Abstract:Post-COVID-19 condition (PCC) is defined as the persistence of symptoms, like fatigue and dyspnea, at least 3 months post-COVID infection. As dyspnea is a common symptom, we attempted to further clinically phenotype those with PCC-associated dyspnea. 1642 adults (average age of 49.6y with 63% female-predominance and BMI of 31.2 kg/m 2 ) with physician confirmed diagnosis of PCC from June 2020–April 2023 in Alberta, Canada were included. Those with dyspnea were more likely to be female (56.5%, p = 0.005) and have higher BMI (31.3 kg/m 2 vs. 29.5 kg/m 2 ; p = 0.0008), history of asthma (21.1% vs. 12.3%; p < 0.001), more persistent PCC symptoms ( p = 0.0001), more functional limitations, as well as lower quality of life ( p < 0.0001). Multivariable-adjusted logistic regression analysis demonstrated dyspnea was independently associated with fatigue (OR = 4.20; CI = 2.71,6.59) and inversely associated with hospitalization for COVID-19 (OR = 0.53; CI = 0.32,0.91), age (OR = 0.98 per one year of age; CI = 0.96,0.99) and 6-min-walk-distance per 10 m difference (OR = 0.98, CI = 0.96,1.0). Fatigue was a predictor of dyspnea, and was associated with milder infection, higher BMI, and reduced 6-min-walk-distance despite normal pulmonary function. Reduced TLC or DLCO was associated with more severe infection and reduced 6-min-walk-distance. Thus, we speculate there are at least two dyspnea-associated phenotypes: phenotype with pronounced fatigue (normal PFT) and phenotype with pronounced pulmonary abnormalities (abnormal PFT). Improved understanding of the dyspnea-associated phenotypes may allow for better targeted rehabilitation.

Cardiopulmonary exercise pattern in patients with persistent dyspnoea after recovery from COVID-19

Abstract:Cause and mechanisms of persistent dyspnoea after recovery from COVID-19 are not well described. The objective is to describe causal factors for persistent dyspnoea in patients after COVID-19. We examined patients reporting dyspnoea after recovery from COVID-19 by cardiopulmonary exercise testing. After exclusion of patients with pre-existing lung diseases, ten patients (mean age 50±13.1 years) were retrospectively analysed between May 14th and September 15th, 2020. On chest computed tomography, five patients showed residual ground glass opacities, and one patient showed streaky residua. A slight reduction of the mean diffusion capacity of the lung for carbon monoxide was noted in the cohort. Mean peak oxygen uptake was reduced with 1512±232 ml/min (72.7% predicted), while mean peakwork rate was preserved with 131±29 W (92.4% predicted). Mean alveolar-arterial oxygen gradient (AaDO2) at peak exercise was 25.6±11.8 mmHg. Mean value of lactate post exercise was 5.6±1.8 mmol/l. A gap between peak work rate in (92.4% predicted) to peak oxygen uptake (72.3% pred.) was detected in our study cohort. Mean value of lactate post exercise was high in our study population and even higher (n.s.) compared to the subgroup of patients with reduced peak oxygen uptake and other obvious reason for limitation. Both observations support the hypothesis of anaerobic metabolism. The main reason for dyspnoea may therefore be muscular.
Persistence of Diffusion Capacity Impairment and Its Relationship with Dyspnea 12 Months after Hospitalization for COVID-19

Abstract:Background: Dyspnea is a common persistent symptom after acute coronavirus disease 2019 illness (COVID-19). One potential explanation for post-COVID-19 dyspnea is a reduction in diffusion capacity. This longitudinal study investigated diffusion capacity and its relationship with dyspnea on exertion in individuals previously hospitalized with COVID-19. Methods: Eligible participants had been hospitalized for the treatment of acute COVID-19 and were assessed at 6 weeks, 6 months, and 12 months after discharge. Pulmonary function testing, diffusion capacity of carbon monoxide (DLCO), blood gas analysis and the level of dyspnea (Borg scale; before and after a 6 min walk test [6 MWT]) were performed. Participants were divided into subgroups based on the presence or absence of dyspnea during the 6 MWT at 12 months after hospitalization. Results: Seventy-two participants (twenty-two female, mean age 59.8 ± 13.5 years) were included. At 12 months after discharge, 41/72 participants (57%) had DLCO below the lower limit of normal and 56/72 (78%) had DLCO < 80% of the predicted value. Individuals with exertional dyspnea had significantly lower DLCO than those without exertional dyspnea (p = 0.001). In participants with DLCO data being available at three timepoints over 12 months (baseline, 6 months, and 12 months) after discharge (n = 25), DLCO improved between 6 weeks and 6 months after hospital discharge, but not thereafter (p = 0.017). Conclusions: About 2/3 of the post-COVID individuals in this study had impaired diffusion capacity at 12 months after hospital discharge. There was an association between persisting dyspnea on exertion and significantly reduced DLCO. Impaired diffusion capacity improved over the first 6 months after hospitalization but not thereafter.

