Characteristics and Correlative Factors of Posterior Corneal Surface Changes after Laser in Situ Keratomileusis

Abstract:OBJECTIVE:To assess the changes of the posterior corneal surface and the factors influencing these changes after laser in situ keratomileusis (LASIK). These factors included the preoperative refraction, preoperative intraocular pressure (IOP), preoperative corneal thickness, spherical equivalent correction and one-week postoperative corneal thickness.METHODS:The posterior corneal radius of curvature for the posterior best fit sphere (PBFS), irregularity of posterior surface, and anterior chamber depth (ACD) were obtained with the scanning slit topography system before and 1 week, 1 month and 3 months after LASIK in 64 eyes of 34 patients with myopic refractive errors of -2.25 to -12.5 diopters. Multiple regression analysis was used to assess the factors that affecting these changes of posterior corneal surface. The difference of radius PBFS and posterior elevation difference (PED) was used as an indicator for the forward shift of posterior corneal surface.RESULTS:The ACD before the surgery and 1 week, 1 month and 3 months after the surgery was (3.2448 +/- 0.2806), (3.2770 +/- 0.2810), (3.2579 +/- 0.2932) and (3.2651 +/- 0.2875) mm, respectively. The PBFS at that time was (6.5095 +/- 0.2177), (6.3731 +/- 0.2127), (6.4257 +/- 0.2358) and (6.4354 +/- 0.2266) mm, respectively. The posterior corneal irregularity at 5 diameters zone was 0.622 +/- 0.142, 0.978 +/- 0.244, 0.884 +/- 0.207 and 0.881 +/- 0.174, respectively The preoperative posterior corneal radius of curvature for PBFS, the irregularity of the posterior surface and the ACD were significantly different from that of postoperative data at each time point (P < 0.01). There was no significant difference in these indexes between 1 month and 3 months postoperatively. Multiple regression analysis showed that factors relevant to the changes of posterior corneal surface were the corrected preoperative refraction (regression coefficient r = 0.0173, P = 0.005) and 1 week postoperative corneal thickness (mm) (regression coefficient r = -1.495, P = 0.001).CONCLUSIONS:The posterior corneal surface after LASIK become steeper and more irregular and remains stable in three months after the surgery. Eyes with thinner corneas and higher myopia requiring more ablation are more predisposed to the changes of posterior corneal surface. The results of the present study should not be used in extremely high degree myopia. A study using a larger group of patients with a longer observation period is required.
A Comparative Study of the Effect of Femtosecond Laser-Assisted Cataract Surgery on Corneal Astigmatism in Post-LASIK Eyes and Virgin Eyes

Abstract:Purpose:To evaluate and compare the effect of femtosecond laser-assisted cataract surgery on corneal astigmatism in post-LASIK eyes and virgin eyes. Patients and Methods:Patients who underwent femtosecond laser-assisted cataract surgery were included in the study and categorized into two groups: Group A, consisting of patients with post-LASIK eyes, and Group B, consisting of patients with virgin eyes. Visual acuity, corneal astigmatism, and surgically induced astigmatism (SIA) were evaluated. Additionally, the correlation between SIA and preoperative corneal astigmatism, mean corneal curvature, and central corneal thickness was also analyzed. Results:A total of 168 eyes were enrolled in this study, with 62 eyes in Group A and 106 eyes in Group B. Significant differences in corneal astigmatism and SIA were observed between the two groups in the early postoperative period following cataract surgery (P<0.05). However, there was no significant difference at 6 months postoperatively (P>0.05). Corneal astigmatism demonstrated an against-The-rule shift in both groups postoperatively. No significant correlation was identified between SIA and preoperative corneal astigmatism, corneal curvature or corneal thickness. Additionally, there was no significant difference observed between the two groups in terms of uncorrected distance visual acuity (UDVA) at 6 months postoperatively. Conclusion:The effect of femtosecond laser-assisted cataract surgery on corneal astigmatism in post-LASIK eyes and virgin eyes was different in the early postoperative period. However, there was no significant difference at 6 months postoperatively. The post-LASIK eyes exhibited a delayed recovery compared to the virgin eyes.
Comparison of the Change in Posterior Corneal Elevation and Corneal Biomechanical Parameters after Small Incision Lenticule Extraction and Femtosecond Laser-Assisted LASIK for High Myopia Correction.

