Purpose: Amiodarone hydrochloride is an antiarrhythmic drug used to treat supraventricular tachycardia. However, there are currently no commercial pediatric forms available to treat young patients. Various oral formulations were previously reported in the literature, but the concentration was lower than the doses prescribed in clinical practice (a loading dose of 500 mg/m2/day for 7-10 days followed by a maintenance dose of 250 mg/m2/day). The objective of this study was to develop an oral liquid formulation of amiodarone hydrochloride at an optimal concentration for use in children and to evaluate its physicochemical and microbiological stability. Methods: No commercial suspension vehicle was used, allowing the choice of excipients. Compounding was performed using hydroxypropylmethylcellulose as thickener, potassium sorbate preservative, citric acid/sodium citrate buffer, sodium saccharin as a , and a strawberry flavoring agent. A concentration of 40 mg/mL was selected based on a 5-year compilation of prescribed doses. Analyses performed were the following: visual and microscopic inspection, testing for antimicrobial preservation, osmolality and pH measurements, quantification of amiodarone hydrochloride by a stability-indicating liquid chromatography method, and a microbiological count. Results: At least 95% of the initial amiodarone hydrochloride remained stable during the 60-day study period under refrigeration. All other tested parameters remained stable at 5 °C. A targeted log reduction of the microorganism inoculum by day 14 and no microbial growth by day 28 were demonstrated in the test for antimicrobial preservation. Conclusion: The stability of 40 mg/mL amiodarone hydrochloride oral suspension was maintained under refrigeration for 60 days before opening bottles and for 1 month after opening bottles.
Use of compounded dispersing media for extemporaneous pediatric syrups with candesartan cilexetil and valsartan

Abstract:Available tablets or capsules for adults are often used to prepare extemporaneously formulated medicines appropriate for children. The most acceptable drug forms in pediatric population are oral liquids and pharmacists use commercial dispersing media to compound syrups from an active substance or from tablets available on the market. In many countries ready-to-use dispersing media are not available or refunded, but pharmacists can use other compounded media, providing their compatibility and stability are proven. The aim of this study was to formulate and evaluate the stability of syrups with candesartan cilexetil (1 mg mL-1) and valsartan (4 mg mL-1) extemporaneously prepared using commercial tablets (Diovan® and Atacand®). The following three different suspending media, which could be easily made in a pharmacy, were investigated: V1 - with xanthan gum (0.5 %), V2 - the USP/NF vehicle for oral solution and V3 - the medium based on a simple sucrose syrup. The stability of preparations was studied during 35 days of storage in a dark place at controlled temperature of 25 and 4 °C. During the study, microscopic observation was carried out and pH, viscosity, and concentration of candesartan cilexetil and valsartan were analyzed. Syrups with valsartan prepared with V2 and V3 media were stable for 3 or 4 weeks when stored at 25 °C, while syrups with candesartan were stable for as long as 35 days. For syrups prepared using V1 medium, the 14-day expiry date was not achieved because of microbial deterioration.

Abstract:In this paper, we propose a 64-bit fast dynamic CMOS comparator with a small transistor count. Major features of the proposed comparator are the rearrangement and re-ordering of transistors in the evaluation block of a dynamic cell, and the insertion of a weak n feedback inverter, which helps the pull-down operation to ground. The simulation results given by prelayout tools, e.g., HSPICE, and post-layout tools, e.g., TimeMill reveal that the delay is around 2.5 ns while the operating clock rate reaches 100 MHz.
Physicochemical Stability of Compounded Allopurinol Suspensions in PCCA Base, SuspendIt

