Abstract:Introduction Alopecia areata (AA) is an autoimmune disease induced by viral infection or vaccination. With the increased incidence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the incidence of AA has also increased. Recently the incidence was found to be 7.8% from a previously reported rate of 2.1%. The physical and psychological damage caused by AA could seriously affect patients' lives, while AA is a challenging dermatological disease owing to its complex pathogenesis.

Abstract:BACKGROUND: The clinical characteristics and pathomechanism for immune-mediated alopecia following COVID-19 vaccinations are not clearly characterized.OBJECTIVE: We investigated the causality and immune mechanism of COVID-19 vaccines-related alopecia areata (AA).STUDY DESIGN: 27 new-onset of AA patients after COVID-19 vaccinations and 106 vaccines-tolerant individuals were enrolled from multiple medical centers for analysis.RESULTS: The antinuclear antibody, total IgE, granulysin, and PARC/CCL18 as well as peripheral eosinophil count were significantly elevated in the patients with COVID-19 vaccines-related AA compared with those in the tolerant individuals (P = 2.03 × 10<sup>-5</sup>-0.039). In vitro lymphocyte activation test revealed that granulysin, granzyme B, and IFN-γ released from the T cells of COVID-19 vaccines-related AA patients could be significantly increased by COVID-19 vaccine excipients (polyethylene glycol 2000 and polysorbate 80) or spike protein (P = 0.002-0.04).CONCLUSIONS: Spike protein and excipients of COVID-19 vaccines could trigger T cell-mediated cytotoxicity, which contributes to the pathogenesis of immune-mediated alopecia associated with COVID-19 vaccines.

Abstract:Background Cases of alopecia areata (AA) attributed to the coronavirus disease 2019 (COVID-19) vaccination have been reported in recent literature. However, these reports are reflective of specific geographic areas, and whether this phenomenon is observed in other regions remains to be investigated. This study focused on the association between AA and COVID-19 vaccination among patients from a large single-center safety net hospital in California. Methodology In this study, using electronic health records of patients and publicly available vaccination data, the demographics of patients including age group, sex, and race along with the vaccination status were carefully reviewed. Results A total of 73 cases of AA in the period from the release of the COVID-19 vaccination on December 17, 2020, to February 10, 2023, were identified. The odds ratios (ORs) for developing AA among the vaccinated and unvaccinated for each demographic level were calculated. Among all vaccinated individuals, the OR for developing AA was 0.58 (95% confidence interval (CI) = 0.35-0.94, p-value = 0.02). Conclusions This investigation noted no apparent increase in the incidence of AA among the vaccinated population compared to the unvaccinated population.

Abstract:Coronavirus disease 2019 (COVID‐19) caused by severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) was declared a pandemic by the World Health Organization, and COVID‐19 continues to have a major impact on society. Numerous studies have reported impaired health sequelae after COVID‐19 recovery, one of which is hair loss. Individuals with hair loss experience a substantial mental burden, which potentially hinders their social life. However, few studies have systematically analyzed the details including hair loss. Therefore, we conducted a narrative review using PubMed on the frequency, associated comorbidities, disease characteristics, and treatment of hair loss after SARS‐CoV‐2 infection (HLASCI). Two search strings were used to identify 28 articles. Of note, most of the literature identified on COVID‐19 sequelae reported an emergence/occurrence of hair loss. HLASCI is speculated to be composed of a heterogeneous population, with the onset or exacerbation of telogen effluvium (TE), anagen effluvium, androgenetic alopecia (AGA), and alopecia areata (AA) reported as possible underlying mechanisms. Among these, acute TE is thought to be the primary cause of HLASCI, with COVID‐19 treatment and TE improvement being considered crucial for HLASCI management. An association between COVID‐19 and AA exacerbation has also been implicated with still insufficient evidence. Spontaneous recovery of TE can be expected once infection reduces; however, faster improvement in symptoms is expected to reduce the mental and social burden of patients. An additional search string identified 11 articles about TE treatment which suggested that the use of minoxidil may be beneficial. Topical minoxidil has been widely used for AGA patients, who have been speculated to exhibit poor resistance to SARS‐CoV‐2. Topical minoxidil may provide relief from HLASCI, but future clinical research is warranted to confirm this observation.

