Abstract:In Canada, as in many other countries, private dental insurance addresses financial barriers to a great extent thereby facilitating access to dental care. That said, insurance does not guarantee affordability, as there are issues with the quality and level of coverage of insurance plans. As such, individuals facing barriers to dental care experience poorer oral health. Therefore, it is important to examine more keenly the socio-demographic attributes of people with private insurance to particularly identify those, who despite having insurance, face challenges in accessing dental care and experience poorer oral health.

Abstract:Introduction: This study investigated parents′ perspectives on access to oral health care and the Interim Canada Dental Benefit (CDB). In the context of Canada′s national health insurance, which historically excluded dental care, the introduction of the Interim CDB in October 2022 represented a paradigm shift towards enhancing dental care accessibility for children under 12 years of age from lower-income families. Methods: This study analyzed aggregate and de-identified data from the comprehensive online survey conducted by The Strategic Counsel for Health Canada, involving 2,203 parents from across Canada. The survey was administered in March of 2023. Paired/overlap t-test for means and paired/overlap z-test for percentages were performed, with statistical significance at p ≤ 0.05. Results: The majority of participants expressed concerns regarding the costs (90.9%) and accessibility (80.9%) of dental care, indicated that regular dental visits for children is important (97.2%), and would take their children more frequently to dental appointments if had extra money (79.9%). Some of the barriers preventing regular dental visits for children included costs of service and transportation and lack of insurance. The majority of parents showed support for the Interim CDB (87%), with the greatest support coming from the provinces of Manitoba and Saskatchewan (90.4%). Conclusion: This research underscores the imperative for ongoing evaluation and policy refinement to ensure the CDCP effectively addresses the nuanced needs of Canadian families, fostering a more inclusive and accessible dental care system. Parents′ concerns regarding dental care and their support for the Interim CDB signal a clear mandate for improving program outreach and accessibility through the Canadian Dental Care Plan (CDCP).
Enhancing seniors' dental care access: Analyzing the impact of government insurance in Canada

Abstract:A crucial policy question for the government is whether publicly funded insurance programs effectively improve access to care. Using 2015 and 2018 Canadian Community Health Survey (CCHS) data, we first estimated the effect of government dental insurance for seniors on promoting regular care access and lowering cost barrier. When controlling for individual heterogeneity, we found that having government coverage is associated with significantly lower probability of reporting avoidance of dental care due to cost compared to having no coverage. This effect is comparable with other types of insurance. However, the impact of the government program on regular access to dental care is modest. Secondly, using a portion of data collected in Alberta, we found that the government plan does not increase the overall coverage rate. Moreover, switching from an employer-based plan to government-provided coverage for seniors reduces the probability of regular access to care and increases the probability of experiencing cost barrier. This finding indicates that without expansion of overall coverage rate, the current government dental program may not be generous enough to offset the negative impact of leaving the employer-based plan.

Abstract:Purpose: This analysis aims to evaluate the association between the time since and reason for a patient's last dental appointment across clinical oral health outcomes. Materials and methods: We used data from the 2017-2020 National Health and Nutrition Examination Survey (NHANES), a cross-sectional, nationally-representative survey of noninstitutionalized US adults. The predictors were the time since and the reason for the last dental appointment (routine vs. urgent). We examined the presence and number of missing teeth and teeth with untreated coronal and root caries. Multivariable regression models with interaction were used to assess the association between the time since the last dental appointment and clinical oral health outcomes among routine and urgent users separately. Results: Two-thirds of the US population had a dental appointment within a year, while nearly 44 million individuals did not visit a dentist for the last three years. The odds of having teeth with untreated coronal or root caries increased with the length of time since the last appointment, and urgent users had worse dental outcomes compared to routine users. Compared to those who had a dental appointment within a year, individuals who had their last dental appointment more than 3 years ago had 2.94 times the average number of teeth with untreated caries among routine users (95%CI=2.39, 3.62) and 1.60 times the average among urgent users (95%CI=1.05, 2.43). Conclusions: Recent, routine dental appointments are associated with improved oral health outcomes. The outcomes reiterate how social determinants of health impact access to oral health care and oral health outcomes.