Abstract:Objectives: Dyspnoea is a common persistent symptom post-coronavirus disease 2019 (COVID-19) illness. However, the mechanisms underlying dyspnoea in the post-COVID-19 syndrome remain unclear. The aim of our study was to examine dyspnoea quality and intensity, burden of mental health symptoms, and differences in exercise responses in people with and without persistent dyspnoea following COVID-19. Methods: 49 participants with mild-to-critical COVID-19 were included in this cross-sectional study 4 months after acute illness. Between-group comparisons were made in those with and without persistent dyspnoea (defined as modified Medical Research Council dyspnoea score ≥1). Participants completed standardised dyspnoea and mental health symptom questionnaires, pulmonary function tests, and incremental cardiopulmonary exercise testing. Results: Exertional dyspnoea intensity and unpleasantness were increased in the dyspnoea group. The dyspnoea group described dyspnoea qualities of suffocating and tightness at peak exercise (p<0.05). Ventilatory equivalent for carbon dioxide (V'E/V'CO2) nadir was higher (32±5 versus 28±3, p<0.001) and anaerobic threshold was lower (41±12 versus 49±11% predicted maximum oxygen uptake, p=0.04) in the dyspnoea group, indicating ventilatory inefficiency and deconditioning in this group. The dyspnoea group experienced greater symptoms of anxiety, depression and post-traumatic stress (all p<0.05). A subset of participants demonstrated gas-exchange and breathing pattern abnormalities suggestive of dysfunctional breathing. Conclusions: People with persistent dyspnoea following COVID-19 experience a specific dyspnoea quality phenotype. Dyspnoea post-COVID-19 is related to abnormal pulmonary gas exchange and deconditioning and is linked to increased symptoms of anxiety, depression and post-traumatic stress.

Abstract:New coronavirus infection may lead to long-term consequences, particularly to post-COVID syndrome, one of the most common manifestations of which is dyspnea. Post-COVID-19 shortness of breath may persist from one to several months and even years that results in low quality of life of patients. The review highlights possible risk factors and causes of dyspnea in post-COVID period such as lung damage, cardiovascular pathology, hyperventilation syndrome, dysfunction of the autonomic nervous system, detraining, anemia, etc. The authors present data about COVID-19-associated causes of dyspnea and severity of acute COVID-19. The review emphasizes the importance of a multidisciplinary approach to the diagnosis and treatment of patients with shortness of breath in post-COVID-19 period.

Abstract:Background: Dyspnoea is a common persistent symptom after COVID-19. Whether it is associated with functional respiratory disorders remains unclear. Methods: We assessed the proportion and characteristics of patients with "functional respiratory complaints" (FRCs) (as defined by Nijmegen Questionnaire >22) among 177 post-COVID-19 individuals who benefited from outclinic evaluation in the COMEBAC study (i.e., symptomatic and/or intensive care unit (ICU) survivors at 4 months). In a distinct explanatory cohort of 21 consecutive individuals with unexplained post-COVID-19 dyspnoea after routine tests, we also analysed the physiological responses to incremental cardiopulmonary exercise testing (CPET). Findings: In the COMEBAC cohort, 37 patients had significant FRCs (20.9%, IC95: 14.9-26.9). The prevalence of FRCs ranged from 7.2% (ICU patients) to 37.5% (non-ICU patients). The presence of FRCs was significantly associated with more severe dyspnoea, lower 6-min walk distance, more frequent psychological and neurological symptoms (cognitive complaint, anxiety, depression, insomnia and post-traumatic stress disorders) and poorer quality of life (all p<0.01). In the explanatory cohort, seven out of 21 patients had significant FRCs. Based on CPET, dysfunctional breathing was identified in 12 out of 21 patients, five out of 21 had normal CPET, three out of 21 had deconditioning and one out of 21 had evidence of uncontrolled cardiovascular disease. Interpretation: FRCs are common during post-COVID-19 follow-up, especially among patients with unexplained dyspnoea. Diagnosis of dysfunctional breathing should be considered in those cases.

