Abstract:Abstract Older adults with burn injuries have a high risk of readmission after the initial burn admission. Frailty is associated with poor outcomes from the initial burn injury, however, it remains unknown if frailty impacts readmission in older adults after the initial index burn admission. Our study aims to examine patient, frailty, burn, surgical, and hospital factors that contribute to 90-day readmissions in older adults with an acute burn. Patients ≥ 50 years of age with an acute burn diagnosis in the Nationwide Readmissions Database (2016-2018) were included. Patients who died during index admissions were excluded from the readmission analysis. Patient, burn, hospital stay, and hospital characteristics were analyzed. Using variables significant from univariate logistic regression analysis, we performed a multivariable logistic regression analysis to identify factors associated with 90-day readmissions. A total of 41,500 weighted index burn admissions were included. The 90-day readmission rate was 25.6% (10,641/41,500). Almost all the readmissions were unplanned (96.5%). The most common primary readmission diagnoses based on Clinical Classifications Software Refined were burn injury and septicemia. Multivariable logistic regression analysis demonstrated an increased hospital frailty risk and lower-body-only burns were associated with an increased risk of readmission. Notably, surgical treatment of the burn injury during the index admission was associated with a decreased risk of readmission. This highlights the importance and challenges of surgical decision-making for older adults with high comorbidity burden. Future studies are needed to develop decision guides to help clinicians identify patients who would benefit from surgical treatment versus more conservative strategies in older adult patients with burns.

Abstract:Abstract Introduction There is an understanding in the burn community that burns occur as a result of something, whether it be medical conditions, psychiatric illness, poverty, living conditions, etc. Social determinants of health (SDOH) are defined as non-medical factors that can affect one’s health. These factors, along with psychiatric illness, can impact inpatient care and discharge planning. Our objective was to determine the prevalence of psychosocial factors in our burn population and analyze their impact on length of stay (LOS) and discharge disposition. Methods A retrospective chart review from January 1, 2022 to December 31, 2022 was performed at a large, urban, American Burn Association (ABA)-verified burn center using data from the ABA Burn Care Quality Platform (BCQP). All admitted patients ≥18 years old who sustained a burn injury were included. The two SDOH captured by BCQP, insurance status and living situation, were collected, in addition to psychiatric diagnosis, substance use disorder and alcohol use disorder. Primary outcome was LOS and secondary outcome was discharge disposition. Continuous variables were represented as median (interquartile range) and categorical variables as n (%). The Wilcoxon rank-sum test and Fisher’s exact test were used to compare LOS and discharge disposition between psychosocial variables of interest. Results Out of 302 patients, 129 (43%) had at least one negative SDOH. Of these 129 patients, 97% of them were uninsured/underinsured and 14% were undomiciled. Of the total population, 16% had a psychiatric diagnosis, 4.6% had alcohol use disorder and 8.6% had substance use disorder with a higher prevalence in the uninsured/underinsured and undomiciled population. There were 18 patients (6%) who were undomiciled prior to admission. The median LOS for all 302 patients was 8 days (IQR: 2, 17) and was not significantly different by insurance. Patients with a psychiatric diagnosis had significantly longer median LOS (15 days) when compared to those without (7 days, p=0.009). Undomiciled patients also had significantly longer median LOS (12 days vs 7 days, p = 0.034). Comparing by insurance, patients were discharged home with self-care at a similar rate (68% for insured, 62% for uninsured/underinsured). Patients in the uninsured/underinsured group were less likely to go to subacute rehab (4.3%) compared to the well-insured (11%). Only 28% of undomiciled patients were discharged back to a shelter or street. Conclusions The care of our burn patients is not defined by insurance status. However, the burn community needs a better, systematic process of collecting additional SDOH to further understand the impact they have on our patients. The longer LOS for those with psychiatric illness and homelessness highlights the need for specialized care both in the inpatient and post-discharge setting. Burn Center Verification and Safety-net Status: Are There Differences in Discharge to Inpatient Rehabilitation?

Abstract:Discharge to acute rehabilitation following major burn injury is crucial for patient recovery and quality of life. However, barriers to acute rehabilitation, including race and payor type impede access. The effect of burn center organizational structure on discharge disparities remains unknown. This study aims to investigate associations between patient demographics, burn center factors, and discharge to acute rehabilitation on a population level. Using the California Healthcare Access and Information Database, 2009-2019, all inpatient encounters at verified and non-verified burn centers were extracted. The primary outcome was the proportion of patients discharged to acute rehabilitation. Key covariates included age, race, burn center safety net status, diagnosis related group, American Burn Association (ABA) verification status, and American College of Surgeons (ACS) Level 1 trauma center designation. Logistic regression and mixed-effects modeling were performed, with Bonferroni adjustment for multiple testing. Among 27,496 encounters, 0.8% (228) were discharged to inpatient rehabilitation. By race/ethnicity, the proportion admitted to inpatient rehabilitation was 0.9% for White, 0.6% for Black, 0.7% for Hispanic, and 1% for Asian. After adjusting for burn severity and age, notable predictors for discharge to inpatient rehabilitation included Medicare as payor (OR 0.30-0.88, p=0.015) compared to commercial insurance, trauma center status (OR 1.45-3.43, p<.001), ABA verification status (OR 1.16-2.74, p=0.008), and safety-net facility status (OR 1.09-1.97, p=0.013). Discharge to inpatient rehabilitation varies by race, payor status, and individual burn center. Verified and safety-net burn centers had more patients discharge to inpatient rehabilitation adjusted for burn severity and demographics.

