Abstract:Background: Arterial switch operation (ASO) has been optimal for children with transposition of great arteries (TGA) in either simple or complex form with an excellent survival rate. This operation was introduced late in China, but there has been a decreasing mortality in recent years. Optimizing the postoperative management has been essential to improve the survival rate after ASO. This study summarizes the experience in the management of the postoperative complications after ASO. Methods: Twenty-eight infants with TGA underwent ASO from January 2004 through December 2006. These patients aged 1-70 days (median, 6 days) had a body weight of 3.36±0.57 kg on average. Before operation, continuous intravenous infusion of prostaglanding E1 was routinely used to keep the ductus open with a SpO2 of 75%-90%. Ten patients required tracheal intubation and mechanical ventilation. Two patients underwent emergency ASO under general anesthesia and low-flow cardiopulmonary bypass or hypothermic circulatory arrest. All the 28 patients were further treated with modified ultrafiltration and delayed sternal closure after cessation of cardiopulmonary bypass. After ASO, they were on circulatory and respiratory support, with antibiotics, nutritional supplement in the ICU. Results: The patients had a cardiopulmonary bypass time of 167±32 minutes and an aortic crossclamping time of 101±24 minutes. Delayed sternal closure was carried out on 3.63±1.49 days after ASO. They had a mechanical ventilation of 5.89±3.02 days and an ICU stay of 10.12±3.25 days. There were 5 deaths after ASO with a mortality rate of 17.9%. Fifteen patients developed low cardiac output syndrome. In 12 patients presenting with cardiac arrhythmias, 9 had paroxysmal supraventricular tachycardia, 1 had frequent ventricular premature beats, 1 had ventricular tachycardia, and 1 had ventricular fibrillation. One patient suffered from pulmonary hypertensive crisis. Three patients with major bleeding and tamponade required emergency mediastinal exploration and finally survived. There were 6 patients with ventilatorassociated pneumonia, 6 with delayed incision healing, and 1 with chylothorax. Conclusions: The complications after ASO are common and complicated. Understanding of the physiological characteristics of infants and the pathophysiological changes, and optimizing postoperative treatment help to improve the survival rate after ASO.

Abstract:Objective: To explore the application value of percutaneous peripheral interventional therapy in pulmonary atresia with intact ventricular septal (PA-IVS). Methods: Retrospective case summary. The data was collected from 25 children who were hospitalized at the Children's Hospital,Zhejiang University School of Medicine from August 2019 to August 2022, had been diagnosed with PA-IVS by echocardiography, and underwent interventional treatment. The sex, age, weight, operation time, radiation exposure time, and radiation dose of the patients were collected. The patients were divided into the arterial duct stenting group and the non-stenting group. Preoperative tricuspid annular diameters and Z scores, right ventricular length diameters, and right ventricular/left ventricular length-diameter ratios were compared by paired t-tests. Right ventricular systolic pressure difference, oxygen saturation, lactic acid before and after the surgery were compared for 24 children who received percutaneous balloon pulmonary valvuloplasty. Right ventricular improvement in 25 children after operation was analyzed. The correlation between postoperative oxygen saturation and postoperative right ventricular systolic blood pressure difference, the degree of pulmonary valve opening and the Z value of tricuspid valve ring in the non-stenting group were analyzed. Results: A total of 25 patients with PA-IVS were enrolled in the study, of whom 19 were males and 6 females, with an age at surgery of 12 (6, 28) days and a weight of (3.7±0.5) kg. One of them underwent only stenting of the arterial duct; 20 children underwent only percutaneous pulmonary valve perforation and balloon angioplasty; 4 children underwent both procedures. The Z-value of the tricuspid ring was -1.5±1.2 in the group with arterial duct stenting, and -0.1±0.4 in the group without stenting (t=2.77, P=0.010). The tricuspid regurgitant flow rate 1 month after surgery was significantly lower than the preoperative ((3.4±0.6) vs. (4.8±0.9) m/s, t=6.62,P<0.001). In the 24 children with percutaneous pulmonary valve perforation and balloon angioplasty, the preoperative right ventricular systolic blood pressure was (110±32) mmHg, and the postoperative systolic blood pressure was (52±19) mmHg (1 mmHg=0.133 kPa) (F=59.55, P<0.001). The factors that may affect postoperative oxygen saturation in 20 cases of non-stenting group were analyzed. The results suggested that the pre and post-operative right ventricular systolic blood pressure differences (r=-0.11, P=0.649), and the pulmonary valve orifice opening (r=-0.31, P=0.201) and tricuspid annulus Z value (r=-0.18, P=0.452) at 1 month after the operation were not significantly correlated with the postoperative oxygen saturation. Conclusions: Interventional therapy can be used as the first choice for one-stage operation of PA-IVS. Percutaneous pulmonary valve perforation and balloon angioplasty are more suitable for children with well-developed right ventricles, tricuspid annulus, and pulmonary arteries. While the smaller the tricuspid annulus, the more dependent it is on the ductus arteriosus and thus patients are more suitable for arterial duct stenting.

