Abstract:Objective:To explore the measurement of portal vein pressure (PVP) based on computational fluid dynamics (CFD) and its clinical value in the preoperative planning of resection of hepatic carcinoma.Methods:From January 2020 to April 2021, CT data of 16 patients with hepatocellular carcinoma were imported into 3d-doctor 4.0 software to reconstruct the liver 3D model(including portal vein) at Yuebei people’s Hospital Affiliated to Medical College of Shantou University: (1) PVP was simulated by ANSYS 19.0 software, and compared with the actual PVP measured during the operation or the intervention. Pearson correlation was used to analyze the correlation between the actual PVP and the simulated PVP; (2) The future liver remnant(FLR) was calculated according to the preoperative surgical planning; (3) The FLR combined with the simulated PVP was used to determine the selection of treatment for hepatocellular carcinoma. Continuous variables were compared using Student’s t-test, Pearson test was used for correlation analysis. Results:There was no significant difference between the simulated PVP and the actual PVP [(22.2±9.2) cmH 2O vs. (20.9±8.4) cmH 2O, t=0.419, P>0.05]. The simulated PVP was positively correlated with the actual PVP ( r=0.965, P<0.01). Based on the simulated PVP combined with the FLR, 14 patients underwent hepatectomy, 2 patients underwent the terminal portal vein embolization combined with TACE. Conclusion:The PVP level based on CFD simulation reflects the actual PVP level, and combined with the FLR can guide the choice of hepatocellular carcinoma treatment.

Abstract:Objective: To analyze the efficacy of branches portal vein embolization (TBPVE) combined with transcatheter arterial chemoembolization (TACE) on liver neoplasms. Methods: From August 2016 to May 2017, there were 13 patients including 11 males and 2 females with primary hepatocellular carcinoma who underwent TBPVE+ TACE , among whom there were 11 cases with a history of HBV infection.Average age of the 13 patients was (60.8±6.2)years. The live function of all patients were Child-Pugh A classification.The CT or MRI images of each patient was reconstructed and the standard liver volume(SLV) before TBPVE+ TACE was (1 181.2±49.3)ml, estimated future liver remnant(FLR) was (326.1±72.1)ml and FLR/SLV was (27.6±6.0)%.The puncture site for TBPVE was determined by the three-dimensional reconstruction of portal vein.CT scan or MRI, AFP and liver function test were repeated after one and two weeks after TBPVE+ TACE.FLR and FLR/SLV were calculated respectively.Hepatectomy would be performed if the patients agreed.The postoperative complications were analyzed. Results: On the 7thday after TBPVE+ TACE, the FLR/SLV was(42.6±8.0)% and the FLR increasement was(56.0±24.6)%.The level of AFP decreased from(87.9±81.8)μg/L to (29.7±20.9)μg/L.On the 14thday after TBPVE+ TACE, the FLR/SLV was(45.8±6.2)% and the FLR increasement was(71.8±29.0)%.Four patients underwent surgery which including 2 right hepatectomies and 2 right trisegmentectomies in 2 weeks after TBPVE+ TACE.Nine patients were performed with targeting intratumoral lactic acidosis TACE (TILA-TACE). No severe complication occurred in all patients. Conclusions: TBPVE could induce a rapid growth of the liver remnant but still with the concern of inducing the growth of neoplasms at the same time.To combine TACE in TBPVE therapy not also can the growth of neoplasms be prevented but also inducing its shrinking.This method might be a new mode for the treatment of hepatocellular carcinoma.
Right portal vein ligation is still relevant for left hemi-liver hypertrophy: results of a comparative study using a propensity score between right portal vein ligation and embolization

