Abstract:This cross-sectional study was designed to develop normative functional fitness standards for the Portuguese older adults, to analyze age and gender patterns of decline, to compare the fitness level of Portuguese older adults with that of older adults in other countries, and to evaluate the fitness level of Portuguese older adults relative to recently published criterion fitness standards associated with maintaining physical independence. A sample of 4,712 independent-living older adults, age 65–103 yr, was evaluated using the Senior Fitness Test battery. Age-group normative fitness scores are reported for the 10th, 25th, 50th, 75th, and 90th percentiles. Results indicate that both women and men experience age-related losses in all components of functional fitness, with their rate of decline being greater than that observed in other populations, a trend which may cause Portuguese older adults to be at greater risk for loss of independence in later years. These newly established normative standards make it possible to assess individual fitness level and provide a basis for implementing population-wide health strategies to counteract early loss of independence.

Abstract:OBJECTIVES: The aim of the current study was to develop sex and age-adjusted criterion-referenced fitness standards to predict independent physical functioning into later life.DESIGN: Cross-sectional.SETTING: National Survey of Physical Fitness and Physical Activity, Portugal.PARTICIPANTS: A national representative sample of 3074 non-institutionalized older adults (65-84years).MEASUREMENTS: Projected capacity for physical independence was assessed by the Composite Physical Function (CPF) scale, and physical fitness was assessed using the Senior Fitness Test battery. ROC analyses were used to evaluate the classification accuracy of physical fitness components, and determine the best cutoff values of functional fitness to predict loss of independence. Stepwise multivariate logistic regression was used to identify sex and age category fitness components that were predictors of independence with follow-up ROC analysis to verify the model's discriminative capability.RESULTS: ROC curve analysis showed that the fitness tests were good in diagnosing physical independence (AUC>0.7). Logistic regression models demonstrated that different fitness components should be targeted according to age and sex, but with an emphasis on aerobic endurance and agility/dynamic balance. Overall, models provided higher AUC (males: 0.79-0.86; females: 0.73-0.85) when comparing the best fitness tests for each sex and age category. The sensitivity of newly developed models ranged from 80.5 to 87.7 in males and from 68.2 to 86.3 in females, while the specificity ranged from 62.1 to 73.8 in males and between 58.2 and 82.9 in females.CONCLUSION: The Senior Fitness Test provides a good field tool that allows the identification of fitness levels that older adults need to achieve to be physically independent later in life, and permits the accurate planning and implementation of exercise-based interventions for older adults.

The impact of an exercise intervention using low-cost equipment on functional fitness in the community-dwelling older adults: A pilot study

Abstract:Exercise interventions have shown significant improvements in the older adult population regarding functional and cognitive capacity. However, much research has applied exercise protocols that are expensive both for research and participation. Thus, high investments that are made are currently not ecologically efficient. The aim of the study was to determine whether the impact of a 20-week multicomponent exercise intervention using low-cost material could improve physical fitness in community-dwelling older adults. We tested the feasibility of this three times per week exercise protocol using an inclusive approach. Eligibility criteria included age ≥65 years, the ability to stand and walk with or without assistive devices, being physically inactive; medical clearance, and living in the community. Outcomes were muscle resistance measured using the chair-stand test for lower limbs, cardiorespiratory capacity measured using the 6-min walk test, and agility and balance using the Timed-up and Go test. Flexibility was measured using the chair sit and reach for lower limbs and the back scratch for upper limbs using the dominant leg and arm, respectively. Hand grip strength was measured using a dynamometer. Thirty participants ( n = 30) were recruited and included in the intention-to-treat analysis. The mean age of participants was 70 years (±3.62 years), 100% were Caucasians, and 50% were female. There was a significant trend toward a clinically important improvement in the chair-stand (t = −5.23; p < 0.001; d = 0.36), arm curl (t = −5.21; p < 0.001; d = 0.74), 6-min walk test (t = −4.69; p = <0.001; d = 0.77), timed-up and go test (t = 8.788; p < 0.001; d = 1.18), and hand grip strength (t = 2.846; p = 0.009; d = 0.23). There were also differences in the back scratch (t = 2.243; p = 0.035; d = 0.29) and chair sit and reach test (t = −3.380; p < 0.001; d = 0.15). This pilot study has provided preliminary evidence that a 20-week community-based low-cost exercise program may be effective in improving overall functional fitness in older people. The pilot trial has provided the necessary data to design future randomized-controlled trials that can be implemented in the community in an ecological feasible manner.

