Abstract:Background. CD4(+)CD25(+) regulatory T cells (Tregs) play an essential role in sepsis-induced immunosuppression. How, the effects of interleukin 36 (IL-36) cytokines on CD4(+)CD25(+) Tregs and their underlying mechanism(s) in sepsis remain unknown. Methods. Our study was designed to investigate the impacts of IL-36 cytokines on murine CD4(+)CD25(+) Tregs in presence of lipopolysaccharide (LPS) and in a mouse model of sepsis induced by cecal ligation and puncture (CLP). IL-36-activated autophagy was evaluated by autophagy markers, autophagosome formation, and autophagic flux. Results. IL-36 alpha, IL-36 beta, and IL-36 gamma were expressed in murine CD4(+)CD25(+) Tregs. Stimulation of CD4(+)CD25(+) Tregs with LPS markedly up-regulated the expression of these cytokines, particularly IL-36 beta. IL-36 beta strongly suppressed CD4(+)CD25(+) Tregs under LPS stimulation and in septic mice challenged with CLP, resulting in the amplification of T-helper 1 response and the proliferation of effector T cells. Mechanistic studies revealed that IL-36 beta triggered autophagy of CD4(+)CD25(+) Tregs. These effects were significantly attenuated in the presence of the autophagy inhibitor 3-methyladenine or Beclinl knockdown. In addition, early IL-36 beta administration reduced the mortality rate in mice subjected to CLP. Depletion of CD4(+)CD25(+) Tregs before the onset of sepsis obviously abrogated IL-36 beta-mediated protection against sepsis. Conclusions. These findings suggest that IL-36 beta diminishes the immunosuppressive activity of CD4(+)CD25(+ )Tregs by activating the autophagic process, thereby contributing to improvement of the host immune response and prognosis in sepsis.

However, the role of PD-1 in naive and memory T-cell responses in sepsis is not well understood. We aimed to determine the expression of PD-1 induced on naive and memory T lymphocytes in patients with sepsis and its association with clinical outcome. Methods: A prospective observational study was conducted at a general intensive care unit (ICU). Whole blood samples were collected from patients within 48 h after sepsis diagnosis. PD-1 expression on naive and memory T cells was measured by flow cytometry. The levels of IFN-γ, IL-2 and TNF-α released by memory T cells were also determined. All patients were followed up to 28 days, and 28-day mortality was recorded. Results: PD-1 expression showed no difference in naive CD4 + T cells ( P =0.617) or naive CD8 + T cells ( P =0.079) between survivors (n = 21) and nonsurvivors (n = 9). Increased PD-1 expression on memory CD4 + T cells was found in nonsurvivors ( P =0.030) and memory CD8 + T cells ( P =0.006) in comparison with survivors. According to the cutoff value of the percentage of PD-1 on memory CD8 + T cells in predicting 28-day mortality of patients with sepsis, patients were divided into two groups. The 28-day mortality rates between the two groups were significantly different ( P =0.009). A Kaplan Meier curve was constructed to derive a hazard ratio of 9.33 (95% CI: 2.52– 34.60) for the percentage of PD-1 on memory CD8 + T cells regarding 28-day mortality. In addition, the IFN-γ secretion of memory CD4 + T cells ( P =0.046) and IL-2 secretion of memory CD8 + T cells ( P =0.014) were significantly greater in survivors than nonsurvivors. Conclusion: Flow cytometric assessment of PD-1 expression on memory CD8 + T cells identifies patients with poor outcomes during sepsis. Keywords: PD-1, memory T cells, sepsis, outcome Sepsis, caused by a dysregulated host response to infection, is a life-threatening condition with organ dysfunction. 1 Approximately 270,000 to 370,000 people die from sepsis every year in the US, making sepsis the leading cause of death for patients with critical illness. 2 In China, one-fifth of patients are admitted to the intensive care unit (ICU) with sepsis and the 90-day mortality rate is as high as 35.5%. 3 The treatment of sepsis remains a major challenge, and there is currently no FDA-approved therapy for sepsis. 4 Even if survival from the cytokine storm improves, previously septic individuals still face a higher risk of mortality, which defines the sepsis-induced immunosuppressive state. 5,6 Sepsis-induced immunosuppression is characterized by an increased sensitivity to new infections unrelated to previous encounters. 6 These severe lesions were sufficient to reduce 5-year survival in the septic cohort compared to the nonseptic cohort. 7 T cells play an important role in the host immune response in sepsis. Sepsis-induced immunologic impairment is typified by a transient reduction in lymphocytes and a decrease in the potency of effector lymphocytes. 8,9 Specifically, lymphopenia caused by sepsis affects both naive T cells and memory T cells. Memory T cells are more susceptible than naive T cells to sepsis-induced cell loss. 10,11 Additionally, memory T cells that survived the cytokine storm were less able to respond to inflammatory signals, 12 suggesting that memory T cells may be a crucial factor in the pathogenic mechanism of sepsis. [Effect of dendritic cells on immune function regulated by programmed cell death-1/programmed cell death-ligand 1 in sepsis]