Abstract:Background: Congenital aortic valve (AV) disease is viewed in 2% of the population and in 20 out of 1000 neonates are presented with bicuspid configuration. Treatment of the congenital AV diseases requires multiple interventions. The ultimate goal is to provide adequate close to physiological left ventricle outflow and minimization of regurgitation. The optimal operative management of AV disease in children and young adults remains controversial. AV neocuspidization with glutaraldehyde‐treated autologous pericardium may be a satisfactory alternative in pediatric cardiac surgery. Objectives: The data of the AVNeo procedure results enable a comparative analysis of various AV reconstructive approaches in the pediatric cohort. In this article, we present the comparative study of various techniques in pediatric AV surgery, including the immediate and midterm outcomes of AVNeo, commissurotomy with aortic leaflet augmentation using pericardial patch ("free style" technique), and Ross procedure. Study Design: We conducted a retrospective analysis of the early and midterm results of surgical AV disease treatment of 44 children in 5 cardiac surgery centers from 2014 to 2022. The patients were divided into 3 groups: group I: glutaraldehyde‐treated autologous pericardium AVNeo (n = 12), group II: "free style" technique (n = 10), and group III: Ross procedure (n = 22). We described the immediate and midterm outcomes. The study design was approved by the ethics committees of the participating centers. Results: A total of 44 children after surgical treatment of the AV disease in 5 cardiac surgery centers were included in the study and were grouped as group I: AVNeo (n = 12), group II: "free style" technique (n = 10), and group III: Ross procedure (n = 22). The median follow‐up period was 57 ± 23.8 months. In‐hospital mortality was 4.5% (n = 2). In the midterm follow‐up, the peak pressure gradient was 18.45 ± 4.63 mmHg for group I and 8.9 ± 1.6 mmHg for group III (p = 0.00001), respectively. The cumulative freedom from reoperations was 100%, 30%, and 95.2% for 40 months of follow‐up and 100%, 30%, and 83.4% for 70 months of follow‐up in I, II, and III groups, respectively. No cases of midterm mortality were detected in all groups. Conclusion: The AV neocuspidization with glutaraldehyde‐treated autologous pericardium has an enormous advantage in pediatric AV surgery, significantly surpassing the freedom from resurgery of repair procedures and having comparable results with the Ross procedure in the midterm follow‐up.

[Percutaneous Balloon Aortic Valvuloplasty for Congenital Valvular Aortic Stenosis in Children].

