Abstract:Purpose: A novel approach using nano technology enhanced radiation modalities is investigated. The proposed methodology uses antibodies labeled with organically inert metals with a high atomic number. Irradiation using photons with energies in the kilo--electron volt (keV) range show an increase in dose due to a combination of an increase in photo-electric interactions and a pronounced generation of Auger and/or Coster-Kronig (A-CK) electrons. Methods: The dependency of the dose deposition on various factors is investigated using Monte Carlo simulation models. The factors investigated include: agent concentration, spectral dependency looking at mono--energetic sources as well as classical bremsstrahlung sources. The optimization of the energy spectrum is performed in terms of physical dose enhancement as well as the dose deposited by Auger and/or Coster-Kronig electrons and their biological effectiveness. Results: A quasi-linear dependency on concentration and an exponential decrease within the target medium is observed. The maximal dose enhancement is dependent on the position of the target in the beam. Apart from irradiation with low photon energies (10 - 20 keV) there is no added benefit from the increase in generation of Auger electrons. Interestingly, a regular 110kVp bremsstrahlung spectrum shows a comparable enhancement in comparison with the optimized mono--energetic sources. Conclusions: In conclusion we find that the use of nano-particle enhanced shows promise to be implemented quite easily in regular clinic on a physical level due to the advantageous properties in classical beams.

Abstract:We provide an experimentalist's perspective on the present state-of-the-art in the studies of low-energy electron interactions with common radiosensitizers, including compounds used in combined chemo-radiation therapy and their model systems. Low-energy electrons are important secondary species formed during the interaction of ionizing radiation with matter. Their role in the radiation chemistry of living organisms has become an important topic for more than 20 years. With the increasing number of works and reviews in the field, we would like to focus here on a very narrow area of compounds that have been shown to have radio-sensitizing properties on the one hand, and high reactivity towards low-energy electrons on the other hand. Gas phase experiments studying electron attachment to isolated molecules and environmental effects on reaction dynamics are reviewed for modified DNA components, nitroimidazoles, and organometallics. In the end, we provide a perspective on the future directions that may be important for transferring the fundamental knowledge about the processes induced by low-energy electrons into practice in the field of rational design of agents for concomitant chemo-radiation therapy.

Abstract:Abstract Purpose: We present a microscopic mechanism that accounts for the outward burst of ``cold" ion species (IS) in a high-energy particle track due to coupling with ''hot" non-ion species (NIS). IS refers to radiolysis products of ionized molecules, whereas NIS refers to non-ionized excitations of molecules in a medium. The interaction is mediated by a quantized field of acoustic phonons, a channel that allows conversion of thermal energy of NIS to kinetic energy of IS, a flow of heat from the outer to the inner core of the track structure. Methods: We perform Monte Carlo (MC) simulations of ionizing radiation track structures in water to score the spatial coordinates and energy depositions that form IS and NIS at atto-second time scales. We calculate temperature profiles of the tracks with MC track structure simulations and verify the results analytically using the Rutherford scattering formulation. These temperature profiles are then used as boundary conditions in a series of multi-scale atomistic molecular dynamic (MD) simulations that describe the sudden expansion and enhanced diffusive broadening of tracks initiated by the non-equilibrium spectrum of high-energy IS. Results: We demonstrate the coexistence of ``hot" NIS with ``cold" IS in the radiation track structures right after their generation. NIS, concentrated within nano-scales volumes wrapping around IS, are the main source of intensive heat-waves and the outward burst of IS due to femto-second time scale IS-NIS coupling. By comparing the transport of IS coupled to NIS with identical configurations of non-interacting IS in thermal equilibrium at room temperature, we demonstrate that the energy gain of IS due to the surrounding hot nanoscopic volumes of NIS significantly increases their effective diffusion constants. Conclusions: The much higher diffusion constants predicted in the present model suggest higher inter-track chemical reaction rates at FLASH-UHDR, as well as lower intra-track reaction rates.

The effect of fibrin polymerization inhibitors on quantitative measurements of plasma fibrinogen.

