Abstract:Background Kaposiform hemangioendothelioma (KHE) is a rare vascular neoplasm affecting infants or young children. KHE includes a spectrum of lesions, ranging from small and superficial tumors to large and invasive lesions with Kasabach-Merritt phenomenon (KMP). Currently, no published studies have reported a KHE presenting as thrombocytopenia and Raynaud phenomenon. Case presentation A 2-year-old boy with right hand swelling and thrombocytopenia was admitted to our hospital. His right hand turned swelling and red, even occasionally cyanotic. This condition became worse in response to cool environments and improved with warming, and platelet counts were between 50 ~ 80 × 10^9/L. Physical examination on admission revealed the swelling and frostbite-like rash of the right-hand fingers, and the skin temperature of the right hand was lower than the left. On day 3 of admission, chest CT results showed an irregular mass on the right side of the spine. The puncture biopsy demonstrated positive CD31, D2-40, and FLI1 immunohistochemical staining, but negative GLUT1 staining, confirming the diagnosis of KHE. Furthermore, endothelin-1 (ET1) expression levels significantly increased, and eNOS and A20 expression levels significantly decreased comparing with control patients. The patient received methylprednisolone and sirolimus treatments, and his condition gradually improved during the follow-up. Conclusions We reported the first case of KHE presenting with thrombocytopenia and Raynaud phenomenon. The development of Raynaud phenomenon could be associated with increased ET-1 and reduced eNOS and A20 expressions. Careful differential diagnosis of hidden KHE should be considered in children with thrombocytopenia and Raynaud phenomenon.
Primary kaposiform hemangioendothelioma of a long bone: two cases in unusual locations with long-term follow up.

Abstract:Kaposiform hemangioendothelioma (KHE) is a rare vascular neoplasm of low malignant potential that mainly affects infants and adolescents. The tumor almost exclusively occurs in somatic soft tissue or the retroperitoneum. We report herein two cases of primary KHE occurring in a long bone without cutaneous changes with long-term follow up in young patients. The patients were a 9-year-old girl and 5-year-old boy presenting with lytic lesions of the femur and humerus, respectively, without cutaneous lesions. Histologically, the neoplasms were comprised of nodules of spindle- to oval-shaped cells growing in an infiltrative fashion. The neoplastic cells formed poorly canalized or slit-like blood vessels alternating with solid spindle areas. Immunohistochemical studies showed that the tumor cells expressed CD31, CD34 and Fli1, but not HHV8, LNA-1 or GLUT1. D2-40 stained the neoplastic spindle cells and lymphatic channels adjacent to vascular lobules. The girl remains well with 15 years and 6 months follow up after a second complete excision. The boy has no signs of recurrence or metastasis nearly 5 years after local complete excision. To our best knowledge, this is the first report in the English literature of primary long bone occurrences of KHE without cutaneous changes with long-term follow up.

Abstract:OBJECTIVE:To explore the clinicopathologic features of kaposiform hemangioendothelioma (KHE).METHODS:The clinicopathologic data were studied in three cases of KHE and review the literatures.RESULTS:Two cases were female and one was male. All cases occurred in infancy. Two tumor located in axillary chest wall and one in lumbar region. All of the three patients had Kasabach-Merritt syndrome. Histologically, the tumor was composed of spindle-shaped cells. in all cases nodular growth pattern was seen. Immunohistochemically, Neoplastic spindled cells expressed CD34 and CD31. Associated lymphangiomatosis was present in two cases. Two tumors were resected completely, one was resected partly. the follow-up period ranged from 6 months to 3 years, and all were alive.CONCLUSIONS:Kaposiform hemangioendothelioma is a rare locally aggressive vascular tumor that mainly occurred in early infancy. It is frequently complicated by Kasabach-Merritt syndrome, and it has features common to both capillary hemangioma and Kaposi sarcoma. The prognosis of KHE is determined by the size, location and the hemorrhage degree of vascular tumor. Better outcome might be achieved in patients with KHE of the skin and in the soft tissues under the skin. It appears that the main treated measure should be wide local excision.
Diagnosis and treatment of Kaposiform hemangioendothelioma of the hand