Abstract:Abstract Introduction Burns are a leading cause of morbidity and mortality, with long-term complications including pain and poor physical function. While neighborhood disadvantage has been shown to be associated with burn severity, its relationship with long-term burn complications has not yet been investigated. Therefore, we hypothesized that higher area of deprivation index (ADI), an aggregate marker of neighborhood disadvantage, will be associated with increased likelihood of pain and poor physical function post-burn. Methods We obtained data from the Burn Model System (BMS) database. We linked patient data with ADI state decile 1-10 (1=least and 10=most disadvantaged) using year of injury and Census Block Group based on residence at time of injury. We used bivariate analyses to identify associations between ADI and patient and burn characteristics. We used multivariate regression to determine whether ADI was associated with self-reported pain and physical function (PROMIS-29 subscales) 6 and 24 months post-burn. Significance was p < 0.05. Results We included 780 patients who were admitted to one of three BMS centers between 2014 and 2021 of whom 538 (69%) were male. Most identified as White (83%), Black (9.8%), or Asian (2.5%). Mean age at time of burn was 46.2 years, mean ADI state decile was 5.75, and mean TBSA was 15.2%. Bivariate analysis demonstrated significant associations between ADI and age at injury, ethnicity, race, pre-injury household income, pre-injury education, increased pain intensity at 6- and 24-month follow-ups, and worse physical function at 24-month follow-up. Multivariate regressions controlling for TBSA, race, age, sex, anxiety, depression, and pain interference demonstrated that ADI was a significant predictor of higher pain intensity at 6- (p=0.008) and 24- (p=0.020) month follow up and worse physical function at 24-month follow up (p=0.011). Conclusions Burn patients living in areas of higher neighborhood disadvantage are more likely to report increased long-term pain and reduced physical function, even after controlling for other known risk factors. Further investigation into the relationship between neighborhood disadvantage and post-burn outcomes in larger, nationally representative samples is warranted. Applicability of Research to Practice Along with existing literature, our findings suggest that policy targeting neighborhood disadvantage may help improve short- and long-term post-burn outcomes and assist in identifying patients who are at increased risk for negative health-related outcomes.

Abstract:Abstract Introduction Burn injury continues to contribute to significant and preventable morbidity and mortality in the United States. Despite an increased focus on racial and ethnic disparities in healthcare, there remains a critical knowledge gap in our understanding of the effect of these disparities on complications in the adult burn population. Methods The American Burn Association’s National Burn Repository data were reviewed from 2010-2018. Information regarding demographics, burn mechanism and severity, complications, and clinical outcomes were recorded. Data analysis was performed using 1:1 propensity-score-matching and logistic regression modeling. Results Among 215,071 adult patients with burn injury, racial distribution was as follows: 65.2% White, 19.1% Black, 2.2% Asian, 0.7% American Indian/Alaskan Native, and 12.8% Other (including Native Hawaiian/Other Pacific Islander, and multiple races). Flame injuries were the most common cause of burns across all races (35.2%), followed by scald burns (23.3%). Most patients, regardless of race, had partial thickness burns. Black and Asian patients were more likely to die compared to White patients (OR 1.10, CI 1.00-1.21; OR 1.35, CI 1.03-1.76, respectively). Compared to their White peers, Black patients were more likely to develop in-hospital complications (OR 1.12, CI 1.06-1.17), while complication rates were lower for American Indian/Alaskan Native and those classified as “Other” (OR 0.93, CI 0.87-0.99; OR 0.080 CI 0.66-0.97, respectively). Black patients, when compared to White patients, were more likely to sustain cardiovascular (OR 1.27 CI 1.13-1.43), gastrointestinal (OR 1.24 CI 1.13-1.43), infectious (OR 1.63 CI 1.45-1.834), and electrolyte/other complications (OR 1.10, CI 1.02-1.20). Patients categorized as Other were less likely to have cardiovascular (OR 0.82 CI 0.69-0.97) and electrolyte/other complications compared to White patients (OR 0.97, CI 0.79-1.00). Black, Asian, and patients categorized as Other had significantly increased length of hospital stay (OR 1.17, CI 1.14-1.21; OR 1.21, CI 1.14-1.29; OR 1.21, CI 1.17-1.25, respectively) and longer duration of mechanical ventilation (OR 1.36, CI 1.26-1.47; OR 1.28, CI 1.04-1.58; OR 1.13, CI 1.02-1.25, respectively). All minority groups had a significantly increased length of ICU stay (OR 1.36, CI 1.26-1.47; OR 1.28, CI 1.04-1.58; OR 1.13, CI 1.02-1.25, respectively) compared to White patients. Conclusions We showed disparities in the post-injury complication rates among adult burn patients. These findings highlight the need for further work to determine the etiology of these disparities to improve burn care for all patients, and to develop systems to prevent their persistence. Applicability of Research to Practice We demonstrated disparities in adult burn patient care impacts complication rates in this population, highlighting the need for further research and system improvements to prevent the persistence of these disparities.

