Abstract:Abstract BACKGROUND Stereotactic radiosurgery (SRS) is a cornerstone in the management of Breast Cancer Metastases to the Brain (BCMB). While control rates are high following SRS, radiation necrosis is a rare but potentially devastating long-term toxicity. There is a clinical need for automated/semi-automated methods to assess tumor response and optimize the RT plans for local control with minimal long-term toxicity. Multiparametric MRI (mpMRI), particularly Apparent Diffusion Coefficient of water (ADC) maps, contain information that is mechanistically relatable to voxel-level tumor response to RT. We report a deep learning-based approach to predict post-SRS ADC maps, FLAIR, T2-weighted (T2W), T1-weighted unenhanced (T1W) and contrast-enhanced (T1WCE) images, from pre-SRS T1W, T1WCE, T2W and FLAIR images, ADC maps, and the delivered RT dose map. These “forward models” will enable the radiation oncologist to simulate radiologic outcomes and iteratively optimize RT plans for local control with minimal toxicity. METHODS We trained a variant of the pix2pix Generative Adversarial Network (GAN) on MRI and RT dose map data from 18 BCMB patients treated with stereotactic radiation with confirmed controlled and locally recurrent metastases. Patients were treated with stereotactic radiation dose of 1-40 Gy between 2013-2019. RESULTS On test data from 6 BCMB patients, the trained forward model predicted post-SRS ADC values within the Gross Tumor Volume (GTV) that were broadly in agreement with ground truth post-SRS ADC maps. In agreement with expectations, the forward model also predicts increasing post-RT ADC within the GTV with increasing simulated RT doses in the range of 1-71 Gy. We have also explored an inverse model to predict the RT dose map required to produce “prescribed” post-SRS ADC values within the GTV. CONCLUSIONS We envision that the forward models will assist the radiation oncologist in initial RT dose plan optimization, while the inverse model may be useful for daily RT plan optimization.

Abstract:Purpose/Objective(s) To develop a novel deep ensemble learning model for accurate prediction of brain metastasis(BM) local control outcomes following stereotactic radiosurgery(SRS). Materials/Methods A total of 114 BMs from 82 patients were evaluated, including 26 BMs that developed biopsy-confirmed local failure post-SRS. The SRS spatial dose distribution( D map ) of each BM was registered to the planning contrast-enhanced T1(T1-CE) MR. Axial slices of the D map , T1-CE, and PTV segmentation( PTV seg ) intersecting the BM center were extracted within a fixed field-of-view determined by the V60% in D map . A spherical projection was implemented to transform planar image content onto a spherical surface using multiple projection centers, and the resultant T1-CE/ D map / PTV seg projections were stacked as a 3-channel variable. Four VGG-19 deep encoders were utilized in an ensemble design, with each sub-model using a different spherical projection formula as input for BM outcome prediction. In each sub-model, clinical features after positional encoding were fused with VGG-19 deep features to generate logit results. The ensemble's outcome was synthesized from the four sub-model results via logistic regression. A total of 10 model versions with random validation sample assignments were trained to study model robustness. Performance was compared to 1) a single VGG-19 encoder; 2) an ensemble with T1-CE MRI as the sole image input after projections; and 3) an ensemble with the same image input design without clinical feature inclusion. Results The ensemble model achieved an excellent AUC ROC =0.89±0.02 with high sensitivity(0.82±0.05), specificity(0.84±0.11), and accuracy(0.84±0.08) results. This outperformed the MRI-only VGG-19 encoder (sensitivity:0.35±0.01, AUC:0.64±0.08), the MRI-only deep ensemble (sensitivity:0.60±0.09, AUC:0.68±0.06), and the 3-channel ensemble without clinical feature fusion (sensitivity:0.78±0.08, AUC:0.84±0.03). Conclusion Facilitated by the spherical image projection method, a deep ensemble model incorporating D map and clinical variables demonstrated an excellent performance in predicting BM post-SRS local failure. Our novel approach could improve other radiotherapy outcome models and warrants further evaluation.