Abstract:PurposeTo compare posterior corneal elevation (PCE) changes and corneal biomechanical changes post fematosecond laser-assisted laser in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE) procedures using the Pentacam system and the Ocular Response Analyzer (ORA).DesignRetrospective observational case series study.Method106 patients with bilateral myopia who underwent either FS-LASIK (56 patients) or SMILE (50 patients) were reviewed, only the right eye was used in the analysis. Inclusion criteria include a spherical equivalent of −6.00DS and completion of 12 months follow up. The main outcome evaluated was change in PCE at 3 months, 6 months and 12 months post-operation using the Pentacam system. Corneal biomechanical parameters were evaluated at 6 and 12 months with the ORA.ResultsPCE change at 3 and 6 months were not significantly different between the two procedures (p=0.064, p=0.109 respectively). At 12 months, PCE change was greater in FS-LASIK than SMILE (p=0.048). One-way ANOVA showed that for either procedure, the change in PCE did not differ at 3, 6 or 12 months post operation. CH and CRF values were reduced after both procedures, with FS-LASIK demonstrating a greater reduction in CRF than SMILE at 6 and 12 months (p=0.037 and p=0.001). Both CH and CRF reduction correlated with PCE increase at 6 and 12 months.ConclusionFS-LASIK demonstrated a greater increase in PCE than SMILE only at 12 months, as well as a greater reduction of CRF than SMILE. There were no significant differences in PCE change over time within either group.
Regional analysis of posterior corneal elevation after laser refractive surgeries for correction of myopia of different degrees

Abstract:Purpose: To evaluate regional changes in the posterior corneal elevation after three laser refractive surgeries for correction of myopia of different degrees. Settings and Design: Retrospective, comparative, and non-randomized study. Methods: Two hundred patients (200 eyes) who underwent laser epithelial keratoplasty (LASEK), femtosecond-assisted laser in-situ keratomileusis (FS-LASIK), and small-incision lenticule extraction (SMILE) were included in this study. According to preoperative spherical equivalent (SE), each surgical group was divided into two refractive subgroups: low-to-moderate myopia (LM group) and high myopia (H group). The posterior corneal elevation from Pentacam Scheimpflug tomography was analyzed preoperatively and at 1 month, 3 months, 6 months, and 12 months postoperatively. Three subregions of the posterior cornea were divided in this study as the central, paracentral, and peripheral regions. Statistical Analysis Used: Generalized Estimating Equations (GEE). Results: For all three surgical groups, similar changing trends were seen in the two refractive subgroups. H group presented a larger changing magnitude than the LM group in FS-LASIK over time ( P < 0.05), whereas no significant difference was noted in the two refractive subgroups of LASEK or SMILE ( P > 0.05). At 12 months postoperatively, the central posterior corneal elevation returned to the preoperative level in LASEK ( P > 0.05) but shifted forward significantly in FS-LASIK and SMILE ( P < 0.05). Conclusion: Different posterior corneal regions respond differently to corneal refractive surgeries. LASEK, FS-LASIK, and SMILE demonstrate different trends in the regional changes in posterior corneal elevation. The corneal shape seems more stable in LASEK than in FS-LASIK and SMILE.

