Abstract:Understanding the organization and mechanical function of the extracellular matrix (ECM) is critical for the development of therapeutic strategies that regulate wound healing following disease or injury. However, these relationships are challenging to elucidate during remodeling following myocardial infarction (MI) due to rapid changes in cellularity and an inability to characterize both ECM microstructure and function non-destructively. In this study, we overcome those challenges through whole organ decellularization and non-linear optical microscopy to directly relate the microstructure and mechanical properties of myocardial ECM. We non-destructively quantify collagen organization, content, and cross-linking within decellularized healthy and infarcted myocardium using second harmonic generation (SHG) and two photon excited autofluorescence. Tensile mechanical testing and compositional analysis reveal that the cumulative SHG intensity within each image volume and the average collagen autofluorescence are significantly correlated with collagen content and elastic modulus of the ECM, respectively. Compared to healthy ECM, infarcted tissues demonstrate a significant increase in collagen content and fiber alignment, and a decrease in cross-linking and elastic modulus. These findings indicate that cross-linking plays a key role in stiffness at the collagen fiber level following infarction, and highlight how this non-destructive approach to assessing remodeling can be used to understand ECM structure-function relationships.
Automated measurement of cardiomyocyte monolayer contraction using the Exeter Multiscope

Abstract:We apply a novel microscope architecture, the Exeter Multiscope, to the problem of acquiring image data in rapid succession from nine wells of a 96 well plate. We demonstrate that the new microscope can detect contraction in cardiomyocyte monolayers which have been plated into these wells. Each well is sampled using 500 x 500 pixels across a 1.4 x 1.4 mm field of view, acquired in three colours at 3.7 Hz per well. The use of multiple illumination wavelengths provides post-hoc focus selection, further increasing the level of automation. The performance of the Exeter Multiscope is benchmarked against industry standard methods using a commercial microscope with a motorised stage and demonstrates that the Multiscope can acquire data almost 40 times faster. The data from both Multiscope and the commercial systems are processed by a 'pixel variance' algorithm that uses information from the pixel value variability over time to determine the timing and amplitude of tissue contraction. This algorithm is also benchmarked against an existing algorithm that employs an absolute difference measure of tissue contraction.

Abstract:Quantification of organic tissue’s material parameters has significant implication in clinic diagnosis of cardiac diseases,since material parameters are far more sensitive measures of the object intrinsic property changes.With known nodal kinematic data and boundary constrains,a new 3D elastic modulus estimation algorithm was based on finite-element method and H∞ filter.Simulation results proved the efficiency of the method compared with the least square method.A myocardiac 3D elastography using cardiac MRI images was presented as well.

Abstract:The heart is a mechanical pump that is electrically driven. We have previously shown that the contractility of the cardiac muscle can reliably be used in order to assess the extent of ischemia using myocardial elastography. Myocardial elastography estimates displacement and strain during the natural contraction of the myocardium using signal processing techniques on echocardiograms in order to assess the change in mechanical properties as a result of disease. In this paper, we showed that elastographic techniques can be used to estimate and image both the mechanics and electromechanics of normal and pathological hearts in vivo. In order to image the mechanics throughout the entire cardiac cycle, the minimum frame rate was determined to be on the order of 150 fps in a long-axis view and 300 fps in a short-axis view. The incremental and cumulative displacement and strains were measured and imaged for the characterization of normal function and differentiation from infracted myocardium. In order to image the electromechanical function, the incremental displacement was imaged inconsecutive cardiac cycles during end-systole in both dogs and humans. The contraction wave velocity in normal dogs was found to be twice higher than in normal humans and twice lower than in ischemic dogs. In conclusion, we have demonstrated that elastographic techniques can be used to detect and quantify the mechanics and electromechanics of the myocardium in vivo. Ongoing investigations entail assessment of myocardial elastography in characterizing and quantifying ischemia and infarction in vivo.
Quantification of myocardial extracellular volume without blood sampling

