Abstract:Background Traditional immunohistochemistry assessment of Ki-67 in breast cancer (BC) via core needle biopsy is invasive, inaccurate, and nonrepeatable. While machine learning (ML) provides a promising alternative, its effectiveness depends on extensive data. Although the current mainstream MRI-centered radiomics offers sufficient data, its unsuitability for repeated examinations, along with limited accessibility and an intratumoral focus, constrain the application of predictive models in evaluating Ki-67 levels. Objective This study aims to explore ultrasound (US) image-based radiomics, incorporating both intra- and peritumoral features, to develop an interpretable ML model for predicting Ki-67 expression in BC patients. Methods A retrospective analysis was conducted on 263 BC patients, divided into training and external validation cohorts. From intratumoral and peritumoral regions of interest (ROIs) in US images, 849 distinctive radiomics features per ROI were derived. These features underwent systematic selection to analyze Ki-67 expression relationships. Four ML models-logistic regression, random forests, support vector machine (SVM), and extreme gradient boosting-were formulated and internally validated to identify the optimal predictive model. External validation was executed to ascertain the robustness of the optimal model, followed by employing Shapley Additive Explanations (SHAP) to reveal the significant features of the model. Results Among 231 selected BC patients, 67.5% exhibited high Ki-67 expression, with consistency observed across both training and validation cohorts as well as other clinical characteristics. Of the 1698 radiomics features identified, 15 were significantly correlated with Ki-67 expression. The SVM model, utilizing combined ROI, demonstrated the highest accuracy [area under the receiver operating characteristic curve (AUROC): 0.88], making it the most suitable for predicting Ki-67 expression. External validation sustained an AUROC of 0.82, affirming the model’s robustness above a 40% threshold. SHAP analysis identified five influential features from intra- and peritumoral ROIs, offering insight into individual prediction. Conclusion This study emphasized the potential of SVM model using radiomics features from both intra- and peritumoral US images, for predicting elevated Ki-67 levels in BC patients. The model exhibited strong performance in validations, indicating its promise as a noninvasive tool to enable personalized decision-making in BC care.

Abstract:To formulate a predictive model for assessing Ki-67 expression in breast cancer by integrating pre-treatment ultrasound features with non-morphological magnetic resonance imaging (MRI) parameters, encompassing functional and hemodynamic indicators. A retrospective study was conducted on 167 patients. All patients underwent a breast mass biopsy for histopathological and Ki-67 analysis prior to neoadjuvant chemotherapy (NAC) treatment. Additionally, all patients underwent ultrasonography and MRI examinations prior to the biopsy. The recorded variables were Ki-67, apparent diffusion coefficient (ADC) values, Max Slope, time to peak (TTP), signal enhancement ratio (SER), early enhancement rate (EER), time-signal intensity curve (TIC), tumor maximum diameter, tumor margins and boundaries, aspect ratio, microcalcification, color Doppler flow imaging grading, resistance index (RI), and axillary lymph node metastasis. Statistical analysis was performed using the R software package. Normally distributed continuous data are presented as mean ± standard deviation (SD), skewed continuous data as median, and categorical variables as frequency or percentage. The dataset was randomly divided into a modeling group and a validation group following a 7:3 ratio, employing a predetermined random seed. The selection of variables was conducted using the random forest algorithm. Specifically, in the initial analysis, we trained a random forest model using all available variables. By evaluating the Gini importance scores of each variable, we identified those that contributed the most to predicting Ki-67 expression. The predictive model for Ki-67 expression was constructed using selected variables: Maximum Diameter, ADC value, SER value, Max Slope value, TTP value, and EER value. Within the validation group, the evaluation metrics demonstrated an Area under the curve of 0.961 with a 95% confidence interval ranging from 0.865 to 0.995. The model achieved a kappa score of 1.00, precision of 0.949, recall of 1, an F1 score of 0.974, sensitivity of 100%, specificity of 85.71%, a positive predictive value of 94.87%, and a negative predictive value of 100%. The combination of non-morphological MRI parameters and pre-treatment ultrasound features in a breast cancer prediction model powered by RF machine learning demonstrated favorable clinical outcomes and improved diagnostic performance.

