Pancreatic Lipomatous Hamartoma Mimicking Other Pancreatic Tumor: a Case Report and Literature Review.

Abstract:Pancreatic lipomatous hamartoma (PLH) is an extremely rare benign entity that forms a mass-like lesion. PLH lacks distinct features, and can be preoperatively misdiagnosed as a pancreatic tumor with lipomatous components, including pancreatic lipomatosis, lipoma, liposarcoma, and malignant tumors with fatty degeneration. Here, we report a case study of PLH in a 73-year-old male who presented with abdominal pain. Abdominal-enhanced computed tomography and magnetic resonance imaging revealed a 4.3×4-cm solid mass with a lipomatous component in the pancreatic head, and the mass displayed a centripetal pattern of contrast enhancement. The patient was preoperatively diagnosed with pancreatic liposarcoma, and subsequently underwent a pancreatoduodenectomy. The postoperative pathology and immunohistochemical analyses confirmed the diagnosis of PLH, which primarily contained mature adipocytes, small ducts, and a few well-preserved pancreatic acini. Although this disease is rare, we suggest that PLH should be considered during the differential diagnosis of pancreatic lesions with lipomatous components. Consideration of the potential for PLH disease might reduce the number of unnecessary resections.
Pancreatic Primary Lymphoma: a Case Report and Review of the Literature.

Abstract:BACKGROUND:Primary pancreatic lymphoma is a rare but treatable malignancy (less than 1% of pancreatic tumors) that may be clinically confused with pancreatic adenocarcinoma.METHODS:In a patient with upper abdominal pain, ultrasonography and CT detected a mass in pancreatic head, which compressed the common bile duct. The patient received a Whipple's operation and intraoperative frozen sections of the mass showed an anaplastic carcinoma. Immunohistochemical staining was used to indicate the origin and prognosis of tumor.RESULTS:Grossly the tumor involved the pancreatic head, soft in consistence and invaded part of the gastric wall. Histologically, the tumor was composed mainly of large and moderate neoplastic cells, which were diffusely positive for CD20 and Bcl-6 antigens, indicating the features of diffusely large B cell lymphoma.CONCLUSIONS:The proper diagnosis of the tumor should be made with CD20 and Bcl-6 immunohistochemical study. Radical surgery is indicated for resectable carcinoma but not for a chemosensitive lymphoma.

Abstract:Primary pancreatic lymphoma (PPL) is an extremely rare malignant tumor that accounts for less than 2% of extranodal malignant lymphomas and 0.5% of pancreatic tumors. The most common symptoms of PPL are abdominal pain (83%), abdominal mass (58%), and weight loss (50%). The final diagnosis of PPL is based on cell histology, and the most common histological subtype is diffuse large B-cell lymphoma (DLBCL). The number of reported cases of primary pancreatic mucosa-associated lymphoid tissue (MALT) lymphoma is limited. The conversion rate of MALT lymphoma varies between 3% and 12%. However, the transformation of primary pancreatic MALT lymphoma to DLBCL has not been reported. We present a case of a 58-year-old man whose main symptoms were nausea and vomiting. Magnetic resonance imaging of the abdomen showed space-occupying lesions in the neck and body of the pancreas. Histopathological examination after surgical resection was diagnosed as primary pancreatic MALT lymphoma. Local radiotherapy was recommended and rejected by the patient's family. The disease progressed in a short time. Left supraclavicular lymph node resection biopsy and pathological examination showed DLBCL. After the initiation of chemotherapy, glaucoma appeared in the right eye during treatment, and chemotherapy was suspended. The disease progressed rapidly and the patient eventually died.

Abstract:Primary cardiac lymphoma (PCL) is a rare hematological malignancy with involvement of the heart and/or pericardium. It belongs to the extra-nodal non-Hodgkin's lymphoma (NHL). The most common histological type is diffuse large B cell lymphoma. It generally carries a poor prognosis due either to a delay in the diagnosis or to infiltration of heart structures. Here, we described a 68-year-old male patient who was diagnosed with PCL and accompanied with severe wound infection. The patient receives PR after 6 cycles of R-CHOP regimen, and the wound infection was healing after treated with broad-spectrum antibiotics. We summarize the literatures, and briefly describe the manifestations, diagnosis, treatments and prognosis of PCL. Echocardiography, CT, MRI and PET-CT are used to evaluate the tumor, and our case is the very first case-report of primary cardiac lymphoma with PET-CT scan evaluation before and after chemotherapy. PCL is a rare disease, which be diagnosed and treated in time may improve its prognosis, however, its biological and clinical characteristics are still unknown. The goals of this article are to raise awareness of this disease, show the difficulties of diagnosing and also put emphasizes on the role of chemotherapy in its treatment, so that PCL can be diagnosed and treated in time.

