Abstract:AIM: To investigate the value of non-contrast CT (NCCT)-based two-dimensional (2D) radiomics features in predicting haematoma expansion (HE) after spontaneous intracerebral haemorrhage (ICH) and compare its predictive ability with the three-dimensional (3D) signature.MATERIALS AND METHODS: Three hundred and seven ICH patients who received baseline NCCT within 6 h of ictus from two stroke centres were analysed retrospectively. 2D and 3D radiomics features were extracted in the manner of one-to-one correspondence. The 2D and 3D models were generated by four different machine-learning algorithms (regularised L1 logistic regression, decision tree, support vector machine and AdaBoost), and the receiver operating characteristic (ROC) curve was used to compare their predictive performance. A robustness analysis was performed according to baseline haematoma volume.RESULTS: Each feature type of 2D and 3D modalities used for subsequent analyses had excellent consistency (mean ICC >0.9). Among the different machine-learning algorithms, pairwise comparison showed no significant difference in both the training (mean area under the ROC curve [AUC] 0.858 versus 0.802, all p>0.05) and validation datasets (mean AUC 0.725 versus 0.678, all p>0.05), and the 10-fold cross-validation evaluation yielded similar results. The AUCs of the 2D and 3D models were comparable either in the binary or tertile volume analysis (all p>0.5).CONCLUSION: NCCT-derived 2D radiomics features exhibited acceptable and similar performance to the 3D features in predicting HE, and this comparability seemed unaffected by initial haematoma volume. The 2D signature may be preferred in future HE-related radiomic works given its compatibility with emergency condition of ICH.(c) 2023 Published by Elsevier Ltd on behalf of The Royal College of Radiologists.
Automated detection of critical findings in multi-parametric brain MRI using a system of 3D neural networks

Abstract:Abstract With the rapid growth and increasing use of brain MRI, there is an interest in automated image classification to aid human interpretation and improve workflow. We aimed to train a deep convolutional neural network and assess its performance in identifying abnormal brain MRIs and critical intracranial findings including acute infarction, acute hemorrhage and mass effect. A total of 13,215 clinical brain MRI studies were categorized to training (74%), validation (9%), internal testing (8%) and external testing (8%) datasets. Up to eight contrasts were included from each brain MRI and each image volume was reformatted to common resolution to accommodate for differences between scanners. Following reviewing the radiology reports, three neuroradiologists assigned each study to abnormal vs normal, and identified three critical findings including acute infarction, acute hemorrhage, and mass effect. A deep convolutional neural network was constructed by a combination of localization feature extraction (LFE) modules and global classifiers to identify the presence of 4 variables in brain MRIs including abnormal, acute infarction, acute hemorrhage and mass effect. Training, validation and testing sets were randomly defined on a patient basis. Training was performed on 9845 studies using balanced sampling to address class imbalance. Receiver operating characteristic (ROC) analysis was performed. The ROC analysis of our models for 1050 studies within our internal test data showed AUC/sensitivity/specificity of 0.91/83%/86% for normal versus abnormal brain MRI, 0.95/92%/88% for acute infarction, 0.90/89%/81% for acute hemorrhage, and 0.93/93%/85% for mass effect. For 1072 studies within our external test data, it showed AUC/sensitivity/specificity of 0.88/80%/80% for normal versus abnormal brain MRI, 0.97/90%/97% for acute infarction, 0.83/72%/88% for acute hemorrhage, and 0.87/79%/81% for mass effect. Our proposed deep convolutional network can accurately identify abnormal and critical intracranial findings on individual brain MRIs, while addressing the fact that some MR contrasts might not be available in individual studies.

