Abstract:Polycystic ovary syndrome (PCOS) is the most common form of chronic female disease associated with androgen excess; perhaps occurring in 5-10% of the reproductive age group in women. PCOS is viewed as a heterogeneous disorder of multifactorial etiology. It is also associated with increased metabolic and cardiovascular risk factors. Objectives: The purpose of this study is to evaluate the serum level of hormones TSH, T3 and T4 in women with PCOS and to investigate the effect of metformin on hormones TSH, T3, and T4 levels in subjects with PCOS. Patients and methods: The present study was conducted at Kirkuk General Hospital and Azadi General Hospital from March 2023 to January 2024.One hundred forty-one women with PCOS were selected as the PCOS group, while 120 healthy women matched for age with the PCOS patients were selected as the control group. The body mass index is determined before and after therapy. They take metformin 850 mg twice daily for three months and provide fasting blood samples on the second day of menstruation before and after treatment. The data collection done through a designed closed and open-ended questionnaire, by using direct interviewing, and Ultrasound examination, Data were analyzed using SPSS for Windows 7. Results: Treatment resulted in a significant decrease in in body mass index at p-values 0.05. This study reveals that the thyroid hormones TSH, T3 and T4 were highly significantly increased (P 0.05) in TSH level in treated group with metformin compared with pre-treatment. Conclusion: Metformin can induce changes of TSH levels in patients with polycystic ovary syndrome.

Abstract:Objective: This study aimed at elucidating the therapeutic effectiveness of metformin in polycystic ovary syndrome (PCOS) patients with normal clinical indices of insulin sensitivity. Methods: This study was a retrospective cohort study. Based on their homeostasis model of assessment of insulin resistance (HOMA-IR) results, PCOS patients (n = 113) were allocated to either the IR Group (IR, HOMA-IR >2.69) or the non-IR Group (NIR, HOMA-IR ≤2.69). Metformin was administered to all patients, and their medical history, menstrual status, and endocrine and metabolic characteristics were obtained before and after treatment. The one-way analysis of variance was used to determine the differences between the IR and NIR groups. The paired t-test was used to determine the differences between pre- and posttreatment results. Results: Analyses of baseline characteristics showed that the incidence of IR and glycolipid metabolism is higher within elder PCOS patients. Metformin improved menstrual cycle and ovulation rate in NIR patients, similar to those in IR patients. In the NIR group, metformin decreased luteinizing hormone (LH) levels, free androgen index (FAI) levels, body mass index, and waist circumference, but had no obvious effect on glucose, insulin, and lipid profiles. Whereas in the IR group, metformin not only decreased LH and FAI levels, but also improved glycolipid metabolism. Conclusions: The effect of metformin on PCOS patients with normal insulin sensitivity index suggests that the mechanism of menstrual improvement may not be related to IR. In addition, metformin can improve glycolipid metabolism. Therefore, metformin is suitable for long-term treatment of PCOS and should be more widely used.

Abstract:Metformin is one of the most widely used antidiabetic drugs and is also effectively used in treating women with polycystic ovary syndrome (PCOS). Our global objective is to separate women with PCOS into subtypes by their response to metformin. This pilot study aims to determine which free fatty acids and amino acids metabolites predict response to metformin as categorized by changes in insulin sensitivity and testosterone concentrations after three months of metformin therapy. We recruited 42 participants with PCOS aged between 18-50 years. Participants were randomly categorized into metformin (n= 29; 32.41 ± 7.4 years) and placebo groups (n=13; 32.15 ± 5.65 years). The Metformin group was challenged with three months of metformin titrated to at least 1500 mg daily. Body weights decreased significantly in the metformin treatment group from 98.55 ± 22.76 to 97.27 ± 23.14 (p-value=0.01) but did not change in the placebo group. Fasting plasma glucose decreased only in the metformin group from 89.25 ± 9.12 to 86.37 ± 9.53. Testosterone levels decreased in the metformin-treated group after 12 weeks from 29.0 (18.5 - 35.5) to 23.0 (15.5 - 32.5), but no significant change was observed in the placebo group from 28.0 (20.0 - 43.0) to 27.0 (21.5 -49.0). However, we did not observe a statistically significant change in insulin sensitivity values in PCOS and placebo groups. Linear regression analysis was performed to check the predictive power of those associated metabolites with delta values of testosterone and insulin sensitivity. Our results showed that changes in insulin sensitivity and testosterone levels after metformin treatment could be significantly predicted by using concentrations of metabolites (Fig.1). [Clinical impact of combination antiretroviral therapy].