Abstract:BACKGROUNDMachine-learning models have demonstrated great promise in predicting tumor infiltration beyond anatomical margins in glioblastoma. Yet standard-of-care (SOC) radiation therapy (RT) planning only utilizes a uniform, isotropic 2cm-expansion of the T1-post-contrast-lesion or T2-lesion to generate a clinical target volume (CTV), without considering heterogeneity in tumor infiltration. We hypothesize that using a novel deep-learning approach to predict regions of tumor progression with metabolic and diffusion-weighted MRI will result in personalized CTVs that are more sensitive to detecting normal-appearing infiltrating tumor beyond the T2-lesion, sparing more healthy brain compared to SOC-CTVs. METHODSAnatomical, diffusion-weighted, and metabolic MRI from 101 patients with glioblastoma, acquired post-surgery but prior to chemoradiation, were retrospectively used to predict regions of new contrast-enhancement or T2-hyperintensity at confirmed progression. A segmentation-based predictive deep-learning approach with personalized loss-functions and evaluation metrics ("Progression-Coverage-Coefficient", PCC) that weight Dice coefficient terms by tumor size was employed with a 66/17/18 train/validation/test split of patients and the resulting DL-CTV compared to two SOC-CTVs: 1) the RTOG[T2-FLAIR+2cm]-CTV, 2) the EORTC[T1c+2cm]-CTV. RESULTSOur best deep-learning-CTV trained using anatomical, diffusion, and metabolic MRI achieved significantly improved median test specificity (0.87 vs. 0.77; p<0.005) and comparable sensitivity (0.92 vs. 0.95; p=0.1) and PCC (0.81 vs. 0.79; p=0.1) to the SOC RTOG[T2-FLAIR+2cm]-CTV, sparing more normal brain tissue. Compared to the more conservative EORTC[T1c+2cm]-CTV, our approach had significantly higher test sensitivity (0.92 vs. 0.85; p<0.00001) and PCC (0.81 vs. 0.77; p=0.003) in covering the progressed lesion, while maintaining similar specificity (0.87 vs. 0.89; p=0.3). Test performance dropped by 9.6% and 12.4% when removing metabolic and both metabolic and diffusion metrics, respectively (p<0.001). CONCLUSIONThis study demonstrates the benefit of using metabolic and diffusion MRI with deep learning for personalized RT planning by targeting regions of anatomically normal infiltrative disease that are highly likely to progress, while reducing treatment to normal brain.

Abstract:Objectives: Stereotactic radiosurgery (SRS), a therapy that uses radiation to treat brain tumors, has become a significant treatment procedure for patients with brain metastasis (BM). However, a proportion of patients have been found to be at risk of local failure (LF) after treatment. Hence, accurately identifying patients with LF risk after SRS treatment is critical to the development of successful treatment plans and the prognoses of patients. To accurately predict BM patients with the occurrence of LF after SRS therapy, we develop and validate a machine learning (ML) model based on pre-treatment multimodal magnetic resonance imaging (MRI) radiomics and clinical risk factors. Patients and methods: In this study, 337 BM patients were included (247, 60, and 30 in the training set, internal validation set, and external validation set, respectively). Four clinical features and 223 radiomics features were selected using least absolute shrinkage and selection operator (LASSO) and Max-Relevance and Min-Redundancy (mRMR) filters. We establish the ML model using the selected features and the support vector machine (SVM) classifier to predict the treatment response of BM patients to SRS therapy. Results: In the training set, the SVM classifier that uses a combination of clinical and radiomics features demonstrates outstanding discriminative performance (AUC=0.95, 95% CI: 0.93-0.97). Moreover, this model also achieves satisfactory results in the validation sets (AUC=0.95 in the internal validation set and AUC=0.93 in the external validation set), demonstrating excellent generalizability. Conclusions: This ML model enables a non-invasive prediction of the treatment response of BM patients receiving SRS therapy, which can in turn assist neurologist and radiation oncologists in the development of more precise and individualized treatment plans for BM patients.

Abstract:Background: The study investigated whether three deep-learning models, namely, the CNN_model (trained from scratch), the TL_model (transfer learning), and the FT_model (fine-tuning), could predict the early response of brain metastases (BM) to radiosurgery using a minimal pre-processing of the MRI images. The dataset consisted of 19 BM patients who underwent stereotactic-radiosurgery (SRS) within 3 months. The images used included axial fluid-attenuated inversion recovery (FLAIR) sequences and high-resolution contrast-enhanced T1-weighted (CE T1w) sequences from the tumor center. The patients were classified as responders (complete or partial response) or non-responders (stable or progressive disease). Methods: A total of 2320 images from the regression class and 874 from the progression class were randomly assigned to training, testing, and validation groups. The DL models were trained using the training-group images and labels, and the validation dataset was used to select the best model for classifying the evaluation images as showing regression or progression. Results: Among the 19 patients, 15 were classified as "responders" and 4 as "non-responders". The CNN_model achieved good performance for both classes, showing high precision, recall, and F1-scores. The overall accuracy was 0.98, with an AUC of 0.989. The TL_model performed well in identifying the "progression" class, but could benefit from improved precision, while the "regression" class exhibited high precision, but lower recall. The overall accuracy of the TL_model was 0.92, and the AUC was 0.936. The FT_model showed high recall for "progression", but low precision, and for the "regression" class, it exhibited a high precision, but lower recall. The overall accuracy for the FT_model was 0.83, with an AUC of 0.885. Conclusions: Among the three models analyzed, the CNN_model, trained from scratch, provided the most accurate predictions of SRS responses for unlearned BM images. This suggests that CNN models could potentially predict SRS prognoses from small datasets. However, further analysis is needed, especially in cases where class imbalances exist.
Development and validation of a radiomics-based prediction pipeline for the response to stereotactic radiosurgery therapy in brain metastases

