Abstract:Background In 2016, the UK had 4.5 million people with diabetes, predominantly Type-2 Diabetes Mellitus (T2DM). The NHS allocates £10 billion (9% of its budget) to manage diabetes. Metformin is the primary treatment for T2DM, but 35% of patients don't benefit from it, leading to complications. This study aims to delve into metformin's efficacy using clinical, genomic, and proteomic data to uncover new biomarkers and build a Machine Learning predictor for early metformin response detection. Methods Here we report analysis from a T2DM dataset of individuals prescribed metformin monotherapy from the Diastrat cohort recruited at the Altnagelvin Area Hospital, Northern Ireland. Results In the clinical data analysis, comparing responders (those achieving HbA1c ≤ 48 mmol/mol) to non-responders (with HbA1c > 48 mmol/mol), we identified that creatinine levels and bodyweight were more negatively correlated with response than non-response. In genomic analysis, we identified statistically significant (p-value <0.05) variants rs6551649 ( LPHN3 ), rs6551654 ( LPHN3 ), rs4495065 ( LPHN3 ) and rs7940817 ( TRPC6 ) which appear to differentiate the responders and non-responders. In proteomic analysis, we identified 15 statistically significant (p-value <0.05, q-value <0.05) proteomic markers that differentiate controls, responders, non-responders and treatment groups, out of which the most significant were HAOX1 , CCL17 and PAI that had fold change ∼2. A machine learning model was build; the best model predicted non-responders with 83% classification accuracy. Conclusion Further testing in prospective validation cohorts is required to determine the clinical utility of the proposed model.

Precision Medicine in Type 2 Diabetes: Using Individualized Prediction Models to Optimize Selection of Treatment

Abstract:Despite the known heterogeneity of type 2 diabetes and variable response to glucose lowering medications, current evidence on optimal treatment is predominantly based on average effects in clinical trials rather than individual-level characteristics. A precision medicine approach based on treatment response would aim to improve on this by identifying predictors of differential drug response for people based on their characteristics and then using this information to select optimal treatment. Recent research has demonstrated robust and clinically relevant differential drug response with all noninsulin treatments after metformin (sulfonylureas, thiazolidinediones, dipeptidyl peptidase 4 [DPP-4] inhibitors, glucagon-like peptide 1 [GLP-1] receptor agonists, and sodium–glucose cotransporter 2 [SGLT2] inhibitors) using routinely available clinical features. This Perspective reviews this current evidence and discusses how differences in drug response could inform selection of optimal type 2 diabetes treatment in the near future. It presents a novel framework for developing and testing precision medicine–based strategies to optimize treatment, harnessing existing routine clinical and trial data sources. This framework was recently applied to demonstrate that “subtype” approaches, in which people are classified into subgroups based on features reflecting underlying pathophysiology, are likely to have less clinical utility compared with approaches that combine the same features as continuous measures in probabilistic “individualized prediction” models.
PERSONALIZED GLYCEMIC RESPONSES TO FOOD AMONG INDIVIDUALS WITH TYPE 2 DIABETES IN INDIA: DEVELOPMENT OF A MACHINE LEARNING PREDICTION MODEL