Falls in Portuguese older people: procedures and preliminary results of the study Biomechanics of Locomotion in the Elderly

Abstract:Aim: The aims of this study were to: (1) present preliminary results about the evaluation of the procedures (physical activity and functional fitness tests) followed in the baseline period of our research program, (2) present a preliminary characterization of Portuguese older people regarding sociodemographic, health, physical activity (PA) and functional fitness (FF) variables (3) identify, within those parameters, the ones which are determinant to predict falls in Portuguese older adults. Material and methods: 647 subjects aged over 65 years were randomly recruited in Lisbon and Tagus Valley area. Trained interviewers administered: (1) a standardized questionnaire that included sociodemographic, health and falls parameters; (2) YPAS questionnaire for PA and (3) six FF tests (30sec Chair-Stand and 8 foot Up&Go from SFT battery and items 4-7 from FAB Scale).Reproducibility and convergent validity of the FF and PA tests were determined by ICC and Pearson correlations. Logistic regression analysis was used to model fall occurrence considering three different fall groups (non-fallers (NF - 0 falls), fallers (F - 1 fall) and recurrent fallers (RF - >1 fall). Results: FF and PA tests showed to have a good convergent validity and reproducibility, giving us confidence about the results obtained.Approximately 37% of the elderly tested fell during the previous year. From these, 41% were RF. Our results showed that age is not a risk factor for falling and that health and FF variables are the most determinant factors to assess fall risk. Conclusion: According to the results, falls might not be an inevitable consequence of age, but instead, mainly associated with poor health and functionality. Moreover, PA seems to play a key role in this process, not only because a higher level of PA will lead to a better functionality, but also because PA was found to be a protective factor for both episodic and recurrent falls.
Is There a Relationship between Anthropometric Indices and Muscular, Motor, and Cardiorespiratory Components of Health-Related Fitness in Active European Older Adults?

Abstract:The aging process induces alterations in the body, resulting in changes in both health-related fitness and specific anthropometric measures. These changes often pose health risks for older adults. The aim of the present study was to investigate whether there is an association between anthropometric indices and muscular, motor, and cardiorespiratory components of health-related fitness in active European older adults. This study included 2687 European older adults, comprising 1999 women and 688 men, with an average age of 70.05 ± 5.5 years. The assessment included health-related fitness using the Senior Fitness Test and anthropometric indices, such as the body adiposity index, body mass index, conicity index, waist-to-hip ratio, and waist-to-height ratio, among others. The results indicated that gender significantly influences the values of physical performance and anthropometric parameters, making them incomparable. The degree of correlation between anthropometric indices and muscular, motor, and cardiorespiratory components of fitness depends on each anthropometric index analysed. The anthropometric index most correlated with physical fitness performance parameters is the waist-to-height ratio (WHR), followed by the body mass index (BMI). Cardiorespiratory endurance and balance are the two physical parameters most correlated with anthropometric indices.