Abstract:ObjectivesThe first treatment strategy for brain metastases (BM) plays a pivotal role in the prognosis of patients. Among all strategies, stereotactic radiosurgery (SRS) is considered a promising therapy method. Therefore, we developed and validated a radiomics-based prediction pipeline to prospectively identify BM patients who are insensitive to SRS therapy, especially those who are at potential risk of progressive disease.MethodsA total of 337 BM patients (277, 30, and 30 in the training set, internal validation set, and external validation set, respectively) were enrolled in the study. 19,377 radiomics features (3 masks × 3 MRI sequences × 2153 features) extracted from 9 ROIs were filtered through LASSO and Max-Relevance and Min-Redundancy (mRMR) algorithms. The selected radiomics features were combined with 4 clinical features to construct a two-stage cascaded model for the prediction of BM patients' response to SRS therapy using SVM and an ensemble learning classifier. The performance of the model was evaluated by its accuracy, specificity, sensitivity, and AUC curve.ResultsRadiomics features were integrated with the clinical features of patients in our optimal model, which showed excellent discriminative performance in the training set (AUC: 0.95, 95% CI: 0.88–0.98). The model was also verified in the internal validation set and external validation set (AUC 0.93, 95% CI: 0.76–0.95 and AUC 0.90, 95% CI: 0.73–0.93, respectively).ConclusionsThe proposed prediction pipeline could non-invasively predict the response to SRS therapy in patients with brain metastases thus assisting doctors to precisely designate individualized first treatment decisions.Clinical relevance statementThe proposed prediction pipeline combines the radiomics features of multi-modal MRI with clinical features to construct machine learning models that noninvasively predict the response of patients with brain metastases to stereotactic radiosurgery therapy, assisting neuro-oncologists to develop personalized first treatment plans.Key Points• The proposed prediction pipeline can non-invasively predict the response to SRS therapy.• The combination of multi-modality and multi-mask contributes significantly to the prediction.• The edema index also shows a certain predictive value.

Post-Radiotherapy PET Image Outcome Prediction by Deep Learning Under Biological Model Guidance: A Feasibility Study of Oropharyngeal Cancer Application

Abstract:Purpose: To develop a method of biologically guided deep learning for post-radiation 18 FDG-PET image outcome prediction based on pre-radiation images and radiotherapy dose information. Methods: Based on the classic reaction–diffusion mechanism, a novel biological model was proposed using a partial differential equation that incorporates spatial radiation dose distribution as a patient-specific treatment information variable. A 7-layer encoder–decoder-based convolutional neural network (CNN) was designed and trained to learn the proposed biological model. As such, the model could generate post-radiation 18 FDG-PET image outcome predictions with breakdown biological components for enhanced explainability. The proposed method was developed using 64 oropharyngeal patients with paired 18 FDG-PET studies before and after 20-Gy delivery (2 Gy/day fraction) by intensity-modulated radiotherapy (IMRT). In a two-branch deep learning execution, the proposed CNN learns specific terms in the biological model from paired 18 FDG-PET images and spatial dose distribution in one branch, and the biological model generates post-20-Gy 18 FDG-PET image prediction in the other branch. As in 2D execution, 718/233/230 axial slices from 38/13/13 patients were used for training/validation/independent test. The prediction image results in test cases were compared with the ground-truth results quantitatively. Results: The proposed method successfully generated post-20-Gy 18 FDG-PET image outcome prediction with breakdown illustrations of biological model components. Standardized uptake value (SUV) mean values in 18 FDG high-uptake regions of predicted images (2.45 ± 0.25) were similar to ground-truth results (2.51 ± 0.33). In 2D-based Gamma analysis, the median/mean Gamma Index (<1) passing rate of test images was 96.5%/92.8% using the 5%/5 mm criterion; such result was improved to 99.9%/99.6% when 10%/10 mm was adopted. Conclusion: The developed biologically guided deep learning method achieved post-20-Gy 18 FDG-PET image outcome predictions in good agreement with ground-truth results. With the breakdown biological modeling components, the outcome image predictions could be used in adaptive radiotherapy decision-making to optimize personalized plans for the best outcome in the future.

