Abstract:People with Alzheimer's disease (AD) diagnosis who get informal care remain at home longer, reducing the demand for healthcare resources but increasing the stress of caregiving. Research on the effectiveness of physical training, psychoeducational, cognitive-behavioural, and health education programs in reducing the caregiver load and enhancing health-related quality of life (HRQoL) exist, but none exist about an integrated interdisciplinary program. The goals of this project are (1) to assess the Integral-CARE Interdisciplinary Program (IP) applicability, safety, effects on HRQoL, and the incremental cost-effectiveness ratio for AD caregivers; (2) to evaluate the IP applicability and cost-effectiveness to enhance the physical, psychoemotional, cognitive-behavioural dimensions, and the health education status of informal caregivers, and (3) to study the transference of the results to the public and private sectors. A randomized controlled trial will be conducted with an experimental (IP) and a control group (no intervention). The PI will be conducted over nine months using face-to-face sessions (twice a week) and virtual sessions on an online platform (once a week). There will be an initial, interim (every three months), and final assessment. Focus groups with social and health agents will be organized to determine the most important information to convey to the public and private sectors in Extremadura (Spain). Applicability, safety, HRQoL, incremental cost-effectiveness ratio, and HRQoL will be the main outcome measures, while secondary measures will include sociodemographic data; physical, psychoemotional, health education, and cognitive-behavioural domains; program adherence; and patient health status. Data will be examined per procedure and intention to treat. A cost-effectiveness study will also be performed from the viewpoints of private and public healthcare resources.
Surviving and Thriving: Qualitative Results from a Multi-Year, Multidimensional Intervention to Promote Well-Being among Caregivers of Adults with Dementia

Abstract:(1) Introduction: Caring for an adult with dementia is both challenging and rewarding. Research indicates that community-based, social support, and/or arts engagement interventions can play a key role in ameliorating the negative outcomes associated with caregiving while enhancing its more positive attributes. This study explores the psychosocial outcomes experienced by dementia caregivers who participated in a multi-year, multidimensional intervention aimed at promoting caregiver and care recipient well-being. This intervention included bringing caregivers and people with Alzheimer’s disease or related dementias (ADRD) to local symphony performances, hosting a social reception prior to the performance, and assessing the outcomes of participation for both caregiver and the care recipient. (2) Materials, Methods, and Analysis: Qualitative data from participant phone interviews (n = 55) as well as focus groups are analyzed using thematic analysis from a phenomenological perspective. (3) Results: Across three years of participation, caregivers reported three main program benefits: relationship building (both with other participants as well as within the broader community); restored humanity (experiencing a greater sense of personal dignity and momentary return to normalcy), and positivity (experiencing positive emotions during the program). (4) Discussion: These findings point to the value of creating caregiver programming that brings together multiple dimensions of successful interventions in order to enhance caregiver experiences and positive intervention outcomes.

Studies on tissue distribution of M1 sperm antigen using sperm-specific monoclonal antibody

Abstract:It was reported that M1 monoclonal antibody (M1 mab) inhibited hamster sperm-egg fusion in a dose-dependent manner. The antigen was localized to the equatorial segment (ES) of the hamster sperm, and that after the acrosome reaction, it was exposed on the surface of the sperm plasma membrane overlying the ES. The M1 antigen therefore fulfilled the main requirements of a sperm component that might be involved in gamete fusion processes. Hence, it was necessary to fully characterized this sperm determinant in order to elucidate its role in fertilization and to assess its potential as contraceptive antigen. In this study, M1 mab was used to analyze the developmental expression of M1 antigen during spermatogenesis and during sperm maturation in the epididymal tract. Indirect immunofluorescent staining (IIF) assay revealed that the M1 antigen was detected only in the cross sections of seminiferous tubules containing spermatids and sperm. The specificity of M1 mab was examined also by IIF assay of cell suspensions prepared from various tissues of the hamster. The result showed that the M1 mab did not cross react with cells isolated from somatic tissues including lung, heart, kidney, spleen, brain and liver. Further detection by im-munoblotting also showed that the M1 antigen appeared to be sperm-specific as it was not detected in a range of somatic cells. It is therefore proposed that the M1 an-tigen which is synthesized only by the germ cells of the testis, also found to be ex-pressed at late spermatid stage and is a sperm-specific protein.
Role of Overload and Psychoemotional Variables on Health-Related Quality of Life in Informal Caregivers of People with Alzheimer's Disease