A New Deep Learning Framework for Accurate Intracranial Brain Hemorrhage Detection and Classification Using Real-Time Collected NCCT Images

Abstract:Brain hemorrhage is a critical medical condition that is likely to cause long-term disabilities and death. Timely and precise emergency care, incorporating the accurate interpretation of computed tomography (CT) images, plays a crucial role in the effective management of a hemorrhagic stroke. However, conventional artificial intelligence methods are capable enough to detect the presence or absence of hemorrhage but fail to detect multiple types of hemorrhage with high accuracy. To address this, the paper introduces an innovative Deep Learning based approach that automatically detects, segments, and classifies subtypes of intracranial hemorrhages. The proposed model is trained and evaluated on two different datasets. It is initially trained on a dataset of CT images from the Radiological Society of North America (RSNA) brain CT hemorrhage database, which contained 752,803 head non-contrast computer tomography images obtained from 2,200 patients. Furthermore, the model's performance is validated using a real-time CT dataset collected from a diagnostic lab, comprising 15,000 CT scan images from 176 patients. The proposed model surpasses standard benchmarks for detection and classification, achieving exceptional metrics. It showcases overall segmentation accuracy with a Dice score and Jaccard Index of 0.99 and 0.88 respectively, while the classification metrics include an accuracy of 0.99, precision, recall, and F1 score of 0.97, 0.98, and 0.97 respectively. When two expert radiologists independently assessed the predicted hemorrhage locations and subtypes, ensuring uniform specificity levels, they determined the observed rate of false positives per patient was less. These results validate its applicability as a dependable clinical decision support tool.

Abstract:Purpose.To determine glioma grading by applying radiomic analysis or deep convolutional neural networks (DCNN) and to benchmark both approaches on broader validation sets.Methods.Seven public datasets were considered: (1) low-grade glioma or high-grade glioma (369 patients, BraTS'20) (2) well-differentiated liposarcoma or lipoma (115, LIPO); (3) desmoid-type fibromatosis or extremity soft-tissue sarcomas (203, Desmoid); (4) primary solid liver tumors, either malignant or benign (186, LIVER); (5) gastrointestinal stromal tumors (GISTs) or intra-abdominal gastrointestinal tumors radiologically resembling GISTs (246, GIST); (6) colorectal liver metastases (77, CRLM); and (7) lung metastases of metastatic melanoma (103, Melanoma). Radiomic analysis was performed on 464 (2016) radiomic features for the BraTS'20 (others) datasets respectively. Random forests (RF), Extreme Gradient Boosting (XGBOOST) and a voting algorithm comprising both classifiers were tested. The parameters of the classifiers were optimized using a repeated nested stratified cross-validation process. The feature importance of each classifier was computed using the Gini index or permutation feature importance. DCNN was performed on 2D axial and sagittal slices encompassing the tumor. A balanced database was created, when necessary, using smart slices selection. ResNet50, Xception, EficientNetB0, and EfficientNetB3 were transferred from the ImageNet application to the tumor classification and were fine-tuned. Five-fold stratified cross-validation was performed to evaluate the models. The classification performance of the models was measured using multiple indices including area under the receiver operating characteristic curve (AUC).Results.The best radiomic approach was based on XGBOOST for all datasets; AUC was 0.934 (BraTS'20), 0.86 (LIPO), 0.73 (LIVER), (0.844) Desmoid, 0.76 (GIST), 0.664 (CRLM), and 0.577 (Melanoma) respectively. The best DCNN was based on EfficientNetB0; AUC was 0.99 (BraTS'20), 0.982 (LIPO), 0.977 (LIVER), (0.961) Desmoid, 0.926 (GIST), 0.901 (CRLM), and 0.89 (Melanoma) respectively.Conclusion.Tumor classification can be accurately determined by adapting state-of-the-art machine learning algorithms to the medical context.
An efficient deep neural network for automatic classification of acute intracranial hemorrhages in brain CT scans

Abstract:Background: Recent advancements in deep learning models have demonstrated their potential in the field of medical imaging, achieving remarkable performance surpassing human capabilities in tasks such as classification and segmentation. However, these modern state-of-the-art network architectures often demand substantial computational resources, which limits their practical application in resource-constrained settings. This study aims to propose an efficient diagnostic deep learning model specifically designed for the classification of intracranial hemorrhage in brain CT scans. Method: Our proposed model utilizes a combination of depthwise separable convolutions and a multi-receptive field mechanism to achieve a trade-off between performance and computational efficiency. The model was trained on RSNA datasets and validated on CQ500 dataset and PhysioNet dataset. Result: Through a comprehensive comparison with state-of-the-art models, our model achieves an average AUROC score of 0.952 on RSNA datasets and exhibits robust generalization capabilities, comparable to SE-ResNeXt, across other open datasets. Furthermore, the parameter count of our model is just 3 % of that of MobileNet V3. Conclusion: This study presents a diagnostic deep-learning model that is optimized for classifying intracranial hemorrhages in brain CT scans. The efficient characteristics make our proposed model highly promising for broader applications in medical settings.
An interpretable artificial intelligence model based on CT for prognosis of intracerebral hemorrhage: a multicenter study

