Abstract:Background Wilms tumor (WT) is the most common renal malignant tumor in children. It occurs primarily at preschool age. The purpose of this review is to present current standards of diagnosis and treatment of WT around the world. Data sources All the recent literature on diagnosis and treatment of WT were searched and reviewed. Results Most cases with WT are sporadic. The current survival in patients with WT is high (90%). Involvement of mutidisciplinary collaborative groups in the diagnosis and treatment of WT. National Wilms Tumor Study Group (NWTSG)/Children’s Oncology Group (COG) and The International Society of Paediatric Oncology (SIOP) are two major guidelines used for the current management of WT worldwide. The major difference exists in the two guidelines is the timing of surgery: SIOP recommends using preoperative chemotherapy, and NWTSG/COG prefers using primary surgery before any adjuvant treatments. Conclusions Most patients with WT have good overall survival outcomes. Further studies should be highlighted on how to use chemotherapy and radiotherapy under more accurate risk-stratified strategies. Surgeons must be more focusing on how to maximize preoperative and postoperative treatment possibilities for achieving optimal results of patients with WT.

Abstract:Wilms' tumour is the most frequent renal tumour in children. Based on the SIOP strategy, children with Wilms' tumour may benefit from preoperative chemotherapy, but few publications address the effect of preoperative transcatheter arterial chemoembolization (TACE) on patients with Wilms' tumours. The aims of this study were to investigate the prognostic relevance of preoperative TACE followed by tumour resection, proliferating cell nuclear antigen (PCNA) and vascular endothelial growth factor (VEGF) expression in patients with Wilms' tumours. Two therapeutic strategies including tumour resection only and TACE, followed by tumour resection were conducted in a cohort of 44 patients with Wilms' tumours. Clinical and follow-up data was analysed. Immunohistochemistry staining was used to explore PCNA and VEGF expression in the Wilms' tumour. Two years tumour-free survival of the patients in the TACE group was significantly higher than that of the patients in the control group (P < 0.001) and recurrence and cases of death within one year in the TACE group was markedly lower than that in the control group (P < 0.001). Fifty-five percent of patients in the control group were PCNA-positive vs. 4.17% of patients in the TACE group (P < 0.001). Fifty percent of patients in the control group were VEGF-positive vs. 29.17% of patients in the TACE group (P > 0.05). Patients with Wilms' tumours benefited from preoperative TACE treatment. PCNA expression was significantly lower in patients in the TACE group than those in the control group. Published online Feb 27, 2024. doi: 10.21037/tp-23-579 It is a pleasure to write the editorial commentary on a landmark paper by Weil and colleagues recently published in the Journal of Clinical Oncology (1). The improvement in Wilms tumor (WT) survival, with 85–90% 5-year survivals, can be attributed to risk adjusted standardization of therapy based on protocols developed by the Children's Oncology Group (COG) in the United States (USA) and SIOP (Societie International Oncology Pediatrique) in Europe over the last half century. The focus has now rightly shifted to understanding long-term health consequences in WT survivors of current treatment protocols. The paper was very well designed to study the long-term health related disease burden and suggest potential changes to treatment paradigms by understanding the consequences of risk adjusted multimodal therapy. It evaluated the incidence of long-term morbidity after treatment for unilateral nonsyndromic WT in a cohort of 2,008 survivors by studying the 35-year cumulative incidence of death and development of any grade 3–5 chronic health condition (CHC) to a matched cohort (2:1) of siblings of cancer survivors. WT patients in the cohort were treated between 1970 and 1999. The use of the Childhood Cancer Survivor Study (CCSS) and linking it to the National Death Index helped establish late mortality (beyond 5 years of therapy) and cause of death in the study. The authors proceed to further classify late