Abstract:Background: In two-stage hepatectomy for bilobar liver metastases from colorectal cancer, future liver remnant (FLR) growth can be achieved using several techniques, such as right portal vein ligation (RPVL) or right portal vein embolization (RPVE). A few heterogeneous studies have compared these two techniques with contradictory results concerning FLR growth. The objective of this study was to compare FLR hypertrophy of the left hemi-liver after RPVL and RPVE. Study design: This was a retrospective comparative study using a propensity score of patients who underwent RPVL or RPVE prior to major hepatectomy between January 2010 and December 2020. The endpoints were FLR growth (%) after weighting using the propensity score, which included FLR prior to surgery and the number of chemotherapy cycles. Secondary endpoints were the percentage of patients undergoing simultaneous procedures, the morbidity and mortality, the recourse to other liver hypertrophy procedures, and the number of invasive procedures for the entire oncologic program in intention-to-treat analysis. Results: Fifty-four consecutive patients were retrospectively included and analyzed, 18 in the RPVL group, and 36 in the RPVE group. The demographic characteristics were similar between the groups. After weighting, there was no significant difference between the RPVL and RPVE groups for FLR growth (%), respectively 32.5% [19.3-56.0%] and 34.5% [20.5-47.3%] (p = 0.221). There was no significant difference regarding the secondary outcomes except for the lower number of invasive procedures in RPVL group (median of 2 [2.0, 3.0] in RPVL group and 3 [3.0, 3.0] in RPVE group, p = 0.001)). Conclusion: RPVL and RPVE are both effective to provide required left hemi-liver hypertrophy before right hepatectomy. RPVL should be considered for the simultaneous treatment of liver metastases and the primary tumor.

Abstract:Objective To explore the clinical outcome of percutaneous microwave/radiofrequency ablation liver partition and portal vein embolization for planned hepatectomy (PAPEP) for the treatment of hepatocelullar carcinoma (HCC) with insufficient future liver remnant (FLR).Methods The retrospective descriptive research method was taken.The clinical data of 2 patients of giant primary HCC (FLR complicated with cirrhosis) who were admitted to the Zhejiang Province People's Hospital from November 2015 to January 2016 were collected.Treatment plan:ultrasound-guided percutaneous microwave ablation (PMA) or percutaneous radio-frequency ablation (PRA) was performed on patients under intravenous anesthesia,at the same time portal vein embolization (PVE) was conducted.FLR was measured by rescan of computed tomography (CT) at 7-9 days postoperatively,according to FLR hyperplasia and general situation of the patient,right trisegmentectomy and expanded right hemihepatectomy were implemented respectively.Observation indicatiors indicators included (1)standard liver volume (SLV) and FLR before PRA/PVE treatment,(2) liver function,body temperature,FLR and time of FLR hyperplasia after PRA/PVE treatment,(3)liver function,body temperature,complications,FLR and duration of hospital stay after hepatectomy.Follow-up using outpatient examination and telephone interview was performed to detect carcinoma recurrence and other follow-up contents up to March 2016.Results (1) Before PRA + PVE,SLVs of 2 patients were 1 103 mL and 1 132mL,FLRs of 2 patients were respectively 417 mL and 297 mL which occupied the SLV at the percentage of 37.8％ and 26.2％ respectively.(2) After PRA + PVE treatment,there were no changes in liver function and fever.After PRA + PVE treatment,FLR of 2 patients increased to 652 mLand 522 mL,respectively,which occupied the SLV at the percentage of 59.1％ and 46.1％ respectively,with increased rates of 56.4％ and 75.6％ and time of FLR hyperplasia of 10 days and 17 days.The two patients received successful hepatectomy,without intraoperative red blood cell infusion.(3) At postoperative day 3,albumin (Alb),alanine transaminase (ALT),aspartate transaminase (AST) and total bilirubin (TBil) of liver function were 28.0 g/L,63 U/L,39 U/L and 19.0 μmol/L in patient 1 with a temperature of 38 degree,and 24.3 g/L,78 U/L,11 U/L and 38.1 μmol/L in patient 2 with a normal temperature.Liver function of 2 patients was improved after conservative treatment.No severe complications were occurred.Rescan of CT showed the FLR of patients continued increasing.The duration of hospital stay of 2 patients was 10 days and 14 days,respectively.Conclusion PAPEP for HCC with insufficient FLR is safe and effective.
Preoperative Portal Vein Embolization for Liver Resection: An updated meta-analysis

