Abstract:BACKGROUND: The nursing shortage and its impact on patient care are well-documented global issues. Patients living with cancer as a chronic illness have many psychosocial problems and often lack adequate support as a result of ineffective nurse-patient communication. A review of the literature on factors influencing the delivery of psychosocial care to cancer patients indicates that the delivery of psychosocial care in routine cancer nursing within a biomedical healthcare system has not been widely explored.OBJECTIVE: To explore patients' perceptions of their experiences with nurse-patient communication in an oncological clinical environment.METHOD: A focused ethnographic study was undertaken in two oncology wards of a hospital in Hong Kong. Data were collected through observations of the ward environment, the activities and instances of nurse-patient communication, semi-structured interviews with patients, and a review of nursing documents.RESULTS: Two main themes were identified: 1. Nurses' workload and the environment and 2. Nurse-patient partnership and role expectations. Within these two themes were related subthemes on: Sympathy for the busy nurses; Prioritizing calls to the nurses; Partnership through relationship; Nurses' role in psychosocial care; and Reduction of psychosocial concerns through physical care.CONCLUSIONS: Many cancer patients do not expect to receive psychosocial care in the form of emotional talks or counseling from busy nurses, but appreciate the attention paid by nurses to their physiological and physical needs. Nurse-patient partnerships in cancer care may reduce the potential workload of nurses. The psychosocial needs of cancer patients could be optimized by providing good physical care through effective communication within a time-constrained oncology setting.

Abstract:Purpose: Cancer patients often experience pain, which can greatly reduce their quality of life. It affects their emotions, cognitive function, and daily interactions. Healthcare providers need to understand the obstacles to pain management to create helpful programs for patients and families. This study focuses on Iranian nurses' views on pain management barriers in cancer patients. Methods: The study utilized a qualitative content analysis method with a purposive sampling approach, involving 27 nurses. Participants were selected to ensure a wide range of perspectives and experiences by considering factors such as gender, age, work experience, education levels, and positions until data saturation was achieved. Inclusion criteria specified a minimum of six months of oncology ward experience and a bachelor's degree or higher in nursing. Nurses with less than six months of oncology experience or lacking a nursing degree were excluded. Data collection was conducted through semi-structured interviews and analyzed using Lundman and Granheim's qualitative content analysis method. Results: One main theme, four main categories, and nine subcategories in the study reflected the nurses' experiences and viewpoints about barriers to pain management. These categories included the marginalization of complementary medicine, medical malpractice, inadequate organizational infrastructure, and personal barriers. Conclusion: The study demonstrated that the barriers to pain management in cancer patients were complex. To improve patients' comfort and quality of life, it is important to identify and address issues from different angles. It is crucial to train patients and healthcare providers in pain management and to address weaknesses in the healthcare system.
Preparedness, resilience and unmet needs of informal caregivers of advanced cancer patients in a Regional Mission Hospital in Kenya: Qualitative Study

Abstract:Background: Cancer is the third highest cause of death in Kenya. Eighty percent of cancer cases arrive at advanced stages, when there is nothing that can be done to cure them, and palliative care is the best alternative. Although the majority of end-of-life care in Kenya is provided at home, little is known about the caregivers' preparedness, resilience and continued unmet needs. The goal of this qualitative study was to explore caregivers' perceived preparedness, resilience and continued unmet needs in their caregiving role to patients with advanced stages of cancer. Methods: A purposive sampling method was used to identify and recruit twelve informal, home-based caregivers of patients with advanced cancer from Kijabe Palliative Clinic data base. Interviews were conducted in patients' homes. The data was analyzed using interpretive phenomenological analysis approach. Ethical considerations were observed. Participants were kept anonymous and confidentiality. Results: Competing tasks, lack of preparedness in handling end-of-life care for patients in advanced stages of cancer were the main concerns. Continued unmet needs and financial stresses, and vulnerability for female caregivers all contributed to increased caregiver burden in this study. Caregivers were however determined and resilient amidst challenges that faced them, they exhibited hope against hopelessness. Some caregivers were vulnerable and faced potential for abuse following anticipated loss of their family member exacerbated psychosocial stress and needs CONCLUSION: Informal caregivers had common unmet needs related to caring for their advanced cancer patients. Whilst family caregivers had huge caregiver burden, insurmountable practical challenges related to role overload and competing tasks, they remained resilient though unprepared in giving end of life care. Recommendations: Caregivers should also be examined, prepared, and supported during clinic reviews. More research is needed on the use of telephones for caregiver follow-up, the impact of introducing caregiver-targeted screening tools on caregiver quality of life and their impact on enhancing caregiver well-being in order to prepare & support them adequately for the caregiving role.