deaths as due to WT relapse, other health related causes (including late effects of therapy), or external causes. Subsequent Malignant Neoplasm (SMN) incidence was ascertained by self-reporting and medical record review. CHCs were graded according to Common Technology Criteria for Adverse Events (CTCAE) as mild (Grade 1); moderate (Grade 2); disabling (Grade 3); life threatening (Grade 4); and fatal (Grade 5). In selecting grade 3–5 CHCs for further study the authors have rightly focused on the most commonly known causes of morbidity after WT surgery including intestinal obstruction needing operative intervention, kidney failure requiring dialysis or transplant, heart failure, and Premature Ovarian Insufficiency (POI) before age 40 years in females. Standardized Short Form-36 answers dichotomized to impaired/non impaired was used to subjectively measure health related quality of life (HRQOL). Covariates included demographic data and treatment exposure. Chemotherapy (CT) exposure was evaluated based on established WT regimens while radiotherapy (RT) exposure was subdivided to (I) ipsilateral flank RT; (II) whole abdominal RT (WART); and (III) whole lung RT (WLRT). Further exposure was evaluated by RT site and RT dose. Survivors were stratified for two field categories [WLRT and abdominal radiotherapy (ART)] and dose was categorized as ≥20 or ≤20 Gy. The authors have included all variables that affect risk for CHCs after WT treatment and develop treatment attributable risk based on exposure. Median age of survivor cohort at diagnosis and follow-up were 3.2 years (range, 0–20.2 years) and 27.8 years (range, 6.4–57.5 years). The true merit of this study lies in the length of follow-up that has been obtained. All survivors in cohort had undergone nephrectomy and nearly all (98.1%) received CT with vincristine (93.8%) and actinomycin D (93.5%) while doxorubicin was used in 44.8%. Use of other agents (etoposide, cyclophosphamide, and platinum agents) was more common after the 1990's as compared to those treated in the 70's and 80's. As would be expected, with greater disease burden, treatment group with >4 medications had the greatest proportion of survivors experiencing early relapse (23.2%). Doxorubicin use was also more common over time but cumulative dose exposure (≥250 mg/m 2 ) decreased over time. Fifty-five percent of survivors received RT with most common fields being flank (27.8%); WART (13.8%) and WLRT (13.4%). There were 142 late deaths (7.8%). Most importantly health related mortality (4.7%) was nearly three times relapse-related mortality (1.6%). This I believe is going to be important in making treatment decisions in the future. Most frequent causes of death were SMN (n=42), WT relapse (n=30), and cardiac. (n=9). Abstract:To investigate and analyze the treatment and Long-term follow-up results of adult Wilms' tumor.Medical records for diagnosed Wilms' tumor in patients aged more than 15 years from Jan. 1970 to Dec. 2011 were reviewed retrospectively. The clinical presentations, stages, operative details, pathologic findings, adjuvant therapies and outcomes were analyzed. All the patients received regular follow-up, and particular attention was paid to the median follow-up period and tumor specific survival time.The records revealed the 10 patients with a median age of 33.5 year. Male and female were each 5. Left was 4 and right was 6. In the study, 80% of the patients had clinical presentations, and 30% of the patients presented with abdominal mass, and 30% of the patients had distant metastasis at the time of diagnosis. The number of the patients with tumor stages I,II, III, and IV were 2, 1, 4, and 3. One patient just underwent fine needle biopsy due to advanced tumor stage, and the others underwent surgical operations. The range of the follow-up time was 12 to 187 months, and the median follow-up period was 20 months. One patient lost the follow up , 5 patients died, 2 patients survived with tumor recurrence, and 2 patients survived without tumor recurrence. The median survival period was 42 months, and one patient lost the follow-up.The Long-term follow-up data demonstrated the poor prognosis of adult Wilms' tumor. Early tumor stage and the combination of operation, radiotherapy and chemotherapy are key factors to improve the outcomes.