Abstract:Background: Carers of people with Alzheimer's disease often have a high degree of commitment and dedication which may also compromise physical and emotional, leisure, and occupational self-care. This study aimed to explore health-related quality of life (HRQoL) and psychoemotional variables in caregivers with and without caregiver overload and its relationship. Methods: A single-measure cross-sectional correlational study was carried out involving 59 informal caregivers of people with Alzheimer's disease with a mean age of 59.30 (±10.58). The participants completed the adult HRQoL questionnaires (EQ-5D-3L), Zarit Burden Inventory test, General Happiness Questionnaire, Satisfaction with Life Scale, Rosenberg self-esteem scale, Occupational Balance Questionnaire (OBQ-E), International Fitness Scale (IFIS), Family Apgar scale, and Duke-UNC-11 Functional Social Support Questionnaire. Results: A significantly higher level of HRQoL (p = 0.029) in subjective happiness (p = 0.018), perceived social support (p = 0.046), avoidance (p = 0.034), occupational balance (p = 0.002), life satisfaction (p = 0.037), and self-perceived physical fitness (p = 0.021) was found in caregivers without perceived overload. Also, HRQoL was directly associated with self-perceived physical fitness (β = 0.534; p < 0.001) and occupational balance (β = 0.375; p < 0.001) and self-esteem (β = 0.249; p < 0.016). Conclusions: Caregivers who do not perceive overload have better levels of HRQoL and psychoemotional variables, establishing a relationship between HRQoL with self-perceived physical fitness, occupational balance, and self-esteem.

Abstract:There is a general consensus that providing care for a relative that has dementia is associated with negative outcomes for caregivers' mental health. There seem to be also associations with negative physical health outcomes, although the literature on this topic is more scarce. Most of the available research consist in studies with a cross-sectional design. Longitudinal studies focused on psychosocial correlates of mental and physical health of the caregivers are lacking, mostly those analyzing the influence of factors such as behavioral and psychological symptoms of the dementia, caregivers` dysfunctional thoughts, and caregivers' ambivalent and guilt feelings. The aim of this presentation will be to describe the findings from the Madrid Caregivers Longitudinal Study, that consist in a two year follow-up of family caregivers of people with dementia. Several models will be described testing the longitudinal effect of psychosocial variables on caregivers' distress (depression and anxiety) and cardiovascular health (measured through biomarkers of inflammation). The practical implications of the findings will be discussed.

Acculturation and the lateralization of speech in the bilingual Native American.

Abstract:Previous reports have suggested that bilingual Native Americans differentiate cognitively in processing their sociologically dominant and non-dominant language. Specifically, there is some evidence that receptive speech functions may be lateralized to the right cerebral hemisphere. To test the notion that this differential specialization may be due to socio-cultural influences, receptive and expressive speech lateralization was examined in a population of acculturated bilingual Native Americans. An analysis of dichotic listening and time-sharing asymmetries indicated that despite evidence for an initial appositional tendency, the Native American subjects were lateralized to the left cerebral hemisphere for both receptive and expressive speech. It is proposed that shifts in the attended perceptual field occur as Native Americans mature perhaps due to an increasing familiarity with a second culture and more propositionally oriented language.

