Abstract:Objective: The major treatment method of vertebral tumor was surgical resection, because of intraopertive much blood loss, vertebral tumor was not all resected, dead rates of postoperation and complications was high. The preoperative selective target arterial embolization of vertebral tumor is a useful technique that can significantly decrease the intraoperative blood loss.Methods: With seldinger's technique made selective angiography, gradually superior selective target arterial embolization of vertebral tumor. Results: The intraoperative blood loss was tremendous reduced for succesful preoperative embolization, with a range from 400 to 3 000ml. Conclusion: The preoperative selective target arterial embolization of vertebral tumor can significantly decrease the intraoperative blood loss. Optimal operative time of surgical treatment of post embolization and the complication and its prevention were discussed.

Abstract:Objective To evaluate the influence of preoperative embolization of middle sacral artery on blood loss during excision of sacral tumor.Methods Totally 125 patients with sacral tumor who underwent artery embolization before excision of sacral tumor from March 2003 to December 2014 were retrospectively analyzed.There were 47 cases of primary tumor (non giant cell tumor), 53 cases of giant cell tumor and 25 cases of metastatic tumor.The intraoperative blood loss during excision of sacral tumor was compared between patients with embolization of middle sacral artery and without embolization of middle sacral artery.Results In 47 cases of primary tumor (non giant cell tumor), the middle sacral artery was embolized in 31 cases and not embolized in 16 cases.There was significant difference regarding blood loss during excision of sacral tumor [1 300 (1 000,2 050) ml vs 2 000 (1 200, 3 400) ml] (P =0.04).In 53 cases of giant cell tumor, the middle sacral artery was embolized in 35 cases and not embolized in 18 cases, showing significant difference regarding blood loss during excision of sacral tumor [2000 (1 200, 3 400) ml vs2 400 (1 500, 4000) ml] (P=0.02).In 25 cases metastatic tumor, the middle sacral artery was embolized in 16 cases and not embolized in 9 cases, showing no significant differenceregarding blood loss during excision of sacral tumor [2 025 (1 000, 3 700) ml vs 1 825 (1 175, 3 450) ml] (P =0.82).Conclusion Embolization of middle sacral artery in preoperative embolization can significantly reduce the intraoperative blood loss during excision of sacral tumor, especially sacral primary tumor.
Combining of Serial Embolization and Denosumab for Large Sacropelvic Giant Cell Tumor: Case Report of 3 Cases.

Abstract:RATIONALE:Both serial arterial embolization (SAE) and denosumab have been proved to be effective in treatment for giant cell tumor (GCT). There is potential synergic effect of combining two methods. The purpose of current study is to justify a new treatment strategy of combination of SAE and denosumab as neoadjuvant or stand-alone treatment for large sacropelvic giant cell tumor.PATIENT CONCERNS:Pelvic and sacral GCTs tend to be very large size and vascular. The concerns of surgical treatment were invasiveness of extensive surgery, intraoperative hemorrhage, nerve function jeopardized and local recurrence. However, SAE alone may not be adequate for complete removal of the tumor.DIAGNOSES:All the three cases were proved to be GCT by core-needle biopsy. Post-treatment pathological change was confirmed by further biopsy.INTERVENTIONS:The patient in Case 1 diagnosed of large recurrent sacral GCT received 6 times of endovascular embolizations with 2-month interval and started on denosumab simultaneously after first session of embolization. The second case was a 22-year-old female presented with a massive iliosacral tumor. SAE was performed for 3 sessions and the denosumab was started simultaneously. The patients was on treatment for half year. Both patients experienced a dramatic decrease in symptoms and concomitant improvement in function after the first embolization and weekly injection of denosumab. Tumor removal was performed on patient in case 2. The last case was a pelvic GCT and the patient received SAE and denosumab for half year. The tumor was then removed with purpose of complete cure.OUTCOMES:The first patient was still on denosumab with stable tumor. The other two patients were both free of recurrence after surgical removal of the tumors. No denosumab was used postoperatively.LESSONS:We reported the first three cases treated by combination of SAE and denosumab in the literature and aim to raise an alternative method for large GCT at challenging anatomical locations, for which surgery would carry significant risk. SAE and denosumab can synergically promote sclerosis and result in significant decrease in pain. It is reasonable to consider using SAE combined with denosumab neoadjuvantly to reduce the extensiveness and morbidity of surgery, however further investigation is warranted.
Effect of Bisphosphonates on Local Recurrence of Giant Cell Tumor of Bone: a Meta-Analysis.

