Abstract:This research proposes an algorithm to preprocess photoplethysmography (PPG) and electrocardiogram (ECG) signals and apply the processed signals to the context aggregation network-based deep learning to achieve higher accuracy of continuous systolic and diastolic blood pressure monitoring than other reported algorithms. The preprocessing method consists of the following steps: (1) acquiring the PPG and ECG signals for a two second window at a sampling rate of 125 Hz; (2) separating the signals into an array of 250 data points corresponding to a 2 s data window; (3) randomizing the amplitude of the PPG and ECG signals by multiplying the 2 s frames by a random amplitude constant to ensure that the neural network can only learn from the frequency information accommodating the signal fluctuation due to instrument attachment and installation; (4) Fourier transforming the windowed PPG and ECG signals obtaining both amplitude and phase data; (5) normalizing both the amplitude and the phase of PPG and ECG signals using z-score normalization; and (6) training the neural network using four input channels (the amplitude and the phase of PPG and the amplitude and the phase of ECG), and arterial blood pressure signal in time-domain as the label for supervised learning. As a result, the network can achieve a high continuous blood pressure monitoring accuracy, with the systolic blood pressure root mean square error of 7 mmHg and the diastolic root mean square error of 6 mmHg. These values are within the error range reported in the literature. Note that other methods rely only on mathematical models for the systolic and diastolic values, whereas the proposed method can predict the continuous signal without degrading the measurement performance and relying on a mathematical model.

Abstract:Objective: Current resuscitation protocols require pausing chest compressions during cardiopulmonary resuscitation (CPR) to check for a pulse. However, pausing CPR during a pulseless rhythm can worsen patient outcome. Our objective is to design an ECG-based algorithm that predicts pulse status during uninterrupted CPR and evaluate its performance. Methods: We evaluated 383 patients being treated for out-of-hospital cardiac arrest using defibrillator data. We collected paired and immediately adjacent ECG segments having an organized rhythm. Segments were collected during the 10s period of ongoing CPR prior to a pulse check, and 5s segments without CPR during the pulse check. ECG segments with or without a pulse were identified by the audio annotation of a paramedic's pulse check findings and recorded blood pressures. We developed an algorithm to predict the clinical pulse status based on the wavelet transform of the bandpass-filtered ECG, applying principle component analysis. We then trained a linear discriminant model using 3 principle component modes. Model performance was evaluated on test group segments with and without CPR using receiver operating curves and according to the initial arrest rhythm. Results: There were 230 patients (540 pulse checks) in the training set and 153 patients (372 pulse checks) in the test set. Overall 38% (351/912) of checks had a spontaneous pulse. The areas under the receiver operating characteristic curve (AUCs) for predicting pulse status with and without CPR on test data were 0.84 and 0.89, respectively. Conclusion: A novel ECG-based algorithm demonstrates potential to improve resuscitation by predicting presence of a spontaneous pulse without pausing CPR. Significance: Our algorithm predicts pulse status during uninterrupted CPR, allowing for CPR to proceed unimpeded by pauses to check for a pulse and potentially improving resuscitation performance.

Abstract:Invasive arterial blood pressure (IBP) monitoring is important to assess patient's cardiovascular competence and guide clinical treatment. Besides, international resuscitation guidelines in force suggest its use during Cardiopulmonary Resuscitation (CPR), but current automated algorithms for IBP variables computation were not designed for cardiac arrest patients. A lack of knowledge is detected in the automated processing of IBP signal during CPR. The aim of this study was to design algorithms for heartbeat detection and for IBP physiological variable computation during CPR, and compare to state-of-the-art (SoA) proposals. The dataset used consists of 81 out-of-hospital-cardiac-arrest (OHCA) patients and two additional public datasets with hemodynamically stable patients. A set of 377 IBP segments, total duration of 1127 min, were extracted from the OHCA dataset during the pauses of chest compressions. The method includes artifact removing from the in IBP using Stationary Wavelet Decomposition and heartbeat detection in the first difference signal. A multicomponent evaluation and two adaptive thresholds were applied to compute IBP physiological variables. Pulsatile segments with heartbeats were discriminated from pulseless segments with mean (standard deviation) sensitivity(Se)/specificity and positive (PPV)/negative predictive values of 98.8(6.9)/91.6(20.2)% and 97.4(9.7)/98.7(6.1)% , respectively. The heartbeat detection showed 96.1(8.3)% of Se, 96.1(7.6)% of PPV and 95.7(6.4)% of F1-score , with absolute errors of 0.55(2.91)/0.39(4.87)/0.78(6.08) mmHg in systolic, diastolic and pulse pressure values, respectively. The proposed algorithms outperformed SoA solutions with both OHCA and stable patients.