Abstract:Background: Palliative care, a crucial aspect of healthcare, faces challenges in psychosocial and emotional management among nurses. With an increasing need for palliative services globally, addressing the emotional well-being of nurses becomes pivotal. Aim: To explore nurses' psychosocial and emotional work experiences in a palliative care department. The focus is on understanding the challenges, identifying coping strategies, and assessing the impact on professional and private life when facing those experiences. Methods: A phenomenologic-hermeneutical study involving semi-structured interviews was conducted to comprehend the lived experiences of ten nurses working in a hospital's palliative care department in Spain. Hermeneutic analysis was employed to extract patterns and insights from their experiences. The COREQ checklist was used to report this study. Findings: Palliative care nurses perceived insufficient preparation in emotional management, grappling with complex family interactions and unique work dynamics. They highlighted the significance of self-protection strategies, experience, clinical sessions, and external resources. Limited training in emotional resilience and challenges posed by the COVID-19 pandemic emerged as organisational barriers. Nurses expressed a desire for enhanced training and resources. Conclusion: The study uncovered a deficiency in the emotional preparedness of palliative care nurses, impacting their professional and personal lives. Despite positive aspects, the emotional toll necessitates attention and intervention. Implications for nursing policy: Comprehensive emotional training for palliative care nurses, addressing barriers, providing resources, and acknowledging emotional labour is necessary. Reinstating paused training sessions and considering specific challenges like those posed by the pandemic are vital. Supporting nurses in their professional and private lives is crucial for sustaining quality care in palliative care departments.

Abstract:Objective: To examine factors that are associated with the apprehension levels of oncology nurses toward hospice care. Factors examined in this study included demographics, nursing experience, education levels, title and post, personal experiences, and attitudes toward end-of-life care.Methods: Questionnaires were provided to nurses (n=201) from three first-tier hospitals in China. A quantitative scale, Professional End-of-life Care Attitude Scale (PEAS), was used to assess personal and professional apprehension levels toward hospice care. The PEAS was translated to Chinese with terms adapted to the cultural environment in China. Statistical analyses were performed to examine the relationships between the apprehension levels and various factors.Results: The total PEAS scores exhibited internal consistency and reliability, with a Cronbach alpha=0.897 and Pearson's r=0.9030. Of the 201 nurses, 184 provided a valid response (91.5%). Education level was significantly correlated with personal (P<0.01) and professional apprehension levels (P<0.05). Higher apprehension level was found in nurses with less education.Conclusion: The PEAS quantitative survey is useful for evaluating apprehension levels of nurses toward hospice care. Nurses with more education experienced less anxiety when providing care for terminally ill patients. The findings suggested that education programs on hospice care could be strengthened to help nurses cope with negative attitudes toward end-of-life care.