Abstract:Background: Giant cell tumor of bone (GCTB) is a locally aggressive tumor, and its postoperative recurrence remains a problem. The present meta-analysis aimed to analyze the effect of bisphosphonates (BPs) on local recurrence of GCTB. Methods: Seven case-control studies were included by computerized searches of bibliographic databases (PubMed, AMED, EMBASE, the Cochrane library, ISI Web of Science, and China National Knowledge Infrastructure). The pooled adjusted ORs were calculated to evaluate the local recurrence of GCTB. Results: The BP group presented significantly lower total local recurrence rate than the control group in GCTB (P<0.01). Subgroup analysis shows BP group presented significantly lower local recurrence than the control group in GCTB with different tumor grades (P<0.05). In patients who underwent intralesional curettage, a significantly lower local recurrence rate was found in the BP group compared with the control group (P<0.01), but no significance was found for patients who underwent wide resection (P=0.16). None of the included studies described severe adverse effects related to BPs. Conclusion: The results confirmed the effect of BPs on reducing the local recurrence of GCTB, and the effect is not influenced by the tumor grades. BPs are benefit for the patients who underwent intralesional curettage but not recommended for those who underwent wide resection.
Preoperative embolization of spinal tumors:An analysis of factors influencing intraoperative blood loss

Abstract:Objective To evaluate the safety of different spinal tumor preoperative embolization and the factors that may influence intraoperative blood loss. Methods Between March 2003 and May 2011A total of 90 spinal tumor patients receiving preoperative embolization were retrospectively analyzed. Multiple factors, including sex, pathological type, embolization agent, interval between embolization and surgery, surgical history (first or not) and tumor invasion range (for cervical, thoracic and lumbar vertebrae) were analyzed for the effect on intraoperative blood loss. Results For all subjects and those with cervical, thoracic and lumbar vertebral tumors, the estimated blood loss (EBL) was signiifcantly lower if the surgery was ifrst surgery (P=0.023 and 0.044, respectively), and the EBL of giant cell tumor (GCT) was signiifcantly higher than other primary tumors (P=0.013 and 0.006, respectively). The EBL was not statistically different with sex, embolization agent, interval between embolization and surgery and tumor invasion range. Conclusion It is safe and effective for preoperative embolization to reduce the intraoperative blood loss and improve the surgical removal rate.
Embolization in the Treatment of an Intraosseous Glomus Tumor in the Upper Thoracic Spine Complicating Compression Myelopathy：a Case Report and a Literature Review.

Abstract:Glomus tumors are very infrequent in the spine. The lesions can grow intraosseously along the entire spinal axis. A single female presenting with back pain from the upper thoracic spine is reported on. Removal of this lesion may require reconstruction of the anterior column with posterior fixation resulting in significant blood loss. The current report describes an embolization procedure prior to removal in order to reduce the significant blood loss that occurs with removal of this lesion, and summarizes the clinical and pathological characteristics of this rare tumor. A single, recent case and removal of an intraosseous tumor arising from the upper thoracic vertebra of T2-T4 is described. A 45-year-old female presenting with symptoms secondary to a glomus tumor of the upper thoracic vertebra of T2-T4 underwent resection of the lesion followed by reconstruction of the anterior column following preoperative emobolization. She had neurological symptoms for 3 years, and an irregular crescent-shaped lesion was seen going through the foramen at T3 to the chest cavity in the MRI scans. The operation was performed with a posterior approach in a single stage. The use of preoperative embolization of the T2-T4 segmental arteries resulted in significantly less blood loss as compared to without an embolization procedure. It was confirmed by histopathological examination that the glomus tumor rose from the smooth muscle cells in the right paravertebral muscles of T2. The glomus tumor has not recurred in the MRI during the five-year follow-up. Intraosseous glomus tumors are rare lesions that may extend into the epidural space and through the neural foramina and chest compartments resulting in neurological compromise. Over time, they may grow very large. Radiotherapy can be useful for eradication of this rare lesion. However, it can reoccur requiring extensive surgery resulting in significant blood loss. Preoperative embolization results in a reduction of blood loss and can be a very useful technique when performing the resection of large lesions suspected to be glomus tumors.
Efficacy and safety of denosumab combined with surgery in treatment of giant cell tumor of pelvis