Abstract:Objectives: To develop and validate a novel interpretable artificial intelligence (AI) model that integrates radiomic features, deep learning features, and imaging features at multiple semantic levels to predict the prognosis of intracerebral hemorrhage (ICH) patients at 6 months post-onset. Materials and methods: Retrospectively enrolled 222 patients with ICH for Non-contrast Computed Tomography (NCCT) images and clinical data, who were divided into a training cohort (n = 186, medical center 1) and an external testing cohort (n = 36, medical center 2). Following image preprocessing, the entire hematoma region was segmented by two radiologists as the volume of interest (VOI). Pyradiomics algorithm library was utilized to extract 1762 radiomics features, while a deep convolutional neural network (EfficientnetV2-L) was employed to extract 1000 deep learning features. Additionally, radiologists evaluated imaging features. Based on the three different modalities of features mentioned above, the Random Forest (RF) model was trained, resulting in three models (Radiomics Model, Radiomics-Clinical Model, and DL-Radiomics-Clinical Model). The performance and clinical utility of the models were assessed using the Area Under the Receiver Operating Characteristic Curve (AUC), calibration curve, and Decision Curve Analysis (DCA), with AUC compared using the DeLong test. Furthermore, this study employs three methods, Shapley Additive Explanations (SHAP), Grad-CAM, and Guided Grad-CAM, to conduct a multidimensional interpretability analysis of model decisions. Results: The Radiomics-Clinical Model and DL-Radiomics-Clinical Model exhibited relatively good predictive performance, with an AUC of 0.86 [95% Confidence Intervals (CI): 0.71, 0.95; P < 0.01] and 0.89 (95% CI: 0.74, 0.97; P < 0.01), respectively, in the external testing cohort. Conclusion: The multimodal explainable AI model proposed in this study can accurately predict the prognosis of ICH. Interpretability methods such as SHAP, Grad-CAM, and Guided Grad-Cam partially address the interpretability limitations of AI models. Integrating multimodal imaging features can effectively improve the performance of the model. Clinical relevance statement: Predicting the prognosis of patients with ICH is a key objective in emergency care. Accurate and efficient prognostic tools can effectively prevent, manage, and monitor adverse events in ICH patients, maximizing treatment outcomes.
Deep learning and machine learning predictive models for neurological function after interventional embolization of intracranial aneurysms

Abstract:Objective: The objective of this study is to develop a model to predicts the postoperative Hunt-Hess grade in patients with intracranial aneurysms by integrating radiomics and deep learning technologies, using preoperative CTA imaging data. Thereby assisting clinical decision-making and improving the assessment and prognosis of postoperative neurological function. Methods: This retrospective study encompassed 101 patients who underwent aneurysm embolization surgery. 851 radiomic features were extracted from CTA images. 512 deep learning features are extracted from last layer of ResNet50 deep convolutional neural network model. The feature screening process pipeline encompassed intraclass correlation coefficient analysis, principal component analysis, U test, spearman correlation analysis, minimum redundancy maximum relevance algorithm and Lasso regression, to identify features most correlated with postoperative Hunt-Hess grading. In the model construction phase, three distinct models were constructed: radiomics feature-based model (RSM), deep learning feature-based model (DLM), and deep learning-radiomics feature fusion model (DLRSCM). The study also calculated the radiomics score and combined it with clinical data to construct a Nomogram for predictive modeling. DLM, RSM and DLRSCM model was constructed by 9 base algorithms and 1 ensemble learning algorithm – Stacking ensemble model. Model performance was evaluated based on the area under the Receiver Operating Characteristic (ROC) curve (AUC), Matthews Correlation Coefficient (MCC), calibration curves, and decision curves analysis. Results: 5 significant radiomic feature and 4 significant deep learning features were obtained through the feature selection process. These features were utilized for model construction. Bootstrap resampling method was used for internal validation of the models. In terms of model evaluation, the DLM model, the stacking ensemble algorithm results achieved an AUC of 0.959 and MCC of 0.815. In the RSM model, the stacking ensemble model AUC was 0.935 and MCC was 0.793. The stacking ensemble model in DLRSCM outperformed others, with an AUC of 0.968 and MCC of 0.820. Results indicated that the ANN performed optimally among all base models, while the stacked ensemble learning model exhibited the highest predictive performance. Conclusion: This study demonstrates that the combination of radiomics and deep learning is an effective approach to predict the postoperative Hunt-Hess grade in patients with intracranial aneurysms. This holds significant value in the early identification of postoperative neurological complications and in enhancing clinical decision-making.