Abstract:Abstract Introduction Existing research has examined the relationship between the amount of total body surface area (TBSA) burn and length of stay (LOS). As a result, the conventional ratio of 1day LOS/1% TBSA ratio has been updated to 1.5days LOS/1% TBSA. In this study, we aim to elucidate patient and injury characteristics that affect our prognostic indicator, leading to a prolonged LOS. Methods This was a secondary analysis of a cohort study of surviving patients admitted to a tertiary adult burn center between January 1, 2006, and June 30, 2021. Adult patients less than 60 years of age were stratified into expected LOS ( < 1.5 days/%TBSA) and greater than expected LOS (>1.5 days days/%TBSA). Patient demographics, TBSA, burn etiology, inhalation injury, pre-admission co-morbidities, and in-hospital complications were tabulated Logistic regression was performed using IBM SPSS Statistics 29 and Stata Statistical Software: Release 18. Results 1796 patients with a mean age of 39 years were included for analysis of the adult population. 905 patients had the expected LOS/TBSA ratio, and 891 exceeded this ratio. Univariable analysis indicated patients with greater age [1.02 (1.01 – 1.03) p < 0.0001], female sex [1.84 (1.49 – 2.27) p < 0.001], diabetes [1.50 (1.00 – 2.24) p = 0.047], psychiatric illness [1.44 (1.11 – 1.88) p = 0.006], and those who experienced complications (such as infection, graft failure, pneumonia, sepsis) [1.44 (1.20 – 1.74) p < 0.0001] during admission were more likely to exceed their predicted LOS. Multivariate analysis found that greater age [1.02 (1.01 – 1.03) p < 0.0001], female sex [1.64 (1.30 – 2.06) p < 0.0001], inhalation injury [2.18 (1.50 – 3.15) p < 0.0001], psychiatric illness [1.50 (1.01 – 1.87) p = 0.04], and complications [2.69 (2.11 – 3.43) p < 0.0001] were contributing factors that increased the likelihood of patients exceeding predicted LOS. Diabetes was not a statistically significant contributor. Conclusions Progress has been made to identify patient and injury factors that increase the likelihood of increased LOS for adult patients with burn injuries. This provides valuable data for physicians to better assess patients and improve their quality of care. Applicability of Research to Practice Identification of key burn patient characteristics aids in prognostication. Determining length of stay aids with coordination of patient care with the multi-disciplinary team.

582 Predictors of Care Discontinuity in a National Sample of Burn Patients

Abstract:Abstract Introduction Continuity of care has become a widely accepted metric of quality of care. It has been shown that care discontinuity is associated with worse outcomes in the surgical and burn populations. However, there is a scarcity of data on factors leading to this discontinuity. The aim of this study was to determine predictors of care discontinuity in a national sample of burn patients. Methods This was a retrospective study using a multicenter longitudinal database from 1996 to 2021. All adult burn patients from participating sites were included. The main outcome was care discontinuity. This was defined by having therapy or burn-related surgeries at non-index (different hospital than the one for the original admission). Multivariable logistic regression was used to examine the association between multiple demographic and clinical factors and care discontinuity. Results A total of 4,167 burn patients were included. Mean age was 43.36 (SD 15.85) years, 73% were male and 70% were white. 505 (12.1%) burn patients reported having therapy or burn-related surgeries at a non-index hospital. After adjusted analysis, we identified several predictors of care discontinuity in the burn population. Older age (Odds Ratio [OR] 1.02, 95% Confidence Interval [CI] 1.01 – 1.03), female sex (OR 1.28, 95% CI 1.01 – 1.62), Total Body Surface Area burned (OR 1.01, 95% CI 1.01 – 1.02) and number of trips to the operating room (OR 1.26, 95% CI 1.19 – 1.32) were associated with higher odds of being readmitted to a non-index hospital. However, racial minority (OR 0.65, 95% CI 0.49 – 0.88) and a longer hospital stay (OR 0.99, 95% CI 0.98 – 0.99) were associated with a lower risk of care discontinuity. Conclusions Care discontinuity was not that common in the burn population, and it is mainly driven by patient demographic and burn-specific clinical characteristics. Further research should focus on developing strategies targeted to address these disparities in order to improve continuity of care in the burn population. Applicability of Research to Practice Understanding the predictors of care discontinuity in the burn population may help clinicians develop strategies to address these disparities, decrease care fragmentation and subsequently have better outcomes.

Associated Predictors of Prolonged Length of Stay in Patients Surviving Extensive Burns: A Large Multicenter Retrospective Study.