Abstract:Allopurinol is an orally administered inhibitor of xanthine oxidase used primarily in the treatment of hyperuricemia associated with gout. Allopurinol reduces serum and urinary uric acid concentrations. Its use should be individualized for each patient. The dosage of allopurinol to accomplish full control of gout and to lower serum uric acid to normal or near-normal levels varies with the severity of the disease, and needs to be flexible to permit precise, customized dose titration for individual patients. This flexibility is readily achieved using an oral liquid dosage form. However, no commercial liquid dosage form of allopurinol currently exists. Allopurinol is commercially available as 100-mg and 300-mg scored tablets. An extemporaneously compounded suspension from pure drug powder or commercial tablets would provide a convenient option to meet unique patient needs. The purpose of this study was to determine the physicochemical stability of extemporaneously compounded allopurinol suspensions in the PCCA Base SuspendIt. This base is a sugar-free, paraben-free, dye-free, and gluten-free thixotropic vehicle containing a natural sweetener obtained from the monk fruit. The study design included two allopurinol concentrations to provide stability documentation over a bracketed concentration range for eventual use by compounding pharmacists. A robust stability-indicating ultra-performance liquid chromatography assay for the determination of the chemical stability of allopurinol in SuspendIt was developed and validated. Suspensions of allopurinol were prepared in SuspendIt at 10.0-mg/mL and 20.0-mg/mL concentrations, selected to represent a range within which the drug is commonly dosed. Samples were stored in plastic amber prescription bottles at two temperature conditions (5°C and 25°C). Samples were assayed initially and at the following time points: 7 days, 14 days, 30 days, 45 days, 60 days, 88 days, 120 days, and 182 days. Physical data such as pH, viscosity, and appearance were also noted. All measurements were obtained in triplicate. A stable extemporaneous product is defined as one that retains at least 90% of the initial drug concentration throughout the sampling period. The study showed that allopurinol concentrations did not go below 93% of the label claim (initial drug concentration) at both temperatures studied. Viscosity and pH values also did not change significantly. This study demonstrates that allopurinol is physically and chemically stable in SuspendIt for 180 days in the refrigerator and at room temperature, thus providing a viable, compounded alternative for allopurinol in a liquid dosage form, with an extended beyond-use-date to meet patient needs.

Abstract:The ability to detect traces of hydrogen peroxide (H 2 O 2 ) in water is an important prerequisite to ensure a safe and reliable use of H 2 O 2 for isolator or cleanroom sanitization. While the residual airborne H 2 O 2 concentration can be easily monitored, detection of trace H 2 O 2 residues in aseptically filled drug products is challenging. In an industrial setting, samples must be pulled, handled, stored, and transported before analysis takes place. Therefore, knowledge about the analyte stability in the relevant matrix is crucial to ensure correct results.The objective of this study was to provide stability data for the analyte at low concentrations and in aqueous solutions. For this, H2O2 was spiked into four different aqueous matrices at two different concentrations and stored up to 60 days at four different storage temperatures. The tested matrices included water, buffer, and an exemplary excipient solution with and without additional protein.A developmental quantitative, fluorometric Amplex UltraRed assay was applied for analysis. The results show clearly, that of the four storage temperatures investigated, only -80°C resulted in reasonably good recovery of the spiked H2O2 content within two weeks or even up to two months of storage.

Abstract:Abstract Objectives Hydroxychloroquine (HCQ) presents many drug properties that increase its therapeutic use. There are, indeed, different research pathways in numerous autoimmune, inflammatory, and infectious diseases, as well as in cancerology. HCQ is only marketed as HCQ sulfate in film-coated or coated tablets for oral use. No pediatric liquid form is currently available on the market. The purpose of the present study is to develop oral liquid formulations for HCQ at 50 mg/mL with two different oral vehicle suspensions, namely ORA-Plus ® /ORA-Sweet ® (ORA) and Syrspend ® SF PH 4 (SYR). Methods The suspension stability was assessed in different storage conditions (4 and 25 °C). A high-pressure liquid chromatography (HPLC) stability-indicating method with UV detection was developed to determine HCQ concentrations in the different formulations, and detect potential degradation products. Physical parameters, e.g. pH and osmolality were also monitored during the period of the stability study. Results HCQ concentration, osmolality, and pH remained stable for 90 days at 4 and 30 °C for HCQ in 50% ORA-Plus ® /50% ORA-Sweet ® . For HCQ suspension in SYR, the suspension remained stable 90 days at 4 °C and 60 days at 30 °C. Conclusions For all preparations, no significant physical or chemical modification was noticed during the period of the study.
Stability of Levothyroxine, Doxycycline, Hydrocortisone, and Pravastatin in Liquid Dosage Forms Stored at Two Temperatures