Abstract:Aim: To investigate the changes in corneal biomechanics and posterior corneal surface elevation after femtosecond laser-assisted in situ keratomileusis (FS-LASIK). Methods: Totally 197 eyes of 100 patients who underwent the FS-LASIK from April 2022 to November 2022 were included. They were divided into three groups according to the ratio of residual corneal stroma thickness/corneal thickness (RCST/CT): Group I (50%≤RCST/CT<55%, 63 eyes of 32 patients), Group II (55%≤RCST/CT<60%, 67 eyes of 34 patients), and Group III (RCST/CT≥60%, 67 eyes of 34 patients). The intraocular pressure (IOP), corneal compensated IOP (IOPcc), corneal hysteresis (CH) and corneal resistance factor (CRF) were measured immediately, 1, and 3mo postoperatively by ocular response analyzer (ORA) and the posterior elevation difference (PED) was measured by Pentacam. Results: After operation, IOP, CH, CRF, and PED were statistically different among the three groups (F=12.99, 31.148, 23.998, all P<0.0001). There was no statistically significant difference in IOPcc among the three groups (F=0.603, P>0.05). The IOP, IOPcc, CH, and CRF were statistical changed after surgery (F=699.635, 104.125, 308.474, 640.145, all P<0.0001). The PED of Group I was significantly higher than that of Group II (P<0.05), and Group II was significantly higher than that of Group III (P<0.05). The PED value of 3mo after surgery decreased in each group compared with 1mo after surgery, but there was no statistical difference (Group I: t=0.82, P=0.41; Group II: t=0.17, P=0.87; Group III: t=1.35, P=0.18). The correlation analysis of corneal biomechanical parameter changes with PED at 1mo and 3mo after surgery showed that ΔIOP, ΔIOPcc, ΔCH, and ΔCRF were not correlated with PED value in three groups (P>0.05). Conclusion: The smaller the RCST/CT, the greater effect on corneal biomechanics and posterior surface elevation. There is no correlation between changes in corneal biomechanics and posterior corneal surface elevation in the range of RCST/CT≥50%.
Comparison of Posterior Corneal Elevation after SMILE and FS-LASIK in Correcting Myopia over −9.0 Diopters

Abstract:Background: To compare the changes in posterior corneal elevation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in correcting myopia over -9 diopters (D). Methods: In this prospective comparative study, 82 eyes of 82 patients scheduled for refractive correction were recruited. Eyes were randomly assigned to the SMILE group (45 eyes, -10.43 +/- 0.92 D) or FS-LASIK group (37 eyes, -10.97 +/- 1.37 D). The posterior corneal surface was measured using a Scheimpflug camera (Pentacam, Oculus, Germany) preoperatively and at 1 day, 1 month, and 6 months after surgery. Posterior corneal elevation in the central point and central 4-mm area, and in various optical zones above the best-fit sphere, was analyzed. A P value of less than 0.05 was considered statistically significant. Results: All surgeries were completed successfully. The safety index and efficacy index were 1.20 and 1.00, respectively, in the SMILE group, and was 1.10 and 0.90, respectively, in the FS-LASIK group. No significant difference existed in all analyzed data before and at 6 months after surgery in both the SMILE group and the FS-LASIK group. Changes in posterior corneal elevation after FS-LASIK were greater than after SMILE, with no statistical significance (P >= 0.07). In the SMILE group, residual bed thickness was found to be moderately negatively correlated with changes in the elevation in the central area (P <= 0.045); whereas it was positively correlated in the peripheral area (P=0.002). Conclusions: SMILE and FS-LASIK presented stable posterior corneal surface in correction of myopia over -9.0 D at the follow-up visit of 6 months.

Abstract:INTRODUCTION: Refractive surgery procedures, transepithelial photorefractive keratectomy (T-PRK), and femtosecond laser in situ keratomileusis (Fs-LASIK) are regarded as safe and efficacious methods for correcting myopia and myopic astigmatism. These two methods do not have many differences in results when treating spherical myopia while differences exist in the treatment of astigmatism correction. Vector analysis presents a powerful tool to show the real differences between these two methods regarding high ocular aberrations and central corneal thickness of treated eyes.AIM: The aim of the study is to investigate changes in higher order ocular aberrations (HOAs) and central corneal thickness (CCT) following treatment of myopia and myopic astigmatism above -5.00DS and up to -2.00DC after either T-PRK or Fs-LASIK.METHODS: Patients (30 eyes per group) underwent T-PRK (group I) or Fs-LASIK (group II) procedure using the Schwind Amaris 750S laser. HOAs (3mm&5mm pupil) and CCT were measured objectively at pre-, 1,3 & 6 months postop in each case.RESULTS: Key results at 6 months were: i) mean values of trefoil (5mm pupil) were 0.092μm (sd,0.055,95% CI 0.072 to 0.112) & 0.126μm (sd,0.078,95% CI 0.098 to 0.154) in group I, and 0.088μm (sd,0.058,95% CI 0.067 to 0.109) & 0.064μm (sd,0.034,95% CI 0.052 to 0.076) in group II (P=0.001 at 6 months); ii) Changes in CTT (ΔCTT) and best spherical equivalent correction (ΔBSE) was significant in group II (ΔCCT=-26.55[ΔBSE]-14.06,R=0.486,P=0.006) but not in group I (p=0.034).CONCLUSIONS: After T-PRK trefoil is worse than Fs-LASIK. The predictability of corneal changes is better following Fs-LASIK.
Changes in Posterior Cornea and Posterior-To-Anterior Curvature Radii Ratio 1 year After LASIK, PRK, and SMILE Treatment of Myopia