Abstract:A number of reports have highlighted discrepancies between the DuPont aca and fibrometer-based methods for quantitating plasma fibrinogen levels. Although many authors have suggested that the presence of fibrin(ogen) degradation products are in some way responsible for the discrepancies, no direct test of this hypothesis has been carried out. In this report, the authors demonstrate that inhibitors of fibrin monomer polymerization have different effects on the aca and fibrometer assays of fibrinogen. Fibrin polymerization inhibitors allow short soluble fibrin polymers to assemble that can scatter light and are therefore detected by the aca method. However, these short polymers do not gel and thus are not detected by the fibrometer. Therefore, the fibrometer method gives lower fibrinogen values than the aca in the presence of fibrin polymerization inhibitors. The authors also assayed plasma from a patient whose fibrinogen did not polymerize normally and therefore was not measurable by the fibrometer assay. The dysfibrinogenemic plasma assayed by the aca method recorded normal fibrinogen levels. In conclusion, the DuPont aca fibrinogen assay is insensitive to alterations of fibrin polymerization whether from inhibitors or a defective fibrinogen molecule.
Research Progress of Radiosensitization Effects of High-Z Metallic Materials in Tumor Radiotherapy

Abstract:Objetcive: To make a comprehensive review on the researches of the radisensitization effects of high-Z metallic materials in tumor radiotherapy,covering the early-stage work,current progress and further problems for future studies.Methods: Literature on the radioenhancing effects at the surface or interface of high-Z materials and the radiosensitization effects of high-Z metallic materials in radiation therapy of tumor,from early-stage work to newly published reseraches,are reviewed in detail,with regard to different types of therapeutic radations and research methods.Results: With different methods like plasmid DNA and cell irradiation,animal tumor treatment and computer-aided simulations,most of recent works concentrate on the radiosensitization effects in traditional X-|? radiotherapy,while some other work is emerging on further experations of the radiosensitization effects for synchrotron radiotherapy and proton/hardron therapy.It is validated by most of the researches that high-Z metallic materials can play an effective role in radiotherapy as a promising kind of radiosensitizer.Conclusions: For a realistic use in clinical radiotrerapy,with the current researches progress,a lot more efforts are still necessary,especially on the mechanism of radiosenstization effects in micro-or nano scale,for further pre-clinical and clinical studies.

Abstract:In this paper we develop a new model for the electron emission of high-Z nanoparticle irradiated by X-rays. This study is motivated by the recent advances about the nanoparticle enhancement of cancer treatment by radiotherapy. Our original approach combines a pure probabilistic analytical model for the photon trajectories inside the nanoparticle and subsequent electron cascade trajectories based here on a Monte-Carlo simulation provided by the Livermore model implemented in Geant4. To compare the nanoparticle and the plane surface electron emissions, we also develop our model for a plane surface. Our model highlights and explains the existence of a nanoparticle optimal radius corresponding to a maximum of nanoparticle electron emission. It allows us to study precisely the nanoparticle photon absorption and electron cascade production depth in the nanoparticle.

Abstract:This work presents a theoretical analysis of electron impact elastic and inelastic processes for oxygen mimic and fluorinated radiosensitizer molecules over a wide energy range, spanning from ionization potential (IP) to 5 keV. The computation to determine elastic ( Q e l ), inelastic cross sections ( Q i n e l ) and total cross sections ( Q T ) is done through the Spherical Complex Optical Potential (SCOP) formalism and ionization cross sections ( Q i o n ) and summed excitation cross sections ( ∑ Q e x c ) are deduced through the Complex Scattering Potential ionization contribution (CSP-ic) formalism. Additionally, a recently developed an alternative approach for complex and large molecules ( 55 ≤ Z ≤ 95 ), namely the Two Parameters Semiempirical Method (2p-SEM), for impact energy 50 eV to 5 keV is employed to estimate Q e l and Q T . These radiosensitizer molecules, imidazole, 2-nitroimidazole, 2-nitrofuran, 4(5)-nitroimidazole, pentafluorobenzoic acid, perfluorothiophenol are of particular interest due to their applications in cancer therapy treatment. Cross sections allow us to assess the probability of various molecular Processes occurring due to electron interaction. This work is the first report of the estimation of this electron driven molecular processes for these radiosensitizers except for imidazole and 2-Nitroimidazole where previous Q i o n results are available. Our Present results are in good accordance with available data from existing literature. The study incorporates various correlations to show size dependency of various cross sections and linear correlation of Q i o n vs. impact energy, Q e l vs. impact energy, Q T vs. impact energy and linear correlation of Q i o n (Max) vs. α I to confirm the credibility and consistency of the present results.