Abstract:OBJECTIVE To assess the effect of the balloon valvuloplasty for congenital valvular aortic stenosis (AS) in children. METHODS A total of twenty one children with AS accepted the treatment of percutaneous balloon aortic valvuloplasty (PBAV). One of them complicated with PDA underwent PDA closure by using Amplatzer occluder in the mean time. PBAV could not be accomplished in 3 cases because the catheter could not be advanced into left ventricle and PBAV was not performed in 2 cases due to the gradient pressures across aortic valves less than 50 mm Hg (1 mm Hg = 0.133 kPa). The procedure was completed in 16 cases. The ratios of balloon/valve were 0.98 +/- 0.04 (0.92 - 1.10). RESULTS 13 cases had more than 50% gradient reduction (81.25%), 2 had 40% - 50% gradient reduction. The follow up period ranged from 3 months to 5 years. The gradient pressures rose to more than 50 mm Hg after follow up in 3 cases and they underwent repeat balloon valvuloplasty procedure or were operated successfully. There was no moderate to severe aortic insufficiency (AI). CONCLUSION The result of balloon aortic valvuloplasty showed the significant hemodynamic improvement with relative safety in pediatric patients. PBAV provides another choice in comparison with surgery.
Ross procedure for surgical treatment of the congenital aortic valve disease

Abstract:Objective To review the clinical experience and result of Ross operation in surgical repair for the congenital aortic valve disease. Methods From March 1998 to July 2002, 16 patients (mean age 14.0± 9.9 years) underwent Ross procedure for congenital aortic valve disease. Among them, 13 patients were younger than 16 years. The diagnosis included bicuspid aortic valve and aortic stenosis in 9, simple aortic stenosis shaped like arched roof in 2, prolapse of aortic valve and aortic insufficiency in 5. Among them, one associated with ventricular septal defect and another with patent ductus arterious. All patients underwent Ross procedure under cardiopulmonary bypass. Results There were no operative death, all patients were discharged uneventfully. Follow-up from 1-48 months (mean 30±13 months), no late deaths, and no valve-related events were observed during the period. The cardiac function was in NYHA class Ⅰ for all patients. The result of echocardiography showed that the function of the aortic valve and the pulmonary homograft valve was well; the hemodynamic of the pulmonary autograft were normal with an average mean gradient of 2.1±0.8 mmHg, and one patients with trivial insufficiency. Serial measurement of the left ventricular outflow tract and aortic root showed that the diameters were increased with patients growing -up,the diameter of the autografts increased 4.0±2.1mm than that of postoperation immediately. The outcomes were satisfactory. Conclusions Ross operation is a safe, effective procedure. It can be used for kinds of congenital aortic valve disease with the advantage of real potential for growth, especially in children and young patients.

Abstract:Objective To report the clinical experience and results of Ross operation in surgical repair for aortic valve disease. Methods From March 1998 to October 1999, 6 patients with age ranged from 5 to 38 (mean ages 14±12) years underwent Ross operation for aortic valve disease. There were 3 male and 3 female patients. Among them, 5 were younger than 16 years and another patient is 38 years. The diagnosis included bicuspid aortic valve and aortic stenosis in 3 (one associated with ventricular septal defect), prolapse of aortic valve and aortic insufficiency in 2 and simple aortic stenosis in 1. All patients underwent Ross operation under cardiopulmonary bypass (CPB). Results There was no operative mortality. All patients were discharged without any complications. Follow up was from 2 21 months. The outcome was satisfactory. Conclusion Ross operation is a safe, effective procedure. It can be used for certain kinds of aortic valve diseases, especially in children and young patients.

Abstract:While the long-term outcome of surgical aortic valvotomy (SAV) appears to be better than that of balloon aortic valvuloplasty (BAV) as the primary procedure of aortic valve stenosis, the surgical strategies and techniques of treating aortic valve disease in children in other situations remain controversial.Valve repair should be first considered while replacement is still unavoidable in some cases, and new repair techniques developed by innovative surgeons are gradually becoming adopted.Some complex repair procedures such as cusp extension, leaflet replacement/reconstruction have provided satisfactory outcomes.The Ozaki technique replaces aortic valve leaflets with glutaraldehyde-treated autologous pericardium instead of replacing the valve entirely.Special instruments have been developed to make the Ozaki technique more reproducible and standardized.Neonates and infants undergoing aortic valve replacement (AVR) are a high-risk group, where repair should be the primary consideration rather than replacement.Several systematic reviews reveal that all currently available aortic valve substitutes such as pulmonary autograft, mechanical prosthesis, homograft and bioprosthesis are associated with suboptimal results in children, but pulmonary autograft appeared to be superior with high freedom from reintervention and better hemodynamic performance.The strategy for treatment of aortic valve disease should be specifically analyzed based on the brief of being beneficial for children.
The Ross-Konno Procedure With or Without Mitral Valve Surgery: A Systematic Review With Individual Data Pooling