Abstract:Extemporaneously prepared liquid dosage forms are needed to administer required medications in infants and young children. The goal of this study was to evaluate the stability of levothyroxine, doxycycline, hydrocortisone, and pravastatin in extemporaneously prepared suspensions stored in plastic prescription bottles under refrigeration and room temperature. Levothyroxine (25 mcg/mL), doxycycline (5 mg/mL), hydrocortisone (2 mg/mL), and pravastatin (10 mg/mL) were each prepared in two groups of suspensions. All of these suspensions were stored in plastic prescription bottles under refrigeration and at room temperature. Levothyroxine was stable for two weeks at 4°C but only one week at 25°C in both suspensions. Doxycycline was stable for two weeks in both suspensions at 4°C and 25°C. Hydrocortisone was stable for the entire two-week study period in both suspensions at both 4°C and 25°C. Pravastatin was stable for the one-week study period in both suspensions at both 4°C and 25°C. These results can be used to offer age-appropriate extemporaneously prepared medications to infants and young children when no suitable commercially available liquid formulations are available.
Determination of Hydroxyurea in Capsules and Biological Fluids by Ion-Selective Potentiometry and Fluorimetry

Abstract:Abstract Two hydroxyurea selective electrodes were investigated with β-cyclodextrin used as ionophore and either tetrakis (p-chlorophenyl) borate (electrode 1), or tetrakis [3,4-bis (trifluoromethyl) phenyl] borate (electrode 2), as a fixed anionic site in a polymeric matrix of carboxylated polyvinyl chloride. Linear responses of hydroxyurea within a concentration range of 10−5–10−3M with slopes of 51.2 and 58.6 mV/decade with pH 3–6 were obtained by using electrodes 1 and 2, respectively. Two spectrofluorimetric methods involving the formation of drug–Al(III) complex (method 3) and drug–Mg(II) complex (method 4) at pH 5 were also investigated. These complexes emit fluorescence at wavelengths of 380 and 355 nm, after excitation at 305 nm, for Al and Mg complexes, respectively. The calibration graphs were rectilinear from 0.5 to 2.5 μg/mL for the Al complex and 1 to 5 μg/mL for the Mg complex. The 4 proposed methods display useful analytical characteristics for determination of hydroxyurea, with average recoveries of 100.2 ± 0.83 and 99.4 ± 1.81% in capsules and 99.7 ± 0.70 and 99.4 ± 1.25% in biological fluids for the potentiometric and fluorimetric methods, respectively. Results obtained by the proposed procedures were statistically analyzed and compared with those obtained by the U.S. Pharmacopeial method. The 4 proposed procedures were also used to determine the stability of the drug in the presence of its degradate, hydroxylamine.

Stability Study of Epirubicin Hydrochloride Solutions for Injection Prepared From Three Different Formulations

Abstract:OBJECTIVE To evaluate the stability of epirubicin hydrochloride solutions for clinical use prepared from three different formulations(one concentrated solution and two powders for injection).METHODS Epirubicin hydrochloride in three formulations was dissolved or diluted in 0.9% sodium chloride to prepare the test solutions with concentration of 0.4 mg·mL-1.These solutions were stored at different temperatures and under different shading circumstances,and samples were collected periodically during the storage to evaluate the stability,including visual inspection,pH value and content of epirubicin.RESULTS All epirubicin solutions were stable when stored at 2-8 ℃ under dark condition for 40 d with average content of greater than 93.76%.When stored at room temperature with or without light exposure for 40 d,only the solution diluted from the concentrated water solution for injection was stable,as the average content remained above 92.57%.While the solutions prepared from the other two powders for injection were not stable,as the average content fell below 90% at room temperature after 2-7 d.CONCLUSION Epirubicin solutions prepared from three different formulations are stable when stored either at 2-8 ℃ under dark condition for 40 d or at room temperature for 24 h.The solution prepared from the concentrated water solution for injection is more stable than the solutions prepared from the two powders for injections when stored at room temperature for a long period(40 d).

