Abstract:Background Spiritual well-being plays an important role in helping patients cope with disease. Previous studies have investigated the association between social support and spiritual well-being, whereas few studies have explored the relationship in patients with esophageal cancer (EC), and the mechanisms behind this pathway have not been thoroughly examined. Objective This study aimed to explore the relationship between social support and spiritual well-being of Chinese patients with EC aged over 50 years and to analyze whether the relationship was mediated by rumination. Methods A cross-sectional survey was conducted with 197 EC patients. Participants completed the general information questionnaire, the Functional Assessment of Chronic Illness Therapy–Spiritual Scale, the Chinese Event Related Rumination Inventory, and the Perceived Social Support Scale (PSSS). Results Results demonstrated that social support of patients with EC aged over 50 years was positively correlated with spiritual well-being and deliberate rumination and negatively correlated with intrusive rumination; spiritual well-being was positively associated with deliberate rumination and negatively correlated with intrusive rumination. The effect of social support on spiritual well-being was partially mediated by deliberate rumination and intrusive rumination. Conclusions The findings suggest that interventions directed toward enhancing social support and deliberate rumination and reducing the level of intrusive rumination may help patients with EC aged over 50 years improve spiritual well-being.

Exploring the relationship between spiritual well-being and death anxiety in patients with gynecological cancer: a cross-section study

Abstract:Abstract Background In recent years, spiritual well-being has gradually gained the attention of health care providers in China, especially those in oncology departments, who have recognized the importance of improving spiritual well-being in cancer patients. Since most of the current research on spiritual well-being has been carried out in areas with religious beliefs, this study was conducted in the context of no development of formal religion. The purpose of this study was to explore the relationship between death anxiety and spiritual well-being and the related factors of spiritual well-being among gynecological cancer patients. Methods This cross-section study was conducted among 586 gynecological cancer patients. The European Organization for Research and Treatment for Cancer Quality of Life Questionnaire-spiritual well-being32 (EORTC QLQ-SWB32) and Templer's Death Anxiety Scale (T-DAS) were used to measure spiritual well-being and death anxiety. The Multiple Linear Regression Model was used to determine the relationship between spiritual well-being and death anxiety. Results For all participants, the highest QLQ-SWB32 centesimal score was 75.13 on the Relationship with Other scale, and the lowest was 60.33 on the Relationship with Someone or Something Greater Scale. The mean Death Anxiety score was 5.31 (SD 3.18). We found that Relationship with Someone or Something Greater was the only scale not associated with death anxiety. Overall, patients with lower death anxiety have a higher level of spiritual well-being. Besides, a high Relationship with Other score was associated with living with a partner (B = 2.471, P < 0.001) and married (B = -6.475, P = 0.001). Patients with higher Global-SWB were retired (B = 0.387, P = 0.019). Conclusions Our study found that the spiritual well-being of patients with gynecological cancer in China was no worse than in other countries with religious beliefs and patients with lower death anxiety have a higher level of spiritual well-being. Clinical staff should pay attention to the spiritual health of cancer patients, and spiritual care should be regarded as an essential element in cancer care.

Factors associated with patient-reported subjective well-being among advanced lung or non-colonic gastrointestinal cancer patients