Abstract:Objective: To study the effect of programmed cell death-1/programmed cell death-ligand 1 (PD-1/PD-L1) regulating dendritic cells (DC) on the immune status of sepsis, and analyze the effect of PD-1/PD-L1 on prognosis. Methods: Twenty-five patients with sepsis in the intensive care unit (ICU) of the Affiliated Hospital of Zunyi Medical University from October 2018 to September 2019 were collected and followed up for 28 days. According to the 28-day survival of patients, patients were divided into survival group and death group. Among them, 10 cases were in the survival group and 15 cases were in the death group. Simultaneously, 20 healthy subjects in our hospital during the same period served as the healthy control group. Peripheral blood of patients with sepsis was taken within 24 hours after diagnosis, and the healthy control group was taken at the time of enrollment. Flow cytometry was used to detect the proportion of CD4+T and CD8+T cells, the ratio of T cell subsets (CD4/CD8), the expression of PD-1 on CD4+T and CD8+T cells, and the expression of PD-L1 and CD86 in DC. Enzyme linked immunosorbent assay (ELISA) was used to detect the levels of interleukin-10 (IL-10) and tumor necrosis factor-α (TNF-α) in serum. Spearman correlation analysis was used to analyze the correlation between CD11c+PD-L1 and CD4+PD-1, CD8+PD-1, TNF-α, DC, CD11c+CD86, T cell subpopulation ratio, CD4+T cells, CD8+T cells, and IL-10. Binary Logistic regression was used to analyze the risk factors affecting the death of patients with sepsis, and receiver operator characteristic curve (ROC curve) was drawn to evaluate the predictive value of independent risk factors on the prognosis of patients. Results: The scores of acute physiology and chronic health evaluation II (APACHE II) and sequential organ failure assessment (SOFA) in the death group were higher than that in the survival group (APACHE II score: 27.0±7.3 vs. 17.0±3.9, SOFA score: 15.1±4.1 vs. 10.7±2.7, both P < 0.05). The ratio of T cell subsets in the survival group and the death group was less than 1, the death group was lower than that in the survival group (CD4/CD8: 0.54±0.15 vs. 0.79±0.09, P < 0.05), and the ratio of T cell subsets in the healthy control group was greater than 1. Compared with healthy control group, the levels of CD4+T cells, CD8+T cells, CD11c+DC, CD11c+CD86, IL-10 and TNF-α in survival group and death group were significantly decreased, the level of CD4+PD-1,CD8+PD-1, CD11c+PD-L1 were significantly increased,and the changes in the above indicators in the death group were significant compared with the survival group [CD4+T cells: 0.14±0.07 vs. 0.22±0.08, CD8+T cells: 0.24±0.07 vs. 0.28±0.10, CD11c+DC: 0.84±0.14 vs. 0.93±0.03, CD11c+CD86: (58.83±20.77)% vs. (78.24±9.39)%, IL-10 (ng/L): 34.22±13.98 vs. 18.49±5.55, TNF-α (ng/L): 95.30±29.33 vs. 67.00±20.16, CD4+PD-1: (39.58±10.08)% vs. (27.03±6.35)%, CD8+PD-1: (38.77±11.91)% vs. (29.15±8.37)%, CD11c+PD-L1: (21.13±11.54)% vs. (12.11±8.34)%, all P < 0.05]. Spearman correlation analysis showed that CD11c+PD-L1 was positively correlated with CD4+PD-1, CD8+PD-1, and IL-10 (r values were 0.748, 0.713, 0.898, all P < 0.05), while was negatively correlated with DC, CD11c+CD86, T cell subpopulation ratio, CD4+T cells, CD8+T cells, and TNF-α (r values were -0.587, -0.906, -0.840, -0.706, -0.513, -0.820, all P < 0.05). Multivariate binary Logistic regression analysis showed that CD4+T PD-1 was an independent risk factor for the prognosis of sepsis patients [odds ratio (OR) = 1.463, 95% confidence interval (95%CI) = 1.032-2.074, P = 0.033]. ROC curve analysis showed that CD4+T PD-1 had certain predictive value for the prognosis of patients with sepsis [area under ROC curve (AUC) = 0.857, 95%CI was 0.709-1.000, P < 0.01). When the best predictive value was 34.48%, the sensitivity, specificity, and accuracy were 66.7%, 90.0%, and 85.7% respectively. Conclusions: Up-regulation of PD-1/PD-L1 in peripheral blood could inhibit the activation and proliferation of DC, affect the activation of T cells, and induce immunosuppressive state. PD-1/PD-L1 can reflect the immune status of patients with sepsis. The expression of PD-1 on CD4+T cells may have important significance for the evaluation of prognosis.