Abstract:Background: Patients with extensive burns are critically ill and have long treatment periods. Length of stay (LOS) is a good measure for assessing treatment. This study sought to identify predictors of prolonged LOS in patients with extensive burns (>= 50% TBSA). Methods: This retrospective multicenter cohort study included adults aged >= 18 years who survived extensive burns in three burn centers in Eastern China between January 2016 and June 2022. Epidemiological, demographic and clinical outcomes data were extracted from electronic medical records and compared between patients with/without prolonged LOS, which was defined as LOS greater than the median. Logistic regression analysis was used to identify predictors of prolonged LOS. Results: The study sample included 321 patients, of whom 156 (48.6%) had an LOS of 58 days (IQR 41.0-77.0). Univariate regression analysis showed that increased total burn area and increased full -thickness burn area; electrical, chemical and other burns; increased erythrocytes, leukocytes, platelets or serum creatinine within 24 h of admission; concomitant inhalation injury, pulmonary edema, sepsis, bloodstream infection, wound infection, pulmonary infection, urinary tract infection, or HB < 70 g/L during hospitalization were associated with prolonged LOS in patients with extensive burns. Increased number of surgical operations, mechanical ventilation and renal replacement therapy were also associated with prolonged LOS (P < 0.05 or P < 0.001). Multivariate regression analysis revealed that increased total burn area (ratio 1.032, 95%CI 1.01-1.055; P = 0.004), electrical and chemical or other burns (3.282, 1.335-8.073; P = 0.01), development of wound infection (2.653 1.285-5.481; P = 0.008) and increased number of operative procedures (1.714, 1.388-2.116, P < 0.001) were significant predictors. Conclusions: Increased area of full -thickness burn,occurrence of electrical and chemical or other burns,occurrence of wound infection and increased number of surgeries are the best predictors of prolonged LOS in patients with extensive burns. 34 Evaluation of Socioeconomic Status and Burn Injuries: A Retrospective Review From a Desert Burn Center

Abstract:Abstract Introduction Incidence of burn injury rate appears to be affected by socioeconomic status (SES). Previous studies have indicated that patients with low SES have an increased risk of unintentional burn injuries. However, this association has not been explored robustly in the Southwest United States. This study aims to investigate the relationship between SES and burn injuries amongst patients in the Southwest United States. Methods A retrospective registry review was performed on all burn injury admissions to our ABA verified burn center from January 1, 2017 to December 31, 2021. Their zip code was then inputted into the Distressed Communities Index (DCI). This is a national database that stratifies communities by zip code into quintiles, indicating overall distress. The DCI was used as a measure of SES as it reflects employment status, education level, poverty rate, median income, business growth, and housing vacancies. Patients who did not report an accurate residential zip code or reported a zip code not included in the DCI were excluded. Many clinical variables were reviewed within each quintile including age, gender, total body surface area (TBSA), ICU days, hospital length of stay (LOS), ventilator days, surgical interventions, and discharge disposition. Student’s t-test and Chi-squared tests were used to compare groups, and statistical significance was defined at p < 0.05. Results A total of 1,323 patients were identified in the study period. The most distressed quintile accounted for 481 (36.4%) admissions. Flame and contact burns were the most common etiology overall (46.5%), as well as within each quintile (Figure 1). The distribution of burn injury etiology was statistically significant by age, %TBSA, gender, race, and ethnicity (p < 0.001, Table 1); however, it was not significant by SES (p = 0.202). Burn severity, categorized as less than or equal to second degree burns, or greater than or equal to third degree burns, showed significance when compared to age, LOS, ICU days, ventilator days, and total surgical interventions performed (p < 0.001). However, the association between SES and burn severity was not found to be significant (p = 0.886). Conclusions The two most distressed quintiles represented over half of the burn admissions. A lower SES did not contribute to increased burn severity or utilization of healthcare resources. This study highlighted differences in burn injuries between age, gender, race, and ethnicity. Applicability of Research to Practice Identifying communities at increased risk for burn injuries can help with public health initiatives, focused on awareness and prevention of these injuries.

Models Predicting Mortality Risk of Patients with Burns to ≥ 50% of the Total Body Surface.

Abstract:Background: Several models predicting mortality risk of burn patients have been proposed. However, models that consider all such patients may not well predict the mortality of patients with extensive burns. Method: This retrospective multicentre study recruited patients with extensive burns (>= 50% of the total body surface area [TBSA]) treated in three hospitals of Eastern China from 1 January 2016 to 30 June 2022. The performances of six predictive models were assessed by drawing receiver operating characteristic (ROC) and calibration curves. Potential predictors were sought via "least absolute shrinkage and selection operator" regression. Multivariate logistic regression was employed to construct a predictive model for patients with burns to >= 50% of the TBSA. A nomogram was prepared and the performance thereof assessed by reference to the ROC, calibration, and decision curves. Result: A total of 465 eligible patients with burns to >= 50% TBSA were included, of whom 139 (29.9%) died. The FLAMES model exhibited the largest area under the ROC curve (AUC) (0.875), followed by the models of Zhou et al. (0.853) and the ABSI model (0.802). The calibration curve of the Zhou et al. model fitted well; those of the other models significantly overestimated the mortality risk. The new nomogram includes four variables: age, the %TBSA burned, the area of full-thickness burns, and blood lactate. The AUCs (training set 0.889; internal validation set 0.934; external validation set 0.890) and calibration curves showed that the nomogram exhibited an excellent discriminative capacity and that the predictions were very accurate. Conclusion: For patients with burns to >= 50%of the TBSA, the Zhou et al. and FLAMES models demonstrate relatively high predictive ability for mortality. An Analysis of Factors Associated with Burn Injury Outcomes in Low- and Middle-Income Countries