Abstract:Objective: The aim of this study was to determine the factors associated with a feeling of well-being using the Edmonton Symptom Assessment Scale (ESAS)-Feeling of Well-Being item (ESAS-FWB; where 0 = best and 10 = worst) among advanced lung or non-colonic gastrointestinal cancer patients who were referred to an outpatient palliative care clinic (OPCC). We also examined the association of performance on the ESAS-FWB with overall survival (OS). Method: We reviewed the records of consecutive patients with incurable advanced lung cancer and non-colonic gastrointestinal cancer presenting to an OPCC from 1 January 2008 through to 31 December 2013. Descriptive statistics were employed to summarize patient characteristics. Multivariate regression analysis was used to determine the factors associated with ESAS-FWB severity. We also examined the association of ESAS-FWB scores and survival using Kaplan-Meier survival analysis. Results: A total of 826 evaluable patients were analyzed (median age = 62 years, 57% male). Median ESAS-FWB scores were five times the interquartile range (5 × IQR; 3-7). ESAS-FWB score was found to be significantly associated with ESAS fatigue (OR = 2.31, p < 0.001); anxiety (OR = 1.98, p < 0.001); anorexia (OR = 2.31, p < 0.001); cut down, annoyed, guilty, eye opener (CAGE) score (hazard ratio [HR] = 1.80, p = 0.008); and family caregiver distress (HR = 1.93, p = 0.002). A worse ESAS-FWB score was significantly associated with decreased OS (r = -0.18, p < 0.001). However, ESAS-FWB score was not independently associated with OS in the final multivariate model (p = 0.35), which included known major clinical prognostic factors. Conclusions: Worse ESAS-FWB scores were significantly associated with high scores on ESAS fatigue, anorexia, anxiety, CAGE, and family caregiver distress. More research is necessary to understand how palliative care interventions are capable of improving the contributory factors related to ESAS-FWB score.

Abstract:PurposeSpiritual needs are important for many patients with life-limiting illnesses. Knowledge of spiritual needs is a first step to their fulfillment; however, such knowledge is lacking for patients with cancer in mainland China. This study evaluated spiritual needs among patients with cancer in China and analyzed their associations with demographic and clinical characteristics.MethodsA descriptive, cross-sectional study was performed. A general information questionnaire and a Chinese version of the Spiritual Needs Scale were used to collect information about patients with cancer. A total of 200 participants were recruited, and 182 completed the survey.ResultsThe overall average spiritual needs score was 3.15. Of five dimensions investigated, the highest and lowest scores were for “hope and peace” (mean = 3.31) and “relationship with transcendence” (mean = 2.96). Of specific needs, “to be accepted,” “to take responsibility for my life,” and “to alleviate my suffering” (means = 3.36–3.38) were valued as most important by participants. “To talk to someone,” “to pray and participate in religious rituals and services,” and “to feel God with me during my struggle against disease” (means = 2.71–2.91) were considered least important. Participants’ spiritual needs differed significantly according to disease stage (p < 0.001), time since diagnosis (p = 0.002), and hospitalization frequency (p = 0.004).ConclusionsThe spiritual needs of patients with cancer should receive more attention in China. Effective assessment tools and procedures should be established to evaluate the spiritual needs of patients and formulate appropriate interventions at an early stage.

Abstract:Objective To explore the status and influencing factors of spiritual needs of patients with gastrointestinal cancer, so as to help the medical staffs to provide spiritual care for patients with gastrointestinal cancer. Methods The general information questionnaire, disease information questionnaire and the Spiritual Needs Assessment for Patients(SNAP) were used to investigate the spiritual needs of 116 patients with gastrointestinal cancer, analyze the influencing factors of the spiritual needs. Results The spiritual needs score of the patients with gastrointestinal cancer were 32-92(69.7 ± 16.9) points. Regression analysis showed that the influencing factors were religious belief, age of illness and number of children (R2=0.480, F=12.353, P<0.01). Conclusions The level of spiritual needs of the gastrointestinal cancer patients is at middle level.With different religious beliefs, age of illness and children number, the level of spiritual needs is different.
Effects of anxiety, depression, and fatigue on quality of life in early esophageal cancer patients following endoscopic submucosal dissection

