Predicting Depression Risk in Patients With Cancer Using Multimodal Data: Algorithm Development Study

Abstract:Background: Patients with cancer starting systemic treatment programs, such as chemotherapy, often develop depression. A prediction model may assist physicians and health care workers in the early identification of these vulnerable patients. Objective: This study aimed to develop a prediction model for depression risk within the first month of cancer treatment. Methods: We included 16,159 patients diagnosed with cancer starting chemo- or radiotherapy treatment between 2008 and 2021. Machine learning models (eg, least absolute shrinkage and selection operator [LASSO] logistic regression) and natural language processing models (Bidirectional Encoder Representations from Transformers [BERT]) were used to develop multimodal prediction models using both electronic health record data and unstructured text (patient emails and clinician notes). Model performance was assessed in an independent test set (n=5387, 33%) using area under the receiver operating characteristic curve (AUROC), calibration curves, and decision curve analysis to assess initial clinical impact use. Results: Among 16,159 patients, 437 (2.7%) received a depression diagnosis within the first month of treatment. The LASSO logistic regression models based on the structured data (AUROC 0.74, 95% CI 0.71-0.78) and structured data with email classification scores (AUROC 0.74, 95% CI 0.71-0.78) had the best discriminative performance. The BERT models based on clinician notes and structured data with email classification scores had AUROCs around 0.71. The logistic regression model based on email classification scores alone performed poorly (AUROC 0.54, 95% CI 0.52-0.56), and the model based solely on clinician notes had the worst performance (AUROC 0.50, 95% CI 0.49-0.52). Calibration was good for the logistic regression models, whereas the BERT models produced overly extreme risk estimates even after recalibration. There was a small range of decision thresholds for which the best-performing model showed promising clinical effectiveness use. The risks were underestimated for female and Black patients. Conclusions: The results demonstrated the potential and limitations of machine learning and multimodal models for predicting depression risk in patients with cancer. Future research is needed to further validate these models, refine the outcome label and predictors related to mental health, and address biases across subgroups.

Abstract:Background: Major depressive disorder is a common mental disorder affecting 5% of adults worldwide. Early contact with health care services is critical for achieving accurate diagnosis and improving patient outcomes. Key symptoms of major depressive disorder (depression hereafter) such as cognitive distortions are observed in verbal communication, which can also manifest in the structure of written language. Thus, the automatic analysis of text outputs may provide opportunities for early intervention in settings where written communication is rich and regular, such as social media and web-based forums. Objective: The objective of this study was 2-fold. We sought to gauge the effectiveness of different machine learning approaches to identify users of the mass web-based forum Reddit, who eventually disclose a diagnosis of depression. We then aimed to determine whether the time between a forum post and a depression diagnosis date was a relevant factor in performing this detection. Methods: A total of 2 Reddit data sets containing posts belonging to users with and without a history of depression diagnosis were obtained. The intersection of these data sets provided users with an estimated date of depression diagnosis. This derived data set was used as an input for several machine learning classifiers, including transformer-based language models (LMs). Results: Bidirectional Encoder Representations from Transformers (BERT) and MentalBERT transformer-based LMs proved the most effective in distinguishing forum users with a known depression diagnosis from those without. They each obtained a mean F1-score of 0.64 across the experimental setups used for binary classification. The results also suggested that the final 12 to 16 weeks (about 3-4 months) of posts before a depressed user's estimated diagnosis date are the most indicative of their illness, with data before that period not helping the models detect more accurately. Furthermore, in the 4- to 8-week period before the user's estimated diagnosis date, their posts exhibited more negative sentiment than any other 4-week period in their post history. Conclusions: Transformer-based LMs may be used on data from web-based social media forums to identify users at risk for psychiatric conditions such as depression. Language features picked up by these classifiers might predate depression onset by weeks to months, enabling proactive mental health care interventions to support those at risk for this condition.
Mental health at different stages of cancer survival: a natural language processing study of Reddit posts