Abstract:Elion (1924) was the first to isolate a thermophilic sulfate reducing bacterium in pure culture. He named the organism Vibrio thermodesulfuricans, and reported that it was capable of sulfate reduction betveen 30 and 65 C, wvith an optimum of 55 C. Baars (1930) and Kluyver and Baars (1932) reisolated V. thermodesulfuricans, and carried out a comparative study of this organism and Vibrio desulfuricans. They reported that it was possible to adapt cultures of V. desulfuricans, initially capable of growth only at 30 C, to grow up to 55 C by gradually increasing the incubation temperature. The reverse phenomenon was also demonstrated with V. thermodesulfuricans. They concluded that V. thermodesulfuricans should be regarded as a temperature adapted strain of V. desulfuricans. The sulfate reducing bacteria are now placed in the genus Desulfovibrio, created by Kluyver and Van Niel (1936). Starkey (1938) isolated cultures from sewage, mud, and soil at 30 and 55 C. Contrary to the findings of earlier investigators, all of the cultures isolated at 55 C were large, slightlv curved, spore-forming rods, while those isolated at 30 C were typical asporogenous short vibrios. The cultures isolated at 30 C were motile by a rapid progressive movement and had one (or two) polar flagellum(a). Those isolated at 55 C had a rotating motility with only a slight progressive movement, and possessed from 1 to 10 or more flagella scattered around the cell. Cultures isolated at 30 C failed to grow when transferred directly to 55 C, but the cultures isolated at 55 C

Abstract:This study was designed to construct normative physical fitness scores for older, functionally independent adults living in the community. The Physical Activities Readiness Questionnaire and Barthel Index were initialized to screen those who have heart illness, arthritis, and functional dependence. After providing informed consent, each participant was instructed to perform seven tests in five categories, including body mass index, muscle strength/endurance (grasp test and 30-s chair stand test), balance (open-eye stand on right foot), flexibility (chair sit-and-reach test), and aerobic endurance (2- and 3-min step tests with preset cadence). Twenty-two assessors were recruited and trained by a physical fitness instructor to ensure acceptable interrater reliability. The valid sample size was 1,104. Test performances were significantly different for male and female participants for all test categories, with the exception of aerobic endurance. Mean scores of all tests correlated negatively with age. The authors constructed the percentile distributions for the seven fitness tests for both genders. Results are expected to be helpful in assessing physical fitness and evaluating physical activity in older adults.

Physical fitness assessment of older adults in the primary care setting

Abstract:Purpose: To explore the relationships between the physical fitness and the demographic characteristics, general health and well-being, social support, and physical activity in a group of community-dwelling older adults. This study also assessed the feasibility of obtaining physical fitness measures in the primary care setting. Data sources: Thirty-four persons older than 60 years completed self-report measures and the Senior Fitness Test (SFT), a battery of fitness measures normed for older adults. Self-report measures included demographics, general health and well-being (SF-36), social support (Social Support for Physical Activity Scale), and physical activity levels (Physical Activity Scale for the Elderly). Conclusions: Physically active older adults had significantly better upper and lower body strength, aerobic endurance, and dynamic balance. Older adults who reported better general health and had fewer chronic conditions scored better on the physical fitness tests. The SFT took 10-15 min to complete in the primary care setting. Implications for practice: Physical fitness measures give primary care providers objective information to develop a physical activity program for their patients. A physical activity program that includes recommendations for aerobic endurance, strength, flexibility, and/or balance that is based on objective findings may be a useful tool to promote better health and physical fitness in older adults.

Abstract:The aim of this study was to compare physical, mental, and biochemical health indicators of 48 older women (67 ± 1 year) who practiced multicomponent exercise program (ME, n = 25) and self-selected physical activity (PA, n = 23) for 6 months. It was an observational study, which aimed to relate a prospective intervention. Displacement speed, lower limb (LL) power, functional capacity, body composition, biochemical profile, physical activity levels (PAL), sedentary behavior (SB), quality of life (QoL), and mental illness risk (MIR) were evaluated. ME presented better values compared to the PA in the gait speed (p = 0.001, large ES), aerobic capacity (p = 0.0001, large ES), agility/dynamic balance (p = 0.0001, large ES), LL flexibility (p = 0.0003, large ES), UL flexibility (p = 0.04, large ES), upper limb (UL) strength (p = 0.07, moderate ES), Total cholesterol (p = 0.009, large ES), triglycerides (p = 0.003, large ES), creatinine (p = 0.007, large ES), glycated hemoglobin (p= 0.007, large ES), and lower mean glucose value (p = 0.008, large ES). ME was more efficient than PA to improve indicators of gait speed, and functional capacity, regulate glycated hemoglobin, blood glucose, and serum creatinine. Thys study also brings practical applications for coaches, which could adapt and use creativity to develop different types of systematized ME, aiming to enhance positive adaptations in the older people at multilevel outcomes.
The relationship between physical fitness and health self-assessment in elderly