Abstract:Monitoring physiological waveforms, specifically hemodynamic variables (e.g., blood pressure waveforms) and end-tidal CO2 (EtCO2), during pediatric cardiopulmonary resuscitation (CPR) has been demonstrated to improve survival rates and outcomes when compared to standard depth-guided CPR. However, waveform guidance has largely been based on thresholds for single parameters and therefore does not leverage all the information contained in multimodal data. We hypothesize that the combination of multimodal physiological features improves the prediction of the return of spontaneous circulation (ROSC), the clinical indicator of short-term CPR success. We used machine learning algorithms to evaluate features extracted from eight low-resolution (4 samples per minute) physiological waveforms to predict ROSC. The waveforms were acquired from the 2nd to 10th minute of CPR in pediatric swine models of cardiac arrest (N = 89, 8-12 kg). The waveforms were divided into segments with increasing length (both forward and backward) for feature extraction, and machine learning algorithms were trained for ROSC prediction. For the full CPR period (2nd to 10th minute), the area under the receiver operating characteristics curve (AUC) was 0.93 (95% CI: 0.87-0.99) for the multivariate model, 0.70 (0.55-0.85) for EtCO2 and 0.80 (0.67-0.93) for coronary perfusion pressure. The best prediction performances were achieved when the period from the 6th to the 10th minute was included. Poor predictions were observed for some individual waveforms, e.g., right atrial pressure. In conclusion, multimodal waveform features carry relevant information for ROSC prediction. Using multimodal waveform features in CPR guidance has the potential to improve resuscitation success and reduce mortality.

Abstract:Objective: Exploit accelerometry data for an automatic, reliable, and prompt detection of spontaneous circulation during cardiac arrest, as this is both vital for patient survival and practically challenging. Methods: We developed a machine learning algorithm to automatically predict the circulatory state during cardiopulmonary resuscitation from 4-second-long snippets of accelerometry and electrocardiogram (ECG) data from pauses of chest compressions of real-world defibrillator records. The algorithm was trained based on 422 cases from the German Resuscitation Registry, for which ground truth labels were created by a manual annotation of physicians. It uses a kernelized Support Vector Machine classifier based on 49 features, which partially reflect the correlation between accelerometry and electrocardiogram data. Results: Evaluating 50 different test-training data splits, the proposed algorithm exhibits a balanced accuracy of 81.2%, a sensitivity of 80.6%, and a specificity of 81.8%, whereas using only ECG leads to a balanced accuracy of 76.5%, a sensitivity of 80.2%, and a specificity of 72.8%. Conclusion: The first method employing accelerometry for pulse/no-pulse decision yields a significant increase in performance compared to single ECG-signal usage. Significance: This shows that accelerometry provides relevant information for pulse/no-pulse decisions. In application, such an algorithm may be used to simplify retrospective annotation for quality management and, moreover, to support clinicians to assess circulatory state during cardiac arrest treatment.

Abstract:The recovery of cerebral circulation during cardiopulmonary resuscitation (CPR) is important to improve the neurologic outcomes of cardiac arrest patients. To evaluate the feasibility of an electroencephalogram (EEG)-based prediction model as a CPR feedback indicator of high- or low-CBF carotid blood flow (CBF), the frontal EEG and hemodynamic data including CBF were measured during animal experiments with a ventricular fibrillation (VF) swine model. The most significant 10 EEG parameters in the time, frequency and entropy domains were determined by neighborhood component analysis and Student’s t-test for discriminating high- or low-CBF recovery with a division criterion of 30%. As a binary CBF classifier, the performances of logistic regression, support vector machine (SVM), k-nearest neighbor, random forest and multilayer perceptron algorithms were compared with eight-fold cross-validation. The three-order polynomial kernel-based SVM model showed the best accuracy of 0.853. The sensitivity, specificity, F1 score and area under the curve of the SVM model were 0.807, 0.906, 0.853 and 0.909, respectively. An automated CBF classifier derived from non-invasive EEG is feasible as a potential indicator of the CBF recovery during CPR in a VF swine model.

