Early Recognition and Management of Pulmonary Embolism Related to Mycoplasma Pneumoniae Infections in Children: Two Case Reports and Literature Review

Abstract:Objective: To explore the clinical characteristics and risk factors of mycoplasma pneumoniae (MP) infections complicated by pulmonary embolism in children. Methods: We report two cases of MP infections complicated by pulmonary embolism (PE). We also reviewed all reports of pulmonary emboli resulting from MP infections in the past fifteen years. Results: Two children with PE were enrolled: a 9-year-old girl presented with bloody effusion and hemoptysis and a 7-year-old boy with chest pain and hemoptysis. We found 7 case reports of MP infection with PE. Nine cases aged from 6 to 13 years were reported in the past 15 years, of which 7 cases were male and 2 female. Five patients had chest pain, three had hemoptysis, and two had bloody pleural effusions. Four patients had emboli in the veins of their lower limbs. The D-dimer levels were increased significantly in all patients. Anti-cardiolipin antibodies of seven cases were positive. Conclusion: MP infection may lead to a hyper-coagulative state and potential to thrombosis because of autoantibodies especially antiphospholipid antibodies. Early diagnosis with anti-coagulation and thrombolysis treatment actively is the key to a better prognosis.

Abstract:Background Pseudomembranous necrotizing laryngotracheobronchitis refers to an acute diffuse necrotizing inflammation in the mucosa of the larynx, trachea, and bronchus. It often occurs in infants and children having viral infections secondary to bacterial infections. Mycoplasma pneumoniae ( M. pneumoniae ) is a common pathogen that causes pneumonia in children. In recent years, serious complications due to M. pneumoniae infection, including necrotizing pneumonia, pulmonary embolism, and pleural effusion, have been increasingly reported. Case presentation An 11-year-old girl was admitted to our unit with cough, fever, and hoarseness persistent for a week. The results of the M. pneumoniae serological test, PCR examination with bronchial aspirate and bronchoalveolar lavage fluid (BALF), next-generation sequencing (mNGS) for BALF, all suggested the presence of M. pneumoniae infection. High-resolution CT scanning of the chest showed inflammation of the middle and lower lobes of the right lung. By bronchoscopy, the necrosis of the vocal cords, trachea, and bronchial mucosa was observed; each bronchial lumen contained a large amount of white viscous sputum. Pathological findings for bronchial mucosa suggested inflammatory necrosis. After administration of azithromycin and glucocorticoids, the symptoms of the patients were ameliorated. After 2 weeks post-discharge, the X-ray scan of her chest indicated the pneumonia resolution in the right lung. Conclusions In patients with pneumonia due to M. pneumoniae infection, which causes obvious hoarseness, bronchoscopy is necessary even if the lung lesions are not massively consolidated. When necrotizing lesions of the larynx, trachea, and bronchi are detected by bronchoscopy, the necrotic tissues in the corresponding parts should be conducted tissue biopsy for pathological examination. Apart from macrolide antibiotics, the administration of small doses of glucocorticoids is necessary.
Nitrogen diffusion in doped TiO2 (110) single crystals: a combined XPS and SIMS study

Abstract:Rutile TiO2 (110) single crystals were doped with nitrogen by heating at 675 °C in flowing NH3 gas. This caused a red shift in the band edge and an increase in visible region absorption. Nitrogen depth profiles obtained using dynamic secondary ion mass spectrometry (SIMS) could best be fitted by assigning three distinct diffusion coefficients. X-Ray photoelectron spectroscopy (XPS) indicated the presence of two surface nitrogen states with binding energies of 395.6 eV (substitutional N) and 399.8 eV (interstitial N). Angle resolved XPS measurements allowed us to link the XPS environments to the SIMS diffusion profiles. Subsequent air annealing at 500 °C led to asymmetric diffusion of the interstitial nitrogen into the bulk, removal of substitutional nitrogen into the gas phase, a decrease in the concentration of Ti3+ and a blue shift in the band edge. These changes could be related to variation in the optical band gap, and it was found that the principle cause of band gap narrowing was substitutional rather than interstitial nitrogen.
The Etiology, Clinical Profile, and Outcome of Diffuse Alveolar Hemorrhage in Children: a Ten-Year Single-Center Experience