Abstract:Introduction: The purpose of this study was to use text-based social media content analysis from cancer-specific subreddits to evaluate depression and anxiety-loaded content. Natural language processing, automatic, and lexicon-based methods were employed to perform sentiment analysis and identify depression and anxiety-loaded content. Methods: Data was collected from 187 Reddit users who had received a cancer diagnosis, were currently undergoing treatment, or had completed treatment. Participants were split according to survivorship status into short-term, transition, and long-term cancer survivors. A total of 72524 posts were analyzed across the three cancer survivor groups. Results: The results showed that short-term cancer survivors had significantly more depression-loaded posts and more anxiety-loaded words than long-term survivors, with no significant differences relative to the transition period. The topic analysis showed that long-term survivors, more than other stages of survivorship, have resources to share their experiences with suicidal ideation and mental health issues while providing support to their survivor community. Discussion: The results indicate that Reddit texts seem to be an indicator of when the stressor is active and mental health issues are triggered. This sets the stage for Reddit to become a platform for screening and first-hand intervention delivery. Special attention should be dedicated to short-term survivors.
Revealing the impact of social circumstances on the selection of cancer therapy through natural language processing of social work notes

Abstract:We aimed to investigate the impact of social circumstances on cancer therapy selection using natural language processing to derive insights from social worker documentation. We developed and employed a Bidirectional Encoder Representations from Transformers (BERT) based approach, using a hierarchical multi-step BERT model (BERT-MS) to predict the prescription of targeted cancer therapy to patients based solely on documentation by clinical social workers. Our corpus included free-text clinical social work notes, combined with medication prescription information, for all patients treated for breast cancer. We conducted a feature importance analysis to pinpoint the specific social circumstances that impact cancer therapy selection. Using only social work notes, we consistently predicted the administration of targeted therapies, suggesting systematic differences in treatment selection exist due to non-clinical factors. The UCSF-BERT model, pretrained on clinical text at UCSF, outperformed other publicly available language models with an AUROC of 0.675 and a Macro F1 score of 0.599. The UCSF BERT-MS model, capable of leveraging multiple pieces of notes, surpassed the UCSF-BERT model in both AUROC and Macro-F1. Our feature importance analysis identified several clinically intuitive social determinants of health (SDOH) that potentially contribute to disparities in treatment. Our findings indicate that significant disparities exist among breast cancer patients receiving different types of therapies based on social determinants of health. Social work reports play a crucial role in understanding these disparities in clinical decision-making.

Abstract:Automated methods have been widely used to identify and analyze mental health conditions (e.g., depression) from various sources of information, including social media. Yet, deployment of such models in real-world healthcare applications faces challenges including poor out-of-domain generalization and lack of trust in black box models. In this work, we propose approaches for depression detection that are constrained to different degrees by the presence of symptoms described in PHQ9, a questionnaire used by clinicians in the depression screening process. In dataset-transfer experiments on three social media datasets, we find that grounding the model in PHQ9's symptoms substantially improves its ability to generalize to out-of-distribution data compared to a standard BERT-based approach. Furthermore, this approach can still perform competitively on in-domain data. These results and our qualitative analyses suggest that grounding model predictions in clinically-relevant symptoms can improve generalizability while producing a model that is easier to inspect.

Abstract:Early risk detection of mental illnesses has a massive positive impact upon the well-being of people. The eRisk workshop has been at the forefront of enabling interdisciplinary research in developing computational methods to automatically estimate early risk factors for mental issues such as depression, self-harm, anorexia and pathological gambling. In this paper, we present the contributions of the BLUE team in the 2021 edition of the workshop, in which we tackle the problems of early detection of gambling addiction, self-harm and estimating depression severity from social media posts. We employ pre-trained BERT transformers and data crawled automatically from mental health subreddits and obtain reasonable results on all three tasks.