Abstract:Background and purpose: Delayed cerebral ischemia is hard to diagnose early due to gradual, symptomless development. This study aimed to develop an automated model for predicting delayed cerebral ischemia following aneurysmal SAH on NCCT. Materials and methods: This retrospective study included 400 patients with aneurysmal SAH (156 with delayed cerebral ischemia) who underwent NCCT. The study used ATT-Deeplabv3+ for automatically segmenting hemorrhagic regions using semisupervised learning. Principal component analysis was used for reducing the dimensionality of deep learning features extracted from the average pooling layer of ATT-DeepLabv3+. The classification model integrated clinical data, radiomics, and deep learning features to predict delayed cerebral ischemia. Feature selection involved Pearson correlation coefficients, least absolute shrinkage, and selection operator regression. We developed models based on clinical features, clinical-radiomics, and a combination of clinical, radiomics, and deep learning. The study selected logistic regression, Naive Bayes, Adaptive Boosting (AdaBoost), and multilayer perceptron as classifiers. The performance of segmentation and classification models was evaluated on their testing sets using the Dice similarity coefficient for segmentation, and the area under the receiver operating characteristic curve (AUC) and calibration curves for classification. Results: The segmentation process achieved a Dice similarity coefficient of 0.91 and the average time of 0.037 s/image. Seventeen features were selected to calculate the radiomics score. The clinical-radiomics-deep learning model with multilayer perceptron achieved the highest AUC of 0.84 (95% CI, 0.72-0.97), which outperformed the clinical-radiomics model (P = .002) and the clinical features model (P = .001) with multilayer perceptron. The performance of clinical-radiomics-deep learning model using AdaBoost was significantly superior to its clinical-radiomics model (P = .027). The performance of the clinical-radiomics-deep learning model and the clinical-radiomics model with logistic regression notably exceeded that of the model based solely on clinical features (P = .028; P = .046). The AUC of the clinical-radiomics-deep learning model with multilayer perceptron (P < .001) and the clinical-radiomics model with logistic regression (P = .046) were significantly higher than the clinical model with logistic regression. Of all models, the clinical-radiomics-deep learning model with multilayer perceptron showed best calibration. Conclusions: The proposed 2-stage end-to-end model not only achieves rapid and accurate segmentation but also demonstrates superior diagnostic performance with high AUC values and good calibration in the clinical-radiomics-deep learning model, suggesting its potential to enhance delayed cerebral ischemia detection and treatment strategies.
Predicting Outcome of Patients With Cerebral Hemorrhage Using a Computed Tomography–Based Interpretable Radiomics Model: A Multicenter Study