Abstract:Background Diffuse alveolar hemorrhage (DAH) is a life-threatening syndrome that may be caused by numerous disorders. There is scant data on the etiology and characteristics of DAH in children. Methods We retrospectively reviewed the clinical records of patients admitted to a tertiary pediatric hospital with DAH over a 10-year period. The syndrome was classified into five groups according to different etiologies, and the characteristics and outcomes of patients were compared. Results A total of 74 children were included in the study. Idiopathic pulmonary hemosiderosis (IPH) was the most frequent cause (64.9%), followed by miscellaneous causes (infection and other conditions) (16.2%), immune-mediated disorders (9.5%), liver dysfunction (5.4%), and cardiovascular disorders (4.1%). The median age of the patients was 3.5 years (ranging from 1.5 to 7 years), and no difference was found in the proportion of fever, crackles, and pulmonary infiltrates among the five etiological groups. There was no difference in the proportion of blood transfusions among the groups. Cardiac catheterization was performed on 31 patients for whom the diagnostic workup was negative and were suspected of having IPH, and abnormal signs were observed and bronchial artery embolization (BAE) was performed in all those patients. The patients with IPH had the lowest mortality, while those with DAH secondary to liver failure had the highest mortality. Patients in BAE group had a shorter duration of corticosteroids and a lower relapse rate than non-BAE group. Conclusions Idiopathic pulmonary hemosiderosis is a common etiology of DAH and has a good prognosis in children. Vascular abnormalities were observed in the patients with IPH. Further studies are needed to clarify the role of vascular malformations in IPH.
Clinical Presentation and Outcome of Pediatric Patients with Hemophagocytic Lymphohistiocytosis in China: A Retrospective Multicenter Study

Abstract:Background: Hemophagocytic lymphohistiocytosis (HLH) is a heterogeneous disease with major diagnostic and therapeutic difficulties. A large-scale multicenter study of pediatric HLH is still lacking in China.Procedure: The Histiocytosis Study Group of the Chinese Pediatric Society conducted this retrospective study in 2014. A total of 323 patients diagnosed with HLH between 2011 and 2013 from 12 hospitals were registered.Results: The median age at diagnosis was 2.2 years (range, 0-14.6 years), with a peak age of HLH onset at 0 to 3 years (63%). Mutations in HLH-related genes were found in 27.9% (24/86) patients who underwent genetic testing. PRF1, UNC13D, STXBP2 and LYST were the predominant genes involved. Sixteen patients (66.7%) presented with only monoallelic mutations in one gene. Epstein-Barr virus (EBV) infection was the major condition related to HLH, which was documented in 74.4% (201/270) of the patients who underwent EBV detection. Of 252 evaluable patients, 64.7% (163) achieved non-active disease at the eighth week and patients treated with a protocol containing etoposide presented higher remission rates (75.6% vs. 46.8%, P < 0.001). In multivariate analysis, a younger age at diagnosis (< 12 months), platelet count less than 80x10(9)/L, central nervous system involvement, and initial treatment using a protocol without etoposide (not HLH-94/04) were independent prognostic factors indicating resistant disease.Discussion: This study first multicenter assessment of HLH in China shows some different features in Chinese children with HLH compared with those in western countries, including older age, vulnerability to EBV infection, and a high proportion of patients with single monoallelic genetic mutations.

