Abstract:Objective To construct an evaluation index system for the core competence of tumor chemotherapy specialist nurses in China and to provide standards for training of tumor chemotherapy specialist nurses.Methods Through literature research and expert meeting,the first draft of index system was established,and consultation questionnaire was compiled.Two rounds of consultation were conducted with 23 experts in China using Delphi method.AHP method was used to determine the weights of indicators at all levels.Results The effective rates of 2 rounds of expert consultation were 100％ and 73.91％,respectively.In the 2 rounds of consultation,the expert authority coefficients were 0.855 and 0.867,respectively.The coordination coefficient of first and second level indexes and expert opinions were 0.693 and 0.371 (P＜0.001),respectively.The final evaluation index system for the core competence of tumor chemotherapy specialist nurses included 5 first-level indexes (positive professional attitude,sufficient clinical practice ability,strong educational counseling ability,basic clinical research and evidencebased practice ability,and basic management ability),16 second-level indexes,and 59 third-level indexes.Conclusion The evaluation index system for chemotherapy specialist nurses based on core competence determined by expert consultation is scientific and reasonable,and can provide a powerful basis for training qualified chemotherapy specialist nurses.
Construction of an index system of core competence assessment for sleep medicine nurse specialists in China: a Delphi study

Abstract:Aim: To construct an evaluation index system for the core competence of nurses in sleep medicine specialties. Background: Specialized nurses in sleep medicine must handle treatment, nursing, and management well, which requires nurses to have excellent competence in knowledge, skills, and ability. However, a competency evaluation system for sleep medicine nurses has not been established in China. Methods: We used a literature review and an expert meeting to establish a draft indicator system. Subsequently, two rounds of correspondence were conducted with 27 experts from 5 provinces using the Delphi method to solicit their opinions on the core competency evaluation indicators for sleep medicine nurse specialists and to qualitatively evaluate the experts' scores. The consensus was defined in advance as a mean score of 4.0 or above, with at least 75% agreement among participants. In this way, the final indicator framework was determined. Results: The final evaluation indexes of the core competencies for sleep medicine Nurse Specialists included 6 first-level indexes (Practical ability, Theoretical Knowledge, Critical thinking, Communication and coordination, Nursing Management, and Professional Development), 16 s-level indexes, and 64 third-level indexes. The effective response rates of the two expert consultation rounds were 100%. The expert authority coefficients were 0.878 and 0.865 in the first and second rounds of consultation, respectively. In the second round of consultation, the first, second, and third indexes of Kendall's coefficient of concordance were 0.373, 0.351, and 0.286, respectively (P < 0.05). Conclusion: The core competence evaluation index system for sleep medicine nurses established in this study is scientific and reliable and can provide a theoretical reference for the training, assessment, and evaluation of sleep medicine nurses in the future. Implications for nursing management: The evaluation index system of sleep medicine nurses' core competence will provide an effective practical framework for nursing management to evaluate, train, and examine the core competence of sleep medicine nurses.

Abstract:Abstract Aim and objective This study was to establish an index system for the evaluation of Chinese infectious disease specialist nurses’ core competence. Background The index system for the evaluation of infectious disease specialist nurses’ core competence has not been established. Design A two-round Delphi survey was conducted to seek opinions from experts about the index system for the evaluation of infectious disease specialist nurses’ core competence. Methods The study adopted several research methods, including literature retrieval, theoretical analysis and qualitative research. Based on the above method, the draft of core competence evaluation index system of infectious disease specialist nurses was constructed. A Delphi survey was used for the study of 30 infectious disease experts from 8 provinces and cities around China. A modified recommendation for the Conducting and Reporting of Delphi studies (CREDES) was also used to guide this study. A STROBE checklist was used. Results The Core Competence Evaluation Index System of Infectious Disease Nurses is composed of 6 primary indicators, namely, Nursing Abilities for Infectious Diseases, Infection Prevention and Control Abilities, Responsiveness to Infectious Diseases, Professional Development Abilities, Communication and Management Abilities, and Professionalism and Humanistic Accomplishment, 16 secondary indicators and 47 tertiary indicators. The authority coefficient, judgment coefficient and familiarity degree of Delphi experts were 0.923, 0.933 and 0.913 respectively. Conclusions The evaluation index system of core competence of diseases specialist nurses is scientific and reliable. It can be reference for future training and assessment of Chinese infectious disease specialist nurses. Relevance to clinical practice Infectious disease specialist nurses are the main force for infectious disease nursing. Their core competence is related to the quality of infectious disease nursing and treatment. The core competence of the nurses is important for identification of training strategies and can be regarded as reference for nurse assessment and promotion. The construction of the index system is based on the consensus of infectious disease experts, which is not only helpful to standardize the training strategies and selection standards of infectious disease specialist nurses in the future, but also meet the society’s needs in clinical infectious disease nursing.