Abstract:The limited availability of psychologists necessitates efficient identification of individuals requiring urgent mental healthcare. This study explores the use of Natural Language Processing (NLP) pipelines to analyze text data from online mental health forums used for consultations. By analyzing forum posts, these pipelines can flag users who may require immediate professional attention. A crucial challenge in this domain is data privacy and scarcity. To address this, we propose utilizing readily available curricular texts used in institutes specializing in mental health for pre-training the NLP pipelines. This helps us mimic the training process of a psychologist. Our work presents CASE-BERT that flags potential mental health disorders based on forum text. CASE-BERT demonstrates superior performance compared to existing methods, achieving an f1 score of 0.91 for Depression and 0.88 for Anxiety, two of the most commonly reported mental health disorders. Our code and data are publicly available.

Symptom-BERT: Enhancing Cancer Symptom Detection in EHR Clinical Notes

Abstract:Context: Extracting cancer symptom documentation allows clinicians to develop highly individualized symptom prediction algorithms to deliver symptom management care. Leveraging advanced language models to detect symptom data in clinical narratives can significantly enhance this process. Objective: This study uses a pretrained large language model to detect and extract cancer symptoms in clinical notes. Methods: We developed a pretrained language model to identify cancer symptoms in clinical notes based on a clinical corpus from the Enterprise Data Warehouse for Research at a healthcare system in the Midwestern United States. This study was conducted in 4 phases:1 pretraining a Bio-Clinical BERT model on one million unlabeled clinical documents,2 fine-tuning Symptom-BERT for detecting 13 cancer symptom groups within 1112 annotated clinical notes,3 generating 180 synthetic clinical notes using ChatGPT-4 for external validation, and4 comparing the internal and external performance of Symptom-BERT against a non-pretrained version and six other BERT implementations. Results: The Symptom-BERT model effectively detected cancer symptoms in clinical notes. It achieved results with a micro-averaged F1-score of 0.933, an AUC of 0.929 internally, and 0.831 and 0.834 externally. Our analysis shows that physical symptoms, like Pruritus, are typically identified with higher performance than psychological symptoms, such as anxiety. Conclusion: This study underscores the transformative potential of specialized pretraining on domain-specific data in boosting the performance of language models for medical applications. The Symptom-BERT model's exceptional efficacy in detecting cancer symptoms heralds a groundbreaking stride in patient-centered AI technologies, offering a promising path to elevate symptom management and cultivate superior patient self-care outcomes.
Detection of Depression-Related Posts in Reddit Social Media Forum

Abstract:Depression is viewed as the largest contributor to global disability and a major reason for suicide. It has an impact on the language usage reflected in the written text. The key objective of our study is to examine Reddit users’ posts to detect any factors that may reveal the depression attitudes of relevant online users. For such purpose, we employ the Natural Language Processing (NLP) techniques and machine learning approaches to train the data and evaluate the efficiency of our proposed method. We identify a lexicon of terms that are more common among depressed accounts. The results show that our proposed method can significantly improve performance accuracy. The best single feature is bigram with the Support Vector Machine (SVM) classifier to detect depression with 80% accuracy and 0.80 F1 scores. The strength and effectiveness of the combined features (LIWC+LDA+bigram) are most successfully demonstrated with the Multilayer Perceptron (MLP) classifier resulting in the top performance for depression detection reaching 91% accuracy and 0.93 F1 scores. According to our study, better performance improvement can be achieved by proper feature selections and their multiple feature combinations.