Abstract:Objective: Emerging global evidence demonstrates marked inter-individual differences in post-prandial glucose response (PPGR) although no such data exists in India and prior studies have primarily evaluated PPGR variation in individuals without diabetes. This study sought to develop a machine learning model to predict individual PPGR responses to facilitate the prescription of personalized diets for individuals with type 2 diabetes. Research Design and Methods: Adults with type 2 diabetes and a hemoglobin A1c (HbA1c) ≥7 were enrolled from 14 sites around India. Subjects wore a continuous glucose monitor and logged meals. PPGR was calculated for each meal, based on the incremental area under the curve, and a machine learning predictor of PPGR was developed using stochastic gradient boosting regression. Model calibration and discrimination was assessed using a Pearson product moment correlation and area under a receiver operating curve (AUC), respectively, and its performance was compared to models based only on meal carbohydrate and calorie content. Results: The study included data from 488 patients (mean age 52.5 years, 36% female, mean duration of diabetes 6.4 years, mean hemoglobin A1c 8.16%). Mean PPGR to common foods varied substantially (e.g. PPGR for aloo paratha with curd ranged from 10 to 170 mg/dl*h). PPGR values predicted by the machine learning model were highly correlated with observed PPGR (r=0.69) and model calibration was substantially stronger than for a model based only on calorie (r=0.57) or carbohydrate (r=0.39) content. The machine learning model also demonstrated very strong discriminative ability (AUC 0.80). Conclusions: A machine learning model built with nutritional content, health habits, biometric information and common laboratory data produced highly accurate individualized predictions of PPGRs that substantially outperformed predictions based upon calorie and carbohydrate content. These results could be used to facilitate the delivery of personalized medical nutritional therapy as is widely recommended by type 2 diabetes practice guideline in India and globally.
Clinical, genomic, and proteomic perspectives in the analysis of comorbid conditions in type 2 diabetes mellitus: a retrospective study

Abstract:Aim Type-2 Diabetes Mellitus (T2DM) affects millions globally, with escalating rates. It often leads to undiagnosed complications and commonly coexists with other health conditions. This study investigates two types of prevalent comorbidities related to T2DM—the circulatory system (DCM1) and digestive system diseases (DCM2)—using clinical, genomic and proteomic datasets. The aim is to identify new biomarkers by applying existing machine learning (ML) based techniques for early detection, prognosis and diagnosis of these comorbidities. Methods Here, we report a cross-sectional retrospective analysis from a T2DM dataset of T2DM associated concordant comorbidities (diseases with shared pathophysiology and management) from the Diastrat cohort (a T2DM cohort) recruited at the Northern Ireland Centre for Stratified Medicine (NICSM), in Northern Ireland. Results In the clinical data analysis, we identified that lipidemia was shown to negatively correlate with depression in the DCM1 group while positively correlate with depression in the DCM2 group. In genomic analysis, we identified statistically significant variants rs9844730 in procollagen-lysine (PLOD2), rs73590361 in beta-1,4-N-acetyl- galactosaminyl-transferase (B4GALNT3) and rs964680 in A kinase (PRKA) anchor protein 14 (AKAP14) which appear to differentiate DCM1 and DCM2 groups. In proteomic analysis, we identified 4 statistically significant proteins: natriuretic peptides B (BNP), pro-adrenomedullin (ADM), natriuretic peptides B (NT-proBNP) and discoidin (DCBLD2) that can differentiate DCM1 and DCM2 groups and have built robust ML model using clinical, genomic, and proteomic markers (0.83 receiver operative characteristics curve area, 84% positive predictive value and 83% negative predictive value and a classification accuracy of 83%) for prediction of DCM1 and DCM2 groups. Conclusion Our study successfully identifies novel clinical, genomic, and proteomic biomarkers that differentiate between circulatory and digestive system comorbidities in Type-2 Diabetes Mellitus patients. The machine learning model we developed demonstrates strong predictive capabilities, providing a promising tool for the early detection, prognosis, and diagnosis of these T2DM-associated comorbidities. These findings have the potential to enhance personalized management strategies for patients with T2DM, ultimately improving clinical outcomes. Further research is needed to validate these biomarkers and integrate them into clinical practice.