Abstract:Background: The Ross-Konno procedure is a technically demanding surgical option to treat multilevel left ventricular outflow tract obstruction. Methods: A systematic review with pooled analyses was conducted according to PRISMA criteria on studies published between January 2000 and May 2022 that assessed outcomes following the Ross-Konno intervention in children. Individual patient data were extracted from published Kaplan-Meier curves using digitalization software. Overall survival and freedom from reintervention were assessed by time-to-event approaches. Determinants of one-year survival were investigated by meta-regression analyses. Results: Ten studies with a total population of 274 patients were included. The overall pooled early (≤30 days) survival rate was 86.9% (95% CI [87.6%-78.4%]). Five-year survival rates in patients without and with (N = 50 [18.2%] of 274 total patients) concomitant mitral valve surgery were 82.5% (95% CI [87.6%-77.4%]) versus 56.1% (95% CI [74.1%-38.1%]), hazard ratio 2.67, 95% CI (1.44-4.93), P < .0001. Five- and ten-year freedom from pulmonary autograft reoperation rates were 93.5% and 90.9%, respectively. Five- and ten-year freedom from right ventricular outflow tract reoperation rates were 74.3% and 57.3%, respectively. By meta-regression analysis, resection of endocardial fibroelastosis (N = 32 [11.7%] of 274 total patients) was associated with superior one-year survival (P = .027). Conclusion: The Ross-Konno procedure is associated with substantial early mortality and gradual attrition thereafter. Mortality is higher in patients with concomitant mitral valve surgery. Resection of endocardial fibroelastosis is associated with superior survival. Right ventricular outflow tract reinterventions are common.
Long-Term Autograft Dilation and Durability After the Ross Procedure are Similar Among Infants, Children, and Adolescents with Primary Aortic Stenosis

Abstract:Objective Autograft durability and remodeling are thought to be superior in younger pediatric patients after the Ross operation. We sought to delineate the fate of the autograft across the pediatric age spectrum in patients with primary aortic stenosis (AS). Methods We retrospectively reviewed patients aged ≤18 years with primary AS who underwent the Ross operation from 1993 to 2020. Patients were categorized by age. The primary endpoint was autograft dimensional change. Secondary endpoints were severe neo-aortic insufficiency (AI) and autograft reintervention. Results One hundred and nineteen patients underwent the Ross operation, including 37 (31.1%) in group I (<18 months), 24 (20.2%) in group II (18 months-8 years), and 58 (48.7%) in group III (8-18 years). All groups exhibited similar annular growth rates within the first 5 postoperative years, followed by a collective decrease in annulus growth rates from years 5-10. Group III experienced rapid sinus dilation in the first 5 years, followed by stabilization of the sinus z-score from years 5-10; groups I and II demonstrated stable sinus z-scores over 10 years. There were 4 (3.4%) early and 2 (1.7%) late mortalities at a median follow-up of 8.1 years [range: 0.01-26.3]. At 15 years, there was a similar incidence of severe neo-AI (0.0%±0.0% vs. 0.0%±0.0% vs. 3.9%±3.9%, p=0.52) and autograft reintervention (8.4%±6.0% vs. 0.0%±0.0% vs. 2.4%±2.4%, p=0.47) in all groups. Conclusions Age at the Ross operation for primary AS does not influence long-term autograft remodeling or durability. Other physiologic or technical factors are likely greater determinants of autograft fate.

