Editorial: hyponatremia – a possible but forgotten consequence of bowel preparation for colonoscopy

Abstract:Both the diagnostic accuracy and safety of colonoscopy depend on the quality of bowel preparation. While adequate colonic cleansing is a prerequisite for an optimal visualisation of colonic mucosa, safety of the formulation is a ‘must’, particularly when endoscopy is performed in healthy subjects for colorectal cancer screening. A systematic review of safety issues associated with bowel cleansing concluded that, with formulations containing polyethylene glycol and electrolytes (PEG-ELS), adverse events are uncommon, but potentially serious. In a recent issue of Alimentary Pharmacology & Therapeutics, Matro et al. added another piece to puzzle. They performed a RCT, comparing PEG 3350 plus sport drink (PEG-SD) with PEG-ELS, specifically designed to evaluate safety measures, with the incidence of hyponatremia as primary end-point. Other serum electrolytes, renal function and volume status were also investigated. Hyponatremia was observed in 3.9% and 2.2% patients taking PEG-SD or PEG-ELS, respectively. Amongst study patients there was a significantly greater decrease in serum sodium in those taking PEG-SD, which is hypo-osmotic and contains less sodium, potassium and chloride, compared to PEG-ELS that is nearly iso-osmotic. In patients developing hyponatremia, the use of diuretics was significantly greater, and the use of ACE inhibitors or angiotensin receptor blockers more common. In addition, significantly more patients with low serum sodium had diabetes and a greater proportion had hypertension, cardiovascular disease or liver disease. While this study adds to the current literature the identification of risk factors for development of asymptomatic hyponatremia, it was not powered to detect the incidence of severe symptomatic hyponatremia, which can occur after different PEG formulations. Indeed, over the past years, nine cases of severe, symptomatic, hypotonic hyponatremia have been reported (Table 1), one of which had a fatal outcome. The manifestations of hypotonic hyponatremia are largely related to dysfunction of the central nervous system, and they are more conspicuous when the decrease in the serum sodium concentration is large or rapid (i.e. occurring within a period of hours), as it is the case of bowel cleansing. Headache, nausea, vomiting, muscle cramps, lethargy, restlessness, disorientation and depressed reflexes can be observed. While most patients with a serum sodium concentration exceeding 125 mmol/L are asymptomatic, those with lower values may have symptoms, especially if the disorder develops rapidly. Complications of severe and rapidly evolving hyponatremia include seizures, coma, permanent brain damage, respiratory arrest, brain-stem herniation, and death. Hypotonic hyponatremia causes the entry of water into the brain, resulting in cerebral oedema. A few hours after the onset of hyponatremia, the brain undergoes a process of volume adaptation, in which intracellular and extracellular solutes are extruded, inducing water loss and ameliorating brain swelling. And indeed, all but one patients had a complete neurological recovery after saline infusion 10 (Table 1). Despite the above reports of asymptomatic and symptomatic electrolyte disturbances, clinical experience and systematic reviews have shown that the bowel cleansing with PEG-based formulations is safe and effective.