Abstract:Objective. Noninvasive blood pressure (BP) measurement technologies have been widely studied, but they still have the disadvantages of low accuracy, the requirement for frequent calibration and limited subjects. This work considers the regulation of vascular activity by the sympathetic nervous system and proposes a method for estimating BP using multiple physiological parameters. Approach. The parameters used in the model consist of heart rate variability (HRV), pulse transit time (PTT) and pulse wave morphology features extracted from electrocardiogram (ECG) and photoplethysmogram (PPG) signals. Through four classic machine learning algorithms, a hybrid data set of 3337 subjects from two databases is evaluated to verify the ability of cross-database migration. We also recommend an individual calibration procedure to further improve the accuracy of the method. Main results. The mean absolute error (MAE) and the root mean square error (RMSE) of the proposed algorithm is 10.03 and 14.55 mmHg for systolic BP (SBP), and 5.42 and 8.19 mmHg for diastolic BP (DBP). With individual calibration, the MAE and standard deviation (SD) is -0.16 +/- 7.96 (SBP) and -0.13 +/- 4.50 (DBP) mmHg, which satisfied the Advancement of Medical Instrumentation (AAMI) standard. In addition, the models are used to test single databases to evaluate their performance on different data sources. The overall performance of the Adaboost algorithm is better on the Multi-parameter Intelligent Monitoring in Intensive Care Unit (MIMIC) database; the MAE between its predicted value and true value reaches 6.6mmHg (SBP) and 3.12mmHg (DBP), respectively. Significance. The proposed method considers the regulation of blood vessels and the heart by the autonomic nervous system, and verifies its effectiveness and robustness across data sources, which is promising for improving the accuracy of continuous and cuffless BP estimation.

Abstract:Cardiovascular disease (CVD) has become a common health problem of mankind, and the prevalence and mortality of CVD are rising on a year-to-year basis. Blood pressure (BP) is an important physiological parameter of the human body and also an important physiological indicator for the prevention and treatment of CVD. Existing intermittent measurement methods do not fully indicate the real BP status of the human body and cannot get rid of the restraining feeling of a cuff. Accordingly, this study proposed a deep learning network based on the ResNet34 framework for continuous prediction of BP using only the promising PPG signal. The high-quality PPG signals were first passed through a multi-scale feature extraction module after a series of pre-processing to expand the perceptive field and enhance the perception ability on features. Subsequently, useful feature information was then extracted by stacking multiple residual modules with channel attention to increase the accuracy of the model. Lastly, in the training stage, the Huber loss function was adopted to stabilize the iterative process and obtain the optimal solution of the model. On a subset of the MIMIC dataset, the errors of both SBP and DBP predicted by the model met the AAMI standards, while the accuracy of DBP reached Grade A of the BHS standard, and the accuracy of SBP almost reached Grade A of the BHS standard. The proposed method verifies the potential and feasibility of PPG signals combined with deep neural networks in the field of continuous BP monitoring. Furthermore, the method is easy to deploy in portable devices, and it is more consistent with the future trend of wearable blood-pressure-monitoring devices (e.g., smartphones and smartwatches).

Abstract:Background and objective: Hypertension is a potentially dangerous health condition that can be detected by measuring blood pressure (BP). Blood pressure monitoring and measurement are essential for preventing and treating cardiovascular diseases. Cuff-based devices, on the other hand, are uncomfortable and prevent continuous BP measurement. Methods: In this study, a new non-invasive and cuff-less method for estimating Systolic Blood Pressure (SBP), Mean Arterial Pressure (MAP), and Diastolic Blood Pressure (DBP) has been proposed using characteristic features of photoplethysmogram (PPG) signals and nonlinear regression algorithms. PPG signals were collected from 219 participants, which were then subjected to preprocessing and feature extraction steps. Analyzing PPG and its derivative signals, a total of 46 time, frequency, and time-frequency domain features were extracted. In addition, the age and gender of each subject were also included as features. Further, correlation-based feature selection (CFS) and Relief F feature selection (ReliefF) techniques were used to select the relevant features and reduce the possibility of over-fitting the models. Finally, support vector regression (SVR), K-nearest neighbour regression (KNR), decision tree regression (DTR), and random forest regression (RFR) were established to develop the BP estimation model. Regression models were trained and evaluated on all features as well as selected features. The best regression models for SBP, MAP, and DBP estimations were selected separately. Results: The SVR model, along with the ReliefF-based feature selection algorithm, outperforms other algorithms in estimating the SBP, MAP, and DBP with the mean absolute error of 2.49, 1.62 and 1.43 mmHg, respectively. The proposed method meets the Advancement of Medical Instrumentation standard for BP estimations. Based on the British Hypertension Society standard, the results also fall within Grade A for SBP, MAP, and DBP. Conclusion: The findings show that the method can be used to estimate blood pressure non-invasively, without using a cuff or calibration, and only by utilizing the PPG signal characteristic features.