Abstract:Introduction: Primary squamous cell carcinoma of the pancreas (SCCP) is a rare malignant tumor that has been reported in individual case reports only. The clinical data on primary SCCP treatment are limited. Therefore, the appropriate management strategy for this disease should be standardized. Case Presentation: We present the case of a 63-year-old man admitted to our hospital for upper left abdominal pain for 2 months without weight loss or jaundice. Enhanced computed tomography (CT) and magnetic resonance imaging (MRI) of the abdomen revealed a mixed solid and cystic lesion in the pancreatic tail, measuring 35 × 37 mm in maximum diameter with enhancement. The patient was diagnosed with primary SCCP without metastasis, based on radiological and pathological findings. He did not receive neoadjuvant therapy postoperatively and was followed up by CT and MRI for 18 months without recurrence or metastasis. Result: Complete resection is the most effective treatment for early stage primary SCCP. Abdominal MRI is an effective imaging tool for preoperative evaluation and postoperative follow-up of primary SCCP. The need for neoadjuvant therapy depends on various factors. Conclusion: Primary SCCP is a tumor with poor prognosis. Risk factor control, early accurate radiologic evaluation, and individualized treatment strategies improve the quality of life and prolong the overall survival period of patients.

Abstract:Primary pancreatic lymphoma (PPL) is a rare malignancy, which is defined as a mass centered in pancreas with involvement of contiguous lymph nodes and distant spread may exist. Accurate diagnosis of PPL prior to pathological confirmation remains challenging, underscoring the critical significance of preoperative imaging assessments. This case report collected two instances of PPL that underwent initial evaluation via 18 F-fluorodeoxyglucose positron emission tomography/computed tomography ( 18 F-FDG PET/CT) between August 2021 and July 2022. Correspondingly, pertinent literature encompassing 18 F-FDG PET/CT data related to PPL was meticulously reviewed. Including our aforementioned pair of cases, a cumulative total of 25 instances of PPL were assembled. The distinctive profile of 18 F-FDG PET/CT images of PPL predominantly manifests as hypermetabolic lesions with diminished density. Primarily characterized by singular lesions and comparatively substantial volumetric dimensions, a total of eleven cases revealed contiguous lymph node engagement, with five instances displaying distant dissemination encompassing lymph nodes in multiple locations. Amongst these, ten patients underwent sequential 18 F-FDG PET/CT follow-up post-intervention. In comparison to pancreatic carcinoma, PPL lesions exhibited heightened hypermetabolism, augmented volumetric proportions, and distinct patterns of distant metastasis. This study indicates that the pivotal role of 18 F-FDG PET/CT in the diagnosis and assessment of therapeutic efficacy in PPL is unequivocal. Combined with the clinical attributes of patients, the integration of 18 F-FDG PET/CT augments the differential diagnostic capacity differentiating PPL from pancreatic carcinoma.

Abstract:Paraganglioma (PGL) is an uncommon tumor located in the head, neck and abdomen. The majority of the tumor is benign and the patient has no obvious clinical symptoms. However, PGL located in the pancreas is rather rare and tends to mimic Castleman's disease, pancreatic neuroendocrine tumors and pancreatic primary tumor. Herein, we reported a patient with PGL that occurred in the neck of the pancreas. A 75-year-old Chinese female presented to our hospital with a complaint of upper abdomen pain for two weeks and she had good past health. The laboratory findings and physical examination were all normal. Preoperative computed tomography (CT) and magnetic resonance imaging revealed a tumor located in the neck of the pancreas and a tentative diagnosis of Castleman's disease or PGL was made. We resected the tumor by laparoscopic surgery. Postoperative pathology and immunohistochemistry confirmed that the tumor was a PGL. The patient was recovered well after a postoperative follow-up of 6 months. PGL located in the neck of the pancreas is difficult to be diagnosed accurately and clinicians have difficulties in distinguishing PGL from Castleman's disease, pancreatic neuroendocrine tumors and pancreatic primary tumor. Fifteen cases were listed to show the characters of PGL located in the pancreas and we also presented the difference among PGL, Castleman's disease and pancreatic neuroendocrine tumor. We showed our experience of treating such a rare tumor hoping to help clinicians correctly diagnose and treat PGL.