Improving 3D dose prediction for breast radiotherapy using novel glowing masks and gradient-weighted loss functions

Abstract:Background: The quality of treatment plans for breast cancer can vary greatly. This variation could be reduced by using dose prediction to automate treatment planning. Our work investigates novel methods for training deep-learning models that are capable of producing high-quality dose predictions for breast cancer treatment planning. Purpose: The goal of this work was to compare the performance impact of two novel techniques for deep learning dose prediction models for tangent field treatments for breast cancer. The first technique, a "glowing" mask algorithm, encodes the distance from a contour into each voxel in a mask. The second, a gradient-weighted mean squared error (MSE) loss function, emphasizes the error in high-dose gradient regions in the predicted image. Methods: Four 3D U-Net deep learning models were trained using the planning CT and contours of the heart, lung, and tumor bed as inputs. The dataset consisted of 305 treatment plans split into 213/46/46 training/validation/test sets using a 70/15/15% split. We compared the impact of novel "glowing" anatomical mask inputs and a novel gradient-weighted MSE loss function to their standard counterparts, binary anatomical masks, and MSE loss, using an ablation study methodology. To assess performance, we examined the mean error and mean absolute error (ME/MAE) in dose across all within-body voxels, the error in mean dose to heart, ipsilateral lung, and tumor bed, dice similarity coefficient (DSC) across isodose volumes defined by 0%-100% prescribed dose thresholds, and gamma analysis (3%/3 mm). Results: The combination of novel glowing masks and gradient weighted loss function yielded the best-performing model in this study. This model resulted in a mean ME of 0.40%, MAE of 2.70%, an error in mean dose to heart and lung of -0.10 and 0.01 Gy, and an error in mean dose to the tumor bed of -0.01%. The median DSC at 50/95/100% isodose levels were 0.91/0.87/0.82. The mean 3D gamma pass rate (3%/3 mm) was 93%. Conclusions: This study found the combination of novel anatomical mask inputs and loss function for dose prediction resulted in superior performance to their standard counterparts. These results have important implications for the field of radiotherapy dose prediction, as the methods used here can be easily incorporated into many other dose prediction models for other treatment sites. Additionally, this dose prediction model for breast radiotherapy has sufficient performance to be used in an automated planning pipeline for tangent field radiotherapy and has the major benefit of not requiring a PTV for accurate dose prediction.
Predicting local control of brain metastases after stereotactic radiotherapy with clinical, radiomics and deep learning features

Abstract:Background and purpose: Timely identification of local failure after stereotactic radiotherapy for brain metastases allows for treatment modifications, potentially improving outcomes. Previous studies showed that adding radiomics or Deep Learning (DL) features to clinical features increased Local Control (LC) prediction accuracy. However, no study has integrated radiomics, DL, and clinical features into machine learning algorithms to predict LC. We examined whether a model using all these features achieves better accuracy than models using only a subset. Materials and methods: We collected pre-treatment brain MRIs and clinical data for 129 patients at the Gamma Knife Center of the Elisabeth-TweeSteden Hospital. Radiomics features (extracted using the Python radiomics feature extractor) and DL features (extracted using a 3D ResNet model) were combined with clinical features. Performance of a Random Forest classifier was compared across four models trained with: clinical features only; clinical and radiomics features; clinical and DL features; and clinical, radiomics, and DL features. Results: The prediction model utilizing only clinical variables provided an Area Under the receiver operating characteristic Curve (AUC) of 0.82 and an accuracy of 75.6%. Adding radiomics features increased the AUC to 0.88 and accuracy to 83.3%, while adding DL features resulted in an AUC of 0.86 and accuracy of 78.3%. The best performance came from combining clinical, radiomics, and DL features, achieving an AUC of 0.89 and accuracy of 87%. Conclusion: Integrating radiomics and DL features with clinical characteristics improves LC prediction after stereotactic radiotherapy for brain metastases. These findings demonstrate the potential for early outcome prediction, enabling timely treatment modifications to improve patient management.
Predicting voxel-level dose distributions of single-isocenter volumetric modulated arc therapy treatment plan for multiple brain metastases

Abstract:Purpose: Brain metastasis is a common, life-threatening neurological problem for patients with cancer. Single-isocenter volumetric modulated arc therapy (VMAT) has been popularly used due to its highly conformal dose and short treatment time. Accurate prediction of its dose distribution can provide a general standard for evaluating the quality of treatment plan. In this study, a deep learning model is applied to the dose prediction of a single-isocenter VMAT treatment plan for radiotherapy of multiple brain metastases. Method: A U-net with residual networks (U-ResNet) is employed for the task of dose prediction. The deep learning model is first trained from a database consisting of hundreds of historical treatment plans. The 3D dose distribution is then predicted with the input of the CT image and contours of regions of interest (ROIs). A total of 150 single-isocenter VMAT plans for multiple brain metastases are used for training and testing. The model performance is evaluated based on mean absolute error (MAE) and mean absolute differences of multiple dosimetric indexes (DIs), including ( D max and D mean ) for OARs, ( D 98 , D 95 , D 50 , and D 2 ) for PTVs, homogeneity index, and conformity index. The similarity between the predicted and clinically approved plan dose distribution is also evaluated. Result: For 20 tested patients, the largest and smallest MAEs are 3.3% ± 3.6% and 1.3% ± 1.5%, respectively. The mean MAE for the 20 tested patients is 2.2% ± 0.7%. The mean absolute differences of D 98 , D 95 , D 50 , and D 2 for PTV60, PTV52, PTV50, and PTV40 are less than 2.5%, 3.0%, 2.0%, and 3.0%, respectively. The prediction accuracy of OARs for D max and D mean is within 3.2% and 1.2%, respectively. The average DSC ranges from 0.86 to 1 for all tested patients. Conclusion: U-ResNet is viable to produce accurate dose distribution that is comparable to those of the clinically approved treatment plans. The predicted results can be used to improve current treatment planning design, plan quality, efficiency, etc.

