Abstract:Importance: Disproportionate levels of violence exist in lesbian, gay, bisexual, queer, asexual, and other sexual identity (LGBQA+) communities, often starting in adolescence and young adulthood, but little research exists on the range of sexual violence (SV) experiences. Objective: To assess risk of experiences of sexual violence among California LGBQA+ adults during adolescence and young adulthood. Design, setting, and participants: This survey study analyzed data from Cal-VEX 2020, a state-representative survey of California residents aged 18 years and older, to assess associations between LGBQA+ identification and types of SV experienced in adolescence (ages 13-17 years) and young adulthood (ages 18-24 years) using gender-stratified adjusted logistic regression. Forms of SV included verbal, cyber, and coercion or physically aggressive sexual harassment; homophobic or transphobic slurs; and forced sex. Exposure: Sexual identity. Main outcomes and measures: The outcome of interest was experiences with any 5 forms of sexual violence (verbal sexual harassment, homophobic or transphobic slurs, cyber sexual harassment, sexual coercion or physically aggressive sexual harassment, and forced sex) during adolescence or young adulthood. Results: A total of 2102 contacted individuals (response rate, 26.2%) completed the Cal-VEX survey and were included in these analyses. Among these, the mean (SD) age was 46.6 (17.7) years, 1149 identified as female, 953 identified as male, and 214 respondents (9.6%) identified as LGBQA+. Compared with heterosexual individuals, LGBQA+ individuals had significantly higher odds of having experienced homophobic or transphobic slurs (women: AOR, 14.65; 95% CI, 5.14-41.77; men: AOR, 14.17; 95% CI, 6.96-28.86) and forced sex (women: AOR, 5.35; 95% CI, 2.74-10.43; men: AOR, 2.68; 95% CI, 1.01-7.10) in adolescence. LGBQA+ women also had higher odds of having experienced verbal and coercion or physically aggressive sexual harassment in adolescence (AOR, 2.33; 95% CI, 1.30-4.19). In young adulthood, LGBQA+ individuals also had higher odds of having experienced homophobic or transphobic slurs (women: AOR, 18.58; 95% CI, 7.12-48.49; men: AOR, 16.73; 95% CI, 8.26-33.92) in young adulthood. LGBQA+ men also had higher odds of having experienced homophobic or transphobic slurs (AOR, 16.73; 95% CI, 8.26-33.92); verbal (AOR, 3.29; 95% CI, 1.44-7.53), cyber (AOR, 6.32; 95% CI, 1.50-26.52), and coercion or physically aggressive (AOR, 5.54; 95% CI, 2.08-14) sexual harassment; and forced sex (AOR, 21.26; 95% CI, 5.63-80.35) in young adulthood. Conclusions and relevance: This survey study found that adult LGBQA+ individuals were at increased risk for having experienced SV across the continuum during adolescence and young adulthood, consistent with prior research. Multifold solutions are needed to support LGBQA+ youth, including altering social norms accepting SV and homophobia, creating safer schools and other institutional environments, and supporting healthy sexual and romantic partnerships.
Healthcare Experiences and Health Outcomes Among Rural LGBTQ+ Individuals

PurposeTo describe healthcare experiences and health outcomes among rural LGBTQ + individuals.Design2022 cross-sectional survey.SettingSouthern Illinois.Sample85 individuals.MeasuresDemographics, sexual orientation and gender identity, healthcare experiences, health outcomes.AnalysisExperiences and outcomes were assessed vs orientation and identity. Distribution comparison was by t-test and chi-square, risk prediction by logistic regression, and significance assumed at P < .050.ResultsBy orientation, participants were: 35.3% gay, 16.5% lesbian, and 45.8% bisexual plus; and by identity they were: 49.4% cisgender, 25.9% transgender, and 24.8% other identity. Survey item responses ranged from 95%-99%. Compared to gay men, lesbians and bisexual plus individuals more frequently reported medical bill payment difficulty (58.3% and 57.9% vs 25.0%; P = .020) and more past month days of poor mental health (19.4 and 15.8 vs 10.6; P = .018). Compared to heterosexual and other identity, transgender individuals less frequently reported having a routine medical provider (72.7% vs 92.7% and 95.0%; P = .037) and more frequently reported past healthcare denial (45.5% vs 17.5% and 18.8%; P = .042). Current health was associated with medical bill payment ability (OR = .33, 95% CI = .13-.86) and respectful treatment by healthcare administrators (OR = 3.90, 95% CI = 1.34-11.35) and clinicians (OR = 3.82, 95% CI = 1.39-10.47). Significance of some findings likely limited due to sample size.ConclusionsOur data describes healthcare experience and health outcome disparities among rural lesbian, gay, bisexual, transgender, queer and other sexual and gender minority individuals, and indicate that clinical experiences directly influence health outcomes.