The landscape of congenital heart disease treated with the Ross procedure

Abstract:Background: The Ross procedure has excellent outcomes in the pediatric population. Some series have reported age- and anatomy-dependent outcomes, but a comprehensive analysis stratified by these variables has not been reported to date. We sought to describe the landscape of congenital heart disease (CHD) treated with the Ross procedure and identify the patients best served by this operation. Methods: Over a total of 30 years, 317 pediatric patients underwent the Ross procedure. These patients were stratified into 4 age groups-neonates (age <31 days; n = 21), infants (age 31 days to 1 year; n = 40), children (age 1-12 years; n = 165), and adolescents (age 13-18 years; n = 91)-and 3 anatomic groups-isolated aortic valve (AV) disease (n = 221), Shone complex or multilevel left ventricular outflow tract (LVOT) obstruction (n = 61), and complex CHD (n = 35). The groups were compared across outcomes. Results: Neonates and infants had the highest rates of Shone syndrome (P < .001), complex CHD (P < .001), concomitant root enlargement (P < .001), and arch procedures (P < .001). Operative mortality and morbidity were 4.4% (n = 14/317) and 14% (n = 44/317), respectively. Both were higher in neonates and infants (P < .001 for both) but lower for patients with isolated AV disease (P < .001 for both). Ten-year survival and freedom from LVOT reintervention were 92% and 81%, respectively, and were significantly better in patients with isolated AV disease compared to those with complex CHD (P < .001 and P = .005, respectively). In neonates and infants with isolated AV disease, operative mortality was 4% (n = 1/23), morbidity was 9% (n = 2/23), 10-year survival was 85.6%, and 10-year freedom from LVOT reintervention was 88.4%. Conclusions: Among pediatric patients, those with isolated AV disease are best served with the Ross procedure, regardless of age. Complex CHD is associated with lower survival and increased risk of LVOT reintervention.
The Ross Operation in Young Patients—a Single-Center, Long-Term Follow-Up Study

Abstract:Background: The standard aortic valve replacement operations in young patients are bioprosthetic or mechanical aortic valve replacement. In this study, we present the long-term results of the Ross operation in young patients. Methods: The Ross operation with root replacement was performed on 795 patients between 1995 and 2020. The endpoints were overall survival, re-operation/reintervention rates on the autograft and homograft/RV-PA conduit (RV, right ventricle; PA, pulmonary artery), and the occurrence of serious adverse events. Results: 795 patients (75% male, 25% female; mean age 43 ± 14 years) underwent the Ross operation with root replacement. The 30-day mortality was 1% (8 patients) with a 95% confidence interval of [0.48; 1.9]. The follow-up was 96 % complete and covered 9540 patient-years, with a mean follow-up time of 12 ± 7 years. The survival rates at 5, 10, 15 and 20 years were 97% [96; 98], 96% [93; 96], 92% [89; 94] and 86% [83; 90]. The rate of autograft survival without reoperation was 94% [92; 96] at 10 years and 80% [76; 85] at 20 years. The rate of homograft/RV-PA conduit survival without reoperation was 95% [93; 97] at 10 years and 85% [81; 90] at 20 years. There were 5 cases of hemorrhage, 14 of thromboembolic complications or apoplexy, and 19 of endocarditis. Conclusion: The Ross operation with root replacement was associated with high survival rates over 25 years of follow-up. The rates of reoperation and reintervention, the morbidity and mortality, and the rate of endocarditis are low. The Ross operation is thus an effective surgical treatment option for young patients with aortic valve disease.

Abstract:As the primary treatment of aortic valve stenosis in children, surgical aortic valvotomy (SAV) and balloon aortic valvuloplasty (BAV) are widely used. With the similar early curative effect to BAV, SAV performs better in long-term follow-up. But the first choice for aortic valve stenosis is still controversial. These years, aortic valve repair is valued and different repair techniques have been reported and achieved good results. The complex repair techniques, as leaflet extension, leaflet replacement or valve reconstruction, are effective in treating diseased valves that cannot be repaired before. Ozaki technology, using glutaraldehyde-treated autologous pericardium to replace the aortic valve, makes the surgery more standardized and reproducible by developing special instruments. Pulmonary autograft (Ross procedure) is considered to be superior to other valve replacement technologies due to the good hemodynamic performance and lower reoperation rate. Mechanical valve is still used in some cases, but the quality of life is low due to the lifelong anticoagulation, and it has a high reintervention rate. The biological valve has been rarely used in children because of its high rate of early calcification and structural failure.
A Midterm Follow-Up Study of the Application of a Confluent Aortic Valve Neocuspidization Technique with Pericardium in Children.