Abstract:After the advent of HIV protease inhibitor in 1995, the clinical feature of HIV disease has been dramatically changed. Combination antiretroviral therapy containing protease inhibitor can suppress the viral load under the detection limit and recover the immune status in HIV-infected individuals. However, it still seems impossible to eradicate the viruses from infected individuals. They will have to continue the combination therapy for life with good compliance because incomplete viral suppression induces the resistant viruses. The clinical feature of opportunistic infections in HIV-infected patients has also been changed. In some cases, prophylaxes for Pneumocystis carinii pneumonia and Mycobacterium avium complex infections are not necessary after the introduction of combination antiretroviral therapy.
METFORMIN EFFECTS ON NEUREGULIN-1 IN POLYCYSTIC OVARIAN WOMEN

Abstract:The study aims to assess the levels of Neuregulin 1 (NRG1) in patients with Polycystic Ovary Syndrome (PCOS) and to determine the impact of metformin treatment on their serum NRG1 levels. PCOS is a common endocrine disorder in women of reproductive age, while NRG1 is linked to regulating inflammation and ovulation. The study was conducted on sixty women with PCOS and thirty healthy women as the control group. Thirty patients completed the follow-up study and continued on metformin treatment for three months. The study was a cross-sectional study done in Salah Al-Deen general hospital/gynaecology and obstetrics department in Tikrit City from November 2022 to January 2023. The participants were diagnosed with PCOs based on the Rotterdam criteria, and their BMI and insulin resistance were measured before and after therapy. Fasting serum NRG1 was also measured. The study found that women with PCOS had increased levels of fasting blood glucose, insulin, and insulin resistance, as well as increased levels of NRG1. However, treatment with metformin for three months resulted in a significant decrease in body mass index, blood glucose, insulin, and insulin resistance. NRG1 level decreased significantly after 3 months of treatment with 850 mg per day with metformin in women with PCOS.
The Influence of OCT3 and MATE2 Genetic Polymorphisms in Poor Response to Metformin in Type 2 Diabetes Mellitus

Abstract:Background: The response of patients with Type 2 diabetes mellitus (T2DM) to metformin may be a variation because of genetic differences in solute carrier (SLC) transporter proteins and other effect factors, which have an important effect on how metformin is processed in the body and its efficiency for glycaemic control. Aim: This study was conducted to investigate the impact of certain genetic variants of the organic cation transporter genes OCT3 (SLC22A3 rs12194182 and rs8187722) and MATE2 (SLC47A2 rs12943590) and their association with glycaemic parameters in patients with T2DM who respond poorly to metformin. Patients and methods: This cross-sectional study involved 150 Iraqi cases with T2DM who were prescribed a daily dose of (1000 mg/day) metformin for a minimum of 3 months. Various parameters included are as follows: demographic data, glycaemic parameters and three SNPs: rs12943590 variant of SLC47A2, rs12194182 and rs8187722 variant of SLC22A3 using the standard PCR-sequencing technique. Results: Thirty-nine patients (26.17%) were responders, whereas 111 patients (73.82%) could not respond to metformin treatment. Upon analysing the genotypes of the rs12943590 variants of SLC47A2, rs12194182 and rs8187722 SNPs of SLC22A3, the present findings revealed a nonsignificant association of genetic variations in all SNPs with metformin response. SLC47A2 (rs12943590) showed nonsignificant associations of the GG, AA and AG genotyping; SLC22A3 (rs12194182) showed nonsignificant associations of the TT, TC and CC genotyping; and SLC22A3 (rs8187722) showed nonsignificant associations of the AA, CC and AC genotyping between two groups. Conclusion: Variations in genes SLC22A3 and SLC47A2 did not have a significant role in the response of patients with T2DM to metformin (1000 mg/day).
Association Between Single Nucleotide Polymorphisms (rs1484215 and rs6495096) in CYP11A1 Gene in Iranian Women with Polycystic Ovary Syndrome

