational studies had limitations like failing to account for confounding factors and short follow-up periods. This study compared the long-term metabolic and anthropometric side effects of seven second-generation antipsychotics (SGAs) in a Chinese schizophrenia population, using a within-subject approach to reduce risk of confounding. : Longitudinal data on SGA prescriptions, concomitant medications, fasting blood glucose, lipid profiles, and BMI were collected from 767 schizophrenia patients, with follow-up up to 18.7 years (median ~6.2 years). Linear mixed models estimated the effects of SGAs, as binary predictors and by dosage, on metabolic profiles. When considering SGAs as binary predictors, clozapine and olanzapine were associated with the most substantial worsening of lipid profiles and BMI, while clozapine alone showed a significant increase in blood glucose. Amisulpride, paliperidone, and quetiapine worsened lipid profiles and increased BMI. Conversely, aripiprazole improved lipid profiles but slightly increased BMI. Examining dosage effects showed consistent results overall. At minimum effective doses, clozapine had the most severe metabolic side effects, followed by olanzapine. Risperidone and aripiprazole had the least metabolic impact, with aripiprazole significantly lowering lipids. This study clarified the long-term, dose-dependent metabolic and anthropometric effects of different SGAs in Chinese schizophrenia patients. Our findings may inform clinicians and SCZ patients of SGA choices.

Abstract:Objective To explore the efficacy and safety of aripiprazole orally disintegrating tablets in the treatment of acute schizophrenia.Methods We recruited 167 patients with schizophrenia,they were randomly divided into ariprazole common tablets treatment group and orally disintegrating tablets treatment group.Treatment efficacy were assessment using the Positive and Negative Syndrome Scale(PANSS) reduction rate and the Treatment Emergent Symptom before treatment and after 2 weeks and 4 weeks treatment.Blood routine,liver and kidney function,blood glucose,blood lipid and ECG were tested,before treatment and after 4 weeks treatment.Results After 2 weeks and 4 weeks treatment,effective rate were 2.38% and 61.91% total in commen tablets group.4.82% and 63.86% in orally disintegrating tablets group,the difference between the two groups were not statistically significant.Adverse reactions mostly appeared within 2 weeks after the treatment in two dosage forms,and mainly of them were dizziness,insomnia,lethargy,akathisia and sinus tachycardia.Conclusion Aripiprazole orally disintegrating tablets and commen tablets have positive effect to improve psychiatric symptoms,and show a good safety and tolerability.

Abstract:With an increase in the global prevalence of dementia, there is also an increase in behavioural and psychological symptoms of dementia (BPSD) for which antipsychotic drugs are often used. Despite several safety warnings on antipsychotic use in dementia, there is little evidence to support the efficacy of antipsychotics in individual BPSD symptoms or to evaluate the drug safety profile by individual antipsychotic drug. There is emerging but scarce evidence that suggests an inter-drug variability between antipsychotic safety outcomes in BPSD. The objective of this review was to examine the existing literature on antipsychotic drug use in dementia patients; in particular to see whether inter-drug differences regarding antipsychotic safety were reported. A literature search was conducted for observational studies published in the English language from 2004 to 2014 that reported the risk of all-cause mortality, cerebrovascular events, pneumonia and other outcomes such as hip/femur fracture, deep vein thrombosis (DVT) and hyperglycaemia. Six of 16 mortality studies (38 %), 7 of 28 stroke studies (25 %), 1 of 6 pneumonia (17 %) studies and 2 of 6 fracture studies (33 %) investigated inter-drug safety outcomes in elderly patients/dementia patients, while to our knowledge, there are no studies investigating the inter-drug variation of deep-vein thrombosis and hyperglycaemia risk. The results of the observational studies provide mixed results on the safety of antipsychotics in BPSD but it is clear that there are differences between the safety profiles of antipsychotic drugs. Robust evidence of such inter-drug variability could significantly improve patient safety as antipsychotics become more targeted to clinical risk factors.

Abstract:OBJECTIVE: We aimed to create hierarchies of the efficacy, acceptability and tolerability of eight atypical antipsychotics in the treatment of Chinese patients with acute schizophrenia.METHOD: We systematically searched for RCT articles published between January 1st 2005 and December 31st 2014 in electronic databases (Medline, Pubmed, Embase, the Cochrane Library and ClinicalTrial.gov for studies in English and the China National Knowledge Infrastructure, Wan Fang, and VIP Information/Chinese Scientific Journals Database for studies in Chinese). The primary outcome was efficacy, as measured by the change of PANSS total score. Pairwise comparisons were performed using random-effects model by the Dersimonian-Laird method and network meta-analyses were performed in a Bayesian set.RESULTS: Sixty high-quality RCTs with 6418 participants were included. A pattern of superiority from olanzapine, paliperidone and amisulpride was seen in the primary outcome. Only paliperidone was found better than aripiprazole (odds ratio, 0.49 [95% credible intervals, 0.25 to 0.99]), ziprasidone (0.42 [0.21 to 0.85]) and quetiapine (0.36 [0.13 to 0.93]) in terms of all-cause discontinuation. The best and worst drugs in terms of weight gain, EPS and somnolence were aripiprazole and olanzapine, clozapine and amisulpride, aripiprazole and clozapine, respectively. The rank of efficacy did not change substantially in sensitivity analyses or in meta-regressions.CONCLUSION: Our findings provided the hierarchies of eight antipsychotics in efficacy, acceptability and tolerability. These findings are expected to help Chinese clinicians to select the appropriate antipsychotic drug for their patients.