Abstract:BackgroundDiabetes mellitus is a chronic disease that impacts an increasing percentage of people each year. Among its comorbidities, diabetics are two to four times more likely to develop cardiovascular diseases. While HbA1c remains the primary diagnostic for diabetics, its ability to predict long-term, health outcomes across diverse demographics, ethnic groups, and at a personalized level are limited. The purpose of this study was to provide a model for precision medicine through the implementation of machine-learning algorithms using multiple cardiac biomarkers as a means for predicting diabetes mellitus development.MethodsRight atrial appendages from 50 patients, 30 non-diabetic and 20 type 2 diabetic, were procured from the WVU Ruby Memorial Hospital. Machine-learning was applied to physiological, biochemical, and sequencing data for each patient. Supervised learning implementing SHapley Additive exPlanations (SHAP) allowed binary (no diabetes or type 2 diabetes) and multiple classification (no diabetes, prediabetes, and type 2 diabetes) of the patient cohort with and without the inclusion of HbA1c levels. Findings were validated through Logistic Regression (LR), Linear Discriminant Analysis (LDA), Gaussian Naïve Bayes (NB), Support Vector Machine (SVM), and Classification and Regression Tree (CART) models with tenfold cross validation.ResultsTotal nuclear methylation and hydroxymethylation were highly correlated to diabetic status, with nuclear methylation and mitochondrial electron transport chain (ETC) activities achieving superior testing accuracies in the predictive model (~ 84% testing, binary). Mitochondrial DNA SNPs found in the D-Loop region (SNP-73G, -16126C, and -16362C) were highly associated with diabetes mellitus. The CpG island of transcription factor A, mitochondrial (TFAM) revealed CpG24 (chr10:58385262, P = 0.003) and CpG29 (chr10:58385324, P = 0.001) as markers correlating with diabetic progression. When combining the most predictive factors from each set, total nuclear methylation and CpG24 methylation were the best diagnostic measures in both binary and multiple classification sets.ConclusionsUsing machine-learning, we were able to identify novel as well as the most relevant biomarkers associated with type 2 diabetes mellitus by integrating physiological, biochemical, and sequencing datasets. Ultimately, this approach may be used as a guideline for future investigations into disease pathogenesis and novel biomarker discovery.

Abstract:Precision pharmacotherapy of diabetes requires judicious selection of the optimal therapeutic agent for individual patients. Artificial intelligence (AI), a swiftly expanding discipline, holds substantial potential to transform current practices in diabetes diagnosis and management. This manuscript provides a comprehensive review of contemporary research investigating drug responses in patient subgroups, stratified via either supervised or unsupervised machine learning approaches. The prevalent algorithmic workflow for investigating drug responses using machine learning involves cohort selection, data processing, predictor selection, development and validation of machine learning methods, subgroup allocation, and subsequent analysis of drug response. Despite the promising feature, current research does not yet provide sufficient evidence to implement machine learning algorithms into routine clinical practice, due to a lack of simplicity, validation, or demonstrated efficacy. Nevertheless, we anticipate that the evolving evidence base will increasingly substantiate the role of machine learning in molding precision pharmacotherapy for diabetes.

Abstract:Physical activity is effective for preventing and treating type 2 diabetes, but some individuals do not achieve metabolic benefits from exercise ("non-responders"). We investigated non-responders in terms of insulin sensitivity changes following a 12-week supervised strength and endurance exercise program. We used a hyperinsulinaemic euglycaemic clamp to measure insulin sensitivity among 26 men aged 40–65, categorizing them into non-responders or responders based on their insulin sensitivity change scores. The exercise regimen included VO2max, muscle strength, whole-body MRI scans, muscle and fat biopsies, and serum samples. mRNA sequencing was performed on biopsies and Olink proteomics on serum samples. Non-responders showed more visceral and intramuscular fat and signs of dyslipidaemia and low-grade inflammation at baseline and did not improve in insulin sensitivity following exercise, although they showed gains in VO2max and muscle strength. Impaired IL6-JAK-STAT3 signalling in non-responders was suggested by serum proteomics analysis, and a baseline serum proteomic machine learning (ML) algorithm predicted insulin sensitivity responses with high accuracy, validated across two independent exercise cohorts. The ML model identified 30 serum proteins that could forecast exercise-induced insulin sensitivity changes.