Abstract:Aims: We conducted a multi-sectoral comprehensive intervention for elderly Alzheimer Disease patients and their caregivers to improve their quality of life. Design: Randomized controlled trial. Methods: From June to June in 2019, 150 Alzheimer Disease patients were randomly selected from a nursing home in Zhejiang province of China, they were randomly divided into the intervention group and the control group. And they were given comprehensive and routine interventions respectively for six months. We used mixed models in our analyses to see how outcomes changed over time and how they were affected by the intervention, which could also solve the problems of missing values and data correlation. Results: After six months, compared with the control group, the communication ability, housework handling ability, self-care ability and life quality of the patients in the intervention group were improved, and the quality of life and caring ability of their caregivers were also significantly improved. Conclusion: The multi-sectoral collaborative care model proposed in this study integrated resources from several departments, effectively improved the quality of life of patients and their caregivers, and provided a way of care services for patients with Alzheimer Disease. Impact: Multi-sectoral comprehensive intervention would improve the life quality of elderly patients with Alzheimer Disease and their caregivers.
Effectiveness of Interventions for Caregivers of People With Alzheimer's Disease and Related Major Neurocognitive Disorders: A Systematic Review

Abstract:Objective: The goal of the evidence review was to evaluate the effectiveness of interventions for caregivers of people with Alzheimer's disease and related major neurocognitive disorders that facilitate the ability to maintain participation in the caregiver role. Method: Scientific literature published in English between January 2006 and April 2014 was reviewed. Databases included MEDLINE, PsycINFO, CINAHL, OTseeker, and the Cochrane Database of Systematic Reviews. Results: Of 2,476 records screened, 43 studies met inclusion criteria. Strong evidence shows that multicomponent psychoeducational interventions improve caregiver quality of life (QOL), confidence, and self-efficacy and reduce burden; cognitive reframing reduces caregiver anxiety, depression, and stress; communication skills training improves caregiver skill and QOL in persons with dementia; mindfulness-based training improves caregiver mental health and reduces stress and burden; and professionally led support groups enhance caregiver QOL. Conclusion: Strong evidence exists for a spectrum of caregiver interventions. Translation of effective interventions into practice and evaluation of sustainability is necessary.

Abstract:Background: Most people living with dementia rely on their family members for the care and support they need. However, unpaid care can lead to multiple negative impacts on the wellbeing and mental health of carers. Objectives: The Objectives of this study was to evaluate the effects of iSupport-Brasil (an online, psychoeducational program) on perceptions of burden and on the mental health of informal caregivers of people living with dementia. Methods: A randomized controlled trial was carried out from January 2023 to April 2024 in Brazil. A total of 162 individuals were randomly allocated to an intervention group (IG) (n = 77) or a control group (CG) (n = 85). The IG had free access to the iSupport-Brasil platform for three months. Simultaneously, the CG participants were provided with a link to access the Brazilian Alzheimer's Association website and a copy of the "Care Guide for Older People", published by the Brazilian Ministry of Health. Pre- and post-intervention assessments were carried out using measures of burden (a single-item Burden Scale and the Zarit Burden Interview – ZBI), and of anxiety and depressive symptoms (Hospital Anxiety and Depression Scale - HADS). Results: Based on an intention-to-treat analysis, a reduction in anxiety symptoms was observed for the IG (p = 0.02). Based on a per-protocol analysis, significant reductions in burden (p <0.01) and anxiety symptoms (p < 0.01) were found for the IG. No significant effects were found for depressive symptoms, although scores decreased. For the CG, the regression model indicated that, on average, burden and depressive symptoms worsened somewhat, but this result was not statistically significant. Conclusions: The iSupport-Brasil program was effective in reducing perceptions of burden and anxiety symptoms among informal caregivers in Brazil. Therefore, we recommended that informal caregivers of people living with dementia use this program to help them broaden their interpersonal coping strategies and improve their emotional health, in addition to using mainstream long-term care support services.