Abstract:Background: Critical care nurses are expected to work with patients who present with a wide range of health problems, but may lack sufficient education, skill development, and resources needed for providing psychosocial cancer care. Objective: The purpose of this study was to gain a deeper understanding of nononcologic critical care nurses' experience working with patients and their families affected by cancer. Method: A phenomenological qualitative study using semistructured interviews was conducted with 15 nononcologic critical care nurses. Colaizzi's 7-step methodology was followed for data analysis and interpretation of findings. Audio recordings were transcribed verbatim, the narrative data were analyzed and coded, and themes were identified that were confirmed by participants. Results: Four themes emerged from the nurses' narratives: lack of professional preparation refers to the perceived lack of education, skill development, and resources; collaboration and teamwork pertain to the importance of communication and moral support; being present expresses their perceived role in providing compassionate care; and nurtured empathy reflects the impact of cancer care on the nurses. Discussion: The narratives provide insight into the experience and perceptions of the nononcologic critical care nurses caring for patients with cancer. Information from the themes and narratives can be used to inform the development of knowledge and skills for nononcologic nurses who provide care to patients with cancer.

Abstract:BACKGROUND: Caring for cancer patients can take a toll on the emotional health of oncology nurses, which may lead to compassion fatigue, resulting in decreased quality of nursing care, absenteeism and decreased retention of staff.AIM: The aim of this study was to describe compassion fatigue from the perspective of oncology nurses. This study is part of a larger mixed-methods action research study to develop an in-facility intervention to manage compassion fatigue in oncology nurses.SETTING: This study was conducted at Durban, KwaZulu-Natal, South Africa.METHODS: The research setting comprised one state hospital (with oncology clinics and wards), a private hospital (with oncology wards) and a hospice in Durban, KwaZulu-Natal, South Africa. Semi-structured individual interviews (guided by Figley's Compassion Fatigue Process, 2005) were conducted with eight participants. Data were analysed using manifest content analysis.RESULTS: Five categories emerged from the data, namely, emotional connection, emotional fatigue, emotional loss, blurring boundaries and acceptance.CONCLUSION: The findings revealed that oncology nurses are affected emotionally in caring for their patients, thus making them prone to compassion fatigue. Oncology nurses need to acknowledge compassion fatigue and be able to self-reflect on how they are managing (both positively and negatively) with the stressors encountered in the oncology wards or units.
Personal and workplace factors influencing the resilience of nurses caring for women with cervical cancer in a resource-constrained setting in Ghana

Abstract:Cervical cancer has been identified as the fourth most common form of malignancy affecting and killing women globally. Nurses caring for cervical cancer patients are exposed to emotional and psychological distress due to late presentation and the burden of care. Resilience has been identified as one of the effective ways of helping nurses to cope well with the stress of oncology nursing, but this remains undetermined in Ghana This study explored personal and workplace factors influencing the resilience of nurses caring for women diagnosed with advanced cervical cancer (stage III and IV) in a resource-constrained setting in Ghana Using a qualitative approach, we recruited twenty nurses and midwives who had worked for a year and above caring for advanced-stage cervical cancer patients at the national referral hospital in Ghana. We conducted in-depth interviews between July, 2022 to September, 2022 which were audio-taped with participants' consent. Transcription was done verbatim, and analysis conducted using thematic analysis approach with the aid of NVivo 10.0. The results revealed experience as a safety toolkit, inherent desire to help/care for the patient, emotional numbness and maintaining professional outlook as personal factors influencing resilience among the participants. Regarding the workplace factors influencing resilience, we identified the main theme of demands of caregiving for advanced cervical cancer patients with the following sub-themes; severity of cases managed, nature of care rendered, activities of care given, reshuffling, schedules and gender mirroring as an exacerbator of psychological suffering. Resilience among nurses and midwives caring for terminally ill cervical cancer patients is influenced by longer years of service, intrinsic motivation to work as a nurse, and the defense strategy of emotional numbness and professionalism at the individual level. Also, the huge demand of caregiving serves as a major workplace factor affecting the resilience of nurses and midwives. We recommend strategies such as regular ward conferences and in-service trainings aimed at enhancing job-experience, inherent desire to render care and professionalism be adopted in resource-constrained settings to improve nurses' resilience. In addition, political actors and management of hospitals must prioritize allocation of resources for advanced cervical cancer care with particular focus on providing more specialized nurses and midwives.