Abstract:Previous studies have proposed alopecia areata (AA) as a potential outcome of COVID‐19 infection among autoimmune diseases, yet the findings might be inconclusive and difficult to generalize due to limited sample sizes and evidence levels. Thus, we aimed to investigate in detail the long‐term risk of AA following SARS‐CoV‐2 infection based on large, binational, general population‐based cohort studies. Our study investigated the long‐term AA risk after SARS‐CoV‐2 infection by analyzing bi‐national, claim‐based cohorts in South Korea and Japan: a Korean nationwide cohort (K‐COV‐N cohort; discovery cohort; total n = 10 027 506) and a Japanese claims‐based cohort (JMDC cohort; validation cohort; total n = 12 218 680). AA was identified based on the international classification of diseases 10th revision code (L63) requiring at least three claims within 1 year. After exposure‐driven propensity score matching, SARS‐CoV‐2 infection was associated with an increased risk of incident AA (aHR, 1.66; 95% CI, 1.38–1.99). This increased risk was observed and persisted for up to 6 months. A similar pattern was observed in the validation cohort. As modifiable factors, severe COVID‐19 increased the risk of AA, whereas receiving two or more doses of the COVID‐19 vaccine before infection decreased the risk of AA. Through a bi‐national cohort study in South Korea and Japan, SARS‐CoV‐2 infection was associated with an elevated risk for incident AA in the aspect of long COVID.

Abstract:Background: Alopecia Areata (AA) is an autoimmune dermatological disease that could be influenced by psychological factors as part of the pathophysiology of the illness. Aims: This review article aims to report on psychodermatological and psychopathological aspects involved in the etiopathogenesis and comorbidities of AA, as well as on the psychiatric and psychological management of affected patients. Methods: We conducted a literature search on PubMed and Google Scholar from January 1980 to May 2021 employing the search terms of alopecia areata, psychological factors, psychological impact, psychodermatology, and psychopathology. All lists of references from the identified articles were screened for further relevant studies. The search was limited to English and Spanish language articles and was supplemented with themed books and book chapters. No specific quality criteria were used for the studies selection. Results: Several authors have found a high comorbidity rate between AA and mental disorders, concluding that stress and psychological factors are involved in both the development and exacerbation of the illness. More evidences are needed in order to describe the associations between the immune response, stress, and the physiological factors observed in AA patients. Conclusion: AA is a complex illness characterized by multifactorial etiology. An interaction between genetic, autoimmune, hormonal, neural, and psychological factors is supposed. Psychopathological aspects of illness need to be better described and considered in the clinical setting.

Abstract:Background : Severe acute respiratory virus syndrome coronavirus 2 (SARS-CoV-2) was responsible for coronavirus disease (COVID-19) pandemic. As patients recovered from COVID-19 infection, hair loss was increasingly observed as a distressing symptom. Methods : This was a prospective cross-sectional study of patients with post COVID-19 hair loss between July to December 2021 at a tertiary care centre. Detailed history, clinical examination, trichoscopy and biochemical tests were performed and recorded. COVID-19 disease severity was assessed based on duration of COVID-19 infection and place of management. Results : The study included 120 patients with mean age being 39.6 years. Majority of the patients were females, treated at home and had COVID-19 infection for >2 weeks. Mean visual analog scale (VAS) score for stress was 5.25. Vitamin D deficiency was present in 56.7% and low ferritin in 30% of cases. Mean time of onset of hair loss post COVID-19 was 49 days. Patients mainly presented with diffuse hair loss. Trichodynia was present in 15.8% of cases. The degree of hair loss was severe in 55.8% of the subjects. Positive hair pull test was seen in 65% of patients. Most common trichoscopic features included single hair follicles' (81.7%) and vellus hair >10% (60%). Conclusions : The mean time of onset of hair loss post COVID-19 infection was less than 2 months. Majority patients had diffuse pattern and severe degree of hair loss. Trichoscopy can aid in unmasking co-existing patterned hair loss in patients presenting clinically with diffuse hair loss.

