Abstract:Alloxan diabetes was induced in inbred rats that then were divided into four groups consisting of unoperated diabetic controls, sham-operated diabetic controls, rats given pancreaticoduodenal isografts, and rats given duct-ligated pancreas isografts. The animals were studied for from 18 months (controls) to two years (transplants) and the following important results were obtained: I) In striking contrast to the diabetic controls, pancreas transplants of both types produced immediate and permanent relief of hyperglycemia, immediate and lasting elevation of serum insulin levels, a normal weight and growth curve, and good health for two years. Removal of the graft was followed by recurrence of severe diabetes. 2) Pancreas transplants of both types prevented the widespread and severe renal, ophthalmic and neural lesions of diabetes that were found in the diabetic controls. 3) The ductligated pancreas graft and pancreaticoduodenal transplant were equally effective in controlling diabetes. Ligation of the pancreatic duct was not followed by significant morphologic or clinical evidence of pancreatitis or by loss of endocrine function. 4) Portal venous drainage of the pancreas transplant was unnecessary for good endocrine function.
A retrospective cohort study of disease-related risk factors for central venous catheter-related symptomatic thrombosis in intensive care unit inpatients

Abstract:ABSTRACT: Central venous catheters (CVC) are widely used in critically ill patients given their benefits in monitoring vital signs, treatment administration, and renal replacement therapy in intensive care unit (ICU) patients, but these catheters have the potential to induce symptomatic catheter-related venous thrombosis (CRVT). This study reported the rate of symptomatic CRVT in ICU patients receiving CVC and analyzed the disease-related risk factors for symptomatic CRVT in ICU patients.A retrospective analysis was performed on the consecutive ICU 1643 critically ill patients with CVCs inserted from January 2015 to December 2019. Symptomatic CRVT was confirmed by ultrasound. CVCs were divided into 2 groups based on the presence of symptomatic CRVT, and the variables were extracted from the electronic medical record system. Logistic univariate and multivariate regression analyses were used to determine the disease-related risk factors of symptomatic CRVT.A total of 209 symptomatic CRVT events occurred among 2114 catheters. The rate of CRVT was 9.5 per 1000 catheter days. Univariate analysis revealed that trauma, major surgery, heart failure, respiratory failure, and severe acute pancreatitis were risk factors for symptomatic CRVT in the ICU. Multivariate analysis showed that trauma (odds ratio [OR], 2.046; 95% confidence interval [CI] [1.325-3.160], P = .001), major surgery (OR, 2.457; 95% CI [1.641-3.679], P = .000), and heart failure (OR, 2.087; 95% CI [1.401-3.111], P = .000) were independent disease-related risk factors for symptomatic CRVT in ICU. The C-statistic for this model was 0.61 (95% CI [0.57-0.65], P = .000).The incidence rate of symptomatic CRVT in the ICU population was 9.5 per 1000 catheter days. Trauma, major surgery, and heart failure are independent disease-related risk factors of symptomatic CRVT.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Purpose</h3><p>Central venous access devices (CVADs) are commonly employed in the management of cancer patients. While having several benefits they are associated with significant risks. We reviewed the incidence and risk factors for catheter‐related thrombosis (CRT) in cancer patients with CVADs.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We performed a prospective observational cohort study of adult patients with cancer requiring a CVAD between January 12 004 and June 292 016. The rate of, and risk factors for the development of, symptomatic catheter‐related thrombosis were evaluated.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>4920 central lines were inserted into 3130 patients. The incidence of CRT was 3.6%. CRT developed a median of 12 days following line insertion. PICCs were associated with the highest rates of CRT (HR 22.2, 95% CI 2.9–170.6). Older age groups developed CRT at lower rates (HR 0.57; 95%CI 0.39–0.84 for age 50–61 years, and HR 0.63; 95% CI 0.45–0.89 for age > 61 years) compared to age < 50 years. Increased CRT was seen in patients with prior CRT (HR 1.81; 95%CI 1.19–2.77). There was a trend to more for cancer subtype with a Khorana tumor score of 1 compared to those with a score of 0 (HR 1.37, 95% CI 1.00–1.88). Hodgkin lymphoma, germ cell and oesophagus cancers had the highest CRT rates. Side of insertion was not associated with thrombosis risk (HR 0.77; 95% CI 0.57–1.05 P = 0.10).