Abstract:Journal of Interpersonal Violence, Ahead of Print. LGBTQ+ (e.g., lesbian, gay, bisexual, transgender, nonbinary, queer) people are often left out of campus sexual and relationship violence (SRV) prevention efforts despite experiencing higher rates of SRV. To inform LGBTQ+-affirming prevention efforts, we use a practice-to-research approach to aggregate wisdom from 32 LGBTQ+ professionals working to address campus SRV among LGBTQ+ college students garnered through semi-structured interviews. Participants shared four approaches to including or excluding LGBTQ+ students in campus SRV prevention programs as well as recommendations to cultivate more LGBTQ+-affirming campus SRV prevention efforts. We summarize recommendations for possible action steps at individual, relationship, community, and policy levels of the social ecological model for LGBTQ+-affirming campus SRV prevention.

Abstract:Background: Traveling to meet sexual partners and substance use are associated with increased risk of infectious disease. It is important to understand what factors may increase substances use or increased infection transmission risk (IITR) sexual behaviors among rural LGBTQ+ individuals. Objective: This study investigates substance use and sexual behaviors associated with increased infection transmission risk among rural LGBTQ+ individuals, and how these are associated with relationship type (friends or romantic partners) and travel distance. Methods: Participants (18 years+, identify as LGBTQ+, and provided a $25 gift card) were recruited from Illinois (25 counties), in 2021. Data included demographics, sexual and drug use behaviors, and assessed how these behaviors varied by relationship type and distance traveled (e.g. when meeting friends out-of-state). Results: The 398 participants were 79.1% White and 12.3% Black. By orientation, 29% heterosexual, 36% gay/lesbian, and 35% bisexual/other. By identity, 43% cisgender male, 51% cisgender female, and 6% genderqueer/other. Alcohol use while visiting out-of-state friends was more frequent among transgender (vs cisgender men; OR = 9.686, 95% confidence interval = 2.123-44.19), and individuals traveling > 1/month (all p < .050). Infection-related sexual behaviors while visiting out-of-state romantic partners was more frequent among prescription medication misuse (all p 1/week (vs < 1/month; OR = 3.399, 95% CI = 1.037-11.144). Conclusion: This study of rural LGBTQ+ identified that alcohol use was associated with travel to visit out-of-state romantic partners, and prescription medication misuse increased infection-related sexual behavior during out-of-state travel. Health professionals can develop substance use and increased infection transmission risk sexual behavior interventions on gender minority groups in rural areas and target those who are traveling.

Voices from the Margin: a Case Study of a Rural Lesbian's Experience with Woman-to-woman Sexual Violence.

Abstract:The purpose of this case study is to understand how a lesbian survivor coped with traumatic stress resulting from woman-to-woman sexual violence in a rural context. This research was grounded on the feminist paradigm, and the case study approach was used for data collection and analysis. The results indicate that the lesbian survivor encountered numerous obstacles to admitting the rape and seeking help. A number of these perceived challenges were associated with the rural sociocultural context (e.g., conservative culture, religious fundamentalism, low levels of anonymity, small gay/lesbian communities, and geographical isolation), myths of lesbian utopia, a heterosexist legal system, and a lack of lesbian-friendly helping resources. Despite being confronted with numerous challenges, the survivor demonstrated her resilience through the use of a variety of strategies to cope with the rape aftermath and detrimental social reactions. Positive social support from her gay and lesbian friends also facilitated her recovery. The findings highlight the importance of considering the intersection of trauma and cultural oppression in understanding lesbian survivors' experiences with same-sex sexual assault.