Abstract:Aims: Cardiac magnetic resonance (CMR) T1 relaxation time mapping is an established technique primarily used to identify diffuse interstitial fibrosis and oedema. The myocardial extracellular volume (ECV) can be calculated from pre- and post-contrast T1 relaxation times and is a reproducible parametric index of the proportion of volume occupied by non-cardiomyocyte components in myocardial tissue. The conventional calculation of the ECV requires blood sampling to measure the haematocrit (HCT). Given the high variability of the HCT, the blood collection is recommended within 24 h of the CMR scan, limiting its applicability and posing a barrier to the clinical routine use of ECV measurements. In recent years, several research groups have proposed a method to determine the ECV by CMR without blood sampling. This is based on the inverse relationship between the T1 relaxation rate (R1) of blood and the HCT. Consequently, a 'synthetic' HCT could be estimated from the native blood R1, avoiding blood sampling. Methods and results: We performed a review and meta-analysis of published studies on synthetic ECV, as well as a secondary analysis of previously published data to examine the effect of the chosen regression modell on bias. While, overall, a good correlation and little bias between synthetic and conventional ECV were found in these studies, questions regarding its accuracy remain. Conclusion: Synthetic HCT and ECV can provide a 'non-invasive' quantitative measurement of the myocardium's extracellular space when timely HCT measurements are not available and large alterations in ECV are expected, such as in cardiac amyloidosis. Due to the dependency of T1 relaxation times on the local setup, calculation of local formulas using linear regression is recommended, which can be easily performed using available data.

Abstract:Automated cardiac image interpretation has the potential to transform clinical practice in multiple ways including enabling low-cost serial assessment of cardiac function in the primary care and rural setting. We hypothesized that advances in computer vision could enable building a fully automated, scalable analysis pipeline for echocardiogram (echo) interpretation. Our approach entailed: 1) preprocessing; 2) convolutional neural networks (CNN) for view identification, image segmentation, and phasing of the cardiac cycle; 3) quantification of chamber volumes and left ventricular mass; 4) particle tracking to compute longitudinal strain; and 5) targeted disease detection. CNNs accurately identified views (e.g. 99% for apical 4-chamber) and segmented individual cardiac chambers. Cardiac structure measurements agreed with study report values (e.g. mean absolute deviations (MAD) of 7.7 mL/kg/m2 for left ventricular diastolic volume index, 2918 studies). We computed automated ejection fraction and longitudinal strain measurements (within 2 cohorts), which agreed with commercial software-derived values [for ejection fraction, MAD=5.3%, N=3101 studies; for strain, MAD=1.5% (n=197) and 1.6% (n=110)], and demonstrated applicability to serial monitoring of breast cancer patients for trastuzumab cardiotoxicity. Overall, we found that, compared to manual measurements, automated measurements had superior performance across seven internal consistency metrics with an average increase in the Spearman correlation coefficient of 0.05 (p=0.02). Finally, we developed disease detection algorithms for hypertrophic cardiomyopathy and cardiac amyloidosis, with C-statistics of 0.93 and 0.84, respectively. Our pipeline lays the groundwork for using automated interpretation to support point-of-care handheld cardiac ultrasound and large-scale analysis of the millions of echos archived within healthcare systems.
Highly automatic quantification of myocardial oedema in patients with acute myocardial infarction using bright blood T2-weighted CMR

Abstract:Background: T2-weighted cardiovascular magnetic resonance (CMR) is clinically-useful for imaging the ischemic area-at-risk and amount of salvageable myocardium in patients with acute myocardial infarction (MI). However, to date, quantification of oedema is user-defined and potentially subjective. Methods: We describe a highly automatic framework for quantifying myocardial oedema from bright blood T2-weighted CMR in patients with acute MI. Our approach retains user input (i.e. clinical judgment) to confirm the presence of oedema on an image which is then subjected to an automatic analysis. The new method was tested on 25 consecutive acute MI patients who had a CMR within 48 hours of hospital admission. Left ventricular wall boundaries were delineated automatically by variational level set methods followed by automatic detection of myocardial oedema by fitting a Rayleigh-Gaussian mixture statistical model. These data were compared with results from manual segmentation of the left ventricular wall and oedema, the current standard approach. Results: The mean perpendicular distances between automatically detected left ventricular boundaries and corresponding manual delineated boundaries were in the range of 1-2 mm. Dice similarity coefficients for agreement (0=no agreement, 1=perfect agreement) between manual delineation and automatic segmentation of the left ventricular wall boundaries and oedema regions were 0.86 and 0.74, respectively. Conclusion: Compared to standard manual approaches, the new highly automatic method for estimating myocardial oedema is accurate and straightforward. It has potential as a generic software tool for physicians to use in clinical practice.
Automated three-dimensional echocardiographic quantification for left ventricular volume and function in patients with hypertrophic cardiomyopathy