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Age < 50, PICC lines and prior CRT were associated with highest CRT rate. Abstract:Importance Central venous catheters (CVCs) are commonly used but are associated with complications. Quantifying complication rates is essential for guiding CVC utilization decisions. Objective To summarize current rates of CVC-associated complications. Data Sources MEDLINE, Embase, CINAHL, and CENTRAL databases were searched for observational studies and randomized clinical trials published between 2015 to 2023. Study Selection This study included English-language observational studies and randomized clinical trials of adult patients that reported complication rates of short-term centrally inserted CVCs and data for 1 or more outcomes of interest. Studies that evaluated long-term intravascular devices, focused on dialysis catheters not typically used for medication administration, or studied catheters placed by radiologists were excluded. Data Extraction and Synthesis Two reviewers independently extracted data and assessed risk of bias. Bayesian random-effects meta-analysis was applied to summarize event rates. Rates of placement complications (events/1000 catheters with 95% credible interval [CrI]) and use complications (events/1000 catheter-days with 95% CrI) were estimated. Main Outcomes and Measures Ten prespecified complications associated with CVC placement (placement failure, arterial puncture, arterial cannulation, pneumothorax, bleeding events requiring action, nerve injury, arteriovenous fistula, cardiac tamponade, arrhythmia, and delay of ≥1 hour in vasopressor administration) and 5 prespecified complications associated with CVC use (malfunction, infection, deep vein thrombosis [DVT], thrombophlebitis, and venous stenosis) were assessed. The composite of 4 serious complications (arterial cannulation, pneumothorax, infection, or DVT) after CVC exposure for 3 days was also assessed. Results Of 11 722 screened studies, 130 were included in the analyses. Seven of 15 prespecified complications were meta-analyzed. Placement failure occurred at 20.4 (95% CrI, 10.9-34.4) events per 1000 catheters placed. Other rates of CVC placement complications (per 1000 catheters) were arterial canulation (2.8; 95% CrI, 0.1-10), arterial puncture (16.2; 95% CrI, 11.5-22), and pneumothorax (4.4; 95% CrI, 2.7-6.5). Rates of CVC use complications (per 1000 catheter-days) were malfunction (5.5; 95% CrI, 0.6-38), infection (4.8; 95% CrI, 3.4-6.6), and DVT (2.7; 95% CrI, 1.0-6.2). It was estimated that 30.2 (95% CrI, 21.8-43.0) in 1000 patients with a CVC for 3 days would develop 1 or more serious complication (arterial cannulation, pneumothorax, infection, or DVT). Use of ultrasonography was associated with lower rates of arterial puncture (risk ratio [RR], 0.20; 95% CrI, 0.09-0.44; 13.5 events vs 68.8 events/1000 catheters) and pneumothorax (RR, 0.25; 95% CrI, 0.08-0.80; 2.4 events vs 9.9 events/1000 catheters). Conclusions and Relevance Approximately 3% of CVC placements were associated with major complications. Use of ultrasonography guidance may reduce specific risks including arterial puncture and pneumothorax.

Abstract:Current research in motor neural prosthetics has focused primarily on issues related to the extraction of motor command signals from the brain (e.g. brain–machine interfaces) to direct the motion of prosthetic limbs. Patients using these types of systems could benefit from a somatosensory neural interface that conveys natural tactile and kinesthetic sensations for the prosthesis. Electrical microstimulation within the dorsal root ganglia (DRG) has been proposed as one method to accomplish this, yet little is known about the recruitment properties of electrical microstimulation in activating nerve fibers in this structure. Current-controlled microstimulation pulses in the range of 1–15 µA (200 µs, leading cathodic pulse) were delivered to the L7 DRG in four anesthetized cats using penetrating microelectrode arrays. Evoked responses and their corresponding conduction velocities (CVs) were measured in the sciatic nerve with a 5-pole nerve cuff electrode arranged as two adjacent tripoles. It was found that in 76% of the 69 electrodes tested, the stimulus threshold was less than or equal to 3 µA, with the lowest recorded threshold being 1.1 µA. The CVs of afferents recruited at threshold had a bimodal distribution with peaks at 70 m s−1 and 85 m s−1. In 53% of cases, the CV of the response at threshold was slower (i.e. smaller diameter fiber) than the CVs of responses observed at increasing stimulation amplitudes. In summary, we found that microstimulation applied through penetrating microelectrodes in the DRG provides selective recruitment of afferent fibers from a range of sensory modalities (as identified by CVs) at very low stimulation intensities. We conclude that the DRG may serve as an attractive location from which to introduce surrogate somatosensory feedback into the nervous system.
Clinical practice guidelines for the management of central venous catheter for critically ill patients