Abstract:Sepsis is a series of life-threatening organ dysfunction caused by an impaired host response to infection. A large number of molecular studies of sepsis have revealed complex interactions between infectious agents and hosts that result in heterogeneous manifestations of sepsis. Sepsis can cause immunosuppression and increase the expression of checkpoint inhibitor molecules, including programmed death protein (PD-1) and programmed death ligand 1 (PD-L1), and thus PD-1 and PD-L1 are thought to be useful as diagnostic and prognostic tools for sepsis. PD-1 is an inhibitor of both adaptive and innate immune responses, and is expressed on activated T lymphocytes, natural killer (NK) cells, B lymphocytes, macrophages, dendritic cells (DCs), and monocytes, whereas PD-L1 is expressed on macrophages, some activated T and B cells, and mesenchymal stem cells as well as various non-hematopoietic cells. This systematic review aims to assess the PD-1 and PD-L1 protein expression levels and concentrations in septic and other infectious patients.

Abstract:ObjectiveProgrammed death 1 (PD-1) and macrophages are the most intriguing candidates in sepsis-induced inflammatory disorders. We aimed to investigate the association between monocyte PD-1 and sepsis severity and the mechanism by which blocking macrophage-associated PD-1 causes inflammatory disorders in sepsis.Materials and methodsWe first measured whether the expression of PD-1 on the monocyte subset is clinically associated with sepsis severity in an observational study. This study included 42 septic patients and 16 healthy controls (HCs) whose serum inflammatory factors were examined by Luminex MagPix. Then, we investigated the effect of PD-1 blockade on macrophages from septic mice (C57BL/6 mice) constructed by caecal ligation and puncture (CLP) via RNA sequencing. The positive genes screened by RNA-seq were verified in LPS-stimulated RAW264.7 cells by Western blot.ResultsThe results showed that the expression of PD-1 on CD14+CD16+ monocytes (intermediate monocytes, IM Mo) was significantly higher in both septic and septic shock patients than in HCs. Further analysis of serum cytokines in septic patients showed that the levels of IL-6 and TNF-α were significantly higher than those in HCs, while serum PD-1 levels were decreased in septic patients. More interestingly, blockade of PD-1 on macrophages from septic mice suppressed the gene expression levels of NLRP3/Caspase-4/AKT2/STAT3. The protein levels associated with pyroptosis including NLRP3, Caspase4, GSDMD and NT-GSDMD were significantly decreased in LPS-stimulated RAW264.7 cells treated with PD-1 antibody.ConclusionOur results suggested that intermediate monocytes with high expression of PD-1 may be involved in the progression of sepsis. PD-1 might play a critical role in regulating the pyroptosis signalling pathway in sepsis.