Abstract:Background: Accurate, reproducible, noninvasive determination of left ventricular (LV) volumes and ejection fraction (EF) is important for clinical assessment, selection of therapy, and serial monitoring of patients with hypertrophic cardiomyopathy (HCM). Current clinical Two-dimensional echocardiography (2DE) may cause inaccurate measurements in patients with HCM because of their asymmetric ventricles and limitations of 2DE technology. Three-dimensional echocardiography (3DE) have demonstrated significantly greater accuracy. However, the time-consuming workflow limits the clinical utility of 3DE. Aim: We aim to compare the performance of a novel automated 3DE system (HeartModel, Philips Healthcare) with 2DE in a group of patients with HCM. Cardiac magnetic resonance (CMR) was reference standard. Methods: Fifty-three patients with HCM were examined by automated 3DE (3DEA), two-dimensional biplane Simpson's method (2DBP), manual 3DE method, and CMR, respectively. For patients with poor automated quantification, manual correction was performed. The Pearson correlation coefficient and Bland-Altman analysis and paired Student t tests were used to assess inter-technique agreement. Results: 3DEA measurements with contour editing correlate well with CMR and manual 2DE and 3DE measurements (r = .80-.96). The analysis time of 3DEA was shorter than that of 2DBP (3DEA, 141 ± 15s; 2DBP, 174 ± 17 s). Inter-observer variability was reduced significantly with use of 3DEA. Conclusion: Compared with current clinical 2DBP method, the analysis time of automated 3DE was much shorter with the added benefit of enhanced accuracy and reproducibility. Patients with asymmetric chamber may rely more on the timesaving automated 3DE quantification in the future.
Data sharing in toxicology: beyond show and tell.

Abstract:In the past several months I have participated in numerous discussions about data sharing. It reminded me of kindergarten. The teacher would ask the students to bring in objects for “show and tell.” I recall proudly showing some artifact of childhood to my peers. I would beam as my friends acknowledge the treasure I beheld with “oohs” and “ahhs.” Yet, minutes later when a friend asks to hold it or play I would reflexively wince as a wave of unease washed over me. What happened? I had something very exciting to share with my friends. Why did I bristle at the logical step of letting my friends experience what I had? I suspect that the visceral resistance to sharing may not be such a bad thing. Those feelings emanate from the strong connection with the object. When one has cared for and nurtured an item, one feels an obligation to protect it. In kindergarten, we were encouraged to overcome these feelings. In fact, when possessions are shared with like-minded friends the experience around that object can grow and lead to new avenues of play and interaction—hobbies, clubs, etc. My teacher was correct. It is good to share.
AUTOMATED CONTOUR TRACKING FOR MYOCARDIAL ELASTOGRAPHY IN VIVO

Abstract:Using a high-resolution (30 MHz) ultrasound system (Vevo 770, VisualSonics Inc.) and a retrospective ECG-gating technique, an extremely high frame-rate (up to 8 kHz) was previously shown achievable for full-view (12-mmtimes12-mm) myocardial elastography. In-vivo experiments were performed in anesthetized normal and infarcted mice. In this paper, myocardial contour was manually initialized in the first frame of the cine-loop and automatically tracked over the entire cardiac cycle, which correctly determined the region of interest (ROI) for better interpretation. The end-systolic cumulative displacement and strain showed that motion and deformation in the infarcted myocardium were significantly reduced, and that the infarcted region underwent thinning, rather than thickening, during systole. In conclusion, high frame rate, full-view myocardial elastography with automated contour tracking could provide regional strain information of the left ventricle throughout an entire cardiac cycle, and characterize normal as well as detect abnormal myocardial function, such as an infarction.