Abstract:Objective: The aim of this study was to develop and validate an ultrasound-based radiomics nomogram model by integrating the clinical risk factors and radiomics score (Rad-Score) to predict the Ki-67 status in patients with breast carcinoma. Methods: Ultrasound images of 284 patients (196 high Ki-67 expression and 88 low Ki-67 expression) were retrospectively analyzed, of which 198 patients belonged to the training set and 86 patients to the test set. The region of interest of tumor was delineated, and the radiomics features were extracted. Radiomics features underwent dimensionality reduction analysis by using the independent sample t test and least absolute shrinkage and selection operator (LASSO) algorithm. The support vector machine (SVM), logistic regression (LR), decision tree (DT), random forest (RF), naive Bayes (NB) and XGBoost (XGB) machine learning classifiers were trained to establish prediction model based on the selected features. The classifier with the highest AUC value was selected to convert the output of the results into the Rad-Score and was regarded as Rad-Score model. In addition, the logistic regression method was used to integrate Rad-Score and clinical risk factors to generate the nomogram model. The leave group out cross-validation (LGOCV) method was performed 200 times to verify the reliability and stability of the nomogram model. Results: Six classifier models were established based on the 15 non-zero coefficient features. Among them, the LR classifier achieved the best performance in the test set, with the area under the receiver operating characteristic curve (AUC) value of 0.786, and was obtained as the Rad-Score model, while the XGB performed the worst (AUC, 0.615). In multivariate analysis, independent risk factor for high Ki-67 status was age (odds ratio [OR] = 0.97, p = 0.04). The nomogram model based on the age and Rad-Score had a slightly higher AUC than that of Rad-Score model (AUC, 0.808 vs. 0.798) in the test set, but no statistical difference (p = 0.144, DeLong test). The LGOCV yielded a median AUC of 0.793 in the test set. Conclusions: This study proposed a convenient, clinically useful ultrasound radiomics nomogram model that can be used for the preoperative individualized prediction of the Ki-67 status in patients with BC.

Abstract:Rationale and Objectives Traditional Ki-67 evaluation in breast cancer (BC) via core needle biopsy is limited by repeatability and heterogeneity. The automated breast ultrasound system (ABUS) offers reproducibility but is constrained to morphological and echoic assessments. Radiomics and machine learning (ML) offer solutions, but their integration for improving Ki-67 predictive accuracy in BC remains unexplored. This study aims to enhance ABUS by integrating ML-assisted radiomics for Ki-67 prediction in BC, with a focus on both intratumoral and peritumoral regions. Materials and Methods A retrospective analysis was conducted on 936 BC patients, split into training ( n = 655) and testing ( n = 281) cohorts. Radiomics features were extracted from intra- and peritumoral regions via ABUS. Feature selection involved Z-score normalization, intraclass correlation, Wilcoxon rank sum tests , minimum redundancy maximum relevance, and least absolute shrinkage and selection operator logistic regression. ML classifiers were trained and optimized for enhanced predictive accuracy. The interpretability of the optimized model was further augmented by employing Shapley additive explanations (SHAP). Results Of the 2632 radiomics features in each patient, 15 were significantly associated with Ki-67 levels. The support vector machine (SVM) was identified as the optimal classifier, with area under the receiver operating characteristic curve values of 0.868 (training) and 0.822 (testing). SHAP analysis indicated that five peritumoral and two intratumoral features, along with age and lymph node status, were key determinants in the predictive model. Conclusion Integrating ML with ABUS-based radiomics effectively enhances Ki-67 prediction in BC, demonstrating the SVM model's strong performance with both radiomics and clinical factors.