Abstract:PURPOSE:Wilms' tumor (WT), or nephroblastoma, is the most common renal malignancy of childhood, and its outcome varies. We aimed to retrospectively review the clinical parameters and outcomes of WT cases treated at our medical center to identify factors associated with its prognosis.METHODS:We retrospectively reviewed WT patients who had undergone surgery and other therapy in our hospital from May 1998 to May 2008. Demographic features, mode of presentation, stage of tumor, histopathologic results, and survival rates were evaluated.RESULTS:Our study included 67 children with WT; 38 were males and 29 were females. The mean age at onset was 39 months. Eighteen cases were diagnosed as Stage I, 23 as Stage II, 20 as Stage III, and 6 as Stage IV. Nephroureterectomy was the initial treatment in 57 children. There were 46 (68.7%) cases of favorable histology and 21 (31.3%) cases of unfavorable (anaplastic) histology. The rates of four-year event-free survival and overall survival were 78.3% and 81%, respectively. The four-year overall survival rate for cases of favorable histology (85.8%) was higher than for those with unfavorable histology (71.4%; p =.028).CONCLUSIONS:The outcomes of WT patients in our institution were little poorer than for those of the third National Wilms Tumor Study. Advanced clinical stage at presentation, unfavorable histology, tumor rupture, relapse, delay of radiotherapy due to ignorance, and premature ending of treatment were the important factors that impacted on the outcome of WT.

Abstract:Background: Wilms' tumor(WT) is the most common malignant renal tumor of childhood. Despite the good prognosis of WT, bilateral Wilms' tumor (BWT) still has a poor outcome. We systematically reviewed the literature on BWT, aiming to define its clinical features, treatment, and outcomes. Methods: PubMed, OVID EMbase, Web of Science, and Cochrane Library were systematically searched for studies published from 1980 to 2017. Case series and comparative studies reported clinical data of BWT patients were included. Results: A total of 32 studies comprising 1457 patients were retained for primary outcome. Hemihypertrophy, cryptorchidism, and Beckwith-Wiedemann syndrome(BWS) are the most common congenital anomalies and syndrome. 86% of patients had favorable histology (FH). Patients with local stage I or II accounted for 64%, and 12.6% had metastasis at diagnosis. Bilateral nephron-sparing surgery (NSS) was achieved in 33.8%. Recurrence and renal failure occurred in 20% and 8%. The overall survival (OS) was 73%. In comparative studies, OS of patients undergoing bilateral NSS was similar to that of other operation types. Conclusion: Prognosis of BWT has been improved but is significantly poorer than WT. Bilateral NSS was recommended by most centers to preserve more renal volume. Surgical complications in the treatment of Wilms' tumor

Abstract:We present our data on the treatment of Wilms' Tumor (WT) with an emphasis on both the positive effect and the adverse effect of preoperative chemotherapy with regard to surgical intervention. From 1980 to 2000 70 children were treated. 57 % received preoperative chemotherapy (ChTx) and 43 % were operated on primarily. 75 % of the tumors responded to ChTx with significant shrinkage of the size. After preoperative ChTx 54 % of the cases were regrouped as stage I, whereas after primary operation 46 % of the patients were grouped as stage I, thus indicating a downstaging with preoperative ChTx. In 8 % of the patients with preoperative chemotherapy intraoperative complications occurred with a rupture of the tumor in 1 case. In contrast, there were intraoperative complications in 25 % of the patients with a primary operation with rupture of the tumor in 3 cases. 1 child (1.5 %) was treated with chemotherapy who did not have a Wilms' tumor but a benign nephroma (CMN). 3 cases had a clear cell sarcoma (CCSK) and in one case histology revealed a rhabdoid tumor (MRTK). In one case of CCSK only histology of the metastases disclosed the correct diagnosis. The rate of postoperative complications such as ileus was the same for both groups. Irrespective of the known adverse effects such as changing tumor histology, which may affect the correct staging, and the remaining risk of an initial inadequate treatment, our data show that the regimen of preoperative chemotherapy as proposed by the SIOP study should not be abandoned. However, the relatively small number of our patients does not allow a significant statement to be made but confirms the results of past SIOP studies.

Abstract:INTRODUCTION Data on hepatitis E virus (HEV) in Brazil are limited. We analyzed 15 years of HEV surveillance data in a major clinical laboratory in São Paulo, Brazil. METHODS The seroprevalence of HEV of 2,271 patients subjected to anti-HEV tests from 1998 to 2013 were analyzed. RESULTS HEV seroprevalence was 2.1%, and the anti-HEV IgM positivity rate was 4.9%. Six hepatitis E patients were identified. CONCLUSIONS HEV seroprevalence and detection rates appear to have increased in recent years. Hepatitis E should be investigated further and included in the differential diagnosis of hepatitis in Brazil.