Abstract:Objective To evaluate the safety and efficacy of denosumab in treatment of patients with pelvic giant cell tumor of bone (GCTB) during perioperative period. Methods This is a retrospective observational study. Twenty-three patients diagnosed with pelvic GCTB undergoing perioperative denosumab treatment in Musculoskeletal Tumor Center of Peking University People's Hospital from January 2014 to December 2016 were reviewed. The subjective adverse reactions and mandibular X-ray films were used to assess the drug safety. As for efficacy, imaging findings (including X-ray, CT, magnetic resonance imaging) were reviewed. MSTS-93 scoring system was applied in the postoperative functional assessment. Histological response rate, objective response rate, clinical benefit rate and event-free survival rate were all used to deficit the efficacy of denosumab in the treatment of pelvic GCTB combined with surgery. All the results of postoperative were compared statistically with pelvic GCTB patients who underwent surgery in the same hospital from 1999 to 2009. Results All the patients were firstly diagnosed as classic GCTB except for one case which was malignant pelvic GCTB. All patients received denosumab preoperatively and/or postoperatively, and the average number of medications was 8.43. According to the surgical patterns, patients were divided into intralesional surgery group (13 cases) and wide resection group (10 cases). The follow-up was 5-47 months(mean:27.30 months),recurrence was observed in 2 cases in the intralesional surgery group, none in the wide resection group. After drug administration, 13 cases were partial response, 7 cases were stable disease, the objective response rate was 65.0 % (13/20), and the histologically clearance rate of giant cells was 85.0 % (17/20). No case of osteonecrosis of the jaw was observed in this study, and all laboratory indicators were normal. The average postoperative MSTS-93 score was 26.87. Compared with pelvic GCTB patients who underwent surgical treatment from 1999 to 2009, in the intralesional surgery group, there was no significant difference in the recurrence rate [15.4 % (2/13) vs. 30.8 % (4/13), P = 0.514], but the limb function was significantly increased (P= 0.002). Conclusions Denosumab combined with surgery plays an important role in the multidisciplinary treatment of pelvic GCTB. The neoadjuvant strategy can reduce patient's intraoperative blood loss by shrinking the tumor size which makes the intralesional curettage surgery possible, and also diminishing the recurrence rate. But more attention should be paid to secondary malignant GCTB during the use of denousmab.
Effect of denosumab combined with surgery on sacral giant cell tumor

Abstract:Objective To evaluate the clinical benefits of denosumab in treatment of sacral giant cell tumor of bone(GCTB) when used preoperatively or postoperatively along with surgery, and to analysis the improvement of sacral nerve function. Methods Thirty patients diagnosed as sacral GCTB in Musculoskeletal Tumor Center of Peking University People's Hospital from April 2014 to July 2016 were divided into control group (10 cases), post-operative group (9 cases), and neoadjuvant group (11 cases). Patients in the post-operative and neoadjuvant group were treated with 120 mg of subcutaneous denosumab every 4 weeks with loading doses on days 8 and 15 of the first cycle. Results Three patients in the control cohort 1 had recurrence (3/10), no recurrence occurred in the post-operative group (0/9), and 3 patients in the neoadjuvant group had recurrence(3/11).There were no significant differences in event-free survival(EFS) among the three groups (P = 0.133). The objective response rate (OTR) was 63.6 % (7/11) in the neoadjuvant group based on the RECIST 1.1 criteria for evaluating the efficacy of solid tumors. Five cases had significant pain improvement (defined 2 points improved) and had much better bladder and bowel functions. Four patients were able to have their indwelling catheters removed after neoadjuvant denosumab treatment. Conclusions Neoadjuvant therapy with denosumab can relieve the symptoms and neurologic deficits caused by nerve compression and can diminish the intraoperative blood loss. Surgical removal of the tumor is still the basic treatment of sacral GCTB.
Surgical Treatment for Pelvic Giant Cell Tumor: a Multi-Center Study.