Abstract:The objective of this article is to provide a guideline for the management of central venous catheter for critically ill patients. Electronic databases of CENTRAL, CINAHL, EMBASE, four Chinese databases (CBM, WANFANG DATA, CAJD, VIP Database) and Google Scholar were searched from inception to August 2017. The reviewers assessed each included study for the risk of bias under the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) framework. The GRADE evidence profile tables were added to each important clinical outcome. The guideline will be updated in a 5-year interval by incorporating new evidence. The guideline panel provided 11 statements on the management of central venous catheter for critically ill patients. Overall, there were 4 strong recommendations, and 7 weak recommendations. They were as follows:(I) we commend the use of catheter impregnation to prevent catheterrelated blood stream infection (1A);(II) we suggest the use of real-time ultrasound guidance for subclavian or femoral vein insertion (2B), and recommend that for internal jugular vein (1A);(III) we suggest the use of realtime color Doppler ultrasound guidance on central venous catheterization for adult and pediatric patients (2C);(IV) we suggest not to use heparin for the maintenance of CVC patency (2A);(V) we suggest the use contrastenhanced ultrasound for the confirmation of central venous catheter placement (2B);(VI) we recommend the use of bedside ultrasound together with agitated or non-agitated normal saline to confirm CVC position (1C);(VII) we suggest to use subclavian site for CVC insertion (2C);(VIII) we …

Abstract:and fibrous healing is proceeding apace in the depths, and whern 5 or even IO local applications at intervals have been necessary to obtain a measure of bacteriostasis, which even then may be only temporary. And as, according to the example cited, it is easier to treat to a successful conclusion the pneumonia from the outset than the complicating lung abscess later, so in cases of infected wounds involving bone it is prudent to employ full chemotherapeutic measures at the time of full surgical treatment rather than later, when sequestra and sinuses may have formed in association with established infection. Full surgical procedure is of first importance, but we would emphasize that in cases of the kind considered here the preand post-operative administration of a sulphonamide, preferably sulphathiazole, is equally essential, and should be supplemented by local application, especially when many fascial planes have been disturbed or there is much tissue damage. For minimum pre-operative medication, 6 g. of sulphathiazole within 4 hours yields a blood concentration of 4 mg. per 100 c.cm., so that intramuscular and intravenous routes are not always necessary to supply the immediately available sulphonamide requisite in the blood and tissue fluids. There is little doubt that sulphathiazole should replace sulphapyridine, both orally and locally, in the treatment of such cases of staphylococcal infection, for this drug is better tolerated and has a greater bacteriostatic action. Further, adequate oral administration of sulphathiazole should be continued as necessary, so that essential tissue-fluid concentrations are present when multiplication of organisms occurs or is likely to occur, thus ensuring that bacteriostasis is sufficient to permit the several body-defence mechanisms to operate unhindered or in conditions as favourable as possible.
Risk Factors of Central Venous Catheter-Related Bloodstream Infection for Continuous Renal Replacement Therapy in Kidney Intensive Care Unit Patients