Abstract:Cardiovascular diseases are now the leading cause of death that endangers people's health. Thus, a precise and reliable blood pressure (BP) prediction method is essential. This paper proposes a noninvasive BP prediction method with the multi-feature fusion of electrocardiogram (ECG), photoplethysmography (PPG), and pressure pulse waveform (PPW). A multi-sensor information acquisition platform was developed to collect cardiovascular-related signals. Besides, the algorithms were designed to clean, preprocess, and extract features from sample data. Furthermore, the BP prediction model was constructed by using feature selection and feature fusion based on Random Forest Regression (RFR). Finally, the importance of features used for blood pressure prediction was analyzed, and the results of RFR-based blood pressure prediction were compared with those of other machine learning algorithms. The mean absolute errors of systolic and diastolic blood pressure prediction reached 0.90 mmHg and 2.47 mmHg, respectively. The results of the BP prediction model based on multi-sensor information fusion meet both the AAMI and BHS standards.

Abstract:Adding cuffless blood pressure (BP) measurement function to wearable devices is of great value in the fight against hypertension. The widely used arterial pulse transit time (PTT)-based method for BP monitoring relies primarily on vascular status-determined BP models and typically exhibits degraded performance over time and is sensitive to measurement procedures. Developing alternative methods with improved accuracy and adaptability to various application scenarios is highly desired for cuffless BP measurement. In this work, we proposed a pattern-fusion (PF) method that incorporates cardiovascular coupling effects in the vascular model by combining three calculation modules - cardiac parameter extraction module, cardiac parameter-to-BP mapping module, and BP regulation module. Specifically, the first module combines feedforward, feedback, and propagation modes to model different modulation functions of a cardiovascular system and is responsible for extracting BP-related features from electrocardiography (ECG) and photoplethysmography (PPG) signals; the cardiac parameter-to-BP mapping module is used to map cardiac parameters into mean blood pressure (MBP) by fusing different features; finally, the BP regulation module recovers accurate systolic BP (SBP) and diastolic BP (DBP) from given MBP. With the concerted use of these three modules, the pattern fusion method consistently demonstrates excellent BP prediction accuracy in a variety of measurement scenarios and durations, exhibiting SBP/DBP mean absolute error (MAE) of 3.65/4.56 mmHg for the short-term (<10 mins) continuous measurement dataset, SBP/DBP MAE of 6.84/3.81 mmHg for the medium-term (avg. > 20 hours) continuous measurement dataset, and SBP/DBP MAE of 6.24/3.65 mmHg for the long-term (> 1 month) intermittent measurement dataset.

Abstract:Abstract Continuous vital signal monitoring is becoming more relevant in preventing diseases that afflict a large part of the world’s population; for this reason, healthcare equipment should be easy to wear and simple to use. Non-intrusive and non-invasive detection methods are a basic requirement for wearable medical devices, especially when these are used in sports applications or by the elderly for self-monitoring. Arterial blood pressure (ABP) is an essential physiological parameter for health monitoring. Most blood pressure measurement devices determine the systolic and diastolic arterial blood pressure through the inflation and the deflation of a cuff. This technique is uncomfortable for the user and may result in anxiety, and consequently affect the blood pressure and its measurement. The purpose of this paper is the continuous measurement of the ABP through a cuffless, non-intrusive approach. The approach of this paper is based on deep learning techniques where several neural networks are used to infer ABP, starting from photoplethysmogram (PPG) and electrocardiogram (ECG) signals. The ABP was predicted first by utilizing only PPG and then by using both PPG and ECG. Convolutional neural networks (ResNet and WaveNet) and recurrent neural networks (LSTM) were compared and analyzed for the regression task. Results show that the use of the ECG has resulted in improved performance for every proposed configuration. The best performing configuration was obtained with a ResNet followed by three LSTM layers: this led to a mean absolute error (MAE) of 4.118 mmHg on and 2.228 mmHg on systolic and diastolic blood pressures, respectively. The results comply with the American National Standards of the Association for the Advancement of Medical Instrumentation. ECG, PPG, and ABP measurements were extracted from the MIMIC database, which contains clinical signal data reflecting real measurements. The results were validated on a custom dataset created at Neuronica Lab, Politecnico di Torino.