Abstract:Long term immunosuppression is problematic during sepsis. The PD-1 and PD-L1 immune checkpoint proteins have potent immunosuppressive functions. Recent studies have revealed several features of PD-1 and PD-L1 and their roles in sepsis. Here, we summarize the overall findings of PD-1 and PD-L1 by first reviewing the biological features of PD-1 and PD-L1 and then discussing the mechanisms that control the expression of PD-1 and PD-L1. We then review the functions of PD-1 and PD-L1 in physiological settings and further discuss PD-1 and PD-L1 in sepsis, including their involvement in several sepsis-related processes and their potential therapeutic relevance in sepsis. In general, PD-1 and PD-L1 have critical roles in sepsis, indicating that their regulation may be a potential therapeutic target for sepsis.

Abstract:Background: Pyroptosis, a type of proinflammatory programmed cell death, drives cytokine storm. Caspase-11-dependent macrophage pyroptosis contributes to mortality during sepsis. Sphingosine-1-phosphate receptor 2 (S1PR2) signaling can amplify interleukin-1 secretion in endotoxin-induced inflammation. Here, we hypothesized that S1PR2 signaling increases caspase-11-dependent macrophage pyroptosis and worsens Gram-negative sepsis outcome. Methods: A Gram-negative sepsis model was induced through intraperitoneal injection of Escherichia coli. Primary peritoneal macrophages isolated from wild-type, S1pr2-deficient (S1pr2(-/-)), or nucleotide-binding oligomerization domain-like receptor protein-3-deficient mice were treated with E. coli. Caspase-11 activation, macrophage pyroptosis, and Ras homolog gene family, member A-guanosine triphosphate levels were assessed in those cells. Additionally, monocyte caspase-4 (an analog of caspase-11) expression and its correlation with S1PR2 expression were determined in patients with Gram-negative sepsis (n = 11). Results: Genetic deficiency of S1PR2 significantly improved survival rate (2/10 [20%] in wild-type vs. 7/10 [70%] in S1pr2(-/-), P = 0.004) and decreased peritoneal macrophage pyroptosis (pyroptosis rate: 353% in wild-type vs. 10 +/- 3% in S1pr2(-/-), P < 0.001). Decreased caspase-11 activation in S1PR2 deficiency cells contributed to the reduced macrophage pyroptosis. In addition, RhoA inhibitor abrogated the amplified caspase-11 activation in wild-type or S1PR2-overexpressing cells. In patients with Gram-negative sepsis, caspase-4 increased significantly in monocytes compared to nonseptic controls and was positively correlated with S1PR2 (r = 0.636, P = 0.035). Conclusions: S1PR2 deficiency decreased macrophage pyroptosis and improved survival in E. coli sepsis. These beneficial effects were attributed to the decreased caspase-11 activation of S1PR2-deficient macrophages. S1PR2 and caspase-11 may be promising new targets for treatment of sepsis.

Abstract:Sepsis, a life-threatening organ dysfunction caused by a dysregulated host response to infection, is a leading cause of death in intensive care units. The development of sepsis-associated organ dysfunction (SAOD) poses a threat to the survival of patients with sepsis. Unfortunately, the pathogenesis of sepsis and SAOD is complicated, multifactorial, and has not been completely clarified. Recently, numerous studies have demonstrated that pyroptosis, which is characterized by inflammasome and caspase activation and cell membrane pore formation, is involved in sepsis. Unlike apoptosis, pyroptosis is a pro-inflammatory form of programmed cell death that participates in the regulation of immunity and inflammation. Related studies have shown that in sepsis, moderate pyroptosis promotes the clearance of pathogens, whereas the excessive activation of pyroptosis leads to host immune response disorders and SAOD. Additionally, transcription factors, non-coding RNAs, epigenetic modifications and post-translational modifications can directly or indirectly regulate pyroptosis-related molecules. Pyroptosis also interacts with autophagy, apoptosis, NETosis, and necroptosis. This review summarizes the roles and regulatory mechanisms of pyroptosis in sepsis and SAOD. As our understanding of the functions of pyroptosis improves, the development of new diagnostic biomarkers and targeted therapies associated with pyroptosis to improve clinical outcomes appears promising in the future.