Abstract:Background: The treatment of aortic valve diseases in children remains a great challenge. We aim to report outcomes and midterm follow-up data of our confluent neocuspidization technique with pericardium for aortic valve replacement (AVR) in children.Methods: A retrospective analysis was performed on all 20 children who underwent the confluent neocuspidization technique with pericardium at Children's Hospital of Fudan University from March 2017 to May 2022. Outcome measures included echocardiographic measurements, surgical intervention, and mortality.Results: A total of 20 patients (17 males vs. 3 females), with a median age of 7.5 years [min-max, 0.3-12 years; interquartile range (IQR), 4.4-9.7 years], a median body weight of 24.0 kg (min-max, 6.0-52.3 kg; IQR, 15.6-31.0 kg), and median aortic valve annulus size before surgery of 19.0 mm (min-max, 11.0-25.0 mm; IQR, 17.1-21.5 mm), underwent the neocuspidization technique with pericardium (17 autologous pericardia and 3 bovine patch). With 50% of bicuspid aortic valve and 50% of tricuspid, they were respectively diagnosed as aortic stenosis (AS) (7/20, 35%), aortic regurgitation (AR) (8/20, 40%) and mixed AS and AR (AS & AR) (5/20, 25%). The median postoperative follow-up time was 19 months (min-max, 5-61 months; IQR, 16.3-35 months). The peak pressure gradient across the aortic valve decreased from 81.0 +/- 37.0 mmHg in AS group and AS & AR group before surgery to 25.9 +/- 15.8 mmHg within 24 hours after surgery (P<0.001) and was mostly around 25 mmHg during follow-up. All patients presented mild or less than mild regurgitation within 24 hours after surgery. There were no hospital mortalities. Three patients needed reintervention during follow-up. There was one late death related to mitral valve stenosis.Conclusions: Though the confluent neocuspidization technique with pericardium provided immediate relief of significant AS or regurgitation, the midterm outcome was suboptimal. More research is needed to find the optimal material for AVR.

Abstract:The Ross procedure is gaining recognition as a significant option for aortic valve replacement (AVR), and is particularly beneficial in specific patient groups. Although categorized as a class IIb recommendation in the 2020 American College of Cardiology (ACC)/American Heart Association (AHA), and the European Society of Cardiology (ESC) management guidelines on valvular heart disease, recent studies bolster its credibility. Research, including a propensity-matched study, underlines the Ross procedure's association with enhanced long-term survival and reduced adverse valve-related events compared to other AVR types. This positions the Ross procedure as a primary option for AVR in young and middle-aged adults within specialized centers, and potentially the only choice for children and infants requiring AVR. This review meticulously examines the Ross procedure, covering historical perspectives, surgical techniques, imaging, and outcomes, including hemodynamic performance and quality of life, especially focusing on pediatric and young adult patients. It explores contemporary techniques and innovations like minimally invasive approaches and tissue engineering, underscoring ongoing research and future directions. A summarization of comparative studies and meta-analyses reiterates the Ross procedure's superior long-term outcomes, valve durability, and preservation of the left ventricular function, accentuating the crucial role of patient selection and risk stratification, and pinpointing areas for future research.

Abstract:The Ross procedure is the best operation to treat aortic stenosis (AS) in young and middle-aged adults. However, its role in non-repairable aortic regurgitation (AR) remains debated since many historical series have reported an increased risk of pulmonary autograft dilatation and subsequent need for reintervention in these patients. Some have attributed these findings to an unrecognized and poorly characterized inherited genetic defect that prevents adaptive remodelling of the pulmonary autograft. Herein, we review the contemporary evidence surrounding the use of the Ross procedure in young adults with AR and put forth the argument that with proper technical refinements, the Ross procedure may still be the best operation to treat these patients. We believe that by tailoring the operation to the patient's anatomy and ensuring strict postoperative blood pressure control, one can achieve excellent results with the Ross procedure, including in this challenging patient population.