Abstract:OBJECTIVE: Although there has been an increasing emphasis on assisting nurses with providing quality hospice care to patients and family members, few studies have explored the challenges that oncology nurses face when delivering hospice care in the Chinese cultural context. The objective of this study was to elucidate the challenges for oncology nurses in providing hospice care for terminally ill cancer patients in mainland China.METHODS: A descriptive qualitative study with purposive sampling using audio-recorded fact-to-face interviews. A total of 13 hospice nurses from four hospitals in Beijing, mainland China, participated in this study. Data collection was from April to June 2019, and thematic analysis method was used to analyze the data.RESULTS: Challenges identified by hospice nurses in providing hospice care for terminally ill cancer patients included: (1) public misperception on hospice care, (2) lack of financial support, (3) fear of medical disputes and legal action, (4) shortage of human resources, (5) insufficient specialization and lack of "hierarchy" training on hospice care, (6) inexperience in communication skills, and (7) lack of self-care and stress management skills.CONCLUSIONS: It is imperative and critical for the government, health-care institutions, and hospice care providers to clearly understand the challenges that currently exist in providing hospice nursing. Joint efforts are needed to overcome those challenges, which might result in qualified hospice nurses and provide evidence for further development of hospice care in mainland China.

Abstract:Abstract Background Patients with advanced breast cancer require consistent help and support from family caregivers. These caregivers often endure financial burdens and psychological stress, with their experiences significantly influenced by sociocultural factors. This study aims to explore the experiences of family caregivers of advanced breast cancer patients in Punjab province, Pakistan. Methods Data was collected through in-depth interviews with fifteen family caregivers of advanced breast cancer patients in three major cities of Punjab, Pakistan. Caregivers, who had been in close contact with the patient for the last two years, were purposively sampled from five major hospitals. The data was analyzed using thematic analysis. Results The study revealed that the experiences of family caregivers are deeply rooted in the sociocultural context. Key themes identified include social responsibility and cultural reciprocity norms; limited awareness and mobility options for caregivers; financial responsibility and strain; impacts of beauty myths and shyness on caregiving attitudes and decisions; the stressful and emotional nature of caregiving; treatment perspectives influenced by social groups; challenges in consulting male physicians and associated stigma; the role of religious beliefs in caregiving; and stress management, with religion often being a coping mechanism. These factors can contribute to delayed treatment decisions for patients. Conclusions Family caregivers are crucial in facilitating timely treatment decisions for advanced breast cancer patients in the Pakistani context. To minimize treatment delays and alleviate caregiver stress, addressing sociocultural barriers in care-seeking is essential. A tailored approach, considering sociocultural and religious factors, is imperative for the management and early diagnosis of breast cancer, necessitating appropriate policymaking and implementation.

Abstract:For rapid deployment of multicast application, Application Layer Multicast (ALM) is an alternate choice of IP multicast, as it supports group communication independent of network layer support. Previous studies on ALM mainly focused on achieving high throughput for bandwidth intensive applications and unfairness of communication quality is still a serious issue in multi-source communication. In this paper, we present an autonomous distributed scheme to construct an ALM tree to improve fairness and overall communication quality among the participants in a multi-source multimedia communication. Extensive simulation shows that our proposed scheme increases the incoming bitrate of the lowest recipient among all the nodes, as well as improving fairness for all receivers.