Abstract:Background: Burn injuries are a major cause of morbidity and mortality within low- and middle-income countries (LMICs). The World Health Organization developed the Global Burn Registry to centralize data collection for the guidance of burn prevention programs. This study analyzed the epidemiologic and hospital-specific factors associated with burn injury outcomes in LMICs and high-income countries (HICs). Methods: A retrospective review was performed using the Global Burn Registry over 3 y. Patients were stratified by income region. Bivariate analyses and stepwise regressions were performed to evaluate patient and hospital demographics and variables associated with injury patterns and outcomes. Outcomes of interest included mortality and length of stay. Results: Over the study period, data were collected on 1995 patients from 10 LMICs (20 hospitals) and four HICs (four hospitals). Significantly higher mortality was seen in LMICs compared with HICs (17% versus 9%; P < 0.001). There was no significant difference between income regions for injury patterns (P = 0.062) or total body surface area of the burn injury (P = 0.077). Of the LMIC hospitals in this data set, 11% did not have reliable access to an operating theater. Conclusions: HICs had a lower overall mortality even with higher rates of concurrent injuries, as well as longer length of stay. LMIC hospitals had fewer resources available, which could explain increased mortality, given similar total body surface area. This study highlights how investing in health care infrastructure could lead to improved outcomes for patients in low-resource settings.
Associations between burn care services and impairment at discharge after burn injury: Analysis of the Global Burn Registry

Abstract:Background Throughout the world, burn injury is a major cause of death and disability. In resource-limited countries, burn injury is one of the leading causes of permanent disability among children who survive traumatic injuries, and burn injury is the fourth leading cause of disability worldwide. This study applied Andersen's model of health care access to evaluate if patient characteristics (predisposing factors), burn care service availability (enabling factors) and injury characteristics (need) are associated with physical impairment at hospital discharge for patients surviving burn injuries globally. Specifically, access to rehabilitation, nutrition, operating theatre, specialized burn unit/ services, and critical care were investigated as enabling factors. The secondary aim was to determine if associations between burn care service availability and impairment differed by country income level. Methods: This is a cross-sectional secondary analysis of prospectively collected data from the World Health Organization, Global Burn Registry. The outcome of interest was physical impairment status at discharge. Simple and multivariable logistic regressions were used to test the unadjusted and adjusted associations between the availability of burn care services and impairment status at hospital discharge, controlling for patient and injury characteristics. Effect modification was analyzed with service by country income level interaction terms added to the models and, if significant, the models were stratified by income. Results: The sample included 6622 patients from 20 countries, with 11.2% classified as having a physical impairment at discharge. In the fully adjusted model, patients had 89% lower odds of having impairment at discharge if the treatment facility provided reliable rehabilitation services compared to providing limited or no rehabilitation services (OR.11, 95%CI.08,.16, p<.01). However, this effect was modified by county income with the strong and significant association only present in high/ upper middle-income countries. Sophisticated nutritional services were also significantly associated with less impairment in high/ upper middle-income countries (OR=.04, 95% CI 0.203, 0.05, p<.01), but significantly more impairment in lower middle/low-income countries (OR=2.01, 95% CI 1.50, 2.69, p<.01). Patients had 444% greater odds of having impairment if treated at a center with specialty burn unit/ services (OR 5.44, 95%CI 3.71, 7.99, p<.01), possibly due to a selection effect. Discussion: Access to reliable rehabilitation services and sophisticated nutritional services were strongly associated with less physical impairment at discharge, but only in resource-rich countries. Although these findings support the importance of rehabilitation and nutrition after burn injury, they also highlight potential disparities in the quantity or quality of services available to burn survivors in poorer countries.

Abstract:Although animals generally prefer to eat foods with familiar rather than unfamiliar flavors, adult golden hamsters (Mesocricetus auratus) were found to do the opposite. After having prolonged exposure to a food with a particular flavor, hamsters were allowed to select between the food with the familiar flavor and the same food with a novel flavor. Hamsters consistently ate more of the food with the novel flavor, and this preference was long-lasting and resistant to extinction. Furthermore, the novelty effect was robust, being manifested in both sexes and under a variety of experimental circumstances. In contrast, rats tested under identical conditions consistently preferred the food with the familiar flavor. The origins of the novelty effect in hamsters remain to be determined.