Abstract:Myocardial fibrosis is a common endpoint of many cardiac diseases and increasingly recognized as a predictor of heart failure, arrhythmia, and death. Recent studies have utilised cardiac computed tomography (CT) scans with delayed phase imaging to quantify diffuse fibrosis of the myocardium. CT extracellular volume (CT-ECV) measurement correlates well with CMR and histological myocardial fibrosis. Furthermore, CT-ECV predicts outcomes such as death, heart failure and arrhythmia in various disease states. This review summarizes the rationale and methodology behind CT-ECV measurement and provides a detailed summary of the current clinical evidence for the use of CT-ECV.

Abstract:OBJECTIVE:To demonstrate the feasibility of the method to detect myocardial function based on integrated backscatter.METHOD:Using the experimental system developed by our research group, the RF signal of B-mode image was sampled and digitized. Wavelet transform method was adopted for cyclic variation of integrated backscatter (CVIB) calculation. The CVIB image of the region of interest (ROI) was obtained.RESULT:The CVIB results of 11 cases with myocardial infarction were provided by data and image. Results of 23 normal people including 8 middle-aged and 15 young were also given for comparison.CONCLUSION:The method based on integrated backscatter can differentiate ischemic myocardium from normal people and it has potential to be used in clinic.
Methodology for Jointly Assessing Myocardial Infarct Extent and Regional Contraction in 3-D CMRI

Abstract:Automated extraction of quantitative parameters from Cardiac Magnetic Resonance Images (CMRI) is crucial for the management of patients with myocardial infarct. This work proposes a post-processing procedure to jointly analyze Cine and Delayed-Enhanced (DE) acquisitions in order to provide an automatic quantification of myocardial contraction and enhancement parameters and a study of their relationship. For that purpose, the following processes are performed: 1) DE/Cine temporal synchronization and 3D scan alignment, 2) 3D DE/Cine rigid registration in a region about the heart, 3) segmentation of the myocardium on Cine MRI and superimposition of the epicardial and endocardial contours on the DE images, 4) quantification of the Myocardial Infarct Extent (MIE), 5) study of the regional contractile function using a new index, the Amplitude to Time Ratio (ATR). The whole procedure was applied to 10 patients with clinically proven myocardial infarction. The comparison between the MIE and the visually assessed regional function scores demonstrated that the MIE is highly related to the severity of the wall motion abnormality. In addition, it was shown that the newly developed regional myocardial contraction parameter (ATR) decreases significantly in delayed enhanced regions. This largely automated approach enables a combined study of regional MIE and left ventricular function.

Scanning the Imaging Horizon for Hypertrophic Cardiomyopathy

Abstract:In this article some of the recent advances in the use of noninvasive imaging applied to patients with hypertrophic cardiomyopathy (HCM) are discussed. Echocardiography and cardiac computed tomography are briefly discussed with respect to their power to detect apical aneurysmal disease. Echocardiographic phenotype-genotype correlations and the use of echocardiography to characterize myocardial work are reviewed. Positron emission tomography is reviewed in the context of ischemia imaging and also in the context of the use of a new tracer that might allow for recognition of early activation of the fibrosis pathway. Next, the technical capabilities of cardiovascular magnetic resonance to measure myocardial perfusion, oxygenation, and disarray are discussed as they apply to HCM. The application of radiomics to improve prediction of sudden cardiac death is touched upon. Finally, a deep learning approach to the recognition of HCM vs phenocopies is presented as a potential future diagnostic aid in the not-too-distant future.
Myocardial Late Iodine Enhancement and Extracellular Volume Quantification with Dual-Layer Spectral Detector Dual-Energy Cardiac CT