Immune-Mediated and Psychiatric Comorbidities Among Patients Newly Diagnosed With Alopecia Areata

Abstract:Importance: Alopecia areata (AA) has been associated with multiple comorbidities, yet information regarding the timing of comorbidity development after AA diagnosis is limited. Objective: To evaluate the prevalence and new-onset incidence of psychiatric and autoimmune comorbidities in patients with AA in the US. Design, setting, and participants: This retrospective cohort analysis used data collected from January 1, 2007, to April 30, 2023, from the Merative MarketScan Research Databases, which contains medical and drug claims data from more than 46 million patients in the US. Data from adolescent and adult patients (aged 12-64 years) diagnosed with AA and patients without AA (ie, controls) were evaluated. For some analyses, patients with AA were matched (1:4) to controls based on sex, age, and geographic region. Main outcomes and measures: Prevalence (at the time of AA diagnosis) and incidence (new onset after AA diagnosis) of psychiatric and autoimmune diseases were reported as percentage of patients. Risk of developing a new-onset psychiatric or autoimmune disease after AA diagnosis was calculated as adjusted hazard ratios (AHRs) with 95% CIs. Results: At baseline, 63 384 patients with AA and 3 309 107 without AA were identified. After matching, there were 16 512 and 66 048 patients in the AA and control groups, respectively, with a mean (SD) age of 36.9 (13.4) years and 50.6% of whom were female. Compared with the unmatched controls, patients with AA had higher prevalence of psychiatric (30.9% vs 26.8%; P < .001) and autoimmune (16.1% vs 8.9%; P < .0001) comorbidities at AA diagnosis; incidence was also higher in patients with AA (without history of these comorbidities) vs the matched control group. Patients with AA vs controls had a significantly higher risk of developing a psychiatric (AHR, 1.3; 95% CI, 1.3-1.4) or autoimmune (AHR, 2.7; 95% CI, 2.5-2.8) comorbidity. Conclusions and relevance: In this cohort study, patients with AA had a higher prevalence of autoimmune and psychiatric comorbidities at AA diagnosis and demonstrated an elevated risk of new-onset autoimmune and psychiatric comorbidities after their diagnosis. These data highlight the most common comorbidities among patients with AA and may help physicians counsel and monitor patients newly diagnosed with AA.

Abstract:Telogen effluvium post-COVID-19 is a condition characterized by the diffuse and reversible loss of scalp hair in the period following infection with SARS-CoV-2, and it is currently the second cause of alopecia in women. In the context of the COVID-19 infection, intense psychological stress contributes to alopecia appearance, along with systemic inflammation, autoimmune reactions, oxidative stress, and virally induced hypoxia. Cytokines with proinflammatory action and vasoactive substances negatively modulate the metabolism of some molecules, such as proteoglycans, involved in the hair follicle's growth cycle. Studies show that a large percentage of hairs will suddenly enter the catagen phase during a moderate to severe COVID-19 infection. In the present paper, we update the data from the literature with a clinical example. Our case highlighted that the telogen effluvium after infections with SARS-CoV-2 is reversible with appropriate dermatological treatment. For therapeutic success, informing the patient about this pathology's self-limited and reversible character is essential to reduce the emotional stress that may aggravate the disease.