Abstract:Introduction In the United States of America, early childhood caries (ECC) is the most common chronic childhood disease of early onset, with dental caries being the most prevalent chronic disease among children aged 6-19 years. Children without an established medical home, from low-income households, and who are uninsured have historically shown to be prone to dental caries and attribute to higher health care costs. Early recognition of these risk factors by a pediatrician helps prevent the development of medical and psychosocial complications in the child. Methods The cross-sectional data of the prevalence of dental caries and dental treatment trends in children and three socioeconomic risk factors, namely establishment of a medical home, household income, and child's health insurance, were accessed from the National Survey of Children's Health (NSCH) for the years 2016-2019. The association of the risk factors with the prevalence of dental caries and with the prevalence of dental treatment were analyzed using two-sample proportion tests and chi-square (χ<sup>2</sup>) tests for dichotomous categorical variables and non-dichotomous categorical variables, respectively. Standardized residuals were calculated and analyzed as well. Furthermore, the odds ratios were calculated and utilized to quantify the influence of each category on the highly associated category with having teeth decay and not receiving dental treatment under each socioeconomic risk factor. Results The results of this study revealed that the three socioeconomic factors considered have statistically significant associations with tooth decay and dental treatment. The prevalence and associative risk of tooth decay and untreated caries were the highest in the children without a medical home. Additionally, the odds of having tooth decay was >50% higher for the children from the lowest household income category (0-99% federal poverty level [FPL]) compared to those from the high household income categories (200-399% FPL and >400% FPL). Public insurance coverage was associated with the highest prevalence of dental caries and not receiving fluoride treatment. Furthermore, the likelihood of not availing dental treatment is nearly two times or more higher for the uninsured children than children having public insurance, or private insurance, or a combination of both. Conclusion Our study findings reveal that children belonging to certain socioeconomic risk categories are at a higher risk of developing dental caries and not receiving dental treatment. As a consequence, the study implies that increased support and expansion of public health insurance will benefit oral health care for the children. Pediatricians play an integral part in developing a medical home for the child by providing preventative dental care and establishing continued care through dental referrals.

Abstract:This study identified the specific factors which influence the treatment of gum disease. Specifically, the aims are to examine i) the differences in the prevalence of self-reported periodontitis treatment in 2011–2012 and 2017–2018, ii) the association between the background characteristics of respondents and treatment of gum disease, iii) the effects of the background characteristics on the treatment of gum disease and iv) reasons for the lack of access to dental care in 2011–2012 and 2017–2018. Data from the National Health an Nutrition Examination Survery (NHANES) oral health questionnaires 2011-2012 and 2017-2018 were used. NHANES is a periodic survey conducted by the National Center for Health Statistics of the Centers for Disease Control and Prevention. This data represents a stratified, multistage probability sample of the civilian noninstitutionalized population in the 50 United States U.S) and the District of Columbia. Binary multivariable logistic regression was used to examine the socioeconomic predictors of self-reported therapy for gum disease. Compared to 2011-2012, more participants accessed oral health care services during 2017-2018, resulting in fewer people seeking treatment for gum disease. Affordability and lack of dental health insurance were the primary reasons for not receiving treatment for gum disease. Compared with Whites, Asians were more likely to report having had treatment, followed by Hispanics and Blacks. Increasing oral health services for the elderly could improve access to care related to periodontal disease in this cohort of patients. In addition, the expansion of dental coverage with reduced out-of-pocket expenditure could improve access to dental services and overall health.

Abstract:Objectives To evaluate access to dental care for children in the United States. Methods The study population included children in 48 states and the District of Columbia. Using multiple data sources, dental care access was estimated at the community level by matching dental care supply and demand using mathematical modeling accounting for access constraints. Outcome measures included percent‐met demand, travel distance, and percentage of underserved and unserved communities. Multiple scenarios to improve Medicaid/CHIP participation of dentists were evaluated. Results Medicaid‐insured and CHIP‐insured children exhibited lower access compared to those privately insured. The percent‐met demand was lower than 50% for Medicaid‐insured children and CHIP‐insured children for 42 and 34 states, respectively. Percent‐met demand was higher than 50% for private‐insured children except for Texas and West Virginia. Increasing Medicaid/CHIP participation of dentists resulted in improving access for public‐insured children. At 100% Medicaid/CHIP participation, all states exhibited different degrees of percent‐met demand increase for publicly insured children, from 7% to 46%. The percent‐met demand across all children ranged in 23.8%–82.9% under 70% participation rate versus 22%–83% under 100% participation rate. No single participation rate improved access for all children uniformly across all states. Conclusions This study found that dental care access was lower for children with public insurance than those with private access across all states, although states responded differently to changes in Medicaid/CHIP participation. Increasing access for children with public insurance would reduce disparities, but overall children's access to dental care would be better improved by expanding the oral health workforce.