Abstract:Background: Portal vein embolization (PVE) is performed before major liver resection to increase liver volume remnant, controversy remains on the adverse effect of PVE on liver tumor patients. The current study highlighted the effect of PVE on the degree of hypertrophy of future liver remnant (FLR) and summarized PVE-related complications, aiming to provide a guideline for surgeons.Methods: A search of current published studies on PVE was performed. Meta-analysis was conducted to assess the effect of PVE on hypertrophy of FLR and summarized PVE-related complications.Results: 26 studies including 2335 patients were enrolled in the meta-analysis. All enrolled studies reported data regarding FLR hypertrophy rate, pooled effect size (ES) for FLR hypertrophy rate using a fixed-effect model was 0.105 (95%CI: 0.094-0.117, p=0.000), indicating PVE is favored in inducing FLR hypertrophy. Metatrim method indicated no obvious evidence of publication bias in the present meta-analysis. 247 (10.6%) patients exhibited PVE-related complications, receiving expectant treatment without affecting planned liver resection. Total 1782 patients (76%) underwent a subsequent liver resection after PVE, which is an encouraging result comparing with traditional resection rate in liver tumor patients.Conclusions: PVE is a safe and effective procedure with a low occurrence of related complications for inducing sufficient hypertrophy of FLR in liver tumor patients, which could elevate the resection rate of liver tumor patients. Careful patient cohort selection is crucial to avoid overuse of PVE in technically resectable patients. Further multiple central clinical trials are conducive to select optimal patient cohorts and provide a guideline for surgeons.

Abstract:Objective To evaluate the technique of selective portal vein embolization before hepatectomy and its value in the preparation of major hepatic resection for those with insufficient future remnant liver.Methods From Jan 2008 to July 2012,6 patients who suffered from hepatic tumors underwent selective ipsilateral portal vein embolization (PVE) due to insufficient future remnant liver volume (FRLV) before second-stage major hepatic resection.Results Technically,all six PVE were completed successfully with only minor liver function damages.The average FRLV increased from (474.33 ± 89.19)cm3 to (722.67 ± 151.51) cm3 (t =-5.587,P =0.003).The average tumor burden (total tumor volume)increased from (134 ± 181) cm3 to (270 ± 346) cm3 (t =-1.64,P =0.16).Five cases underwent secondstage major hepatectomy 6 weeks after PVE,while in 1 case a resection attempt was abandoned because of uncontrolled tumor growth during the period.During the follow-up period (median 37 months),1 died,4 survived,2 were tumor-free.Conclusions Selective portal vein embolization is a safe and effective method to induce hepatic hypertrophy in the appropriate clinical setting.Before PVE,hepatic tumor should be controlled beforehand with chemotherapy or TACE to ensure the scheduled second-stage hepatectomy.
Hepatic venous pressure gradient after portal vein embolization: An accurate predictor of future liver remnant hypertrophy