Abstract:Background Long COVID is defined as persistency of symptoms, such as exertional dyspnea, twelve weeks after recovery from SARS-CoV-2 infection. Objectives To investigate ventilatory efficiency by the use of cardiopulmonary exercise testing (CPET) in patients with exertional dyspnea despite normal basal spirometry after 18 (T 18 ) and 36 months (T 36 ) from COVID-19 pneumonia. Methods One hundred patients with moderate-critical COVID-19 were prospectively enrolled in our Long COVID program. Medical history, physical examination and lung high-resolution computed tomography (HRCT) were obtained at hospitalization (T 0 ), 3 (T 3 ) and 15 months (T 15 ). All HRCTs were revised using a semi-quantitative CT severity score (CSS). Pulmonary function tests were obtained at T 3 and T 15 . CPET was performed in a subset of patients with residual dyspnea (mMRC ≥ 1), at T 18 and at T 36 . Results Remarkably, at CPET, ventilatory efficiency was reduced both at T 18 (V' E /V'CO 2 slope = 31.4±3.9 SD) and T 36 (V' E /V'CO 2 slope = 31.28±3.70 SD). Furthermore, we identified positive correlations between V' E /V'CO 2 slope at T 18 and T 36 and both percentage of involvement and CSS at HRCT at T 0 , T 3 and T 15 . Also, negative linear correlations were found between V' E /V'CO 2 slope at T 18 and T 36 and DL CO at T 3 and T 15 . Conclusions At eighteen months from COVID-19 pneumonia, 20% of subjects still complains of exertional dyspnea. At CPET this may be explained by persistently reduced ventilatory efficiency, possibly related to the degree of lung parenchymal involvement in the acute phase of infection, likely reflecting a damage in the pulmonary circulation.

Abstract:Background: Patients with post-COVID-19 condition frequently suffer from chronic dyspnoea. The causes and mechanism for dyspnoea in these patients without evidence of structural lung disease are unclear. Methods: Patients treated for COVID-19 at Charité University Hospital in Berlin received pulmonary function testing including respiratory muscle strength tests and completed health-related quality-of-life questionnaires during follow-up. Patients with post-COVID-19 condition during outpatient follow-up with fatigue and exertional intolerance (PCF) were compared to patients with post-COVID-19 condition with evidence of chronic pulmonary sequelae (post-COVID-19 restriction (PCR)) as well as to patients without post-COVID-19 condition (NCF). Results: A total of 170 patients presented for follow-up. 36 participants met criteria for PCF, 28 for PCR and 24 for NCF. PCF patients reported dyspnoea in 63.8%. % predicted value of respiratory muscle strength (median (IQR)) was reduced in PCF (55.8 (41.5-75.9)) compared to NCF and PCR (70.6 (66.3-88.9) and 76.8 (63.6-102.2), respectively; p=0.011). A pattern of reduced forced vital capacity (FVC), but normal total lung capacity (TLC), termed complex ventilatory dysfunction defined as TLC - FVC >10% predicted was observed and occurred more frequently in PCF (88.9%) compared to NCF and PCR (29.1% and 25.0%, respectively; p<0.001). Conclusion: Dyspnoea in PCF is characterised by reduced respiratory muscle strength and complex ventilatory dysfunction indicating neuromuscular disturbance as a distinct phenotype among patients with post-COVID-19 condition. These observations could be a starting point for developing personalised rehabilitation concepts.

Abstract:Objective: Contributing to elucidate the pathophysiology of dyspnoea and exertion intolerance in post-COVID syndrome patients with normal cardiopulmonary imaging and functional tests at rest, while determining their fitness and level of endurance in order to individualize working parameters for physical rehabilitation. Material and methods: After an anamnesis and clinical examination at rest, 27 subjects (50±11.9 years) (14 women) with post-COVID syndrome of more than 6 months of evolution performed a continuous maximal-incremental graded cardiopulmonary exercise test (CPET) with breath-by-breath gas-exchange monitoring and continuous ECG registration, on an electromagnetically braked cycle ergometer. The values obtained were compared with those of reference, gender or controls, using the Chi-square, t-Student or ANOVA test. Results: The clinical examination at rest and the CPET were clinically normal and without adverse events. Reasons for stopping exercise were leg discomfort. It is only worth noting a BMI=29.9±5.8kg/m2 and a basal lactate concentration of 2.1±0.7mmol/L. The physiological assessment of endurance showed the following results relative to predicted VO2máx: 1)peakVO2=80.5±18.6%; 2)VO2 at ventilatory threshold1 (VO2VT1): 46.0±12.9%; 3)VO2VT2: 57.2±16.4%; 4)working time in acidosis: 5.6±3,0minutes; and 5)maximum lactate concentration: 5.1±2.2mmol/L. Conclusions: The CPET identified limited aerobic metabolism and early increase in glycolytic metabolism as causes of dyspnoea and exercise intolerance, determined fitness for physical rehabilitation, and individualized it based on the level of endurance.