Abstract:Background: The purpose of this study was to discuss the clinical results which related to the location of giant cell tumors (GCTs) in the pelvis so as to determine the ideal surgical treatment protocol.Methods: We report 29 cases who accepted surgical treatment from five clinical centers during the last 12 years. All patients were divided into three groups according to tumor locations, and they were also classified into two groups in light of surgical treatments. The parameters for outcome evaluation consisting of general condition, surgical complications, local disease control, and Musculoskeletal Tumor Society (MSTS) 93 functional score had been analyzed, respectively.Results: Surgical treatment in the acetabular area usually resulted in postoperative complications and poor function. One patient who accepted intralesional surgery and two who accepted wide resection had local recurrence. The mean functional score was 25.4 for the 8 patients who received intralesional surgery and 21.9 for the 21 patients who received wide resection. Surgical complications occurred in 1 patient who underwent intralesional surgery and the other 6 patients who underwent wide resection.Conclusions: We conclude that surgical treatment of pelvic GCTs in the acetabular area is difficult to select as it is always accompanied by complications and poor function. Compared to wide resection, intralesional surgery combined with a meticulous preoperative planning may lower the recurrence rate and obtain favorable postoperative functional results.
A case of preoperative embolization for a giant hypervascular pancreatic serous cystic neoplasm in pancreaticoduodenectomy

Abstract:Background: Preoperative vascular embolization is an effective strategy for managing meningiomas, neck paragangliomas, renal cell carcinomas, and bone metastasis by reducing the intraoperative bleeding volume and operation time. Although hypervascular tumors also occur in the pancreas, preoperative embolization for these tumors is not commonly practiced. We herein present a case of a giant serous cystic neoplasm (SCN) of the pancreas with significant arterial vascularity that was managed with preoperative interventional radiology and subsequently resected via pancreaticoduodenectomy. Case presentation: A 60-year-old man presented with an 8-cm hypervascular tumor located at the head of the pancreas, identified as an SCN on pathologic examination. The tumor had increased by 13 mm over 5 years, necessitating surgical intervention. Computed tomography revealed a substantial blood supply to the tumor from the dorsal pancreatic artery and gastroduodenal artery, both branches of the superior mesenteric artery. To mitigate the risk of severe intraoperative bleeding from this giant hypervascular tumor, branches of the dorsal pancreatic artery and gastroduodenal artery were embolized using metallic coils and further secured using a gelatin sponge 1 day prior to pancreatectomy. During the laparotomy, the tumor appeared to have decreased in size, likely because of reduced distension and congestion. Despite significant adhesions to surrounding tissues secondary to prolonged compression and inflammation, the pancreaticoduodenectomy was completed successfully in 5 h and 15 min with blood loss of 763 mL. The patient was discharged on postoperative day 15 without complications. Conclusions: Preoperative arterial embolization for hypervascular pancreatic tumors might control the risk of massive intraoperative bleeding, contributing to a favorable postoperative outcome. Utilizing interventional radiology for preoperative inflow control is one of the beneficial strategies for pancreatectomy in patients with a giant SCN.
The Analysis of the Treatment of Giant Cell Tumor of the Pelvis and Sacrum