Abstract:Background: Genetic factors are significantly have important role in the etiology of the polycystic ovary syndrome (PCOS). This study examined the possible relation of rs1484215 and rs6495096 polymorphisms of CYP11A1 gene in Iranian women with PCOS. Methods: The population of the case-control research included 100 women presenting with PCOS and 100 women as a control group who were referred to Infertility Center in Qom, Iran. The genotypes of rs1484215 and rs6495096 polymorphisms in CYP11A1 gene were detected with the tetra-ARMS PCR method. The independent segregation of alleles was tested for the Hardy-Weinberg equilibrium (HWE). Differences in quantitative traits were assessed between each group with a single PCOS feature and control group using Mann-Whitney U test for categorical variables and student's t-test for continuous variables. Statistical analysis of allele and genotype frequencies between women with PCOS and control was performed using the chi-square test. Significance level was defined as p<0.05. Results: There was a significant association of C with G alleles in rs6495096 polymorphism and susceptibility to PCOS (p=0.001), but no significant relation was found between C and T alleles in rs1484215 polymorphism and susceptibility to PCOS. Also, GG genotype of rs6495096 was significantly associated with the waist-to-hip ratio (WHR) and infertility duration compared to CG and CC (p<0.01). However, rs1484215 showed no association with these variables. Conclusion: The results from the research indicated that rs6495096 polymorphism of CYP11A1 gene is related to the PCOS risk in Iranians women.
Comparison of Various Therapeutic Approaches to Manage Infertility in Polycystic Ovarian Syndrome

Abstract:Objective: To compare the efficacies of common therapeutic regimens and their combinations, used in polycystic ovarian syndrome (PCOS) to improve fertility in reproductive-age women. Study design: A descriptive study. Place and Duration of the Study: Department of Obstetric Gynaecologist, Medicare Cardiac and General Hospital, Karachi, Pakistan, from November 2022 to July 2023. Methodology: Out of 300 patients with the symptoms of menstrual irregularities and infertility, 152 were diagnosed as PCOS patients based on the ultrasound and hormonal assays and selected for study purpose. They were divided according to their therapeutic regimen into four treatment groups, treated by different therapeutic agents. Group A received metformin 500 mg/day (n = 38); Group B received metformin + myo-inositol 1g (n = 49); Group C received metformin + letrozole 2.5 mg (n = 36), and Group D received metformin + letrozole + myo-inositol (n = 29), orally for three months. All continuous variables, such as body mass index (BMI), FSH, LH, FT4, and FSI were analysed by applying t-test to all therapeutic groups, keeping p ≤0.05 as the level of significance. Results: HCG-positive was found as 86% (n = 33) in Group A, 63% (n = 31) in Group B, 52% (n = 19) in Group C, and 27% (n = 08) in Group D. There were statistically significant (p <0.001) changes in BMI, FSH, LH, FT4, and FSI as well. Metformin alone and metformin plus myo-inositol came out to be more effective than other regimens. Conclusion: Metformin alone and myo-inositol plus metformin are effective therapeutic options in PCOS-induced infertility problems. Abstract:Polycystic ovary syndrome (PCOS) is a reproductive disorder with multiple etiologies, mainly characterized by the excess production of androgens. It is equally contributed to by genes and environment. The CYP11A1 gene is imperative for steroidogenesis, so any dysregulation or mutation in this gene can lead to PCOS pathogenesis. Therefore, nucleotide diversity in this gene can be helpful in spotting the likelihood of developing PCOS. The present study was initiated to investigate the effect of single nucleotide polymorphisms in human CYP11A1 gene on different attributes of encoded mutated proteins, i.e., sub-cellular localization, ontology, half-life, isoelectric point, instability index, aliphatic index, extinction coefficient, 3-D and 2-D structures, and transmembrane topology. For this purpose, initially coding sequence (CDS) and single nucleotide polymorphisms (SNPs) were retrieved for the desired gene from Ensembl followed by translation of CDS using EXPASY tool. The protein sequence obtained was subjected to different tools including CELLO2GO, ProtParam, PHYRE2, I-Mutant, SIFT, and PolyPhen. It was found that out of seventy-eight SNPs analyzed in this project, seventeen mutations, i.e., rs750026801 in exon 1, rs776056840, rs779154292 and rs1217014229 in exon 2, rs549043326 in exon 3, rs755186597 in exon 4, rs1224774813, rs757299093 and rs1555425667 in exon 5, rs1454328072 in exon 7, rs762412759 and rs755975808 in exon 8, and rs754610565, rs779413653, rs765916701, rs1368450780, and rs747901197 in exon 9 considerably altered the structure, sub-cellular localization, and physicochemical characteristics of mutated proteins. Among the fifty-nine missense SNPs documented in present study, fifty-five and fifty-three were found to be deleterious according to SIFT and PolyPhen tools, respectively. Forty-nine missense mutations were analyzed to have a decreasing effect on the stability of mutant proteins. Hence, these genetic variants can serve as potential biomarkers in human females for determining the probability of being predisposed to PCOS.