Abstract:Objective: To explore potential predictors of RMPP in early stage.Study design: The prospective study, multicenter study was conducted in Zhejiang, China from May 1st, 2019 to January 31st, 2020. Children aged 29 days to 14 years old, with fever time during 48 to 120 hours were included. A total of 1428 children completed the study. A questionnaire was designed to collect patients’ information. Pharyngeal swab samples were collected. M. pneumoniae DNA in pharyngeal swab specimens were detected. Whether the patients develop to RMPP were assessed. Logistic regression analyses were used to examine associations between clinical data and RMPP.Results: The ages of the patients ranged from 34 days to 13.9 years with a median 4.3 years. The positive rate of M. pneumoniae -DNA was 37.4% (534/1428), and 446 cases were Mycoplasma pneumoniae pneumonia (MPP). In MPP patients, 55 cases were RMPP (12.3%), others were general MPP (GMPP) patients (n=391, 87.7%). Only the peak body temperature before the first visit and LDH level in RMPP patients were higher than that in GMPP [39.6 (39.1-40.0) °C vs. 39.2 (38.9-39.7) °C, p=0.003, and 332.5 (278.8-392.1) U/L vs. 310.5 (259.0-358.8) U/L, p=0.024]. Logistic regression also only included the above two parameters in the prediction probability. The area under ROC curve of the prediction probability π of RMPP was 0.682 (95% CI, 0.593-0.771), P<0.01. The cut-off value was 0.12. Sensitivity and specificity of the prediction probability π in cut-off value was 0.64 and 0.70, respectively.Conclusions: AND RELEVANCE A prediction probability, calculating from the peak body temperature before the first visit and LDH level for early identifying RMPP from other MPP within 2-5 days of fever duration, with a cut-off value of 0.12 may be helpful in clinical practice.

Abstract:Mycoplasma pneumoniae is a major cause of communityacquired respiratory infections in children and adults. A number of severe pulmonary complications have been reported with M. pneumoniae infection. These complications include obliterative bronchitis, bronchiectasis, pleural effusion, interstitial fibrosis, lung abscess, acute respiratory distress syndrome (ARDS) and necrotizing pneumonitis. Necrotizing pneumonitis, which has been described as a suppurative complication of Streptococcus pneumoniae infection, is a rare manifestation of M. pneumoniae infection. Herein we report on five pediatric patients with necrotizing pneumonitis due to M. pneumoniae and review the relevant literature.

Abstract:SummaryBackground and objectiveThe mechanisms of Mycoplasma pneumoniae induced lung inflammation are not clearly understood yet. This study investigated whether activated T cells in the airway contributed to the pulmonary inflammation in patients with severe Mycoplasma pneumoniae pneumonia (MPP).MethodsBALF were collected in all 45 patients with MPP (MPP, n = 45), including mild (Mild, n = 20) and severe (Severe, n = 25) group. BALF in 20 of all 25 severe cases with MPP at the recovery stage (Rec‐severe, n = 20) were collected again. The control group consisted of 20 patients with airway foreign body aspiration (Con., n = 20). CD3+ T cells, CD69+, HLA‐DR+, CD25+ on CD3+ T cells in BALF were determined by flow cytometry. Levels of IL‐6, IL‐10 in BALF were determined by ELISA, and percentage of neutrophils was counted.ResultsA significant increased percentage of neutrophils and levels of IL‐6 and IL‐10, decreased percentage of CD3+ T cells, increased expressions of CD69+, HLA‐DR+ or CD25+ on CD3+ T cells were observed in children with MPP compared with the control group (P < 0.05 or 0.01). Compared to mild group, the percentage of neutrophils, CD3+ CD25+, IL‐6, and IL‐10 were increased in children with severe MPP (P < 0.01 or 0.05). Compared to acute stage, an increased percentage of CD3+ T cells, decreased percentage of neutrophils and IL‐6 level, and expressions of CD3+ CD69+ or CD3+ CD25+ were observed at the recovery stage in children with severe MPP (P < 0.01 or 0.05).ConclusionsIncreased activation of T cell in BALF may play an important role in the inflammatory response of acute and severe MPP. IL‐6 may predict the severity and prognosis and provide a better assessment for patient care. However, the underlying mechanism awaits further detailed investigations. Abstract:Mycoplasma pneumoniae(MP)remains to be an important pathogen of childhood community-acquired pneumonia,and also has close relationship to asthma exacerbation. There is a much higher incidence of MP infection in older children than younger children. The latter,although with a lower infection rate,usually show a more severe clinical picture,and is prone to have tachypnea and retractions. High fever and irritative cough are characteristics of MP infections in older children. Serological method is the standard one for diagnosing MP infection,while PCR is more suitable for early diagnosis of MP infection in younger and immunocompromised patients. Macrolide is still the drug of choice for treatment of childhood MP pneumonia,but how to manage the refractory MP pneumonia has become a big challenge to clinical researchers.
Dilemmas in diagnosis and management of hemophagocytic lymphohistiocytosis in children