Abstract:BACKGROUND: Cancer patients need not only well-planned treatment, but also comprehensive nursing care provided with compassion, competence, and conscience. Nursing presence is an essential part of the care process in all nursing interventions. AIM: This study aimed to identify the barriers to the nursing presence in oncology care units. MATERIALS AND METHODS: A qualitative content analysis study was carried out with the participation of 27 nurses who were chosen by purposive sampling. The data collection instruments were semi-structured interviews and observation. The interviews were recorded and transcribed, and then coded and analysed by the Graneheim and Lundman’s content analysis methodology. The criteria proposed by Guba and Lincoln were used to ensure the validity of the research. RESULTS: From the data analysis, the researchers were able to obtain a primary theme labelled “Rocky road ahead of nursing presence” and two subthemes labelled “Difficult and stressful work environment” and “Dysfunctional rules and regulations” with several subcategories including “exposure to violence”, “shortage of nursing staff”, “inattention to the needs of nurses”, “organizational unfairness”, “excessive paperwork”, and “need for detailed documentation”. CONCLUSION: There are numerous challenges ahead of achieving satisfactory nursing presence and quality care in the oncology care units. The findings highlight the key role of organisational conditions in the nursing presence and the dire need to pay further attention to the motivational factors.
Exploring the emotional labour of paediatric oncology nurses and its impact on their well-being: An integrative review

Abstract:Purpose: To explore emotional labour in paediatric oncology nurses along with the negative impact of emotional labour on these nurses. Methods: The MEDLINE, CINALH, ScienceDirect, and PubMed databases were searched for relevant evidence published from 2000 to 2023. Selected studies were evaluated for quality and synthesized into themes. The Preferred Reporting Items for Systematic reviews and Meta-Analyses literature search extension was followed. Results: Eighteen empirical studies were reviewed. Nurses acknowledged the importance of managing and displaying emotions through emotional labour to gain patients' and families' trust and provide care in a safe and comfortable environment. They described emotional labour in the forms of empathy and close relationships. The frequency and intensity of performing emotional labour vary among nurses based on different factors. Nurses reported feelings associated with burnout, compassion fatigue, secondary traumatic stress, and emotional dissonance. The three main coping behaviours were avoidance, changing practice and sharing feelings. Conclusion: Although using emotional labour facilitates high-quality care, it may negatively influence nurses' well-being without implementing effective supportive interventions. A considerable number of paediatric oncology nurses intended to leave the unit. The proposed solutions are to address this issue at the organizational level by providing training and support services to enhance nurses' competence in performing this high-level skill and minimize its negative impacts.

Abstract:We sought to determine the relative protective effects of an intracellular-like, calcium-bearing, crystalloid solution (ICS) and a calcium-free, extracellular-like solution (ECS). Both solutions were similar in concentrations of potassium ions (25 to 26 mEq/L), in pH (7.7 to 7.8), and in osmolarity (340 to 360 mOsm/L). Normothermia was used to obviate masking the true effects of the solutions with the independent effects of hypothermia. Fifty-seven dogs were placed on cardiopulmonary bypass and had global myocardial ischemia for one hour. Continuous infusions and bolus injections into the aortic root of control solutions (n = 24), ECS (n = 15), and ICS (n = 16) were used. Two-hour postischemic ventricular performance was superior for ICS in comparison with ECS or control solutions regardless of administration mode. The multidose mode gave improved results with control and ECS groups. The use of an ICS yields improved postischemic performance in normothermic dog hearts.

Abstract:Background The global population of long-term cancer survivors is increasing, thanks to advances in treatments and care. Healthcare systems are working to address the unique needs of these individuals. However, there remains a knowledge gap concerning nurses’ view on cancer survivorship care. Objective To identify nurses’ perspective of care for long-term cancer survivors and their families. Methods This qualitative descriptive study used 5 focus groups comprising 33 nurses from primary healthcare and specialized oncology care. Data analysis was conducted through thematic analysis, and the study received ethical approval. Results Long-term cancer survivors and their families often remained unrecognized as a distinct group within the healthcare system. Consequently, nurses provide what can be termed as “opportunistic care” during nurse-survivor encounters, addressing health needs beyond the purpose of the initial healthcare visit. This absence of a systematic or structured approach for this patient group has prompted nurses to seek the establishment of a comprehensive framework through survivorship care plans, thus ensuring a continuum of care for this specific population. Conclusion The lack of a structured approach to caring for long-term cancer survivors and their families, often invisible as a distinct population group, results in nurses providing care on an opportunistic basis. Implications for practice It is crucial to develop and implement survivorship care plans tailored to this population’s needs. Simultaneously, it is important to advance research in this area and establish an educational framework for nurses, enabling them to effectively address the care of long-term cancer survivors and their families.