Abstract:Background: The process of population aging inclines to seek determinants of the quality of life in the older people. Health self-assessment is the one of the main elements of the quality of life. In the older people it is associated with functional efficiency. The aim of the study was to determine correlations of physical fitness and health self-assessment. Methods: The study group included 123 people aged 60 to 86 years. Physical fitness was evaluated using the Senior Fitness Test (SFT). Self-esteem of health was assessed by the SF-36 questionnaire. Results: The analysis included sex factor and the occurrence of chronic diseases. When comparing with healthy participants, people suffering from chronic diseases revealed a lower level of physical fitness (P < .05) and health self-assessment (P < .05). The results of a significant percentage of study participants were worse than norms proposed as standards. The percentage of people below norms varied depending on sex, age, and SFT assessment and ranged from 0 to 89.5%. Conclusion: Physical fitness and health self-assessment among elderly may be strongly determined by cultural conditions, for example, habits, lifestyle in various regions. The application of conclusions suggests that the key element of rehabilitation programs among elderly should be focused on improving coordination and locomotor capabilities. Assessment of the elderly is more clearly associated with physical fitness in women than in men and also more in patients chronically ill than in healthy person. Comprehensive assessment of physical fitness according to standardized values does not indicate the diversity by sex.

Abstract:Purpose: Experimental studies investigating the outcomes of physical exercise among age-stratified groups of older women are still incipient. This study aimed to investigate the effects of a ten-month multicomponent physical program on the physical fitness of older women in three age-stratified groups (60–69 years, 70–79 years, and ≥80 years). Method: Participants ( n = 141) were divided into three age groups: GR1 60–69 years ( n = 45; 65.2 ± 2.3 years); GR2 70–79 years ( n = 67; 72.9 ± 2.9 years), GR3 ≥80 years ( n = 29; 83.5 ± 4.5 years). The participants performed the multicomponent physical program and were evaluated before and after the intervention with Senior Fitness Test. Data were analyzed using generalized estimating equations, Bonferroni test ( p ≤.05) and effect size (Cohen's d ). Results: For the group factor, there were significant differences in lower limb strength ( p = .003), upper limb strength ( p < .001), upper limb flexibility ( p < .001), balance/agility ( p < .001), and cardiorespiratory fitness ( p < .001). For the time factor, significant differences were observed in lower limb strength (GR2, p = .014, small effect size), upper limb strength (GR1, p = .003; GR2, p < .001; GR3, p = .017; small effect sizes), lower limb flexibility (GR1, p = .025, non-significant effect size), cardiorespiratory fitness (GR1, p < .001, medium effect size; GR2, p = .002; small effect size). Conclusion: Physical fitness improved with training, but effects differed between age groups. Positive effects were observed for GR1 and GR2, whereas GR3 showed maintenance of physical fitness. Aging interferes more strongly in women aged 80 years and older and it is necessary for specific training programs for this age group.