Predicting Type 2 Diabetes Metabolic Phenotypes Using Continuous Glucose Monitoring and a Machine Learning Framework

Abstract:Type 2 diabetes (T2D) and prediabetes are classically defined by the level of fasting glucose or surrogates such as hemoglobin A1c. This classification does not take into account the heterogeneity in the pathophysiology of glucose dysregulation, the identification of which could inform targeted approaches to diabetes treatment and prevention and/or predict clinical outcomes. We performed gold-standard metabolic tests in a cohort of individuals with early glucose dysregulation and quantified four distinct metabolic subphenotypes known to contribute to glucose dysregulation and T2D: muscle insulin resistance, beta-cell dysfunction, impaired incretin action, and hepatic insulin resistance. We revealed substantial inter-individual heterogeneity, with 44% of individuals exhibiting dominance in muscle or liver IR, and 16%, 13%, and 9% exhibiting dominance in beta-cell, incretin, or both, respectively. Further, with a frequently-sampled oral glucose tolerance test (OGTT), we developed a novel machine learning framework to predict metabolic subphenotypes using features from the dynamic patterns of the glucose time-series ("shape of the glucose curve"). The glucose time-series features identified insulin resistance, beta-cell deficiency, and incretin defect with auROCs of 95%, 89%, and 88%, respectively. These figures are superior to currently-used estimates. The prediction of muscle insulin resistance and beta-cell deficiency were validated using an independent cohort. We then tested the ability of glucose curves generated by a continuous glucose monitor (CGM) worn during at-home OGTTs to predict insulin resistance and beta-cell deficiency, yielding auROC of 88% and 84%, respectively. We thus demonstrate that the prediabetic state is characterized by metabolic heterogeneity, which can be defined by the shape of the glucose curve during standardized OGTT, performed in a clinical research unit or at-home setting using CGM. The use of at-home CGM to identify muscle insulin resistance and beta-cell deficiency constitutes a practical and scalable method by which to risk stratify individuals with early glucose dysregulation and inform targeted treatment to prevent T2D.

Integrated biomarker profiling for predicting the response of type 2 diabetes to metformin

Abstract:Aim To explore biomarkers that can predict the response of type 2 diabetes (T2D) patients to metformin at an early stage to provide better treatment for T2D. Methods T2D patients with (responders) or without response (non‐responders) to metformin were recruited, and their serum samples were used for metabolomic analysis to identify candidate biomarkers. Moreover, the efficacy of metformin was verified by insulin‐resistant mice, and the candidate biomarkers were verified to determine the biomarkers. Five different machine learning methods were used to construct the integrated biomarker profiling (IBP) with the biomarkers to predict the response of T2D patients to metformin. Results A total of 73 responders and 63 non‐responders were recruited, and 88 differential metabolites were identified in the serum samples. After being verified in mice, 19 of the 88 were considered as candidate biomarkers. Next, after metformin regulation, nine candidate biomarkers were confirmed as the biomarkers. After comparing five machine learning models, the nine biomarkers were constructed into the IBP for predicting the response of T2D patients to metformin based on the Naïve Bayes classifier, which was verified with an accuracy of 89.70%. Conclusions The IBP composed of nine biomarkers can be used to predict the response of T2D patients to metformin, enabling clinicians to start a combined medication strategy as soon as possible if T2D patients do not respond to metformin.