Abstract:Background: The incidence of central venous catheter-related bloodstream infection (CRBSI) for continuous renal replacement therapy (CRRT) in kidney intensive care unit (ICU) patients is worthy of particular attention and recently, we analyzed clinical characteristics and risk factors of CRBSI for CRRT in our kidney ICU patients. Methods: To be part of this retrospective study, 1,523 patients who had a central venous catheter (CVC) for CRRT during the period April 2010 to May 2015 in our centre were enrolled. The clinical features and pathogens of CRBSI patients were investigated. Patients who also had CRRT of kidney ICU hospitalization without CRBSI were enrolled in a 1: 2 ratio as control. Risk factors of the CRBSI were analyzed. Results: A total of 57 patients had central venous CRBSI. The incidence of the infection was 3.7%. The mean rate of CRBSI was 3.9 per 1,000 catheter days, and the catheter median indwelling time was 14 (7–30) days. The most common pathogens were Gram-positive bacteria, which were noted in 29 cases (50.9%), followed by Gram-negative bacteria (36.8%). The most common pathogens causing CRBSI were Staphylococcus aureus (10 cases) and sewer enterobacteriaceae (10 cases) followed by Staphylococcus epidermidis (9 cases). CVC insertion sites included internal jugular vein (33 cases) and femoral vein (24 cases), accounting for 2.9% of internal jugular vein catheterization (1,140 cases) and 6.3% of femoral vein catheterization (383 cases) respectively. In total, 16, 20, 7 and 14 cases of CRBSI were noted in Spring, Summer, Autumn and Winter, accounting for 28.1, 35.1, 12.3 and 24.6% respectively. The most common infectious manifestations were chills (68.4%), fever (100%), and septic shock (49.1%). Multivariate analysis showed that catheterization of the femoral vein, long catheter indwelling time, low CD4+ lymphocytes and high acute physiology and chronic health evaluation (APACHE) II scores were independent factors associated with CRBSI. Conclusions: The incidence of CRBSI in our kidney ICU was 3.7%. Central venous CRBSI for CRRT was associated with catheterization of the femoral vein, long catheter indwelling time, compromised immune function and high APACHE II scores. Understanding pathogens and risk factors for central venous CRBSI in kidney ICU can help doctors prevent and treat CRBSI earlier.

Risk Factors for Central Venous Access Device-Related Thrombosis in Hospitalized Children: A Systematic Review and Meta-Analysis

Abstract:Objective: To identify the potential associations of patient-, treatment-, and central venous access device (CVAD)-related factors with the CVAD-related thrombosis (CRT) risk in hospitalized children. Methods: A systematic search of PubMed, EMBASE, Web of Science, the Cochrane Library, China National Knowledge Infrastructure, Wanfang, and VIP database was conducted. RevMan 5.3 and Stata 12.0 statistical software were employed for data analysis. Results: In terms of patient-related factors, the patient history of thrombosis (odds ratio [OR] = 3.88, 95% confidence interval [CI]: 2.57-5.85), gastrointestinal/liver disease (OR = 1.85, 95% CI: 0.99-3.46), hematologic disease (OR = 1.45, 95% CI: 1.06-1.99), and cancer (OR = 1.58, 95% CI: 1.01-2.48) were correlated with an increased risk of CRT. In terms of treatment-related factors, parenteral nutrition (PN)/total PN (OR = 1.70, 95% CI: 1.21-2.39), hemodialysis (OR = 2.17, 95% CI: 1.34-3.51), extracorporeal membrane oxygenation (OR = 1.51, 95% CI: 1.31-1.71), and cardiac catheterization (OR = 3.92, 95% CI: 1.06-14.44) were associated with an increased CRT risk, while antibiotics (OR = 0.46, 95% CI: 0.32-0.68) was associated with a reduced CRT risk. In terms of the CVAD-related factors, CRT risk was more significantly increased by peripherally inserted central catheter than tunneled lines (OR = 1.81, 95% CI: 1.15-2.85) or totally implantable venous access port (OR = 2.81, 95% CI: 1.41-5.60). And subclavian vein catheterization significantly contributed to a lower CRT risk than femoral vein catheterization (OR = 0.36, 95% CI: 0.14-0.88). Besides, multiple catheter lines (OR = 4.06, 95% CI: 3.01-5.47), multiple catheter lumens (OR = 3.71, 95% CI: 1.99-6.92), central line-associated bloodstream infection (OR = 2.66, 95% CI: 1.15-6.16), and catheter malfunction (OR = 1.65, 95% CI: 1.07-2.54) were associated with an increased CRT risk. Conclusion: The exact identification of the effect of risk factors can boost the development of risk assessment tools with stratifying risks.
Central Venous Catheter-related Thrombosis in Senile Male Patients： New Risk Factors and Predictors