Methods: 127 sepsis patients were included, including 79 cases in the survival group and 48 cases in the death group. The results of all patients on admission were recorded. After screening the risk factors of 28-day mortality, the receiver operating characteristic curve (ROC) was used to determine their predictive value for the 28-day mortality rate on admission, and the Kaplan-Meier curve was drawn to compare the 28-day mortality rate between groups. Finally, patients with cytokine and lymphocyte subsets results were included for investigating the possible causes of death through correlation analysis. Results: APACHE II (acute physiology and chronic health evaluation II), SOFA (Sequential Organ Failure Assessment) and red blood cell distribution width (RDW) were the risk factors for 28-day mortality in sepsis patients (OR: 1.130 vs.1.160 vs.1.530, P < 0.05). The area under the curve (AUC), sensitivity and specificity of APACHE II, SOFA and RDW in predicting the mortality rate at 28 days after admission in sepsis patients were 0.763 vs 0.806 vs 0.723, 79.2% vs 68.8% vs 75.0%, 65.8% vs 89.9% vs 68.4%. The combined predicted AUC was 0.873, the sensitivity was 89.6%, and the specificity was 82.3%. The Kaplan-Meier survival curve showed that the 28-day mortality rates of sepsis patients with APACHE II≥ 18.5, SOFA≥ 11.5 and RDW≥ 13.8 were 58.5%, 80.5% and 59.0%, respectively. In the death group, APACHE II was positively correlated with SOFA, IL-2, and IL-10, and RDW was positively correlated with PLT, TNF-α, CD3 + lymphocyte count, and CD8 + lymphocyte count. Conclusion: Sepsis patients with high APACHE II, SOFA and RDW levels at admission have an increased 28-day mortality rate. The elevation of these indicators in dead patients are related to immune dysfunction. The consensus on sepsis in 1991 declared that sepsis was caused by systemic inflammatory response syndrome (SIRS) of the host against infection. 2 Sepsis complicated with organ dysfunction is known as the severe type, which can progress to septic shock. 3 With the lack of further understanding of sepsis, its definition was gradually revised and improved. In 2016, sepsis was redefined as life-threatening organ dysfunction caused by the uncontrolled inflammatory response of the body. Judging by the acute increase of the SOFA score > 2 points, as described in Sepsis 3.0. 4 Sepsis patients have a high mortality rate. More than 250,000 sepsis patients die yearly in the United States, and the inpatient mortality rate is about 26%. 5 However, timely targeted interventions can improve the outcome of sepsis patients. 6 Therefore, identifying sepsis patients with a high risk of death in the early stage has significant clinical value. Research has shown that sepsis can be divided into an initial immune activation stage and a chronic immune suppression stage, ultimately leading to immune cell death. Abstract:A better understanding of sepsis-induced immunosuppression pathophysiology is desirable for the development of novel therapeutic strategies to prevent and reduce the rates of secondary infections and their associated mortality. Here we demonstrate that PD-L1+CD44+B220LowCD138+IgM+ regulatory plasma cells (PCs) are induced in a murine model of sepsis-induced immune alterations and in critically ill patients with bacterial sepsis and COVID-19. This was revealed both by detailed analysis of their phenotypical features and gene expression profile and by functional explorations comparing capacity of purified B cells and PCs to suppress T cell proliferation and IFNɣ secretion ex vivo. Sepsis-induced regulatory PCs exerted their suppressive function on T cells through IL-10 production and increased PD-L1 expression independently of regulatory T cells. Our findings thus reveal a novel pathophysiological mechanism of sepsis-induced immunosuppression that involves regulatory PCs. As such, these PCs constitute valid therapeutic targets to improve immune cell functions impaired by sepsis.

