Abstract:Objective To evaluate the diagnostic value of Chinese assay of (1,3)-β-D-glucan in invasive fungal infections (IFI) and compare with Japenese assay.Methods A retrospective study was performed on 203 patients at risk of IFI (totally 342 sanmples) in our hospital from April,2008 to May,2009.Paired-sample t test was analyzed in 100 samples which were detected by both Chinese and Japanese assay.Sensitivity,specificity,positive predictive value (PPV) and negative predictive value (NPV) were calculated under different criterion,which was compared by McNemar paired χ2 test.Results The significance values of t test between the two assays were 0.235 for IFI samples and 0.076 for non-IFI samples,respectively.For Chinese assay ,the sensitivity (78.3%,56.5%,25.0%,25.0%) and specificity (67.2%,85.0%,64.9%,87.7%) were obtained by single cutoff values or double cutoff values of 20 pg/mL,50 pg/mL respectively.Conclusions There is no statistical significance between Chinese and Japanese assays.The specificity with single cutoff value of 50 pg/mL was significantly elevated without evident influence on sensitivity compared with that with 20 pg/mL.The NPV and specificity were elevated with double cutoff values.
The diagnostic value of detecting plasma 1,3-β-D-glucan for invasive fungal infections

Abstract:Objective To evaluate the diagnostic value of the detection of plasma 1,3-β-D-glucan for invasive fungal infections(IFI).Methods Plasma of patients with IFI,bacterial pneumonia,and oral pharyngeal fungal colonization,and healthy controls were collected from January 2005 to February 2006 in Nanjing General Hospital of Nanjing Military Command of PLA.G-test was used to measure the concentration of 1,3-β-D-glucan in the plasma.Results The concentration of 1,3-β-D-gluean in plasma of patients with IFI[(29.5±11.5)ng/L]was significantly higher than that of patients with bacterial pneumonia[(13.1± 5.2)ng/L],fungal colonization[(12.7±5.1)ng/L],and healthy controls[(11.7±3.5)ng/L],P<0.01.The concentration of 1,3-β-D-glucan in patients with bacterial pneumonia was not different as compared to that of patients with fungal colonization,and healthy controls,P>0.05.The concentration of 1,3-β-D-glucan in plasma of patients with invasive aspergillosis and invasive eandidiasis were(24.7±5.8) ng/L and(33.3±11.4)ng/L,respectively,the difference being not significant,P>0.05.The concentration of 1,3-β-D-glucan in plasma of a patients with invasive pulmonary Cryptococcus neoformans infection and a patient with histoplasmosis were 13.6 ng/L and 25.7 ng/L,respectively.If 20 ng/L was taken as the cut-off value,the sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)of 1,3-β-D-gluean were 84%,91%,84% and 91% respectively.Conclusion The concentration of 1,3-β-D-glucan in plasma of patients with IFI increases markedly,which indicates that the detection of 1,3-β-D-glucan in plasma is a useful method to diagnose IFI,but can not be used to differentiate aspergillosis from yeast fungus infection.If Fungitec-G kit is used to detect 1,3-β-D-glucan and 20 ng/L is used as the cut-off value,the sensitivity and specificity for IFI are high.

Abstract:OBJECTIVE:To evaluate the significance of a serum 1,3-beta-D-glucan (BG) assay for early diagnosis of invasive fungal infection (IFI) and determine its cutoff value in Chinese post-hematopoietic stem cell transplant (HSCT) recipients.METHODS:Serum BG levels were measured twice weekly by Glucatell kit (G-Test) in 36 post-HSCT patients with suspected IFI. The sensitivities and specificities of the assay for clinical proven IFI and non-IFI patients were calculated retrospectively according to different G-Test positive criteria and determined its cutoff.RESULTS:The sensitivity, specificity, positive and negative predictive values were 81.0%, 81.8%, 89.5% and 69.2% (P=0.002) respectively, as the cutoff value was set at more than 80 ng/L once or 60 ng/L consecutively twice.CONCLUSION:The cutoff value of G test in Chinese post HSCT patients basically is the same as specified in the instruction of the kit, and it is a quick and reliable method for early diagnosis of IFI.

