Efficacy and Safety of Benralizumab for Eosinophilic Asthma: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Abstract:Context: Benralizumab is a humanized monoclonal antibody that targets the alpha chain of the IL-5 receptor (IL-5R alpha) and is currently being assessed in clinical trials for asthma control. Objective: Our systematic review and meta-analysis intends to evaluate the therapeutic efficacy and safety of benralizumab in patients with eosinophilic asthma. Data sources and extraction: Literature searches of PubMed, Embase, and the Cochrane Library were performed to identify randomized controlled trials of benralizumab and clinic outcomes in asthmatics. Results: In total, 7 articles with 2,321 subjects met our inclusion criteria. From this pooled analysis, we found that benralizumab significantly reduces exacerbations (RR: 0.63, 95% CI: 0.52-0.76, p < 0.00001; l(2) = 52%, p = 0.06) compared to placebo in eosinophilic asthma. There was no statistical trend for improvement in forced expiratory volume in 1 second or asthma control indices such as Quality of Life Assessment (AQLQ) and Asthma Control Questionnaire score in benralizumab-treated patients. In addition, safety data indicated that benralizumab administration resulted no increasing incidence of adverse events and was well tolerated (RR: 1.00, 95% CI: 0.95-1.05, p = 0.96; l(2) = 40%, p = 0.13). Conclusion: These results demonstrate the efficacy and safety of benralizumab for asthma patients with severe or uncontrolled symptoms and elevated eosinophils and provide support for benralizumab as an ideal option to treat asthma in this patient population.

Methods: This was a prospective, observational, multicenter post-marketing study (ClinicalTrial.gov, NCT03588546). The safety and tolerability of benralizumab over 1 year were assessed by the incidence of adverse events (AEs), serious AEs, adverse drug reactions (ADRs), and serious ADRs. Patient background characteristics indicating a more frequent onset of ADRs with benralizumab were explored. The main effectiveness assessment was the change in Asthma Control Questionnaire-5 (ACQ-5) score from baseline. Patients with baseline ACQ-5 scores ≥ 1.5 were defined as having severe uncontrolled asthma. Results: In total, 632 patients were evaluated for safety and 274 for effectiveness; 139 patients were included in the severe uncontrolled asthma subgroup. ADRs were reported in 12.7% and serious AEs in 13.0% of patients. Serious infections occurred in 3.8%, serious hypersensitivity in 0.3%, and malignancy in 0.3% of patients. No helminthic infections occurred. In the effectiveness population, benralizumab improved the mean (standard deviation [95% confidence interval]) ACQ-5 score by − 1.16 (1.40 [− 1.36, − 0.96]) from baseline; forced expiratory volume in 1 second by 0.151 (0.440 [0.09, 0.21]) L; and Mini-Asthma Quality of Life questionnaire score by 1.16 (1.29 [0.94, 1.38]) at the last observation. The annual asthma exacerbation rate was 0.42. A greater ACQ-5 score improvement was observed among patients with eosinophilic asthma characteristics. Conclusion: No new safety concerns were raised, and patients experienced benefits consistent with previous studies of benralizumab, thus supporting the use of benralizumab for the add-on maintenance treatment of patients with eosinophilic severe uncontrolled asthma. Abstract:Asthma is an airway disease characterized by airway inflammation. It is associated with significant morbidity, mortality, and costs to the health-care system and society. Eosinophils and interleukin-5 (IL-5) play a key role in the inflammatory response in T-helper 2-high asthma. IL-5 is pivotal for eosinophil development, maturation, and survival in tissues. Asthma severity has been related to both airway and peripheral blood eosinophilia. Areas covered: In this review, we present the pharmacokinetics and pharmacodynamics of benralizumab in addition to efficacy and safety data in the treatment of severe eosinophilic asthma. Expert commentary: Benralizumab is a potent biologic targeting the IL-5 receptor on eosinophils and basophils leading to depletion of target cells in peripheral blood and tissues. It is effective in reducing asthma exacerbations and has an oral corticosteroid-sparing effect in the treatment of asthma patients who are not controlled with medium-to-high-dose inhaled corticosteroids and long-acting beta agonists. The side effect profile is similar to placebo with the most reported adverse events being nasopharyngitis and worsened asthma in the pivotal trials. Its long-term safety is currently under investigation.