Abstract:Sexual minority (SM) men's sexual revictimization (SR; i.e. experiences of adult victimization among childhood sexual abuse survivors) is an understudied topic despite evidence that SM men are disproportionately impacted by sexual violence (SV) over the life course. This study addresses this gap utilizing a diverse sample (n = 2859) of SM men (age 18-30) from the U.S. Results demonstrated that over 10% of SM men had experienced SR. Further, strength-based (e.g. sense of LGBTQIA2S+ community) and minority-stress (e.g. internalized homonegativity) related factors were examined as moderators of the relationship between child sexual abuse (CSA) and adult sexual assault victimization (ASAV) in the past six months. One factor emerged as a significant moderator of the CSA-ASAV relationship: perceived discrimination. Practice-based implications are provided.

Abstract:Journal of Interpersonal Violence, Ahead of Print. Intimate partner sexual violence (IPSV) is a substantial public health issue faced by youth, with disparities along lines of race, gender, and sexual orientation. Using an intersectional framework, the current study describes the prevalence of self-reported IPSV victimization across intersecting social positions among adolescents. High school students who completed the 2019 Minnesota Student Survey were asked whether they had ever experienced IPSV. Using exhaustive Chi-Square Automatic Interaction Detection (eCHAID), responses were modeled using five indicators: racial/ethnic identity, transgender/genderqueer/genderfluid identity, sex assigned at birth, sexual orientation, and school location. The prevalence of IPSV victimization within each end group identified by the eCHAID varied from 2.6% to 32.0%. The highest prevalence of IPSV among all students was observed in subgroups defined by multiple marginalized social positions; for example, 32.0% among lesbian, gay, bisexual, queer, pansexual, or questioning (LGBQ+) students of color who were transgender, genderqueer, genderfluid or unsure of their gender, and assigned male at birth; 22.8% among LGBQ+ students assigned female at birth and enrolled in a Greater Minnesota (outside of the Twin Cities) school; and 22.2% among Native+ or missing race LGBQ+ students assigned female at birth and enrolled in a metropolitan school. Within the eCHAID decision tree, several subsamples of students were split into two more subsamples with a twofold or greater statistically significant difference in IPSV victimization prevalence between them. Findings from this study highlight notable disparities in the prevalence of IPSV victimization among Minnesota youth and demonstrate the importance of detailing the distribution of interpersonal violence outcomes across youth with multiple marginalized identities. Findings suggest that disparities reported along one categorical axis only may miss important nuances in how risk for IPSV is distributed. Population health researchers should utilize methods that allow for the explication of complex intersections that characterize individuals' social positions in estimating the prevalence of sexual violence.

Abstract:Sexual violence (SV) on college campuses disproportionately affects cisgender (nontransgender) women, sexual minorities (e.g., gays/lesbians, bisexuals), and gender minority (e.g., transgender/nonbinary) people. This study investigates gender and sexual behavior differences in common SV intervention targets—SV-related knowledge, prevention behaviors, and care-seeking. We analyzed cross-sectional survey data, collected in 9/2015–3/2017, from 2202 students aged 18–24 years attending college health and counseling centers at 28 Pennsylvania and West Virginia campuses. Multivariable multilevel models tested gender and sexual behavior differences in SV history; recognition of SV; prevention behaviors (self-efficacy to obtain sexual consent, intentions to intervene, positive bystander behaviors); and care-seeking behaviors (knowledge of, self-efficacy to use, and actual use of SV services). Adjusting for lifetime exposure to SV, compared with cisgender men, cisgender women had higher recognition of SV and reproductive coercion, prevention behaviors, and care-seeking self-efficacy (beta range 0.19–1.36) and gender minority people had higher recognition of SV and intentions to intervene (beta range 0.33–0.61). Cisgender men with any same-gender sexual partners had higher SV knowledge (beta = 0.23) and self-efficacy to use SV services (beta = 0.52) than cisgender men with only opposite-gender partners. SV history did not explain these differences. Populations most vulnerable to SV generally have higher SV knowledge, prevention behaviors, and care-seeking behaviors than cisgender men with only opposite-gender sexual partners. Innovative SV intervention approaches are necessary to increase SV-related knowledge among heterosexual cisgender men and may need to target alternative mechanisms to effectively reduce inequities for sexual and gender minority people.