Abstract:Background: It's really important to measure the actual functional physical fitness of elderly independent living persons from different environments to know the level and rate of decrease may inform about the threat of loss of functional independence, hence the need to monitor and assess the senior's motor realm and adapt to it the appropriate programs and treatments in the care for the elderly person. Methods: The study involved 5367 people (mean age 69,63 ± 7,06), including 4164 women (mean age 69,55 ± 7,11) and 1203 men (mean age 69,91 ± 6,86) aged 60 to 93 years old. We have measured basic anthropometric features and physical fitness (by using Senior Fitness Test). Results: The average values in individual SFT tests significantly decrease along with age. After age of 80 and 85 there were no sex differences in SFT. The largest deficits concern the dynamic balance and the decrease reaches 69% in men and 62% in women A significantly higher rate of decline in aerobic capacity concerns men (43%) than women (36.9%). A clearly lower rate of loss occurs in the muscular strength of the lower and upper body and does not exceed 30%. Conclusions: The results are of great clinical importance for the development of effective prevention and gerontological education programs in terms of promoting active lifestyle and successful ageing, at the same time limiting the economic consequences of treatment and hospitalisation.
Functional fitness benchmark values for older adults: a systematic review

Abstract:Introduction The use of normative values and/or standards of functional fitness in adults is relevant to overall health and well-being. The objectives of the study were: to identify the physical tests of the senior fitness test (SFT) that have been applied since its proposal and to describe the proposed percentiles according to age, sex and country. Methods A systematic review study was conducted in the Pubmed and Scopus databases. As eligibility criteria, we considered the period from 1999 to 2022 that presented data on SFT test used in the population over 60 years of age and that described normative values through percentiles. MeSH were used as: (1) Physical fitness, Exercise test, Senior Fitness Test, Functional fitness, Cardiorespiratory fitness, (2) older adult, aged, (3) Reference standards, standards, standards of care. Boolean operators “AND” and “OR” were included. Data extracted from the selected studies included: year of publication, country, sample age, sample size, sample sex, fitness component. Results and discussion Seven studies were identified in five countries (03 in China, 01 in Poland, 01 in Portugal, 01 in Spain and 01 in United States). The age range ranged from 60 to 103 years. The studies were conducted in both sexes. The study with the smallest sample size was by Chung et al. (China) with 944 participants and the largest number of participants was the study by Rikli and Jones in the United States with 7,183 participants. In general, no study was able to complete 100% (8 components) of the tests proposed in the SFT. Normative values were presented through percentile distribution (p10, p50 and p90) organized by age ranges. Males presented better performance in FPF tests than females in all tests. Since the first publication of the SFT until 2022, seven articles have been published in countries such as United States, China (three regional studies), Poland, Portugal and Spain. No study has published the complete battery with its eight components. The percentiles of functional fitness reflect decline with advancing age. Mediterranean Diet-Based Sustainable Healthy Diet and Multicomponent Training Combined Intervention Effect on Body Composition, Anthropometry, and Physical Fitness in Healthy Aging

Abstract:Background: Diet and exercise interventions have been associated with improved body composition and physical fitness. However, evidence regarding their combined effects in older adults is scarce. This study aimed to investigate the impact of a combined 12-week Mediterranean diet-based sustainable healthy diet (SHD) and multicomponent training (MT) intervention on body composition, anthropometry, and physical fitness in older adults. Methods: Diet intervention groups received a weekly SHD food supply and four sessions, including a SHD culinary practical workshop. The exercise program included MT 50 min group session, three times a week, on non-consecutive days. Body composition and physical fitness variables were assessed through dual X-ray absorptiometry, anthropometric measurements, and senior fitness tests. Repeated measures ANOVA, with terms for group, time, and interaction, was performed. Results: Our results showed that a combined intervention significantly lowered BMI and total fat. Also, significant differences between assessments in all physical fitness tests, except for aerobic endurance, were observed. Adjusted models show significant differences in BMI (p = 0.049) and WHR (p = 0.037) between groups and in total fat (p = 0.030) for the interaction term. Body strength (p < 0.001), balance tests (p < 0.001), and aerobic endurance (p = 0.005) had significant differences amongst groups. Considering the interaction term, differences were observed for upper body strength (p = 0.046) and flexibility tests (p = 0.004 sit and reach, p = 0.048 back scratch). Conclusions: Our intervention study demonstrates the potential of implementing healthy lifestyle and sustainable models to promote healthy and active aging.