Abstract:Depression constitutes a significant mental health condition, impacting an individual's emotional state, thought processes, and ability to carry out everyday tasks. Depression is defined by ongoing feelings of sadness, diminished interest in previously enjoyed activities, alterations in hunger, sleep disturbances, decreased vitality, and challenges with focus. The impact of depression extends beyond the individual, affecting society at large through decreased productivity and higher healthcare costs. In the realm of social media, users often express their thoughts and emotions through posts, which can provide insightful data for identifying patterns of depression. This research aims to detect depression early by analyzing social media user content with machine learning techniques. We have built advanced machine learning models using a benchmark depression database containing 20,000 tagged tweets from user profiles identified as depressed or non-depressed. We are introducing an innovative BERT-RF feature engineering method that extracts Contextualized Embeddings and Probabilistic Features from textual input. The Bidirectional Encoder Representations from Transformers (BERT) model, based on the Transformer architecture, is used to extract Contextualized Embedding features. These features are then fed into a random forest model to generate class probabilistic features. These prominent features aid in enhancing the identification of depression from social media. In order to classify tweets using the features derived from the BERT-RF features selection step, we have used five popular classifiers: Random Forest (RF), Multilayer Perceptron (MLP), K-Neighbors Classifier (KNC), Logistic Regression (LR), and Long Short-Term Memory (LSTM). Evaluation experiments show that our approach, using BERT-RF for feature engineering, enables the Logistic Regression model to outperform state-of-the-art methods with a high accuracy score of 99%. We have validated the results through k-fold cross-validation and statistical T-tests. We achieved 99% k-fold accuracy during the validation of the proposed approach. This research contributes significantly to computational linguistics and mental health analytics by providing a robust approach to the early detection of user depression from social media content.

Automated Depression Detection in Text Data: Leveraging Lexical Features, Phonesthemes Embedding, and RoBERTa Transformer Model

Abstract:Depression is a prevalent mental disorder characterized by persistent sadness, lack of interest, and diminished pleasure. Detecting depression is crucial for timely intervention and support. In this paper, we address the task of depression detection in text data, focusing on binary classification and regression. We present our approach, leveraging a dataset comprising labeled messages from Telegram groups related to mental disorders. We begin by exploring the existing literature on depression detection, highlighting the challenges faced and the methods employed. Our approach involves data pre-processing, lexical feature extraction, phonesthemes embedding, and using the RoBERTa transformer model. We achieved promising results in the training phase through rigorous experimentation and model refinement. However, we encountered challenges upon evaluating our approach in the MentalRiskEs evaluation. We identified areas for improvement, particularly in latency and speed of detection for real-time monitoring of depression-related risks. This research contributes to the ongoing efforts in automating depression detection and provides insights into the potential of text analysis techniques for mental health assessment. We remain committed to further enhancing our methodology and advancing the field to improve the well-being of individuals affected by depression.

Diagnostic performance of amplified Mycobacterium tuberculosis direct test with cerebrospinal fluid, other nonrespiratory, and respiratory specimens

Abstract:The Gen-Probe Amplified Mycobacterium tuberculosis Direct Test (MTD) was adapted to be used for cerebrospinal fluid (CSF) and a large variety of other nonrespiratory specimens. Standardized with artificially spiked dilution series of CSF, the modified MTD procedure consists of (i) increasing the amount of sample 10-fold, (ii) pretreating the specimen with a detergent, and (iii) increasing the amplification time from 2 to 3 h. Performance of MTD in a clinical mycobacteriology laboratory was tested over an extended period of time, involving a total of 322 nonrespiratory as well as 1,117 respiratory specimens from 998 patients. Results from MTD were compared with those from microscopy, culture, analysis of tuberculostearic acid by gas-liquid chromatography-mass spectrometry (CSF only), and the final clinical diagnosis. When MTD results were compared with resolved data, the sensitivity, specificity, and positive and negative predictive values for MTD were 93.1, 97.7, 90.0, and 98.5%, respectively, for nonrespiratory specimens and 86.6, 96.4, 76.8, and 98.1%, respectively, for respiratory specimens. Our data demonstrate that (i) MTD is a robust, highly sensitive and specific technique for the rapid detection of M. tuberculosis complex in all types of clinical specimens, (ii) there was no statistically significant difference (P > 0.005) in sensitivity and specificity for nonrespiratory compared with respiratory specimens, and (iii) repeating all MTDs which yield a result between 30,000 and 200,000 relative light units would help prevent a large number of false positives and, thus, enhance test specificity.
Detecting Reddit Users with Depression Using a Hybrid Neural Network SBERT-CNN