Abstract:Objective To evaluate the performance of the galactomannan emzyme immunoassay (GM test) and (1,3 ) β-D-glucan assay (G test) for the diagnosis of invasive fungal infection (IFI). Methods A retrospective study was performed in 115 hospitalized patients at Peking University First Hospital who were at risk of IFI. Patients were diagnosed as IFI according to revised definitions of invasive fungal disease from the European Organization for Research and Treatment of Cancer/Invasive Fungal Infections Cooperative Group and the National Institute of Allergy and Infectious Diseases Mycoses Study Group (EORTC/MSG) Consensus Group. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated at different cutoff values for two assays respectively. Two tests were combined to evaluate the changes of sensitivity, specificity, PPV and NPV. Results The best sensitivity (54. 5%, 63.6% )and specificity(77.9% ,69.2% ) were obtained with the cutoff values of 0.5 and 20×10~3 pg/Lin GM test and G test respectively. The PPV were 20.7% and 17.9%, and the NPV were 94.2% and 94.7% respectively. The sensitivity increased to 90.9% and the specificity was 52.9% after a combined utility of two tests. Conclusion The GM test and G tests are both useful in diagnosis of IFI with the cutoff values of0.5 and 20×10~3 pg/L. A better sensitivity is acquired if there is a combined utility of two tests.

Abstract:Objective:To evaluate the value of serum galactomannan(GM) enzyme immunoassay combine with 1,3 beta-glucan(G) assay for diagnosis of invasive fungal infection(IFI) in hematological disorders.Method:Blood samples were obtained from 42 patients with neutropenic hematological disorder,fever and shown no response to broad-spectrum antibiotics therapy.Serum concentration of GM and G were detected by enzyme linked immunosorbent assay.The proven IFI and non-IFI patients were defined by retrospective clinical analysis according to domestic IFI diagnostic criteria.Result:GM positive was defined as I index1.0 and G positive was defined as G60 ng/L,The sensitivity an specificity of both GM and G assay were 77.8% and 90.3% and the positive and negative predictive values were 82.4% and 87.5% respectively.The performance of G-based diagnosis appears to be better for sensitivity.GM assay was shown to have a higher specificity than BG assay.Conclusion:Serum GM enzyme immunoassay combine with 1,3 beta-glucan(G) assay for diagnosis of IFI in hematological disorders increased sensitivity of detecting IFI and decreased false negative cases,and it could be a more reliable method for early diagnosis of IFI.

Abstract:OBJECTIVES: The serum (1,3)-beta-d-glucan (BDG) assay gives quicker results and has higher sensitivity than blood cultures, therefore it is advised for early diagnosis of invasive candidemia and/or discontinuation of empirical therapy. Its sensitivity may depend on different factors. The aim of our study was to analyse the in vitro and in vivo BDG levels in clinical isolates of three species of Candida responsible for candidemia.METHODS: C. albicans, C. parapsilosis, and C. auris strains were collected from blood cultures of patients who had a concurrent (-1 to +3 days) serum BDG test (Fungitell assay). Supernatants of all strains were tested in quadruplicate for BDG levels.RESULTS: Twenty-two C. auris, 14 C. albicans, and ten C. parapsilosis strains were included. The median BDG levels in supernatants were 463 pg/mL (interquartile range [IQR] 379-648) for C. auris, 1080 pg/mL (IQR 830-1276) for C. albicans, and 755 pg/mL (IQR 511-930) for C. parapsilosis, with the significant difference among the species (p < 0.0001). Median serum BDG levels (IQR) were significantly lower in case C. auris and C. parapsilosis vs. C. albicans (p < 0.0001), respectively, 50 pg/mL (IQR 15-161) and 57 pg/mL (IQR 18-332), vs. 372 pg/mL (IQR 102-520). Sensitivity of serum BDG was 39% (95% confidence interval [CI], 18-64) in case of C. auris, 30% (95% CI, 8-65) C. parapsilosis and 78% (95% CI, 49-94) C. albicans candidemia.DISCUSSION: In our centre C. auris and C. parapsilosis strains have lower BDG content as compared with C. albicans, with a potential impact on serum BDG performance for the diagnosis of candidemia.