Abstract:BACKGROUND BioGlue is a commonly used sealant in thoracic surgery. Prolonged air leak and presence of bronchopleural fistulae (BPF) are often encountered in clinical practice. We therefore, investigated the role and the efficacy of BioGlue in these scenarios. METHODS A systematic review was conducted by searching Medline [1966-2016] and Cochrane Central Register of Controlled Trials (CENTRAL) [1999-2016] along with reference lists of the included studies. Included studies reported on thoracic surgery operations and use of BioGlue in thoracic surgical procedures, whereas excluded studies met at least one of the following criteria: non-English language studies, non-human population, studies on surgical specialties other than Thoracic surgery, reviews and meta-analyses and sealants other than BioGlue. RESULTS Twelve studies with a total number of 194 patients were included. Amongst them, 178 were treated for alveolar air leaks (AAL), 14 for BPF and 2 for lymphatic leaks. BioGlue was utilized at the time of initial operation in 172 (96.7%) patients for AAL, while at secondary intervention in 13 (92.9%) for BPF and 1 (50%) for lymphatic leak. In terms of AAL, only 2 out of 4 studies showed statistically significant reduction in duration of air leak, duration of intercostal drainage and length of stay (LOS) when BioGlue was applied. No complications were encountered after using BioGlue in sealing BPF, apart from the re-application of BioGlue in 3 cases. CONCLUSIONS Although BioGlue has been shown to be efficient in treating AAL, it should be used with caution against BPF, despite encouraging preliminary results. Potential adverse effects must always be taken into consideration. Future randomized controlled trials are warranted in an attempt to establish its benefit in current clinical practice.

Abstract:Background: Older patients with severe asthma (SA) often have more comorbidities and are at higher risk of treatment related side effects. Most phase 3 biologic (mAb) studies excluded patients ≥75 years. We therefore evaluated clinical outcomes in patients ≥75 years receiving mAb therapy at our centre. Methods: Retrospective review of all patients ≥75 years treated with mAb for SA between January '17 and August '23. Results are median (IQR). Results: Of 74 patients, 44 (59%) were male, age 79 (77-82) years. 69 (95%) had adult-onset asthma. 30 (45%) patients were on benralizumab, 39 (53%) mepolizumab, 4 (5%) omalizumab, and 1 (1%) dupilumab. 71 (95%) were self-administering the mAb at home. After 12 months of treatment, 64 (86%) had a positive response to mAb defined as ≥50% reduction in exacerbations and/or maintenance oral corticosteroid (mOCS). Exacerbations reduced from 4 (0-14) in the year pre-mAb to 0 (0-5), p<0.001. Of the 36 patients on mOCS, 20 (55%) weaned off OCS for asthma. ACQ6 improved: 2.8 (0.0-5.5) to 1.5 (0.0-4.5), p<0.001. Asthma remission (off mOCS, no exacerbations, and ACQ <1.5) was achieved by 10 (14%) patients. 6 (8.1%) were mAb non-responders. There was a high-prevalence of polypharmacy- 89% were prescribed ≥5 medications. Multi-morbidity was common: 100% had ≥2 comorbidities, 96% had ≥3 comorbidities and 67% had ≥4 comorbidities. 8 patients died during the review period, none due to asthma. Adverse events were rare (4 patients, 5%): rash, joint pains, hair loss and weight loss. Conclusion: Despite a high prevalence of co-morbidities and polypharmacy, mAb use in patients ≥75 years with SA has similar, good clinical outcomes to patients <75 years in clinical trials and real-world studies.