Abstract:Rationale and objectives: Current radiomics research primarily focuses on intratumoral regions and fixed peritumoral areas, lacking optimization for accurate Ki-67 prediction. This study aimed to develop machine learning (ML) models to analyze radiomic features from Automated Breast Volume Scanning (ABVS) images of different peritumoral region sizes to identify the optimal size for accurate preoperative Ki-67 prediction. Materials and methods: A total of 668 breast cancer patients were enrolled and divided into training (486) and testing (182) cohorts. In the training cohort, ML models were developed for intratumoral and peritumoral regions (2, 4, 6, 8, and 10 mm). Relevant Ki-67 features for each ROI were identified, and different models were compared to determine the optimal one. These models were validated using a testing cohort to find the most accurate peritumoral region for Ki-67 prediction. SHAP (Shapley Additive Explanations) analysis was performed to identify key radiomic features from the optimal model. Results: The Extreme Gradient Boosting (XGBoost) model for the intratumoral region combined with the 6 mm peritumoral region achieved the highest predictive accuracy, with an AUC of 0.957 in the training cohort and 0.920 in the testing cohort. Calibration curves confirmed reliability, and decision curve analysis demonstrated the highest net benefit. SHAP indicated that 6 mm peritumoral radiomic features are more significant than intratumoral features. Conclusion: The XGBoost model using ABVS-derived radiomic features from both the intratumoral and 6 mm peritumoral regions provides the most accurate preoperative Ki-67 prediction.

Abstract:Objective Early and accurate prediction of axillary lymph node metastasis (ALNM) is crucial in determining appropriate treatment strategies for patients with early-stage breast cancer. The aim of this study was to evaluate the efficacy of radiomic features extracted from ultrasound (US) images combined with machine learning (ML) methods in predicting ALNM to improve diagnostic accuracy and patient prognosis. Methods In this retrospective study, data of 282 early-stage breast cancer patients from two centers were analyzed. We considered clinicopathological characteristics, conventional US features, contrast-enhanced ultrasound (CEUS) characteristics, and radiomics features. Radiomics features were extracted from US images, and using least absolute shrinkage and selection operator (LASSO) regression, 12 key features were selected to compute a Radiomics score (Rad-score). A nomogram was developed based on these features, alongside five ML models: Logistic Regression (LR), Naive Bayes (NB), Support Vector Machine (SVM), K-Nearest Neighbors (KNN), and Extreme Gradient Boosting (XGBoost). Model performance was evaluated using metrics such as the area under the curve (AUC), accuracy (ACC), sensitivity (SEN), specificity (SPE), negative predictive value (NPV), and positive predictive value (PPV). Results Both the nomogram and ML models, including the Rad-score combined with histologic type, significantly predicted ALNM. Among all models, the XGBoost model showed the best performance with an AUC of 0.810 and an accuracy of 84.1% in the external test set, surpassing the nomogram and other ML models. SHapley Additive exPlanations (SHAP) analysis further provided insights into the influence of individual radiomics features on ALNM prediction. Conclusions While the nomogram provides a useful traditional statistical approach, integrating radiomics features with ML, particularly the XGBoost model enhanced by SHAP interpretability, offers superior predictive accuracy for ALNM in early-stage breast cancer patients.
Radiomics Features On Ultrasound Imaging For The Prediction Of Disease-Free Survival In Triple Negative Breast Cancer: A Multi-Institutional Study