Abstract:Purpose: The purpose of this study was to compare changes in the posterior curvature and the posterior–anterior radii ratio of the cornea, 1 year postoperatively in laser in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), and small incision lenticule extraction (SMILE). Methods: This retrospective study was performed at a single surgical center. 199 eyes were included in the study from 119 patients with manifest refraction spherical equivalents from −7.61 to −2.54 D. 67 eyes underwent LASIK, 89 underwent PRK, and 43 underwent SMILE. Both preoperative and 1-year postoperative front and back sagittal keratometry were measured at 4- to 6-mm zones around the corneal vertex. Corneal asphericity (Q-value) was measured at an 8-mm zone around the corneal vertex. Results: The average change in the posterior–anterior radii ratio after LASIK, PRK, and SMILE did not differ between surgery groups at 4 mm (LASIK: −0.075, PRK: −0.073, SMILE: −0.072, P = 0.720), 5 mm (LASIK: −0.072, PRK: −0.068, SMILE: −0.068, P = 0.531), or 6 mm (LASIK: −0.075, PRK: −0.071, SMILE: −0.072, P = 0.456) zones. Anterior Q-value significantly positively increased after all 3 surgeries ( P < 0.001). The posterior Q-value also significantly positively increased after LASIK ( P < 0.001) and SMILE ( P < 0.001), but not after PRK ( P = 0.227). Both anterior and posterior keratometric power decreased significantly after LASIK, PRK, and SMILE for all diameters. Conclusions: The change in the posterior–anterior radii ratio was not influenced by the type of refractive surgery performed, as indicated by statistically identical preoperative, postoperative, and delta values. In addition, the posterior cornea exhibited paracentral flattening after LASIK, SMILE, and PRK and increased oblateness after LASIK and SMILE.

Abstract:Background To compare the changes in posterior corneal surface after small-incision lenticule intrastromal keratoplasty (SMI-LIKE) and femtosecond laser-assisted lenticule intrastromal keratoplasty (FS-LIKE) for hyperopia correction. Methods In this prospective comparative randomized study, 23 eyes with hyperopia were recruited. Eyes were categorized into two groups—SMI-LIKE group (11 eyes) and FS-LIKE group (12 eyes). Lenticules from myopia small incision lenticule extraction were implanted into a pocket (SMI-LIKE group) or at a depth of 100 µm under a flap (FS-LIKE group). Posterior corneal elevations in the center, mid-periphery, and periphery, as well as mean keratometry of the posterior corneal surface (Kmb) were measured using a Pentacam over a three-month follow-up. Results All surgeries were completed successfully and no complications occurred. At one day postoperatively, there was a slight backward change with SMI-LIKE and a forward change with FS-LIKE in the central region of the posterior corneal elevation. Conversely, the peripheral area showed forward displacement in SMI-LIKE and an apparent backward change in FS-LIKE. The mid-peripheral regions manifested a backward change after the procedure throughout the entire follow-up in both groups. Kmb exhibited flattening at one month postoperatively and subsequently returned to its original level at three months after SMI-LIKE while in FS-LIKE, Kmb steepened after lenticule implantation with a significant change noted at one day postoperatively ( P = 0.001). Conclusions Posterior corneal surface after SMI-LIKE and FS-LIKE exhibited different change patterns in various corneal regions, with the most prominent change occurring at one day postoperatively during the three-month follow-up. Short term changes in corneal stress-strain index and other corneal biomechanical parameters post-laser in situ keratomileusis