Abstract:Abstract Introduction Mental illness and substance abuse (SA) are risk factors for burns and associated with poor outcomes. Low health literacy, adverse social determinants of health (SDoH), community level disparity, and intimate partner violence (IPV) are associated with poor outcomes, injury recidivism and readmissions in other patient populations. There are currently no formalized strategies for assessment of such factors in burn centers. This study examines the implementation of a standardized screening instrument for risks and SDoH, their prevalence, supportive services provided, and factors associated with unplanned emergency department (ED) visits and readmissions. Methods A screening assessment for health literacy, 4 specific SDoH, depression, suicidality, SA and IPV was integrated into the care of burn patients. Patient addresses were linked to the Area Deprivation Index (ADI, range 1-10). Registry and medical record data for adult patients treated March 2022 – March 2023 were retrospectively reviewed. Data include clinical characteristics, demographics, screening implementation and results, supportive services, unplanned ED visits and readmissions. Descriptive statistics were used and between group differences were assessed using student’s T-tests and Chi-Square analyses with statistical significance defined as p< 0.05. Results 305 patients were included for analysis. The mean age was 48.8, TBSA was 6.7%, LOS was 10.3 days, and ADI was 5. 215 of patients (70.5%) received screening by members of the clinical team. Of those screened, 22.4% had low or inadequate health literacy, 24% were positive for SA, 9% for depression, 10.7% for suicidality, 10.7% for IPV, and 22% had at least one adverse SDoH. Patients that were screened received more supportive in-hospital services (i.e. SA peer counselor, IPV social worker, psychologist). 58 (19%) and 42 (13.8%) of patients had unplanned ED visits and readmissions, respectively. 34.5% of ED visits and 66.7% of readmissions were related to the index injury. ADI was not associated with unanticipated ED visits or admissions. Patients screening positive for depression, SA, suicidality, housing instability, food insecurity, and transportation barriers were significantly more likely to have an unanticipated all-cause ED visit following discharge, but not readmission. Conclusions A screening tool was administered to 70.5% of patients to identify risks and adverse SDoH that ranged in prevalence from 9-24%, and increased in-hospital supportive services. Screening results were not associated with repeat admissions, but some were associated with increased risk of all-cause ED visits after discharge. Applicability of Research to Practice This study demonstrates feasibility of a universal screening tool in the burn population that identifies risks and SDoH, and can help clinical teams mobilize hospital resources to reduce poor outcomes. Supportive services after discharge may reduce ED visits related to other causes.

Abstract:Abstract Introduction The recovery journey for individuals with major burns is often protracted, necessitating extended stays at rehabilitation facilities before they can return home. We aimed to describe national resource utilization and outcomes related to inpatient rehabilitation following burn injury. Methods In this retrospective population-based study, we examined all admissions to inpatient rehabilitation facilities from April 1, 2016-March 31, 2023, available from a national repository of health administrative data. This database aggregates information from participating adult inpatient rehabilitation facilities (including 104 programs by May 2022) in 9 provinces. Results During the 7-year study period, we identified 513 burn patients who were admitted to inpatient rehabilitation in the database. Burn patients were overall younger, with a greater proportion of burn survivors under 45 years of age (25.9%-54.2%) compared to the general rehabilitation population (5.5%-6.1%). Throughout the study period, burn patients consistently experienced a longer median length of stay, which generally increased from 26.5 days in 2017 to 30.5 days in 2023. In contrast, the median length of stay for the general rehabilitation population remained stable between 22.0-23.0 days. Throughout the study period, 66.1% of burn patients reported improved pain levels upon discharge from the rehabilitation facility. Moreover, burn patients demonstrated enhancements in their motor functional scores, with an average increase score of 20.3-29.8 per year, as determined by the 18-item Functional Independence Measure instrument. Conclusions Burn patients experience extended rehabilitation admissions when compared to the general rehabilitation population. Their improvements in pain and functional scores emphasize the importance of rehabilitation in facilitating their recovery and improving their quality of life. Applicability of Research to Practice This national population-based study underscores the importance of rehabilitation services in the recovery of burn patients.

Establishment of a Predictive Model for Acute Respiratory Distress Syndrome and Analysis of Its Predictive Value in Critical Burn Patients

Abstract:Objective: To establish a predictive model for acute respiratory distress syndrome (ARDS) in critical burn patients with the screened independent risk factors, and to validate its predictive value. Methods: Totally 131 critical burn patients (101 males and 30 females, aged 18-84 years) who met the inclusion criteria were admitted to the Department of Burns of the Second Affiliated Hospital of Zhejiang University School of Medicine from January 2018 to December 2019. A retrospective case-control study was conducted. The patients were divided into ARDS group (54 cases) and non-ARDS group (77 cases) according to whether ARDS occurred or not. The statistics of patients in the two groups were recorded including the gender, age, burn index, combination of inhalation injury, smoking history, delayed resuscitation, indwelling nasogastric tube, and complication of sepsis, and the data were statistically analyzed with independent sample t test, chi-square test, and Fisher's exact probability test. The multivariate logistic regression analysis was performed on the indicators with statistically significant differences between the two groups to screen the independent risk factors for developing ARDS in critical burn patients, and the corresponding nomograph prediction model for the risk of ARDS in critical burn patients was established. The risk scores for patients developing ARDS were therefore obtained based on the above-mentioned nomograph, and the corresponding receiver operating characteristic (ROC) curve was drawn to calculate the area under the curve. The internal validation of the above-mentioned ARDS prediction model was performed using the Bootstrap method, and the area under the ROC curve was calculated for modeling group (79 cases) and validation group (52 cases), respectively. A calibration curve was drawn to assess the predictive conformity of the above-mentioned ARDS prediction model for the occurrence of ARDS in critical burn patients. Results: The burn index, proportion of combination of inhalation injury, and proportion of complication of sepsis of patients were significantly higher in ARDS group than in non-ARDS group (t=0.36, χ2=33.78, 49.92, P<0.01). The gender, age, smoking history, delayed resuscitation, and indwelling nasogastric tube of patients in ARDS group were close to those in non-ARDS group (P>0.05). The multivariate logistic regression analysis showed that the burn index, combination of inhalation injury, and complication of sepsis were the independent risk factors for developing ARDS in critical burn patients (odds ratio=1.05, 15.33, 5.02, 95% confidence interval=1.01-1.10, 2.65-88.42, 1.28-19.71, P<0.05 or P<0.01). The overall area under the ROC curve of the above-mentioned ARDS prediction model was 0.92 (95% confidence interval=0.88-0.97), and the area under the ROC curve was 0.95 and 0.91 (95% confidence interval=0.90-1.00, 0.86-0.97) for validation group and modeling group, respectively. When applying the above-mentioned ARDS prediction model for ARDS incidence prediction, there might be some risk of overestimating ARDS incidence when the prediction probability was <35.0% or >85.0%, and some risk of underestimating ARDS incidence when the prediction probability was 35.0%-85.0%. Conclusions: The burn index, inhalation injury, and sepsis are the independent risk factors for the occurrence of ARDS in critical burn patients. The risk prediction model for ARDS based on these three indicators has good predictive ability for ARDS in critical burn patients.