Abstract:Background and Objective: Pulmonary fungal infections (PFIs) are bronchial or pulmonary diseases caused by fungi. It is necessary to explore an accurate, convenient and rapid diagnostic method for PFIs. This study was conducted to investigate the diagnostic significance of (1,3) beta-D-glucan (BDG) detection in PFI. Materials and Methods: This is a retrospective study. A total of 139 PFI patients were performed with bronchoalveolar lavage. Alveolar lavage fluid (4 mL) and fasting peripheral venous blood (2 mL) of the patients were prepared to conduct BDG detection using the ELISA method. Following this test, the patient's sputum was collected for fungal culture and the expression level of BDG in alveolar lavage fluid and blood was determined, a diagnostic value of sputum culture and BDG assay results towards PFI was explored. Results: As 126 patients were diagnosed with PFI and another 13 patients were diagnosed with bacterial infections after re-examination. Among the diagnosed patients, Aspergillus accounted for the highest proportion, 36.51%. For the diagnosed patients, the BDG expression in alveolar lavage fluid and blood specimens was sharply uplifted when compared with the undiagnosed patients (p<0.001). Among all pathogenic strains, the BDG expression reached the highest in subjects who were pathogenic to Cryptococcus (p<0.001). Conclusion: The BDG is highly efficient in diagnosing PFI. It can be used as the best clinical diagnosis target for PFI combined with a fungal culture.
Variable reliability of the (1,3)-β-d-glucan test for screening Pneumocystis pneumonia in HIV-negative patients depending on the underlying condition

Abstract:(1,3)-β- d -Glucan (BG) assay is a non-invasive test commonly used in the diagnostic of invasive fungal diseases. Given its high sensitivity, it was suggested that a negative BG result is sufficient for excluding the diagnosis of Pneumocystis pneumonia (PCP). However, suboptimal performance has been described in human immunodeficiency virus (HIV)-negative patients, particularly those with haematological malignancies. We aimed to assess the sensitivity of the BG assay for diagnosing PCP in HIV-negative patients based on their underlying PCP risk factors. We conducted a single-center, retrospective study (2009–2021) enrolling HIV-negative patients diagnosed with PCP and who underwent BG testing. Patients colonized with Pneumocystis jirovecii were included as a control group. In all, 55 PCP patients and 61 colonized patients met the inclusion criteria. Patients were further categorized according to the underlying condition that exposes patients to PCP. Median BG concentration was significantly higher in the PCP group than in the colonization group (500 vs. 31 pg/ml; P < 10 −4 , Mann–Whitney test) and the BG assay demonstrated a sensitivity of 85% and a specificity of 82% for PCP diagnosis. Notably, sensitivity was significantly higher in non-cancer patients (100%) compared to those with solid cancer (72%) and haematologic cancer (79%) ( P < .05, Fischer's exact test). These findings strengthen the high performance of BG testing for screening PCP in non-cancer patients, comparable to that observed in HIV-infected individuals. In contrast, they highlight its low reliability in patients with malignancies, emphasizing the importance of considering underlying conditions when interpreting BG results and refining the role of the test in PCP diagnosis. Pneumocystis jirovecii is a fungus that causes pulmonary infections in immunocompromised persons. The serum (1,3)-β- d -glucan assay is widely used to screen for Pneumocystis pneumonia, but its reliability in at-risk patients without acquired immunodeficiency syndrome requires further clarification.

Abstract:Objective To investigate the diagnostic values of (1,3)-β-D-glucan (G) and galactomannan (GM) for invasive fungal infection (IFI) in patients of acute radiation sickness (ARS).Methods Samples of periogeral blood,pharyngeal secretion,urine,and feces were collected from 316patients with ARS and suspected to suffer from IFI,192 males and 124 females,aged 60.50(1-96),with the underlying diseases of blood or respiration systems.Platelia Aspergillus EIA kit was used to detect the plasma BG (G test),and ELISA was used to detect the serum GM (GM test).Fungal culture and bacterial culture were performed.Results The positive rates of G test,GM test,and fungal culture were 36.33%,35.84% ,and 34.18% respectively,but the positive rate of fungal culture of blood sample was 1/316 only.Pearson correlation analysis showed that G test,GM test and fungal culture test were positively correlated with IFI clinical diagnosis respectively (x2 = 0.564,0.357,0.727,P ＜ 0.05).Conclusions Easy to operate,rapid,and highly sencitive,G test and GM test can be used as adjunctive methods for early IFI diagnosis in ARS patients.