Abstract:A facile sonochemical approach was used to deposit 3-5 nm monodisperse gold nanoparticles on porous SiO2-WO3 composite spheres, as confirmed by powder X-ray diffraction (XRD) and transmission electron microscopy (TEM). High-resolution TEM (HR-TEM) and energy dispersive X-ray spectroscopy (EDS) further characterized the supported Au nanoparticles within the Au-SiO2-WO3 composite. These analyses showed isolated Au nanoparticles within both SiO2- and WO3-containing regions. Selective etching of the SiO2 matrix from Au-SiO2-WO3 yielded a pure Au-WO3 material with well-dispersed 10 nm Au nanoparticles and moderate porosity. This combined sonochemical-nanocasting technique has not been previously used to synthesize Au-WO3 photocatalysts. Methylene blue (MB) served as a probe for the adsorption capacity and visible light photocatalytic activity of these WO3-containing catalysts. Extensive MB demethylation (azures A, B, C, and thionine) and polymerization of these products occurred over WO3 under dark conditions, as confirmed by electrospray ionization mass spectrometry (ESI-MS). Photoirradiation of these suspensions led to further degradation primarily through demethylation and polymerization pathways, regardless of the presence of Au nanoparticles. Ring-opening sulfur oxidation to the sulfone was a secondary photocatalytic pathway. According to UV-vis spectroscopy, pure WO3 materials showed superior MB adsorption compared to SiO2-WO3 composites. Compared to their respective nonloaded catalysts, Au-SiO2-WO3 and Au-WO3 catalysts exhibited enhanced visible light photocatalytic activity toward the degradation of MB. Specifically, the rates of MB degradation over Au-WO3 and Au-SiO2-WO3 during 300 min of irradiation were faster than those over their nonloaded counterparts (WO3 and SiO2-WO3). These studies highlight the ability of Au-WO3 to serve as an excellent adsorbant and photodegradation catalyst toward MB.

Abstract:Objectives: Caregiving can be a daunting and isolating experience, especially when supporting a loved one with Dementia The objective of this intervention was to provide a series of educational sessions available to caregivers to help educate them on strategies to enhance their well-being and interactions with a loved one living with dementia. Methods: The Caregiver Literacy Series is a compilation o 18 webinar sessions with therapeutic workbooks designed to help caregivers understand the nature of specific dementia-related issues, and designed to help the caregiver build some personal strategy to help better manage their caregiving role. Based upon a Framework that uses the Perceived Self-Efficacy Theory, each webinar provides educational materials, and resources and is designed to help build an action plan for the caregiver. Topics include "What is Caregiving?", "Caregiving and Compassion Fatigue and Self-Care" and topics address coping and communication strategies. The webinar sessions were administered monthly and semi-monthly to consumers through a local Alzheimer's Association network in the rural Midwest of the United States. Results: Feedback from consumers who have used the materials suggest that the materials have provided some measure of information and helpful educational materials. The workbooks have also been an effective tool to help guide and empower the caregiver. Conclusion: The Caregiver Literacy Series provides some effective and needed materials to help equip caregivers living with a loved one that has Dementia or Alzheimer's disease with some measure of health literacy and empowers them to feel some sense of empowerment and comfort in the process.

Abstract:Geographical variations in the HLA-DQ genotypes associated with risk for type 1 diabetes were evaluated in Finland. Samples of 280 diabetic children diagnosed in Turku (south-west of the country) and 405 in Oulu (north of the country) were studied as well as a series of 14 096 and 10 016 newborns collected from the same hospitals. There were no major differences in the risk or protection conferred by various HLA-DQB1 genotypes between south-western and northern parts of the country when genotypes of children with type 1 diabetes from these two centres were compared with those of newborns, representing the background populations. However, the distribution of various genotypes was different, both in diabetic children and in newborns, when compared between the two regions (P < 0.0001, chi2 test). These differences reflected the allele frequencies in newborn cohorts in which HLA-DQB1*02 and DQB1*0301 were found more often in Turku and DQB1*0302 more often in Oulu (P < 0.0001 for all differences). Similar types of differences were detected when children who were diagnosed as having diabetes during the national 'Childhood Diabetes in Finland' (DiMe) study between the years 1986-1989 were compared according to their residence. The observed differences in genotype and allele frequencies demonstrate the heterogeneity for HLA alleles even in a population that is generally regarded as highly homogeneous. These differences also affect the sensitivity and efficiency of the screening programme used for identifying infants with genetic susceptibility to IDDM in the ongoing Finnish Diabetes Prediction and Prevention Study.
Impact of aldehyde content on amphotericin B-dextran imine conjugate toxicity.