Abstract:Purpose: To report the short-term changes in a corneal stress-strain index (SSI) and other corneal biomechanical parameters post-laser in situ keratomileusis (LASIK) surgery. Methods: A retrospective study was conducted at a tertiary eye care center wherein patients who had undergone LASIK (microkeratome blade and femtosecond bladeless LASIK) between July and December 2019 were enrolled. Patients of age group 20-40 years, best-corrected visual acuity of 20/20, intraocular pressure (IOP) <22 mmHg, pre-LASIK pachymetry >500 microns, and corneal astigmatism ≤3.00 D were included. Subjects with a prior history of refractive surgery, any other ocular or systemic disease, poor-quality scans, intraoperative complications, and missing data were excluded. Corneal biomechanical properties including SSI were analyzed using Corvis ST and compared using the Paired T-test for each group separately at pre-LASIK, and 1-month post-operatively. Results: Overall, 202 eyes were reviewed, and 79 eyes fulfilled the inclusion criteria. Forty-three and 36 eyes had undergone Microkeratome Blade LASIK (Group I) and Femto LASIK (Group II), respectively. Overall, 29 and 26 corneal biomechanical parameters out of 33 changed significantly post-Microkeratome Blade LASIK and Femto LASIK, respectively. Statistically significant changes were noted in all the parameters at A1, maximum and Vinciguerra screening parameters (P < 0.001), however, no changes were noted in SSI in both the groups when compared with the pre-surgery data. Conclusion: Though the reduction in SSI was not statistically significant, other biomechanical parameters showed significant biomechanical changes pre- and post-LASIK surgeries in both the groups. However, a long-term study with a larger sample size would be required to understand the changes and stability in SSI post-refractive surgery.
Long-term corneal multifocal stability following a presbyLASIK technique analysed by a light propagation algorithm

Abstract:Background: There is a lack of scientific evidence on long-term follow-up of the outcomes of the presbyLASIK techniques. This study aimed to objectively evaluate corneal stability three years following excimer laser central presbyLASIK. Methods: This is a longitudinal, retrospective, observation of a consecutive series of cases comprising 24 eyes which had been treated by central myopic or hyperopic Presbymax central presbyLASIK. Eyes treated with the same version of presbyLASIK software were included and followed by corneal topography at three months, one and three years after surgery. Based on the corneal topography data, customised software - based on a light propagation algorithm and developed using Matlab software - was used to analyse the simulated behaviour of light through the ocular media. The range of objective corneal depth of focus was also measured. Results: Results were assessed separately for myopes and hyperopes at pupillary diameters of 3.0 and 6.0 mm. The results showed corneal multifocality outcomes to remain constant (p > 0.05) throughout the three-year follow-up period. Conclusion: The use of light propagation analysis of the multifocal anterior corneal surface following a Presbymax central presbyLASIK procedure demonstrated stable outcomes over a three-year follow-up period.
Changes in Anterior Chamber after Myopic and Hyperopic FS-LASIK

Abstract:This study evaluated the changes and explored influential factors in the anterior chamber after femtosecond laser-assisted in situ keratomileusis (FS-LASIK) on the basis of patients with different degrees of myopia, refractive status, and age. Patients underwent uneventful FS-LASIK for the treatment of ametropia. All patients with myopia were classified into a low age group (18–40 years) or high age group (> 40 years). Patients in the low age group (18–40 years) were subdivided into myopia and hyperopia groups. According to the preoperative spherical equivalent values, the myopia group was further divided into low myopia, moderate myopia, and high myopia groups. We measured the magnitude of anterior chamber depth (ACD), angle (ACA), volume (ACV), lens rise, central corneal thickness (CCT), and mean pupil power (MPP) of all patients using Sirius (version 3.2, CSO, Italy) before and 3 months post operation. A total of 140 eyes of 87 patients were evaluated. The magnitudes of ACD, ACA, and ACV decreased significantly postoperatively in both low age (P < 0.001, P < 0.001, P < 0.001, respectively) and high age group (P < 0.001, P < 0.001, P < 0.001, respectively) of patients with myopia, while an increasing tendency of ACD (2.85 ± 0.38 mm preoperatively vs 2.89 ± 0.09 mm postoperatively) and ACA (39.16° ± 7.30° preoperatively vs 39.37° ± 7.68° postoperatively) was found in hyperopia. There was no correlation between different degrees of myopia and the changes in the anterior chamber. Anterior chamber parameters decreased significantly and approximately in the same degree in low and high age groups postoperatively, while an increasing tendency of ACD and ACA was found in hyperopia, indicating that it is probably a corneal magnification effect that influences changes in the anterior chamber.