Abstract:Background Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening entity which is characterized by severe hyperinflammation. Now the HLH-2004 protocol has been widely accepted and clinically used; however, many questions still remain in clinical practice. In this review, we discuss the dilemmas in the diagnosis and treatment of HLH in children. Data sources Original research for articles and literature reviews published in PubMed was carried out using the key term “hemophagocytic lymphohistiocytosis”. Results As the gene sequencing technology progresses, the range of causal mutations and primary HLH has been redefined. The monoallelic variants may contribute to the pathogenesis of the disease. Many conditions without defective cytotoxicity of T or NK cells may lead to HLH, such as primary immunodeficiency (PID) and dysregulated immune activation or proliferation (DIAP). HLH shares overlapping clinical and laboratory characteristics with severe sepsis, but usually the single values are more pronounced in HLH than sepsis. H score is another approach to help the diagnosis of secondary HLH. Specific Th1/Th2 cytokine patterns are very helpful tools to differentiate HLH (reactivation of HLH) from sepsis. Moreover, it also has been used successfully to stratify the therapy intensity. The treatment of HLH should consider underlying diseases, triggers and severity. HLH-94 is recommended for patients who need etoposide-based therapy. Conclusions Dramatic progress has been made during the past decades in understanding the pathophysiology of HLH. However, diagnosis and treatment of HLH remain with many dilemmas because of the heterogeneous nature of the disease. Better understanding new gene defects and more effective diagnostic approaches and salvage regimens are goals for the future.

Abstract:Background: The present study was conducted to determine the inflammatory response in the lungs of children with Mycoplasma pneumoniae pneumonia (MPP). Methods: This study retrospectively analyzed cytokine levels, cytological findings, and M. pneumoniae (MP)-DNA level in the bronchoalveolar lavage fluid (BALF) of 96 children with MPP. The study utilized Spearman's correlation method to evaluate the contribution of BALF and blood parameters in MPP children. Results: (1) A total of 96 MPP children were classified into the Low MP-DNA MPP group (BALF MP-DNA ≤ 105 copies/mL) and the High MP-DNA MPP group (BALF MP-DNA > 105 copies/mL); the Non-fever MPP group (no fever during the entire course of pneumonia) and the Fever MPP group; the Defervescence MPP group (fever had subsided at the time of bronchoscopy) and the Fervescence MPP group; and the Mild MPP group and the Severe MPP group. (2) The High MP-DNA MPP, Fever MPP, Fervescence MPP, and Severe MPP groups had higher levels of interleukin (IL)-6, IL-10, and tumor necrosis factor-α (TNF-α) in their BALF (all p < .05). (3) The proportions of neutrophils and macrophages in the BALF of the High MP-DNA MPP and Fever MPP groups increased and decreased, respectively (all p < .05). (4) In the BALF of MPP children, MP-DNA, IL-6, IL-10, TNF-α, and interferon gamma (IFN-γ) levels positively correlated with neutrophil proportion while negatively correlated with macrophage proportion (all p < .05). (5) The MP-DNA, IL-6, IL-10, TNF-α, and IFN-γ levels in the BALF of MPP children were positively correlated with the levels of C-reactive protein, procalcitonin, lactic dehydrogenase, fibrinogen, and d-dimer, while they were negatively correlated with the albumin level (all p < .05). Conclusions: In children with MPP, the pulmonary inflammatory immune response was stronger in the High MP-DNA MPP, Fever MPP, Fervescence MPP, and Severe MPP groups. The relationship between pulmonary cytokine levels, MP-DNA load, and serum inflammatory parameters were found to be weak.