Abstract:Abstract Introduction Chronic co-morbidities impact the management of burn patients, as well as their short and long-term outcomes. Conversely and cumulatively, burn injury and the resulting hypermetabolic state can also worsen chronic co-morbidities. The prevalence of diagnosed or undiagnosed existing co-morbidities in burn patients is not well characterized. This study aims to identify the prevalence of new and/or unmanaged chronic co-morbidities after burn injury with admission lab work to determine whether opportunities exist in this area to improve burn care management and manage co-morbidity care. Methods This retrospective review included all patients over the age of 18 years old admitted to a regional burn center over a 6-month period (Jan- July 2022). Patient demographics, burn injury characteristics, laboratory values for hemoglobin A1c (Ha1c) and screening tests for HIV and acute hepatitis were collected. Undiagnosed diabetes mellitus (DM) was defined as no prior diagnosis/documentation of DM, no prior medication prescribed for glycemic control, and Ha1c ≥ 6.5. Uncontrolled DM was defined as a prior diagnosis/documentation of DM and a Ha1c > 9. New diagnosis of HIV was defined as no prior diagnosis/documentation of HIV, no anti-retroviral medication prescribed, and a positive screening HIV Ab/Ag result. A new diagnosis of hepatitis C was defined as no prior diagnosis/documentation of hepatitis C and a positive result on the acute hepatitis panel. Results Of the 268 patients who presented to the hospital for burn injury, 265 patients were included in the analysis. The mean age was 48.5 and 56% were male. Based on insurance status, 50% of patients had Medicaid/Medicare, 33% had private insurance, and 5% had no insurance. Within our population, 2.2% of patients experienced homelessness. Prior to admission, 16.9% of patients had a diagnosis of DM, of which 22% had uncontrolled DM. There were 7 patients (3.2%) newly diagnosed with DM, and about 60% of these patients had Ha1c levels > 9, a level high enough to consider immediate initiation of insulin therapy. Other screening tests identified a new diagnosis of HIV in 4 patients (1.5%) and hepatitis C in 9 patients (4%). For patients previously diagnosed with HIV or hepatitis C, 22% had unmanaged HIV and 75% have never received treatment for hepatitis C. Conclusions Burn injury was used as an opportunity to identify patients with new or unmanaged chronic co-morbidities. A high prevalence was seen in our population, indicating a need to continue screening. Identifying co-morbidities impacted treatment management and referral to other providers, including diabetes educators, infectious disease, and primary care physicians. Applicability of Research to Practice A screening tool to identify new or unmanaged co-morbidities is easy to implement and has the potential to improve acute and long-term management and outcomes after burn injury.

Abstract:Abstract Introduction Our state has the second highest rate of drug overdose deaths in the country, with an epicenter located in our greater metropolitan area. Burn patients using drugs can fall victim to burns commonly related to drug use or risky behavior while intoxicated. As our institution is the safety net for this community, we aimed to analyze burn presentation and outcomes of burn patients with active drug use to help clinicians anticipate challenges related to drug usage and limited socioeconomic support. Methods Retrospective cohort analysis was performed of patients over the age of 18 years presenting to our institution from 2015 to 2020 with burn injuries. We considered drug use to include marijuana, intravenous drug use, cocaine, methamphetamines, opiates, and alcohol use disorder. We analyzed admission rate with logistic regression, controlling for differences in inhalation burns, related traumatic injury, and other confounding factors. Results We reviewed a total of 1339 burn patients with 30.19% reporting drug use. On presentation, burn patients using drugs had a greater percentage of full thickness burns (+0.56%, p< 0.02), TBSA (+1.84%, p< 0.03), burn related trauma (+8.53%, p< 0.01), and concomitant inhalation burns (+3.65%, p< 0.01). Flame burns were most common overall, with scald (-6.15%) and explosion burns (+2.61%) showing the greatest change in frequency for patients using drugs (p< 0.03). During their hospital course, a larger proportion of burn patients using drugs were admitted (+16.75%, p< 0.01), and drug use was associated with a 2.4 increased log odds of admission (p< 0.01, 95% CI: 1.7-3.4). LOS, surgical intervention, ICU days, and infection showed no significant difference between cohorts. Patients using drugs were less likely to follow-up (-7.39%, p< 0.01) after discharge. Conclusions Burn victims who use drugs do not experience clinically significant increase in burn severity, such as TBSA and depth, compared with non-drug users, but were more likely to have inhalation burns and associated traumatic injury. A larger proportion of burn patients using drugs were uninsured or on Medicaid, suggesting the presence of unmanaged chronic conditions that could better explain higher rates of admission after controlling for their burns. Drug use furthermore where more likely to be lost to follow-up, possibly attributed to decreased access to healthcare secondary to insurance status and limited financial resources. Therefore, we must carefully consider discharge disposition and barriers to care to minimize post burn complications. Applicability of Research to Practice We aim to help clinicians treat burn patients who use drugs by anticipating challenges related to their usage and limited socioeconomic support.