Abstract:Objective To construct the evaluation index system of the core competence of emergency trauma advanced practice nurses,and to provide reference for the training and evaluation of emergency trauma advanced practice nurses. Methods The draft of the evaluation index of the core competence of advanced practice nurses in emergency trauma was prepared by referring to domestic and foreign literature and research group discussion. From March to May 2021,18 experts were consulted by Delphi method for two rounds to complete the screening and modification of the index,and the weight of the index was determined by analytic hierarchy process. Results The evaluation index system of the core competence of advanced practice nurses in emergency trauma included 4 firstlevel indexes（clinical nursing practice ability,trauma nursing non-technical ability,quality continuous evaluation ability,professional development ability）,14 second-level indexes and 42 third-level indexes. The authority coefficients of the two rounds of expert consultation were 0.914,and the effective response rates were 100%. The Kendall harmony coefficients of the first,second and third indexes of the second round of expert consultation were 0.056,0.187and 0.216,respectively（all P＜0.001）. Conclusion The evaluation index of the core competence of advanced practice nurses in emergency trauma is scientific,reasonable,comprehensive and targeted,which can provide reference for the training and evaluation of advanced practice nurses in emergency trauma.
Establishing Performance Evaluation for Quality Inspection of Specialty Nurses

Abstract:Purpose: The present study aimed to establish an index system for the performance evaluation of specialty nurses (SNs) in tertiary hospitals. Background: An objective index system for performance evaluation of SN has not yet been established in China. Design: A 2-round Delphi survey sought opinions from experts about the index system for SNs' performance evaluation in tertiary hospitals in China. Methods: Delphi survey was used to inquire approximately 20 experts from the fields of nursing management, nursing education, and clinical nursing. We determined the weight coefficient of each index of performance evaluation based on the opinion. Finally, the index of the quality evaluation was established for SN. Results: A total of 20 experts from 10 provinces in China reached a consensus on the tertiary indexes of the assessment model. The indexes contained first-level (4), second-level (16), and third-level (24) indicators. The 4 aspects of the performance evaluation, including clinical specialist practice assessment, nursing research, education assessment, medical cooperation recognition, and personal comprehensive ability assessment, reached consensus. Conclusion: Establishing the performance evaluation for SNs aided the SNs in achieving the best clinical practice after training. The performance evaluation still needed to be continuously improved.

Abstract:Objective:To construct the evaluation index system of pediatric specialized nurses' core competence, so as to provide a basis for accurate measurement of pediatric specialized nurses' core competence.Methods:A research group was established in August 2021. Based on the quality onion model and literature analysis, a qualitative interview was conducted in September 2021 with 10 provincial and above hospitals pediatric specialist nurses, 5 pediatric nursing managers, 5 pediatric chief physicians and 5 primary caregivers for children selected by objective sampling. From September 2021 to February 2022, questionnaires were sent to 16 experts via email or WeChat for three rounds of Delphi expert consultation. The evaluation indexes of pediatric specialized nurses' core competence were determined according to the authority coefficient, Kendall coordination coefficient, coefficient of variation and other factors.Results:Among three rounds of expert consultation, the effective recovery rates of the questionnaire were all 100.00% (16/16) , and the authority coefficients were 0.813, 0.888 and 0.895, respectively, and the Kendall coordination coefficients were 0.175 (χ 2=289.348, P<0.01) , 0.239 (χ 2=408.131, P<0.01) and 0.288 (χ 2=467.280, P<0.01) , respectively. The final evaluation index system of pediatric specialized nurses' core competence included 6 first level indicators, 18 second level indicators and 84 third level indicators. Conclusions:The evaluation index system of pediatric specialized nurses' core competence is scientific and reliable, which can be used as an effective tool to measure the core competence of pediatric specialized nurses.

Abstract:Objective? To develop a systematic, scientific and objective competency model for clinical nurse specialist in critical care. Methods? Literature research, focus group interview and the Delphi method were adopted to develop the competency model. The weight of each indicator was determined by the analytic hierarchy process. Results? The effective response rates of two rounds of expert consultation were 100% and 93.8%, the expert authority coefficients were 0.86 and 0.88, and the coordination coefficients were 0.258～0.360 (P＜0.05) and 0.332～0.367 (P＜0.05), respectively. The final competency model included 6 first-level indexes, 18 second-level indexes and 83 third-level indexes. Conclusions? The constructed competency model for clinical nurse specialist in critical care is scientific and reliable, which can provide reference for selection, training, evaluation and management of specialists in the future.