Abstract:Anaplastic large-cell lymphoma (ALCL) is a subtype of T-cell lymphoma. This disease mainly affects lymph nodes, although extranodal sites may also be involved. Lymphoma in the pancreas is a rare clinical entity whether it manifests as primary or extranodal involvement. We discuss an unusual case of a 29-year-old male patient who presented with epigastric pain and a right neck mass. The patient's symptoms, physical examination, and laboratory tests prompted further investigation using imaging modalities such as CT, MRI, and endoscopic ultrasound, which revealed the presence of soft tissue masses in the right supraclavicular region and an ill-defined lesion within the pancreatic head. These findings eventually led to the identification of secondary extranodal pancreatic lymphoma. Fine needle biopsy (FNB) established an ultimate diagnosis of anaplastic lymphoma kinase (ALK)-positive ALCL.

Abstract:BACKGROUND: Pancreatic lymphomas (PLs) represent <2% of all lymphomas and <0.5% of all pancreatic neoplasms. An accurate histologic diagnosis of PL is needed to predict prognosis and adequately treat the patient. This study aims to investigate the demographic, clinical, and pathological factors affecting the prognosis and survival of pancreatic diffuse large B-cell lymphoma (DLBCL).METHODS: Demographic and clinical data from 493 cases of DLBCL of the pancreas were identified between 2000 and 2018 using the Surveillance, Epidemiology, and End Results (SEER) database.RESULTS: The most common age group was between the ages of 70 and 79 years (27.0%). While 44% of cases involved distant sites (a proxy for secondary pancreatic DLBCL), regional and localized involvement was seen in 33%, with the most common cause of death being a primary pancreatic DLBCL. Most patients (71%) received only chemotherapy (systemic therapy). The overall five-year observed survival was 46% (95% CI, 43.5-48.3). The one-year and five-year survival with chemotherapy only was 68% (95% CI, 65.3-70.3) and 48% (95% CI, 44.7-50.5), respectively. The one-year and five-year survival with surgery and chemotherapy was 96% (95% CI, 91.3-99.9) and 80% (95% CI, 71.4-89.2), respectively. Surgery with chemotherapy (HR: 0.397 (95% CI, 0.197-0.803), p = 0.010) were both positive predictors in survival prognosis. Multivariable analysis identified age >55 years (HR: 2.475 (95% CI, 1.770-3.461), p < 0.001), distant stage (HR: 6.894 (95% CI, 4.121-11.535), p < 0.001), and undergoing no surgery (HR: 2.610 (95% CI, 1.307-5.215), p = 0.007) as negative predictors for survival.CONCLUSION: PLs are rare malignant pancreatic neoplasms with DLBCL being the most common histological subtype. An accurate and timely diagnosis of pancreatic DLBCL is necessary to implement effective treatments and reduce mortality. Systemic therapy (chemotherapy) with or without surgical therapy improved survival. Increased age and regional and distant spread negatively impacted survival.

Abstract:Objective To summarize the experience in diagnosis and treatment of primary pan-creatic lymphoma. Methods The clinical data of 8 patients with primary pancreatic lymphoma treatedin our hospital from October 2002 to August 2006 were retrospectively analyzed. Results Seven tumors were located in the head of pancreas and 1 in the body and tail of pancreas. Clinical manifesta-tions included epigastric pain,epigastric tenderness,nausea and vomiting,jaundice,back pain,epigastric mass,fever,loss of weight etc. Five patients had dilatation of bile and pancreatic duct and 4 of them had slightly increase of CA19-9 in the serum while all other patients had normal level of tumor markers.Three patients had increase of β2-microglobulin and LDH in the serum. One patient underwent Whip-ple procedure,CHOP and Mabthera chemotherapy and 2 CHOP and Mabthera chemotherapy. Four patients received tumor resection and palliative operation. For these 4 patients,2 received CHOP and Mabthera chemotherapy and 1 Mabthera chemotherapy. Biopsy was performed in only 1 patient. Con-clusion The symptoms of primary pancreatic lymphoma are not specific. Its auxiliary examination can provide important information for diagnosis. Chemotherapy is the main treating tool and its prognosis is obviously superior to pancreatic cancer.
Effect of an oxytocin antagonist on prostaglandin F2 alpha secretion and the course of luteolysis in sows.