Abstract:PURPOSE: To explore the usefulness of myocardial late iodine enhancement (LIE) and extracellular volume (ECV) quantification by using dual-energy cardiac CT.MATERIALS AND METHODS: In this single-center retrospective study, a total of 40 patients were evaluated with LIE CT by using a dual-layer spectral detector CT system. Among these, 21 also underwent cardiac MRI. Paired image sets were created by using standard imaging at 120 kVp, virtual monochromatic imaging (VMI) at 50 keV, and iodine density imaging. The contrast-to-noise ratio and image quality were then compared. Two observers assessed the presence of LIE and calculated the interobserver agreements. Agreement between CT and cardiac MRI when detecting late-enhancing lesions and calculating the ECV was also assessed.RESULTS: The contrast-to-noise ratio was significantly higher by using VMI than by using standard 120-kVp imaging, and the mean visual image quality score was significantly higher by using VMI than by using either standard or iodine density imaging. For interobserver agreement of visual detection of LIE, the agreement for VMI was excellent and the κ value (κ, 0.87) was higher than that for the standard 120-kVp (κ, 0.70) and iodine density (κ, 0.83) imaging. For detecting late-enhancing lesions, agreement with cardiac MRI was excellent by using VMI (κ, 0.90) and iodine density imaging (κ, 0.87) but was only good by using standard 120-kVp imaging (κ, 0.66). Quantitative comparisons of the ECV calculations by using CT and cardiac MRI showed excellent correlation (r 2 = 0.94).CONCLUSION: Dual-energy cardiac CT can assess myocardial LIE and quantify ECV, with results comparable to those obtained by using cardiac MRI.© RSNA, 2019See also the commentary by Litt in this issue.

Detection and quantification of regional wall motion abnormalities using phase analysis of equilibrium gated cardiac studies

Abstract:Phase images obtained from equilibrium gated cardiac studies were evaluated for qualitative and quantitative information and correlated with contrast angiography in 33 patients. The left ventricular region of interest was obtained by a semiautomatic procedure which avoided underestimation of size but also eliminated extraventricular pixels. Phase images and phase distribution histograms were arranged in three standard displays which included the whole heart, isolated left ventricle, isolated abnormal areas, and quantification of maximum phase shift in the whole free border and in the inferolateral and posterolateral segments. Only the free left ventricular border was evaluated. According to contrast angiography results, four categories were obtained: normal, hypokinetic, akinetic, and dyskinetic. The best correlation with contrast angiography was found with the results obtained by dividing the left ventricular free border in two segments (r = 0.91). Scheffe's test for multiple comparisons showed significant differences between each of the four categories. Expressed in phase shifts from the histogram mode, the lower limits for the three abnormal categories were established at 30 degrees, 78 degrees, and 156 degrees respectively for hypokinetic, akinetic, and dyskinetic segments. For distinguishing normal from abnormal segments, sensitivity = 83%, specificity = 94%, and accuracy = 89%. The main reasons for discrepant results appeared to be small hypokinetic areas in an otherwise normal ventricle, very large area of hypokinesia, segments adjacent to an area of marked abnormality, and ventricles with asynchrony (wave-like motion). The method described provides an image which characterizes myocardial synchrony, generates clear boundaries for abnormal areas, and lends itself to quantification.

Abstract:Background and Objectives: Myocardial perfusion imaging (MPI) has an important role in the non-invasive investigation of coronary artery disease. The interpretation of MPI studies is mainly based on the visual evaluation of the reconstructed images, while automated quantitation methods may add useful data for each patient. However, little evidence is currently available regarding the actual incremental clinical diagnostic performance of automated MPI analysis. In the present study, we aimed to assess the correlation between automated measurements of Summed Stress Score (SSS), Summed Rest Score (SRS) and Summed Difference Score (SDS), with the corresponding expert reading values, using coronary angiography as the gold standard. Materials and Methods: The study was conducted at the Nuclear Medicine Laboratory of the University Hospital of Larissa, Larissa, Greece, οver an one-year period (January 2019-January 2020). 306 patients, with known or suspected coronary artery disease, were enrolled in the study. Each participant underwent a coronary angiography, prior to or after the scintigraphic study (within a three-month period). Either symptom-limited treadmill test, or pharmacologic testing using adenosine or regadenoson, was performed in all participants, and the scintigraphic studies were carried out using technetium 99m (99mTc) tetrofosmin (one-day stress/rest protocol). Coronary angiographies were scored according to a 4-point scoring system (angiographic score; O: normal study, 1: one-vessel disease, 2: two-vessel disease, 3: three-vessel disease). Moreover, automated measurements of SSS, SRS and SDS were derived by three widely available software packages (Emory Cardiac Toolbox, Myovation, Quantitative Perfusion SPECT). Results: Interclass Correlation Coefficients of SSS, SRS and SDS between expert reading and software packages were moderate to excellent. Visually defined SSS, SRS and SDS were significantly correlated with the corresponding results of all software packages. However, visually defined SSS, SRS and SDS were more strongly correlated with the angiographic score, indicating a better performance of expert reading when compared to automated analysis. Conclusions: Based on our results, visual evaluation continues to have a crucial role for the interpretation of MPI images. Software packages can provide automated measurements of several parameters, particularly contributing to the investigation of cases with ambiguous scintigraphic findings.