Abstract:Continuous and noninvasive monitoring of blood pressure has numerous clinical and fitness applications. Current methods of continuous measurement of blood pressure are either invasive and/or require expensive equipment. Therefore, we investigated a new method for the continuous estimation of two main features of blood pressure waveform: systolic and diastolic pressures. The estimates were obtained from a photoplethysmography signal as input to the fifth order autoregressive moving average models. The performance of the method was evaluated using beat-to-beat full-wave blood pressure measurements from 15 young subjects, with no known cardiovascular disorder, in supine position as they breathed normally and also while they performed a breath-hold maneuver. The level of error in the estimates, as measured by the root mean square of the model residuals, was less than 5 mmHg during normal breathing and less than 8 mmHg during the breath-hold maneuver. The mean of model residuals both during normal breathing and breath-hold maneuvers was considered to be less than 3.2 mmHg. The dependency of the accuracy of the estimates on the subject data was assessed by comparing the modeling errors for the 15 subjects. Less than 1% of the models showed significant differences (p < 0.05) from the other models, which indicates a high level of consistency among the models.

The traditional intra-arterial method is too invasive for routine healthcare settings, whereas cuff-based devices are inferior in reliability and comfortable for long-term BP monitoring during arrhythmias. The study aimed to investigate an indirect method for continuous and cuff-less BP estimation based on electrocardiogram (ECG) and photoplethysmogram (PPG) signals during arrhythmias and to test its reliability for the determination of BP using invasive BP (IBP) as reference.Methods: Thirty-five clinically stable patients (15 with ventricular arrhythmias and 20 with supraventricular arrhythmias) who had undergone radiofrequency ablation were enrolled in this study. Their ECG, PPG, and femoral arterial IBP signals were simultaneously recorded with a multi-parameter monitoring system. Fifteen features that have the potential ability in indicating beat-to-beat BP changes during arrhythmias were extracted from the ECG and PPG signals. Four machine learning algorithms, decision tree regression (DTR), support vector machine regression (SVR), adaptive boosting regression (AdaboostR), and random forest regression (RFR), were then implemented to develop the BP models.Results: The results showed that the mean value ± standard deviation of root mean square error for the estimated systolic BP (SBP), diastolic BP (DBP) with the RFR model against the reference in all patients were 5.87 ± 3.13 and 3.52 ± 1.38 mmHg, respectively, which achieved the best performance among all the models. Furthermore, the mean error ± standard deviation of error between the estimated SBP and DBP with the RFR model against the reference in all patients were −0.04 ± 6.11 and 0.11 ± 3.62 mmHg, respectively, which complied with the Association for the Advancement of Medical Instrumentation and the British Hypertension Society (Grade A) standards.Conclusion: The results indicated that the utilization of ECG and PPG signals has the potential to enable cuff-less and continuous BP estimation in an indirect way for patients with arrhythmias.

Abstract:An important means for preventing and managing cardiovascular disease is the non-invasive estimation of blood pressure. There is particular interest in developing approaches that provide accurate cuffless and continuous estimation of this important vital sign. This paper proposes a method that uses dynamic changes of the pulse waveform over short time intervals and calibrates the system based on a mathematical model that relates reflective PTT (R-PTT) to blood pressure. An advantage of the method is that it only requires collecting the photoplethysmogram (PPG) using one optical sensor, in addition to initial non-invasive measurements of blood pressure that are used for calibration. This method was applied to data from 30 patients, resulting in a mean error (ME) of 0.59 mmHg, a standard deviation of error (SDE) of 7.07 mmHg, and a mean absolute error (MAE) of 4.92 mmHg for diastolic blood pressure (DBP) and an ME of 2.52 mmHg, an SDE of 12.15 mmHg, and an MAE of 8.89 mmHg for systolic blood pressure (SBP). These results demonstrate the possibility of using the PPG signal for the cuffless continuous estimation of blood pressure based on the analysis of calibrated changes in cardiovascular dynamics, possibly in conjunction with other methods that are currently being researched.