Abstract:Background: Socioeconomic variables continue to play a major role in the global persistence of dental health inequities. The purpose of this study is to evaluate the relationship between socioeconomic characteristics and dental health outcomes at a tertiary-care facility over a 10-year period. Methods: Patient demographics, socioeconomic factors, and dental health outcomes were analyzed retrospectively from dental health records. To determine the relationships between socioeconomic characteristics and disparities in oral health, statistical studies were carried out. Results: Tooth cavities and untreated dental problems were more common in people with lower incomes and educational levels. There were notable differences in oral health outcomes between various socioeconomic groups. Conclusion: In conclusion, this research shows that there is a persistent relationship at a tertiary-care facility between socioeconomic characteristics and disparities in oral health. Reducing disparities in dental health outcomes and advancing oral health equity require addressing socioeconomic causes.
Examining the Effect of Income-Based Inequalities and Dental Service Provision on Dental Service Utilization among Older Australians: A Multiple Mediation Analysis

Abstract:Introduction: Public service provision is one of the keys to reducing inequalities in the utilization of dental services. Given the increase in the aging population, there should be a focus on older adults' oral health. However, this is often overlooked. Objectives: This study investigates the effectiveness of public services in reducing income-related inequalities in dental service utilization among older South Australians. Methods: A multiple counterfactual mediation analysis using the ratio of mediator probability weighting approach was used to explore the proposed mediation mechanism using a South Australian population of older adults (≥65 y). The exposure variable in the analysis was income, and the mediators were concession cards and the last dental sector (public or private). The outcome variable was the time of last dental visit. Results: Half of the older adults with high income (≥$40,000) owned a concession card, and 10% of those who attended public dental services belonged to this group. Interestingly, only 16.3% of the study participants had visited the public dental sector at their last dental appointment. Results showed a negligible indirect effect (odds ratio [OR], 0.99; 95% confidence interval [CI], 0.85-1.05) and a significant direct effect (OR, 3.09; 95% CI, 2.24-4.87). By changing the potential outcome distributions to the counterfactual exposure distributions and taking the mediators' distribution as a counterfactual exposure distribution, the odds of dental visits occurring before the past 12 mo approximately tripled for low-income compared to high-income individuals. Conclusion: Income inequalities were associated with relatively delayed dental visits in older South Australians, and provision of public services could not improve this pattern. This might happen due to inequitable access to concession cards and public services. A review of policies is required, including addressing income inequalities and implementing short-term approaches to improve service utilization patterns in older South Australians. Knowledge transfer statement: The findings of this study can enable policymakers for informed decision-making about the provision of public dental services for older Australians. This study emphasizes the importance of reviewing the current public dental services and subsidies and implementing short-term approaches to reduce income inequalities for older Australians.

Abstract:The objective of this study was to quantify the magnitude of absolute and relative oral health inequality in countries with similar socio-political environments, but differing oral health care systems such as Canada, the United States (US), and the United Kingdom (UK), in the first decade of the new millennium. Clinical oral health data were obtained from the Canadian Health Measures Survey 2007–2009, the National Health and Nutrition Examination Survey 2007–2008, and the Adult Dental Health Survey 2009, for Canada, the US and UK, respectively. The slope index of inequality (SII) and relative index of inequality (RII) were used to quantify absolute and relative inequality, respectively. There was significant oral health inequality in all three countries. Among dentate individuals, inequality in untreated decay was highest among Americans (SII:28.2; RII:4.7), followed by Canada (SII:21.0; RII:3.09) and lowest in the UK (SII:15.8; RII:1.75). Inequality for filled teeth was negligible in all three countries. For edentulism, inequality was highest in Canada (SII: 30.3; RII: 13.2), followed by the UK (SII: 10.2; RII: 11.5) and lowest in the US (SII: 10.3; and RII: 9.26). Lower oral health inequality in the UK speaks to the more equitable nature of its oral health care system, while a highly privatized dental care environment in Canada and the US may explain the higher inequality in these countries. However, despite an almost equal utilization of restorative dental care, there remained a higher concentration of unmet needs among the poor in all three countries.