Abstract:BACKGROUND: The impact of portal hemodynamic variations after portal vein embolization on liver regeneration remains unknown. We studied the correlation between the parameters of hepatic venous pressure measured before and after portal vein embolization and future hypertrophy of the liver remnant after portal vein embolization.METHODS: Between 2014 and 2017, we reviewed patients who were eligible for major hepatectomy and who had portal vein embolization. Patients had undergone simultaneous measurement of portal venous pressure and hepatic venous pressure gradient before and after portal vein embolization by direct puncture of portal vein and inferior vena cava. We assessed these parameters to predict future liver remnant hypertrophy.RESULTS: Twenty-six patients were included. After portal vein embolization, median portal venous pressure (range) increased from 15 (9-24) to 19 (10-27) mm Hg and hepatic venous pressure gradient increased from 5 (0-12) to 8 (0-14) mm Hg. Median future liver remnant volume (range) was 513 (299-933) mL before portal vein embolization versus 724 (499-1279) mL 3 weeks after portal vein embolization, representing a 35% (7.4-83.6) median hypertrophy. Post-portal vein embolization hepatic venous pressure gradient was the most accurate parameter to predict failure of future liver remnant to reach a 30% hypertrophy (c-statistic: 0.882 [95% CI: 0.727-1.000], P < 0.001). A cut-off value of post-portal vein embolization hepatic venous pressure gradient of 8 mm Hg showed a sensitivity of 91% (95% CI: 57%-99%), specificity of 80% (95% CI: 52%-96%), positive predictive value of 77% (95% CI: 46%-95%) and negative predictive value of 92.3% (95% CI: 64.0%-99.8%). On multivariate analysis, post-portal vein embolization hepatic venous pressure gradient and previous chemotherapy were identified as predictors of impaired future liver remnant hypertrophy.CONCLUSION: Post-portal vein embolization hepatic venous pressure gradient is a simple and reproducible tool which accurately predicts future liver remnant hypertrophy after portal vein embolization and allows early detection of patients who may benefit from more aggressive procedures inducing future liver remnant hypertrophy. Abstract:Purpose To compare the safety and effectiveness of liver vein deprivation (LVD) and portal vein embolization (PVE) in patients scheduled to undergo liver resection. Materials and Methods This retrospective cohort study included 59 patients who underwent either PVE (n = 28) or LVD (n = 31) in preparation for liver resection. The primary outcome was percent change in future liver remnant volume (FLRV). Secondary endpoints were degree of hypertrophy (DH) and kinetic growth rate (KGR). Results Low baseline FLRV, time interval in days between the procedure and follow-up imaging (Ti) positively impacted the primary and secondary endpoints in both groups. Percent change in FLRV was higher in the LVD group (52.8% ± 5.3%) than in the PVE group (22.3% ± 3.0%; P <.001). DH was also higher in the LVD group (15.4% ± 1.7%) than in the PVE group (6.4% ± 0.9%; P 35% were excluded from the analysis, LVD group demonstrated higher values than the PVE group in KGR (0.57%/d ± 0.06%/d vs 0.29%/d ± 0.05%/d; P <.001), percent change in FLRV (64.2% ± 6.0% vs 25.9% ± 4.3%; P <.001), and DH (15.4% ± 1.4% vs 6.6% ± 1.0%; P <.001). No adverse events were noted in either group. Conclusion LVD appears to be safe and may be superior to PVE in inducing hypertrophy of FLR in patients scheduled to undergo surgical resection.

Preoperative right portal vein embolisation: indications and results

Abstract:Purpose: The purpose of this retrospective study was to evaluate the efficacy of right portal vein embolisation (PVE) in inducing contralateral liver hypertrophy before extended hepatectomy. Materials and methods: Twenty-six consecutive patients, 14 with liver metastases (ten from colorectal cancer; four from carcinoid tumours) and 12 with biliary cancers (ten Klatskin tumours; one gallbladder tumour; one intrahepatic cholangiocarcinoma) with insufficient predicted future remnant liver (FRL) underwent right PVE to induce hypertrophy of the contralateral hemiliver prior to surgical resection. Total liver volume, tumour volume and FRL volume were calculated on a 3D workstation. The ratio of the FRL to the total functional liver volume was <30% in all patients. Results: The FRL volume increased by 5%-25% (15% on average) after right PVE in patients with liver metastases and by 9%-19% (14% on average) in patients with biliary cancers. In all patients, the ratio of FRL to functional liver volume was >or=30% after right PVE. No postoperative deaths due to severe liver failure occurred in the 20 patients who underwent extended hepatectomy. Conclusions: Right PVE extends the indications for hepatectomy in patients with liver metastases and those with biliary cancers who have an insufficient potential hepatic functional reserve.
Effects of Liver Cirrhosis on Portal Vein Embolization Prior to Right Hepatectomy in Patients with Primary Liver Cancer