Abstract:Macrophage mitochondrial dysfunction is associated with immunosuppression and poor prognosis of septic patients. Mitochondrial fragmentation drives mitochondrial dysfunction. Our previous study has found that S1PR2 regulates macrophage phagocytosis during sepsis, while the role of S1PR2 in immunosuppression and the mechanisms remain further studied. This study aimed to unveil the relationship between macrophage mitochondrial fragmentation and sepsis-induced immunosuppression, as well as the S1PR2-related mechanisms thereof. Peripheral blood monocytes were collected from healthy controls (n = 12), nonseptic critical controls (n = 13) and septic patients (n = 19). Peritoneal macrophages were harvested from wildtype and S1pr2-/- mice (MMRRC strain iD, 12830) after cecal ligation and puncture (CLP). Mitochondrial ultrastructure was evaluated using transmission electron microscopy. The impact of mitochondrial ultrastructure alteration on immunosuppression of monocytes-macrophages was evaluated. Compared with nonseptic and healthy controls, peripheral blood monocytes from septic patients exhibited increased S1PR2 expression, mitochondrial fragmentation, and mitochondrial dysfunction. Mitochondrial fragmentation was negatively associated with HLA-DR expression. S1PR2 expression was positively correlated with mitochondrial fragmentation and negatively correlated with HLA-DR expression. In mice subjected to CLP, S1PR2 depletion ameliorated macrophage mitochondrial fragmentation and dysfunction, boosted immunity, and improved survival. Mechanistically, in response to sepsis, S1PR2 activates ROCK I to induce Drp1 phosphorylation, resulting in Drp1-dependent mitochondrial fragmentation of macrophages. Drp1 inhibition by Mdivi-1 mitigated S1PR2-induced macrophage immunosuppression and improved the prognosis of mice following CLP. In conclusion, S1PR2-induced mitochondrial fragmentation is a crucial factor mediating septic immunosuppression, highlighting its potential as a promising therapeutic target in sepsis.

Abstract:Key PointsSepsis is the systemic inflammatory response that occurs following severe infections and is characterized by a range of features, which might include fever, hypotension, altered mental status and shortness of breath.Although the predominant theory has been that the death of patients with sepsis was due to an over-exuberant inflammatory response, it is now apparent that many deaths are due to failure of the host to mount an effective immunological response. As the sepsis progresses, patients develop a state of immunoparalysis, marked by an inability of the host to eradicate the invading pathogen and predisposition to secondary infections.A major cause of the immunoparalysis of sepsis is the loss of key immune effector cells, including dendritic cells and lymphocytes. These cells die owing to sepsis-induced apoptosis.Uptake of apoptotic cells by professional scavenging cells induces a T helper 2 (TH2) phenotype or anergy in these phagocytic cells, thereby contributing to the immune suppression.Blockade of sepsis-induced apoptosis by a number of methods, including overexpression of B-cell lymphoma 2 (BCL-2) or AKT results in improved survival. This finding suggests that apoptosis is an important process in the pathophysiology of the disorder.It is now clear that caspases have other functions in the immune system in addition to their role as cell-death proteases. Caspases might also regulate inflammation, cellular activation and cellular proliferation.Strategies to block sepsis-induced apoptosis might represent a novel therapy of this highly lethal disorder.