Abstract:Since the outbreak of the coronavirus (COVID-19) pandemic, most attention has focused on containing transmission and addressing the surge of critically ill patients in acute care settings. As we enter the second phase of the pandemic, emphasis must evolve to post-acute care of COVID-19 survivors. Persisting cardiorespiratory symptoms have been reported at several months after the onset of the infection. Information is lacking on the pathophysiology of exercise intolerance after COVID-19. Previous outbreaks of coronaviruses have been associated with persistent dyspnea, muscle weakness, fatigue and reduced quality of life. The extent of Covid-19 sequelae remains to be evaluated, but persisting cardiorespiratory symptoms in COVID-19 survivors can be described as two distinct entities. The first type of post-Covid symptoms are directly related to organ injury in the acute phase, or the complications of treatment. The second type of persisting symptoms can affect patients even with mild initial disease presentation without evidence of organ damage. The mechanisms are still poorly qualified to date. There is a lack of correlation between initial symptom severity and residual symptoms at exertion. We report exercise hyperventilation as a major limiting factor in COVID-19 survivors. The origin of this hyperventilation may be related to an abnormality of ventilatory control, by either hyperactivity of activator systems (automatic and cortical ventilatory control, peripheral afferents, and sensory cortex) or failure of inhibitory systems (endorphins) in the aftermath of pulmonary infection. Hyperventilation-induced hypocapnia can cause a multitude of extremely disabling symptoms such as dyspnea, tachycardia, chest pain, fatigue, dizziness and syncope at exertion.

Abstract:We investigated the triboluminescence (TL) of sugars using an innovative experimental approach: the laser-induced shock wave (LISW) technique. We found that the LISW could induce very bright TL in crystalline sugars, the intensity of which was shown to be 10(5) times higher than that obtained by conventional manual hand rubbing. We also applied the LISW technique to amorphous sugar samples. Although it was supposed that TL could not be excited in amorphous solids of sugars having isotropic structures, we revealed that LISW could induce bright TL in amorphous sugars similar to that induced in crystalline sugars. On the basis of the experimental results showing the dynamic behavior of the sample fracture combined with those of the TL, we discuss these novel TL mechanisms in sugars. We believe that the shock wave technique opens a new channel for investigating the nature of TL.

Abstract:Dyspnea is a common yet poorly understood symptom of long COVID, affecting many patients. This brief report examines the role of dysfunctional breathing in persistent dyspnea among patients with mild post-COVID-19 using hyperventilation provocation tests (HVPT). In this case series, six patients with unexplained dyspnea and normal cardiopulmonary function underwent HVPT. Despite normal exercise testing results, all patients exhibited delayed PETCO 2 recovery, indicative of a hyperventilation pattern consistent with chronic hyperventilation syndrome, without typical symptomatic manifestations. These findings suggest underlying post-COVID respiratory dysregulation, emphasizing the importance of targeted diagnostic and therapeutic approaches for persistent respiratory symptoms in long COVID patients.