Improving blood glucose level predictability using machine learning

Abstract:AimsThis study was designed to improve blood glucose (BG) level predictability and future hypoglycemic and hyperglycemic event alerts through a novel patient‐specific supervised‐machine‐learning (SML) analysis of glucose level based on a continuous‐glucose‐monitoring system (CGM) that needs no human intervention, and minimizes false‐positive alerts. Materials And MethodsThe CGM data over 7‐50 nonconsecutive days from 11 type‐1 diabetic patients aged 18‐39 with a mean HbA1C of 7.5 ± 1.2% were analyzed using four SML models. The algorithm was constructed to choose the best‐fit model for each patient. Several statistical parameters were calculated to aggregate the magnitudes of the prediction errors. ResultsThe personalized solutions provided by the algorithm were effective in predicting glucose levels 30 minutes after the last measurement. The average root‐mean‐square‐error was 20.48 mg/dL and the average absolute‐mean‐error was 15.36 mg/dL when the best‐fit‐model was selected for each patient. Using the best‐fit‐model, the true‐positive‐hypoglycemia‐prediction‐rate was 64%, whereas the false‐positive‐ rate was 4.0%, and the false‐negative‐rate was 0.015%. Similar results were found even when only CGM samples below 70 were considered. The true‐positive‐hyperglycemia‐prediction‐rate was 61%. ConclusionsState‐of‐the‐art SML tools are effective in predicting the glucose level values of patients with type‐1diabetes and notifying these patients of future hypoglycemic and hyperglycemic events, thus improving glycemic control. The algorithm can be used to improve the calculation of the basal insulin rate and bolus insulin, and suitable for a closed loop "artificial pancreas" system. The algorithm provides a personalized medical solution that can successfully identify the best‐fit method for each patient.This article is protected by copyright. All rights reserved.
Implementation of five machine learning methods to predict the 52-week blood glucose level in patients with type 2 diabetes

Abstract:Objective: For the patients who are suffering from type 2 diabetes, blood glucose level could be affected by multiple factors. An accurate estimation of the trajectory of blood glucose is crucial in clinical decision making. Frequent glucose measurement serves as a good source of data to train machine learning models for prediction purposes. This study aimed at using machine learning methods to predict blood glucose for type 2 diabetic patients. We investigated various parameters influencing blood glucose, as well as determined the most effective machine learning algorithm in predicting blood glucose. Patients and methods: 273 patients were recruited in this research. Several parameters such as age, diet, family history, BMI, alcohol intake, smoking status et al were analyzed. Patients who had glycosylated hemoglobin less than 6.5% after 52 weeks were considered as having achieved glycemic control and the rest as not achieving it. Five machine learning methods (KNN algorithm, logistic regression algorithm, random forest algorithm, support vector machine, and XGBoost algorithm) were compared to evaluate their performances in prediction accuracy. R 3.6.3 and Python 3.12 were used in data analysis. Results: The statistical variables for which p< 0.05 was obtained were BMI, pulse, Na, Cl, AKP. Compared with the other four algorithms, XGBoost algorithm has the highest accuracy (Accuracy=99.54% in training set and 78.18% in testing set) and AUC values (1.0 in training set and 0.68 in testing set), thus it is recommended to be used for prediction in clinical practice. Conclusion: When it comes to future blood glucose level prediction using machine learning methods, XGBoost algorithm scores the highest in effectiveness. This algorithm could be applied to assist clinical decision making, as well as guide the lifestyle of diabetic patients, in pursuit of minimizing risks of hyperglycemic or hypoglycemic events.

Abstract:Highlights • We used multiple machine-learning algorithms to explore gut microbiota, serum metabolites, and lipids signatures in individuals with T1D. • Our findings provide new evidence investigating the interplay between the identified microbes and serum metabolites/lipids in shaping the host functional status and affecting the development and outcomes of T1D. • The merits of our study also included the well-characterized participants, the targeted and quantitative dimension of multi-omics, and the combined use of machine learning and other biostatistics. Introduction Type 1 diabetes (T1D) is a complex disorder influenced by genetic and environmental factors. The gut microbiome, the serum metabolome, and the serum lipidome have been identified as key environmental factors contributing to the pathophysiological mechanisms of T1D. Objectives We aimed to explore the gut microbiota, serum metabolite, and serum lipid signatures in T1D patients by machine learning. Methods We evaluated 137 individuals in a cross-sectional cohort involving 38 T1D patients, 38 healthy controls, and 61 T1D patients for validation. We characterized gut microbiome, serum metabolite, and serum lipid profiles with machine learning approaches (logistic regression, support vector machine, Gaussian naive Bayes, and random forest). Results The machine learning approaches using the microbiota composition did not accurately diagnose T1D (model accuracy=0.7555), while the accuracy of the model using the metabolite composition was 0.9333. Based on the metabolite composition, 3-hydroxybutyric acid and 9-oxo-ode (area under curve=0.70 and 0.67, respectively, both increased in T1D) were meaningful overlap metabolites screened by multiple bioinformatics methods. We confirmed the biological relevance of the microbiome, metabolome, and lipidome features in the validation group. Conclusion By using machine learning algorithms and multi-omics, we demonstrated that T1D patients are associated with altered microbiota, metabolite, and lipidomic signatures or functions.