Abstract:OBJECTIVE:To discuss the surgical management, local recurrence rate and complications of giant cell tumor (GCTs) of the pelvic and sacrum.METHODS:From December 1997 to December 2005 the data of 46 patients of GCTs of pelvic and sacrum treated were reviewed. There were 25 men and 21 women with the average age of 32 years old ranging from 17 to 64. Out of 46 patients,there were 24 tumors involved the sacrum and the others at the pelvis. According to the site of the tumors on the bone, they was classified into three regions: 8 patients involved region I (ilium), 10 region II (acetabulum) and 4 region III (ischiopubic). Two patients had lesions located at S(1-5), 4 at S(1-4), 12 at S(1-3), 5 at S(1-2) and 1 at S(3-5). Surgical management: 2 patients received 3 times of operations and 7 underwent 2 operations. There were 19 patients managed with intralesional marginal excision and 2 patients with intralesional marginal excision and adjuvant radiotherapy, another 3 patients with widely marginal excision as the treatment of sacral lesions. Nineteen patients underwent the enbloc excision except 2 involved ischium and 1 involved ilium for the managements of pelvic lesions.RESULTS:One recurrent patient with the large, ragged tumor died of serious infection in 2 weeks after the second surgery. The remain 45 patients had followed-up durations ranging from 12 months to 8 years. One patient of malignant giant cell tumor of sacrum died at 15 months after surgery. One patient with postoperation sarcoma underwent reoperation and radiotherapy but died at the 13th month. One patient with sacral lesion occurred pulmonary metastases in two years after surgery, and received chemotherapy with ADM, DDP and IFO. One year later there was no much change in metastatic tumor. One patient with acetabular lesion underwent curettage before local recurrence and pulmonary metastases were found in 2 years. The patient was treated with pulmonary radiation, widely marginal excision and hemi-pelvic prostheses reconstruction. There was no progression in metastatic lesion during the following up. The local recurrence rate of GCT at sacrum was 9/24 (37.5%), and at pelvis was 2/22 (9.1%). However, 2 patients with ischium lesions both recurred after curettage. Nineteen lesions with enbloc excision showed no recurrence at all.CONCLUSIONS:The treatment for GCT of the pelvic and sacrum should be more aggressive because of high incidence of local recurrence after intralesional excision. Although it might induce sacral nerve deficit, widely marginal excision is the best surgical procedure because of its low recurrence rate.
A Nonrandomized Controlled Study of Sacral Giant Cell Tumors with Preoperative Treatment of Denosumab

Abstract:The aim of this nonrandomized controlled study (level 3) was to evaluate whether preoperative denosumab treatment can reduce intraoperative blood loss, facilitate surgical treatment, and improve local control of sacral giant-cell tumor (GCT). Surgical treatment of sacral GCT is very difficult due to extensive bone destruction and complex anatomical structures. The huge intraoperative blood loss may interrupt surgical management and judgment of tumor range. Denosumab can inhibit the differentiation of osteoclast-like giant cells and bone destruction by blocking RANKL-RANK pathway. Study group (preoperative denosumab treatment) and control group (no denosumab treatment) were matched for age, gender, tumor site, staging, and tumor size. In study group, enhanced computed tomography (CT) was performed before and after denosumab treatment. The comparison parameters between 2 groups: CT enhancement rate, intraoperative blood loss, and oncologic outcome. The mean preoperative time of denosumab treatment was 5.2 months in study group. The mean CT enhancement rate of study group was 2.60 before treatment and 1.37 after treatment (P=.012). The posttreatment CT enhancement rate of study group was significantly lower than that of control group (P=.007). The mean intraoperative bleeding of study group and control group was 2166.7 and 5240 mL, respectively (P=.040). The mean operative time of study group and control group was 268.3 and 268.5 minutes, respectively (P=.997). The recurrence rate of study group (66.7%) was significantly higher than that of control group (0%) (P=.046). Preoperative denosumab treatment has the tendency to reduce blood supply and intraoperative bleeding of sacral GCT. But the sclerosis and bony separation can increase the difficulty of tumor curettage and lead to high recurrence rate after denosumab treatment. It is necessary to study the best surgical opportunity after denosumab treatment and precise method to judge tumor range.
Clinical outcome ofintralesional curettage for sacrum giant cell tumor