A Longitudinal Study of IPV Victimization Among Sexual Minority Youth

Abstract:Although intimate partner violence (IPV) is highly prevalent among lesbian, gay, bisexual, and transgender (LGBT) youth, little is known regarding its developmental patterns, risk factors, or health-related consequences. We examined IPV victimization in an ethnically diverse community-based convenience sample of 248 LGBT youth (aged 16-20 at study outset) who provided six waves of data across a 5-year period. Results from multilevel models indicated high, stable rates of IPV victimization across this developmental period (ages 16-25 years) that differed between demographic groups. Overall, 45.2% of LGBT youth were physically abused and 16.9% were sexually victimized by a dating partner during the study. Odds of physical victimization were 76% higher for female than for male LGBT youth, 2.46 times higher for transgender than for cisgender youth, and 2 to 4 times higher for racial-ethnic minorities than for White youth. The prevalence of physical IPV declined with age for White youth but remained stable for racial-ethnic minorities. Odds of sexual victimization were 3.42 times higher for transgender than for cisgender youth, 75% higher for bisexual or questioning than for gay or lesbian youth, and increased more with age for male than female participants. Within-person analyses indicated that odds of physical IPV were higher at times when youth reported more sexual partners, more marijuana use, and lower social support; odds of sexual IPV were higher at times when youth reported more sexual partners and more LGBT-related victimization. In prospective analyses, sexual IPV predicted increased psychological distress; both IPV types marginally predicted increased marijuana use.

Abstract:While national trends indicate improvement in campus climates for LGBTQ+ students, these patterns may not exist on campuses in the Southeast region of the United States, particularly given conservative backlash in the region. Drawing from focus group and survey data, we investigated how students in a rural Southern location perceive campus and community space with regard to safety and support, and how the intersections of gender and sexual orientation affect those perceptions. We found LGBTQ+ students had a mostly positive perception of their campus experience, despite reports of harassment, perceptions of unsafe spaces, and some lack of awareness of resources. However, students rate the climate for queer-spectrum students more positively than that for trans-spectrum students. Cis-gender, heterosexual students were largely accepting of their LGBTQ+ peers and were mostly unaware of LGBTQ+ harassment in spaces on campus. Students' positive assessments exist despite a lack of systematic, institutional-level programming to integrate them into the campus community but are influenced by the work of faculty and staff allies through classes, curriculum, and programs. Our analyses inform recommendations for policies and initiatives and illuminate unique challenges facing queer students on campuses in conservative, rural areas.

Abstract:This study explored lived experiences of LGBTQIA+ survivors of intimate partner violence (IPV). Seven participants completed a one-on-one, in-depth interview to share their experiences of IPV and any internal or external factors that influenced whether they sought support services or reported victimization. Thematic analysis revealed four overarching themes: (a) health complications, (b) coping mechanisms, (c) barriers to seeking professional help, and (d) suggestions for professionals. Results contribute insight into internal and external barriers LGBTQIA+ IPV survivors face when accessing support services and outline practical approaches for professionals providing support to LGBTQIA+ IPV survivors.

Evaluation of traditional risk factors for intimate partner violence among sexual and gender minority youth.

Abstract:Objective: Sexual and gender minority youth assigned female at birth (SGM-AFAB) experience higher rates of intimate partner violence (IPV) than heterosexual and cisgender youth. To inform efforts to reduce these disparities, we explored whether IPV risk factors identified in the general population are associated with IPV among SGM-AFAB young people. Method: Using multiwave longitudinal data from 400 SGM-AFAB youth (Ages 16–20 at baseline), we estimated between- and within-persons effects of demographic/contextual characteristics (gender, sexual identity, race/ethnicity, socioeconomic status), developmental/background factors (childhood violence), and psychological/behavioral factors (antisocial behavior, depression, problematic alcohol, and cannabis use) on a range of IPV experiences (victimization and perpetration of psychological, physical, sexual, and SGM-specific IPV). Results: In this SGM-AFAB sample, IPV experiences were associated with many traditional risk factors identified in the general population, including race, economic stress, childhood violence, antisocial behavior, depression, and use of substances (particularly cannabis). In contrast to previous research, we did not find that SGM youth with transgender or gender nonbinary identities, or with bi- or pan-sexual identities, were at greater risk for IPV than other SGM youth. Very few putative risk factors were associated with SGM-specific IPV. Conclusions: Findings suggest SGM youth could benefit from IPV prevention approaches that target common risk factors at multiple ecological levels (policies to reduce poverty and racism, parenting programs, interventions to reduce mental health and substance use problems). Continued research is needed to explore how the risk for IPV among SGM-AFAB youth may vary by gender identity, sexual identity, and stigma-based experiences.