Abstract:The secondary neutron fields at the deep tumor therapy terminal at HIRFL (Heavy Ion Research Facility in Lanzhou) were investigated. The distributions of neutron ambient dose equivalent were measured with a FHT762 Wendi-II neutron ambient dose equivalent meter as the 12C ions with energies of 165, 207, 270, and 350 MeV/u bombarded on thick tissue-like targets. The thickness of targets used in experiments is larger than the range of the carbon ions. The neutron spectra and dose equivalent is simulated by using FLUKA code, and the results agree well with the experimental data. The experiment results showed that the neutron dose produced by fragmentation reactions in tissue can be neglected in carbon-ion therapy, even considering their enhanced biological effectiveness. These results are also valuable for radiation protection, especially in the shielding design of high energy heavy ion medical machines.

Abstract:In high-energy photon therapy, several radiation protection issues result from photonuclear reactions. In this study, the photoneutron radiation field around a Varian Clinac linear accelerator in 18 MV-X mode within two different radiotherapy bunkers was investigated by means of Monte Carlo simulations using the FLUKA code as well as ambient dose-equivalent measurements. Furthermore, the activation of the air inside the treatment room due to photonuclear reactions (13N and 15O) and the capture of photoneutrons moderated down within the bunker (41Ar) was studied by FLUKA simulations. From the simulation results, the annual effective dose to medical workers due to photoneutrons and activated air was estimated. The emission of radioactivity due to ventilation of the treatment room was found to be negligible.

Abstract:Carbon ions offer significant advantages for deep-seated local tumors therapy due to their physical and biological properties. Secondary particles, especially neutrons caused by heavy ion reactions should be carefully considered in treatment process and radiation protection. For radiation protection purposes, the FLUKA Code was used in order to evaluate the radiation field at deep tumor therapy room of HIRFL in this paper. The neutron energy spectra, neutron dose and energy deposition of carbon ion and neutron in tissue-like media was studied for bombardment of solid water target by 430MeV/u C ions. It is found that the calculated neutron dose have a good agreement with the experimental date, and the secondary neutron dose may not exceed one in a thousand of the carbon ions dose at Bragg peak area in tissue-like media.
Nanodosimetric investigation of the track structure of therapeutic carbon ion radiation. Part 2: Detailed radiation transport and track structure simulation

Abstract:Previously reported nanodosimetric measurements of therapeutic-energy carbon ions penetrating simulated tissue have produced results that are incompatible with the predicted mean energy of the carbon ions in the nanodosimeter and previous experiments with lower energy monoenergetic beams. The purpose of this study is to explore the origin of these discrepancies. Detailed simulations using the Geant4 toolkit were performed to investigate the radiation field in the nanodosimeter and provide input data for track structure simulations, which were performed with a developed version of the PTra code. The Geant4 simulations show that with the narrow-beam geometry employed in the experiment, only a small fraction of the carbon ions traverse the nanodosimeter and their mean energy is between 12 % and 30 % lower than the targeted values. Only about one-third or less of these carbon ions hit the trigger detector. The track structure simulations indicate that the observed enhanced ionization cluster sizes are mainly due to coincidences with events in which carbon ions miss the trigger detector. In addition, the discrepancies observed for high absorber thicknesses of carbon ions traversing the target volume could be explained by assuming an increase in thickness or interaction cross-sections in the order of 1 %. The results show that even with strong collimation of the radiation field, future nanodosimetric measurements of clinical carbon ion beams will require large trigger detectors to register all events with carbon ions traversing the nanodosimeter. Energy loss calculations of the primary beam in the absorbers are insufficient and should be replaced by detailed simulations when planning such experiments. Uncertainties of the interaction cross-sections in simulation codes may shift the Bragg peak position.