Abstract:Dose planning for Gamma Knife radiosurgery (GKRS) uses the magnetic resonance (MR)-based tissue maximum ratio (TMR) algorithm, which calculates radiation dose without considering heterogeneous radiation attenuation in the tissue. In order to plan the dose considering the radiation attenuation, the Convolution algorithm should be used, and additional radiation exposure for computed tomography (CT) and registration errors between MR and CT are entailed. This study investigated the clinical feasibility of synthetic CT (sCT) from GKRS planning MR using deep learning. The model was trained using frame-based contrast-enhanced T1-weighted MR images and corresponding CT slices from 54 training subjects acquired for GKRS planning. The model was applied prospectively to 60 lesions in 43 patients including benign tumor such as meningioma and pituitary adenoma, metastatic brain tumors, and vascular disease of various location for evaluating the model and its application. We evaluated the sCT and compared between treatment plans made with MR only (TMR 10 plan), MR and real CT (rCT; Convolution with rCT [Conv-rCT] plan), and MR and synthetic CT (Convolution with sCT [Conv-sCT] plan). The mean absolute error (MAE) of 43 sCT was 107.35 ± 16.47 Hounsfield units. The TMR 10 treatment plan differed significantly from plans made by Conv-sCT and Conv-rCT. However, the Conv-sCT and Conv-rCT plans were similar. This study showed the practical applicability of deep learning based on sCT in GKRS. Our results support the possibility of formulating GKRS treatment plans while considering radiation attenuation in the tissue using GKRS planning MR and no radiation exposure.

Abstract:High-grade glioma (HGG) presents significant treatment challenges, particularly in identifying less responsive tumor areas to target with adaptive radiotherapy (RT). This study introduces a patient-specific computational pipeline designed to predict and spatially map HGG response to RT, focusing on regions demonstrating resilience to RT. Our computational pipeline is centered around a family of 3D biophysical models describing tumor cell invasion, proliferation, and response to RT which can be personalized for individual patients using longitudinal multiparametric MRI data. Utilizing longitudinal multiparametric MRI data reporting on cell density (diffusion weighted MRI) and the extent of disease ( T 2 -FLAIR and contrast-enhanced T 1 -weighted MRIs) from 21 patients, ranging from baseline to three weeks into RT, we calibrated a family of biophysical models to forecast tumor cell density up to the final week of RT and one-month follow-up. By employing pareto tail analysis on empirical cumulative distribution function fits of voxel-wise changes in apparent diffusion coefficient maps (ΔADC) between the future images (end of RT, one-month follow-up) and pre-treatment images, we identified areas of increased tumor cellularity on both the ground truth and model predicted cellularity maps. The analysis differentiated tumor voxels into "non-responding" and "responding" categories, based on cutoffs determined through ROC analysis across various conditions—including assessments of both enhancing and non-enhancing tumor regions at different time points. Optimal ΔADC cutoffs for predicted and measured data were established, leading to high area under curve (AUC; ranging from 0.803 to 0.934) values and significant Youden's index scores (ranging from 0.526 to 0.765) across all conditions. Our findings demonstrate the feasibility of this novel pipeline in delineating biologically relevant RT targets, offering a potential tool for enhancing the efficacy of adaptive RT strategies in treating HGG.

Abstract:A method is described for the simultaneous determination of Cu, Fe, and Zn in 25 ..mu..L of serum, using simultaneous, multielement atomic absorption spectrometry (SIMAAC) and graphite furnace atomization. Peak height measurements are more adversely affected than peak area measurements by both the organic and inorganic constituents in serum. Peak area measurements are significantly more precise and accurate than peak height measurements. Determinations are carried out at a 1:21 dilution of serum, in the concentration range with the optimum precision and minimum carryover contamination and memory effects for these elements, using SIMAAC. The serum specimens are diluted with Triton X-100 (10 mL/L), 10 mM nitric acid, and 30 mM ammonium nitrate. The use of peak area measurements and atomization from a L'vov platform permits calibration with acidified aqueous standards. A commercially available, protein-based reference material, Cation-Cal (American Dade) and a bovine serum pool are used as controls. The method is shown to have good correlation to comparative methods (single-element flame AAS after deproteinization) for the analysis of 37 serum samples.