Abstract:The study of the quantum behaviour of Josephson junctions is interesting not only by itself, but also for the technological importance in electronics employing superconducting weak links. Recently great attention has been devoted to the effects of Resonant Macroscopic Quantum Tunneling (RMQT) between levels the same energy in neighbouring wells of the potential shape describing the junction. This can produce voltage spikes along the supercurrent branch of the I-V curves and peaks in the current switching distributions. Schmidt, Clealand, and Clarke have presented a detailed analysis of the resonant tunnel dynamics for the T=0 case. The present paper is based on a different theoretical approach, first proposed by Larkin and Ovchinnikov, which allows one to include explicitly the temperature. We study the phenomenon in its time dependence and this allows us to consider the lifetime of the voltage spikes as well as the effect of the RMQT on the quantum escape rate out of the V=0 state.<<ETX>>

Abstract:Background In the early 2020s, nearly half of New Zealand adults reported that cost of treatment had prevented them from accessing dental care, with higher rates among Māori, Pasifika and individuals living in the most deprived areas. Unaffordable dental care may be explained by a rise in dental service fees over time relative to personal income, as documented in New Zealand between 1978 and 1993. However, there have been no contemporary estimates in New Zealand of how the affordability of dental care has changed. The aims of this study were to analyse the change in dental treatment fees and the personal income of New Zealanders from 1978 to 2023 and to explore differences in affordability of dental care by ethnicity. Methods Average fees for dental treatments were sourced from surveys completed by practising New Zealand dentists. Earnings (from 1978) and personal income data (full population from 2000 and by ethnicity from 2008) were sourced from Statistics NZ and NZ Official Yearbooks. Inflation‐adjusted changes in average fees, weekly personal earnings and income were calculated as a percentage change from 1978 levels for fees and earnings and from 2000 for personal income. Results For the five dental treatments with data available from 1978 to 2023, fees increased in the range of 75%–236%, while earnings increased by 46% over the same period. Fees for other treatments (with data available from 1981 to 2009) similarly increased and mostly surpassed changes in earnings. From 2008 to 2023 the overall increase in personal income (about 21% across all ethnic groups) kept pace with the rising cost of most treatments. However, due to persistent income inequalities, in 2023, Māori and Pasifika would need to spend a higher proportion of their weekly income (approximately 16% and 23% respectively) to receive the same dental treatments as NZ Europeans. Conclusions Fees for dental treatments have risen markedly in recent decades, more sharply than the price of other goods and services.

Abstract:Objectives To assess the association between dental insurance, dental care utilization and oral health‐related impact on daily activities among patients visiting Health Resources and Services Administration (HRSA)‐funded health centers in the United States. Methods This cross‐sectional study examined the 2014 Health Center Patient Survey (HCPS), a survey of patients who received care at US health centers. Logistic regression analyses were conducted to explore the likelihood of the oral health‐related impact on daily activities domains such as school/work performance, sleeping, eating/chewing, social and home activities considering sociodemographic, dental insurance, and dental care utilization. Results Among 7002 total participants, 6890 self‐reported oral health‐related impact on daily activities. Adults aged 18–44 were 2.6 times more likely to report an overall impact on everyday life activities compared to children and adolescents. After adjusting for sociodemographics, Hispanic patients were less likely to report any oral health‐related impacts, while patients visiting rural centres reported higher odds of oral health related impairment. Patients who experienced delays in receiving dental care or were unable to access dental care when needed were 4.5 times more likely to encounter difficulties in their daily activities. Conclusions This study linked limited dental insurance and care to increased oral health issues impacting daily life, particularly among adults, rural populations, and those facing delay in dental care. This highlights the need for policy reforms and interventions aimed at improving dental care access to mitigate the detrimental effects of poor oral health on daily functions for vulnerable populations.