Abstract:Mycoplasma is one of the most common pathogens causing community-acquired pneumonia in pediatric patients. In recent years, the number of refractory or severe cases with drug resistance has been gradually increasing and cases that developed embolism after Mycoplasma pneumoniae (M. pneumoniae) infection have been reported. The present study retrospectively analyzed the clinical features, diagnosis and treatment of M. pneumoniae pneumonia (MPP) combined with pulmonary embolism (PE) in a series of 7 cases encountered between January 1st, 2016 to August 1st, 2019 at the Department of Pediatric Intensive Care Unit of The First Hospital of Jilin University (Changchun, China). Combined with relevant Chinese and international studies published during the last two decades, a comprehensive analysis was performed. All of the pediatric patients of the present study had fever, cough and dyspnea respiratory symptoms at onset and the disease progressed rapidly. Thereafter, PE was confirmed by a series of examinations. Pulmonary CT indicated patchy inflammations and significantly elevated D-dimer levels, accompanied by positive anticardiolipin antibodies. Furthermore, a filling defect in the pulmonary artery branch was observed on CT pulmonary angiography (CTPA) examination. In 2 cases, the condition was improved with anti-infection and anticoagulation treatment with low-molecular-weight heparin and warfarin, respectively, and the pulmonary embolism disappeared after 3-4 months. A total of 5 cases, who were not responsive to the drug treatment, underwent surgical resection. During the operation, the local tissues were determined to be infarcted and the pathological diagnosis was consistent with pulmonary infarction. Among the 5 cases, 2 died of Acute Respiratory Distress Syndrome at 3-8 days after the operation. The remaining patients underwent 6-12 months of follow-up and respiratory rehabilitation and their quality of life is now good. In conclusion, compared with healthy individuals, pediatric patients with critical MPP have an elevated risk of embolism. It is necessary to be vigilant regarding whether MMP is combined with PE and perform timely CTPA examination. Early detection, early treatment and surgical intervention (if necessary) may significantly reduce the risk of mortality and disability.

[Clinical features of children with Mycoplasma pneumoniae pneumonia and peripheral lymphocytopenia]

Abstract:Objective: To study the clinical features of children with Mycoplasma pneumoniae pneumonia (MPP) and peripheral lymphocytopenia. Methods: A total of 310 MPP children who were hospitalized and underwent bronchoalveolar lavage from June 2018 to June 2019 were enrolled and divided into two groups: simple MPP group with 241 children (without peripheral lymphocytopenia) and MPP + peripheral lymphocytopenia group with 69 children. The two groups were compared in terms of clinical data and treatment outcome. Results: Compared with the simple MPP group, the MPP + peripheral lymphocytopenia group had significantly longer duration of fever and length of hospital stay and significant increases in C-reactive protein, lactate dehydrogenase, and Mycoplasma pneumoniae DNA copies in bronchoalveolar lavage fluid (P < 0.05). Compared with the simple MPP group, the MPP + peripheral lymphocytopenia group had significantly higher incidence rates of intrapulmonary consolidation, extrapulmonary complications, and serious lesions under bronchoscopy (erosion or sputum bolt) and a significantly higher proportion of patients with severe MPP (P < 0.05). Conclusions: Children with MPP and peripheral lymphocytopenia tend to have more severe immunologic injury. Peripheral blood lymphocyte count may be used to evaluate the severity of MPP.
Malignancy-associated Hemophagocytic Lymphohistiocytosis in Children: a 10-Year Experience of a Single Pediatric Hematology Center.