Abstract:Background Limbs pneumatic compression during cardiopulmonary resuscitation (CPR) for cardiac arrest patient were rarely reported. Purpose To evaluate the effect of this therapy on the patients of cardiac arrest. Methods A randomized, controlled and trial, a total of 62 elderly (75.8±8.3 yrs) cardiac arrest patients were included and randomized to "CPR group" (n=31) and "CPR + Pneumatic Compression group" (n=31). Each individual in CPR group received standard CPR. In addition to the standard CPR, all patients in "Pneumatic Compression group" accepted intermittent pneumatic compression (IPC) by Air Compression Therapy System, which digital six-tiered rippling full-body pressure (0.45–0.6 kg/cm2) and 1 minute interval every 5 minutes. Results Compared with standard CPR group, significant improvements of cardiac output (CO) and survival of patient were found in Pneumatic Compression group (Figure 1). Patients with pneumatic compression had lower incidence (19.4% vs. 45.2%, p=0.03) of acute kidney injury (AKI) than that in standard CPR group. Besides, the lower occurrence of hypoxic ischemic encephalopathy (HIE) was observed in Pneumatic Compression group (Figure 2). There was no ischemic myonecrosis of limbs caused by compression. Conclusions Intermittent limbs pneumatic compression during cardiopulmonary resuscitation (CPR) maybe could improve the blood flow of the crucial organs (e.g. brain) and increase survival of cardiac arrest patients, which is superior to the strategy for CPR alone. Abstract:Background: This study was a first step to facilitate the development of automated decision support systems using cardiac output (CO) for combat casualty care. Such systems remain a practical challenge in battlefield and prehospital settings. In these environments, reliable CO estimation using blood pressure (BP) and heart rate (HR) may provide additional capabilities for diagnosis and treatment of trauma patients. The aim of this study was to demonstrate that continuous BP and HR from the arterial BP waveform coupled with machine learning (ML) can reliably estimate CO in a conscious sheep model of multiple hemorrhages and resuscitation. Methods: Hemodynamic parameters (BPs, HR) were derived from 100-Hz arterial BP waveforms of 10 sheep records, 3 hours to 4 hours long. Two models (mean arterial pressure, Windkessel) were then applied and merged to estimate COVS. ML was used to develop a rule for identifying when models required calibration. All records contained 100-Hz recording of pulmonary arterial blood flow using Doppler transit time (COFP). COFP and COVS were analyzed using equivalence tests and Bland-Altman analysis, as well as waveform and concordance plots. Results: Baseline COFP varied from 3.0 L/min to 5.4 L/min, while posthemorrhage COFP varied from 1.0 L/min to 1.8 L/min. A total of 315,196 pairs of data were obtained. Equivalence tests for individual records showed that COVS was statistically equivalent to COFP (p < 0.05). Smaller equivalence thresholds (<0.3 L/min) indicated an overall high COFP accuracy. The agreement between COFP and COVS was -0.13 (0.69) L/min (Bland-Altman). In an exclusion zone of 12%, trending analysis found a 92% concordance between 5-minute changes in COFP and COVS. Conclusion: This study showed that CO can be reliably estimated using BPs and HR from the arterial BP waveform in combination with ML. A next step will be to test this approach using noninvasive BPs and HR.
Using Photoplethysmography to Detect Real-time Blood Pressure Changes with a Calibration-free Deep Learning Model

Abstract:Blood pressure (BP) changes are linked to individual health status in both clinical and non-clinical settings. This study developed a deep learning model to classify systolic (SBP), diastolic (DBP), and mean (MBP) BP changes using photoplethysmography (PPG) waveforms. Data from the Vital Signs Database (VitalDB) comprising 1,005 ICU patients with synchronized PPG and BP recordings was used. BP changes were categorized into three labels: Spike (increase above a threshold), Stable (change within a plus or minus threshold), and Dip (decrease below a threshold). Four time-series classification models were studied: multi-layer perceptron, convolutional neural network, residual network, and Encoder. A subset of 500 patients was randomly selected for training and validation, ensuring a uniform distribution across BP change labels. Two test datasets were compiled: Test-I (n=500) with a uniform distribution selection process, and Test-II (n=5) without. The study also explored the impact of including second-deviation PPG (sdPPG) waveforms as additional input information. The Encoder model with a Softmax weighting process using both PPG and sdPPG waveforms achieved the highest detection accuracy--exceeding 71.3% and 85.4% in Test-I and Test-II, respectively, with thresholds of 30 mmHg for SBP, 15 mmHg for DBP, and 20 mmHg for MBP. Corresponding F1-scores were over 71.8% and 88.5%. These findings confirm that PPG waveforms are effective for real-time monitoring of BP changes in ICU settings and suggest potential for broader applications.