Abstract:The role of oxytocin (OT) in the regulation of prostaglandin F2 alpha (PGF2 alpha) secretion during luteolysis in gilts was studied using a highly specific OT antagonist (CAP-581). In Experiment 1 gilts on Days 14 to 19 of the oestrous cycle in Latin square design were used, to determine the dose and time of application of OT and CAP. In Group I (n = 6) gilts were treated intravenously with saline or with 10, 20 and 30 IU of OT. Concentrations of the main PGF2 alpha metabolite i.e. 13,14-dihydro-15-keto-prostaglandin F2 alpha (PGFM) were measured in blood samples as uterine response to the treatment. Twenty IU of OT was the most effective to stimulate PGFM release and this dose was used after CAP treatment in gilts of Groups II, III and IV. Gilts of Group II (n = 3) were injected into the uterine horns (UH) with saline (5 ml/horn) or CAP (2 mg, 3 mg and 4 mg; half dose/horn) and OT was injected (i.v.) 30 min thereafter. Any of the CAP doses given into the UH affected PGFM plasma concentrations stimulated by OT. In Group III (n = 4) gilts were infused (i.v.) for 30 min with CAP (9 mg, 14 mg and 18 mg/gilt) followed by 20 IU of OT. All doses of CAP effectively inhibited OT-stimulated PGF2 alpha release, therefore 9 mg was selected for the further studies. Gilts of Group IV (n = 4) received OT 4, 6 and 8 h after CAP to define how long CAP blocks the OT receptors. Concentrations of PGFM increased after any of this period of time. Thus, we concluded that 9 mg of CAP infused every 4 h will effectively block OT receptors. In Experiment 2, gilts (n = 4) received CAP as a 30-min infusion every 4 h on Days 12-20 of the oestrous cycle. Control gilts (n = 3) were infused with saline. CAP infusions diminished the height of PGFM peaks (P < 0.05). Frequency of the PGFM (P < 0.057) and OT (P < 0.082) peaks only tended to be lower in the CAP-treated gilts. Peripheral plasma concentrations of progesterone (P4) and oestradiol-17 beta (E2) and the time of luteolysis initiation as measured by the decrease of P4 concentration were the same in CAP- and saline-treated gilts. The macroscopic studies of the ovaries in gilts revealed lack of differences between groups. We conclude that OT is involved in the secretion of luteolytic PGF2 alpha peaks but its role is limited to controlling their height and frequency. Blocking of OT receptors did not prevent luteolysis in sows.
1429 Choledocholithiasis in Primary Hepatic Lymphoma (PHL): an Additional Hurdle to PHL Diagnosis?

Abstract:INTRODUCTION: Primary hepatic lymphoma (PHL) is a rare disorder involving solely liver at the time of initial presentation. Diffuse large B-cell lymphoma (DLBCL) variant in PHL is scarce. Patients with Immune suppression and HIV infection are at increased risk for the PHL-DLBCL variant. We present an individual, whose initial clinical presentation is masked my choledocholithiasis, subsequently diagnosed with PHL-DLBCL by histopathology. CASE DESCRIPTION/METHODS: A 59-year-old-man with prostate cancer (Gleason score of 7) managed conservatively for the last two years presented to the ED with intermittent epigastric pain of one-week duration associated with subjective fever and night sweats. On examination, he was afebrile and hemodynamically stable. The liver was palpable below the right costal margin and found to have inguinal lymphadenopathy. The laboratory data revealed obstructive jaundice with a cholestatic pattern (Table 1). Abdominal U/S and MRCP revealed cholelithiasis with normal biliary anatomy. Viral hepatitis, autoimmune and metabolic markers of liver disease were normal. Given the worsening of the obstructive pattern of liver tests, the patient underwent ERCP which showed two small stones in the distal CBD. Cholestasis improved, and the patient was discharged home in stable condition. Three weeks later he was readmitted with significant weight loss, malaise and night sweats. The laboratory investigation showed worsening of cholestasis with no imaging evidence of biliary stent obstruction. Hence, liver biopsy was performed to evaluate infiltrative liver disease. His hospital course was complicated by multiple organ failure requiring mechanical ventilator support and renal replacement therapy. Despite all resuscitative efforts, patient's general condition deteriorated and expired. His liver biopsy was reported with findings suggestive of DLBCL. Extrahepatic involvement was ruled out by negative lymph node biopsy. DISCUSSION: PHL is very rare primary hepatic malignancy and, DLBCL variant is extremely rare. As laboratory and imaging findings in PHL are non-specific, it is difficult to distinguish from infiltrative, granulomatous and other malignant liver diseases. Liver biopsy is essential for diagnosis and identifying the immunophenotype. It is vital to consider this condition as outcomes in PHL vary from other liver diseases. Our patient is a unique example where coexistent choledocholithiasis posed a diagnostic challenge like prior reported cases in literature.
Primary Colonic Lymphoma: Report of Two Cases and a Literature Review