Abstract:Abstract Background β-d-glucan (BG) detection was first available in Thailand in 2016 to aid diagnosis of invasive fungal infections (IFIs). Given a paucity of real-world experience of BG use in resource-limited countries, this study was conducted to describe appropriateness of BG testing and sequelae of BG results. Methods A retrospective study included all patients with at least 1 BG ordered at King Chulalongkorn Memorial Hospital, Bangkok, Thailand during March 2016 to December 2018. Descriptive statistics were used. Results 83 patients were tested by BG assay (Fungitell, Associates of Cape Cod, Inc.): 6 with hematopoietic stem cell transplant, 12 with solid-organ transplant, 20 with active cancer receiving chemotherapy, 34 receiving high dose steroids (≥20 mg/day of prednisone for ≥3 weeks) and 11 with other conditions. Seventy-three patients were tested under infectious disease (ID) service’s recommendations. There were 13 and 20 cases of proven and probable IFIs, respectively. Among 13 proven IFIs, there were 11 positive, 1 indeterminate and 1 negative (mucormycosis) BG results. Among 49 cases with positive BG results, 24 were determined to be false-positive results. Median turn-around time for BG results was 16 (IQR: 9–23) days. Due to high turn-around time, only 8 patients were started on antifungal agent(s) and 3 underwent bronchoscopy due to positive BG results. All proven IFI cases were started on antifungal treatment prior to BG availability. Conclusion Approximately 87% of BG use in Thailand was ordered in patients with risk factors for IFIs. This could be due to majority of BG test was recommended by ID specialist. Despite being used in right clinical context, 49% had false-positive BG results. Another barrier of BG use in Thailand was high turn-around time due to small numbers of BG ordered and relative high cost to run the assay. Therefore, the utility of BG for aiding diagnosis or management of fungal infection in our setting is limited. Disclosures All authors: No reported disclosures.

Abstract:Pneumocystis jirovecii pneumonia (PCP) can affect various types of immunocompromised patients. We sought to evaluate the diagnostic accuracy of (1 → 3)-β-D-glucan (BDG) for the diagnosis of PCP. We carried out a meta-analysis of relevant studies, identified through PubMed and Scopus. Eligible studies were those that reported BDG diagnostic data in cases with documented PCP and controls with other conditions. Cases of invasive fungal infections and healthy controls were excluded. We performed a bivariate meta-analysis of sensitivity and specificity and constructed a hierarchical summary receiver operating characteristics (HSROC) curve. Fourteen studies were included in the meta-analysis. BDG data were analysed for 357 PCP cases and 1723 controls. The average (95% confidence interval) sensitivity and specificity of BDG were 94.8% (90.8–97.1%) and 86.3% (81.7–89.9%), respectively. The positive and negative likelihood ratios were 6.9 (5.1–9.3) and 0.06 (0.03–0.11), respectively. The area under the HSROC curve was 0.965 (0.945–0.978). Serum BDG shows excellent sensitivity and very good specificity in the diagnosis of PCP. Still, in clinical practice the test results should be interpreted in the context of the underlying clinical characteristics of the individual patient.
Value of 1-3-β-D glucan in diagnosis of AIDS-related Pneumocystis pneumonia

Abstract:OBJECTIVE To explore the clinical value of domestic plasma 1-3-β-D glucan test reagent in diagnosis of AIDS complicated with Pneumocystis pneumonia (PCP) and evaluation of curative effect.METHODS A total of 65 AIDS patients complicated with pulmonary infections who did not have other fungal infections and were treated in the hospital from Jan 2015 to Jun 2016 were enrolled in the study.According to the criteria for diagnosis of PCP in the Third Edition of the Guidelines for Diagnosis and Treatment of HIV/AIDS(2015),27 AIDS patients complicated with PCP were screened out and assigned as the case group,and 38 AIDS patients complicated with other pulmonary infections were set as the control group.The concentration of plasma 1-3-β-D glucan was detected by using domestic test reagents at the admission to the hospital and after the treatment for 3 weeks,and the statistical analysis was performed.RESULTS The median concentration of plasma 1-3-β-D glucan of the case group was 40.15 pg/ml,higher than 5.92 pg/ml of the control group(P＜0.001).The analysis of receiver-operating-characteristic (ROC) curve indicated that the optimal threshold of the plasma 1-3-β-D glucan was 16.51 pg/ml in the diagnosis of PCP,the area under curve (AUC) was 0.925 (95％CI 0.864～0.985,P＜0.001),and the sensitivity and specificity were 85.20％ and 86.80％,respectively.The plasma 1-3-β-D glucan level of the case group was 4.28 pg/ml after the treatment for 3 weeks,lower than 40.31 pg/ml before the treatment (P =0.005).CONCLUSION The domestic plasma 1-3-β-D glucan test reagent has significant clinical value in diagnosis of AIDS complicated with PCP and evaluation of curative effect.