Abstract:BACKGROUND: Emotional problems such as anxiety, depression, and fatigue may affect the quality of life (QoL) of cancer patients. However, there are no related studies investigating the effects of emotional problems on the QoL of early esophageal cancer (EC) patients receiving endoscopic submucosal dissection (ESD).METHODS: This is a prospective observational study enrolling 75 early EC patients. Demographic and clinical characteristics of enrolled patients were recorded. QoL was assessed according to the European Organization for Research and Treatment of Cancer QoL (EORTC QLQ-C30). The Beck Anxiety Scale and the Beck Depression Scale were used to measure the severity of anxiety symptoms and depression intensity, respectively. Fatigue was assessed according to the symptom subscale of the EORTC QLQ-C30. Multivariable logistic regression models were constructed to determine the causes of emotional problems and linear regression models were employed to identify the independent risk factors for reduced QoL.RESULTS: Of the enrolled early EC patients, 21, 15, and seven had symptoms of mild, moderate and severe anxiety, respectively. Also, 15, 19, and two patients had symptoms of mild, moderate, and severe depression, respectively. In total, 27 patients frequently felt fatigue. The multivariable logistic regression model revealed that the financial capacity to pay for treatment had an important effect on the prevalence of anxiety and depression symptoms in enrolled patients (both P values <0.001). Age, gender, education status, and tumor size also played a role in emotional problems. Meanwhile, fatigue was found to be related only to age and gender. The mean score for QoL was 76.9±15.2. After adjusting for confounding factors using the multivariable linear regression model, moderate or severe symptoms of anxiety, depression, and fatigue, older age, and insufficient capacity to pay were identified as independent risk factors for reduced QoL.CONCLUSIONS: Emotional distress is a significant problem for early EC patients receiving ESD treatment as it may affect their QoL.

Associations of Changes in Intestinal Flora and Inflammatory Factors with Prognosis of Patients with Esophageal Cancer

Abstract:This study aims to explore the associations of changes in intestinal flora and inflammatory factors with the prognosis of patients with esophageal cancer (EC). A total of 40 EC patients treated and 40 normal people who underwent gastroscopy and CT examination for gastrointestinal discomfort during the same period were selected as the participants of the study. The endotoxin level, colonization ability of intestinal flora, and distribution of intestinal flora (Bifidobacterium, Lactobacillus, Escherichia coli, and Enterococcus) were compared between the two groups. The levels of inflammatory factors interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP), and tumor necrosis factor-α (TNF-α) were also compared between the two groups. All participants were followed up for 3 years, and the associations of survival time with colonization ability of intestinal flora and changes in hs-CRP were analyzed. Finally, the univariate and multivariate logistic regression analyses were performed for related factors affecting the survival time of EC patients. In the observation group, the endotoxin level was significantly higher ( P < 0.05 ), the colonization ability of intestinal flora was significantly weaker ( P < 0.05 ), the levels of Bifidobacterium and Lactobacillus were obviously lower ( P < 0.05 ), and the levels of Escherichia coli and Enterococcus were obviously higher than those in the normal group ( P < 0.05 ). Besides, the observation group had abnormal and evidently higher levels of IL-6, hs-CRP, and TNF-α than the normal group ( P < 0.05 ). The survival time was positively correlated with the colonization ability of intestinal flora ( P < 0.05 ), but negatively correlated with the changes in hs-CRP ( P < 0.05 ). Moreover, the increased level of endotoxin, weakened colonization ability of intestinal flora, abnormal distribution of intestinal flora, and elevated levels of inflammatory factors were all related and independent risk factors affecting the survival time of EC patients. In EC patients, the endotoxin level markedly rises, the colonization ability of intestinal flora declines, and there are intestinal flora disorders and enhanced inflammatory response. With the decline in colonization ability of intestinal flora and the increase of inflammatory response, the survival time of EC patients will be shortened.