Abstract:ABSTRACT Objective: Malignancy-associated hemophagocytic lymphohistiocytosis (M-HLH) in children is a relatively rare but life-threatening secondary hemophagocytic lymphohistiocytosis (sHLH). Until now, only a limited number of cases regarding children with M-HLH has been reported. Methods: We conducted a retrospective study of 27 children with M-HLH, who admitted to our center between July 2007 and October 2019. The clinical data and laboratory data were analyzed. Results: The median age of the children with M-HLH was 7 years. Underlying diseases included myeloid malignancy (n = 6), lymphoid malignancy (n = 18) and unknown type lymphoma (n = 3). The one-year mortality rate was 56%. All patients had persistent fever. The clinical manifestations included hepatomegaly (89%), splenomegaly (67%) and central nervous system symptoms (56%). Thirteen children (48%) had Epstein–Barr virus (EBV) infection. No significant differences were observed between EBV-positive and negative M-HLH patients in terms of most clinical indicators. However, EBV-positive M-HLH patients showed prolonged activated partial thromboplastin time (APTT) and more hemophagocytosis in the bone marrow (BM) in contrast to EBV-negative patients. Eighteen patients (67%) received the HLH-94/04 regimen as the initial treatment. There were no significant differences in the overall survival (OS) between EBV-positive and negative patients. Patients with prolonged APTT had a significantly poorer OS than other patients (p = 0.012). Conclusions: The M-HLH children with EBV infection are more likely to have prolonged APTT and more hemophagocytosis in BM. The M-HLH children had a poor prognosis, especially those with prolonged APTT.

Abstract:Objective To investigate the clinical features,treatment and prognosis of the children with malignancy-associated hemophagocytic syndrome (MAHS) in Beijing in recent decade.Methods The clinical data of the patients with MAHS under 18 years old from July 2007 to February 2018 collected by the Society of Beijing hemophagocytic syndrome were analyzed retrospectively.Results There were 46 patients under 18 years old with MAHS in all(male 27,female 19).The patients with MAHS took up 8.9％ of the patients with hemophagocytic lymphohistiocytosis (HLH) (46/519 cases) from the area during that period.The median age of onset had 13.5 years (0.9-18.0 years).Thirty-five patients had lymphoma (76.0％),9 cases had leukemia (19.6％),1 case had myelodysplastic syndrome with refractory anemia with excess blast (RAEB-T),and 1 case had Epstein-Barr virus (EBV) associated lymphoproliferative disease (borderline tumor stage).All the patients had a fever.A half of them suffered from hepatosplenomegaly and 7 patients (15.2％) had neurological symptoms.Tbe common laboratory abnormalities included cytopenias,hemophagocytosis in bone marrow (81.8％,36/44 cases),elevated serum ferritin (87.8％,36/41 cases),and elevated sCD25 (100.0％,15/15 cases),decreased nature killer (NK) activity (61.1％,11/18 cases),and plasma EBV-DNA positive (57.9％).Four patients did not receive treatment,the rest were treated by several chemotherapy protocols including the HLH-94/2004 protocol.Five patients (10.8％) received the allogeneic hematopoietic stem cell transplantation,and 1 case received the splenectomy therapy.The mortality was 58.7％.Four heterozygous mutations of SH2D1A,UNC13D and PRF1 genes were found in 2 patients.Conclusions MAHS in children progresses rapidly,with poor prognosis,and it is often complicated with EBV infection.The children with MAHS may develop genetic defects similar to primary hemophagocytic syndrome.Nowadays,there is no standard treatment for MAHS,so the individualized treatment is to be explored.

Abstract:BACKGROUND: Mycoplasma pneumoniae is a pathogen that causes respiratory diseases in children. Infections caused by M. pneumoniae are usually self-limited but occasionally can be severe. We observed emerging cases of severe mycoplasma infection requiring extracorporeal membrane oxygenation (ECMO). Thus, we investigated chronological changes in the molecular features of the M. pneumoniae and its clinical impacts among the pediatric population.METHODS: From 2011 to 2019, respiratory samples were collected from patients younger than 18 years old with pneumonia in a tertiary children's hospital. Focused multiple-locus variable number of tandem repeats analysis (MLVA) typing was performed on samples positive for M. pneumoniae in 2016 and 2019. Clinical data from the patients' electronic medical records were collected. We described the annual trend of macrolide resistance and MLVA type and analyzed the associations between clinical manifestations and MLVA types.RESULTS: The percentage of macrolide-resistant (MLR) M. pneumoniae gradually increased from 22% (27/122) in 2015 to 70% (82/117) in 2019. Among the MLRM. pneumoniae, the predominant strain shifted from type P (31% [13/42]) to type A (40% [19/46]). The demographics, initial presentations, and clinical courses of the subjects with MLRM. pneumoniae did not differ significantly between 2011 and 2019. However, in 2019, two fulminant cases requiring venovenous ECMO were observed, which indicates that more attention to the clinical severity of MLRM. pneumoniae infections is warranted.CONCLUSION: Obtaining accurate information on macrolide susceptibility is crucial for physicians to initiate appropriate antibiotic treatment in a timely fashion. Although we could not identify significant differences among mycoplasma pneumonias caused by different MLVAs over a span of 9 years, the emergence of severe mycoplasma infections requiring ECMO was clinically significant, and further monitoring was required.