Abstract:The purpose of this study was to describe pediatric oncology nurse managers’ (NMs) perspectives of palliative care/end-of-life (PC/EOL) communication. The study, guided by group-as-a-whole theory and empirical phenomenology, was part of a larger, multisite study aimed at understanding pediatric oncology nurses’ experiences of PC/EOL communication. Nurses were assigned to focus groups based on length or type of experience (i.e., nurses with 5 years’ work experience and NMs). Eleven NMs from three Midwestern pediatric hospitals with large oncology programs participated in one focus group. The participants’ mean years of experience was 15.8 in nursing and 12 in pediatric oncology; 90% had a BSN or higher degree; all had supervisory responsibilities. The authors identified 2,912 meaning statements, which were then analyzed using Colaizzi’s method. Findings include NMs’ overall experience of “Fostering a Caring Climate,” which includes three core themes: (1) Imprint of Initial Grief Experiences and Emotions; (2) Constant Vigilance: Assessing and Optimizing Family-Centered Care; and (3) Promoting a Competent, Thoughtful, and Caring Workforce. Findings indicate that pediatric oncology NMs draw on their own PC/EOL experiences and their nursing management knowledge to address the PC/EOL care learning needs of nursing staff and patient/family needs. NMs need additional resources to support nursing staff’s PC/EOL communication training, including specific training in undergraduate and graduate nursing programs and national and hospital-based training programs.

Abstract:Background This study examines the experiences of geriatric nurses in palliative care. It aims to understand how these experiences influence their well-being and the quality of care. Methods Conducted in the Alahsa region of Saudi Arabia, the study employs qualitative methods, utilizing in-depth interviews with 12 geriatric nurses. The analysis employs a thematic approach, enriched by iterative reflections within a multidisciplinary research team. Results The analysis reveals main themes: 1) the deep emotional connections between nurses and their patients; 2) the challenges faced, including compassion fatigue, high patient mortality, and communication hurdles; 3) the impact of these challenges on the quality of care, highlighting issues like diminished empathy; 4) the coping strategies used by nurses, such as self-care practices and continuous education. Conclusions The study concludes that coping strategies, including self-care and ongoing professional development, are vital for sustaining the nurses' well-being and ensuring the continued provision of high-quality care to Older Adults patients

Oncology Nurses' Identification with Their Responsibilities and Negative Experience in Caring for Dying Cancer Patients

Abstract:Objective:To explore oncology nurses'attitudes on caring for dying cancer patients.Methods:Totally 463 oncology nurses from 19 nationwide hospitals were investigated by a self-designed scale.The scale was comprised of two subscales to evaluate respondents'agreement with their responsibilities and negative experience in caring for dying cancer patients.There were three dimensions in each subscale,including symptom management,psychological counseling and family support.Results:①Generally oncology nurses agreed with their responsibilities in the three aspects of caring for dying patients.But lower scores were reported in the responsibility of helping patients to deal with dying and death.②Oncology nurses demonstrated high negative experience levels in providing care for dying patients.The nurses reported highest negative feelings on cancer patients'family support,then symptom management.They felt difficult to talk about death and dying with patients.③Statistically significant negative relationships were noted between responsibility agreement and negative experience in the aspect of symptom management and psychological counseling.Conclusion:Continuous education for oncology nurses in palliative care should be further emphasized to ensure more effective and professional care for end stage cancer patients.