Abstract:Benralizumab is a promising drug for severe uncontrolled asthma. This study aimed to clarify the effectiveness of benralizumab in a real-life setting.Subjects included 24 patients with severe type 2 asthma who received benralizumab between April 2018 and July 2019. Changes in parameters, exacerbation frequency, and oral corticosteroid (OCS) use after 4 and 24 weeks of administration were examined. The parameters included the Global Evaluation of Treatment Effectiveness (GETE) scale, Asthma Control Questionnaire (ACQ), Asthma Control Test (ACT), blood eosinophils, fractional exhaled nitric oxide (FeNO), and spirometry. The response to treatment was defined as follows: for patients with exacerbations or OCS use before treatment initiation, a reduction of ≥50% in exacerbation frequency or OCS use; and for patients without exacerbations or OCS use, an improvement of ≥0.5 in ACQ scores and ≥3 in ACT scores, or of ≥10.38% in FEV1.Twenty-one patients completed the treatment for 24 weeks. Excellent and good GETE scales and ACQ and ACT improvement were found in 67% of the patients at 4 weeks, and the effect continued until 24 weeks. The patients' rate with exacerbations was significantly reduced compared to the previous 24 weeks before administration. In 17 patients receiving OCS, the use could be reduced or quit in 14 patients. Overall, 16 patients (76.2%) met the responder definition and could be predicted by the baseline eosinophil count and FeNO levels with the best cutoff values of 100/μL and 40 ppb, respectively.Blood eosinophil and FeNO could predict benralizumab effectiveness.

Abstract:Background: Asthma exacerbations cause major morbidity and their treatment with oral corticosteroids [OCS] can lead to toxicity. In clinical trials, benralizumab, an Interleukin-5 receptor-α blocker, reduced exacerbations by approximately 50%. Research showed that exacerbations on mepolizumab are either eosinophilic or infective. Aim of BenRex was to study inflammatory and physiological features of exacerbations while on benralizumab. Methods: 15 UK centres participated. Benralizumab was initiated as per NICE/SMC guidance. Subjects were asked to contact the site if they exacerbated and before starting OCS/antibiotics, to have clinical review, spirometry, fractional exhaled nitric oxide (FeNO) and give samples of sputum, urine, nasosorption, gargle and blood. Results: Of 157 subjects enrolled, 91 (58%) exacerbated during the 12-18 month follow up. Of 272 exacerbations, 120 visits were on-site before treatment. See participant characteristics with and without an exacerbation in the Table. At exacerbation, median [IQR] for blood eosinophils x109/L was 0 [0,0], sputum eosinophils% was 0 [0,0] and FeNO ppb was 49 [29,92]. Median [IQR] sputum neutrophil% was 66.63[34.00, 84.00] and mean (SD) CRP mg/L was 14.6 (23). Conclusion: During exacerbations on benralizumab, eosinophils remain suppressed in the blood and airways, FeNO remains elevated and infective features such as sputum neutrophilia and rise in CRP occur.

Abstract:Background: Biological drugs have been recognized as a breakthrough in the treatment of severe refractory asthma. This retrospective real-life observational study aims to evaluate the effect of add-on benralizumab on lung function, exacerbation rate, oral corticosteroids (OCS) reduction and asthma control questionnaire (ACQ) score after 52 weeks.Methods: In this observational study, a cohort of 18 patients with severe eosinophilic asthma (SEA) according to the ERS / ATS and GINA 2020 classifications, with reference to the Pulmonology Unit of the Azienda USL - IRCCS, Reggio Emilia, Italy, were enrolled from 1 September 2019 to 31 August 2020. For each patient, the following data were collected: demographic data (age, sex, age of onset of asthma, history of smoking and atopy); comorbidity; clinical data (lung function, exacerbations, emergency room visits and hospitalizations); asthma control questionnaire (ACQ); biomarkers (blood eosinophil count and total serum IgE); asthma control drugs as high-dose inhaled corticosteroids / long-acting beta-adrenoceptor agonists (ICS / LABA), long-acting muscarinic antagonists (LAMA), leukotriene receptor antagonists (LTRA), theophylline, OCS. The benralizumab 30 mg treatment schedule was based on the currently recommended dosing regimen.Results: After end-of-treatment (EOT), a complete weaning of all patients from OCS was confirmed. After 26 weeks, the number of exacerbations decreased from 2.90 to 0.05 (p<0.0001), hospitalizations and ACQ score decreased from 3.37 to 0.97 (p<0.0001). At EOT, the number of exacerbations was unchanged, while no hospitalizations had occurred. Overall, lung function markedly improved over the study period. After 52 weeks, the increase in FEV1 from baseline was 26.8% (p=0.0002). The subset of patients with nasal polyposis (NP) had an increase of nearly 50% (1008 ml) and patients with blood eosinophils count (BEC) greater than 500 cells/μl showed an increase of 68% (1081 ml) in FEV1 at EOT.Conclusions: The notable improvement in respiratory function is a significant result in this study and it is much higher than what has emerged to date. This result, together with the OCS sparing effect and the excellent clinical control of asthma, makes benralizumab a reliable and safe therapeutic option for SEA.
Evaluation of the Polycentric above Knee Prosthesis