Abstract:Objective To retrospectively review 10 years'experience in diagnosis and treatment of Wilm's tumor,and to determine the prognostic factors. Methods Sixty-nine patients with Wilms'tumor were enrolled in this study. Patient'gender,age at diagnosis,clinical symptoms,tumor stage,pathology and therapy were analyzed statistically. Kaplan-Meier survival analysis was applied to esti-mate the survival time of patients with different stage and pathological features. Results The average age at onset of tumor was (3.25±2.78)years old. Abdominal masses were the major signs. Of the 69 patients,there were 18 cases of stage Ⅰ,25 of stage Ⅱ,20 of stage Ⅲ and 6 of stage Ⅳ. The pa-tients were treated with multimodal therapy consisting of extirpative surgery,chemotherapy and limit-ed radiotherapy. The patients were followed up for 4 months to 123 months with a mean survival time of (41±30.9) months. The survival rates of each stage were 100% in stage Ⅰ,76% in stage Ⅱ,70% in stage Ⅲ,and 50% in stage Ⅳ. There was no difference of survival time among the stage Ⅰ,Ⅱ and Ⅲ patients. Compared with the stage Ⅰ,Ⅱ and Ⅲ patients,the survival time of the patients with stage Ⅳ tumor was significantly decreased (P= 0.04). The survival rates of the patients with fa-vorable histology (FH) Wilms'tumors was 86.9%,and decreased to 65.2% in the patients with unfa-vorable histology (UH) tumors. Significant difference of survival time between FH patients and UH patients was found using Kaplan-Meier survival analysis (P = 0.004). Conclusions Clinical stage and pathological features were the important prognostic factors of Wilms'tumor.

Abstract:Background: Wilms tumor (WT) survival has been affected by the evolution in clinical and biological prognostic factors. Significant differences in survival rates indicate the need for further efforts to reduce these disparities. This study aims to evaluate the clinicopathological data impact on survival among patients after Wilm's diagnosis. Methods: The study utilized the SEERStat Database to identify Wilms tumor patients, applying SEERStat software version 8.3.9.2 for data extraction. Selection criteria involved specific codes based on the International Classification of Diseases for Oncology (ICDO-3), excluding cases with unknown SEER stage, incomplete survival data, unknown size, or lymph node status. Statistical analyses, including Kaplan-Meier estimates and Cox regression models, were conducted using R software version 3.5. Standardized mortality ratios (SMR) were computed with SEER*Stat software, and relative and conditional survival analyses were performed to evaluate long-term survival outcomes. Results: Of 2273 patients diagnosed with Wilms tumor, (1219 patients, 53.6% were females with an average age group of 3-8 years (50.2%). The overall mean survival after five years of diagnosis was 93.6% (2.6-94.7), and the overall mean survival rate was 92.5% (91.3-93.8) after ten years of diagnosis. Renal cancers were identified as the leading cause of death (77.3%), followed by nonrenal cancers (11%) and noncancer causes (11%). Additionally, robust relative survival rates of 98.10%, 92.80%, and 91.3% at one, five, and ten years, respectively, were observed, with corresponding five-year conditional survival rates indicating an increasing likelihood of survival with each additional year post-diagnosis. Univariate Cox regression identified significant prognostic factors: superior CSS for patients below 3 years (cHR 0.48) and poorer CSS for those older than 15 years (cHR 2.72), distant spread (cHR 10.24), regional spread (cHR 3.09), and unknown stage (cHR 4.97). In the multivariate model, age was not a significant predictor, but distant spread (aHR 9.22), regional spread (aHR 2.84), and unknown stage (aHR 4.98) were associated with worse CSS compared to localized tumors. Conclusion: This study delving into WT survival dynamics reveals a multifaceted landscape influenced by clinicopathological variables. This comprehensive understanding emphasizes the imperative for ongoing research and personalized interventions to refine survival rates and address nuanced challenges across age, stage, and tumor spread in WT patients.
Psychological, educational, and social late effects in adolescent survivors of Wilms tumor: A report from the Childhood Cancer Survivor Study