Abstract:Central venous catheterization (CVC)-related venous thrombosis is a common but serious clinical complication, thus prevention and treatment on this problem should be extensively investigated. In this research, we aimed to investigate the incidence rate of CVC-related venous thrombosis in senile patients and give a further discussion on the related risk factors and predictors. A total of 324 hospitalized senile male patients subjected to CVC were selected. Retrospective investigation and analysis were conducted on age, underlying diseases, clinical medications, catheterization position and side, catheter retention time, and incidence of CVC-related venous thrombosis complications. Basic laboratory test results during catheterization and thrombogenesis were also collected and analyzed. Among the 324 patients, 20 cases (6.17%) of CVC-related venous thrombosis were diagnoseds. The incidence rate of CVC-related venous thrombosis in subclavian vein catheterization was significantly lower than that in femoral vein catheterization (P<0.01) and that in internal jugular vein catheterization (P<0.05). No statistically significant difference was found between femoral vein catheterization and internal jugular vein catheterization (P<0.05). Previous venous thrombosis history (P<0.01), high lactate dehydrogenase level (P<0.01), low high-density lipoprotein (HDL) level (P<0.05), and low albumin level (P<0.05) were found as risk factors or predictors of CVC-related venous thrombosis in senile male patients. Subclavian vein catheterization was the most appropriate choice among senile patients to decrease the incidence of CVC-related venous thrombosis. Previous venous thrombosis history, high lactate dehydrogenase level, low HDL level, and low albumin level were important risk factors in predicting CVC-related venous thrombosis.

Abstract:BACKGROUND: There are limited data on the long-term effects of implementing a central venous catheter (CVC) program for prevention of CVC infections. The aims of this study were to evaluate the incidence of CVC colonization, catheter-related infections (CRI), catheter-related bloodstream infections (CRBSI), and their risk factors over a 6-year period in a hospital with an active CVC team.METHODS: We conducted a continuous prospective study aiming to include all CVCs used at our hospital during the years 2004 to 2009, evaluating colonization, CRI, CRBSI, and possible risk factors.RESULTS: A total of 2,772 CVCs was used during the study period. Data on culture results and catheterization time were available for 2,045 CVCs used in 1,674 patients. The incidences of colonization, CRI, and CRBSI were 7.0, 2.2, and 0.6 per 1,000 CVC-days, respectively. Analysis of quarterly incidences revealed 1 occasion with increasing infection rates. Catheterization time was a risk factor for CRI but not for CRBSI. Other risk factors for CRI were hemodialysis and CVC use in the internal jugular vein compared with the subclavian vein. Hemodialysis was the only risk factor for CRBSI.CONCLUSION: We found that a CRI prevention program led by an active CVC team and adhered to by the entire staff at a county hospital is successful in keeping CVC infections at a low rate over a long period of time.

Abstract:The correlations between original bias and biases on days 2 through 6, respectively, were -0.34, 0.10, 0.09, 0.18, and 0.02. None of these was statistically significant. Also, such changes in bias as did exist from original test to subsequent test .did not differ across days in any systematic fashion (F = 1.39, d.f. = 4.45, P > .25). It seems reasonable to conclude, therefore, that the procedures used do not give rise to any systematic alteration of thebias of the beetle. Although alternative interpretations of our results may still be possible, the available evidence suggests that analysis of the phenomenon in terms of physiological and biochemical memory mechanisms is justifiable. Abstract:Objective: To determined the current incidence and acute complications of asymptomatic central venous catheter (CVC)-related deep venous thrombosis (DVT) in critically ill children. Study design: We performed a prospective cohort study in 3 pediatric intensive care units. A total of 101 children with newly inserted untunneled CVC were included. CVC-related DVT was diagnosed using compression ultrasonography with color Doppler. Results: Asymptomatic CVC-related DVT was diagnosed in 16 (15.8%) children, which equated to 24.7 cases per 1000 CVC-days. Age was independently associated with DVT. Compared with children aged <1 year, children aged >13 years had significantly higher odds of DVT (aOR, 14.1, 95% CI, 1.9-105.8; P = .01). Other patient demographics, interventions (including anticoagulant use), and CVC characteristics did not differ between children with and without DVT. Mortality-adjusted duration of mechanical ventilation, a surrogate for pulmonary embolism, was statistically similar in the 2 groups (22 ± 9 days in children with DVT vs 23 ± 7 days in children without DVT; P = .34). Mortality-adjusted intensive care unit and hospital lengths of stay also were similar in the 2 groups. Conclusion: Asymptomatic CVC-related DVT is common in critically ill children. However, the acute complications do not seem to differ between children with and without DVT. Larger studies are needed to confirm these results. Future studies should also investigate the chronic complications of asymptomatic CVC-related DVT.