Abstract:The optimal design of human lower limb prostheses, in particular of knee devices, is fundamental in order to restore the lost functionality and aesthetic aspect of the amputee’s locomotion. Among all knee devices, the four-bar linkage is still the most widespread mechanism, since, despite its simplicity, it allows the prosthesis to be sufficiently stable and, at the same time, to replicate the natural motion of the joint with a sufficient accuracy. This paper presents an optimization procedure for the synthesis of a four-bar linkage for knee prosthesis. Starting from an experimental reference motion and given some patient-specific requirements related to his capacity to control and to stabilize the prosthesis, the procedure identifies the four-bar linkage that best-fits the experimental motion, at the same time respecting the given specifications. During optimization procedure a set of four bar knee configurations are evaluated to allow a more normal gait to be achieved. Geometry of these new designs increases stability while allowing swing flexion, thereby rendering a locking feature unnecessary. Simulation shows that increase in instant center will be apple advantage for the user and also minimize the energy consumption. Extra toe clearance as compared to single axis knee is also providing a more natural gait.
Rapid onset of effect of benralizumab in a severe eosinophilic and allergic asthma patient with allergic bronchopulmonary aspergillosis

Abstract:There is limited data on the use of benralizumab in patients with severe asthma, who have allergic bronchopulmonary aspergillosis (ABPA). We report the case of a 65-year-old woman with combined severe eosinophilic and allergic asthma, who presented with refractory respiratory symptoms, hypereosinophilia and high immunoglobulin E (IgE) level. The patient had consistently poor Asthma Control Test (ACT) scores, despite a maximum dose of inhalation therapy. Upon further investigations, she was diagnosed with concomitant ABPA. The patient was started on oral prednisolone and itraconazole, but her symptoms persisted. She was then started on subcutaneous omalizumab, but switched to benralizumab after developing a severe allergic reaction. The patient experienced rapid clinical improvements after the first dose of subcutaneous benralizumab. Benralizumab demonstrated a significant role in reducing the exacerbation rate and oral corticosteroid use in this patient, as well as improving lung function, asthma control, and quality of life measures.
Navigating biologic therapies in elderly asthma

Abstract:The prevalence of asthma among the elderly population has witnessed a notable rise, presenting unique challenges in diagnosis and management. Biologic therapies, such as omalizumab, mepolizumab, reslizumab, benralizumab, dupilumab, and tezepelumab, have demonstrated efficacy in targeting specific pathways associated with severe asthma in elderly individuals. However, a significant research gap exists in the application of these therapies in elderly asthma patients. Despite the considerable size of the elderly asthma population and the social and economic burden that this specific demographic imposes on society, the available body of research catering to this group is limited. Notably, no RCTs have been expressly designed for the elderly across all asthma biologic therapies. Moreover, most RCTs have set upper age cutoffs, commonly 75 years old, and exclusion criteria for common comorbidities in the elderly, thus marginalizing this group from pivotal research. This underscores the crucial need for intentional inclusion of elderly participants in separately designed clinical trials and more researches, aiming to augment the generalizability of findings and enhance therapeutic outcomes. Given the distinct physiological changes associated with aging, there may be a concern regarding the efficacy and safety of biologic therapies in the elderly compared to non-elderly adults, posing a barrier to their use in this population. However, observational studies have shown similar benefits of these therapies in elderly individuals as seen in non-elderly adults. Other anticipated challenges related to initiating biologic therapy in elderly people with asthma including dosing consideration and monitoring strategies, which are important areas of investigation for optimizing asthma management will be discussed in this review. In summary, this review navigates the current landscape of biologic therapies for elderly asthma, offering valuable insights for various stakeholders, including researchers, healthcare providers, and policymakers, to advance asthma care in this vulnerable population. We propose that future research should concentrate on tailored, evidence-based approaches to address the undertreatment of elderly asthma patients.
Benralizumab in severe eosinophilic asthma by previous biologic use and key clinical subgroups: real-world XALOC-1 programme