Abstract:Objective With expert questionnaire and investigation,this study aimed to build the orthopedic specialized nurses core competence evaluation index system,which was suitable.Methods At the early stage of this study,literature retrieval method and expert interview were used to build the draft of core competence evaluation index for orthopedic specialized nurse.Then the experts engaged in orthopedic clinical nursing,orthopedic nursing education,nursing management and nursing research,were selected from class a tertiary hospital in Beijing,Tianjin,Shanghai,Shanxi,Sichuan and etc provinces.With two-round of Delphi survey,the authors integrated the experts' advice,and the core competence evaluation index system and items of each level were determinate ultimately.Results By applying the above methods,the authors established the core competence evaluation index system of orthopedics specialized nurse.The evaluation index system includes 6 primary indexes:professional attitude,professional practice ability,critical thinking ability,nursing management ability,communication ability and the professional development ability.The second indicator had 21 items,and the third indicator had 65 items.After two rounds of Delphi method,the expert positive coefficient were 0.90 and 0.94,the authority coefficient were 0.87 and 0.86,the coordination coefficient was 0.247-0.263.Through the data,expert opinion of index entried to converge,they approved the items more.Conclusions Through a series of scientific research methods,the core competence evaluation index system of orthopedic specialized nurse was established.This system may provide comprehensive and quantifiable assessment for orthopedic specialized nurses' evaluation,and has a guiding significance for training orthopedic specialized nurses in the future.

Abstract:Objective:To construct the evaluation index system of core competence for NICU nurses, and to provide basis for training and assessment.Methods:The first draft of the core competence model and evaluation index system for nurses in the NICU was constructed by literature review, and group discussions. The Delphi method was used to conduct 2 rounds of consultation with 19 experts from 8 provinces and cities, and the weight of indexes at all levels was determined by the Delphi method and Precedence Chart method.Results:The constructed evaluation index system for the core competence of nurses in the NICU included 5 first-level indicators, 11 second-level indicators, and 34 third-level indicators. In the two rounds of expert consultation, the experts′ positive coefficients were all 100%, the authority coefficients were 0.934 and 0.945, and the Kendall′s rank-order correlation coefficients of all levels of indicators were 0.255-0.309, 0.202-0.297, all P<0.01. Conclusions:The evaluation index system for NICU nurses is scientific, reliable and feasible, which can be used in clinical nursing.

Abstract:Background: Scientific and technological innovation has become the "key variable" of national social and economic development. Nurses' innovation ability can improve the quality of patient care, thus shorten the hospital stay, reduce the medical costs, and promote the rapid development of the nursing industry. Nowadays nurses have gradually involved in various innovation, but less concerns are focus on the evaluation of nurse's innovation competence. Objective: To develop evaluation index system for nurse's innovation competence. Participants: Eleven experts with innovation experience from different regions in China. Methods: Literature review and qualitative interview were used to establish first version of index system. Then, a two-round Delphi expert consultation was conducted to collect opinions from 11 experts. Then the final version of nurses' innovation competence evaluation index was used in one tertiary hospital for preliminary application. Results: The response rate of the two rounds was more than 90%, the expert familiarity coefficient was 0.818, the judgment coefficient was 0.918, and the authority coefficient was 0.868. The two rounds of expert coordination coefficient test showed P < 0.05. The evaluation index system of nurses' innovation competence includes 4 first-level indicators, 9 second-level indicators and 36 third-level indicators. The scores of innovation production and innovation subject were the lowest among all the first-level indicators when use this index for preliminary application. Conclusion: The constructed evaluation index system of nurses' innovation competence are scientific, reliable and practical, and can be used to promote nurse's innovation competence and innovation conversion competence in clinical practice. Hospital managers should pay more attention on cultivate nurse's innovation competence and provide resources and multi-form platforms to support the nurses with innovative ideas and willingness.
Establishment of evaluation index system for core competence of infection management professionals based on Delphi method