Abstract:Objective: To delineate the impact of treatment exposures and chronic health conditions on psychological, educational, and social outcomes in adolescent survivors of Wilms tumor. Methods: Parent reports from the Childhood Cancer Survivor Study were analyzed for 666 adolescent survivors of Wilms tumor and 698 adolescent siblings. Adjusting for race and household income, survivors were compared to siblings on the Behavior Problems Index and educational outcomes. Multivariable modified Poisson regression estimated relative risks (RR) for therapeutic exposures and chronic health conditions (CTCAE 4.03 graded) among survivors, adjusting for sex, race, income, and age at diagnosis. Results: Compared to siblings, adolescent survivors of Wilms tumor were more likely to take psychoactive medication (9.4% vs. 5.1%, p < 0.001) and utilize special education services (25.5% vs. 12.6%, p < 0.001) but did not differ significantly in emotional and behavioral problems. Survivors were less likely to be friendless (7.2% vs. 10.1%, p = 0.04) but were more likely to have difficulty getting along with friends (14.5% vs. 7.8%, p < 0.001). Among survivors, use of special education services was associated with abdomen plus chest radiation (RR = 1.98, CI:1.18-3.34). Those with grade 2-4 cardiovascular conditions had higher risk for anxiety/depression (RR = 1.95, CI:1.19-3.19), headstrong behaviors (RR = 1.91, CI:1.26-2.89), and inattention (RR = 1.56, CI:1.02-2.40). Conclusions: Adolescent survivors of Wilms tumor were similar to siblings with respect to mental health concerns overall but were more likely to require special education. Monitoring of psychosocial and academic problems through adolescence is warranted, especially among those treated with radiation to the abdomen plus chest or with cardiac conditions.

Abstract:From 2003 to 2013, 55 patients (median age 3.3 years; 29 males, 26 females) with unresectable, metastatic, or diffuse anaplastic histology (AH) Wilms tumor were treated with neoadjuvant transcatheter arterial chemoembolization (TACE) and systemic chemotherapy. Characteristics of patients were maximal tumor diameter greater than 10 cm, involvement of periaortic lymph nodes, tumor thrombus in inferior vena cava/right atrium, distal metastasis, or diffuse AH. The chemoembolic emulsion for TACE consisted of pirarubicin, vindesine, and iodized oil. For the tumor with distal metastasis or diffuse AH, cisplatin was added in the chemoembolic emulsion. Intravenous chemotherapy with vindesine and actinomycin D was administered 2-3 weeks after TACE. For the patients with distal metastasis or diffuse AH Wilms tumor, intravenous chemotherapy consisted of ifosfamide and etoposide. Nephrectomy was performed 2-3 weeks after preoperative combination therapy. Surgical stage was assigned according to local operative findings in terms of the National Wilms Tumor Study (NWTS) Group combined with the pretherapeutic imaging to define metastatic disease. Postoperative treatment was based on tumor histology and surgical stage. All patients were followed up for 17-141 months (median: 82 months). No cardiotoxicity, renal insufficiency, and hepatic dysfunction after neoadjuvant TACE and systemic chemotherapy were found. Oral mucositis developed in 5 patients, grade I-II marrow suppression developed in 12 patients, and 19 patients became moderately febrile. In terms of response evaluation criteria in solid tumors, partial response (PR) in 34 (61.8%), stable disease (SD) in 19 (34.5%), and progressive disease (PD) in 2 (3.6%) patients were observed. Four of five patients had complete regression of inferior vena cava tumor thrombus. Atrial tumor thrombus retreated to inferior vena cava in one of two patients. Distant metastasis disappeared in four of six cases. Fifty patients (90.1%) underwent complete tumor resection. Tumor spillage occurred in 3 patients (5.5%). Two patients (3.6%) had microscopic residual disease. Surgical stages were stage II in 25, stage III in 24, and stage IV in 6 patients. On pathologic examination, tumor necrosis was >90% in 14 (25.5%), 50%-90% in 23 (41.8%), and <50% in 18 cases (32.7%). The 5-year event-free survival was 92.7%, and the overall survival was 94.5%. These preliminary results suggest that the use of neoadjuvant TACE and systemic chemotherapy may provide a promising choice in the treatment of unresectable, metastatic, or diffuse AH Wilms tumor in children. Further investigations are necessary.