Abstract:Objectives: To explore the predictive value of radiomics nomogram using pretreatment ultrasound for disease-free survival (DFS) after resection of triple negative breast cancer (TNBC).Methods and materials: A total of 486 TNBC patients from 3 different institutions were consecutively recruited for this study. They were categorized into the primary cohort (n = 216), as well as the internal validation cohort (n = 108) and external validation cohort (n = 162). In primary cohort, least absolute shrinkage and selection operator logistic regression algorithm was used to select recurrence-related radiomics features extracted from the breast tumor and peritumor regions, and a radiomics signature was constructed derived from the grayscale ultrasound images. A radiomic nomogram integrating independent clinicopathological variables and radiomic signature was established with uni- and multivariate cox regressions. The predictive nomogram was validated using an internal cohort and an independent external cohort regarding abilities of discrimination, calibration and clinical usefulness.Results: The patients with higher Rad-score had a worse prognostic outcome than those with lower Rad-score in primary cohort and two validation cohorts (All p < 0.05). The radiomics nomogram indicated more effective prognostic performance compared with the clinicopathological model and tumor node metastasis staging system (p < 0.01), with a training C-index of 0.75 (95% confidence interval (CI), 0.71-0.80), an internal validation C-index of 0.73 (95% CI, 0.69-0.78) and an external validation 0.71 (95% CI,0.66-0.76). Moreover, the calibration curves revealed a good consistency for survival prediction of the radiomics model.Conclusions: The ultrasound-based radiomics signature was a promising biomarker for risk stratification for TNBC patients. Furthermore, the proposed radiomics modal integrating the optimal radiomics features and clinical data provided individual relapse risk accurately.Advances in knowledge: The radiomics model integrating radiomic signature and independent clinicopathological variables could improve individual prognostic evaluation and facilitate therapeutic decision-making, which demonstrated the incremental value of the radiomics signature for prognostic prediction in TNBC.

Abstract:Background: Axillary lymph nodes (ALN) status serves as a crucial prognostic indicator in breast cancer (BC). The aim of this study was to construct a radiogenomic multimodal model, based on machine learning (ML) and whole-transcriptome sequencing (WTS), to accurately preoperative evaluate the risk of ALN metastasis (ALNM), drug therapeutic response and avoid unnecessary axillary surgery in BC patients. Methods: In this study, we conducted a retrospective analysis of 1078 BC patients from The Cancer Genome Atlas (TCGA), The Cancer Imaging Archive (TCIA), and Foshan cohort. These patients were divided into the TCIA cohort(N=103), TCIA validation cohort(N=51), Duke cohort(N=138), Foshan cohort(N=106), and TCGA cohort(N=680). Radiological features were extracted from BC radiological images and differentially expressed gene expression was calibrated using WTS technology. A support vector machine (SVM) model was employed to screen radiological and genetic features, and a multimodal model was established based on radiogenomic and clinical pathological features to predict ALNM and stratify. The accuracy of the model predictions was assessed using the area under the curve (AUC) and the clinical benefit was measured using decision curve analysis (DCA). Risk stratification analysis of BC patients was performed by gene set enrichment analysis (GSEA), differential comparison of immune checkpoint gene expression, and drug sensitivity testing. Results: For the prediction of ALNM, rad-score was able to significantly differentiate between ALN- and ALN+ patients in both the Duke and Foshan cohorts ( P <0.05). Similarly, the gene-score was able to significantly differentiate between ALN- and ALN+ patients in the TCGA cohort ( P <0.05). The radiogenomic multimodal nomogram demonstrated satisfactory performance in the TCIA cohort (AUC 0.82, 95% CI: 0.74-0.91) and TCIA validation cohort (AUC 0.77, 95% CI: 0.63-0.91). In the risk sub-stratification analysis, there were significant differences in gene pathway enrichment between high and low-risk groups ( P <0.05). Additionally, different risk groups may exhibit varying treatment responses to chemotherapy (including Doxorubicin, Methotrexate and Lapatinib) ( P <0.05). Conclusion: Overall, the radiogenomic multimodal model employs multimodal data, including radiological images, genetic and clinicopathological typing. The radiogenomic multimodal nomogram can precisely predict ALNM and drug therapeutic response in BC patients.