Abstract:BackgroundAlopecia areata (AA) is an autoimmune disorder and chronic recurrent inflammatory disease that results in non-scarring hair loss. ObjectivesOur aim is to investigate several parameters related to autoimmunity and inflammation in AA patients and to evaluate their association with this disease. MethodsThis study included a total of 672 eligible AA patients and 580 age- and sex-matched healthy individuals who were treated at a third-class hospital in Hefei from January 2016 to May 2020. Data for serum C-reactive protein (CRP), 25-hydroxy vitamin D (25(OH)D), T3, T4, thyroid‑stimulating hormone (TSH), thyroid antibodies (TPOAbs and TGAbs), antinuclear antibodies (ANA), complements (C3, C4), and several immunoglobulins (IgA, IgM, and IgG) were collected in this study. ResultsRegarding autoimmune-related functional indicators, there were no statistically significant differences between TSH, TGAbs, C3, C4, IgA, IgM, and IgG levels between AA patients and healthy controls. Only T3, T4, TPOAbs, and ANA values were significantly abnormal in the AA group compared with the healthy individuals (p < 0.05). In addition, the mean serum 25(OH)D concentration was significantly lower in the patient group than that in control group (p < 0.05), and serum CRP was significantly increased (p < 0.05). ConclusionAlthough the etiopathogenesis of AA is not clear, the importance of monitoring the levels of T3, T4, TPOAbs, ANA, and 25 (OH)D in AA cases is indispensable.

Abstract:Abstract Alopecia areata (AA) is the most common cause of immune-mediated alopecia. Due to the autoimmune nature of AA, researchers have investigated its potential links with infectious diseases. Herpes zoster (HZ) is one of the most common infectious diseases encountered in clinical practice, with a lifetime risk of 30%. However, only a few studies have reported a potential association between AA and HZ. This nationwide, population-based cohort study aims to explore the bidirectional relationship between AA and HZ, using data from the National Health Insurance Research Database (NHIRD) of Taiwan between 1997 and 2013. Individuals aged ≥ 10 years with a diagnosis of AA or HZ were included. Both AA and HZ diagnoses were confirmed by board-certified dermatologists on at least three occasions. For every patient with AA or HZ, 10 matched controls were randomly selected. These individuals were matched for age, sex, monthly income and residence. The primary outcomes assessed were new-onset HZ for patients with AA and new-onset AA for patients with HZ. Using a Cox regression model with adjustment for potential confounders, we assessed the risk of HZ in patients with AA, and vice versa. The cumulative incidence of disease was analysed using Kaplan–Meier survival curves with log-rank tests. In study 1, we included 9072 patients with AA and 90 720 matched controls. In study 2, we included 31 570 patients with HZ and 315 700 matched controls. Compared with controls, patients with AA had a 3.93-fold [95% confidence interval (CI) 3.42–4.52] increased risk for HZ, while patients with HZ had a 5.20-fold (95% CI 4.21–6.42) increased risk for AA. Both associations remained significant after stratification by sex and age. The 15-year cumulative incidence of HZ in the AA group was significantly higher than in the controls, and vice versa (P < 0.001). Furthermore, patients with AA had 4.97-fold (95% CI 2.55–9.68) and a 3.42-fold (95% CI 2.55–4.59) elevated risk for HZ ophthalmicus (HZO) and HZ with nervous system complications, respectively, in comparison with controls. Patients with HZO and HZ with nervous system complications exhibited 3.85-fold (95% CI 1.43–10.4) and a 5.00-fold (95% CI 3.42–7.32) increased risk for AA, respectively, compared with controls. We also found that patients with AA had a significantly younger age at HZ diagnosis compared with controls (40.6 vs. 50.6 years, P < 0.001). Our study revealed a bidirectional association between AA and HZ. Clinicians should remain vigilant regarding this link in both patient cohorts.

Abstract:Background: Alopecia areata (AA) is an autoimmune disorder characterized by hair loss on the scalp, face, and other body areas. Despite affecting approximately 2% of the global population, there has been no previous bibliometric analysis specifically focusing on AA treatment, that can guide researchers in exploring promising treatment options and directing future research efforts. Summary: This study conducted a bibliometric analysis of AA treatment research, encompassing publications from 2003 to 2022. A total of 1323 papers from 65 countries, predominantly led by the United States and China, were included in the analysis. The number of publications related to AA treatment showed a notable increase over the years. Prominent research institutions included the University of Manchester, Icahn School of Medicine at Mount Sinai, University of Miami, and Columbia University. Among the journals, Dermatologic Therapy stood out as the most popular, while the Journal of the American Academy of Dermatology appeared as the most frequently co-cited publication. Key Messages: 1. This bibliometric study provides a comprehensive overview of the research trends and advancements in AA treatment. 2. The analysis identified key research hotspots, including variants, platelet-rich plasma (PRP), janus kinase (JAK) inhibitors, and regulatory T (Treg) cells. 3. Researchers specializing in AA can use these findings as a valuable reference to augment their study and explore new avenues for research in treating AA.