Increasingly adopted Continuous Glucose Monitoring (CGM) devices offer detailed, non-intrusive and real time insights into a patient's blood glucose concentrations. Leveraging advanced Machine Learning (ML) Models as methods of prediction of future glucose levels, gives rise to substantial quality of life improvements, as well as providing a vital tool for monitoring diabetes. A regression based prediction approach is implemented recursively, with a series of Machine Learning Models: Linear Regression, Hidden Markov Model, Long-Short Term Memory Network. By exploiting a patient's past 11 hours of blood glucose (BG) concentration measurements, a prediction of the 60 minutes is made. Results will be assessed using performance metrics including: Root Mean Squared Error (RMSE), normalised energy of the second-order differences (ESOD) and F1 score. Research of past and current approaches, as well as available dataset, led to the establishment of an optimal training methodology for the CITY dataset, which may be leveraged by future model development. Performance was aligned with similar state-of-art ML models, with LSTM having RMSE of 28.55, however no significant advantage was observed over classical Auto-regressive AR models. Compelling insights into LSTM prediction behaviour could increase public and legislative trust and understanding, progressing the certification of ML models in Artificial Pancreas Systems (APS).

The Use of Machine Learning Techniques to Determine the Predictive Value of Inflammatory Biomarkers in the Development of Type 2 Diabetes Mellitus

Abstract:Background: Certain inflammatory biomarkers, such as interleukin-6, interleukin-1, C-reactive protein (CRP), and fibrinogen, are prototypical acute-phase parameters that can also be predictors of cardiovascular disease. However, this inflammatory response can also be linked to the development of type 2 diabetes mellitus (T2DM). Methods: We performed a cross-sectional, retrospective study of hypertensive patients in an outpatient setting. Demographic, clinical, and laboratory parameters, such as the Homeostatic Model Assessment of Insulin Resistance (HOMA-IR), CRP, and fibrinogen, were recorded. The outcome was progression to overt T2DM over the 12-year observation period. Results: A total of 3,472 hypertensive patients were screened, but 1,576 individuals without T2DM were ultimately included in the analyses. Patients with elevated fibrinogen, CRP, and insulin resistance had a significantly greater incidence of progression to T2DM. During follow-up, 199 patients progressed to T2DM. Multivariate logistic regression analyses showed that body mass index [odds ratio (OR) 1.04, 95% confidence interval (CI): 1.01-1.07], HOMA-IR (OR 1.13, 95% CI: 1.08-1.16), age (OR 1.05, 95% CI: 1.03-1.07), log(CRP) (OR 1.37, 95% CI: 1.14-1.55), and fibrinogen (OR 1.44, 95% CI: 1.23-1.66) were the most important predictors of progression to T2DM. The area under the receiver operating characteristic curve (AUC) of this model was 0.76. Using machine learning methods, we built a model that included HOMA-IR, fibrinogen, and log(CRP) that was more accurate than the logistic regression model, with an AUC of 0.9. Conclusion: Our results suggest that inflammatory biomarkers and HOMA-IR have a strong prognostic value in predicting progression to T2DM. Machine learning methods can provide more accurate results to better understand the implications of these features in terms of progression to T2DM. A successful therapeutic approach based on these features can avoid progression to T2DM and thus improve long-term survival.