Abstract:Deaths due to invasive fungal disease (IFD) have been increasing every year. Early and rapid detection is important to reduce the mortality rate associated with IFD. In this study, we explored a novel diagnostic method for detecting IFD, which involves the G Factor α subunit (GFαSub) from Limulus polyphemus . The GFαSub double-sandwich method was developed to detect (1,3)-β- D -glucans in human serum using purified GFαSub and horseradish peroxidase-labeled GFαSub. The GFαSub double-sandwich method and the G test were performed and compared. Using GFαSub sequence analysis, the expression plasmid pET30a-GFαSub252-668 was synthesized, and GFαSub252-668 was expressed and purified via isopropyl-β-d-thiogalactoside induction and nickel-nitrilotriacetic acid affinity. The optimization method was established via the orthogonal method. Using this method, the sera of 36 patients with IFD and 92 volunteers without IFD underwent detection, and the receiver operating characteristic curve of the GFαSub252-668 double-sandwich method was described. The sensitivity and specificity of the GFαSub252-668 double-sandwich method were 91.67 and 82.61%, respectively, and there was good correlation with the G test for the serum specimens of 36 patients with pulmonary IFD ( R 2 = 0.7592). In conclusion, our study suggests that the GFαSub252-668 double-sandwich method was satisfactory at detecting IFD cases. This method can be promoted and further developed as a novel method for diagnosing IFD.

Abstract:Purpose: To explore the clinical value of β-D-glucan (BDG) testing and next-generation metagenomic sequencing (mNGS) for detecting the pathogens of fungal endophthalmitis (FE). Methods: This study included 32 cases (32 eyes) with FE and 20 cases (20 eyes) with intraocular inflammation caused by other etiologies. All patients underwent extraction of aqueous humor or vitreous fluid samples for BDG testing and mNGS. The diagnostic performance and total clinical concordance rate (TCCR) of BDG testing and mNGS for FE were evaluated and calculated based on the results of the clinical diagnosis. Results: Among the clinically diagnosed FE, the positivity rates of BDG testing and mNGS (90.63%) were both significantly higher ( P <0.001) than that of microbial cultures (53.13%). There was 100% consistency in pathogen identification using mNGS and culture identification for culture-positive cases. The area under the curve (AUC) was 0.927 for BDG testing and 0.853 for mNGS. When the 2 tests were combined, the sensitivity (93.75%), specificity (100.00%), and TCCR (96.15%) were all improved compared with the single tests. Conclusions: The positive rates of BDG test and mNGS were markedly higher than those of cultures in FE identification. The combination of these 2 tests showed improved performance when compared with individual tests.

Abstract:Timely diagnosis of invasive fungal diseases (IFDs) is important, as delays in treatment initiation are associated with increased mortality rates. However, early diagnosis of IFDs in immunocompromised patients remains difficult. The conventional diagnostic methods currently used for IFDs are not sufficiently effective. Molecular tests, such as polymerase chain reaction (PCR)-based assays, have great potential to improve the early diagnosis of IFDs due to their sensitivity and specificity. In the present study, the diagnostic performance of panfungal PCR assays in IFD patients who received bone marrow transplantation was evaluated. The results suggested that panfungal PCR assay offered a quick and convenient guide for clinical decision-making by identifying higher numbers of fungal species in comparison with the conventional blood culture method. Furthermore, panfungal PCR assay exhibited a sensitivity of 93% and a specificity of 71% in the diagnosis of IFD patients based on the EORTC/MSG criteria. Thus, the present study concluded that the reported PCR-based method was effective and sensitive in early IFD diagnosis and should be integrated into clinical decision-making for the treatment of IFDs in the future.
Negative serum (1,3) -β-D-glucan has a low power to exclude Pneumocystis jirovecii pneumonia (PJP) in HIV-uninfected patients with positive qPCR