Abstract:INTRODUCTION: Polycystic ovarian syndrome (PCOS) presents as an extremely heterogeneous condition that includes chronic anovulation, hyperandrogenism, hyperlipidemia, and hyperinsulinemia along with enlarged polycystic ovaries. Insulin resistance is thought to be a pathogenic factor in women having PCOS along with increased incidence of metabolic disturbances.MATERIALS AND METHODS: After obtaining ethical clearance, the girls of various colleges in Patiala were verbally sensitized to the symptoms of PCOS, and then given semistructured proformas subsequent to written informed consent. The students suspected on self-evaluation, and on evaluation of the proformas were investigated in the Out Patient Department, and patients reporting to Obstetrics and Gynecology Department with similar symptoms were also included after written informed consent. The patients were divided with simple randomization into two groups of 25 each. Group 1 (treatment group) was put on metformin therapy for three months (500 mg thrice a day), and Group 2 (control group) were given placebo thrice a day for the same duration. The patients were followed up after three months of metformin therapy (500 mg thrice a day) in Group 1, and placebo therapy in Group 2. Body weight, fasting blood glucose, blood glucose after 2 h of 75 g of glucose, and fasting serum insulin levels were assessed initially, and then after three months.RESULTS: The mean age of patients in the present study was 21.46 ± 4.45 years. About 54% patients in the present study had BMI ≥25 kg/m<sup>2</sup>. About 68% of the patients had menstrual irregularity and PCO on ultrasonography (USG). About 44% patients in Group 1 (metformin) had increased insulin levels initially (>20 μU/ml), which were decreased to 16% after three months of metformin therapy. About 52% patients in Group 2 had increased insulin levels that were decreased to 48% after the placebo therapy.CONCLUSION: It has been concluded from this study that metformin significantly lowers insulin levels in patients with PCOS; in both obese and nonobese; which points towards its potential usefulness in treatment of PCOS patients, though it had no significant effect on body mass index in 12 weeks.
Investigation of the FSHR, CYP11, and INSR Mutations and Polymorphisms in Iranian Infertile Women with Polycystic Ovary Syndrome (PCOS)

Abstract:Background: Polycystic ovary syndrome (PCOS) is the most common cause of ovarian dysfunction associated with infertility, Oligomenorrhea or amenorrhea, hirsutism, acne, and obesity. A large body of evidence unraveled, three major groups of genes play critical roles in underlying PCOS molecular mechanism. The aim of this study is to investigate critical exonic variant of FSHR, CYP11, and INSR and determine the functionality of these mutations in Iranian patients with PCOS. Methods: In this case-control study, 130 patients with PCOS who referred to the Vali-e-Asr Hospital with infertility were included. DNA extracted from three ml of peripheral blood of the participants for DNA extraction. The PCR was conducted for each gene and the PCR product was genotyped by sequencing. Results: The data showed that there were two polymorphisms in INSR genes which did not change the protein sequences; these alterations can also be considered as a single nucleotide polymorphism (SNP). Moreover, any exonic variant has not been detected in CYP11B1. Whereas, two missense mutation have been detected in FSHR gene including p.Ala307Thr and p.Asn680Ser. It has been shown that the polymorphisms of the FSHR gene affect the hormone response in the ovaries. Our data demonstrated that the FSHR mutations frequencies were higher in the patients with PCOS rather than control people significantly. Conclusion: These data showed that the polymorphisms of FSHR were significantly associated with PCOS in Iranian infertile women. Further studies with larger sample sizes are needed to be performed for explore the strength of the association.