Abstract:Two 60+-membered libraries of alpha-galactosylceramides have been prepared by reactions between activated ester resins and two core, fully deprotected galactosylated sphingoid bases. The libraries were evaluated for their ability to stimulate CD1d-restricted NKT cells, using in vitro stimulation of a murine NKT cell hybridoma line and for their ability to induce the expansion of NKT cells from peripheral blood mononuclear cells (PBMC) of a normal human subject. Our results showed that many compounds constructed on a C18-phytosphingosine base had significant stimulatory activity in both assays. Because no product purification was required, this approach is particularly attractive as a method for rapid synthesis of large libraries of potential immunomodulatory glycosylceramides.
Quality and Stability Evaluation of Extemporaneously Compounded Losartan Potassium Oral Suspension at the Hospital Pharmacy

Abstract:Aims: The purpose of this study was to evaluate the quality of an extemporaneous formulation of losartan potassium oral suspensions from crushed losartan potassium tablets to ensure that the compounded suspension maintained its quality attributes during its storage period. Methodology: Losartan potassium suspensions were compounded extemporaneously in the same way they are prepared for patients at a hospital pharmacy. The suspensions were kept in the refrigerator at 4°C and evaluated immediately and after 9, 18, and 28 days. Suspension ease of redispersion, color, odor, pH, particle size, zeta potential, viscosity, and sedimentation volume were all evaluated. In addition, microbiological stability, drug content, and drug dissolution were assessed. Results: During the study period, the extemporaneously compounded suspension retained its color and odor, and the pH profile remained consistent. Moreover, all suspensions were easily resuspended in a homogeneous liquid with gentle shaking, and no caking was detected. Furthermore, the results of the microbiological test revealed no microbial growth. The content uniformity and dissolution test results met the pharmacopeial requirements. The findings revealed that the losartan potassium compounding procedure was reliable and capable of producing 2.5 mg/mL of losartan potassium suspension using commercially available tablets and Ora-Blend as a suspending vehicle. Furthermore, after 28 days in the refrigerator (4°C), the suspension had acceptable quality features. Conclusion: Extemporaneously compounded suspension allows physicians to prescribe a variable dose that adjusts to each patient’s needs, providing treatment when the liquid dosage form is unavailable.
Stability of Drugs Stored in Helicopters for Use by Emergency Medical Services: A Prospective Observational Study

Abstract:Study objective: Drugs stored in rescue helicopters may be subject to extreme environmental conditions. The aim of this study was to measure whether drugs stored under the real-life conditions of a Swiss helicopter emergency medical service (HEMS) would retain their potency over the course of 1 year. Methods: A prospective, longitudinal study measuring the temperature exposure and concentration of drugs stored on 2 rescue helicopters in Switzerland over 1 year. The study drugs included epinephrine, norepinephrine, amiodarone, midazolam, fentanyl, naloxone, rocuronium, etomidate, and ketamine. Temperatures were measured inside the medication storage bags and the crew cabins at 10-minute intervals. Drug stability was measured on a monthly basis over the course of 12 months using high-performance liquid chromatography. The medications were considered stable at a minimum remaining drug concentration of 90% of the label claim. Results: Temperatures ranged from -1.2 °C to 38.1 °C (29.84 °F to 100.58 °F) inside the drug storage bags. Of all the temperature measurements inside the drug storage bags, 37% lay outside the recommended storage conditions. All drugs maintained a concentration above 90% of the label claim. The observation periods for rocuronium and etomidate were shortened to 7 months because of a supply shortage of reference samples. Conclusion: Drugs stored under the real-life conditions of Swiss HEMS are subjected to temperatures outside the manufacturer's approved storage requirements. Despite this, all drugs stored under these conditions remained stable throughout our study. Real-life stability testing could be a way to extend drug exchange intervals.
Stability of Certain Amino Acids in a Parenteral Nutrition Solution