Abstract:Depression is a widespread mental health issue, affecting an estimated 3.8% of the global population. It is also one of the main contributors to disability worldwide. Recently it is becoming popular for individuals to use social media platforms (e.g., Reddit) to express their difficulties and health issues (e.g., depression) and seek support from other users in online communities. It opens great opportunities to automatically identify social media users with depression by parsing millions of posts for potential interventions. Deep learning methods have begun to dominate in the field of machine learning and natural language processing (NLP) because of their ease of use, efficient processing, and state-of-the-art results on many NLP tasks. In this work, we propose a hybrid deep learning model which combines a pretrained sentence BERT (SBERT) and convolutional neural network (CNN) to detect individuals with depression with their Reddit posts. The sentence BERT is used to learn the meaningful representation of semantic information in each post. CNN enables the further transformation of those embeddings and the temporal identification of behavioral patterns of users. We trained and evaluated the model performance to identify Reddit users with depression by utilizing the Self-reported Mental Health Diagnoses (SMHD) data. The hybrid deep learning model achieved an accuracy of 0.86 and an F1 score of 0.86 and outperformed the state-of-the-art documented result (F1 score of 0.79) by other machine learning models in the literature. The results show the feasibility of the hybrid model to identify individuals with depression. Although the hybrid model is validated to detect depression with Reddit posts, it can be easily tuned and applied to other text classification tasks and different clinical applications.

CancerBERT: a BERT model for Extracting Breast Cancer Phenotypes from Electronic Health Records

Abstract:Accurate extraction of breast cancer patients' phenotypes is important for clinical decision support and clinical research. Current models do not take full advantage of cancer domain-specific corpus, whether pre-training Bidirectional Encoder Representations from Transformer model on cancer-specific corpus could improve the performances of extracting breast cancer phenotypes from texts data remains to be explored. The objective of this study is to develop and evaluate the CancerBERT model for extracting breast cancer phenotypes from clinical texts in electronic health records. This data used in the study included 21,291 breast cancer patients diagnosed from 2010 to 2020, patients' clinical notes and pathology reports were collected from the University of Minnesota Clinical Data Repository (UMN). Results: About 3 million clinical notes and pathology reports in electronic health records for 21,291 breast cancer patients were collected to train the CancerBERT model. 200 pathology reports and 50 clinical notes of breast cancer patients that contain 9,685 sentences and 221,356 tokens were manually annotated by two annotators. 20% of the annotated data was used as a test set. Our CancerBERT model achieved the best performance with macro F1 scores equal to 0.876 (95% CI, 0.896-0.902) for exact match and 0.904 (95% CI, 0.896-0.902) for the lenient match. The NER models we developed would facilitate the automated information extraction from clinical texts to further help clinical decision support. Conclusions and Relevance: In this study, we focused on the breast cancer-related concepts extraction from EHR data and obtained a comprehensive annotated dataset that contains 7 types of breast cancer-related concepts. The CancerBERT model with customized vocabulary could significantly improve the performance for extracting breast cancer phenotypes from clinical texts.

Abstract:Editor, I read the recent case report on nonsecretory multiple myeloma with a great interest.1 Middela et al. noted that nonsecretory multiple myeloma is a case with diagnostic dilemma and “a high index of suspicion must be borne in mind when excluding multiple myeloma as a cause of pain, pathological fracture or lytic lesion”.1 Indeed, it is accepted that diagnosis of nonsecretory multiple myeloma is very difficult. In this case, although there are some soft signs showing myeloma nature (decreased normal immunoglobulin and lytic bone lesion), the lack of abnormal secreted protein brings the diagnostic difficulty. Recently, the new laboratory technique for detecting of low level of abnormal secreted protein has been developed and can be the new hope in the fast and easy diagnosis of nonsecretory multiple myeloma. The “FreeliteTM test” is an example. This test helps determine low level of abnormal free light chain and has been accepted as the new tool for diagnosis of non secretory multiple myeloma.2,3