Abstract:Trimerization of [Pd(R2PCH2CH2PR2){1,2-C6H4(SiH2)2}] (1; where R Me or Et) has been demonstrated to produce a trinuclear complex 2where two of the Pd atoms can be readily characterized as PdII centers but the nature of the third, central Pd atom is less clear (Scheme 1, by-products not shown).[1] While this Pd atom (Pd1) is drawn in Scheme 1 as bonding directly to two Si atoms and further interacting with two Si Si bonds, the interatomic distances from the X-ray crystal-structure data could also be interpreted to be consistent with an absence of Si Si bonds and instead six Pd Si bonds,[1] that is, the central metal would formally be PdVI. Prediction of Ki‐67 of Invasive Ductal Breast Cancer Based on Ultrasound Radiomics Nomogram

Abstract:Purpose The objective of this research was to develop and validate an ultrasound‐based radiomics nomogram for the pre‐operative assessment of Ki‐67 in breast cancer (BC). Materials and Methods From December 2016 to December 2018, 515 patients with invasive ductal breast cancer who received two‐dimensional (2D) ultrasound and Ki‐67 examination were studied and analyzed retrospectively. The dataset was distributed at random into a training cohort (n = 360) and a test cohort (n = 155) in the ratio of 7:3. Each tumor region of interest was defined based on 2D ultrasound images and radiomics features were extracted. ANOVA, maximum correlation minimum redundancy (mRMR) algorithm, and minimum absolute shrinkage and selection operator (LASSO) were performed to pick features, and independent clinical predictors were integrated with radscore to construct the nomogram for predicting Ki‐67 index by univariate and multivariate logistic regression analysis. The performance and utility of the models were evaluated by plotting receiver operating characteristic (ROC) curves, decision curve analysis (DCA), and calibration curves. Results In the testing cohort, the area under the receiver characteristic curve (AUC) of the nomogram was 0.770 (95% confidence interval, 0.690–0.860). Abstract:Triple negative breast cancer (TNBC) is a breast cancer subtype with unfavorable prognosis. We aimed to establish a machine learning-based ultrasound radiomics model to predict disease-free survival (DFS) in TNBC. Invasive TNBC>T1b between January 2009 and June 2018 with preoperative ultrasound were enrolled and assigned to training and independent test cohort. Radiomics and clinicopathological features related with DFS were selected by univariate and multivariate regression analysis. Training cohort of combined features was resampled with SMOTEENN to balance distribution and put into classifiers. Areas Under Curves (AUCs) of models were compared by DeLong's test. 562 women were included with 68 DFS events observed. Twenty prognostic radiomics features were extracted. Machine learning model by Naïve Bayes combining radiomics, clinicopathological features, and SMOTEENN had an AUC of 0.86 (95% CI 0.84-0.88), with sensitivity of 74.7% and specificity of 80.1% in training cohort. In independent test cohort, this three-combination model delivered an AUC of 0.90 (95% CI 0.83-0.95), higher than models based on radiomics (AUC=0.69, P=0.016) or radiomics + SMOTEENN (AUC=0.73, P=0.019). Integrating machine learning radiomics model based on ultrasound and clinicopathological features can predict DFS events for TNBC patients.