Abstract:The stability of a commercially available hyperalimentation solution containing a mixture of 15 crystalline amino acids was studied. Twelve of the 15 amino acids were studied. Time and temperature were the variables. The commercial product was obtained in kit form and compounded according to the manufacturer's specifications. No additives were incorporated into the solutions. Each test solution contained 4.25% crystalline amino acids in 25% dextrose solution. Controls were also compounded containing 4.25% crystalline amino acids in Sterile Water for Injection, USP. The samples were stored in evacuated, sterile containers at 5 0. ambient room temperature and at 37 C. Analyses were performed at 0, 1, 3, 4, 8 and 12 weeks of storage. Each amino acid assay was performed on a Beckman Amino Acid analyzer using a single column methodology. The pH and chromagen formation were also monitored. The solution decomposition was followed by measuring the changes in concentration of the amino acids. Solutions stored at 4 C appeared to be stable over the 12- week period. The amino acid content of those samples stored at room temperature and at 37 C deteriorated significantly. Color formation and pH depression were also significant at room temperature and 37 C.

Abstract:In hospital practice it is sometimes necessary to prepare suspensions from solid dosage forms. A list of those in common use is presented. Whilst the manufacturer of the solid dosage form will usually supply a formula for preparing a suspension, stability data are not normally available. An investigation was therefore made into the stability of two such suspensions, propranolol hydrochloride (Inderal) and ampicillin (Penbritin). With propranolol hydrochloride suspension the preservative was inactivated and there was evidence of an increased rate of degradation, partly dependent on pH. The ampicillin suspension, at an unadjusted acid pH, was not satisfactory. Alternative formulae, with a shelf life of at least two weeks, are suggested.

Abstract:The effect of multi-walled carbon nanotubes (MWCNTs) and processing methods on the morphological, crystalline, dynamic mechanical, mechanical and electrical properties of MWCNT/polypropylene (PP) composites has been investigated by using field emission scanning electron microscopy (SEM), differential scanning calorimetry (DSC), dynamic mechanical analysis (DMA), tensile and electric conductivity tests. The MWCNTs have been functionalized covalently and noncovalently for better dispersion in the PP matrix. A homogeneous dispersion of MWCNTs was achieved in the PP matrix as evidenced by scanning electron microscopy. Differential scanning calorimetric (DSC) results confirmed that the incorporation of the MWCNTs effectively enhanced the crystallization of the PP matrix through heterogeneous nucleation. The glass transition temperature increased from 8 degrees C for the pure PP to 26 degrees C for the composite with 10 wt% MWCNT-COOH. The present investigation revealed that the mechanical, thermal as well as electrical properties of carbon nanotubes filled polymer composites were strongly dependent on the state of dispersion, mixing and processing methods.
Approaches to improve the stability of the antiviral agent UC781 in aqueous solutions

Abstract:In this work, we evaluated the chemical stability profiles of UC781 based solutions to identify excipients that stabilize the microbicidal agent UC781. When different antioxidants were added to UC781 in sulfobutylether-beta-cyclodextrin (SBE-beta-CD) solutions and subjected to a 50 degrees C stability study, it was observed that EDTA was a better stabilizing agent than sodium metabisulfite, glutathione or ascorbic acid. Some antioxidants accelerated the degradation of UC781, suggesting metal-catalyzed degradation of UC781. Furthermore, we observed substantial degradation of UC781 when stored in 1% Tween 80 and 1% DMSO solutions alone or in those with 10mM EDTA. On the other hand, improved stability of UC781 in the presence of 100 and 200mM of EDTA was observed in these solutions. The addition of both EDTA and citric acid in the stock solutions resulted in recovery of more than 60% of UC781 after 12 weeks. Generally, 10% SBE-beta-CD in the presence of EDTA and citric acid stabilized UC781 solutions: the amount of UC781 recovered approaching 95% after 12 weeks of storage at 40 degrees C. We also showed that the desulfuration reaction of the UC781 thioamide involves oxygen by running solution stability studies in deoxygenated media. Improved stability of UC781 in the present study indicates that the incorporation of EDTA, citric acid and SBE-beta-CD and the removal of oxygen in formulations of this drug will aid in increasing the stability of UC781 where solutions of the drug are required.