Abstract:Background Pivotal Phase 3 trials and real-world studies have demonstrated benralizumab's overall efficacy and safety in severe eosinophilic asthma (SEA). Additional large-cohort data are needed to confirm its real-world effectiveness in SEA according to previous biologic use and key baseline characteristics important for treatment selection. Methods XALOC-1 is a large, multinational, retrospective, observational, real-world study programme of benralizumab in adults with SEA. This 48-week integrated analysis assessed annualised exacerbation rate (AER), maintenance oral corticosteroid (mOCS) use, asthma symptom control and lung function during a 12-month baseline period and up to 48 weeks after benralizumab initiation. Subgroup analyses were based on previous biologic use and key baseline clinical characteristics (mOCS use, blood eosinophil count, exacerbation history, age at asthma diagnosis, fractional exhaled nitric oxide level and presence of atopy and chronic rhinosinusitis with nasal polyps). Results Of 1002 patients analysed, 380 were biologic-experienced. At Week 48, 71.3% were exacerbation-free ( versus 17.2% at baseline); relative reduction in AER was 82.7% overall and 72.9% in biologic-experienced patients; rates were maintained across all key clinical characteristic subgroups. Of patients using mOCS at baseline (n=274), 47.4% (130/274) eliminated their use by Week 48; the mean reduction from baseline in daily dose was 51.2% and, notably, 34.9% in biologic-experienced patients (n=115). Clinically significant improvements in asthma symptom control and lung function were observed. Conclusion In this large, real-world programme, SEA patients treated with benralizumab had substantial improvements in clinical outcomes irrespective of previous biologic use and key clinical characteristics important to therapeutic decision-making in clinical practice.

Abstract:Background: Benralizumab is indicated as add-on therapy in patients with uncontrolled, severe eosinophilic asthma; it has not yet been evaluated in a large Asian population with asthma in a clinical trial. Objective: To evaluate the efficacy and safety of benralizumab in patients with severe asthma in Asia. Methods: MIRACLE (NCT03186209) was a randomized, Phase 3 study in China, South Korea, and the Philippines. Patients aged 12-75 years with severe asthma receiving medium-to-high-dose inhaled corticosteroid/long-acting β2-agonists, stratified (2:1) by baseline blood eosinophil count (bEOS) (≥300/μL; <300/μL), were randomized (1:1) to benralizumab 30 mg or placebo. Endpoints included annual asthma exacerbation rate (AAER; primary endpoint), change from baseline at Week 48 in pre-bronchodilator (BD) forced expiratory volume in 1 second (pre-BD FEV1) and total asthma symptom score (TASS). Safety was evaluated ≤ Week 56. Results: Of 695 patients randomized, 473 had baseline bEOS ≥300/μL (benralizumab n = 236; placebo n = 237). In this population, benralizumab significantly reduced AAER by 74% (rate ratio 0.26 [95% CI 0.19, 0.36], p < 0.0001) and significantly improved pre-BD FEV1 (least squares difference [LSD] 0.25 L [95% CI 0.17, 0.34], p < 0.0001) and TASS (LSD -0.25 [-0.45, -0.05], p = 0.0126) versus placebo. In patients with baseline bEOS <300/μL, there were numerical improvements in AAER, pre-BD FEV1, and TASS with benralizumab versus placebo. The frequency of adverse events was similar for benralizumab (76%) and placebo (80%) in the overall population. Conclusions: MIRACLE data reinforces the efficacy and safety of benralizumab for severe eosinophilic asthma in an Asian population, consistent with the global Phase 3 results.