Abstract:Abstract Background: Aromatase is a key steroidogenic enzyme that catalyzes the conversion of androgens to estrogen. Several studies reported the association of the SNP 50 (rs2414096) in aromatase enzyme with hyperandrogenism. Aim : To investigate the association of genetic polymorphism of aromatase enzyme with polycystic ovary syndrome (PCOS). Material and Method : The study consisted of 62 women diagnosed with PCOS and 60 healthy women as a control group. Individuals were genotyped for rs2414096 of aromatase enzyme by using polymerase chain reaction‐ restriction fragment length polymorphism. Statistical analysis was done by SPSS program. Results: Frequency of (GG) and (GA&AA) genotypes were 64.5% and 35.5% in PCOS group compared to 76.7% and 23.3% in control group. Statistical analysis demonstrated that carriers of (GA&AA) genotype were at higher risk for PCOS compared to carriers of (GG) genotype (OR= 1.81, 95%CI= 0.8‐3.99). Carrier of A allele were also at higher risk of PCOS compared to carrier of G allele (OR=1.3, 95% CI= 0.7‐ 2.3). Conclusion: Polymorphism of rs 2414096 in CYP19 is associated with the pathogenesis of PCOS. Key word : Aromatase, Polymorphism, PCOS.

Abstract:Polycystic ovary syndrome is a multifactorial condition associated with reproductive and endocrine organs and might cause infertility and metabolic abnormalities in childbearing age. PCOS seems to be a multifactorial disorder resulting from the combination of several genetic and environmental factors. Little research has been conducted to date on the impact of polymorphisms in infertility. We aim to review the appearance of polymorphisms in females of diverse ethnicities and their effect on infertility in the population with polycystic ovary syndrome. There have been numerous reports of the importance of the steroidogenesis pathway and genetic variants in PCOS pathogenesis. The most important genes that play a role in the aetiology of PCOS are CYP11A1, CYP17A1, and CYP19A1. We evaluated the occurrence of polymorphisms in various ethnicities in the CYP11A1, CYP17A1, and CYP19A1 genes and their efficacy on increasing PCOS risk with infertility. Our findings revealed that polymorphisms in various ethnicities are associated with the risk of PCOS with infertility. Although conflicting results regarding CYP11A1, CYP17A1, and CYP19A1 polymorphisms and their influence on PCOS with infertility have been reported in a small number of papers, the authors feel this may be attributable to the sample size and ethnic composition of the examined populations. In conclusion, our study strongly suggests that the CYP11A1, CYP17A1, and CYP19A1 genes might significantly enhance the probability of developing PCOS with infertility.

Abstract:Polycystic ovary syndrome (PCOS) is the most common disorder of ovarian function in premenopausal women. PCOS is characterised by chronic anovulation and androgen excess with clinical manifestation of irregular menstrual cycles, hirsutism and/or acne. Insulin resistance with resultant hyperinsulinaemia, irrespective of excess weight or frank obesity, has been reported in patients with PCOS, and, as insulin has a direct effect on ovarian androgen production in vitro, insulin resistance may play a crucial role in the physiopathology of PCOS. Although the molecular mechanism(s) of insulin resistance in PCOS is unclear, excessive insulin-independent serine phosphorylation of the β subunit of the insulin receptor, as reported in some patients with PCOS, has been put forward as a new mechanism for insulin resistance.Insulin-sensitising agents have recently been investigated for their role in the short term treatment of insulin resistance in PCOS. Controlled studies have shown that metformin administration, by promoting bodyweight loss, can decrease fasting and stimulated plasma insulin levels. However, other studies have shown metformin 500mg 3 times daily to decrease insulin secretion and to reduce ovarian production of 17α-hydroxyprogesterone with recovery of spontaneous or clomifene-induced ovulation, independently of weight loss. These findings suggest a new indication for metformin and present insulin-sensitising agents as a novel approach in the treatment of ovarian hyperandrogenism and abnormal ovulation in PCOS. They also suggest that long term administration of metformin might be helpful in treating insulin resistance, thus reducing risks of type 2 (non-insulin-dependent) diabetes and cardiovascular disease in these patients.