Purpose: To develop a program of chemical drug stability analysis in collaboration between Hospital Pharmacy, Medical Laboratory and Scientific Support Unit to determine the long-term stability of largely used injectable anti-infectious and non-anti-infectious drugs. Material and methods: After a setup of the High Performance Liquid Chromatography (HPLCI method, 28 drugs were reconstituted in laminar air flow hood, 17 of them stored directly at 5 +/- 3 degrees C and 19 stored in the freezer at -20 degrees C, thawed by microwave following a standardised procedure and stored at 5 +/- 3 degrees C before use. Concentration stability was evaluated by regression analysis. Results: For each drug, long-term stability has varied from 11 days to 180 days. The freeze-thaw treatment by microwave may enhance the stability (from 30 to 120 days) and allow batch-scale production of intravenous drugs, less expensive in term of manpower and sterile device than a drug reconstitution at the ward. The results were published by 55 posters in international congress and by 36 publications in national and international pharmaceutical journals. Conclusions: Our findings contribute to enhance the scale of drugs that may be take on by a CIVAS.
Physicochemical and Microbiological Stability of Compounded Bethanechol Chloride Oral Suspensions in PCCA Base, SuspendIt®

Abstract:Bethanechol chloride is a cholinergic agent used to treat acute postoperative and postpartum nonobstructive (functional) urinary retention and for neurogenic atony of the urinary bladder with retention. It is available in the United States as tablets for oral administration in four dosage strengths: 5 mg, 10 mg, 25 mg, and 50 mg. A review of the therapeutic uses of bethanechol chloride reveals the need for flexibility in dosing. This flexibility is readily achieved using an oral liquid dosage form. However, no commercial liquid dosage form of bethanechol chloride currently exists. An extemporaneously compounded suspension from pure drug powder or commercial tablets would provide a flexible, customizable option to meet unique patient needs with convenient and accurate dosing options. The purpose of this study was to determine the physicochemical and microbiological stability of extemporaneously compounded bethanechol chloride suspensions using two brands of commercially available tablets (Amneal and Upsher-Smith) in the PCCA Base, SuspendIt. This base is a sugar-free, paraben-free, dye-free, and gluten-free thixotropic vehicle containing a natural sweetener obtained from the monk fruit. The study design included two bethanechol chloride concentrations to provide stability documentation over a bracketed concentration range for eventual use by compounding pharmacists. A robust stability-indicating ultra-high-performance liquid chromatographic assay for the determination of the chemical stability of bethanechol chloride in PCCA SuspendIt was validated. Suspensions of bethanechol chloride were prepared from the tablets in PCCA SuspendIt at 1-mg/mL and 5-mg/mL concentrations, selected to represent a range within which the drug is commonly dosed. Samples were stored in amber plastic prescription bottles at room temperature (25°C). Samples were assayed initially, and on the following time points (days): 14, 30, 60, 90, and 180. Physical data such as pH and appearance were also noted. Microbiological stability was tested. A stable extemporaneous product is defined as one that retains at least 90% of the initial drug concentration throughout the sampling period and is protected against microbial growth. Using this criterion, no significant degradation of the bethanechol chloride was observed over the 180-day test period for either concentration at room temperature. Drug concentrations were at, or above 93% of initial values for both brands of commercially available tablets. No microbial growth was observed. pH values remained fairly constant. This study demonstrates that bethanechol chloride tablets are physically, chemically, and microbiologically stable in PCCA SuspendIt for 180 days at room temperature at both concentrations studied, thus providing a viable, compounded alternative for bethanechol chloride in a liquid dosage form, with an extended BUD to meet patient needs.