Abstract:In order for the future energy needs of humanity to be adequately and sustainably met, alternative energy techniques such as artificial photosynthesis need to be made more efficient and therefore commercially viable. On a grand scale, the energies coming to and leaving from the earth are balanced. With the fast increasing waste heat produced by human activities, the balance may be shifted to threaten the ecosystem in which we reside. To avoid such dire consequences, it is necessary to power human activities using energy derived from the incoming source, which is predominantly solar irradiation. Indeed, most life on the surface of the earth is supported, directly or indirectly, by photosynthesis that harvests solar energy and stores it in chemical bonds for redistribution. Being able to mimic the process and perform it at high efficiencies using low-cost materials has significant implications. Such an understanding is a major intellectual driving force that motivates research by us and many others. From a thermodynamic perspective, the key energy conversion step in natural photosynthesis happens in the light reactions, where H₂O splits to give O₂ and reactive protons. The capability of carrying out direct sunlight-driven water splitting with high efficiency is therefore fundamentally important. We are particularly interested in doing so using inorganic semiconductor materials because they offer the promise of durability and low cost. In this Account, we share our recent efforts in bringing semiconductor-based water splitting reactions closer to reality. More specifically, we focus on earth-abundant oxide semiconductors such as Fe₂O₃ and work on improving the performance of these materials as photoelectrodes for photoelectrochemical reactions. Using hematite (α-Fe₂O₃) as an example, we examine how the main problems that limit the performance, namely, the short hole collection distance, poor light absorption near the band edge, and mismatch of the band edge energetics with those of water redox reactions, can in principle be addressed by adding nanoscale charge collectors, forming buried junctions, and including additional light absorbers. These results highlight the power of forming homo- or heterojunctions at the nanoscale, which permits us to engineer the band structures of semiconductors to the specific application of water splitting. The key enabling factor is our ability to synthesize materials with precise control over the dimensions, crystallinity, and, most importantly, the interface quality at the nanoscale. While being able to tailor specific properties on a simple, earth-abundant device is not straightforward, the approaches we report here take significant steps towards efficient artificial photosynthesis, an energy harvesting technique necessary for the well-being of humanity.
Automatic assessment of left ventricular function for hemodynamic monitoring using artificial intelligence and transesophageal echocardiography

Abstract:We have developed a method to automatically assess LV function by measuring mitral annular plane systolic excursion (MAPSE) using artificial intelligence and transesophageal echocardiography (autoMAPSE). Our aim was to evaluate autoMAPSE as an automatic tool for rapid and quantitative assessment of LV function in critical care patients. In this retrospective study, we studied 40 critical care patients immediately after cardiac surgery. First, we recorded a set of echocardiographic data, consisting of three consecutive beats of midesophageal two- and four-chamber views. We then altered the patient's hemodynamics by positioning them in anti-Trendelenburg and repeated the recordings. We measured MAPSE manually and used autoMAPSE in all available heartbeats and in four LV walls. To assess the agreement with manual measurements, we used a modified Bland–Altman analysis. To assess the precision of each method, we calculated the least significant change (LSC). Finally, to assess trending ability, we calculated the concordance rates using a four-quadrant plot. We found that autoMAPSE measured MAPSE in almost every set of two- and four-chamber views (feasibility 95%). It took less than a second to measure and average MAPSE over three heartbeats. AutoMAPSE had a low bias (0.4 mm) and acceptable limits of agreement (− 3.7 to 4.5 mm). AutoMAPSE was more precise than manual measurements if it averaged more heartbeats. AutoMAPSE had acceptable trending ability (concordance rate 81%) during hemodynamic alterations. In conclusion, autoMAPSE is feasible as an automatic tool for rapid and quantitative assessment of LV function, indicating its potential for hemodynamic monitoring.