Abstract:Introduction: Polycystic ovary syndrome (PCOS) is an endocrine-metabolic disorder that occurs due to a hormone imbalance in the ovaries. It can cause the development of fluid-filled cysts on the follicles, causing infertility. Further, PCOS can cause insulin resistance and hyperandrogenism, increasing the amount of glucose and testosterone levels in the body. As a result, PCOS symptoms could include irregular or absent periods, weight gain, hirsutism, and more. PCOS patients are also at higher risk of developing type 2 diabetes, depression, and high blood pressure. Currently, there are no cures for PCOS, but it can be managed with lifestyle changes, contraceptive pills for irregular periods, and fertility treatments. Two of these treatments include inositol and metformin. The objective of the study is to ascertain how these therapies affect PCOS symptoms such as hyperandrogenism, insulin sensitivity, and infertility. Methods: Primary articles were found through the PubMed and OVID-Medline databases to investigate the effect of inositol or metformin on PCOS. Keywords such as PCOS, inositol, metformin, myo-inositol, insulin sensitivity, androgens, and fertility were used. Research published before 2010 was excluded. Results: The results found that metformin and inositol decreased the amounts of free testosterone and glucose in women with PCOS. Further, oocyte quality and production were improved in women taking metformin and inositol. These results were seen in women of all ages and of all BMIs. Discussion: Both metformin and MI are shown to significantly improve PCOS symptoms of hyperandrogenism, insulin sensitivity, and fertility issues. Although some limitations were present within the review and the studies examined due to exclusions, the review presents a comprehensive outlook on the impact of the interventions mentioned. Establishing a foundation for clinical trials would allow for the integration of these interventions into healthcare settings. Conclusion: A combination of both metformin and inositol, as well as diet and lifestyle changes can significantly improve the symptoms and life quality of individuals with PCOS. Future research would benefit from further studies done into investigating the combined effects of metformin and MI, as well as further assessment of secondary symptoms, as well as regarding the dosage of myo-inositol and metformin.
The Comparative Effects of Myo-Inositol and Metformin Therapy on the Clinical and Biochemical Parameters of Women of Normal Weight Suffering from Polycystic Ovary Syndrome

Abstract:Background: Polycystic ovary syndrome (PCOS) is a multisystem reproductive–metabolic disorder and the most common endocrine cause of infertility. The objective of our study was to determine the influence of myo-inositol (MI) on insulin resistance (IR), menstrual cycle regularity, and hyperandrogenism in women suffering from PCOS with normal BMI and diagnosed IR. Methods: We performed a prospective randomized controlled trial (RCT) that included 60 participants with PCOS who had IR and a normal BMI. Two groups were formed. A group of thirty patients received MI, and thirty patients in the control group received metformin (MET). Results: A statistically significant reduction in the area under the curve (AUC) of insulin values during the oral glucose tolerance test (OGTT) was recorded in both examined groups after the applied therapy with MI and MET. The regularity of the menstrual cycle in both groups was improved in >90% of patients. A statistically significant decrease in androgenic hormones (testosterone, SHBG, free androgen index—FAI, androstenedione) was recorded in both groups and did not differ between the groups. Conclusions: Both MI and MET can be considered very effective in the regulation of IR, menstrual cycle irregularities, and hyperandrogenism in women with PCOS.