Abstract:Primary colonic lymphoma is a very rare malignant tumor with no standard treatment. We report two cases of primary colonic lymphoma successfully treated with surgery and chemotherapy, and chemotherapy alone, respectively. The first case was a 61-year-old woman who presented with abdominal pain of more than 1 month. The patient was diagnosed with a colonic tumor, and immunohistochemical examinations confirmed the initial diagnosis of colonic lymphoma. The patient underwent laparoscopic-assisted right hemicolectomy followed by postoperative adjuvant chemotherapy with the cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) regimen, combined with targeted therapy with rituximab (R-CHOP). The second case was a 78-year-old man who presented with a complaint of abdominal distention for more than 1 year. Diffuse large B-cell lymphoma was definitively diagnosed by immunohistochemical examinations, and the patient underwent systemic chemotherapy with the R-CHOP regimen. Primary colonic lymphoma is a rare type of non-Hodgkin's lymphoma (NHL), and the clinical treatment is not standardized, unlike for many other types of lymphoma. Therefore, treatment is mainly based on the patient’s symptoms to determine whether surgery or systemic chemotherapy is appropriate. Rituximab is effective in some patients and may play an important role in the treatment of unresectable or asymptomatic colonic lymphoma.

Abstract:T-cell lymphomas are a rare group of hematologic malignancies, occurring in approximately 0.05 cases per 100,000 people. This case report highlights an unusual presentation of T-cell lymphoma with pancreaticobiliary metastasis, initially presenting as cholestatic liver injury, and underscores the importance of considering hematologic malignancies in the differential diagnosis of obstructive jaundice.

Abstract:Primary breast lymphoma (PBL) is a rare malignant lymphoid neoplasm limited to the breast, accounting for about 0.15% of all malignant breast tumors and 1.7% to 2.2% of extra-nodal lymphomas. PBL must be distinguished from conventional breast carcinomas due to different therapeutic approaches. A 25-year-old female presented with a left breast mass. Histopathology and immunohistochemical tests confirmed the diagnosis of diffuse large B-cell lymphoma (DLBCL). She had no similar lesions elsewhere in the body. She received 1 cycle of R-CHOP chemotherapy but absconded from the treatment and succumbed afterward while at home. Recent developments in DLBCL treatment have greatly improved patient outcomes by incorporating targeted medicines like rituximab, increased chemotherapy regimens, new drugs, and individualized treatment techniques. PBL appears to have a worse prognosis; thus, delay or abscondment from treatment is of serious concern when it comes to improving the prognosis of patients with PBL.
Effective Management of a Rare Case of Primary Renal Intravascular Large B-cell Lymphoma with Modified R-CHOP Regimen: A Case Report

Abstract:Intravascular large B-cell lymphoma, known for its diverse organ involvement, presents significant diagnostic challenges, particularly when it affects the kidneys. This report highlights a rare case of primary renal intravascular large B-cell lymphoma in a 60-year-old male patient, who presented with persistent fever and renal dysfunction. The case underscores the intricacy of diagnosis and the efficacy of personalized treatment. Following the identification of primary renal intravascular large B-cell lymphoma, a modified R-CHOP regimen was administered, resulting in notable amelioration of symptoms and renal function following the initial treatment cycle. The patient achieved sustained complete remission without any complications after completing five subsequent R-CHOP cycles and two additional cycles of rituximab monotherapy, as confirmed by recent assessments. He is currently under regular follow-up for ongoing monitoring and improvement. This case adds to the limited yet expanding pool of knowledge concerning intravascular large B-cell lymphoma, emphasizing the necessity for personalized therapeutic strategies in atypical presentations. It also highlights the importance of early detection and customized intervention in managing rare lymphoma subtypes with unique organ involvement.