Abstract:Background Preoperative assessment of lymphovascular invasion (LVI) in invasive breast cancer (IBC) is of high clinical relevance for treatment decision‐making and prognosis. Purpose To investigate the associations of preoperative clinical and magnetic resonance imaging (MRI) characteristics with LVI and disease‐free survival (DFS) by using machine learning methods in patients with IBC. Study Type Retrospective. Population Five hundred and seventy‐five women (range: 24–79 years) with IBC who underwent preoperative MRI examinations at two hospitals, divided into the training (N = 386) and validation datasets (N = 189). Field Strength/Sequence Axial fat‐suppressed T2‐weighted turbo spin‐echo sequence and dynamic contrast‐enhanced with fat‐suppressed T1‐weighted three‐dimensional gradient echo imaging. Assessment MRI characteristics (clinical T stage, breast edema score, MRI axillary lymph node status, multicentricity or multifocality, enhancement pattern, adjacent vessel sign, and increased ipsilateral vascularity) were reviewed independently by three radiologists. Logistic regression (LR), eXtreme Gradient Boosting (XGBoost), k‐Nearest Neighbor (KNN), and Support Vector Machine (SVM) algorithms were used to establish the models by combing preoperative clinical and MRI characteristics for assessing LVI status in the training dataset, and the methods were further applied in the validation dataset. The LVI score was calculated using the best‐performing of the four models to analyze the association with DFS. Statistical Tests Chi‐squared tests, variance inflation factors, receiver operating characteristics (ROC), Kaplan–Meier curve, log‐rank, Cox regression, and intraclass correlation coefficient were performed. The area under the ROC curve (AUC) and hazard ratios (HR) were calculated. A P‐value <0.05 was considered statistically significant. Results The model established by the XGBoost algorithm had better performance than LR, SVM, and KNN models, achieving an AUC of 0.832 (95% confidence interval [CI]: 0.789, 0.876) in the training dataset and 0.838 (95% CI: 0.775, 0.901) in the validation dataset. The LVI score established by the XGBoost model was an independent indicator of DFS (adjusted HR: 2.66, 95% CI: 1.22–5.80). Data Conclusion The XGBoost model based on preoperative clinical and MRI characteristics may help to investigate the LVI status and survival in patients with IBC. Abstract:To establish a predictive model incorporating conventional ultrasound, strain elastography and clinicopathological features for Ki-67 expression in small breast cancer (SBC) which defined as maximum diameter less than2 cm. In this retrospective study, 165 SBC patients from our hospital were allocated to a high Ki-67 group ( n = 104) and a low Ki-67 group ( n = 61). Multivariate regression analysis was performed to identify independent indicators for developing predictive models. The area under the receiver operating characteristic (AUC) curve was also determined to establish the diagnostic performance of different predictive models. The corresponding sensitivities and specificities of different models at the cutoff value were compared. Conventional ultrasound parameters (spiculated margin, absence of posterior shadowing and Adler grade 2–3), strain elastic scores and clinicopathological information (HER2 positive) were significantly correlated with high expression of Ki-67 in SBC (all p < .05). Model 2, which incorporated conventional ultrasound features and strain elastic scores, yielded good diagnostic performance (AUC = 0.774) with better sensitivity than model 1, which only incorporated ultrasound characteristics (78.85%vs. 55.77%, p = .000), with specificities of 77.05% and 62.30% ( p = .035), respectively. Model 3, which incorporated conventional ultrasound, strain elastography and clinicopathological features, yielded better performance (AUC = 0.853) than model 1 (AUC = 0.694) and model 2 (AUC = 0.774), and the specificity was higher than model 1 (86.89% vs. 77.05%, p = .001). The predictive model combining conventional ultrasound, strain elastic scores and clinicopathological features could improve the predictive performance of Ki-67 expression in SBC.

Abstract:Objectives: This study aimed to create and validate a radiomics nomogram for non-invasive preoperative Ki-67 expression level prediction in patients with bladder cancer (BCa) using contrast-enhanced CT radiomics features. Methods: A retrospective analysis of 135 patients was conducted, 79 of whom had high levels of Ki-67 expression and 56 of whom had low levels. For the dimensionality reduction analysis, the best features were chosen using the least absolute shrinkage selection operator and one-way analysis of variance. Then, a radiomics nomogram was created using multiple logistic regression analysis based on radiomics features and clinical independent risk factors. The performance of the model was assessed using the Akaike information criterion (AIC) value, the area under the curve (AUC) value, accuracy, sensitivity, and specificity. The clinical usefulness of the model was assessed using decision curve analysis (DCA). Results: Finally, to establish a radiomics nomogram, the best 5 features were chosen and integrated with the independent clinical risk factors (T stage) and Rad-score. This radiomics nomogram demonstrated significant correction and discriminating performance in both the training and validation sets, with an AUC of 0.836 and 0.887, respectively. This radiomics nomogram had the lowest AIC value (AIC = 103.16), which was considered to be the best model. When compared to clinical factor model and radiomics signature, DCA demonstrated the more value of the radiomics nomogram. Conclusion: Enhanced CT-based radiomics nomogram can better predict Ki-67 expression in BCa patients and can be used for prognosis assessment and clinical decision making.