Abstract:Abstract Background: To summarize characteristics of hemoptysis in children associated with pulmonary vascular malformations, and to identify the pathological mechanism, potentially. Methods: Children were recruited who were diagnosed with hemoptysis for pulmonary vascular malformations without congenital heart disease (CHD). The demographics, history, clinical characteristics, laboratory findings, pathogens testing, imaging outcomes and prognosis were systemically reported. The nonparametric Mann-Whitney U test using SPSS 24.0 was employed to compare the continuous laboratory variables, and Fisher’s exact test was applied for data enumeration. A P-value of <0.05 was considered statistically significant. Results: In sum, 27 children were enrolled in this retrospective study with a medium age of 9.1 years old, among whom 9.1% were in preschool-group, 72.7% in school-group, and 18.2% in adolescent-group. Different hemoptysis degrees were observed from individuals, and the moderate and severe conditions were commonly in elder children. Meanwhile some antecedent symptoms were recorded, mainly cough and fever before hemoptysis occurrence. Pathogens testing indicated proofs for infection of MP in 68.2% patients, and 37.5% were further identified with MP-DNA-PCR. Images revealed interstitial changes in most hemoptysis cases, especially in the inferior lobe of right lung for cases with MP infection, and endovascular contrast detected 24 patients with abnormal bronchial arteries (BAs) causing bronchial artery-pulmonary artery fistula and 3 patients suffered from pulmonary arteriovenous fistula (PAVF). After the transcatheter plug closure, a recurrent hemoptysis occurred in 8 patients suffered from an infection of MP initially, and the OR of clinical worsening in Group 1 where cases were considered with an infection of MP, was 2:1. Conclusions: Pulmonary vascular malformations can be commonly detected in elder children. An infection of MP possibly acts as a potential trigger inducing the rupture of affected vessels, which could take a higher risk of clinical worsening.
Desmoinesian coal beds of the Eastern Interior and surrounding basins: The largest tropical peat mires in Earth history

Abstract:The Colchester, Springfield, and Herrin Coals of the Eastern Interior Basin are some of the most extensive coal beds in North America, if not the world. The Colchester covers an area of more than 100,000 km^, the Springfield covers 73,500-81,000 km^, and the Herrin spans 73,900 km^. Each has correlatives in the Western Interior Basin, such that their entire regional extent varies from 116,000 km^to 200,000 km^. Correlatives in the Appalachian Basin may indicate an even more widespread area of Desmoinesian peatland development, although possibly sUghtly younger in age. The Colchester Coal is thin, but the Springfield and Herrin Coals reach thicknesses in excess of 3 m. High ash yields, dominance of vitrinite macerals, and abundant lycopsids suggest that these Desmoinesian coals were deposited in topogenous (groundwater fed) to soligenous (mixed-water source) mires. The only modern mire complexes that are as widespread are northern-latitude raised-bog mires, but Desmoinesian Midcontinent paleomires were topogenous and accumulated within 10° of the paleo-equator. The extent and thickness of Desmoinesian paleomires resulted from the coincidence of prime peat-forming factors, including a seasonally wet paleoclimate; cyclothemic transgressions and base-level rise above extensive, low-relief cratonic areas floored by vast, impermeable paleosols; broad floodplains along large rivers with a groundwater table high enough to hydrologically link peatlands and keep them wet; low, relatively uniform rates of tectonic subsidence; and accumulation in a basin surrounded by low relief, which led to minimal sediment input.