Abstract:Objective To investigate the clinical features and the diagnosis and treatment of Kaposiform hemangioendothelioma ( KHE ) of the hand. Methods From 2010 to 2013, 5 patients with KHE of the hand were treated in our department, including 4 males and 1 female. Their mean age was 4.6 years old ( range;6 months-33 years ), among whom there was 1 adult patient of 33 years old and 4 infants. No abnormal coagulation function was found before the operation. Extended local resection of tumors were performed on all the patients. Results The diagnosis of KHE was conifrmed by the postoperative pathology. A multiple lobular and invasive growth pattern was observed in the tumors, and vascular endothelial cells were shown. The densely packed spindle-shaped tumor cells and sieve-like or slit-like blood vessels could be seen, with focal hyperplasia of glomeruloid structures. Bone destruction was found in some cases in the histological or radiographic examination. The immunohistochemical staining showed all the vascular markers were positive including cluster of differentiation ( CD ) 31, CD34, ( friend leukemia integration 1 transcription factor ) Fli-1 and Factor VIII. The smooth muscle antibodies ( SMA ) were focally positive and the focal expression of cytokeratin ( CK ) was noticed in 1 case. All the patients were followed up for a mean period of 18 months ( range;12-25 months ). No recurrence was found but only in 1 patient at 1 year after the operation, who underwent surgery again. Conclusions Simple extended resection of tumors is an effective method in the treatment of KHE of the hand, with excellent outcomes.

Abstract:Purpose To investigate the clinico-pathologic features,biological behavior of Kaposiform hemangioendothelioma(KHE)of bone and its differential diagnosis.Methods Two cases of KHE of long bone were observed by HE,immunohistochemical stainings and electromicroscopy examination,and analyzed with clinical features.Results Two cases of KHE occurring in children presented with focal vague pain of long bone,with focal increased skin temperature in 1 case,for more than one year.Kabasach-Merritt syndrome was not found in both cases.Imaging examination showed focal destruction of bone with honeycomb appearance or low density.Histologically,the neoplasm had a characteristic appearance of irregular nodules or multi-lobulated growing in an infiltrative fashion.Some nodules consisted of small,round or irregular capillary-size structure,usually with fascicles of spindle cells.Slit-like vessels were found in the zone of spindle cells.Some nodules contained glomeruloid nests of epithelioid cells with cytoplasmic vacuoles and hemosiderin.Immunostaing showed that the tumor cells including spindle and epithelioid cells were strongly positive for CD31、CD34 and Fli-1,and focally positive for SMA,but negative for GLUT1、CKpan and FⅧRAg.Ki-67 was sparsely positive in the neoplasm.Conclusions KHE of bone is a rare neoplasm of malignancy potential primarily occurring in children and infants.The diagnosis of KHE was dependent on microscopy and immunohistochemistry examination,which should be distinguished from juvenile hemangioma,Kaposi sarcoma,tufted angioma and spindle cell haemangioendothelioma.Complete surgical excision,when possible,is the most effective treatment of KHE of bone.