The influence of beam delivery uncertainty on dose uniformity and penumbra for pencil beam scanning in carbon-ion radiotherapy

Abstract:The dose uniformity and penumbra in the treatment field are important factors in radiotherapy, which affects the outcomes of radiotherapy. In this study, the integrated depth-dose-distributions (IDDDs) of 190 MeV/u and 260 MeV/u carbon beams in the active spot-scanning delivery system were measured and calculated by FLUKA Monte Carlo simulation based on the Heavy Ion Medical Machine (HIMM). Considering the dose distributions caused by secondary particles and scattering, we also used different types of pencil beam (PB) models to fit and compare the spatial distributions of PB. We superposed a bunch of PB to form a 20×20 cm 2 treatment field with the double Gaussian and double Gaussian logistic beam models and calculated the influence of beam delivery error on the field flatness and penumbra, respectively. The simulated IDDDs showed good agreement with the measured values. The triple Gaussian and double Gaussian logistic beam models have good fitness to the simulated dose distributions. There are different influences on dose uniformity and penumbra resulting from beam uncertainties. These results would be helpful for understanding carbon ion therapy, and physical therapists are more familiar with beam characteristics for active scanning therapy, which provides a reference for commissioning and optimization of treatment plans in radiotherapy.

Ionization detail parameters and cluster dose: A mathematical model for selection of nanodosimetric quantities for use in treatment planning in charged particle radiotherapy

Abstract:Objective: To propose a mathematical model for applying Ionization Detail (ID), the detailed spatial distribution of ionization along a particle track, to proton and ion beam radiotherapy treatment planning (RTP). Approach: Our model provides for selection of preferred ID parameters (I_p) for RTP, that associate closest to biological effects. Cluster dose is proposed to bridge the large gap between nanoscopic I_p and macroscopic RTP. Selection of I_p is demonstrated using published cell survival measurements for protons through argon, comparing results for nineteen Ip: N_k; k = 2,3,...,10, the number of ionizations in clusters of k or more per particle, and F_k; k = 1,2,...,10, the number of clusters of k or more per particle. We then describe application of the model to ID-based RTP and propose a path to clinical translation. Main results: The preferred I_p were N_4 and F_5 for aerobic cells, N_5 and F_7 for hypoxic cells. Signifcant differences were found in cell survival for beams having the same LET or the preferred N_k. Conversely, there was no signi?cant difference for F_5 for aerobic cells and F_7 for hypoxic cells, regardless of ion beam atomic number or energy. Further, cells irradiated with the same cluster dose for these I_p had the same cell survival. Based on these preliminary results and other compelling results in nanodosimetry, it is reasonable to assert that I_p exist that are more closely associated with biological effects than current LET-based approaches and microdosimetric RBE-based models used in particle RTP. However, more biological variables such as cell line and cycle phase, as well as ion beam pulse structure and rate still need investigation. Signifcance: Our model provides a practical means to select preferred I_p from radiobiological data, and to convert I_p to the macroscopic cluster dose for particle RTP.

Abstract:I propose to use high brightness electron beam with 1 to 100 MeV energy as tool to combat tumor or cancerous tissues in deep part of body. The method is to directly deliver the electron beam to the tumor site via a small tube that connected to a high brightness electron beam accelerator that is commonly available around the world. Here I gave a basic scheme on the principle, I believe other issues people raises will be solved easily for those who are interested in solving the problems.
Possible mechanisms and simulation modeling of FLASH radiotherapy

Abstract:FLASH radiotherapy (FLASH-RT) has great potential to improve patient outcomes. It delivers radiation doses at an ultra-high dose rate (UHDR: ≥ 40 Gy/s) in a single instant or a few pulses. Much higher irradiation doses can be administered to tumors with FLASH-RT than with conventional dose rate (0.01-0.40 Gy/s) radiotherapy. UHDR irradiation can suppress toxicity in normal tissues while sustaining antitumor efficiency, which is referred to as the FLASH effect. However, the mechanisms underlying the effects of the FLASH remain unclear. To clarify these mechanisms, the development of simulation models that can contribute to treatment planning for FLASH-RT is still underway. Previous studies indicated that transient oxygen depletion or augmented reactions between secondary reactive species produced by irradiation may be involved in this process. To discuss the possible mechanisms of the FLASH effect and its clinical potential, we summarized the physicochemical, chemical, and biological perspectives as well as the development of simulation modeling for FLASH-RT.
Successful treatment of ulcerative bronchiolitis in a woman with refractory Mycobacterium intracellulare infection

Abstract:We present a case of a woman with medically refractory ulcerative colitis (UC) who developed severe bronchiectasis, bronchitis, bronchiolitis and Mycobacterium intracellulare (MAC) infection 2 years after total colectomy. Despite being on optimal therapy for her MAC and sterilising multiple consecutive sputum cultures, she remained highly symptomatic, and this led to further investigations that revealed the presence of UC-related airways disease. Addition of immunosuppressive therapy to her antimycobacterial treatment resulted in sustained and complete clinical remission of her disease. To our knowledge, this is the only case published in the literature that describes a case of successful treatment of concomitant UC-related pulmonary disease and symptomatic MAC.