Abstract:Introduction: Severe eosinophilic asthma (SEA) is marked by persistent airway inflammation and recurrent exacerbations, unmitigated by standard high-dose inhaled therapies. Benralizumab, a humanized monoclonal antibody, targets the interleukin 5 receptor alpha chain on eosinophils, providing a unique treatment option. Materials and Methods: In this study, 24 SEA patients (11 women and 13 men, average age 60) received benralizumab 30 mg. The regimen consisted of 30 mg injections every 4 weeks for the initial three doses, then every 8 weeks thereafter, aiming to assess the therapy's long-term impact on eosinophil counts, lung function, and quality of life over three years. Results: Treatment significantly reduced blood eosinophil levels (p=0.001) and improved lung function, as shown by increases in the Tiffeneau-Pinelli index (p=0.002) and forced vital capacity (p < 0.001). Quality of life significantly improved from the first dose (p < 0.001). However, after over three years, 5 patients developed side effects, notably chronic sinusitis, leading to discontinuation in 2 cases due to non-response to sinusitis treatment. Conclusion: Benralizumab effectively reduces eosinophil counts, enhances lung function, and improves quality of life in SEA patients over the long term. Yet, a small fraction experienced adverse effects such as chronic sinusitis, highlighting the need for vigilant monitoring and management in prolonged treatments. This study confirms benralizumab's sustained efficacy and underscores the importance of tailored patient care strategies.

Abstract:Objective:To assess the clinical effectiveness and safety of Omalizumab for treating pediatric allergic asthma in real world in China.Methods:The clinical data of children aged 6 to 11 years with allergic asthma who received Omalizumab treatment in 17 hospitals in China between July 6, 2018 and September 30, 2020 were retrospectively analyzed.Such information as the demographic characteristics, allergic history, family history, total immunoglobulin E (IgE) levels, specific IgE levels, skin prick test, exhaled nitric oxide (FeNO) levels, eosinophil (EOS) counts, and comorbidities at baseline were collected.Descriptive analysis of the Omalizumab treatment mode was made, and the difference in the first dose, injection frequency and course of treatment between the Omalizumab treatment mode and the mode recommended in the instruction was investigated.Global Evaluation of Treatment Effectiveness (GETE) analysis was made after Omalizumab treatment.The moderate-to-severe asthma exacerbation rate, inhaled corticosteroid (ICS) dose, lung functions were compared before and after Omalizumab treatment.Changes in the Childhood Asthma Control Test (C-ACT) and Pediatric Asthma Quality of Life Questionnaire (PAQLQ) results from baseline to 4, 8, 12, 16, 24, and 52 weeks after Omalizumab treatment were studied.The commodity improvement was assessed.The adverse event (AE) and serious adverse event (SAE) were analyzed for the evaluation of Omalizumab treatment safety.The difference in the annual rate of moderate-to-severe asthma exacerbation and ICS reduction was investigated by using t test.The significance level was set to 0.05.Other parameters were all subject to descriptive analysis.A total of 200 allergic asthma patients were enrolled, including 75.5% ( n=151) males and 24.5% ( n=49) females.The patients aged (8.20±1.81) years. Results:The median total IgE level of the 200 patients was 513.5 (24.4-11 600.0) IU/mL.Their median treatment time with Omalizumab was 112 (1-666) days.Their first dose of Omalizumab was 300 (150-600) mg.Of the 200 cases, 114 cases (57.0%) followed the first Omalizumab dosage recommended in the instruction.After 4-6 months of Omalizumab treatment, 88.5% of the patients enrolled ( n=117) responded to Omalizumab.After 4 weeks of treatment with Omalizumab, asthma was well-controlled, with an increased C-ACT score [from (22.70±3.70) points to (18.90±3.74) points at baseline]. Four-six months after Omalizumab administration, the annual rate of moderate-to-severe asthma exacerbation had a reduction of (2.00±5.68) per patient year( t=4.702 5, P<0.001), the median ICS daily dose was lowered [0 (0-240) μg vs. 160 (50-4 000) μg at baseline] ( P<0.001), the PAQLQ score was improved [(154.90±8.57) points vs. (122.80±27.15) points at baseline], and the forced expiratory volume in one second % predicted (FEV 1%pred) was increased [(92.80±10.50)% vs. (89.70±18.17)% at baseline]. In patients with available evaluations for comorbidities, including allergic rhinitis, atopic dermatitis or eczema, urticaria, allergic conjunctivitis and sinusitis, 92.8%-100.0% showed improved symptoms.A total of 124 AE were reported in 58 (29.0%) of the 200 patients, and the annual incidence was 0(0-15.1) per patient year.In 53 patients who suffered AE, 44 patients (83.0%) and 9 patients (17.0%) reported mild and moderate AE, respectively.No severe AE were observed in patients.The annual incidence of SAE was 0(0-1.9) per patient year.Most common drug-related AE were abdominal pain (2 patients, 1.0%) and fever (2 patients, 1.0%). No patient withdrew Omalizumab due to AE. Conclusions:Omalizumab shows good effectiveness and safety for the treatment of asthma in children.It can reduce the moderate-to-severe asthma exacerbation rate, reduce the ICS dose, improve asthma control levels, and improve lung functions and quality of life of patients.