Ross Confers More Favorable Left Ventricular Remodeling Compared With Mechanical Aortic Valve Replacement

Abstract:Background: Aortic valve disease results in left ventricular (LV) dilation and/or hypertrophy. Valve intervention may improve, but not normalize flow dynamics. We hypothesized that LV remodeling would be more favorable following the Ross procedure versus mechanical aortic valve replacement (mAVR). Methods: Patients who were 18 to 50 years of age and underwent Ross or mAVR from 2000 to 2016 at a single institution were retrospectively reviewed. Propensity score matching was performed and yielded 27 well-matched pairs. Demographics and echocardiographic variables of LV morphology and wall thickness were collected. Those with > mild residual valve disease were excluded. Primary endpoints included LV morphology. T test and Fisher exact test analysis were used for statistical comparison. Results: Average age at operation (Ross 35.3 ± 10.2 vs mAVR 37.3 ± 8.9 years) did not differ. Indication for operation was similar between groups. Preoperative echocardiographic variables did not differ. At average follow-up duration (Ross 7.9 ± 2.4 vs mAVR 7.3 ± 2.4 years), wall thickness was significantly smaller for Ross compared with mAVR (P = .00715). Only 4/27 (15%) of mAVR patients had normalized LV parameters compared with 16/27 (59%) of Ross patients (P = .000813). Residual hypertrophy was the most common long-term abnormality for mAVR. Conclusion: Following aortic valve replacement with the Ross procedure or mechanical aortic valve prosthesis, the Ross conferred more favorable LV remodeling compared with mAVR. Future directions include analyzing longer follow-up to determine if patterns persist and the impact on cardiac morbidity and mortality.

Abstract:Objective:To retrospectively explore clinical features, surgical treatments, mid/long-term survival rates and risk factors for reoperation in children with congenital atrioventricular septal defect(AVSD).Methods:From January 2009 to December 2019, 125 hospitalized children(46 boys, 79 girls)were operated.There were complete atrioventricular septal defect(C-AVSD, n=72), partial atrioventricular septal defect(P-AVSD, n=34)and intermediate atrioventricular septal defect(I-AVSD, n=19). The median operative age was 14 months.According to anatomical classifications, atrial defect repair, atrioventricular valve reshaping and modified mono/double-patching were performed.Demographic data, in-hospital mortality and reoperation rate were recorded.Univariate/multivariate correlation and Cox regression analyses were performed.Results:Atrioventricular septal defect repair and atrioventricular valvuloplasty were successfully performed under cardiopulmonary bypass with cardiopulmonary bypass time(102±35)min, aortic arrested time(67±26)min, ventilator assist time(35±18)hours and postoperative hospitalization stay(13.8±4.5)days.Postoperative complications included low cardiac output(n=8), severe left atrioventricular valve regurgitation(n=3), left atrioventricular valve regurgitation with stenosis(n=1), residual atrial septal shunt(n=1), left ventricular outflow tract stenosis(n=1), arrhythmia(n=1), intracranial hemorrhage(n=1), lung infection(n=7, including 1 case of severe pneumonia)and sepsis(n=2). Overall postoperative mortality rate was 6.4%(8/125)and mortality rate 4.0%(2/46)in the past 5 years.All death cases belonged to C-AVSD and death cause was low cardiac output syndrome(n=3), sepsis(n=2), arrhythmia(n=1), intracranial hemorrhage(n=1)and severe pneumonia(n=1). The reoperative rate was 6.4%(8/125), including early operation(n=6, during hospitalization or within 1 month post-discharge), including left atrioventricular regurgitation(n=3), left ventricular outflow tract obstruction(n=1)and arrhythmia(n=1), intracranial hemorrhage(n=1); late operation for severe left atrioventricular valve regurgitation(n=2, 1 month post-discharge)and 1 case with concurrent left atrioventricular valve stenosis.Conclusions:The mid/long-term surgical outcome of congenital atrioventricular septal defect is excellent.And left ventricular dysplasia is a risk factor for reoperation and long-term survival.