Abstract:Preoperative portal vein embolization (PVE) induces compensatory hyperplasia of the future liver remnants (FLR), thus increasing resectability in the non-cirrhotic patients with primary liver cancer (PLC). However, it is unclear if it is similar in patients with liver cirrhosis. Therefore, the present study investigated the PVE value prior to liver resection in patients with PLC, and the liver cirrhotic effects on the compensatory hypertrophy of FLRs following PVE. In the present study, 21 patients with PLC who successfully underwent hepatic resection subsequent to PVE, were retrospectively examined. The patients were divided into a non-cirrhosis group and a cirrhosis group according to the absence or presence of cirrhosis, respectively. The FLR volume between the two groups of patients was compared. There was a significant difference in the FLR volume for all patients prior to, and 4-6 weeks following, PVE (P<0.001). PVE induced significant compensatory hypertrophy in the FLRs whether in the non-cirrhosis group (P=0.002) or cirrhosis group (P<0.001). However, no significant difference was identified between the two groups with respect to FLR volume enlargement 4-6 weeks following PVE (P=0.373). In conclusion, PVE prior to hepatectomy may promote FLR compensatory hypertrophy and an increase in the resectability of PLC tumors. No significant effects of liver cirrhosis were identified on liver lobe hyperplasia following PVE.
Portal Vein Thrombosis Can Aggravate Esophageal Variceal Bleeding After Endoscopic Variceal Ligation

Abstract:Background: We aimed to evaluate whether portal vein thrombosis (PVT) and other factors influence esophageal variceal bleeding (EVB) after endoscopic variceal ligation (EVL) for liver cirrhosis patients. Methods: We retrospectively enrolled 255 patients with EVL in our hospital and parameters of age, gender, blood platelet, albumin, total bilirubin (TBil), D-dimer, activated partial thromboplastin time (APTT), prothrombin time (PT), Child-Pugh classification, portal vein thrombosis (PVT) and splenectomy were collected. Results: EVB patients had higher PVT diagnosed rate than non-EVB patients (P=0.009) and the rate of EVB had an increasing trend with the severity of ascites (P=0.034). On multivariate analysis, we still found PVT and ascites obviously affected incidence rate of EVB (P=0.026, P=0.027, respectively). Conclusion: PVT and ascites per se appeared to be the cause of EVB subsequent to EVL. If detected, anticoagulation or treatments refer to PVT and ascites needed to conducted in order to reduce the incidence of EVB after EVL.

Abstract:Preoperative portal vein embolization (PVE) is increasingly used to optimize the volume and function of the future liver remnant (FLR) and to reduce the risk for complications of major hepatectomy for hepatocellular carcinoma (HCC) or hilar cholangiocarcinoma (CCA). In patients with HCC who are candidates for extended hepatectomy and in patients with HCC and well-compensated cirrhosis who are being considered for major hepatectomy, FLR volumetry is routinely performed, and PVE is employed in selected cases to optimize the volume and function of the FLR prior to surgery. Similarly, in patients with hilar CCA who are candidates for extended hepatectomy, careful preoperative preparation using biliary drainage, FLR volumetry, and PVE optimizes the volume and function of the FLR prior to surgery. Appropriate use of PVE has led to improved postoperative outcomes after major hepatectomy for these diseases and oncological outcomes similar to those in patients who undergo resection without PVE. Specific indications for PVE are being clarified. FLR volumetry is necessary for proper selection of patients for PVE. Analysis of the degree of hypertrophy of the FLR after PVE (a dynamic test of liver regeneration) complements analysis of the pre-PVE FLR volume (a static test). Together, FLR degree of hypertrophy and FLR volume are the best predictors of outcome after major hepatectomy in an individual patient, regardless of the degree of underlying liver disease. This article synthesizes the literature on the approach to patients with HCC and CCA who are candidates for major hepatectomy. The rationale and indications for FLR volumetry and PVE and outcomes following PVE and major hepatectomy for HCC and CCA are discussed.