Abstract:Conclusions: Ipsilateral and contralateral medial vestibular nuclei (MVN) neurons respond differently to systemic injection of baclofen (4-amino-3-(4-chlorophenyl)-butanoic acid), illustrating the plastic changes of the type B γ-aminobutyric acid (GABAB) receptor during vestibular compensation. Objectives: To investigate the responsiveness of MVN neurons to baclofen during the early, partially compensated period after unilateral surgical labyrinthectomy. Materials and methods: MVN were localized using field potentials evoked by electrical stimulation, along with a stereotaxic atlas of rat brain. Neuronal activity in MVN was recorded and analyzed in rats that had undergone labyrinthectomy with and without administration of baclofen. Results: After left labyrinthectomy the mean discharge rate in ipsilesional MVN decreased, but it was nearly restored by postoperative day 8. Baclofen (3 mg/kg) reversed the mean discharge rates between bilateral MVN at days 4 and 8 after surgical labyrinthectomy. In addition, the reduction ratio of the right MVN neurons was higher than that of the left MVN neurons.

Abstract:The goal of this article is to analyse leisure time physical activity (LTPA) and health-driven motivations to engage in such activity among elderly people in the European Union. We use as a base the recommendations of the World Health Organisation (WHO) and the theory of the correlation between physical activity according to individual factors (age, gender, socio-economic status) and contextual factors (habitat, community infrastructures, the model of the welfare state of the country of residence). Data are taken from Eurobarometer 88.4. The Generalized Structural Equation Model (GSEM) methodology was used, with the STATA program. The results show that 65.3% of EU citizens over the age of 60 engage in some form of LTPA, that 40.4% do so for health reasons, and that only 32.3% engage in LTPA that meets the minimum guidelines set by the WHO. In addition, there are large differences based on individual and contextual characteristics. The following group was found to be those who practice the most: men, with high socio-economic status, belonging to the middle and upper social classes, living in rural areas where there is infrastructure for physical activity, and above all, in the countries of the Nordic model of social welfare.
A study on the physical aging characteristics of the older people over 70 years old in China

Abstract:Aim: In China, with the increase of life expectancy and the decrease of fertility rate, the aging problem has become increasingly prominent, and the physical problems of the older people over 70 years are the key and difficult problems. Method: Based on the interactive logic between the aging problem and the older people health, in the study, a questionnaire survey and a nationwide physical fitness test were carried out on the older people over 70, to divide into different age groups (70-74 years old, 75-79 years old, 80-84 years old, 85 years old and older) and different genders. There were 8,400 valid samples, and 1,050 persons in each group. One-way ANOVA was used to compare the differences among groups of different ages, and a broken line chart was drawn to discuss the aging characteristics of various physical indexes of the older people over 70 in China. Result: (1) Body morphology: male waist circumference, male waist-to-height ratio and female BMI showed a gradual downward trend with the increase of age; (2) Physiological function: male and female vital capacity showed a decreasing trend with the increase of age, while female pulse pressure showed a gradual upward trend. (3) Physical quality: the indicators of male and female muscle strength, flexibility quality, aerobic endurance and balance showed a downward trend with the increase of age. Conclusion: Vital capacity, flexibility quality, muscle strength, aerobic endurance, balance ability and so on, decreased significantly with the growth of age. 80 years old is the inflection point of the rapid decline of various indicators. Blood pressure, silent pulse, BMI, waist-to-hip ratio, waist-to-height ratio and other indicators did not change regularly with age. Indicators such as blood pressure, BMI, waist-to-hip ratio and waist-to-height ratio were in the high-risk range of metabolic diseases and cardiovascular and cerebrovascular diseases. The study conducted physical fitness test on the older people over 70 years old in 7 geographical regions of China, which is the first nationwide physical fitness test for the older people, which is an extension and expansion of the national physical fitness monitoring system, and also shows that the test indicators involved in the "Health fitness scale" are simple and feasible. And the study added a series of test data over 70 years old, which is the basis for scientific and reasonable formulation of physical fitness evaluation standards for the older people, and is of great significance for improving the national physical fitness database and grasping the dynamic changes of national physical health status, and providing data support for scientific guidance of physical exercise for the older people.
Engagement with life and psychological well-being in late adulthood: Findings from community-based programs in Portugal