Abstract:Among various types of ion species, carbon ions are considered to have the most balanced, optimal properties in terms of possessing physically and biologically effective dose localization in the body. This is due to the fact that when compared with photon beams, carbon ion beams offer improved dose distribution, leading to the concentration of the sufficient dose within a target volume while minimizing the dose in the surrounding normal tissues. In addition, carbon ions, being heavier than protons, provide a higher biological effectiveness, which increases with depth, reaching the maximum at the end of the beam's range. This is practically an ideal property from the standpoint of cancer radiotherapy. Clinical studies have been carried out in the world to confirm the efficacy of carbon ions against a variety of tumors as well as to develop effective techniques for delivering an efficient dose to the tumor. Through clinical experiences of carbon ion radiotherapy at the National Institute of Radiological Sciences and Gesellschaft für Schwerionenforschung, a significant reduction in the overall treatment time with acceptable toxicities has been obtained in almost all types of tumors. This means that carbon ion radiotherapy has meanwhile achieved for itself a solid place in general practice. This review describes clinical results of carbon ion radiotherapy together with physical, biological and technological aspects of carbon ions.

Abstract:Monte Carlo simulations are performed on the dosimetric effect of metallic nanoparticles in a clinical proton irradiation. With an in-water hitting model of a single nanoparticle, the secondary electrons dose, deposited around the particle surface, is calculated for the proton irradiations in a typical spread-out Bragg peak. The dose enhancement, as the ratio of electron doses from the target particle and background water, is evaluated for the dependence on the depth of hitting, particle size, elements, coating material and thickness. The results indicate a significant dose enhancement on the particle surface within similar to 100 nm, but a fast decay in further distance. The dose enhancement presents a consistency along the spread-out Bragg peak, a positive dependence on both the particle size and electron density, but a strong attenuation by surface coating. Particle cluster may increase the individual dose enhancement by electron crossfire, but is only noticeable in a compact case. The dose enhancement potentiates a radiosensitization use of metallic nanoparticles in clinical proton therapy, but challenging meanwhile with the narrow range of enhancement effect.
The low-LET radiation contribution to the tumor dose in diffusing alpha-emitters radiation therapy

Abstract:Diffusing alpha-emitters Radiation Therapy (Alpha DaRT) enables the use of alpha particles for the treatment of solid tumors. It employs interstitial sources carrying a few uCi of Ra-224, designed to release its short-lived progeny, which emit alpha particles, beta, Auger, and conversion electrons, x- and gamma rays. These atoms diffuse around the source and create a lethal high-dose region, measuring a few mm in diameter. Previous studies focused on the alpha dose alone. This work addresses the electron and photon contributed by the diffusing atoms and by the atoms on the source surface, for both a single source and multi-source lattices. This allows to evaluate the low-LET contribution to the dose and demonstrate the sparing of surrounding healthy tissue. The dose is calculated using Monte Carlo codes. We compare the results of a simple line-source to those of a full simulation, which implements a realistic source geometry and the spread of the diffusing atoms. We consider two extreme scenarios: low diffusion and high Pb-212 leakage, and high diffusion and low leakage. The low-LET dose in source lattices is calculated by superposition of single-source contributions. We found that for sources carrying 3 uCi/cm Ra-224 arranged in a hexagonal lattice with 4 mm spacing, the minimal low-LET dose between sources is 18-30 Gy for the two scenarios and is dominated by the beta contribution. The low-LET dose drops below 5 Gy 3 mm away from the lattice. The accuracy of the line-source approximation is 15% for the total low-LET dose over clinically relevant distances (2-4 mm). For 3 uCi/cm Ra-224 sources, the contribution of the low-LET dose can reduce cell survival by up to 2-3 orders of magnitude. Increasing source activities by a factor of 5 can bring the low-LET dose to therapeutic levels leading to a self-boosted configuration, and potentially allowing to increase the lattice spacing.