Abstract:Objective The aim of this study was to develop and validate an interpretable and highly generalizable multimodal radiomics model for predicting the prognosis of patients with cerebral hemorrhage. Methods This retrospective study involved 237 patients with cerebral hemorrhage from 3 medical centers, of which a training cohort of 186 patients (medical center 1) was selected and 51 patients from medical center 2 and medical center 3 were used as an external testing cohort. A total of 1762 radiomics features were extracted from nonenhanced computed tomography using Pyradiomics, and the relevant macroscopic imaging features and clinical factors were evaluated by 2 experienced radiologists. A radiomics model was established based on radiomics features using the random forest algorithm, and a radiomics-clinical model was further trained by combining radiomics features, clinical factors, and macroscopic imaging features. The performance of the models was evaluated using area under the curve (AUC), sensitivity, specificity, and calibration curves. Additionally, a novel SHAP (SHAPley Additive exPlanations) method was used to provide quantitative interpretability analysis for the optimal model. Results The radiomics-clinical model demonstrated superior predictive performance overall, with an AUC of 0.88 (95% confidence interval, 0.76–0.95; P < 0.01). Compared with the radiomics model (AUC, 0.85; 95% confidence interval, 0.72–0.94; P < 0.01), there was a 0.03 improvement in AUC. Furthermore, SHAP analysis revealed that the fusion features, rad score and clinical rad score, made significant contributions to the model's decision-making process. Conclusion Both proposed prognostic models for cerebral hemorrhage demonstrated high predictive levels, and the addition of macroscopic imaging features effectively improved the prognostic ability of the radiomics-clinical model. The radiomics-clinical model provides a higher level of predictive performance and model decision-making basis for the risk prognosis of cerebral hemorrhage.

Differentiation of Brain Abscess From Cystic Glioma Using Conventional MRI Based on Deep Transfer Learning Features and Hand-Crafted Radiomics Features

Abstract:Objectives: To develop and validate the model for distinguishing brain abscess from cystic glioma by combining deep transfer learning (DTL) features and hand-crafted radiomics (HCR) features in conventional T1-weighted imaging (T1WI) and T2-weighted imaging (T2WI).Methods: This single-center retrospective analysis involved 188 patients with pathologically proven brain abscess (102) or cystic glioma (86). One thousand DTL and 105 HCR features were extracted from the T1WI and T2WI of the patients. Three feature selection methods and four classifiers, such as k-nearest neighbors (KNN), random forest classifier (RFC), logistic regression (LR), and support vector machine (SVM), for distinguishing brain abscess from cystic glioma were compared. The best feature combination and classifier were chosen according to the quantitative metrics including area under the curve (AUC), Youden Index, and accuracy.Results: In most cases, deep learning-based radiomics (DLR) features, i.e., DTL features combined with HCR features, contributed to a higher accuracy than HCR and DTL features alone for distinguishing brain abscesses from cystic gliomas. The AUC values of the model established, based on the DLR features in T2WI, were 0.86 (95% CI: 0.81, 0.91) in the training cohort and 0.85 (95% CI: 0.75, 0.95) in the test cohort, respectively.Conclusions: The model established with the DLR features can distinguish brain abscess from cystic glioma efficiently, providing a useful, inexpensive, convenient, and non-invasive method for differential diagnosis. This is the first time that conventional MRI radiomics is applied to identify these diseases. Also, the combination of HCR and DTL features can lead to get impressive performance.
Deep radiomic features from MRI scans predict survival outcome of recurrent glioblastoma

Abstract:This paper proposes to use deep radiomic features (DRFs) from a convolutional neural network (CNN) to model fine-grained texture signatures in the radiomic analysis of recurrent glioblastoma (rGBM). We use DRFs to predict survival of rGBM patients with preoperative T1-weighted post-contrast MR images (n=100). DRFs are extracted from regions of interest labelled by a radiation oncologist and used to compare between short-term and long-term survival patient groups. Random forest (RF) classification is employed to predict survival outcome (i.e., short or long survival), as well as to identify highly group-informative descriptors. Classification using DRFs results in an area under the ROC curve (AUC) of 89.15% (p<0.01) in predicting rGBM patient survival, compared to 78.07% (p<0.01) when using standard radiomic features (SRF). These results indicate the potential of DRFs as a prognostic marker for patients with rGBM.