Abstract:Sumanta Banerjee (suman5ban@yahoo.com) is best known for his book In the Wake of Naxalbari: A History of the Naxalite Movement in India (1980). The Maoists want a provision in the electoral rules to arm the voter with the right to reject a candidate, but if the voters are granted such a provision, will they allow them to participate in the elections, or still insist on boycotting them? It is high time the Maoists recognise that the vast majority of the Indian electorate, despite their disillusionment with the present political leadership, are not going to boycott elections. The Lok Sabha elections were inaugurated with a fanfare of bomb blasts, killing of security personnel and poll officials, burning of polling stations, and a sensational hijacking of a train, where the hostages were served sattu and biscuits before being let off after about four hours! This mixed display of violence and charity was a demonstration of the muscle power of the Communist Party of India (Maoist) [CPI (Maoist)], which had given a call for the boycott of elections. By disrupting the electoral process through such acts, it managed to make its presence felt in its pockets of influence in Jharkhand, Bihar, Chhattisgarh, and a few other states. Significantly, its activists and guerrillas physically targeted only the candidates and the state’s representatives – the security forces, the poll officials – and thankfully refrained from attacking the voters who came in large numbers (often representing 50 to 60% of the electorate in these areas). The Maoist call for boycotting the elections, the party’s attempts to bring this about by large-scale attacks on the electoral machinery, and yet, the willingness of the villagers in their strongholds to queue up to cast their votes, present a peculiar web of complexities. At one level, the boycott call by the Maoist party may make sense if we follow the arguments that it has put forward in its bulletin released on the eve of the elections. Non-invasive prediction of axillary lymph node dissection exemption in breast cancer patients post-neoadjuvant therapy: A radiomics and deep learning analysis on longitudinal DCE-MRI data

Abstract:Purpose: In breast cancer (BC) patients with clinical axillary lymph node metastasis (cN+) undergoing neoadjuvant therapy (NAT), precise axillary lymph node (ALN) assessment dictates therapeutic strategy. There is a critical demand for a precise method to assess the axillary lymph node (ALN) status in these patients. Materials and methods: A retrospective analysis was conducted on 160 BC patients undergoing NAT at Fujian Medical University Union Hospital. We analyzed baseline and two-cycle reassessment dynamic contrast-enhanced MRI (DCE-MRI) images, extracting 3668 radiomic and 4096 deep learning features, and computing 1834 delta-radiomic and 2048 delta-deep learning features. Light Gradient Boosting Machine (LightGBM), Support Vector Machine (SVM), RandomForest, and Multilayer Perceptron (MLP) algorithms were employed to develop risk models and were evaluated using 10-fold cross-validation. Results: Of the patients, 61 (38.13 %) achieved ypN0 status post-NAT. Univariate and multivariable logistic regression analyses revealed molecular subtypes and Ki67 as pivotal predictors of achieving ypN0 post-NAT. The SVM-based "Data Amalgamation" model that integrates radiomic, deep learning features, and clinical data, exhibited an outstanding AUC of 0.986 (95 % CI: 0.954-1.000), surpassing other models. Conclusion: Our study illuminates the challenges and opportunities inherent in breast cancer management post-NAT. By introducing a sophisticated, SVM-based "Data Amalgamation" model, we propose a way towards accurate, dynamic ALN assessments, offering potential for personalized therapeutic strategies in BC.