Abstract:Neutrophils and T lymphocytes are closely related to occurrence of immunosuppression in sepsis. Studies have shown that neutrophil apoptosis decreases and T lymphocyte apoptosis increases in sepsis immunosuppression, but the specific mechanism involved remains unclear. In the present study, we found Toll-like Receptor 2 (TLR2) and programmed death-ligand 1 (PD-L1) were significantly activated in bone marrow neutrophils of wild-type mice after LPS treatment and that they were attenuated by treatment with C29, an inhibitor of TLR2. PD-L1 activation inhibits neutrophil apoptosis, whereas programmed death protein 1 (PD-1) activation promotes apoptosis of T lymphocytes, which leads to immunosuppression. Mechanistically, when sepsis occurs, pro-inflammatory factors and High mobility group box-1 protein (HMGB1) passively released from dead cells cause the up-regulation of PD-L1 through TLR2 on neutrophils. The binding of PD-L1 and PD-1 on T lymphocytes leads to increased apoptosis of T lymphocytes and immune dysfunction, eventually resulting in the occurrence of sepsis immunosuppression. In vivo experiments showed that the HMGB1 inhibitor glycyrrhizic acid (GA) and the TLR2 inhibitor C29 could inhibit the HMGB1/TLR2/PD-L1 pathway, and improving sepsis-induced lung injury. In summary, this study shows that HMGB1 regulates PD-L1 and PD-1 signaling pathways through TLR2, which leads to immunosuppression.

Abstract:ABSTRACT. The attained growth was assessed in a cross‐sectional study of Saudi children 0–71 months of age, from 3 different geographical areas of rural Saudi Arabia. individual data on weight, height, weight for height and head circumference were expressed in SD scores of international standards, and data on upper arm muscle and fat areas in percent of the reference median values. The mean (SEM) weight, height, weight for height and head circumference of the entire material were ‐1.7 (0.05), ‐1.6 (0.06), ‐0.9 (0.05) and ‐1.6 (0.06), respectively. The median upper arm muscle area was 79 percent, and fat area 57 percent of the reference medians. The differencies in mean weight, height and weight for height were trivial between the 3 geograhical areas. All mean values, except that of weight for height were clearly below the means of the reference in children 0–5 months. All mean values declined significantly, however, during the next 6 months. The growth deficits thus obtained remained unchanged or increased slightly up to 6 years. According to WHO criteria, 41 percent were classified as chronically, and 12 percent as acutely undernourished. The growth pattern of rural Saudi children was similar to that of children in most developing countries and distinctly different from that of privileged Saudi children, whose growth pattern resembled that of Western reference populations.

Abstract:Background Immunosuppression mediated by CD4+ T cell apoptosis and dysfunction is a key factor in promoting the progression of sepsis. Endoplasmic reticulum (ER) stress participates in the apoptosis and dysfunction of immune cells. Aim We aimed to investigate the role of ER stress inhibition in CD4+ T cells in both in vitro and in vivo models of sepsis. Methods In vitro model of sepsis was established with lipopolysaccharide (LPS) and the rat model of sepsis was established using cecal ligation and puncture (CLP). After the LPS treatment or CLP, ER stress inhibitors including 4‐PBA, SNJ‐1945, and SP600125 were used to treat cells or rats, and the CD4+ T cells were obtained by magnetic bead sorting. The effects of ER stress inhibitors on apoptosis and the function of CD4+ T cells were evaluated. Results After the LPS stimulation or CLP, the levels of ER stress and downstream markers (PERK, eIF2α, IRE‐1α, ATF6, ATF4, XBP‐1s, GRP78, CHOP, and p‐JNK) were increased in CD4+ T cells at the beginning of sepsis. Meanwhile, the number of apoptotic CD4+ T cells markedly increased. In addition, sepsis impaired the function of CD4+ T cells, manifested by the increased population of Th1, Th2, Th17, and Treg, as well as the production of TNF‐α, interleukin (IL)‐6, IL‐4, and IL‐10. However, inhibitors of ER stress, JNK, and calpain all decreased the induction of Th1 and Th17, enhanced the increase of Th2 and Treg, decreased the production of TNF‐α and IL‐6, and enhanced the production of IL‐4 and IL‐10. Conclusion Our findings indicate that ER stress inhibitors may play a protective role by reducing CD4+ T cell apoptosis and maintaining CD4+ T cell function, which may be useful for enhancing the immune function and poor prognosis of patients with sepsis.