Abstract:Objective:To evaluate the surgical outcome ofintralesional curettage for sacral giant cell tumor under effective blood control.Method:A total of 43 patients with sacrum giant cell tumor were reviewed retrospectively,of these,35 cases were followed up.There were 18 males and 17 females with the average age of 32 years(range,16-61 years).The lesion site was noted as S1-S3 in 12 cases,S1-S4 in 9,S1-S5 in 5,S1-S2 in 3,S2-S5 in 2 and S3-S5 in 1 respectively,in addition,3 cases had L5-S1 segments involved.A large soft tissue mass and devoid calcification usually presented in all cases.All patients were treated surgically with intralesional and marginal resection in 31 and wide resection in 4.Of the 35 patients,23 underwent intraoperative blood control by temporary abdominal arterial blockade(artery blocking group),which included 6 tourniquet block of abdominal aorta through anterior approach and 17 balloon occlusion of abdominal aorta introduced from femoral artery under fluoresce guide before operation.The other 12 patients did not receive abdominal aorta blockade(artery unblocking group).No patients received radiotherapy after operation except one progressing into malignant tumor.All cases were followed up for an average of 37 months(range,12-84 months).Result:The average blood loss was 3278ml for blocking group and 5150ml for unblocking one,which showed significant difference between two groups(P=0.006).14 cases(40%) were complicated with postoperative incision problems which resolved after debridement and drainage.7 cases experienced cerebrospinal fluid leakage after operation,which was healed through conventional treatment and intravenous antibiotics.In this study,local recurrence rate was 30.43%(7/23) for blocking group and 66.67%(8/12)for unblocking group,which showed statistical difference between two groups(P=0.03).Conclusion:Temporary abdominal aorta blockade during the resection of sacral giant cell tumor can effectively reduce operative blood loss and ensure complete tumor removal as well as reducing the local recurrence rate.
Pre-operative Embolization of Intracranial and Extracranial Tumors: A Review of 37 Cases

Abstract:Objective: To examine the safety and efficacy of pre-operative embolization of intra-and extracranial tumors and determine the selection criteria of patients for this procedure. Material and method: Between June 2008 and August 2012, 37 patients (17 males, 20 females; mean age, 44.2+14.2years), underwent pre-operative embolization of intra- and extracranial tumors, were retrospectively reviewed. Tumor characteristics (type, location, volume, percentage of supplying artery, presence of an early draining vein), angiographic extent of tumor devascularization, timing between embolization and surgery, estimated blood loss, and complication related embolization were evaluated. Results: There were 37 tumors (mean volume, 90.9+83.6 cm3) composed of 18 meningiomas, six hemangioblastomas, six hemangiopericytomas, one metastasis, one osteoblastoma, one osteosacroma, one neurofibroma, one central neurocytoma, one glomus jugulare, one mixed oligoastrocytoma, and one glioblastoma multiforme. Early of draining veins were visualized in 24 patients (64.9%). Failure of pre-operative embolization occurred in four patients. Median time to surgery after embolization was seven days (ranged 1-171 days). There was statistically significant difference between grading of angiographic devascularization and estimated blood loss (p = 0.009, Kruskal-Wallis test). Two patients (5.4%) had embolization-related complications, including hemorrhage during sub-selective catheterization and postoperative scalp necrosis. Conclusion: Although pre-operative embolization of intra- and extracranial tumors was safe, only extensive or complete angiographic devascularization has been effective in less intra-operative blood loss. From this present study, indications regarding when to perform pre-operative embolization include history of excessive bleeding from previous surgery, known hypervascular tumor types (e.g., hemangiopericytoma, hemangioblastoma, paraganglioma), the presence of multiple flow voids on MRI, hypervascular tumors of skull or scalp, deep-seated tumors (e.g., cranial base tumor, intraventricular tumor) with difficulty in early surgical access of the main feeding vessels, and tumors associated with intratumoral aneurysm.
Functional Outcomes of Limb Salvage Surgery in Patients with Giant Cell Tumor of Bone of the Lower Extremities: A Cross-sectional Comparative Study