Abstract:OBJECTIVE: To determine the outcomes of Femtosecond Assisted Laser in situ Keratomileusis (Femto LASIK) on eyes with myopia and compound myopic astigmatism in terms of efficacy, safety, accuracy, predictability, and stability of the procedure.STUDY DESIGN: Quasi-experimental study. Place and Duration of the Study: Refractive Surgical Department, Armed Forces Institute of Ophthalmology (AFIO), Rawalpindi, Pakistan, from January 2014 to August 2019.METHODOLOGY: Participants aged 18 years and above with upto -12D (dioptre of myopia, underwent preoperative detailed work-up with history, ocular examination, subjective refraction and assessment on topography, tomography, and aberrometry. Suitable candidates underwent Femto LASIK and were re-evaluated at 1st postoperative day, end of 1st week, 1 month, 3 months, 6 months and 1 year. Results were analysed and represented in form of standard graphs for refractive surgery. Results: Postoperative UDVA (uncorrected distance visual acuity) of 20/40 was achieved in 99% of patients. Efficacy index was 1.02 + 0.15. Safety index was 1.04 + 0.199. None of the patients lost more than one line in postoperative CDVA (corrected distance visual acuity) when compared to preoperative CDVA. All the eyes (100%) were accurately treated within +1.0 DS of intended spherical equivalent (SEQ) range. Mean SEQ showed stability with 1% eyes recorded to have more than 0.5D change or more over 12 months.CONCLUSION: Femto LASIK is an effective, safe, accurate, predictable, and stable procedure for correction of myopia and compound myopic astigmatism.KEY WORDS: Femtosecond, Laser in situ Keratomileusis, Refractive surgery, Myopia, Compound myopic astigmatism, Uncorrected distance visual acuity.

Abstract:AIM: To compare the difference of biomechanical stability after a femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and laser-assisted subepithelial keratomileusis(LASEK)by ocular response analyzer(ORA). METHODS: This prospective study was conducted at the First Affiliated Hospital of Harbin Medical University, and myopic patients with the equivalent sphere between -2.00 and -5.00 diopters from January 2016 and December 2017 were enrolled. All the subjects were divided into FS-LASIK and LASEK group respectively according to different surgical methods. There were 64 patients(100 eyes)participated in the FS-LASIK group while 53 patients(100 eyes)in the LASEK surgery group. Corneal hysteresis value(CH)and corneal resistance factor value(CRF)were measured pre-, 1mo and 3mo postoperation of all the subjects. And the variance analysis of two groups was repeated to compare the overall difference between the two modes of operation from pre-operation to post operation. RESULTS: The CH and CRF value of the two groups were different before and after operation(P<0.05). The comparison results of repeated measurements showed that the CH value and CRF value of the surgical methods were reduced in 1 and 3mo after operation, respectively, and there was significance(P<0.05). The CH and CRF of FS-LASIK group was lower than LASEK group only in 1mo after the operation(P<0.05). CONCLUSION: Both surgeries could affect the biomechanical stability of cornea. From the point of view of biomechanical stability, LASEK operation is safer than FS-LASIK operation and reduces the possibility of postoperative refractive regression.
Corneal sensitivity and ocular surface health in patients undergoing femtosecond LASIK retreatment for residual refraction