Abstract:This article presents the experience of successfully switching therapy from omalizumab 150 mg to benralizumab 30 mg/1 ml in a patient with a combined allergic and eosinophilic phenotype in the presence of hypersensitivity to nonsteroidal anti-inflammatory drugs. The effectiveness of biological therapy was evaluated when switching from omalizumab 150 mg subcutaneously at a dose of 600 mg for 36 weeks. Therapy for the drug benralizumab 30 mg/1 ml subcutaneously the first three injections monthly, the rest a month later for 52 weeks with bronchial asthma (BA), a severe uncontrolled course with a combined allergic and eosinophilic phenotype in the presence of hypersensitivity to nonsteroidal anti-inflammatory drugs in a patient Ch., born in 2004. Switching therapy from omalizumab 150 mg to benralizumab 30 mg/1 ml allowed to achieve complete control of asthma symptoms (AST = 23 points), to achieve the absence of asthma exacerbations during 52 weeks, restore respiratory function to normal values, as well as improve the quality of life. The study reflects the good tolerability, high efficacy and safety of biological therapy when switching from one genetically engineered biological drug (GIBP) omalizumab 150 mg to another GIBP benralizumab 30 mg/1 ml in severe uncontrolled asthma with a combined allergic and eosinophilic phenotype in the presence of hypersensitivity to nonsteroidal anti-inflammatory drugs. Therapy with benralizumab 30 mg/1 ml in severe BA has demonstrated a more effective clinically significant improvement in the course of the disease, control of symptoms of the disease. Reduction of exacerbations, normalization of respiratory function indicators, complete control of the disease has been achieved. Consequently, the use of different biological molecules for the therapy of BA with a combined allergic and eosinophilic phenotype contributes to achieving disease control, improving the patient's quality of life and reducing the dose of oral glucocorticosteroids. The targeted biological drug benralizumab 30 mg/1 ml has a targeted effect on the key links in the pathogenesis of severe uncontrolled asthma with a combined allergic and eosinophilic phenotype in the presence of hypersensitivity to nonsteroidal anti-inflammatory drugs and reduces the burden of severe disease.

Abstract:Background: Asthma is a chronic respiratory condition that affects millions of adults, and reduces patients quality of life(QoL). Several studies proven that biological therapy(BT) may be effective in the improvement of lung pulmonary function(LPF), asthma control(AC) and QoL. In this study, the authors propose to evaluate LPF, AC, asthma exacerbations(AE) and QoL before and after BT. Method: Retrospective study including older asthma patients under BT followed in a Severe Asthma Unit in a Portuguese hospital. Data was collected from the patient's clinical files:demographic characteristics, type of BT, LPF, AE and questionnaires to assess AC and QoL before and 12months after BT. Results: Of the 47patients,30 were female, mean age 74years old. 21 under mepolizumab, 17omalizumab, 8benralizumab and 1reslizumab. LFT was assessed at baseline and after BT. There was an increase of FEV1 in all patients. Regarding AC, the mean value of ACT score before treatment was 14,5 and 12months after was 20,7 points. Regarding AE, before BT all the patients had AE in the last year, and 12months after BT, only 36.1%had AE, of which only 4patients required hospitalization. We also found an improvement in the patient's QoL, measured by mini-AQLQ from 3.2 to 5.1 points. Conclusion: This study demonstrates that the diagnostic investigation of asthma should be carried out regardless of the patient's age and that BT can be an effective therapy even in elderly asthmatic patients, by improving LPF, by reducing AE and improving QoL. Although in the short term, BT may be associated with higher costs, in the long term it appears to reduce healthcare costs as it reduces the need to access healthcare services.