Abstract:PurposeTo investigate the potential of texture analysis and machine learning to predict treatment response to transarterial radioembolization (TARE) on pre-interventional cone-beam computed tomography (CBCT) images in patients with liver metastases.Materials and MethodsIn this IRB-approved retrospective single-center study 36 patients with a total of 104 liver metastases (56 % male, mean age 61.1 ± 13 years) underwent CBCT prior to TARE and follow-up imaging 6 months after therapy. Treatment response was evaluated according to RECIST version 1.1 and dichotomized into disease control (partial response/stable disease) versus disease progression (progressive disease). After target lesion segmentation, 104 radiomics features corresponding to seven different feature classes were extracted with the pyRadiomics package. After dimension reduction machine learning classifications were performed on a custom artificial neural network (ANN). Ten-fold cross validation on a previously unseen test data set was performed.ResultsThe average administered cumulative activity from TARE was 1.6 Gbq (± 0.5 Gbq). At a mean follow-up of 5.9 ± 0.8 months disease control was achieved in 82 % of metastases. After dimension reduction, 15 of 104 (15 %) texture analysis features remained for further analysis. On a previously unseen set of liver metastases the Multilayer Perceptron ANN yielded a sensitivity of 94.2 %, specificity of 67.7 % and an area-under-the receiver operating characteristics curve of 0.85.ConclusionOur study indicates that texture analysis-based machine learning may has potential to predict treatment response to TARE using pre-treatment CBCT images of patients with liver metastases with high accuracy.

In general, Mg++ and K+ act on the cellular adhesion and on the stability of the membrane by forming bonds between the charges of the lipids and the proteins. Mg++ acts on the passive K+ transfer either by activation or inhibition. In addition, Mg++ acts on the K+ active transfer: the presence of Mg++ is essential to maintain the activity of the membrane (Na+K) adenosine triphosphatase (ATPase).

Abstract:Background The advancements in artificial intelligence and computational power have made deep learning an attractive tool for radiotherapy treatment planning. Deep learning has the potential to significantly simplify the trial‐and‐error process involved in inverse planning required by modern treatment techniques such as volumetric modulated arc therapy (VMAT). In this study, we explore the ability of deep learning to predict organ‐at‐risk (OAR) dose–volume histograms (DVHs) of left‐sided breast cancer patients undergoing VMAT treatment based solely on their anatomical characteristics. The predicted DVHs could be used to derive patient‐specific dose constraints and dose objectives, streamlining the treatment planning process, standardizing the quality of the plans, and personalizing the treatment planning. Purpose This study aimed to develop a deep learning‐based framework for the prediction of organ‐specific dose–volume histograms (DVH) based on structures delineated for left‐sided breast cancer treatment. Methods We used a dataset of 249 left‐sided breast cancer patients treated with tangential VMAT fields. We extracted delineated structures and dose distributions for each patient and derived slice‐by‐slice DVHs for planning target volume (PTV) and organs‐at‐risk. The patients were divided into training (70%, n = 174), validation (10%, n = 24), and test (20%, n = 51) sets. Collected data were used to train a deep learning model for the prediction of the DVHs based on the delineated structures. The developed deep learning model comprised a modified DenseNet architecture followed by a recurrent neural network. Results In the independent test set (n = 51), the point‐wise differences in the slice‐by‐slice DVHs between the clinical and predicted DVHs were small; the mean squared errors were 3.53, 1.58, 2.28, 3.37, and 1.44 [×10−4] for PTV, heart, ipsilateral lung, contralateral lung, and contralateral breast, respectively. With the derived cumulative DVHs, the mean absolute difference ± standard deviation of mean doses between the clinical and the predicted DVH were 0.08 ± 0.04 Gy, 0.24 ± 0.22 Gy, 0.73 ± 0.46 Gy, 0.07 ± 0.06 Gy, and 0.14 ± 0.14 Gy for PTV, heart, ipsilateral lung, contralateral lung, and contralateral breast, respectively. Conclusions The deep learning‐based approach enabled automatic and reliable prediction of the DVH based on delineated structures. The predicted DVHs could potentially serve as patient‐specific clinical goals used to aid treatment planning and avoid suboptimal plans or to derive optimization objectives and constraints for automated treatment planning.