Abstract:Background: To review the clinical characteristics and survival of infants diagnosed with a primary renal tumor in the first 7 months of life. Procedure: A retrospective data review of patients registered in five large international protocols (SFOP/GPOH/SIOP9/93-01, UKW3 and NWTSG 4 and 5) spanning 1985-2002. Results: 750 (7.2%) of 10,430 registered patients were diagnosed with a renal tumor before age 213 days. Tumor types were Wilms tumor (WT) 58%; congenital mesoblastic nephroma (CMN) 18%; malignant rhabdoid tumor (MRTK) 8%; clear cell sarcoma (CCSK) 2%; non-Wilms tumor (unspecified) 6%; histology unknown, 9%. CMN predominated among tumors diagnosed in the first month of life (54%) but its relative contribution diminished to <10% of all cases diagnosed after the age of 3 months (P < 0.001). Among 639 cases with specified histology and stage, 9/11 stage IV tumors were MRTK, 37/39 bilateral tumors were WT. In 626 children where surgical approach was specified, 522 had immediate nephrectomy. For all cases, 5 years event-free survival (EFS) was 80% and overall survival (OS) 86%. Five years EFS and OS respectively by tumor type were WT (86%, 93%), CMN (94%, 96%), CCSK (49%, 51%), MRTK (16%, 16%). Conclusion: Renal tumors diagnosed in the first 7 months of life generally have an excellent prognosis though histology is an important prognostic factor. In the first 2 months of life the prevalence of CMN is high. The relative occurrence of WT increases rapidly with age thereafter. Bilateral tumors are usually WT. Tumors with metastases at diagnosis are usually MRTK.

Abstract:The nephroblastoma or Wilms tumor (WT) is the most common renal tumor in childhood, representing approximately 6-7% of all pediatric cancers, with a yearly incidence of 10 cases in one million children less than 15 years old, and continues to arouse interest by remarkable actual therapeutic successes, consecutive to the multidisciplinary approach. Its maximum incidence is around the age of 3-3.5 years old, having an equal frequency in males and females. We present the case of a child, aged three years and five months, who was diagnosed with WT (nephroblastoma) with triphasic pattern, stage II tumor, and admitted to the Department of Oncopediatry for chemotherapeutic treatment and clinico-biological investigations.

Abstract:Significant progress has been made in the management of Wilms tumor (WT) in recent years, mostly as a result of collaborative efforts and the implementation of protocol-driven, multimodal therapy. This article offers a comprehensive overview of current multidisciplinary treatment strategies for WT, whilst also addressing recent technical innovations including nephron-sparing surgery (NSS) and minimally invasive approaches. In addition, surgical concepts for the treatment of metastatic disease, advances in tumor imaging technology and potentially prognostic biomarkers will be discussed. Current evidence suggests that, in experienced hands and selected cases, laparoscopic radical nephrectomy and laparoscopic-assisted partial nephrectomy for WT may offer the same outcome as the traditional open approach. While NSS is the standard procedure for bilateral WT, NSS has evolved as an alternative technique in patients with smaller unilateral WT and in cases with imminent renal failure. Metastatic disease of the lung or liver that is associated with WT is preferably treated with a three-drug chemotherapy and local radiation therapy. However, surgical sampling of lung nodules may be advisable in persistent nodules before whole lung irradiation is commenced. Several tumor markers such as loss of heterozygosity of chromosomes 1p/16q, 11p15 and gain of function at 1q are associated with an increased risk of recurrence or a decreased risk of overall survival in patients with WT. In summary, complete resection with tumor-free margins remains the primary surgical aim in WT, while NSS and minimally invasive approaches are only suitable in a subset of patients with smaller WT and low-risk disease. In the future, advances in tumor imaging technology may assist the surgeon in defining surgical resection margins and additional biomarkers may emerge as targets for development of new diagnostic tests and potential therapies.