Abstract:OBJECTIVE To establish the evaluation index system for core competence of infection management professionals so as to provide objective basis for training and evaluation of full-time staff.METHODS From Aug 2016 to Oct 2016,the Delphi expert consultation was conducted to 34 experts who were engaged in control of nosocomial infections,nursing,clinical work,and medical technique in 22 hospitals of 7 cities of China.The indexes were screened out,the index weight was determined by using analytic hierarchy process,and the evaluation index system for core competence of infection management professionals was established.RESULTS After two rounds of consultation,the positive coefficients of experts were 94.44％ and 100.00％,respectively;the authority coefficients were 0.84 ％ and 0.88 ％,respectively.The index system for evaluation of core competence of infection management professionals,including 5 primary indexes,15 secondary indexes,and 71 tertiary indexes,was established after two rounds of expert consultation.CONCLUSION The index system for evaluation of core competence of infection management professionals is scientific and reliable,it can provide quantified bases for appointment,selection,training,assessment,and evaluation of the infection management professionals and set the goals for the infection management professionals to achieve through practice.
Development of a tool for assessing the clinical competency of Chinese masters nursing students based on the mini-CEX: a Delphi method study

Abstract:Objective To construct a scientific and systematic competency evaluation tool for master of nursing specialists (MNS) and to provide a reference for the training, assessment and competency evaluation of MNS. Methods A first draft of the indicators for assessing MNS core competencies was developed on the basis of published research and group discussions. Between June and December 2020, the indicators were revised using two rounds of the Delphi expert consultation method, with questionnaires completed by 16 experts from five provinces in China. Results The valid retrieval rate of the two questionnaires was 100.00%, and the coefficient of expert authority was 0.931. The Kendall's concordance coefficients of the two rounds of questionnaires were 0.136 (p<0.05) and 0.147 (p<0.05), respectively. Consensus was reached on the seven dimensions and 52 items of the MNS competency assessment instrument. The instrument dimensions included nurse-patient communication (9 items), health assessment (7 items), clinical decision-making (8 items), operational skills (7 items), health promotion (6 items), humanistic care (9 items) and organisational effectiveness (6 items). Conclusions The MNS competency assessment tool constructed in this study is focused and highly credible. The findings can be used as a guide for the training, assessment and competence evaluation of MNS in the future.

Methods: Based on the iceberg theory, the first draft of the evaluation index system for home care competence of wound ostomy specialist nurses was developed through literature research and semi-structured interview. Under the guidance of the Delphi method, the research group conducted a letter consultation with 20 experts from 11 provinces in China from December 2021 to January 2022. A Delphi Study to Construct an Index of Practice for Community Nurses Providing Transitional Home Care for Patients with Chronic Diseases

Abstract:Community nurses play a key role in providing continuous home care for patients with chronic diseases. However, a perfect system of responsibilities and requirements has not yet been formed, and nurses cannot provide high-quality nursing services for home-based patients. We attempted to construct an index of the scope of practice for community nurses providing home-based transitional care for patients with chronic diseases and to guide nurses in playing an active role in transitional care work. From March to May 2023, 14 representative community nurses from the Shanghai Community Health Service Center were selected for group interviews and 2 rounds of Delphi consultation. A total of 14 valid questionnaires were collected. The authority coefficients were 0.94 and 0.93, and the Kendall coefficients were 0.56 and 0.59 for the 2 rounds of expert consultation ( P < .05). Finally, an index system, including 6 primary indices (transitional caring provider, patient self-management facilitator, community group intervention organizer, home caregiver supporter, family physician team collaborator and supervisor of home medical equipment use, and medical waste disposal) was constructed for community nurses involved in providing home-based transitional care for patients with chronic diseases. The weight values of the 6 indices were 0.19, 0.17, 0.21, 0.13, 0.14 and 0.16, respectively (CR = 0.035, and the consistency test was passed), and 16 secondary indicators and 42 tertiary indicators were identified. In this Delphi study, an index system that can be used to determine community nurses’ roles in providing home-based transitional and continuous care for patients with chronic diseases was successfully established. The index system is considered reliable and easy to use and will provide a meaningful reference for community nurses and policy-makers.

Abstract:ObjectiveTo develop a scientific and reasonable competency evaluation index system for operating room nurses (ORN) in order to provide a tool for selecting, training and evaluating operating room nurses in China.MethodsA multiple-method approach including in-depth interviews and the Delphi and Analytic Hierarchy Process (AHP) was used. Eighteen interviews were undertaken to explore the components of ORN competencies, and then 30 operating room experts were consulted to gain consensus. Finally, the AHP was adopted to determine the relative weights of competency elements, thus increasing the accuracy of the results.ResultsResults of the interviews suggested that there were 32 items of ORN core competency. Results of the DHP (Delphi and AHP) revealed 22 ORN core competencies in four dimensions, such as specialized knowledge, professional ability, personality and self-motivation. The weights of these four dimensions were 0.477, 0.138, 0.256, and 0.128, respectively.ConclusionsThe construction of an ORN core competency evaluation index system in this study contained not only explicit competence but also implicit competence, which reflected the requirements for operating room nurses comprehensively in China. In particular, as the system presented the specialized features of an operating room, it would be of great importance for both nurses and hospitals.