Abstract:The biocompatibility of oxidized dextran (40 kDa) was investigated in vitro. The contribution of aldehyde groups to the toxicity of polymer-drug conjugates, such as dextran-amphotericin B (AmB) was evaluated. Oxidized dextran was proved to be toxic against the RAW 264.7 cell line with an IC50 of 3 micromol/mL aldehydes. Modification of aldehyde groups and their reaction with ethanolamine reduced the toxicity at least 15-fold. Accordingly, the antifungal and antileishmanial dextran-AmB imine conjugate, which contains unreacted aldehyde groups, was modified with ethanolamine and compared to dextran-AmB amine and imine conjugates. Modification of the imine conjugate with ethanolamine reduced its toxicity toward the RAW cell line by 100%. The effect on Leishmania major parasites was 5 times higher than that of the dextran-AmB amine conjugate. The dextran-AmB-ethanolamine conjugate was at least 15 times less hemolytic than free AmB. Stability and drug release profiles in buffer solution were investigated. The imine conjugates released free AmB while the amine conjugate did not. It is concluded that aldehyde groups may contribute to cell toxicity. This toxicity is reduced by converting the aldehyde groups into imine conjugates with ethanolamine. The results have direct implications toward the safety of AmB-polysaccharide conjugates used against fungal and leishmanial infections.
Affective Impact on Informal Caregivers over 70 Years of Age: A Qualitative Study

Abstract:Given today's rapidly ageing society, family members providing informal care to dependent older adults face ever-increasing challenges. The aim of this study was to describe the affective impact on older adults over 70 years of age caring for a dependent older person at home. A qualitative study was designed from a phenomenological perspective. Thirteen in-depth interviews were conducted with caregivers aged 70 or older. A content analysis of the interviews was carried out in five stages. Three themes were identified: "Emotions", "Feelings", and "Looking to the future". Caregivers express negative emotions (sadness, anger, and fear) and feelings of social and emotional isolation, and they feel abandoned by health professionals, family, and friends. In conclusion, prolonged caregiving by older adults has a negative affective impact and creates uncertainty about the future. There is a need to devise social and healthcare policies and actions, creating social support networks to improve their health and emotional wellbeing.

Abstract:Background: Day care programs for older adults with dementia is an effective strategy that allows prolonging the functionality of the elderly, their support and that of their family reducing home care costs and delaying the institutionalization of patients with cognitive impairment. Objectives: The aim of the present work is to describe the main goals and the implementation of a multidisciplinary day care program for patients with dementia and their family caregivers in Argentina. Methods: Design: Prospective cohort study. Subjects: 22 patients and their family caregivers, who attend a day care program in Argentina. Instruments: socio-demographic and psychosocial questionnaire of both patient and caregiver (built ad hoc) Patient: MMSE, GDS, HAD-A, PANAS- VRARG, EQ-5D and instrumental and basic abilities of daily living. Family caregiver: Zarit Burden Interview. Procedure: The intervention for patients consists of two different lines of work, one for the patient, and another for the family member. The activities are coordinated by a multidisciplinary group of professionals. Regarding the caregiver, individual orientation interviews and therapeutic group spaces and psycho-educational material are offered. Results: Preliminary data of 22 patients and caregivers, who have taken part in the program so far, indicate that: Patient: median age was years 75,27 old (ds = 9,28), % 63,6 of which are woman. The values obtain in the instrument are: MMSE 22,45 (ds = 4,78) GDS 4,47 (ds = 3,73), HAD-A 3,66 (ds = 3,39), (PANAS -E-VRARG) Negative items 7,70 (ds = 3,11) (PANAS -E-VRARG) Positive ítems 18,7 (ds = 3,13), EQ-5D 77,35 (ds = 16,21) and BHDL 93,63 (ds = 13,10), IHDL 9,1 (ds = 3,31). And for family caregivers the mean age was 65,27 old (ds = 11,37), % 66,71 of which are woman. Caregivers Zarit Burden Interview mean score was 40, 18 (ds = 11,35). Discussion: Our findings concur with those presented in scientific literature and support the WHO recommendations regarding the need to implement multidisciplinary programs for patients with dementia and family caregivers, in order to moderate the impact that providing care for an elderly relative with a neurological disease has on them.