Abstract:Radiomics has shown a capability for different types of cancers such as glioma to predict the clinical outcome. It can have a non-invasive means of evaluating the immunotherapy response prior to treatment. However, the use of deep convolutional neural networks (CNNs)-based radiomics requires large training image sets. To avoid this problem, we investigate a new imaging features that model distribution with a Gaussian mixture model (GMM) of learned 3D CNN features. Using these deep radiomic features (DRFs), we aim to predict the immune marker status (low versus high) and overall survival for glioma patients. We extract the DRFs by aggregating the activation maps of a pre-trained 3D-CNN within labeled tumor regions of MRI scans that corresponded immune markers of 151 patients. Our experiments are performed to assess the relationship between the proposed DRFs, three immune cell markers (Macrophage M1, Neutrophils and T Cells Follicular Helper), and measure their association with overall survival. Using the random forest (RF) model, DRFs was able to predict the immune marker status with area under the ROC curve (AUC) of 78.67, 83.93 and 75.67\% for Macrophage M1, Neutrophils and T Cells Follicular Helper, respectively. Combined the immune markers with DRFs and clinical variables, Kaplan-Meier estimator and Log-rank test achieved the most significant difference between predicted groups of patients (short-term versus long-term survival) with p\,=\,4.31$\times$10$^{-7}$ compared to p\,=\,0.03 for Immune cell markers, p\,=\,0.07 for clinical variables , and p\,=\,1.45$\times$10$^{-5}$ for DRFs. Our findings indicate that the proposed features (DRFs) used in RF models may significantly consider prognosticating patients with brain tumour prior to surgery through regularly acquired imaging data.

Abstract:Abstract Pachybrachis sassii, new species is described from Giglio Island, of the Tuscan Archipelago (Italy). The new species belongs to the nominotypical subgenus and is closely related to Pachybrachis salfii Burlini, 1957, from which it differs in the shape of the median lobe of the aedeagus and in the pattern of the yellow raised spots on the elytra and pronotum. Ecological observations are made. The neotype of Pachybrachis salfii from Colloreto, Monte Pollino (Italy) is designated.

Abstract:Background: Deep learning techniques have achieved high accuracy in image classification tasks, and there is interest in applicability to neuroimaging critical findings. This study evaluates the efficacy of 2D deep convolutional neural networks (DCNNs) for detecting basal ganglia (BG) hemorrhage on noncontrast head CT. Materials and Methods: 170 unique de-identified HIPAA-compliant noncontrast head CTs were obtained, those with and without BG hemorrhage. 110 cases were held-out for test, and 60 were split into training (45) and validation (15), consisting of 20 right, 20 left, and 20 no BG hemorrhage. Data augmentation was performed to increase size and variation of the training dataset by 48-fold. Two DCNNs were used to classify the images-AlexNet and GoogLeNet-using untrained networks and those pre-trained on ImageNet. Area under the curves (AUC) for the receiver-operator characteristic (ROC) curves were calculated, using the DeLong method for statistical comparison of ROCs. Results: The best performing model was the pre-trained augmented GoogLeNet, which had an AUC of 1.00 in classification of hemorrhage. Preprocessing augmentation increased accuracy for all networks (p<0.001), and pretrained networks outperformed untrained ones (p<0.001) for the unaugmented models. The best performing GoogLeNet model (AUC 1.00) outperformed the best performing AlexNet model (AUC 0.95)(p=0.01). Conclusion: For this dataset, the best performing DCNN identified BG hemorrhage on noncontrast head CT with an AUC of 1.00. Pretrained networks and data augmentation increased classifier accuracy. Future prospective research would be important to determine if the accuracy can be maintained on a larger cohort of patients and for very small hemorrhages.

Abstract:Brain tumor diagnosis using MRI scans poses significant challenges due to the complex nature of tumor appearances and variations. Traditional methods often require extensive manual intervention and are prone to human error, leading to misdiagnosis and delayed treatment. Current approaches primarily include manual examination by radiologists and conventional machine learning techniques. These methods rely heavily on feature extraction and classification algorithms, which may not capture the intricate patterns present in brain MRI images. Conventional techniques often suffer from limited accuracy and generalizability, mainly due to the high variability in tumor appearance and the subjective nature of manual interpretation. Additionally, traditional machine learning models may struggle with the high-dimensional data inherent in MRI images. To address these limitations, our research introduces a deep learning-based model utilizing convolutional neural networks (CNNs).Our model employs a sequential CNN architecture with multiple convolutional, max-pooling, and dropout layers, followed by dense layers for classification. The proposed model demonstrates a significant improvement in diagnostic accuracy, achieving an overall accuracy of 98% on the test dataset. The precision, recall, and F1-scores ranging from 97 to 98% with a roc-auc ranging from 99 to 100% for each tumor category further substantiate the model's effectiveness. Additionally, the utilization of Grad-CAM visualizations provides insights into the model's decision-making process, enhancing interpretability. This research addresses the pressing need for enhanced diagnostic accuracy in identifying brain tumors through MRI imaging, tackling challenges such as variability in tumor appearance and the need for rapid, reliable diagnostic tools.