Abstract:Abstract Introduction There is an increased incidence of renal failure in the expanding aging population. However, the effect of renal failure as a co-morbid condition on burn mortality remains overlooked in conventional predictive models. Our retrospective analysis of burn survivors and non-survivors aims to critically evaluate management of burn patients with pre-existing renal failure. Methods We conducted a retrospective analysis utilizing data from the Institutional Burn Center registry, which was linked to clinical and administrative records. Our study included all adult patients admitted to the Burn Center between January 1, 2012, and December 31, 2022. We collected and analyzed various parameters, including patient demographics, length of hospital stay (LOS), comorbid conditions, and mortality outcomes. We evaluated the association between renal failure and burn mortality using logistic regression modeling. Results Over the ten-year study period, a total of 9,955 adult patients were admitted to the BICU. 124 patients presented with a diagnosis of renal failure: 18 non-survivors, 106 survivors. Our analysis revealed that renal failure was associated with a nearly three-fold increase in the odds of mortality (OR 2.64, CI 1.33-5.01, p=0.004) when accounting for factors present in traditional predictive models such as burn size, age, and inhalational injury. Additionally, renal failure was identified as a co-morbidity in 6% of all deaths for this time frame and 14.5% of patients with this diagnosis on admission are deceased. These findings underscore the significant impact of renal failure on burn patient outcomes. Conclusions As our population continues to age, burn providers will increasingly encounter the complex challenge of managing an acute burn with multiple comorbidities. Our study highlights the intuitive understanding that patients with pre-existing renal failure have a heightened mortality risk when suffering from an acute burn injury. In light of this, it is imperative to consider potential adjustments to burn resuscitation and care protocols for this at-risk population, with the ultimate goal of enhancing the chances of survival to hospital discharge. Applicability of Research to Practice This study emphasizes the pressing need for further research into the effects of multiple comorbidities on mortality among burn patients. The insights gained from this research have practical implications for refining clinical approaches and optimizing care strategies for this vulnerable patient population.

Abstract:Abstract Introduction Mortality following burn injury is influenced by many objective factors. Over the past several decades, numerous predictive formulas have been developed to estimate the probability of death from burn injury. Despite the preponderance of models, there are relatively few widely accepted objective measures found to impact mortality in burn patients. These factors include sex, age, burn depth, TBSA, and presence of inhalation injury. In this study, we retrospectively analyzed mortality in the burn patients at our level one trauma center to identify prognostic factors. Methods A retrospective chart review was performed on all patients between 2015-2020 over the age of 18 that presented to our trauma center for burns. Significant risk factors for the prediction of mortality based on a set of objective variables were identified using a stepwise forward logistic-regression analysis. Results Of the 963 patients (mean [±SD] age, 47±17; mean TBSA, 9±13), 96% lived to discharge. The identified risk factors for death were increased age (OR 1.08, 95% CI 1.04-1.11, p< 0.001), increased TBSA (OR 1.10, 95% CI 1.06-1.13, p< 0.001), the presence of inhalation injury (OR 4.72, 95% CI 1.49-14.94, p=0.008), third degree burn depth (OR 6.09, 95% CI 1.15-32.00, p=0.033), and having diabetes (OR 3.61, 95% CI 1.17-11.08, p=0.025 (Table 1). The probability of death from patients who experience burns is described by the equation: probability=1/(1+e^(-log f ()it)) With logit equal too: logit = -9.584 + 0.072*(Age) + 0.090*(TBSA) + 1.804 (Presence Third Degree Burns) -2.231 (Presence Second & Third-Degree Burns) + 1.282 (Presence Diabetes) + 1.551 (Presence Inhalation Burns) Categorical values should be treated as 1 if they are present and 0 if they are absent. Conclusions Between 1990 and 2010 the number of people living with diabetes tripled, and the annual incidence doubled. Diabetic wound healing is characterized by excessive inflammation and reduced angiogenesis, resulting in increased risk for complications. Diabetes is an established comorbidity in burn patients, but has never been identified as a significant risk factor for mortality. Applicability of Research to Practice Analysis of burn patients at our level 1 trauma center identified history of diabetes as a significant risk factor for mortality and suggests that the inclusion of diabetic status in future mortality models would increase their prognostic value in comparable populations.