Abstract:With the current advancement in the Internet, there has been a growing demand for building intelligent and smart systems that can efficiently address the detection of health-related problems on social media, such as the detection of depression and anxiety. These types of systems, which are mainly dependent on machine learning techniques, must be able to deal with obtaining the semantic and syntactic meaning of texts posted by users on social media. The data generated by users on social media contains unstructured and unpredictable content. Several systems based on machine learning and social media platforms have recently been introduced to identify health-related problems. However, the text representation and deep learning techniques employed provide only limited information and knowledge about the different texts posted by users. This is owing to a lack of long-term dependencies between each word in the entire text and a lack of proper exploitation of recent deep learning schemes. In this paper, we propose a novel framework to efficiently and effectively identify depression and anxiety-related posts while maintaining the contextual and semantic meaning of the words used in the whole corpus when applying bidirectional encoder representations from transformers (BERT). In addition, we propose a knowledge distillation technique, which is a recent technique for transferring knowledge from a large pretrained model (BERT) to a smaller model to boost performance and accuracy. We also devised our own data collection framework from Reddit and Twitter, which are the most common social media sites. Finally, we employed word2vec and BERT with Bi-LSTM to effectively analyze and detect depression and anxiety signs from social media posts. Our system surpasses other state-of-the-art methods and achieves an accuracy of 98% using the knowledge distillation technique.

A textual-based featuring approach for depression detection using machine learning classifiers and social media texts

Abstract:<p>Depression is one of the leading causes of suicide worldwide. However, a large percentage of cases of depression go undiagnosed and, thus, untreated. Previous studies have found that messages posted by individuals with major depressive disorder on social media platforms can be analysed to predict if they are suffering, or likely to suffer, from depression. This study aims to determine whether machine learning could be effectively used to detect signs of depression in social media users by analysing their social media posts—especially when those messages do not explicitly contain specific keywords such as 'depression' or 'diagnosis'. To this end, we investigate several text preprocessing and textual-based featuring methods along with machine learning classifiers, including single and ensemble models, to propose a generalised approach for depression detection using social media texts. We first use two public, labelled Twitter datasets to train and test the machine learning models, and then another three non-Twitter depression-class-only datasets (sourced from Facebook, Reddit, and an electronic diary) to test the performance of our trained models against other social media sources. Experimental results indicate that the proposed approach is able to effectively detect depression via social media texts even when the training datasets do not contain specific keywords (such as 'depression' and 'diagnose'), as well as when unrelated datasets are used for testing.</p>

Cutaneous Surgeons Cannot Predict Blood‐Thinner Status by Intraoperative Visual Inspection

Abstract:&NA; Cutaneous surgeons cannot predict blood‐thinner status by intraoperative visual inspection. Many surgeons believe they can discern whether a patient is taking an anticoagulant or a platelet inhibitor (blood thinner) by visual inspection of intraoperative oozing. However, there is little objective evidence to support this strongly held belief. The authors’ objective was to determine whether visual inspection of intraoperative oozing during cutaneous surgery is accurate in identifying use of blood thinners. Blinded physician evaluators observed intraoperative oozing in 110 patients having cutaneous excisional surgery, rated the amount of oozing, and judged the likelihood that the patient was taking a blood‐thinning agent. On the basis of the impressions of the most senior evaluator, 43 patients having used aspirin in the past 14 days or warfarin or vitamin E in the past 2 days, seven were judged as definitely or probably taking blood‐thinning agents (sensitivity, 16.3 percent; 95 percent confidence interval, 6.8 to 30.7 percent). Of 67 patients who did not report recent use of a blood‐thinning agent, just 11 were judged as definitely or probably taking blood‐thinning agents (false‐positive rate, 16.4 percent; 95 percent confidence interval, 8.5 to 27.5 percent). The level of training of the physicians doing the judging did not affect diagnostic sensitivity. Only 10 of the 110 patients (9.1 percent) were assessed as exhibiting excessive oozing, and of those, only four (40 percent) were actually taking a blood thinner. Results were similar when only patients who were taking aspirin or warfarin were analyzed. Thus, contrary to a commonly and strongly held belief, visual inspection of intraoperative oozing during cutaneous excisional surgery correlates poorly with blood‐thinner use by patients. The data add further evidence that use of blood thinners does not have an objectively measurable adverse effect during cutaneous surgery.
Depression Detection on Social Media with Large Language Models