Abstract:Introduction/Background: Central venous catheters (CVC) are commonly utilized in cancer patients for prolonged systemic anti-cancer treatments but increased the risk of right atrial thrombosis (C THR ). Cardiac MRI (CMR) tissue characterization is commonly performed to identify C THR based on absence of vascularity. Research Questions/Hypothesis: How do catheters affect thrombus formation? Goals/Aims: To evaluate predictors of thrombus formation in cancer patients with CVC. Methods/Approach: The population comprised of adult (≥18yo) systemic cancer pts with indwelling CVC who underwent contrast-enhanced CMR (0.2 mmol/kg gadolinium). C THR were defined using the reference of LGE-CMR via established criteria (absence of contrast enhancement). CVC depth was measured from CXR/CT. Abstract:Background: Central venous catheters (CVC) are vital to the management of critically ill children. Despite efforts to minimize complications, central line associated bloodstream infection (CLABSI) and venous thromboembolisms (VTE) still occur. Methods: We performed a retrospective review of a prospectively collected database for children admitted to the pediatric intensive care unit (PICU) between November 2013 and December 2016. Results: In total, 2714 CVC were in place, 979 of which were percutaneous CVC. During the study period, 21 CLABSI (1.6/1000 line days) were identified, of which, nearly half (n = 9, 42.9%) were associated with percutaneous CVC (2.6/1000 line days). Poisson regression analysis did not identify a single risk factor for CLABSI when adjusting for line type, anatomic location and laterality of placement, geographic location of placement, length of PICU admission, presence of gastrostomy tube, concurrent mechanical ventilation, age, weight, and height. Forty clinically significant VTE (2.9/1000 line days) were identified, with percutaneous CVC having the highest incidence (7.5/1000 line days, p < 0.001). Of percutaneous CVC, clinically significant VTE were more often associated with femoral vein cannulation (14.8/1000 line days) compared to internal jugular and subclavian vein (2.5 and 2.4/1000 line days, respectively, p < 0.001). Conclusion: This data suggests that the femoral site may be an important risk factor that should be considered in prevention strategies for catheter-associated VTE in children. Abstract:PURPOSE:This is a single-center, descriptive report of the management and complications of venous catheter use in 19 severely burned passengers from a bus fire that occurred on July 5, 2014, in Hangzhou.METHODS:We recorded the parameters of the catheters insertion and indwelling. Sampling of each removed catheter was conducted to monitor for catheter-related infections. Bedside ultrasound screening was performed for recording central venous catheter (CVC)-related complications.RESULTS:Of the 174 venous accesses placed, 108 were CVCs. 27 (25.0%) catheter tip cultures (CTC) were positive; 12 (11.1%) were catheter-related blood stream infections (CRBSI). Acinetobacter baumannii was the most prominent bacterial infection for both CTC- (55.56%) and CRBSI- (75.00%) positive catheters. CTC- and CRBSI-positive rates were higher during the emergency stage, and both dropped rapidly after reform measures (chi-square test, p = 0.003), and all were negative after the no.8 catheters. Accumulative regression results indicated that total body surface area burned (TBSA), number of catheters, days of indwelling, and bloodstream infections were independently associated with CTC results, while gender and number of catheters were independently associated with CRBSI results. 1 femur vein thrombosis was detected and cured.CONCLUSIONS:Bedside ultrasound and professional IV team for CVC management are pivotal for massive burn victims. Their intervention helps control CVC-related infections and other complications. A. baumannii was the most frequent bacterial infection found in both CTC- and CRBSI-positive catheters. Several most important factors associated with catheter-related infections were concluded. This information alerts us to watch for patients with such warning factors.