Abstract:This study documented between-group differences in factors associated with lifetime sexual victimization in a sample of young sexual minority men. Diverse samples of gay ( N = 205, age M = 24.33 years) and bisexual ( N = 201, age M = 23.31 years) men were recruited using the CloudResearch platform to assess recent experiences of intimate partner violence (IPV). Participants were categorized into four groups, cross-classified by dichotomous self-reports of (a) childhood sexual abuse (CSA) and (b) sexual IPV. Principal components analysis was used to reduce the number of variables associated with sexual IPV in three domains: Past-year substance use involvement, minority stress, and relationship characteristics, separately for each sample. Gay men reporting both CSA and sexual IPV reported significantly higher mean factor scores for a principal component with high loadings for recent substance use, daily discrimination, relational aggression and relational victimization, compared to other groups of gay men. Bisexual men who experienced sexual IPV reported significantly higher mean factor scores for a principal component with high loadings for five measures of minority stress, compared to counterparts with no history of sexual victimization. Adult sexual IPV among gay men reporting CSA appears to occur in conditions that include harmful substance use, daily discrimination experiences, and relationship violence. Sexual IPV among bisexual men is associated with multiple minority stressors. Our findings highlighted different patterns of risk factors for sexual IPV among sexual minority men, providing information for tailored risk reduction initiatives, including the need for trauma-informed services and specialized training for service providers.

Sexual Violence among LGBQ Community College Students: A Comparison with their Heterosexual Peers

Abstract:Although community colleges enroll nearly half of the undergraduate population in the U.S., scarce research exists on the sexual violence perpetrated against their student body, especially those students who identify as lesbian, gay, bisexual, and queer (LGBQ). Findings from an ARC3 (Administrator-Researcher Campus Climate Collaborative) Climate Survey, which was administered at seven community colleges in a Northeastern state, indicate high rates of sexual violence reported by community college students, particularly among students who identify as LGBQ, compared to their heterosexual counterparts. Almost two-thirds (63.6%) of non-heterosexual participants reported some form of sexual violence since they enrolled at their community college, with more than 35% of this demographic reporting two or more forms of sexual violence during that same time period. Additionally, LGBQ students who reported sexual violence had significantly lower well-being scores and perceptions of campus climate (e.g., perceptions of safety).
Violence Victimization and Depressive Symptoms Among a Sub-Sample of Sexual and Gender Minority Adults in a Population-Based South African Study

Abstract:Journal of Interpersonal Violence, Ahead of Print. Sexual and gender minority (SGM) adults in South Africa face high levels of violence and poor mental health outcomes. Interventions to prevent these negative health implications are hampered by a lack of representative data among this population. This study aims to quantify the associations between three forms of violent victimization and depressive symptoms in a sub-sample of SGM drawn from a population-based cross-sectional study in Gauteng, South Africa. Data come from the sixth Quality of Life survey conducted in South Africa's Gauteng province. Brief screeners assessed childhood sexual abuse (CSA), past-year intimate partner violence (IPV), non-partner violence, and depressive symptoms. Three survey-weighted logistic regression analyses were fit to model associations between elevated depressive symptoms and CSA, past-year IPV, and past-year non-partner violence, controlling for socio-demographics (age, race, sex, area of residence, education, socioeconomic status, and recent employment). N = 1,328 SGM respondents were included. Over 40% (n = 537) reported depressive symptoms, while 17% (n = 222) reported CSA, 5% (n = 67) reported IPV, and 16% (n = 208) reported non-partner violence. CSA and non-partner violence were associated with significantly higher odds of reporting depressive symptoms (aOR: 1.51, 95% CI [1.03, 2.23]; aOR: 1.84, [1.24, 2.73], respectively). IPV was not associated with elevated depressive symptoms (aOR: 1.17, [0.64, 2.16]). In all models, employment in the past 7 days was associated with significantly lower odds of reporting depressive symptoms. Recent and childhood violence is a major burden that is associated with elevated symptoms of depression among SGM in urban South Africa. Community-tailored interventions and policy-related advocacy related to employment and violence prevention may alleviate depressive symptoms in SGM adults in Gauteng.