Abstract:Abstract Introduction: Radiotherapy is dependent on an expert-based model that impedes implementation of many potentially useful applications that could aid decision makers. This model is a barrier to a learning healthcare system due to noise introduced by decision-maker variation. Surmounting this barrier will require compelling clinical outcome data and highly useful and accurate information systems. We sought to develop new augmented-intelligence capabilities to better predict both high-dose lung irradiation treatment efficacy (i.e., local tumor control) and toxicity (i.e., radiation-induced pneumonitis). Methods: We curated one of the largest datasets of lung stereotactic body radiotherapy (SBRT) datasets globally (~1200 cases). We input pre-therapy lung computed tomography (CT) images using a bounding box approach that captures the gross tumor volume and adjacent normal lung into a multi-task deep neural network to generate an image fingerprint that predicts local failures after treatment. Separately, we input axial slices from CT scans of the lung into convolutional neural network (CNN) and trained using an outcome per slice method. We then aggregated intra-patient slices into a recurrent neural network (RNN) to estimate individual patient risk for pneumonitis. We validated our findings with the independent study cohort. Risk scores for local failure and pneumonitis were combined with clinical variables to assess the relative contribution of the deep learning-derived image risk scores. Results: A total of 1045 patients met our eligibility criteria. Radiation treatments in patients with high local failure risk scores failed at a significantly higher rate than in patients with low scores; 3-year cumulative incidence of local failure in the internal study cohort was 20·3% (16·0-24·9) in patients with high scores and 5·7% (95% CI 3·5-8·8) in patients with low scores (hazard ratio [HR]=3·64 [95% CI 2·19-6·05], p<0·0001). Models that included the local failure risk score and clinical variables predicted treatment failures with a concordance index (C-index) of 0·72 (95% CI 0·67-0·77), a significant improvement compared with classical radiomics (p<0·0001) or clinical variables (p<0·0001) alone. Radiation pneumonitis risk scores were highly predictive of toxicity, with an accuracy and CI of 0.78 (95% CI 0.72-0.85). A model that includes dosimetry variates (V20), improved the CI to 0.85 (95% CI 0.80-0.90). Conclusions: The image-based deep learning frameworks developed herein represent the first opportunity to use medical images to individualize radiotherapy dose and toxicity predictions. Our results signify a new roadmap for deep learning-guided predictions and treatment guidance in the image-replete and highly standardized discipline of radiation oncology. MRI-Based Radiomics Signature for the Prediction of Response of Lung Cancer Brain Metastases after Whole-Brain Radio-Therapy

Abstract:Local response prediction for brain metastases (BMs) from lung cancer after Whole-Brain Radiotherapy (WBRT) is challenging, as existing criteria are based solely on unidimensional measurements. This study sought to determine whether radiomic features of lung cancer BMs derived from pre-treatment magnetic resonance imaging (MRI) could be used to predict local response following WBRT.A total of 88 Lung Cancer patients with BMs treated with WBRT were analyzed. After volumes of interest were drawn, 944 radiomic features including first-order, shape, Gray Level Co-occurrence Matrix (GLCM), Gray Level Dependence Matrix (GLDM), Gray Level Run Length Matrix (GLRLM), Gray Level Size Zone Matrix (GLSZM), Neighborhood Gray Tone Difference Matrix (NGTDM), and Laplacian of Gaussian (LoG) features were extracted, using the baseline pre-treatment post-contrast T1 (T1c) and T2 fluid-attenuated inversion recovery (FLAIR) MRI sequences, respectively. Local response status was determined by contrasting the baseline and follow-up MRI according to the RANO-BM criteria. The independent samples t test or Mann-Whitney U test, and then least absolute shrinkage and selection operator (LASSO) were used for dimensionality reduction and feature selection. An adaboost classifier was trained using the selected radiomic features and evaluated using the area under the receiver operating characteristic curve (AUC) in both the training and testing sets. Other discrimination metrics, including classification accuracy, positive predictive value (PPV), negative predictive value (NPV), sensitivity, and specificity, were also calculated.The optimal radiomics signature was developed based on a multivariable logistic regression with 4, 5, 6 radiomic features on T1c, T2 FLAIR and T1c+T2 FLAIR, respectively. The radiomics model based on T1c features presented the AUC of (0.920 vs. 0.805, respectively) for both the training and testing sets, followed by T2 FLAIR features (0.893 vs. 0.701, respectively), and T1c+T2 FLAIR features (0.971 vs. 0.857, respectively). The classification accuracy of the radiomics model also well predicted the local response of BMs for both the models), and training and testing sets (T1c: 82.9% vs. 77.8%, T2 FLAIR: 82.9% vs. 77.8%, T1c+T2 FLAIR: 90.0% vs. 77.8%, respectively).Radiomics holds promise for predicting local tumor response following WBRT in patients with lung cancer and brain metastases. A predictive model built on radiomic features from an institutional cohort performed well on cross-validation testing. These results warrant further validation in independent datasets. Such work could prove invaluable for guiding management of individual patients and assessing outcomes of novel interventions.
Predict Post-Radiotherapy PET Image for Anal Cancer Patients Treated with Chemoradiation Using 3D U-Net Deep Learning