Abstract:Background and objectives: Giant cell tumor of bone (GCTB) is a benign aggressive tumor primarily treated with surgery. Neoadjuvant treatment with denosumab or zoledronic acid is a common adjunct given to down-stage tumors and facilitate limb sparing surgery. This study sought to determine the characteristics, outcomes, and occurrence of complications following resection (RS) or extended curettage (EC) for GCTB of the lower extremities. Correlation of neoadjuvant therapy with the occurrence of complications was also investigated. Methods: This is an analytical cross-sectional study of 30 patients diagnosed with GCTB of the lower extremity treated between 2015 to 2022 in a single tertiary hospital. Functional outcomes were determined using the 1993 version of the Musculoskeletal Tumor Society (MSTS) score. Mean follow-up for all patients was 2.6 years (SD 1.8). Twenty-two patients (73%) underwent resection, while eight (27%) patients underwent extended curettage. Of the 30 patients, 26 (87%) patients received neoadjuvant therapy, with 21 (81%) given denosumab and five (19%) given zoledronic acid. Results: Functional outcomes were excellent for 23 patients (77%), with no significant difference between RS and EC groups. Nine complications occurred in the RS group, including dehiscence (n=3), superficial infection (n=2), implant failure (n=1), nonunion (n=1), palsy (n=1), and implant irritation (n=1). Five complications occurred in the EC group, four of which were noted to be recurrences, with one case of deep infection. Recurrence was noted to be significantly higher (p=0.0004) in the EC group. Separate correlation analysis showed no significant difference in incidence of complications but found that duration of surgery was significantly longer (p=0.0001), and intraoperative blood loss was significantly higher (p=0.0072) in the RS group. No significant difference (p=0.78) was noted in complication rate between patients given denosumab versus zoledronic acid. Conclusions: Functional outcomes of EC and RS appear to be comparable, including the incidence of complications. However, recurrence was noted to be significantly higher in EC. There appears to be no clear advantage between denosumab or zoledronic acid for GCTB. As a neoadjuvant medication and/or to control tumor progression, zoledronic acid may be the more economic option especially for patients in developing countries.
Computed Tomography Perfusion Combined with Preoperative Embolization for Reducing Intraoperative Blood Loss in Separation Surgery for Thoracolumbar Metastases

Abstract:Study Design. A prospective consecutive case study. Objective: This study aimed to assess the accuracy of computed tomography perfusion (CTP) in evaluating the vascularity of thoracolumbar metastases and to determine the impact of combining CTP with preoperative embolization on reducing intraoperative blood loss during separation surgery. Summary of Background Data. Surgery for thoracolumbar metastases is a complex procedure with the potential for substantial blood loss. Therefore, assessing tumor vascularity before surgery and taking measures to minimize intraoperative blood loss is essential. Methods. A total of 62 patients with thoracolumbar metastases were prospectively enrolled. All patients underwent separation surgery using the posterior approach. Prior to surgery, the vascularity of the metastases was evaluated using CTP. Based on the CTP results, patients were categorized into hypervascular and hypovascular groups. Preoperative angiography and embolization were performed for the hypervascular group. Clinical data were abstracted, including intraoperative blood loss, perioperative complications, VAS score, neurological status, and the accuracy of vascularity evaluation by CTP confirmed by angiography. Chi-square testing was used to compare categorical variables, while independent sample t-tests were employed to compare continuous variables, with paired t-tests were used to assess differences from preoperative to postoperative time points. Results. The mean intraoperative blood loss was 485±167 mL and 455±127.6 mL in the two groups, respectively. The accuracy of vascularity evaluation by CTP was 100%. In the hypervascular group, 80.6% of the patients experienced at least one level of improvement in neurological status, while the hypovascular group had 81.5% of patients with similar improvement. None of the patients experienced neurological deterioration. There was a significant reduction in VAS score in both groups after the operation. Conclusion. The vascularity of thoracolumbar metastases could be accurately evaluated using noninvasive CTP. When combined with preoperative embolization, this approach effectively and safely reduced intraoperative blood loss in the setting of separation surgery.