Abstract:Abstract Background It has been believed that traditional handcrafted radiomic features extracted from magnetic resonance imaging (MRI) of tumors are normally shallow and low-ordered. Recent advancement in deep learning technology shows that the high-order deep radiomic features extracted automatically from tumor images can capture tumor heterogeneity in a more efficient way. We hypothesize that MRI-based deep radiomic phenotypes have significant associations with molecular profiles of breast cancer tumors. We aim to identify deep radiomic features (DRFs) from MRI, evaluate their significance in predicting breast cancer (BC) clinical characteristics and explore their associations with multi-level genomic factors. Methods A denoising autoencoder was built to retrospectively extract 4,096 DRFs from 110 BC patients’ MRI. Visualization and clustering were applied to these DRFs. Linear Mixed Effect models were used to test their associations with multi-level genomic features (GFs) (risk genes, gene signatures, and biological pathway activities) extracted from the same patients’ mRNA expression profile. A Least Absolute Shrinkage and Selection Operator model was used to identify the most predictive DRFs for each clinical characteristic (tumor size (T), lymph node metastasis (N), estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) status). Results Thirty-six conventional radiomic features (CRFs) for 87 of the 110 BC patients provided by a previous study were used for comparison. More than 1,000 DRFs were associated with the risk genes, gene signatures, and biological pathways activities (adjusted P -value 0.9) using DRFs. These DRFs showed significant powers of stratifying patients, linking to relevant biological and clinical characteristics. As a contrast, only eight risk genes were associated with CRFs. The RFs performed worse in predicting clinical characteristics than DRFs. Conclusions The deep learning-based auto MRI features perform better in predicting BC clinical characteristics, which are more significantly associated with GFs than traditional semi-auto MRI features. Our radiogenomic approach for identifying MRI-based imaging signatures may pave potential pathways for the discovery of genetic mechanisms regulating specific tumor phenotypes and may enable a more rapid innovation of novel imaging modalities, hence accelerating their translation to personalized medicine.

Abstract:This study aims at analyzing the separability of acute cerebral infarction lesions which were invisible in CT. 38 patients, who were diagnosed with acute cerebral infarction and performed both CT and MRI, and 18 patients, who had no positive finding in either CT or MRI, were enrolled. Comparative studies were performed on lesion and symmetrical regions, normal brain and symmetrical regions, lesion, and normal brain regions. MRI was reconstructed and affine transformed to obtain accurate lesion position of CT. Radiomic features and information gain were introduced to capture efficient features. Finally, 10 classifiers were established with selected features to evaluate the effectiveness of analysis. 1301 radiomic features were extracted from candidate regions after registration. For lesion and their symmetrical regions, there were 280 features with information gain greater than 0.1 and 2 features with information gain greater than 0.3. The average classification accuracy was 0.6467, and the best classification accuracy was 0.7748. For normal brain and their symmetrical regions, there were 176 features with information gain greater than 0.1, 1 feature with information gain greater than 0.2. The average classification accuracy was 0.5414, and the best classification accuracy was 0.6782. For normal brain and lesions, there were 501 features with information gain greater than 0.1 and 1 feature with information gain greater than 0.5. The average classification accuracy was 0.7480, and the best classification accuracy was 0.8694. In conclusion, the study captured significant features correlated with acute cerebral infarction and confirmed the separability of acute lesions in CT, which established foundation for further artificial intelligence-assisted CT diagnosis.