Abstract:Wilms tumor is the most frequent kind of kidney cancer in kids. It is responsible for around 6% of all tumors in children. In children under the age of 15, the incidence is 7.6 cases per million, with 75 percent of cases occurring before the child reaches the age of five. A variant term is nephroblastoma. It is most widespread between the ages of three and four, after which it becomes much less frequent. Clinical findings: Wilms tumor in children produces one or more of the following signs and symptoms: abdominal pain, increasing abdominal girth, lack of appetite, fever, blood in the urine, nausea or vomiting, constipation, shortness of breath. Diagnostic evaluation: Increase abdominal girth, pain, fever,vomiting, cried at the time of urination, hematuria, black stool, pallor, hypertension, and moderate microcytic anemia (iron deficiency). Ultrasonography: Showed splenomegaly. Therapeutic investigation: X-ray of abdomen and chest, IVP, USG, MRI, CT scan, renal function test and urine analysis, liver function test, bone marrow study, and histology. Outcome: After treatment, the child shows great improvement, his fever, abdominal pain, blood in urine is minimized, and his Hb level is increased from 9gm% to 11.1gm%.

Abstract:Objective To evaluate the long-term efficacy of multidisciplinary therapy on childhood Wilms′ tumor.Methods Twenty-two consecutively diagnosed patients were recruited from May 1994 to Dec.2009.They were aged 4 to 145 months (the media 17.5 months).Diagnosis and treatment were decided and conducted by applying therapeutic regimens stratified by stage and histology in accordance with National Wilms′ Tumor Study criteria of USA.The median follow-up duration was 52.0 months (0.5 to 187.0 months).SPSS 11.0 software was used to analyze the data.Results According to the tumor stage criteria after operation,7 cases were at stage Ⅰ,8 cases at stage Ⅱ,6 cases at stage Ⅲ and 1 case at stage Ⅳ.Pathologic analysis showed that 21 cases were at favorable-histology,1 case at unfavorable-histology.At present,19 patients were alive without tumor except 2 patients died of disease.One case was loss of follow up at 1 year after operation.The 4-year event-free survival rate was 90.15%.Conclusions Multidisciplinary therapy protocol for childhood Wilms′ tumor can achieve good effects.Treatment must be done strictly according to the therapy protocol.

Abstract:Wilms tumor (WT), or nephroblastoma, is the predominant renal malignancy in the pediatric population. This narrative review explores the evolution of personalized care strategies for WT, synthesizing critical developments in molecular diagnostics and treatment approaches to enhance patient-specific outcomes. We surveyed recent literature from the last five years, focusing on high-impact research across major databases such as PubMed, Scopus, and Web of Science. Diagnostic advancements, including liquid biopsies and diffusion-weighted MRI, have improved early detection precision. The prognostic significance of genetic markers, particularly WT1 mutations and miRNA profiles, is discussed. Novel predictive tools integrating genetic and clinical data to anticipate disease trajectory and therapy response are explored. Progressive treatment strategies, particularly immunotherapy and targeted agents such as HIF-2α inhibitors and GD2-targeted immunotherapy, are highlighted for their role in personalized treatment protocols, especially for refractory or recurrent WT. This review underscores the necessity for personalized management supported by genetic insights, with improved survival rates for localized disease exceeding 90%. However, knowledge gaps persist in therapies for high-risk patients and strategies to reduce long-term treatment-related morbidity. In conclusion, this narrative review highlights the need for ongoing research, particularly on the long-term outcomes of emerging therapies and integrating multi-omic data to inform clinical decision-making, paving the way for more individualized treatment pathways.