Abstract:Purpose/Objective(s) This study aims to predict voxel-wise therapy response PET image for anal cancer radiotherapy utilizing pre-radiotherapy PET/CT, planning targets and dose, toward estimating the treatment outcome and reducing hematologic toxicity. Materials/Methods 18FDG-PET/CT scans were obtained from locally advanced anal cancer patients with 2-week prior to (pre-RT), 12-week following treatment (post-RT). All PET/CT images were registered to the planning CT. Patients were treated with the IMRT with the mean dose of 55.8Gy. The prediction of post-RT PET image was constructed by 3D U-Net deep learning with five spatial levels. The input of 3D U-Net has seven separate channels, including PTV, V40, pelvic bone, bladder, Pre-RT PET, planning CT, and planned dose. All images were resized to the volumes with 128 × 128 × 128 voxels. The prediction was calculated in the volume of interested (VOI) where the dose is larger than 5% prescribed dose. To reduce the bias due to high bladder uptake, the SUV in the bladder of post-RT PET was reduced to the mean liver uptakes, and the resultant post-RT PET images were used in the study. Online data augmentation was applied during both training and testing stages. The performance of the prediction model was evaluated by the voxel-based mean absolute error and joint histograms. The ANVOA was used to identify difference between the predicted post-RT PET images and ground truth images for VOIs, including PTV and OARs. Results Fifty-five patients with pre-RT and post-RT PET images were included in the study. Among them, 30 patients were randomly selected as training data set, and 10 patients were used for the validation set, and 15 patients were used as the test set. For the evaluated volumes (> 5% prescribed dose), the similarity of predicted image and ground truth were evaluated by the joint histogram with voxelwise correlation (mean correlation coefficient, 91. 21% +/- 5.36). The difference between the mean SUV of predicated PET image and the ground truth was evaluated by ANOVA with p=0.422. In the volume of PTV, the mean SUV of predicted image was 1.10+/-0.11, and 1.08+/-0.30 for ground truth, p=0.988. In the volume of V40, the mean SUV of predicted image was 1.00+/-0.06, and 1.05+/-0.20 for ground truth, p=0.549. We also evaluated the mean uptake of pelvic bone marrow with results of 0.89+/-0.20 for predicated image, and 0.81+/-0.11 for ground truth, p=0.261. Our results show that there is no significant difference in the target volumes and OARs between predicted PET image and ground truth images. Conclusion Our approach can effectively predict the post-RT PET image based on the pre-treatment image and treatment information. 3D deep learning prediction of metabolic treatment outcome provides a feasible approach for evaluating efficacy of local control of anal cancer treatment. The results can be used to predict local failure of disease, toward implementing response-driven adaptive radiotherapy.

Prediction of Obliteration After the Gamma Knife Radiosurgery of Arteriovenous Malformations Using Hand-Crafted Radiomics and Deep-Learning Methods

Abstract:Introduction: Brain arteriovenous malformations (bAVMs) are vascular abnormalities that can be treated with embolization or radiotherapy to prevent the risk of future rupture. In this study, we use hand-crafted radiomics and deep learning techniques to predict favorable vs. unfavorable outcomes following Gamma Knife radiosurgery (GKRS) of bAVMs and compare their prediction performances. Methods: One hundred twenty-six patients seen at one academic medical center for GKRS obliteration of bAVMs over 15 years were retrospectively reviewed. Forty-two patients met the inclusion criteria. Favorable outcomes were defined as complete nidus obliteration demonstrated on cerebral angiogram and asymptomatic recovery. Unfavorable outcomes were defined as incomplete obliteration or complications relating to the AVM that developed after GKRS. Outcome predictions were made using a random forest model with hand-crafted radiomic features and a fine-tuned ResNet-34 convolutional neural network (CNN) model. The performance was evaluated by using a ten-fold cross-validation technique. Results: The average accuracy and area-under-curve (AUC) values of the Random Forest Classifier (RFC) with radiomics features were 68.5 ±9.80% and 0.705 ±0.086, whereas those of the ResNet-34 model were 60.0 ±11.9% and 0.694 ±0.124. Four radiomics features used with RFC discriminated unfavorable response cases from favorable response cases with statistical significance. When cropped images were used with ResNet-34, the accuracy and AUC decreased to 59.3 ± 14.2% and 55.4 ±10.4%, respectively. Conclusions: A hand-crafted radiomics model and a pre-trained CNN model can be fine-tuned on pre-treatment MRI scans to predict clinical outcomes of AVM patients undergoing GKRS with equivalent prediction performance. The outcome predictions are promising but require further external validation on more patients.