Abstract:Abstract PD-L1 is a ligand for PD-1, and its expression has been shown to be upregulated in neutrophils harvested from septic patients. However, the effect of PD-L1 on neutrophil survival and sepsis-induced lung injury remains largely unknown. In this study, PD-L1 expression correlated negatively with rates of apoptosis in human neutrophils harvested from patients with sepsis. Coimmunoprecipitation assays on control neutrophils challenged with interferon-γ and LPS showed that PD-L1 complexes with the p85 subunit of phosphatidyl 3-kinase (PI3K) to activate AKT-dependent survival signaling. Conditional CRE/LoxP deletion of neutrophil PD-L1 in vivo further protected against lung injury and reduced neutrophil lung infiltration in a cecal ligation and puncture (CLP) experimental sepsis animal model. Compared with wild-type animals, PD-L1–deficient animals presented lower levels of plasma tumor necrosis factor-α and interleukin-6 (IL-6) and higher levels of IL-10 after CLP, and reduced 7-day mortality in CLP PD-L1–knockout animals. Taken together, our data suggest that increased PD-L1 expression on human neutrophils delays cellular apoptosis by triggering PI3K–dependent AKT phosphorylation to drive lung injury and increase mortality during clinical and experimental sepsis.

Abstract:Tea extracts are used in many over-the-counter preparations claiming to promote weight loss. The rationale for this usage includes reports that green tea extract increases thermogenesis, and extracts of green and black tea and mulberry leaf inhibit the digestion/absorption of carbohydrate and fat. The investigators in this study tested the potential of increasing doses of a mixture of three extracts (50-percent black tea, 20-percent green tea, and 30-percent mulberry) to induce weight loss, steatorrhea, and blood lipid alterations in rats ingesting a high-fat diet, ad lib. The mixture was incorporated into chow in quantities of 0.5-, 3.0-, and 6.0 percent by weight; a control group received only chow. Food intake and weight were monitored daily, and quantitative fecal fat measurements were obtained weekly for four weeks. The 3.0- and 6.0-percent chows significantly increased fecal fat excretion to 15 percent of dietary fat intake (controls: 5 percent); however, no significant reduction in weight gain was observed. After four weeks of treatment, the 3.0- and 6.0-percent dosages were associated with significant reductions in serum triglycerides and increases in high-density lipoprotein (HDL) cholesterol. However, these chow concentrations were associated with significant increases in serum ALT, and the 6.0-percent chow markedly increased serum alkaline phosphatase. This study does not provide support for the utility of this combination of black tea, green tea, and mulberry extracts in weight-loss regimens and indicates that high doses of this extract combination may be hepatotoxic.
Peripheral immune cell death in sepsis based on bulk RNA and single-cell RNA sequencing

Abstract:Background Immune cell activation in early sepsis is beneficial to clear pathogens, but immune cell exhaustion during the inflammatory response induces immunosuppression in sepsis. Here, we studied the relationship between immune cell survival status and the prognosis of sepsis patients. Methods Sepsis patients admitted to our hospital with a diagnosis time of less than 24 h were recruited. RNA sequencing technologies were used to study functional alterations in various immune cells in peripheral blood mononuclear cells (PBMCs) from sepsis patients. Flow cytometry and electron microscopy were performed to study cell apoptosis and morphological alterations. Results A total of 68 sepsis patients with complete data were enrolled and divided into survival (45 patients) and death (23 patients) groups according to their prognosis. Patients in the death group had significantly increased lactic acid levels compared with those in the survival group, but there was no significant difference in other physiological and coagulation functional indicators between the two groups. Bulk RNA sequencing showed that cell death-related pathways and biomarkers were highly enriched and activated in the PBMCs of the death group than that in the survival group. Signs of mitochondrial damage, autophagosomes, cell surface damage and cell surface pore forming were also more pronounced in PBMCs from the death group under electron microscopy. Further single-cell RNA sequencing revealed that cell death occurred mainly in myeloid cells rather than lymphocytes at the early stage of sepsis; cell death patterns of destructive necrosis and pyroptosis were predominant in neutrophils, and apoptosis, autophagy and ferroptosis with less damage to the surroundings were predominant in monocytes. Conclusion Cell death mainly occurs in monocytes and neutrophils in the PBMCs of sepsis at the early stage. The study provides a perspective for the immunotherapy of early sepsis targeting immune cell death.