Abstract:Background: We previously reported that the estimated glomerular filtration rate (eGFR) was decreased among survivors of unilateral, non-metastatic, non-syndromic Wilms tumor (WT) treated with whole abdomen radiation therapy (WART) compared to controls, but not compared to unirradiated WT survivors (Pediatr Blood Cancer 2020;67:e28271). The impact of sex on estimates of kidney function among adult survivors of WT has not been well-documented. Methods: We clinically evaluated male and female WT survivors with creatinine and cystatin C, calculated eGFR using the Chronic Kidney Disease - Epidemiology (CKD-EPI) equations with and without cystatin C and measured 99mTc diethylenetriamine pentaacetic acid (DTPA) plasma clearance. WT survivors consisted of 20 treated with unilateral nephrectomy (UN), non-nephrotoxic chemotherapy (NNC) and whole abdomen radiation therapy (WART) (mean - 20.2 Gray (Gy); standard deviation (SD) - 9.12 Gy), and 20 treated with UN, no radiation therapy, and NNC. Correlations between 99mTc DTPA clearance and eGFR were calculated. Results: Among female WT survivors, none of the eGFR calculations was statistically significantly correlated with 99mTc DTPA clearance. Among male WT survivors, 99mTc DTPA clearance correlated well with eGFR calculations that included creatinine. Results are shown in the Table. Conclusions: Among female survivors of unilateral, non-metastatic, non-syndromic WT who have undergone UN, eGFR is poorly correlated with 99mTc DTPA clearance. Evaluation of treatment-related changes in body composition may provide insight into these results. Abstract:OBJECTIVE To investigate the clinical features, diagnosis and treatment of uveitis associated with juvenile chronic arthritis (JCA). METHODS A retrospective study was performed on the clinical data of 26 patients with uveitis associated with JCA, referred to Zhongshan Ophthalmic Center from 1996 to 2002. Taking of history, examination with slit-lamp microscope and ophthalmoscope were carefully performed in all of these patients. Laboratory tests including antinuclear antibodies, erythrocyte sedimentation rate, rheumatoid factor, C reactive protein and antistreptolysin O were used to disclose possible causes. Human leukocyte antigen B 27 and X-ray examination of sacroiliac joints and spine were carried out if necessary. Data about the treatment, visual outcome and complications were analyzed. RESULTS Twenty-six patients, 11 males and 15 females, were included in the present studies. Age of onset of JCA and uveitis averaged 8 years and 9 years, respectively. Twenty-one patients had chronic anterior uveitis. Acute anterior uveitis and chronic panuveitis were noted in 3 and 2 patients, respectively. Twenty-two patients had bilateral uveitis, the other 4 had unilateral involvement. The ophthalmologic examination revealed that 33 of the 48 affected eyes showed mild aqueous humor flare, 24 had few cells in the anterior chamber. Complicated cataract, band keratopathy and secondary glaucoma were noted in 30, 20 and 12 eyes, respectively. The laboratory examination revealed positive antinuclear antibodies in 18 cases and rheumatoid factor positive in one case. In acute episode, patients were treated with extensive topical cycloplegic agents and corticosteroids eyedrops. In the 6 cases with severe uveitis, 3 patients were treated with cyclosporine A (5 mg.kg(-1).d(-1)) and the other 3 were treated with chlorambucil (0.1 mg.kg(-1).d(-1)). The intraocular inflammation in all of these patients was satisfactorily controlled with these treatments. Visual acuity improvement was noted in 32 affected eyes but not in the other 16 eyes which had already serious complications before the treatment. CONCLUSIONS Uveitis associated with JCA is characterized by a chronic and recurred iridocyclitis, which usually developed within 5 year after JCA onset. Although the intraocular inflammation associated with JCA is usually white uveitis, complications such as cataract, secondary glaucoma and keratopathy occur frequently in these patients. The diagnosis is mainly based on typical clinical manifestations, the history of arthritis and positive antinuclear antibodies. Adequate application of cycloplegic agents, corticosteroids eyedrops, immunosupressives and the regular follow-up should be kept in mind in the treatment of these patients.