Abstract:Purpose: To assess corneal sensitivity and the ocular surface in patients undergoing primary femtosecond laser in situ keratomileusis (FS-LASIK) and those undergoing FS-LASIK retreatment under the same flap due to residual refractive error. Setting: Ekol Eye Hospital, Izmir, Turkey. Design: Prospective case series. Methods: 19 patients with previous FS-LASIK who had myopic and astigmatic refractive error were included in the study group, and 19 age-matched and sex-matched patients undergoing FS-LASIK for the first time as the control group. Corneal sensitivity, Schirmer test, tear film break-up time (TBUT), Oxford grading scheme for ocular surface staining, and Ocular Surface Disease Index (OSDI) were measured preoperatively and at postoperative 1 week and 1, 3, and 6 months. Results: The mean refractive correction in the study and control groups, respectively, was 2.18 ± 0.78 diopters (D) (range: 1.00 to 3.50) and 2.76 ± 1.20 D (range: 1.00 to 4.50; P = .07). Corneal esthesiometry results in the study and control groups, respectively, were 6.10 ± 12.55 vs 9.90 ± 11.50 mm at 1 week ( P = .001), 41.95 ± 6.98 vs 45.09 ± 5.88 mm at 1 month ( P = .004), 56.09 ± 3.37 vs 56.19 ± 2.52 mm at 3 months ( P = .8), and 58.60 ± 2.01 vs 58.80 ± 1.39 mm at 6 months ( P = .5). Significant difference between the 2 groups in Schirmer test score that disappeared at postoperative 3 months and in TBUT and ocular surface staining that disappeared at 6 months, whereas the statistically significant difference in OSDI score persisted at 6 months ( P = .03) was detected. Conclusions: Corneal surface sensitivity and ocular surface health are more impaired in patients undergoing FS-LASIK retreatment due to residual refractive error.
Three-Year Stability of Posterior Corneal Elevation after Small Incision Lenticule Extraction (smile) for Moderate and High Myopia

Abstract:PURPOSE: To investigate long-term changes in posterior corneal elevation after small incision lenticule extraction (SMILE).METHODS: Thirty-six eyes of 20 patients (7 male, 13 female) who underwent SMILE were recruited for this prospective consecutive study. The mean preoperative manifest spherical equivalent was -6.24 +/- 1.47 diopters (D) (range: -3.88 to -8.75 D). Posterior corneal surfaces were measured by a Scheimpflug camera (Pentacam; Oculus Optikgerate GmbH, Wetzlar, Germany) preoperatively and at 1 month, 6 months, 12 months, 2 years, and 3 years after surgery. Posterior central elevation (PCE), posterior maximal elevation (PME), and posterior elevation at the thinnest corneal point (PTE) in the central 4 mm area above the best fit sphere (BFS) were analyzed. The BFS was the same across all followup periods and determined by the preoperative data. A P value of less than.05 was considered statistically significant.RESULTS: All surgeries were executed without complications and no case of keratectasia was observed during the follow-up. An uncorrected distance visual acuity of 20/20 or better was achieved in 100% of eyes at the last visit. The mean change of PCE, PME, and PTE at 3 years after the operation was -2.39 +/- 2.85, 0.50 +/- 3.33, and -2.33 +/- 2.90 mu m, respectively. There were significant differences in the measurements of PCE and PTE before surgery and 3 years after surgery (P =.009); however, there was no significant difference in PME before surgery and 3 years after surgery (P = 1.000). No correlation was found between changes in posterior corneal elevation and residual bed thickness, ablation depth, and preoperative thinnest corneal thickness.CONCLUSIONS: The posterior corneal surface, measured using the Pentacam, was stable after SMILE in the long-term follow-up. The cause of the slight backward change of PCE and PTE needs further study.
The Changes of Posterior Corneal Surface and High-Order Aberrations after Refractive Surgery in Moderate Myopia