Abstract:Study question: What are the consequences of polycystic ovary syndrome (PCOS) pathology and metformin-pretreatment in vivo in women with PCOS on the metabolism and steroid production of follicular phenotype- and long-term cultured-granulosa cells (GC)? Summary answer: PCOS pathology significantly compromised glucose metabolism and the progesterone synthetic capacity of follicular- and long-term cultured-GCs and the metabolic impact of PCOS on GC function was alleviated by metformin-pretreatment in vivo. What is known already: Granulosa cells from women with PCOS have been shown to have an impaired insulin-stimulated glucose uptake and lactate production in vitro. However, these results were obtained by placing GCs in unphysiological conditions in culture medium containing high glucose and insulin concentrations. Moreover, existing data on insulin-responsive steroid production in vitro by PCOS GCs vary. Study design, size and duration: Case-control experimental research comparing glucose uptake, pyruvate and lactate production and progesterone production in vitro by GCs from three aetiological groups, all undergoing IVF; healthy control women (Control, n = 12), women with PCOS treated with metformin in vivo (Metformin, n = 8) and women with PCOS not exposed to metformin (PCOS, n = 8). The study was conducted over a period of 3 years between 2007 and 2010. Participants/materials, setting, methods: Rotterdam criteria were used for the diagnosis of PCOS; all subjects were matched for age, BMI and baseline FSH. Individual patient cultures were undertaken with cells incubated in a validated, physiological, serum-free culture medium containing doses of 0-6 mM glucose and 0-100 ng/ml insulin for 6 h and 144 h to quantify the impact of treatments on acute and long-term metabolism, respectively, and progesterone production. The metabolite content of spent media was measured using spectrophotometric plate reader assay. The progesterone content of spent media was measured by enzyme-linked immunosorbent assay. Viable GC number was quantified after 144 h of culture by the vital dye Neutral Red uptake assay. Main results and the role of chance: Granulosa cells from women with PCOS pathology revealed reduced pyruvate production and preferential lactate production in addition to their reduced glucose uptake during cultures (P < 0.05). Metformin pretreatment alleviated this metabolic lesion (P < 0.05) and enhanced cell proliferation in vitro (P < 0.05), but cells retained a significantly reduced capacity for progesterone synthesis compared with controls (P < 0.05). Limitations, reasons for caution: Although significant treatment effects were detected in this small cohort, further studies are required to underpin the molecular mechanisms of the effect of metformin on GCs. Wider implications of the findings: The individual patient culture strategy combined with multifactorial experimental design strengthens the biological interpretation of the data. Collectively, these results support the notion that there is an inherent impairment in progesterone biosynthetic capacity of the GCs from women with PCOS. The positive, acute metabolic effect and the negative long-term steroidogenic effect on GCs following metformin exposure in vivo may have important implications for follicular development and luteinized GC function when the drug is used in clinical practice. Abstract:Polycystic ovary syndrome (PCOS) is a common and heterogeneous disorder of women of reproductive age, characterized by hyperandrogenism and chronic anovulation. Metformin may lower the androgen level in follicular fluid, affecting local levels of IGF (Insulin like growth factor)s, and may produce improved ovarian stimulation. Metformin is used in PCOS cases in order to decrease plasma luteinizing hormone (LH), hyperinsulinemia, and ovarian androgen levels. Metformin increases the ovulatory rate, cervical scores and pregnancy rate. This review article focuses on the combined effect of metformin administration in PCOS patients.

Abstract:Polycystic ovary syndrome (PCOS) is an endocrine and metabolic dysfunction. This study aims to compare the oral and local treatments of metformin or its nanoparticles (NPs11) for ameliorating PCOS in rats. Rats were divided into 4 groups: the control group with no drug treatment; the PCOS group, where subcutaneous testosterone was given (10 mg/kg/day) for 28 days; the MET group, where metformin was administered orally or locally; and the NP group, where metformin NPs11 were also administered orally or locally. Oral administrations were for 21 days, while local injection was performed once surgically. After 7 weeks, all rats were sacrificed; blood glucose and serum hormonal levels and lipid profile were estimated, and the ovaries were assessed by histopathological, Ki-67 immunohistochemical, and histomorphometric evaluations. Blood glucose levels were significantly decreased in groups of orally administered metformin or NPs11 only, while the most efficient option for modulating PCOS-induced hormonal and lipid profile changes was intraovarian injection of NPs11. The ovaries of PCOS rats demonstrated large follicular cysts, massive collagen depositions, and attenuated Ki-67 immunoexpression. Also, the PCOS group revealed a significant decrease in the count of all stages of growing follicles, corpora lutea, granulosa cell layer thickness, and surface area of corpora lutea, in addition to an increase in the number of atretic follicles and follicular cysts, theca cell layer thickness, and surface area of the follicular cysts. All these parameters were recovered with metformin or their NPs11 treatments in different degrees, while local injection of NPs11 was the best option.