Abstract:Background Alopecia areata (AA) is a common, acquired, and nonscarring type of hair loss that affects people of every generation and is intractable in severe and relapsing cases. Patients with AA, especially those with greater scalp involvement, have poor health-related quality-of-life scores. Purpose: Following our previous review article in the April 2017 issue of the Journal of Dermatological Science, we aim to provide a pair of review articles on recent progress in multidisciplinary approaches to AA. Main findings: We found more than 1800 publications on AA from July 2016 to December 2022. Conclusions: In this review, we focused on the latest information on the epidemiology, comorbidities, and pathogenesis of AA.

Abstract:Background: Alopecia areata (AA) is a frequent, reversible kind of hair loss. It is commonly seen as patchy regions of full hair loss on the scalp and other body parts that might lead to complete loss of all body hair. The condition is characterized by localized inflammatory lesions with perifollicular T-cell infiltrates, demonstrating the importance of local cytokine generation in the formation of patchy hair loss. IL-1β is a major inhibitor of hair development in vitro. Its impact is blocked by the interleukin-1 receptor antagonist, IL-1ra. Objective: To review the epidemiology, clinical characteristics, pathogenesis, and associated genetic factors and the emerging therapeutics for AA in this review article. Greater knowledge of the disease pathophysiology may contribute to establishing novel medicines that are more targeted and effective against AA. Methodology: The databases were searched for articles published in English in 3 databases [PubMed – Google scholar science direct] and Boolean operators (AND, OR, NOT) had been used such as [Alopecia Areata AND Genetic Polymorphisms OR Overview] and in peer-reviewed articles between January 2001 and May 2022. Conclusion: Comprehensive understanding of the genetic basis of AA may mediate an efficient therapeutic approach for managing and treating this life quality affecting condition.
Ranatuerins: antimicrobial peptides isolated from the skin of the American bullfrog, Rana catesbeiana.

Abstract:Nine peptides, termed ranatuerins 1-9, with antimicrobial activity towards Staphylococcus aureus, were isolated from an extract of the skin of the adult American bullfrog, Rana catesbeiana. In common with other cytolytic peptides from Ranid frogs, (e.g. ranalexin, gaegurins, brevinins), ranatuerins 1 and 4 contain an intramolecular disulfide bridge forming a heptapeptide ring whereas in ranatuerins 2 and 3 the disulfide bridge forms a hexapeptide ring. The structurally related ranatuerins 5-9 comprise 12 - 14 amino acids and show sequence similarity towards the hemolytic peptides A1 and B9 previously isolated from the skin of Rana esculenta. Of the peptides purified, ranatuerin 1 (SMLSVLKNLGKVGLG FVACKINKQC) showed the broadest spectrum of antimicrobial action with inhibitory activity against S. aureus, Escherichia coli and Candida albicans.
Interleukin gene polymorphisms and alopecia areata: A systematic review and meta-analysis

Abstract:Alopecia areata (AA) is an autoimmune disease that is characterized by non-scarring hair loss on the scalp or any surface with hair. The clinical signs of AA differ from little well-defined patches of balding to the diffuse inclusion of the scalp or the whole body. [ 1 ] Each patient experiences a unique and unpredictable course, and it is categorized into the following 3 groups based on its severity and the areas of hair loss: (1) alopecia areata in patches, the most common type, is characterized by round or oval patches on the head or other parts of the body in 90% of cases; (2) alopecia totalis, in which the scalp is completely or nearly completely devoid of hair; and (3) alopecia universalis, the most severe condition that stands out from the other 2 due to the fact that hair loss affects the entire body in addition to the scalp and face. [ 2 ] At present, approximately 2% of the general population, across all racial, gender, and age groups, has been affected by AA. [ 1 ] It can affect people of all ages, however, the prevalence of occurring alopecia areata seems higher in children (1.92%) than in adults (1.47%). [ 3 ] The immune response, genetic and environmental factors play a very important role in the onset and progression of alopecia areata. [ 4 , 5 ]