Abstract:Purpose: Paragangliomas represent a surgical challenge due to their hypervascularization. The preoperative selective embolization of these tumors significantly decreases intraoperative blood loss. However, the literature on preoperative embolization in glomus tumors is limited. The aim of this study is to contribute additional evidence regarding the role of preoperative embolization, as well as to evaluate risks and complications in the treatment of glomus tumors. Methods: A retrospective evaluation of all the embolizations of glomus tumors from 2009 to 2023 was conducted. The primary outcome parameter was the rate of devascularization after embolization and the occurrence of significant perioperative hemorrhages. The secondary outcome was embolization-related complications. Results: Twenty-one embolizations in 20 patients were investigated in the study. In 43% of the cases more than 90% devascularization was achieved by embolization, while in the remaining cases, 80 to 90% devascularization was reached. In one case (5%), significant perioperative bleeding after embolization occurred. In one case (5%), a symptomatic complication occurred periinterventionally due to the brief dislocation of the coaxial and microcatheter into the internal carotid artery (ICA), which led to fresh punctate DWI lesions on the subsequent MRI. No patients developed nerve palsy following embolization. Conclusions: The preoperative embolization of glomus tumors can lead to significant tumor devascularization and a reduction in perioperative bleeding, with a low complication rate.

Abstract:Objective: To study the impact to operation safety of preoperative renal artery embolization for management of ≥10 cm renal cell carcinoma. Methods: The clinical data of 239 cases with ≥10 cm renal cell carcinoma which all had underwent operation in Department of Urology, Peking University First Hospital from January 2002 to December 2014 were retrospectively analyzed. Fifty-three patients underwent preoperative renal artery embolization (therapeutic group) and 186 patients did not (control group). The effect of embolization on operative time, transfusion requirements, hospitalization, ICU stay and perioperative complications were analyzed by comparing the two groups using rank sum test and χ(2) test or Fisher exact test. Results: Comparing the therapeutic group and control group, there was significant difference in tumor location (on the left or right). The mean age, sex, mean primary tumor size, and TNM stage were similar in both groups. Comparing the therapeutic group and control group, there were more open surgeries in therapeutic group (96.2% vs. 82.3%, χ(2)=6.438, P=0.013). There were no significant differences in mean operative time (238 (525) minutes vs. 208 (583) minutes, Z=-2.182, P=0.062). The mean blood transfusion (700 (1 900) ml vs. 925 (8 800) ml, Z=-1.064, P=0.006) had significant difference. The therapeutic group had a longer mean hospitalization (21 (50) days vs. 15 (79) days, Z=-4.322, P=0.000) and higher rate of intensive care unit stay (54.7% vs. 34.4%, χ(2)=6.103, P=0.027). There was no significant difference in perioperative complications between two groups (0 vs.3.2%, P=0.408). Conclusion: Preoperative renal artery embolization in ≥10 cm renal cell carcinoma patients undergoing operation provides benefit in increasing operation safety and reducing perioperative death.

Abstract:Genetic parameters for female reproduction, lamb growth, and lamb scrotal circumference in a composite population of 50% Dorset, 25% Rambouillet and 25% Finnsheep breeding selected for ability to lamb in fall were estimated by REML using various animal models. Traits considered were spring fertility, litter size, weight at weaning (WWT), 90 d (WT90), and 120 d (WT120), and scrotal circumference at weaning (WSC), 90 d (SC90), and 120 d (SC120). Fertility was defined as 1 or 0 for ewes that lambed or failed to lamb, respectively, in fall and averaged .59 for adults (3 yr and older), .45 for second-lambing ewes, .18 for 19-mo-old ewes, and .11 for 12-mo-old ewes. Heritability estimates for fertility varied among models from .07 to .11. Heritability of fall litter size was .10. For body weights, estimates of additive direct, additive maternal, permanent environmental maternal, and litter effects as proportions of phenotypic variance were .04, .10, .05, and .11, respectively, for WWT, .09, .05, .14, and .01 for WT90, and .18, .0, .13, and .06 for WT120. Heritability estimates for WSC, SC90, and SC120 were .15, .25, and .02, respectively. Estimates of genetic correlations among body weights ranged from .77 to unity. Estimates of genetic correlations of fertility with fall litter size, WT90, and SC90 were .56, -.31, and .29, respectively. Genetic correlations of fall litter size with WT90 and SC90 were .33 and .36, respectively. Estimated genetic correlations revealed no major genetic antagonisms with ability to lamb in fall.