Abstract:PurposeAlthough spiritual well-being (SWB) is gaining increasing attention within the international palliative care (PC) guidelines, a lack of insight exists into the correlates and course of SWB among cancer patients. We therefore conducted a prospective observational study to capture trend of SWB and to identify their predictors in Chinese inpatients with terminal cancer receiving short-term PC.MethodsA prospective observational study was conducted of terminal cancer inpatients in the hospice ward, Shengjing Hospital of China Medical University. A total of 108 patients completed self-report questionnaires on Functional Assessment of Chronic Illness Therapy-Spiritual Well-being, Hospital Anxiety and Depression Scale, Numerical Rating Scales, and Life Orientation Scale-Revised anonymously at baseline; SWB, depression, anxiety, and pain were subsequently assessed at 1-week interval. Multilevel regression was used to analyze the temporal course and predictors of SWB.ResultsPatients’ existential well-being (B = − 0.99, p = 0.008; 95%CI = − 1.72 to − 0.26) and meaning dimension (B = − 0.87, p < 0.001; 95% CI = − 1.29 to − 0.43) significantly decreased after admission to the PC unit, but peace and faith did not change over time. Increases in depression and pain were related to lower existential well-being, particularly in the meaning dimension. Optimism–pessimism moderated the linear trend of existential well-being and meaning domain, such that those with higher optimism and lower pessimism paired with a decrease in outcomes.ConclusionsTerminal cancer patients experienced worsening existential well-being, particularly in the meaning facet while hospitalized, indicating that PC should include content that targets the existential concerns of spirituality in China. These findings also supported the need for an integrated PC to address personality traits and emotional and physical distress in this population.

Abstract:BACKGROUND: Cancer is a growing threat to human health. Due to the double torment of cancer and cancer treatment, patients with advanced cancer generally have a low quality of life. At present, there is a lack of systematic spiritual care plans for patients with advanced cancer as well as systematic guidance plans on the specific clinical application of spiritual care for advanced cancer patients. We hypothesized that our care plan would be effective in improving the spiritual and mental health and quality of life of patients with advanced cancer.AIM: To construct a spiritual care plan suitable for Chinese patients with advanced cancer through literature analysis.METHODS: From February to December 2018, through purpose sampling, we selected 100 advanced cancer patients from the Oncology Department and Hospice Ward of a tertiary hospital in Liaoning Province who met the study standards. Patients were randomly divided into experimental and control groups, with 50 cases in each group. The control group received the current routine care, while the experimental group received the advanced cancer spiritual care intervention in addition to routine care.RESULTS: After the intervention, the overall spiritual health score for the experimental group was higher than that of the control group (4.68 ± 1.36 vs 3.63 ± 1.71). The difference between the groups was statistically significant (P < 0.05). The proportion of anxiety-free patients in the experimental group was 95.45%, which was significantly higher than the 60.98% in the control group. Moreover, the proportion of non-depressed patients in the experimental group was 97.73%, which was significantly higher than the 85.37% in the control group (P < 0.05). The overall quality of life score for the experimental group was significantly higher than that of the control group (5.36 ± 1.16 vs 4.39 ± 1.36, P < 0.05).CONCLUSION: Our spiritual care plan for patients with advanced cancer could improve their spiritual health and quality of life and reduce negative mental health symptoms.

Perioperative Comprehensive Supportive Care Interventions for Chinese Patients with Esophageal Carcinoma: a Prospective Study

Abstract:OBJECTIVE: To assess the effects of perioperative comprehensive supportive care interventions on outcome of Chinese esophageal cancer patients in a prospective study.METHODS: 60 patients with primary esophageal carcinoma were randomized into an intervention group (IG, n=31) and a control group (CG, n=29). The Chinese version of symptom checklist-90 (SCL-90) was adopted to assess their psychological status. The interventions, including health education, psychological support, stress management, coping strategies and behavior training, were carried out in 3 phases (preoperative, postoperative I and postoperative II), and psychological effects were thereafter evaluated accordingly before surgery, and 1 week, 4 weeks and 24 weeks post-surgery. Medical costs were estimated at discharge. Survival of patients was estimated each year post-surgery. General health status and satisfaction-with-hospital were surveyed by a follow-up questionnaire 4 years post-surgery.RESULTS: All the subjects demonstrated higher scores in the preoperative phase than the normal range of Chinese population concerning 7 psychological domains including somatization, obsessive-compulsive, depression, anxiety, hostility, phobic anxiety and paranoid ideation. Although no significant difference was observed between the two groups at admission, the scores of IG, which tended to decrease at a faster rate, were generally lower than those of CG at weeks 1, 4 and 24 post-surgery. The length of hospital stay and medical costs of IG were significantly less than those of CG and satisfaction-with-hospital was better. However, there was no significant difference in 4-year survival or health status between two groups.CONCLUSIONS: Appropriate perioperative comprehensive supportive care interventions help to improve the psychological state of Chinese patients with esophageal carcinoma, to reduce health care costs and to promote satisfaction of patients and their families with hospital.