Abstract:Liraglutide, a glucagon-like peptide-1 receptor agonist, is effective in the treatment of type 2 diabetes mellitus (T2DM) and obesity. Despite its benefits, including improved glycemic control and weight loss, the common metabolic changes induced by liraglutide and correlations between those in rodents and humans remain unknown. Here, we used advanced machine learning techniques to analyze the plasma metabolomic data in diet-induced obese (DIO) mice and patients with T2DM treated with liraglutide. Among the machine learning models, Support Vector Machine was the most suitable for DIO mice, and Gradient Boosting was the most suitable for patients with T2DM. Through the cross-evaluation of machine learning models, we found that liraglutide promotes metabolic shifts and interspecies correlations in these shifts between DIO mice and patients with T2DM. Our comparative analysis helped identify metabolic correlations influenced by liraglutide between humans and rodents and may guide future therapeutic strategies for T2DM and obesity.

Machine Learning Reveals Metabolic Signatures in Patients with Type 1 Diabetes

Abstract:Background: Metabolic dysbiosis has been linked to the development of type 1 diabetes. However, there are few studies reflecting the metabolic signatures in patients with type 1 diabetes based on machine learning. Therefore, we aim to investigate the serum metabolic alterations and signatures in patients with type 1 diabetes. Methods: We recruited 29 type 1 diabetes patients and 29 healthy controls with matching age, sex and ethnicity. Serum metabolites were analyzed using liquid chromatograph-mass spectrometry (LC-MS). Four machine-learning approaches (Logistic Regression, Support Vector Machine, Gaussian Naive Bayes, and Random Forest) were used to screen potential T1D-related biomarkers. Results: Between the T1D group and the control group, 150 different metabolites were identified. These metabolites were significantly enriched in three metabolic pathways (P<0.05), purine metabolism, ketone body synthesis and degradation, and methyl butyrate metabolism. We combined comprehensive analysis and four machine-learning algorithmsto screen for biomarkers associated with type 1 diabetes. The top 20 metabolites coincided with 150 metabolites identified by LC-MS. In addition, three metabolites were identified as T1D diagnostic biomarkers, including docosahexaenoic acid, estrane and inosine. The combination of these metabolites also accurately diagnosis type 1 diabetes. Conclusions: In this study, purine metabolism, synthesis and degradation of ketone bodies, and impaired methyl butyrate metabolism were identified as metabolic disturbance associated with type 1 diabetes. These biomarkers by using machine-learning approaches may have important clinical significance in the diagnosis of type 1 diabetes and in the follow of responses to therapeutic interventions. Abstract:Insulin Resistance (IR) is a component of the pathogenesis of type 2 diabetes mellitus (T2DM), and risk factor for cardiovascular and neurodegenerative diseases. Amino acid and lipid metabolomic IR diagnostics associate with future T2DM risk in epidemiological cohorts. Whether these assays can accurately detect altered IR following treatment has not been established. In the present study we evaluated the ability of metabolomic diagnostics to predict altered IR following exercise treatment. We evaluated the performance of two distinct insulin assays and built combined clinical and metabolomic IR diagnostics. These were utilised to stratify IR status in the pre intervention fasting samples in three independent cohorts (META PREDICT (MP, n=179), STRRIDE AT/RT (S2, n=116) and STRRIDE PD (SPD, n=149)). Linear and Bayesian projective prediction strategies were used to evaluate biomarkers for fasting insulin and HOMA2IR and change in fasting insulin with treatment. Both insulin assays accurately quantified international standard insulin (R2>0.99), yet agreement for fasting insulin was less congruent (R2=0.65). Only the high-sensitivity ELISA assay could identify the mean effect of treatment on fasting insulin. Clinical metabolomic models were statistically related to fasting insulin (R2 0.33 0.39) but had modest capacity to classify HOMA2IR at a clinically relevant threshold. Furthermore, no model predicted treatment responses in any cohort. Thus, we demonstrate that the choice of insulin assay is critical when quantifying the influence of lifestyle on fasting insulin, while none of the clinical metabolomic biomarkers, validated in cross sectional data, are suitable for monitoring longitudinally changes in IR status