Abstract:Background: The optimal surgery for liver cancer cases with small future liver remnant (FLR) remains controversial; associating liver partition and portal vein ligation for staged hepatectomy (ALPPS), portal vein embolization (PVE), and portal vein ligation (PVL) are all used.Methods: We conducted a systematic search in the EMBASE, Medline and Cochrane databases, without year or language restrictions, to identify studies that compared PVE, PVL, and ALPPS. We compared the feasibility, safety, and efficacy of PVE, PVL, and ALPPS using network meta-analyses. this study do not involve ethical issues.Results: Ten studies and 451 patients were included. PVE had a significantly reduced FLR increase (42.2%; 95% CI, 26.52-57.87; P< 0.001) and resection completion rate (89.2%; OR =0.108; 95% CI, 0.02-0.585; P=0.01) compared with ALPPS. PVE also had a non-significant reduction in morbidity and mortality, and a non-significant increase in waiting time from first intervention to removal of tumors than ALPPS. Similarly, PVL had a significantly reduced FLR increase (47.65%; 95% CI, 29.43-65.88%; P< 0.001) and resection completion rate (91.6%; OR = 0.084; 95% CI, 0.013-0.532; P=0.009) compared with ALPPS; as well as a non-significant reduction in morbidity and mortality, and increase in waiting time versus ALPPS. Rank analysis indicated that ALPPS was superior, with respective probabilities of 100%, 99.4%, and 44.9% for FLR increase, resection completion, and waiting time; PVE and PVL were not significantly different.Conclusions: ALPPS was significantly more efficient than PVE and PVL regarding promotion of FLR hypertrophy, resection completion, and waiting time. The three techniques were similar regarding morbidity and mortality.
[Effect of Liver Cirrhosis on Percutaneous Selective Portal Vein Embolization for Primary Liver Cancer].

Abstract:OBJECTIVE:To evaluate the clinical value of PVE (portal vein embolism) prior to surgery in primary liver cancer (PLC) patients and the effect of liver cirrhosis on liver lobe hyperplasia after PVE.METHODS:13 patients with primary liver cancer non-suitable for curative hepatectomy underwent k sequential transcatheter arterial chemoembolization (TACE) (1-3 times) and percutaneous selective portal vein embolization (PVE) when the remnant liver volumes were predicted to be insufficient. All patients were divided into non-cirrhosis (n = 7) and cirrhosis group (n = 6). Left liver remnant volumes were assessed by computed tomography (CT) before and after PVE. Right liver resection was performed when the remnant liver volume was sufficient.RESULTS:All patients underwent TACE treatment was successful. The frequency of TACE was 1-3. PVE was all successfully performed at weeks 2-4 after final TACE, all PVE patients subsequently underwent hepatic lobectomy. Left liver volume increased from (457.0 ± 121.0) cm(3) pre-PVE to (633.6 ± 120.2)cm(3) post-PVE. Hepatic lobe volume increased (44.4 ± 39.7)%. Statistical difference existed in left hepatic lobe volume before and weeks 4-6 after PVE (P = 0.000). The mean volume of left liver, calculated before and 4-6 weeks after PVE, increased from (442.0 ± 96.8) to (652.3 ± 115.8) cm(3) in non-cirrhotic group and from (474.5 ± 152.4) to (611.7 ± 132.3) cm(3) in cirrhotic group. Hepatic lobe volume increased (54.5 ± 50.7)% and (32.7 ± 19.9)% respectively. Statistical differences were both detected in left hepatic lobe volume before and 4-6 weeks after PVE (P = 0.011, P = 0.003). However, no significant inter-group difference existed at Weeks 4-6 weeks (P = 0.295) . Liver function damage was minimal after PVE and no serious complications occurred.CONCLUSION:Sequential transcatheter arterial chemoembolization, TACE and percutaneous selective PVE before surgery may cause remnant liver compensatory hypertrophy and increase the resection rate of primary liver cancer. Liver cirrhosis has no significant effect on liver lobe hyperplasia after PVE.