Abstract:Background: Respiratory symptoms may persist for several weeks following the initial coronavirus disease 2019 (COVID-19) infection. The aims and objectives were to assess the clinical symptoms, pulmonary functions, and radiological changes and to assess the cardio-vascular complications in post-COVID-19 patients. Methods: This observational study was conducted in the Department of Pulmonary Medicine in collaboration with the Department of Cardiology, SCBMCH, Cuttack, from March 2021 to August 2022 on 75 post-COVID-19 patients with respiratory symptoms from 4 weeks to 2 years after treatment for COVID-19 infection. Post-COVID patients having previous respiratory diseases were excluded from the study. Results: Among 75 patients, the most common age group was 18-30 years with a male-to-female ratio of 2.5:1. Based on O2 requirement, patients were divided into the mild symptomatic group and moderate to severe pneumonia group. The most common respiratory symptom was dyspnea, followed by cough with expectoration. Bilateral crepitations were found in 17% of cases. C-reactive protein (CRP) and D-dimer were increased in 38.6% and 32% of patients, respectively. 42.6% had abnormal chest X-ray, and the most common abnormal finding was reticular thickening. In spirometry, the restrictive pattern and mixed pattern were the predominant types documented in 49.3% and 13.3% of cases, respectively, which were significant in the moderate-severe group. Diffusion capacity of the lungs for carbon monoxide (DLCO) was performed in only 19 patients (mild group 13 and moderate-severe group 6). Twelve (63.2%) patients had abnormal DLCO. P- values were significant for RV (0.0482) and RV/TLC (0.0394). High-resolution computed tomography (HRCT) of the thorax was abnormal in 55.7% with the most common abnormalities as inter- and intra-lobular septal thickening. The left ventricular ejection fraction was preserved in all patients, with right atrium and right ventricle enlargement in 2.6% and pulmonary hypertension in 4.0% of participants. Conclusion: All post-COVID-19 patients having respiratory symptoms after recovery from acute COVID-19 may be referred by family care physicians to a dedicated post-COVID center for further evaluation, management, and early rehabilitation to decrease the morbidity in recovered patients. Persistent increased blood parameters like TLC, N/L ratio, RBS, CRP, and D-dimer seen in recovered post-COVID-19 patients. The long-term impact of CT findings on respiratory symptoms, pulmonary functions, and quality of life is unknown. Cardiovascular abnormalities in post-COVID-19 patients are infrequent.

Bronchodilator response in Post-COVID-19 patients undergoing pulmonary rehabilitation

Abstract:Introduction: SARS-CoV-2 infections can result in a broad spectrum of symptoms from mild to life-threatening. Long-term consequences on lung function are not well understood yet. Methods: In our study, we have examined 134 post-COVID patients (aged 54.83±14.4 years) with dyspnea on exertion as a leading symptom 6 weeks to 24 months after a SARS-CoV-2 infection for bronchodilator responsiveness during their stay in our pulmonary rehabilitation clinic. Results: Prior to bronchial dilation, six out of 134 patients (4.47%) presented an FEV1/FVC ratio below lower limit of normal (Z-score=-1.645) indicative of an obstructive airway disease. Following inhalation of a ß2-adrenergic agonist we measured a mean FEV1 increase of 181.5 mL in our cohort, which was significantly elevated compared to a historical control group (ΔFEV1 = 118 mL). 28.7% of the patients showed an increase greater than 200 mL and 12% displayed a significant bronchodilation response (>200 mL ΔFEV1 and >12% FEV1 increase). Interestingly, no significant difference in bronchial dilation effect was observed when comparing patients hospitalized and those non-hospitalized during the course of their SARS-CoV-2 infection. Conclusion: Our data provides evidence for increased prevalence of obstructive ventilatory defects and increased bronchodilator responsiveness in patients with persisting symptoms after COVID-19. Depending on the extent of this complication, post-COVID patients may benefit from an adapted ß2-inhalation therapy including subsequent reevaluation.

Abstract:Objective: to evaluate the functional capacity of individuals with persistent symptoms after severe COVID-19 infection compared to control individuals after 26 months. Methods and results: This was a cross-sectional study with 17 adults who recovered from severe COVID-19 (with long COVID), and 17 adults in a paired control group (no long COVID) regarding functional capacity by 6-minute walk test (6MWT) associated with gas analysis. All assessments occurred between 26±9 months after acute disease. Dyspnea was still reported by 100% of the subjects (mMRC ≥2). The patients were matched by age (severe COVID-19: 46±8 vs control group: 45±8years, p=0.59) and weight (severe COVID-19: 78 ±20 vs control group: 83±19 years, p=0.59). The severe COVID-19 (long COVID group) group showed a lower peak oxygen uptake (VO2 peak) (11±05 vs. control group 19±09 mlO2.kg-1.min-1, p=0.005), reduced peak ventilatory (VE) (40±16 vs. control group 22±08 L/s, p=0.002) and reduced distance on 6MWT (416±94m vs. control group 523±95m, p=0.002). Furthermore, we analyzed ventilatory and metabolic behavior during 6MWT, patients with severe COVID-19, which showed a marked reduction in VO2 peak (mlO2.kg-1.min-1), VE (L/s) and heart rate (bpm) (Figure 1). Conclusion: After 26 months of hospital discharge from severe COVID-19, aerobic capacity, ventilatory, and metabolic function remain markedly reduced.