Abstract:Objective. The purpose of this study was to investigate patients with esophageal cancer symptom experiences before seeking medical help and psychosocial responses. Methods. Within one month of diagnosis, semistructured interviews were held with 14 adult patients with esophageal cancer. To fully comprehend the symptom experiences that patients with esophageal cancer had prior to seeking medical help and the psychosocial responses they had to these experiences, thematic analysis and interpretative phenomenological analysis were used. Results. Patients with esophageal cancer recounted their symptom experiences, and psychosocial responses were categorized into three main themes: physical, psychological, and social. Three subthemes were included in the physiological aspect: early symptoms (warning signs), specific symptoms (the catalyst for seeking medical help), and concurrent symptoms (masked side effects). Four subthemes were associated with psychological aspects: unfairness, regret, worry, and acceptance. Two subthemes were related to the social element, namely, dragging down one’s family and societal self-isolation. Conclusion. Due to the self-remitting nature of early symptoms, the normalization of their interpretation, and concealment by other illnesses, patients with esophageal cancer may delay medical help-seeking. The findings could serve as a reference for healthcare professionals to implement tailored psychosocial interventions and help patients identify symptoms early in order to seek medical help.

Abstract:e24111 Background: Many gastroesophageal cancer patients in China are not made aware of their cancer diagnosis/prognosis. Explorations of the differences in depression, anxiety and quality of life between cancer patients with and without such awareness are rare. The aim of this study was to evaluate the correlation between cancer awareness status and quality of life, anxiety and depression in gastroesophageal cancer patients. Methods: Participants were gastroesophageal cancer patients recruited from a medical center in North China. The degree of awareness of their cancer diagnosis/prognosis was evaluated via interviews. Data were collected using the Self-Rating Anxiety/Depression Scale and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 and were analyzed using quantitative methods. Results: The study included 157 gastroesophageal cancer patients: 31 were completely unaware of their cancer diagnosis/prognosis (19.75%); 51 were partly aware (32.48%); and 75 were fully aware (47.78%). Thirty patients (19.11%) experienced anxiety and 35 (22.9%) depression. Unawareness of the real disease status was significantly correlated with worse quality of life (cognitive functioning, social functioning and appetite loss) ( Ps < 0.05) and anxiety ( P = 0.003). Conclusions: More than 50% of gastroesophageal cancer patients were unaware their diagnosis/prognosis completely and were more likely to experience worse quality of life and anxiety. Medical staff and family members should take appropriate measures to make patients aware of their cancer diagnosis/prognosis, as awareness is valuable for improving quality of life and negative emotions, contributing to the successful overall management of gastroesophageal cancer.

The correlation between common postoperative complications and quality of life, serum tumor markers, and prognosis in patients with esophageal cancer