Abstract:Evidence shows that caring for a family member with a psychotic disorder generates a significant emotional impact and repercussions on social functioning. However, this caregiving experience can be influenced by cultural factors. The aim of this study is to explore the experience and perception of a group of family caregivers of patients with psychotic disorders of different durations regarding the emotional and social functioning repercussions of caregiving on them in Andalusia. Three focus groups (28 participants) were conducted. Data were subject to a thematic analysis. Three themes emerged from the data: 1) Emotional invasion; 2) Disruption in the caregiver's life project; and 3) Changes in the family system. Participants reported experiencing sadness, fear, overwhelm, hopelessness, and uncertainty. Caregivers' accounts reflected an interruption of their daily tasks, as well as the need to reformulate plans or goals outside the family life that require long-term commitment, such as professional development or even their residence. The narratives indicated an impact on all roles within the family system, particularly for the mother. The results are consistent with existing research. Healthcare professionals and programs should take into account the costs associated with caregiving in order to minimize them, given the impact it has on both the patient's and the caregivers' health.

Abstract:Background: There is a lack of evidence regarding the effectiveness of empowerment healthy education for caregivers of Alzheimer's patients. Objective: To explore the effectiveness of the intervention of health education guided by empowerment theory on dementia knowledge, caregiving readiness, positive caregiving emotions, anxiety, and depression in informal Alzheimer's disease caregivers. Design: A single-blinded, randomized controlled trial. Setting: A teaching hospital in Tianjin, China. Participants: Eighty caregivers of Alzheimer's disease patients. Methods: Participants were recruited from the hospital and randomly assigned to either experimental or control group. The experimental group underwent a 12-weeks, one-to-one intervention of six session lasting 45-60 min each. The control group received conventional health education. Outcome measures included dementia knowledge, caregiver readiness (primary outcomes), positive caregiving emotions, anxiety, and depression (secondary outcomes). Results: After 12 weeks, the intervention group exhibited significantly higher levels of dementia knowledge, caregiver readiness, and positive caregiving emotions compared with the control group. Furthermore, levels of hospitalization-related anxiety and depression were lower in the intervention group. All study results of this study showed statistically significant differences (p < 0.05). Discussion: Empowerment theory-based health education appears to be an effective intervention for improving caregiver and readiness to care for caregivers of Alzheimer's disease individuals. The intervention may help reduce caregivers' anxiety and depression levels.
Study of the effects of mindfulness training on quality of life of patients with Alzheimer's disease and their caregivers (Dyad Mindfulness Project)

Abstract:Background: The training of mindfulness enhances the emotive and cognitive equilibrium and well-being in older adults. So some authors hypothesize it is an appropriate approach to cultivating these benefits and a good quality of life in Alzheimer's patients (AD-P) at early stage and in their caregivers (Cg) (Dyad). Aim: Our main hypothesis is that AD-P and Cg who completed the program of Mindfulness-Based Interventions (MBIs) training would experience improvements in well-being and mood and cognitive performance (AD-P: attention and memory). Methods: The perspective randomized controlled will enroll the Dyad of 80 Alzheimer's patients (age ≥ 60) and 80 caregivers (age > 18) who will be randomly assigned to 6 months MBI training in groups and to control group study. All outcome will be analyzed before intervention (baseline), after follow-up 1 (at the end of treatment), after 6 months (follow-up 2) and after 1 year (follow-up 3) from the end of treatment. Discussion: AD-P-expected outcomes: (a) improvement of the quality of life (QoL-AD); (b) decrease of depression (BDI); (c) improvement of spiritual well-being (Facit SWB Index); (d) improvement of attention and good cognitive performance Rey Memory Test (MFIT); (e) decrease of neuropsychiatric symptoms. (NPI)-Cg-expected outcomes: (a) improvement of the quality of life (SF36); (b) decrease of the care burden (CBI); (c) decrease of depression (BDI); (d) improvement of self-awareness (FFMQ); (e) increase in spiritual-well-being (Facit SWB). Conclusions: This study could provide a clues on multidisciplinary interventions as secondary prevention of Alzheimer's dementia and on stress reduction in family caregivers.