Abstract:PURPOSE: To compare forward shift of posterior corneal surface and higher-order aberration (HOA) changes after LASIK, LASEK, and wavefront-guided LASEK surgery in moderate myopiaMETHODS: One hundred eighty four eyes undergoing LASIK, LASEK and wavefront-guided LASEK with VISX STAR S4 were included in this study. The posterior corneal elevation was measured with Orbscan before, 2 and 4 months after surgery. Changes of the elevation were assessed using the difference map generated from preoperative and postoperative elevation maps. The values of higher-order aberrations were evaluated preoperatively and 2 months postoperatively with Wavefront aberrometer.RESULTS: The posterior corneal surface displayed forward shift of 27.2+/-11.45 micrometer, 24.3+/-9.76 micrometer in LASIK group, 23.4+/-10.5 micrometer, 23.6+/10.55 micrometer in LASEK group, 24.0+/-14.95 micrometer, 28.4+/-14.72 micrometer in wavefront-guided LASEK group at 2 months and 4 months, respectively. There were no statistically significant differences among those three groups, and between 2 and 4 months. The root mean score (RMS) of HOA was increased after LASIK and LASEK (p=0.000, p=0.000, respectively). The mean change of HOA-RMS was significantly smaller in wavefront-guided LASEK than LASIK or LASEK (p=0.000, p=0.000, respectively, Bonferroni-corrected).CONCLUSIONS: The changes of posterior corneal surface forward shift showed no difference among LASIK, LASEK and wavefront-guided LASEK in moderate myopia. HOAs were significantly increased after LASIK and LASEK. The changes of HOAs were significant smaller in wavefront-guided LASEK than LASIK or LASEK.
Clinical Characteristic and Tear Film Biomarkers After Myopic FS-LASIK: 1-Year Prospective Follow-up

Abstract:Purpose: To assess the long-term (1-year) effect of myopic femtosecond laser-assisted in situ keratomileusis (FSLASIK) on clinical characteristics and tear film biomarkers. Methods: Eighty eyes from 80 patients who underwent FSLASIK were evaluated. Ocular surface symptoms and signs were evaluated using specific questionnaires and tests. The corneal nerves and dendritic cells were examined using in vivo confocal microscopy. Corneal sensitivity was evaluated using a Cochet-Bonnet esthesiometer. Tear inflammatory cytokines and neuropeptides were evaluated using Luminex immunoassay. These examinations were performed preoperatively and at 1, 3, 6, and 12 months postoperatively. Results: Seventy-three participants completed all follow-up visits. Following FS-LASIK, ocular symptoms and signs (except Schirmer I test) worsened at 1 month but corneal and conjunctival stainings improved by 3 months. The numbers of dendritic cells and activated dendritic cells increased at the 3-month postoperative visit and recovered to preoperative levels by the 6-month visit. Ocular symptoms and corneal sensitivity recovered to preoperative levels at the 12-month visit. Tear break-up time and corneal nerve morphology were not recovered to preoperative status at the 12-month visit. Interleukin (IL)-1β, IL-17A, tumor necrosis factor-α, and substance P tear levels significantly increased at all postoperative visits compared to preoperative levels. Corneal staining scores positively correlated with tear IL-1β and IL-17A levels, whereas corneal nerve morphology positively correlated with corneal sensitivity and negatively correlated with substance P levels. Conclusions: Although most clinical variables improved at 12 months postoperatively, some tear inflammatory cytokines and substance P remain altered beyond 12 months, indicating that ocular homeostasis is not completely recovered. Abstract:PURPOSE:To compare changes in the corneal thickness profile before and 6 months after femtosecond laser in situ keratomileusis (LASIK) for hyperopia.METHODS:In a prospective noncomparative case series study, 24 eyes of 20 hyperopic patients undergoing femtosecond LASIK were examined preoperatively and 6 months postoperatively. Corneal profile was measured using Pentacam HR device. Paired t test was used to compare preoperative and postoperative values. Spearman correlation analysis was performed to evaluate the relationship between the central corneal thickness changes and attempted spherical equivalent refraction (SER).RESULTS:The mean uncorrected distance visual acuity significantly improved after surgery (P<0.001). Significant differences in central, midperipheral, ablative annular, and peripheral corneal thicknesses were observed from preoperatively to 6 months postoperatively (all P<0.001). There was no significant correlation between the changes in the central corneal thickness and attempted SER (P=0.23).CONCLUSIONS:The corneal thicknesses across the whole ablation zone including central corneal thickness, significantly decrease postoperatively compared with preoperatively.