Abstract:Kaposiform hemangioendothelioma (KHE) is a rare vascular neoplasm that mostly appears in infancy or early childhood. Most KHE occurred on the limbs and trunk with cutaneous lesions. Approximately 12% of KHE patients manifested as deep masses and spinal involvement is extremely rare. KHE may develop into life-threatening thrombocytopenia and consumptive coagulopathy, known as the Kasabach-Merritt phenomenon (KMP), especially in patients with retroperitoneal involvement. The thrombocytopenia is usually severe, with a median platelet count of 21 × 109/L at the initial presentation of KMP. Here, firstly we described a case of a 13-month-old girl with KHE who presented the movement limitation of the lower extremity caused by spinal involvement with a normal platelet count. 18F-fluorodeoxyglucose-positron emission tomography/CT (18F-FDG PET/CT) showed mildly elevated metabolism in the lesion, suggesting a probably low-grade malignant tumor. Then the patient was diagnosed with KHE by biopsy. After 6-month sirolimus monotherapy, the size of the retroperitoneal lesion was reduced significantly and the patient showed improvement in clinical symptoms. This case demonstrated the advantage of 18F-FDG PET/CT in the evaluation of disease activity in KHE and the possibility of using 18F-FDG PET/CT to guide therapy and prognostication.

Abstract:Introduction: Kaposiform hemangioendothelioma (KHE) is a rare, locally invasive vascular tumor that mostly appears in infants and adolescents. KHE with spinal involvement is extremely rare. The aim of this study was to review the imaging features, clinical manifestations and treatment of KHE patients with spinal involvement. Materials and Methods: We reviewed patients with KHE who were admitted to Pediatric Surgery of West China Hospital of Sichuan University from April 2014 to August 2020, and the cases were evaluated. Results: Seven patients with spinal involvement were enrolled in the study, including four (57.1%) males and three (42.9%) females. The age at onset ranged from 1.0 day to 4.0 years, with an average of 1.6 years. Five (71.4%) had pain due to bone destruction, three patients (42.9%) had decreased range of motion (ROM), four (57.1%) patients had scoliosis, two (28.6%) patients developed claudication, and three patients (42.9%) presented with a soft tissue mass in the neck of the back. Five patients (71.4%) had the Kasabach-Merritt phenomenon (KMP), with a minimum platelet value of 8 × 10 9 /L. All patients were treated with sirolimus, and showed regression of the lesion and/or normalization of the hematologic parameters. Conclusion: KHE with spinal involvement is difficult to diagnose due to its rarity and variable symptoms, which need to be recognized to start early treatment. The management of KHE with spinal involvement should be performed by a multidisciplinary team. Sirolimus can improve outcomes in patients with KHE with spinal involvement.

Clinical and Imaging Features of Kaposiform Hemangioendothelioma.

Abstract:Objective: Kaposiform hernang end hell a (KHE) s a unique locally aggressive vascular tumor with poor prognosis. The aim of this study is to assess the clinical and imaging features of KHE, and to compare the differences between solitary and diffusive infiltrative subtype further. Methods: The clinical and radiological findings of a cohort of 25 cases with histologically proven KHE, between June 2011 and June 2016, were reviewed retrospectively. 7 solitary and 18 diffusive infiltrative subtypes KHE were included. The differences of clinical and imaging features between these two subtypes were compared statistically by Wilcoxon rank sum test and Fisher exact test. Results: The median age was 4 months old. 20 cases (80%) were accompanied by Kasabach-Merritt phenomenon (KMP). Most KHE located in trunk and/or extremity. The masses showed inhomogeneous echogenicity and were rich in vascularity on ultrasound: showed isoattenuation relative to muscle on unenhanced CT, isointense (n = 15) or slightly hyperintense (n = 7) T-1 weighted imaging (T1WI) signal relative to muscle, mainly heterogeneous hyperintense or htly hyperintense with speckled hypointense (n = 17) T2WI signal (77%) relative to muscle, and notable (n = 15) and moderate (n = 3) enhancement. Feeding and draining vessels were revealed in 15 cases. Five masses with DWI showed slightly restricted diffusivity, with average apparent diffusion coefficient value of (1.28 +/- 0.09) x 10(-3) mm(2) s(-1). Necrosis and hemorrhage were also found. Compared with solitary ones, diffusive infiltrative KHE were larger, more commonly accompanied by KMP and reticular lymphedema, and more frequently located in trunk and/or extremity. Conclusion: Five masses with DWI showed slightly restricted diffusivity. A hypervascular mass accompanied by KMP and reticular lymphedema, with speckled hypointense signal T2WI signal, especially in pediatric patients, is highly suggestive of the diagnosis of KHE. Advances in knowledge: Speckled hypointense signal T2WI signal, and notable enhancement were unique features of KHE. KHE showed slightly restricted diffusivity on DWI, commonly accompanied by KMP and reticular lymphedema.
Adult-onset multifocal kaposiform hemangioendothelioma in the bone marrow, lung, liver, and brain: a case report