Abstract:Alopecia areata (AA) is a common autoimmune-mediated hair loss disorder in humans with an estimated lifetime risk of approximately 2 %. Episodes of hair loss usually begin with isolated hairless patches that may progress to complete hair loss over the entire body. A familial occurrence of AA is well established, with recurrence risks of about 6–8 % in first-degree relatives. AA is a multifactorial disorder involving both environmental and genetic risk factors. Previous research has identified 14 susceptibility loci, most of which implicate genes involved in the immune response. The following review presents a summary of the latest findings from genome-wide association, sequencing and gene expression studies of AA, as well as their contribution to the recent therapeutic developments.

Abstract:Recent progress in material science has proved that high-temperature superconductors, such as bulk melt-processed yttrium barium copper oxide (YBCO) single domains, have a great potential to trap significant magnetic fields. In this paper, we will describe a novel method of YBCO magnetization that only requires the applied field to be at the level of a permanent magnet. Instead of applying a pulsed high magnetic field on the YBCO, a thermally actuated material (TAM), such as Mg0.15Cu0.15 Zn0.7Ti0.04Fe1.96O4, has been used as an intermedium to create a travelling magnetic field by changing the local temperature so that the local permeability is changed to build up the magnetization of the YBCO gradually after multiple pumping cycles. It is well known that the relative permeability of ferrite is a function of temperature and its electromagnetic properties can be greatly changed by adding dopants such as Mg or Ti; therefore, it is considered to be the most promising TAM for future flux pumping technology. Ferrite samples were fabricated by means of the conventional ceramic method with different dopants. Zinc and iron oxides were used as raw materials. The samples were sintered at 1100 °C, 1200 °C , and 1300 °C . The relative permeability of the samples was measured at temperatures ranging from 77 to 300 K. This work investigates the variation of the magnetic properties of ferrites with different heat treatments and doping elements and gives a smart insight into finding better ferrites suitable for flux pumping technology.
Effect of Traditional Therapeutics on Prevalence and Clinical Outcomes of Coronavirus Disease 2019 in Chinese Patients with Autoimmune Diseases

Abstract:The impact of the Coronavirus disease 2019 (COVID-19) pandemic on autoimmune diseases (AID) patients has been an important focus. This study was undertaken to characterize the incidence, clinical manifestations and hospitalization among AID affected by COVID-19 and to analyze the association between immunomodulatory medication and these outcomes. Clinical, demographic, maintenance treatment, symptoms and disease course data and outcomes of AID patients with COVID-19 infection were assessed via an online survey tool and printed copy from 1 January till February 28, 2023. A total of 432 patients with AID were enrolled in the study. The results showed the most common conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) was hydroxychloroquine (HCQ). The usage of csDMARDs didn't increase the risk of COVID-19 infection. Patients who warranted hospitalization were significantly older. ILD was associated with higher hospitalization rate. No csDMARDs other than calcineurin inhibitor (CNI) was associated with increased risk of hospitalization. HCQ intake was associated with cough. Compared with no glucocorticoids (GCs) group, high doses of GCs were accompanied with higher proportion of gastrointestinal symptoms and tachycardia, lower proportion of sore throat and ageusia. GCs didn't provoke the COVID‐19 infection in patients with AID, but chronic use of oral GCs was significantly more common in those requiring hospitalization, and higher dose of GCs were correlated with higher risk of hospitalization. 97 patients discontinued csDMARDs after infection, which resulted in an elevated risk of hospitalization. Meanwhile, withdrawal of csDMARDs was associated with higher odds of disease flare and lower proportion of remission than maintenance groups. Collectively, our analysis provides the evidence that maintenance treatment of csDMARDs may be more prudent for AID patients during COVID-19 pandemic.