Abstract:Journal of Interpersonal Violence, Ahead of Print. Youth dating violence and youth suicide are both major public health concerns. However, little research examines the intersection of these issues among lesbian, gay, bisexual, transgender, queer, and questioning (LGBTQ) youth. The current study examined the association between physical dating violence victimization and mental health using online survey data collected from 39,126 LGBTQ youth of ages 13 to 24 in the United States. Overall, 11% of LGBTQ youth who were in a relationship in the past year experienced physical dating violence victimization, with 4% having experienced it once and 7% having experienced it multiple times. Rates of experiencing physical dating violence victimization varied based on a number of youth characteristics and environmental factors. Past-year physical dating violence victimization was associated with nearly four times the odds of attempting suicide in the past year. Given the intersection of suicide risk and dating violence among LGBTQ youth, dating violence prevention organizations must be LGBTQ-inclusive, and suicide prevention must be equipped to address dating violence victimization. The higher rates of physical dating violence victimization among multiple marginalized and socioeconomically disadvantaged LGBTQ youth suggest the need for an intersectional approach that addresses the structural factors that place LGBTQ youth at risk for physical dating violence.

Interpersonal Violence Among Heterosexual and Sexual Minority Youth: Descriptive Findings From the 2017 Youth Risk Behavior Surveillance System

Abstract:Interpersonal violence is pervasive among youth in the United States. Sexual minority youth are disproportionately impacted. Utilizing data from the 2017 Youth Risk Behavior Surveillance System (YRBS, N = 12,868), this study describes the disparities in victimization among heterosexual and sexual minority youth. Sexual minority status was operationalized to include nonheterosexual identity (e.g., bisexual) and/or behavior (e.g., reporting same- and different-sex partners). Other predictors included sex (male/female), a sexuality*sex interaction term, age, and race/ethnicity. Outcomes included lifetime forced sexual intercourse and 12-month incidence of sexual violence, physical violence in relationships, sexual violence in relationships, in-school bullying, and cyberbullying. Approximately 15% ( n = 1,974) of participants were classified as sexual minority youth and 48% ( n = 6,213) self-identified as female. More than half (56%, n = 7,190) identified as racial and/or ethnic minorities. In bivariate logistic regression, sexual minority youth were significantly more likely to report all outcomes ( p < .001). In multivariable logistic regression, sexual minority and female youth were significantly more likely to report all outcomes ( p < .001). With the exception of physical violence in dating, sexuality*sex interaction terms were significant predictors for all outcomes ( p < .001), indicating that the risks associated with sexual minority status are heightened for male youth. This study concludes with a call for more attention toward violence among and against sexual minority youth. Although some prevention and response efforts address the unique needs and experiences of these youth, particularly antibullying initiatives, efforts to address sexual and dating violence often emphasize heterosexual contexts and communities.

Abstract:Sexual stigma operates at multiple levels (institutional, group, individual), which serves to disadvantage sexual minority (LGBQ+) individuals and increases risk for deleterious outcomes. The current study evaluated a novel multilevel sexual stigma model of intimate partner violence (MLSSM-IPV) that incorporates multiple levels of sexual stigma as related to IPV risk via several pathways (e.g. hazardous drinking, affective symptoms). We evaluated this model in a longitudinal study of LGBQ+ undergraduate college students ( n = 2,415) attending 18 universities who completed surveys in the Fall and Spring semesters. Group-level sexual stigma on each campus was assessed via surveys with heterosexual students ( n = 8,517) and faculty, staff, and administrators ( n = 2,865), and institutional-level stigma was evaluated via a campus climate assessment. At the campus level, institutional stigma was related to LGBQ+ students' self-stigma and identity concealment. Moreover, self-stigma prospectively predicted IPV victimization, and hazardous drinking mediated the relations between self-stigma and IPV perpetration and victimization. Results suggest that interventions addressing stigma and hazardous drinking may be efficacious in reducing IPV among LGBQ+ students. Further, comprehensive efforts to improve campus climate for LGBQ+ students are likely to produce a plethora of benefits for these students.