Abstract:Kaposiform hemangioendothelioma (KHE), a rare form of vascular neoplasm, is typically seen in children. In this paper, we report a unique case of KHE replacing bone marrow tissue mimicking myeloproliferative neoplasm with additional involvement in the lung, liver, and brain in a 60-year-old Caucasian woman. The patient was initially seen in the hematology department for the chief complaint of epigastric pain and anemia. Abdominal magnetic resonance imaging (MRI) revealed mild splenomegaly with iron deposition secondary to extramedullary hematopoiesis. Additional workup was inconclusive. Subsequent bone marrow and lung biopsies eventually revealed bone marrow with extensive grade 3 fibrosis and multiple foci of low-grade vasoformative neoplasm in the lung suggestive of KHE. Although rare, KHE can present as an aggressive disease with indolent behavior in adults and can be distinguished from other vascular malignancies based on histopathology and imaging findings.

Abstract:AIM: To improve the clinical diagnosis and recognition of hepatic epithelioid angiomyolipoma (HEAML).METHODS: Four cases of primary HEAML were confirmed based on the pathology archive system in our hospital from January 2009 to November 2015. The general state, clinical symptoms, imaging manifestations, histological results and immunohistochemistry of these patients were retrospectively reviewed and analyzed. Studies of HEAML published in the last 15 years were collected from pubmed and MEDLINE to summarize the clinical symptoms, imaging characteristics, pathological features and management of HEAML.RESULTS: Four cases of primary HEAML were retrieved from our archives. These included three female patients and one male patient, with a mean age of 41.8 +/- 11.5 years (ranging from 31 to 56 years). The mean tumor size was 7.3 +/- 5.5 cm (ranging from 3.0 to 15 cm). In the contrast-enhanced imaging, the tumor was obviously enhanced in the arterial phase, but enhanced continuously or exhibited a slow-density masse during the venous and delayed phases. Histologically, the tumors mainly consisted of epithelioid cells that comprised approximately 95% of the total neoplastic mass. Although no metastases occurred in our patients, pathological studies revealed necrosis, mitotic figures and liver invasion in two patients, which indicates aggressive behavior. Immunohistochemical staining revealed that human melanoma black 45 (HMB-45) and Melan-A were positive in 4 cases. We only identified 81 cases with primary HEAML, including our present patients, from 26 articles available from PubMed and MEDLINE. The majority of the papers were published as case reports. Only 5 (5/75, 6%) cases were associated with tuberous sclerosis complex (TSC). More than half (35/66) were discovered incidentally upon physical examination. Approximately 65% (22/34) of the patients were misdiagnosed with HCC or other tumors before surgery. Approximately 10% (8/81) of the patients with HEAML had recurrence or metastasis after surgery, which was a very high and alarming rate.CONCLUSION: HEAML is a very rare primary hepatic tumor that is often misdiagnosed before surgery. Patients should be followed closely after surgery because of its malignant potential.
Retroperitoneal kaposiform hemangioendothelioma with kasabach-merritt phenomenon in children: A case report and review of the literature