Abstract:Depression harms. However, due to a lack of mental health awareness and fear of stigma, many patients do not actively seek diagnosis and treatment, leading to detrimental outcomes. Depression detection aims to determine whether an individual suffers from depression by analyzing their history of posts on social media, which can significantly aid in early detection and intervention. It mainly faces two key challenges: 1) it requires professional medical knowledge, and 2) it necessitates both high accuracy and explainability. To address it, we propose a novel depression detection system called DORIS, combining medical knowledge and the recent advances in large language models (LLMs). Specifically, to tackle the first challenge, we proposed an LLM-based solution to first annotate whether high-risk texts meet medical diagnostic criteria. Further, we retrieve texts with high emotional intensity and summarize critical information from the historical mood records of users, so-called mood courses. To tackle the second challenge, we combine LLM and traditional classifiers to integrate medical knowledge-guided features, for which the model can also explain its prediction results, achieving both high accuracy and explainability. Extensive experimental results on benchmarking datasets show that, compared to the current best baseline, our approach improves by 0.036 in AUPRC, which can be considered significant, demonstrating the effectiveness of our approach and its high value as an NLP application.

Using Social Media to Help Understand Patient-Reported Health Outcomes of Post-COVID-19 Condition: Natural Language Processing Approach

Abstract:Background: While scientific knowledge of post-COVID-19 condition (PCC) is growing, there remains significant uncertainty in the definition of the disease, its expected clinical course, and its impact on daily functioning. Social media platforms can generate valuable insights into patient-reported health outcomes as the content is produced at high resolution by patients and caregivers, representing experiences that may be unavailable to most clinicians. Objective: In this study, we aimed to determine the validity and effectiveness of advanced natural language processing approaches built to derive insight into PCC-related patient-reported health outcomes from social media platforms Twitter and Reddit. We extracted PCC-related terms, including symptoms and conditions, and measured their occurrence frequency. We compared the outputs with human annotations and clinical outcomes and tracked symptom and condition term occurrences over time and locations to explore the pipeline's potential as a surveillance tool. Methods: We used bidirectional encoder representations from transformers (BERT) models to extract and normalize PCC symptom and condition terms from English posts on Twitter and Reddit. We compared 2 named entity recognition models and implemented a 2-step normalization task to map extracted terms to unique concepts in standardized terminology. The normalization steps were done using a semantic search approach with BERT biencoders. We evaluated the effectiveness of BERT models in extracting the terms using a human-annotated corpus and a proximity-based score. We also compared the validity and reliability of the extracted and normalized terms to a web-based survey with more than 3000 participants from several countries. Results: UmlsBERT-Clinical had the highest accuracy in predicting entities closest to those extracted by human annotators. Based on our findings, the top 3 most commonly occurring groups of PCC symptom and condition terms were systemic (such as fatigue), neuropsychiatric (such as anxiety and brain fog), and respiratory (such as shortness of breath). In addition, we also found novel symptom and condition terms that had not been categorized in previous studies, such as infection and pain. Regarding the co-occurring symptoms, the pair of fatigue and headaches was among the most co-occurring term pairs across both platforms. Based on the temporal analysis, the neuropsychiatric terms were the most prevalent, followed by the systemic category, on both social media platforms. Our spatial analysis concluded that 42% (10,938/26,247) of the analyzed terms included location information, with the majority coming from the United States, United Kingdom, and Canada. Conclusions: The outcome of our social media-derived pipeline is comparable with the results of peer-reviewed articles relevant to PCC symptoms. Overall, this study provides unique insights into patient-reported health outcomes of PCC and valuable information about the patient's journey that can help health care providers anticipate future needs.