Abstract:Background: Esophageal cancer (EC) is highly malignant, with poor prognosis. The main forms of treatment are surgery, radiotherapy, and chemotherapy. In recent years, the incidence and mortality rate of patients with EC has improved. However, the factors that affect the quality of life of EC patients are unclear. This study investigated the postoperative complications and quality of life of EC patients, and identified the related factors. The relationship between complications and quality of life was explored so as to provide guidance for the clinical treatment and rehabilitation these patients.Methods: A total of 120 patients with EC who underwent surgery at the First People's Hospital of Lianyungang from January 2016 to August 2017 were retrospectively enrolled in this study. The patient's subjective quality of life evaluation was documented and clinical data were collated. The correlation between the incidence of postoperative complications and the patient's quality of life, serum tumor markers, and prognosis was analyzed.Results: A total of 36 (30%) EC patients experienced postoperative complications. There were 12 cases of pulmonary complications (10%), 8 cases of arrhythmia (6.67%), 3 cases of anastomotic fistula (2.5%), 5 cases of pneumothorax and pleural effusion (4.17%), and 4 cases of gastroesophageal reflux (3.33%). Incision infection occurred in 2 patients (1.67%) and there was 1 case (0.83%) of chylothorax. Empyema was reported in 1 patient (0.83%). There were significant differences in the overall condition, physical function, fatigue, pain, and swallowing pain between patients with postoperative complications and those without complications (P<0.05). Preoperative and postoperative expression of the tumor marker CYFRA21-1 was found to be independent risk factors for postoperative complications after EC surgery.Conclusions: EC is a common malignant tumor with a high incidence of postoperative complications. Patients with high CYFRA21-1 expression should be aware of the higher risk of postoperative complications. Patients with complications have poorer quality of life and obvious symptoms of fatigue, pain, and swallowing pain. Health education and dietary guidance should be provided to such patients to improve their symptoms.

Spiritual care needs and their associated influencing factors among elderly patients with moderate-to-severe chronic heart failure in China: A cross-sectional study

Abstract:Abstract Background The significance of spiritual care needs among chronic diseases patients has been emphasized across countries and cultures in many studies. However, there were few studies on spiritual care needs among elderly patients with moderate-to-severe chronic heart failure (CHF) in China. Objective To investigate spiritual care needs and associated influencing factors among elderly patients with moderate-to-severe CHF, and to examine the relationships among spiritual care needs, self-perceived burden, symptom management self-efficacy, and perceived social support. Methods A cross-sectional design was implemented, and the STROBE Checklist was used to report the study. A convenience sample of 474 elderly patients with moderate-to-severe CHF were selected from seven hospitals in Tianjin, China. The sociodemographic characteristics questionnaire, the Spiritual Needs Questionnaire Scale, the Self-Perceived Burden Scale, the Self-efficacy for Symptom Management Scale, and the Perceived Social Support Scale were used. Descriptive statistics, univariate, multiple linear regression, and Pearson's correlation analysis were used to analyze data. Results The total score of spiritual care needs among 474 elderly patients with moderate-to-severe CHF was 37.95 ± 14.71, which was moderate. Religious belief, educational background, self-perceived burden, symptom management self-efficacy, and perceived social support were the main factors affecting spiritual care needs, and spiritual care needs were negatively correlated with self-perceived burden ( r = −0.637, p < 0.01) and positively correlated with symptom management self-efficacy ( r = 0.802, p < 0.01) and social support ( r = 0.717, p < 0.01). Significance of results The spiritual care needs of elderly patients with moderate-to-severe CHF were moderate, which were influenced by five factors. It is suggested that clinical nurses, families, and society should take targeted spiritual care measures to improve patients’ symptom management self-efficacy and perceived social support from many aspects, and reduce self-perceived burden to meet their spiritual care needs and improve the quality and satisfaction of spiritual care in nursing practice.

Abstract:Background: Psychological, social and spiritual needs are often unmet during the care of patients with cancer in Taiwan. Aim: The purpose of this study was to confirm the spiritual care models including the spiritual distress symptoms (SDS), spiritual distress symptoms interventions (SDSI), and spiritual distress outcomes criteria (SDOC) of patients with cancer in the initial, relapse and terminal stages. Method: This cross-sectional survey collected data from 150 professional nurses about their perceptions of the SDS, SDSI and SDOC for patients with cancer. Results: The significant total effects of SDS on SDOC by SDSI of the patients with cancer in the initial, relapse and terminal stages were found. Additionally, the direct effects of SDS on SDOC by SDSI of the patients with cancer from the initial, relapse to terminal stage were gradually enhanced. Conclusions: The relationship between spiritual distress symptoms, interventions and outcomes was significantly higher from the initial to relapse state until at the end of life, based on the perceptions of 150 professional nurses.