Abstract:Objectives: Human aging is a multidirectional, multidimensional, and multicausal process that reflects biological, psychological, and sociocultural influences, which act in distinct combinations throughout the life-span. Proactivity towards avoiding the usual aging process is needed. This study analyses the long-term effects of participation in Community-Based Programs on psychological well-being. Method: A sample of 150 community-dwelling participants enrolled in Community-Based Programs, aged 55 to 84 years and living in three Portuguese localities were matched by age (55-64, 65-74, 75-84 years), gender, and locality with a comparison group of non-participants. We administered a multidimensional gerontological protocol which included socio-demographic information, measures of health/disease, functional ability, social network, cognitive performance and psychological well-being. Hierarchical regression models were used to test the effects of Community-Based Programs on psychological well-being adjusting for remaining variables. Results: Overall, psychological well-being is positively associated with household income and satisfaction with health. Nevertheless, in participants, psychological well-being builds predominantly upon social network and is not associated with a moderate inability or cognitive deficits, contrasting with psychological well-being in non-participants. After adjusting for background variables, psychological well-being was positively associated with health satisfaction and social network and negatively related to moderate inability. Further, a significant interaction of participation in Community-Based Programs with age, points out higher levels of psychological well-being in participants contrasting with a downward trend in non-participants. After stratification by age, psychological well-being increases with time attending Community-Based Programs in the oldest (75-84 years) contrasting with the remainder. Conclusions: Participation in Community-Based Programs may improve the negative effects of the aging process on psychological well-being. This positive effect as age increases may be linked to a reinforcement of social network, valued more by participants in Community-Based Programs. Furthermore, the programs may act as a healing/maintenance strategy in persons with moderate inability and/or cognitive deficits.
Is Self-Reported Physical Fitness Useful for Estimating Fitness Levels in Children and Adolescents? A Reliability and Validity Study

Abstract:Background and objective: The assessment of physical fitness has become a necessary issue in epidemiological studies, since a reduction in fitness is directly associated with early mortality. Therefore, the development of simple, accurate, and inexpensive methods is necessary to measure physical fitness. This study aimed to determine the reliability and validity of the criteria and constructs of the International Fitness Scale (IFIS), Portuguese version, in Brazilian pediatric populations. Methods: A total of 190 children aged 3-10 years and 110 adolescents aged 11-17 years were enrolled in an observational study of reliability and validity. For reliability, the participants completed a questionnaire twice (with an interval of 15 days). To test the criterion validity, we analyzed the agreement between the questionnaire and physical tests (20-m shuttle run test, handgrip strength, standing long jump tests, 4 × 10-m shuttle run test, and back-saver sit and reach test), and the construct validity was estimated by agreement between the questionnaire and high blood pressure. The reliability was analyzed by kappa coefficients. The agreement between the testing and retesting of the questionnaire was evaluated by kappa coefficients. We applied a 2 × 2 table to estimate the specificity, sensitivity, and accuracy of the questionnaire. Results: The mean age of the children was 6.7 years (n = 190), and for the adolescents it was 14.6 years (n = 110). The questionnaire reliability showed an almost perfect score (κ ≥ 0.93 in children and κ ≥ 0.88 in adolescents). The questionnaire showed moderate criterion validity (κ ≥ 0.40 in children and adolescents) as well as moderate construct validity (κ ≥ 0.40) in the components of general conditioning, cardiorespiratory capacity, muscular strength, and speed/agility in children and in the components of cardiorespiratory capacity, muscle strength, and speed/agility in adolescents. The questionnaire was a sensitive method for measuring physical fitness. Conclusions: The Portuguese version of the IFIS is a reliable and valid method for measuring physical fitness in pediatric populations.