Abstract:Objectives: Alzheimer's disease (AD) is a common neurodegenerative disorder that primarily affects older individuals. Due to its high incidence, an accurate and efficient stratification system could greatly aid in the clinical diagnosis and prognosis of AD patients. Convolutional neural networks (CNN) approaches have demonstrated exceptional performance in the automated stratification of AD, mild cognitive impairment (MCI) and cognitively normal (CN) participants using MRI, owing to their high predictive accuracy and reliability. Therefore, we aimed to develop an algorithm based on CNN and radiomic features derived from ROIs of bilateral hippocampus and amygdala in brain MRI for stratification between AD, MCI and CN. Methods: In this study, we proposed a CNN and radiomic features-based algorithm using the Alzheimer's Disease Neuroimaging Initiative (ADNI) database. T1-weighted images were used. We utilized three datasets, including AD (199 cases, 602 images), MCI (200 cases, 948 images), and CN (200 cases, 853 images), to perform binary classification (AD vs. CN, AD vs. MCI, and MCI vs. CN). Finally, we obtained the accuracy (ACC) and the area under the curve of the receiver operating characteristic curve (AUC) to evaluate the performance of the algorithm. Results: Our proposed algorithm achieved acceptable overall discrimination accuracy. In the term of AD vs CN, radiomic-based algorithm alone obtained ACC of 82.6 % and AUC of 88.8, CNN-based algorithm obtained ACC of 80 % and AUC of 87.2 and their fusion showed ACC of 84.4 % and AUC of 90. In the term of MCI vs CN, radiomic-based algorithm alone obtained ACC of 71.6 % and AUC of 77.8, CNN-based algorithm obtained ACC of 69 % and AUC of 75 and their fusion showed ACC of 72.7 % and AUC of 80. In the term of AD vs MCI, radiomic-based algorithm alone obtained ACC of 57 % and AUC of 57.5, CNN-based algorithm obtained ACC of 56.6 % and AUC of 57.7 and their fusion showed ACC of 58 % and AUC of 59.5. Conclusion: In conclusion, it has been determined that hippocampus and amygdala-based stratification using CNN features and radiomic features-based algorithm is a promising method for the classification of AD, MCI, and CN participants. Advances in knowledge: This study proposed an automated procedures based on MRI-derived radiomic features and CNN for classification between AD, MCI and CN.

Abstract:Background: Alzheimer's disease (AD) is the most common form of progressive and irreversible dementia, and accurate diagnosis of AD at its prodromal stage is clinically important. Currently, computer-aided diagnosis of AD and mild cognitive impairment (MCI) using F-18-fluorodeoxy-glucose positron emission tomography (F-18-FDG PET) imaging is usually based on low-level imaging features or deep learning methods, which have difficulties in achieving sufficient classification accuracy or lack clinical significance. This research therefore aimed to implement a new feature extraction method known as radiomics, to improve the classification accuracy and discover high-order features that can reveal pathological information. Methods: In this study, F-18-FDG PET and clinical assessments were collected in a cohort of 422 individuals [including 130 with AD, 130 with MCI, and 162 healthy controls (HCs)] from the Alzheimer's Disease Neuroimaging Initiative (ADNI) and 44 individuals (including 22 with AD, and 22 HCs) from Huashan Hospital, Shanghai, China. First, we performed a group comparison using a two-sample Student's t test to determine the regions of interest (ROIs) based on 30 AD patients and 30 HCs from ADNI cohorts. Second, based on two time scans of 32 HCs from ADNI cohorts, we used Cronbach's alpha coefficient for radiomic feature stability analyses. Pearson's correlation coefficients were regarded as a feature selection criterion, to select effective features associated with the clinical cognitive scale [clinical dementia rating scale in its sum of boxes (CDRSB); Alzheimer's disease assessment scale (ADAS)] with 500-times cross-validation. Finally, a support vector machine (SVM) was used to test the ability of the radiomic features to classify HCs, MCI and AD patients. Results: As a result, we identified brain regions which were mainly distributed in the temporal, occipital and frontal areas as ROIs. A total of 168 radiomic features of AD were stable (alpha > 0.8). The classification experiment led to maximal accuracies of 91.5%, 83.1% and 85.9% for classifying AD versus HC, MCI versus HCs and AD versus MCI. Conclusion: The research in this paper proved that the novel approach based on high-order radiomic features extracted from F-18-FDG PET brain images that can be used for AD and MCI computer-aided diagnosis.