Abstract:Concrete is the most commonly used alley flooring in confined dairy herds because of its qualities of construction and ease of cleaning. Nevertheless, the hardness, abrasiveness, and slipperiness of concrete floors have adverse effects on animal well-being and health, and yielding rubber flooring is becoming popular as a way of improving the flooring conditions on walkways. The aim of this study was to investigate preferences of dairy cows for rubber compared with concrete flooring under the conditions of a commercial dairy farm. The study was conducted in an organic dairy herd with free-stall housing. Floor preference was tested on groups of standing cows in a 120-m2 holding pen before milking, and 1 yr later on a 12- x 3-m walkway. The holding pen and the walkway were divided lengthwise into 2 identical sections. Two types of solid rubber mats (soft and extra soft) were tested against solid concrete in the holding pen. Slatted and solid rubber mats were tested against slatted concrete in the walkway. Each floor type was tested over 4 d on the left side and 4 d on the right side of the holding pen and the walkway, respectively. Concrete flooring on both sides of the sections was tested as a control before the testing of different section materials. All observations of the distribution of cows in the sections were made from video recordings captured in association with the afternoon milking. The number of cows on each section was recorded approximately every 7 min in the holding pen, and continuously on the walkway. A significantly higher proportion of cows stood on the side with the soft and extra soft rubber mats (65.1 +/- 2.7 and 69.3 +/- 2.6%, respectively, mean +/- SEM) compared with the control distribution when only the solid concrete was available (50.9 +/- 3.9%). A significantly higher proportion of nonlame cows walked exclusively on the side with the slatted (64.5 +/- 5.4%, d 4) or solid rubber mats (68.2 +/- 5.1%, d 4) compared with controls (28.9 +/- 4.3%). Lame cows within a group of walking cows did not show a higher preference for soft flooring as distinct as nonlame cows (52.7 +/- 6.9 and 59.4 +/- 6.2% for the solid and slatted rubber mats, respectively, at d 4 vs. 40.3 +/- 6.2% for control), presumably because of competition with other, higher ranked cows. It was concluded that the majority of cows preferred to walk and stand on soft rubber rather than on concrete flooring.

Abstract:Catheter‐related deep vein thrombosis (CRT) is one of the major complications that may occur among patients requiring indwelling venous catheters and can lead to potentially life‐threatening complications. The risk of CRT is a result of multiple risk factors and despite constant advances in catheter materials and technologies, its incidence is still considerable. Available evidence on the management of CRT remains scarce and controversial. The development of new anticoagulant drugs against contact pathway activation may offer further advantages in this setting. Background Catheter‐related thrombosis (CRT) is one of the major complications affecting patients with indwelling venous catheters, usually involving the upper extremity deep venous system. This condition can lead to potentially life‐threatening complications such as pulmonary embolism and sepsis. The risk of developing CRT varies depending on type of catheters and patient characteristics. Despite advances in materials and technologies, the actual incidence of CRT is still considerable. Available evidence on CRT management remains controversial, and clinical guidelines base their recommendations on data from non‐catheter related upper extremity or lower extremity deep venous thromboses. Aims This narrative review aims to describe the epidemiology of CRT, to review the available evidence on its management and to highlight the current unmet needs. Methods No formal search strategy was applied for the revision of the literature. The main sources of information used were Medline and guidelines from international societies. Content The management of CRT requires a careful balance between the risk of thrombus progression, recurrent events, and systemic embolization and the increased bleeding risk in often fragile patients. Open issues include the optimal management of the catheter and the type and duration of anticoagulant therapy. Direct oral anticoagulants are increasingly prescribed, representing an important alternative to the standard of care low molecular weight heparins in selected cases. The development of new anticoagulant drugs such as factors XI and XII inhibitors may offer further advantages in this context. Conclusions The management of CRT is still challenging with constant need for updated evidence to support tailored approaches.