Abstract:Abstract The aim of this study was to examine the function of the lateral parabrachial area (LPB) in relation to the intragastric administration of nutrients. The consumption of flavors associated with intragastric nutrient administration and the subsequent food and water intake were measured in rats with lesions in the LPB. The results showed that bilateral LPB lesions prevented development of aversions and induced flavor preference when there was a delay between the presentation of a flavor and the intragastric administration of nutrients. However, these lesions did not disrupt development of the aversive process when there was no delay between the presentations. Likewise, the LPB lesions increased subsequent food intake when there was a delay but not when there was no delay between the presentations. In contrast, the water intake was reduced in both situations. These results are interpreted in terms of a dual visceral system for processing the intragastric effects of foods.

Abstract:Introduction This systematic review aimed to compare liver venous deprivation (LVD) with portal vein embolization (PVE) in terms of future liver volume, postoperative outcomes, and oncological safety before major hepatectomy. Methods We conducted this systematic review and meta-analysis following the PRISMA guidelines 2020 and AMSTAR 2 guidelines. Comparative articles published before November 2022 were retained. Results The literature search identified nine eligible comparative studies. They included 557 patients, 207 in the LVD group and 350 in the PVE group. This systematic review and meta-analysis concluded that LVD was associated with higher future liver remnant (FLR) volume after embolization, percentage of FLR hypertrophy, lower failure of resection due to low FLR, faster kinetic growth, higher day 5 prothrombin time, and higher 3 years’ disease-free survival. This study did not find any difference between the LVD and PVE groups in terms of complications related to embolization, FLR percentage of hypertrophy after embolization, failure of resection, 3-month mortality, overall morbidity, major complications, operative time, blood loss, bile leak, ascites, post hepatectomy liver failure, day 5 bilirubin level, hospital stay, and three years’ overall survival. Conclusion LVD is as feasible and safe as PVE with encouraging results making some selected patients more suitable for surgery, even with a small FLR. Systematic review registration The review protocol was registered in PROSPERO before conducting the study (CRD42021287628).

Abstract:Background: Preoperative portal vein embolization (PVE) is frequently used to improve future liver remnant volume (FLRV) and to reduce the risk of liver failure after major liver resection. Objective: This paper aimed to assess postoperative outcomes after PVE and resection for suspected perihilar cholangiocarcinoma (PHC) in an international, multicentric cohort. Methods: Patients undergoing resection for suspected PHC across 20 centers worldwide, from the year 2000, were included. Liver failure, biliary leakage, and hemorrhage were classified according to the respective International Study Group of Liver Surgery criteria. Using propensity scoring, two equal cohorts were generated using matching parameters, i.e. age, sex, American Society of Anesthesiologists classification, jaundice, type of biliary drainage, baseline FLRV, resection type, and portal vein resection. Results: A total of 1667 patients were treated for suspected PHC during the study period. In 298 patients who underwent preoperative PVE, the overall incidence of liver failure and 90-day mortality was 27% and 18%, respectively, as opposed to 14% and 12%, respectively, in patients without PVE (p < 0.001 and p = 0.005). After propensity score matching, 98 patients were enrolled in each cohort, resulting in similar baseline and operative characteristics. Liver failure was lower in the PVE group (8% vs. 36%, p < 0.001), as was biliary leakage (10% vs. 35%, p < 0.01), intra-abdominal abscesses (19% vs. 34%, p = 0.01), and 90-day mortality (7% vs. 18%, p = 0.03). Conclusion: PVE before major liver resection for PHC is associated with a lower incidence of liver failure, biliary leakage, abscess formation, and mortality. These results demonstrate the importance of PVE as an integral component in the surgical treatment of PHC.