Abstract:Background In addition to deconditioning, persistent low-grade inflammation following acute SARS-CoV-2 infection may contribute to systemic problems, which supports the need for further assessment of cardiopulmonary conditioning. In fact, the impairment of the respiratory system in the acute phase of COVID-19 has the potential to significantly impact functional capacity in patients with post-COVID-19 syndrome (PCS), with dynamic hyperinflation (DH) and reduced ventilatory reserve (RV). Objective To investigate the dynamic ventilatory responses and their influence on the functional capacity to exercise in these patients. Methods Between March and October 2022, a cross-sectional study was carried out with 16 patients with PCS aged ≥18 years attended at the Piquet Carneiro Polyclinica, at the State University of Rio de Janeiro. Patients with a history of COVID-19 pneumonia with persistence of respiratory symptoms after 3 months of the acute phase were included. Patients without a previous diagnosis of COVID-19 confirmed by RT-PCR (reverse-transcription polymerase chain reaction) and those who failed to perform the protocol tests were excluded. Patients underwent impulse oscillometry (IOS), spirometry, 6-minute walk test (6MWT) with Spiropalm®-6MWT, and cardiopulmonary exercise test (CPET). A ≥100 ml decrease in inspiratory capacity (IC) during exercise was defined as HD. Ventilatory reserve (VR) indicates how close minute ventilation (VE) approaches maximum voluntary ventilation (MVV) during a given activity and was calculated as the difference between MVV and VEpeak ([MVV-VEpeak]/MVV); VR <30% was considered to be ventilatory limitation on exertion. Results Median age and time since diagnosis of COVID-19 were 57 (50–59) years and 98 (93–106) days, respectively. Regarding spirometry, 12.5% and 50% of the participants had an abnormal spirometry and an altered IOS, respectively, and the difference in resistance between 4 Hz and 20 Hz (R4-R20) was detected in 31.2% of the participants. cases. Regarding cardiopulmonary performance during exercise, the median distance in the 6MWT was 83 (78–97) % of predicted, with HD and VR <30% observed in 62.5% and 12.5% of participants, respectively. In CPET, the median peak oxygen consumption (VO2peak) was 19 (14–37) ml/kg/min. There was a significant correlation of the distance covered in the 6MWT with both R4-R20 (rs = -0.499, P = 0.039) and VO2peak (rs = 0.628, P = 0.009). Conclusion Our findings suggest that HD and, to a lesser extent, low VR are contributors to poor exercise performance that is associated with peripheral airway disease. Implications Based on these results, we obtained precise ventilatory and metabolic measurements, which we can consider as an important factor for more assertive exercise prescription during the rehabilitation of these patients. Moreover, these results are promising if we consider that they were obtained with simple, cheap, and portable ventilatory and metabolic measurement systems, easily applicable in real-world environments.

Abstract:Aim: To evaluate lung ventilation in non-hospitalised patients with long COVID (NHLC) and dyspnoea, patients with NHLC without dyspnoea and controls. Methods: NHLC patients and control subjects (fully recovered from COVID-19) were recruited as part of the EXPLAIN study. Subjects with smoking history >10 years, previous respiratory illness or abnormal CT imaging were excluded. 129 Xe ventilation MR imaging was acquired at 1.5T. Ventilation defect percentage (VDP), low ventilation percentage, normal ventilation percentage, hyper-ventilated percentage and ventilation heterogeneity index were calculated. Groupwise statistical tests (Kruskal-Wallis or Mann-Whitney U tests) were performed using SPSS. Results: 57 patients with NHLC and dyspnoea (median 43 years old, 68% female) 17 patients with NHLC without dyspnoea (46 years, 35% female) and 21 controls (39 years, 71% female) had successful ventilation imaging. No significant differences in any ventilation metrics were found between groups (p>0.6 in all cases). 12/57 (21%) patients with long COVID and dyspnoea had abnormal VDP (VDP>2%), 1/17 (6%) subjects without dyspnoea and 1/21 (5%) control subjects had VDP>2% (Figure 1). Most patients had normal spirometry (FEV1 Z<-1.64: 2/69, FVC Z<-1.64: 1/69, FEV1/FVC Z<-1.64: 6/69). Conclusions: 20% of NHLC patients with dyspnoea had abnormal lung ventilation despite normal PFTs.