Abstract:Background Pathological axillary lymph node (pALN) burden is an important factor for treatment decision‐making in clinical T1‐T2 (cT1‐T2) stage breast cancer. Preoperative assessment of the pALN burden and prognosis aids in the individualized selection of therapeutic approaches. Purpose To develop and validate a machine learning (ML) model based on clinicopathological and MRI characteristics for assessing pALN burden and survival in patients with cT1‐T2 stage breast cancer. Study Type Retrospective. Population A total of 506 females (range: 24–83 years) with cT1‐T2 stage breast cancer from two institutions, forming the training (N = 340), internal validation (N = 85), and external validation cohorts (N = 81), respectively. Field Strength/Sequence This study used 1.5‐T, axial fat‐suppressed T2‐weighted turbo spin‐echo sequence and axial three‐dimensional dynamic contrast‐enhanced fat‐suppressed T1‐weighted gradient echo sequence. Assessment Four ML methods (eXtreme Gradient Boosting [XGBoost], Support Vector Machine, k‐Nearest Neighbor, Classification and Regression Tree) were employed to develop models based on clinicopathological and MRI characteristics. The performance of these models was evaluated by their discriminative ability. The best‐performing model was further analyzed to establish interpretability and used to calculate the pALN score. The relationships between the pALN score and disease‐free survival (DFS) were examined. Statistical Tests Chi‐squared test, Fisher's exact test, univariable logistic regression, area under the curve (AUC), Delong test, net reclassification improvement, integrated discrimination improvement, Hosmer‐Lemeshow test, log‐rank, Cox regression analyses, and intraclass correlation coefficient were performed. A P‐value <0.05 was considered statistically significant. Results The XGB II model, developed based on the XGBoost algorithm, outperformed the other models with AUCs of 0.805, 0.803, and 0.818 in the three cohorts. The Shapley additive explanation plot indicated that the top variable in the XGB II model was the Node Reporting and Data System score. In multivariable Cox regression analysis, the pALN score was significantly associated with DFS (hazard ratio: 4.013, 95% confidence interval: 1.059–15.207). Data Conclusion The XGB II model may allow to evaluate pALN burden and could provide prognostic information in cT1‐T2 stage breast cancer patients. Level of Evidence 3 Technical Efficacy Stage 2

Abstract:Objective This study seeks to construct a machine learning model that merges clinical characteristics with ultrasound radiomic analysis—encompassing both the intratumoral and peritumoral—to predict the status of axillary lymph nodes in patients with early‐stage breast cancer. Methods The study employed retrospective methods, collecting clinical information, ultrasound data, and postoperative pathological results from 321 breast cancer patients (including 224 in the training group and 97 in the validation group). Through correlation analysis, univariate analysis, and Lasso regression analysis, independent risk factors related to axillary lymph node metastasis in breast cancer were identified from conventional ultrasound and immunohistochemical indicators, and a clinical feature model was constructed. Additionally, features were extracted from ultrasound images of the intratumoral and its 1–5 mm peritumoral to establish a radiomics feature formula. Furthermore, by combining clinical features and ultrasound radiomics features, six machine learning models (Logistic Regression, Decision Tree, Support Vector Machine, Extreme Gradient Boosting, Random Forest, and K‐Nearest Neighbors) were compared for diagnostic efficacy, and constructing a joint prediction model based on the optimal ML algorithm. The use of Shapley Additive Explanations (SHAP) enhanced the visualization and interpretability of the model during the diagnostic process. Results Among the 321 breast cancer patients, 121 had axillary lymph node metastasis, and 200 did not. The clinical feature model had an AUC of 0.779 and 0.777 in the training and validation groups, respectively. Radiomics model analysis showed that the model including the Intratumor +3 mm peritumor area had the best diagnostic performance, with AUCs of 0.847 and 0.844 in the training and validation groups, respectively. The joint prediction model based on the XGBoost algorithm reached AUCs of 0.917 and 0.905 in the training and validation groups, respectively. SHAP analysis indicated that the Rad Score had the highest weight in the prediction model, playing a significant role in predicting axillary lymph node metastasis in breast cancer. Conclusion The predictive model, which integrates clinical features and radiomic characteristics using the XGBoost algorithm, demonstrates significant diagnostic value for axillary lymph node metastasis in breast cancer. This model can provide significant references for preoperative surgical strategy selection and prognosis evaluation for breast cancer patients, helping to reduce postoperative complications and improve long‐term survival rates. Additionally, the utilization of SHAP enhancing the global and local interpretability of the model.