Abstract:Objective: To investigate the clinical features, diagnosis and treatment methods and prognosis of retroperitoneal Kaposiform hemangioendothelioma (R-KHE) in children. Methods: The clinical data of an infant with R-KHE was retrospectively analyzed. Literature on R-KHE in pediatrics were retrieved in databases including Wanfang, CNKI and PubMed as of April 2022. Results: A 1 month and 6 days female infant with R-KHE was reported. After the diagnosis was confirmed by biopsy and pathological examination, the patient was treated by interventional embolization, and a combined therapy with glucocorticoid, vincristine, sirolimus and propranolol. The patient has been followed up for 1 year and 2 months, and is still alive with tumor. Through literature search, a total of 15 children, together with the case in our report, were included. The main manifestations were diversity among those patients. 14 cases have combined Kasabach-Merritt phenomenon (KMP). 6 cases accepted surgery plus drug therapy. 4 cases accepted only surgery, and 4 cases only accepted drug therapy. While drug therapy plus radiotherapy were employed to 1 case. Improvement was observed in 11 cases, with significantly reduced tumor and survival with tumor. Tumor disappeared completely in 2 cases. While 2 cases suffered death. Conclusion: R-KHE has diverse clinical presentations and non-specificity in symptoms and imaging examinations, and most cases accompanied with KMP. Methods for R-KHE treatment include surgical resection, interventional embolization and drug therapy. Close attention needs to be paid to the adverse reactions of the drug during the course of treatment.

Abstract:Kaposiform hemangioendothelioma (KHE) is a vascular neoplasm characterized by abnormal angiogenesis and lymphangiogenesis. Here, we present a case of a 19‐month‐old male with KHE of the right leg with bony involvement who was initially misdiagnosed with infantile hemangioma. Due to its heterogeneous presentation and frequent occurrence of comorbidities such as Kasabach‐Merritt phenomenon, clinical and pathological correlation is essential for diagnosis of KHE.

Abstract:Background: Kaposiform hemangioendothelioma (KHE) is a rare vascular tumor with a high risk of mortality. Few studies with large samples of KHE have been reported. KHE may develop into the Kasabach–Merritt phenomenon (KMP), which is characterized by thrombocytopenia and consumptive coagulopathy. The features of severe symptomatic anemia and life-threatening low platelets make the management of KHE associated with KMP challenging. Objective: The aim of this study was to examine the clinical characteristics of patients with KHE and discuss the treatment experience for different risk groups of KHE. Methods: Through a retrospective review of 70 patients diagnosed with KHE between 2017 and 2022 in our center, we classify lesions into three clinicopathological stages based on the tumor involving depth, and divided the severity of KHE into three levels by estimating clinicopathological stages and severity of thrombocytopenia. Treatments of different severity groups were estimated with sufficient data. Results: In our cohort, 27% were neonates, and KHE lesion occurred at birth in 84% of patients. There was a slight male predominance (32 girls and 38 boys). Common clinical characteristics included associated coagulation disorder (100%), locally aggressive cutaneous blue–purple mass (89%), thrombocytopenia (78%), and local pain or joint dysfunction (20%). The lower extremities were the dominant location (35%), followed by the trunk (29%), the maxillofacial region and neck (24%), and the upper extremities (10%). Of the total cohort, 78% developed KMP; the median age at which thrombocytopenia occurred was 27.8 days. The median platelet count of patients who were associated with KMP was 24,000/μL in our cohort. Ninety-two percent of patients were given surgery treatment and 89% of these patients were given high-dose methylprednisolone (5-6 mg/kg daily) before surgery. In 55 patients with KMP, 36% were sensitive to high-dose corticosteroid therapy. Patients from the low-risk group (eight cases) underwent operation, all of whom recovered without recurrence after a maximum follow-up of 5 years. Out of 26 patients from the high-risk group, 25 underwent surgery treatment, with 1 case undergoing secondary surgery after recurrence and 1 case taking sirolimus. Out of 36 cases from the extremely high-risk group, 32 underwent surgery (including 2 cases who underwent external carotid artery ligation and catheterization), 3 of whom underwent secondary operation after recurrence, and the remaining 4 cases took medicine. The mean length of having sirolimus was 21 months; two cases stopped taking sirolimus due to severe pneumonia. Two cases died at 1 and 3 months after discharge. Conclusions: Our study describes the largest assessment of high-risk patients with KHE who have undergone an operation to date, with 5 years of follow-up to track recovery, which provides invaluable knowledge for the future treatment of patients with KHE and KMP from different risk groups: Early surgical intervention may be the most definitive treatment option for most patients with KHE; multimodality treatment is the best choice for the extremely high-risk group.