Abstract:Background: The financial burden of oral diseases is a growing concern as the medical expenses rise worldwide. The aim of this study was to investigate the dental expenditure, analyze its progressivity and horizontal inequality under the general health finance and insurance system, and identify the key social determinants of the inequality for Chinese adults. Methods: A secondary analysis used the data of 13,464 adults from the 4 th National Oral Health Epidemiological Survey (NOHES) in China was undertaken. The dental expenditure was collected and divided into out-of-pocket and health insurance payments. Horizontal inequality index and Kakwani index were used to analyze the horizontal inequality and progressivity, respectively. The decomposition model of the concentration index was set up to explore the associated socioeconomic determinants. Results: The results showed that a mean dental expenditure per capita of Chinese adults was $20.55 (95% Confidence Interval-CI: 18.83,22.26). Among those who actually used dental service, the cost was $100.95 (95%CI: 93.22,108.68). Over 90% of dental spending was due to out-of-pocket expenses. For self-reported oral health, the horizontal inequality index was -0.1391 and for the decayed tooth (DT), it was -0.2252. For out-of-pocket payment, the Kakwani index was -0.3154 and for health insurance payment it was -0.1598. Income, residential location, educational attainment, oral hygiene practice, self-reported oral health, age difference were the main contributors to the inequality of dental expenditure. Conclusion: Dental expenditure for Chinese adults was at a lower level due to underutilization. The ratio of payments of dental expenditure and utilization was disproportional, whether it was out-of-pocket or insurance payment. Individuals who were more in need of oral care showed less demand for service or not required service in time. For future policy making on oral health, it is worth the effort to further promote the awareness of the importance of oral health and utilization of dental service.
Access to dental care for persons with developmental disabilities in Ontario

Abstract:Objectives: This study was undertaken to determine the proportion of persons primarily with developmental disabilities who encounter difficulties accessing dental care in Ontario, to identify perceived barriers to accessing dental care and to determine if persons with disabilities and their caregivers believe that oral health is important. Methods: Community organizations providing services mainly to persons with developmental disabilities in Ontario were recruited to circulate a questionnaire to their members by mail or the Internet. Fourteen organizations mailed out a total of 1,755 paper questionnaires in autumn 2006, of which 420 (23.9%) were returned; in addition, 236 Internet questionnaires were returned. Results: Of the 656 paper and Internet responses, 634 were deemed valid. Most of the respondents had developmental disabilities. Almost three-quarters of respondents (464 [73.2%] ) reported being able to access dental services in Ontario. Personal (internal) factors were more likely to represent barriers to dental care than external factors. Conclusions: The majority of persons with disabilities and most caregivers believed that oral health is important for overall health.

Abstract:Objectives This study aimed to assess the association between affordability in terms of difficulty paying dental bills in Australian dollars and dental service use in the presence of sociodemographic confounders, and to assess the role of dental anxiety and satisfaction with dental professionals as mediators. The second aim was to investigate how dental anxiety and satisfaction with dental professionals modify the association between affordability and use of dental services in Australian adults. Methods Longitudinal data from the Australian National Study of Adult Oral Health (2004–06 and 2017–18) was used. Poisson regression and path analysis were conducted to determine the association between affordability and frequency of use of dental services. Effect measure modification (EMM) analysis was performed by stratification of dental anxiety and satisfaction with dental professionals. Results The study included 1698 Australian adults and identified that the prevalence of low frequency of dental visits was 20% more for those who had difficulty paying dental bills. Adults with dental anxiety (prevalence ratio [PR] = 1.14) and those who were dissatisfied with dental professionals (PR = 1.17) had a higher prevalence of low frequency of dental visits in the presence of difficulty paying dental bills. This indicated that dental anxiety and dissatisfaction with dental professionals were effect modifiers on this pathway. Conclusions Adults who experience dental anxiety and dissatisfaction with dental professionals are more likely to avoid dental visits when faced with difficulty paying dental bills. However, it is important to note that these associations do not necessarily imply a causal relationship.