Methods Through semi‐structured interviews, a literature review, and team discussion, the first draft of competency training index system in public health emergencies for community nurses was formed. The Delphi method was used for two rounds of expert consultation to screen, modify and determine indicators. Finally, the analytic hierarchy process was utilized to calculate the weights of indicators at all levels. Results A total of 15 expert consultants were included. The effective recovery rate of the two rounds of expert consultation questionnaire was 100%, the expert authority coefficient was 0.880, 0.887, and the Kendall coordination coefficient was 0.172 and 0.171 (p < 0.001). Expert opinions have determined that the final revised competency index system in public health emergencies for community nurses training consists of 2 primary indicators, 10 secondary indicators, and 46 tertiary indicators. Conclusion The study has developed a competency training index system in public health emergencies for community nurses. This system can serve as a foundation for standardized training in community health service institutions. It is characterized by its high demand, scientific approach, reliability, and rationality.

Construction of the Evaluation Index System of Core Competency of Full-Time Nurses for Emergency Chest Pain

Abstract:Objective:To construct the evaluation index system of core competency of full-time nurses for emergency chest pain, so as to provide basis for the training and management of emergency chest pain full-time nurses.Methods:In January 2021, a research group was established to initially construct the evaluation index system of core competency of full-time nurses for emergency chest pain through literature review, theoretical analysis and group discussion. From March to May 2021, the Delphi method was used to conduct two rounds of consultation with 22 experts from 22 Class Ⅲ Grade A general hospitals in 10 provinces and municipalities to complete indicator construction and weight setting. The expert positive coefficient was expressed by the questionnaire response rate. The expert authority coefficient was calculated by the arithmetic mean of the judgment basis coefficient and the familiarity coefficient. The degree of coordination of expert opinions was expressed by Kendall's W harmony coefficient. Results:The authority coefficient of 22 experts was 0.973, and the positive coefficients of the two rounds of consultation were 100% (22/22) . The Kendall's W of the second round of consultation for the first, second and third level indicators were 0.346, 0.425 and 0.266 respectively ( P<0.01) . Finally, the evaluation index system of core competency of full-time nurses for emergency chest pain included 5 first-level indicators, 14 second-level indicators and 69 third-level indicators. Conclusions:The evaluation index system of core competency of full-time nurses for emergency chest pain is scientific, reasonable and comprehensive, which can provide reference for the training and management of emergency chest pain full-time nurses.
Evaluation of core competencies of nurses by novel holistic assessment system

Abstract:Objective: To develop an integrated measurement system for competence assessment of nurses. Methods: Extensive literature review was performed to acquire data on relevant competence frameworks, study methods, and background information. Initial draft was developed as a result of utilizing theoretical analysis approach for data analysis, comparison, and synthesis. Delphi method was adopted to screen and identify core competencies for completing this assessment system. Content validity of the assessment scale was tested by evaluation from a sub-group of Delphi experts, and a questionnaire survey was conducted among nurses to determine its reliability. Results: This assessment system for core competencies of nurses identified 9 main domains of core competencies (the first-level domains) for nurses, which were divided into a total of 31 constituent subcompetencies (the second-level domains). Items included in the first-level domains and their corresponding weights were as follows: clinical nursing competency (W=0.1271), professional quality (W=0.1250), level of knowledge (W=0.1197), communication competency (W=0.1188), physiopsychological constitution (W=0.1165), management competency (W=0.1027), teaching competency (W=0.0983), scientific research competency (W=0.0972), and development work (W=0.0951). Participating experts rated the importance of the second-level domains by composite index, with the three most important items of sense of responsibility, first-aid skills and specialized nursing skills, while teaching organization, patient family management and research awareness were rated as the lowest importance. Validity examination of the competency assessment scale resulted in the Item-level Content Validity Index (I-CVI) of 0.92-1.00, and the Scale-level Continent Validity Index (S-CVI) of 0.94, with the inter-rate agreement of 0.94. Weighted Kappa coefficient was 0.77 ± 0.14 for test-retest reliability of this assessment scale. Conclusions: This assessment system for core competencies of nurses provides honest, objective and comprehensive evaluation for ICU nurses’ core competencies.