Abstract:BackgroundKaposiform hemangioendothelioma (KHE) is a rare vascular tumor affecting infants and young children. Although benign, it can be associated with an aggressive locally growing tumor and/or a life-threatening Kasabach–Merritt phenomenon (KMP). To date, only reviews of limited cases have been performed. We, therefore, conducted a comprehensive literature search to collect relevant data and make recommendations for future treatment trials.MethodsReview of the available literature between 1993 and 2017 revealed a total of 105 publications involving 215 patients of less than 21 years of age. To this, we added 12 from our department and 4 from the Cooperative Weichteilsarkomstudie database.ResultsWe found that KMP was present in 79% of the infants, in 47% of the 1–5-year olds, in 43% of the 6–12-year olds, and in 10% of the 13–21-year-old patients. KMP was present in nearly all (94%) patients with retroperitoneal tumors and in all patients with extra-regional tumors. The median size of a KHE without KMP was 12 cm2 as compared to 49 cm2 when associated with a KMP. With complete (not further classifiable if R0 or R1) resection, all patients were cured. If inoperable, response regarding KMP/regression of tumor size was seen in 29/28% with steroid-, 47/39% with vincristine-, 44/43% with interferon alpha-, 65/61% with anti-platelet agents-, and in 97/100% with sirolimus-containing therapies.ConclusionsPatients with progressive KHE should undergo resection whenever it is considered a safe option. If inoperable, sirolimus should be the first choice for treating KMP and reducing tumor size.

Abstract:A case of Kaposiform is reported. A one-year-old male infant developed painful hard nodules in the right lower abdomen when he was 4 months old. The nodules enlarged gradually. Physical examination showed reddish hard plaques in the right lower abdomen and right groin area. Histological examination showed compact nodular aggregates of spindled cells and ovoid cells in both dermis and subcutaneous tissues with vascularization. The tumor cells were CD31 and CD34 positive. Kaposiform hemangioendothelioma was diagnosed and radiotherapy and chemotherapy were advised.