Abstract:Objective: The current study evaluated the diagnostic performance of serum (1,3)-beta-D Glucan (BDG) in differentiating PJP from P. jirovecii-colonization in HIV-uninfected patients with P. jirovecii PCR-positive results. Methods: This was a single-center retrospective study between 2019 and 2021. The diagnosis of PJP was based on the following criteria: detection of P. jirovecii in sputum or BAL specimen by qPCR or microscopy; Meet at least two of the three criteria: (1) have respiratory symptoms of cough and/or dyspnea, hypoxia; (2) typical radiological picture findings; (3) receiving a complete PJP treatment. After exclusion, the participants were divided into derivation and validation cohorts. The derivation cohort defined the cut-off value of serum BDG. Then, it was verified using the validation cohort. Results: Two hundred and thirteen HIV-uninfected patients were enrolled, with 159 PJP and 54 P. jirovecii-colonized patients. BDG had outstanding specificity, LR, and PPV for PJP in both the derivation (90.00%, 8.900, and 96.43%) and the validation (91.67%, 9.176, and 96.30%) cohorts at ≥ 117.7 pg/mL. However, it had lower sensitivity and NPV in the derivation cohort (89.01% and 72.97%), which was even lower in the validation cohort (76.47% and 57.89%). Of note, BDG ≥ 117.7 pg/mL has insufficient diagnostic efficacy for PJP in patients with lung cancer, interstitial lung disease (ILD) and nephrotic syndrome. And although lymphocytes, B cells, and CD4+ T cells in PJP patients were significantly lower than those in P. jirovecii-colonized patients, the number and proportion of peripheral blood lymphocytes did not affect the diagnostic efficacy of serum BDG. Conclusions: Serum BDG ≥ 117.7 pg/mL could effectively distinguish P. jirovecii-colonization from infection in qPCR-positive HIV-uninfected patients with infectious diseases, solid tumors (excluding lung cancer), autoimmune or inflammatory disorders, and hematological malignancies. Of note, for patients with lung cancer, ILD, and nephrotic diseases, PJP should be cautiously excluded at BDG < 117.7 pg/mL.

Abstract:Purpose: The study aimed to evaluate the corrected colonization index (CCI) and (1, 3)-ß-D Glucan (BDG) in diagnosis of Invasive Candidiasis (IC) in critically ill pediatric patients. Methods: A prospective observational study in a tertiary care (PICU) were surveyed for Candida colonization and CCI was calculated. For cases with suspicious clinical presentation, samples were cultured, and double(1,3) ß-D- glucan (BDG) performed. Results: According to the European Organization for Research and Treatment of Cancer EORTC case definition for critically ill non-neutropenic patients, only 7.14 % (9/188) were diagnosed as IC (4 proven and 5 probable cases). The combined use of CCI with BDG proved to have excellent discriminative power AUC= 0.946, improved sensitivity 87.5 % and specificity 85.71 %. Conclusion: The key in diagnosis of IC relies in compiling proofs from the clinical context, high CCI (≥ 0.4) and BDG.

Abstract:Abstract Background Precision medicine risk stratification is desperately needed to both avoid systemic antifungals treatment delay and over prescription in the critically ill with risk factors. The aim of the present study was to explore the combination of host immunoparalysis biomarker (monocyte human leukocyte antigen-DR expression (mHLA-DR)) and Candida sp wall biomarker β- d -glucan in risk stratifying patients for secondary invasive Candida infection (IC). Methods Prospective observational study. Two intensive care units (ICU). All consecutive non-immunocompromised septic shock patients. Serial blood samples ( n = 286) were collected at day 0, 2 and 7 and mHLA-DR and β- d -glucan were then retrospectively assayed after discharge. Secondary invasive Candida sp infection occurrence was then followed at clinicians’ discretion. Results Fifty patients were included, 42 (84%) had a Candida score equal or greater than 3 and 10 patients developed a secondary invasive Candida sp infection. ICU admission mHLA-DR expression and β- d -glucan (BDG) failed to predict secondary invasive Candida sp infection. Time-dependent cause-specific hazard ratio of IC was 6.56 [1.24–34.61] for mHLA-DR 350 pg/mL. Predictive negative value of mHLA-DR > 5000 Ab/c and BDG > 350 pg/mL combination at day 7 was 81% [95% CI 70–92]. Conclusions This study suggests that mHLA-DR may help predicting IC in high-risk patients with septic shock. The added value of BDG and other fungal tests should be regarded according to the host immune function markers.