Abstract:Background: Inadequate volume of future liver remnant (FLR) is a major challenge for hepatobiliary surgeons treating large or multiple liver tumors. As an alternative to associating liver partition and portal vein ligation (ALPPS) for staged hepatectomy and liver venous deprivation (LVD) using stage 1 interventional radiology for vascular embolization combined with stage 2 open liver resection have been used. Case summary: A novel modified LVD technique was performed in a patient with pancreatic neuroendocrine tumor with liver metastases by using stage 1 laparoscopic ligation of the right hepatic vein, right posterior portal vein, and short hepatic veins combined with local excision of three liver metastases in the left hemiliver. The operation was followed three days later by interventional radiology to embolize an anomalous right anterior portal vein to complete LVD. A stage 2 laparoscopic right hemihepatectomy and pancreaticosplenectomy were then carried out. Conclusion: The minimally invasive technique promoted a rapid increase, comparable to ALPPS, in volume of the FLR after the stage 1 operation to allow the laparoscopic stage 2 resection to be performed.

Abstract:AIM: To assess the effect of portal vein embolization (PVE) on intrahepatic recurrence rate after right hepatectomy for unilobar colorectal liver metastases (CLM).SUMMARY AND BACKGROUND: Recent research suggests that CLM could spread retrogradely through the portal vein. PVE may reduce tumor shedding by the occlusion of distal portal branches. However, no study reported the clinical effect of PVE on intrahepatic recurrence after CLM resection.PATIENTS AND METHODS: Between 1995 and 2003, 44 patients requiring a right hepatectomy for unilobar CLM were operated in our institution. Right hepatectomy was performed after PVE in 23 patients (group A) and without PVE in 21 (group B). Surgical outcome and site of recurrence were analyzed.RESULTS: The postoperative mortality was nil. Overall morbidity and transitory liver failure rates were similar in groups A and B (43.4% and 17.3% vs. 33.3% and 14.2%, respectively). The 3- and 5-year overall survival rates did not differ in group A and B patients (61.2% and 43.7% vs. 49.7% and 35.5%, respectively; P = 0.862). The disease-free survival rate was similar in both groups. Thirty patients (68.2%) developed recurrences. Recurrences were intrahepatic in 22 patients (50%) and extrahepatic in 27 (61.3%). Intrahepatic recurrence rate was significantly lower in group A compared with group B (26.0% vs. 76.1% respectively; P < 0.001). PVE, number of CLM, and administration of neoadjuvant chemotherapy were independent prognostic factors for intrahepatic recurrences.CONCLUSION: This study showed that PVE reduces intrahepatic recurrence rate after right hepatectomy for unilobar CLM.

Abstract:Background: Liver venous deprivation (LVD) is a recent radiological technique that has shown promising results on Future Remnant Liver (FRL) hypertrophy. The aim of this retrospective study is to compare the segmentary hypertrophy of the FRL after LVD and after portal vein embolization (PVE). Methods: Patients undergoing PVE or LVD between April 2015 and April 2020 were included. The segmentary volumes (seg 4, seg2+3 and seg1) were assessed before and after the radiological procedure. Results: Forty-four patients were included: 26 undergoing PVE, 10 LVD and 8 eLVD. Volume gain of both segment 1 and segments 2+3 was significantly higher after LVD and eLVD than after PVE (segment 1: 27.33 ± 35.37 after PVE vs. 38.73% ± 13.47 after LVD and 79.13% ± 41.23 after eLVD, p = 0.0080; segments 2+3: 40.73% ± 40.53 after PVE vs. 45.02% ± 21.53 after LVD and 85.49% ± 45.51 after eLVD, p = 0.0137), while this was not true for segment 4. FRL hypertrophy was confirmed to be higher after LVD and eLVD than after PVE (33.53% ± 21.22 vs. 68.63% ± 42.03 vs. 28.11% ± 28.33, respectively, p = 0.0280). Conclusions: LVD and eLVD may induce greater hypertrophy of segment 1 and segments 2+3 when compared to PVE.