Abstract:Kaposiform hemangioendothelioma (KHE) is a rare, locally aggressive vascular tumor with high morbidity and mortality. The aim of this study was to evaluate ultrasonographic findings associated with KHE. The clinical and ultrasonographic findings of a cohort of 64 cases with pathologically proven KHE were retrospectively reviewed and analyzed between November 2014 and February 2021. Two subtypes were divided according to the presence or absence of the Kasabach-Merritt phenomenon (KMP). The KMP risk factors in patients with KHE were analyzed statistically. Among the 64 cases of KHE, 43 (67.2%) were accompanied by KMP. There was a positive correlation between the appearance of KMP and tumor size. KHEs had an increased risk of developing KMP if the lesions measured were >6 cm and if they belonged to the deep or mixed subtype. On ultrasonography, all KHE lesions were heterogeneous, and 81.3% were hypoechoic; 93.8% of KHEs exhibited ill-defined margins, 68.7% had strands branching into the adjacent tissue, and 84.4% presented marked hypervascularity. Elastography showed that central hypoechogenic lesion areas were hard, and surrounding hyperechogenic lesion areas were soft. KHEs can occur in different parts of childrens' bodies. On ultrasonography, the main findings are heterogeneous low erosions, indistinct margins, branching strangulation into adjacent tissues, and obvious hypervascularity. Patients with lesions larger than 6 cm or belonging to deep or mixed subtypes (musculoskeletal infiltrates) are at risk for developing KMP, and clinicians should be vigilant. Das kaposiforme Hämangioendotheliom (KHE) ist ein seltener, lokal aggressiver Gefäßtumor mit hoher Morbidität und Mortalität. Ziel dieser Studie war es, die mit dem KHE verbundenen Ultraschallbefunde zu untersuchen. Die klinischen und sonografischen Befunde einer Kohorte von 64 Fällen mit pathologisch bestätigtem KHE wurden zwischen November 2014 und Februar 2021 retrospektiv überprüft und analysiert. Zwei Subtypen wurden nach dem Vorhandensein oder Fehlen des Kasabach-Merritt-Phänomens (KMP) unterteilt. Die KMP-Risikofaktoren bei Patienten mit KHE wurden statistisch analysiert. Von den 64 KHE-Fällen waren 43 (67,2%) von KMPs begleitet. Es bestand eine positive Korrelation zwischen dem Auftreten eines KMP und der Tumorgröße. KHEs hatten ein erhöhtes Risiko, KMP zu entwickeln, wenn die Läsionen >6 cm groß waren und wenn sie zum tiefen oder gemischten Subtyp gehörten. Bei der Ultraschalluntersuchung waren alle KHE-Läsionen heterogen und 81,3% echoarm. 93,8% der KHEs wiesen unscharfe Ränder auf, 68,7% hatten Stränge, die in das angrenzende Gewebe verzweigten, und 84,4 % zeigten eine ausgeprägte Hypervaskularität. Die Elastografie zeigte, dass die zentralen hypoechogenen Bereiche der Läsion hart und die umgebenden hyperechogenen Bereiche der Läsion weich waren. KHEs können an verschiedenen Körperstellen von Kindern auftreten. Bei der Ultraschalluntersuchung sind die wichtigsten Befunde heterogene niedrige Erosionen, unscharfe Ränder, Verästelungen, die in angrenzende Gewebe einwachsen, und eine offensichtliche Hypervaskularität. Bei Patienten mit Läsionen, die größer als 6cm sind oder zu tiefen oder gemischten Subtypen (muskuloskelettale Infiltrate) gehören, besteht das Risiko, dass sie ein KMP entwickeln, und die Ärzte sollten besonders wachsam sein. Abstract:BACKGROUND:Kaposiform hemangioendothelioma (KHE) is a rare vascular neoplasm that mainly affects infants. KHE rarely develops in adolescents and adults. These tumors tend to be locally invasive, but are not known to produce distant metastases. Numerous treatment modalities are available for KHE, but the optimal therapy is unknown.CASE REPORT:A 51-year-old woman was diagnosed with KHE of the ilium in September 2005. The lesion recurred within 5 years of local excision, and was subsequently treated with CyberKnife. Within 1 month of CyberKnife therapy, pain intensity was significantly reduced and the patient's quality of life was significantly improved. Since January 2011, she has remained pain-free and has had no signs of recurrence or metastasis for more than 2 years following CyberKnife therapy.CONCLUSION:We report the first CyberKnife treatment of an adult with KHE of the bone without accompanying cutaneous changes and Kasabach-Merritt syndrome. CyberKnife treatment could be a useful temporizing measure for bone KHE.

Abstract:Background Kaposiform hemangioendothelioma (KHE) is a rare and invasive vascular tumor that mainly occurs in children and is rarely seen in adults. We report a case of KHE found on the eyelid of an adult patient. Case report We present an adult patient in whom KHE recurred 6 months after tumor resection. He underwent second surgical resection and intraoperative chemotherapy. There was no evidence of recurrence at the 3-year follow-up. Conclusion KHE in adults is easy to be misdiagnosed. KHE can be treated by surgical resection. Complete resection of the tumor and intraoperative chemotherapy may help prevent a recurrence.
