Abstract:Background: The anterior communicating artery complex, which is composed of anterior cerebral artery (ACA), anterior communicating artery (ACoA), and recurrent artery of Heubner, accounts for the anterior half of the circle of Willis and is a favorite site of aneurysm formation; ACoA accounts for most of intracranial aneurysms. Few authors have studied the microanatomy of the ACA in detail using different specimens; none of these studies involve Chinese specimen. We have studied this area on adult Chinese brain with the aid of surgical microscope. Methods: The anterior communicating artery complexes of 90 cerebral hemispheres from 45 formalin-fixed cadavers were dissected under microscope. Results: The length and diameter of the recurrent artery and their perforating branches were measured. Their supplying areas, the adjacent structures, and their variations were also recorded. Conclusion: The cerebral vessels of native people have many variations in structure. The result of this study is quite different from that of the foreign literatures. The microanatomy of cerebral vessels also provides important data in its morphology for neurosurgeons. (c) 2006 Elsevier Inc. All rights reserved.

Abstract:This in vitro study aims at clarifying the relation between the oscillatory flow of cerebrospinal fluid (CSF) in the cerebral aqueduct, a narrow conduit connecting the third and fourth ventricles, and the corresponding interventricular pressure difference. Dimensional analysis is used in designing an anatomically correct scaled model of the aqueduct flow, with physical similarity maintained by adjusting the flow frequency and the properties of the working fluid. The time-varying pressure difference across the aqueduct corresponding to a given oscillatory flow rate is measured in parametric ranges covering the range of flow conditions commonly encountered in healthy subjects. Parametric dependences are delineated for the time-averaged pressure fluctuations and for the phase lag between the transaqueductal pressure difference and the flow rate, both having clinical relevance. The results are validated through comparisons with predictions obtained with a previously derived computational model. The parametric quantification in this study enables the derivation of a simple formula for the relation between the transaqueductal pressure and the stroke volume. This relationship can be useful in the quantification of transmantle pressure differences based on non-invasive magnetic-resonance-velocimetry measurements of aqueduct flow for investigation of CSF-related disorders.

Anatomical Features of the Ventricular System Relevant to Endoscopic Surgery for Hydrocephalus

Abstract:In endoscopic surgery for hydrocephalus and associated intraventricular lesions, a thorough understanding of the required microsurgical anatomy is paramount. Endoscopic procedures in hydrocephalus treatment typically fall into two categories: ventriculocisternostomy and ventriculostomy for obstructive hydrocephalus. In the former, precise knowledge of intraventricular structures, such as the configuration of the ventricles and the path of internal vessels within them, is essential. In the latter, a comprehensive grasp of neural pathways, neural nuclei, and especially venous pathways beneath the ventricular wall is crucial for surgical success. Given that many cases exhibit deviations from normal anatomy, careful examination of preoperative images and a solid understanding of anatomical landmarks during surgery are indispensable. This is particularly critical in endoscopic procedures, which may lack stereoscopic vision, underscoring the importance of acquiring visual cues during the surgical intervention.
Intradural Subtemporal Keyhole Approach with Neuronavigational Assistance to the Petroclival and Ventrolateral Brainstem Regions: Quantitative Analysis of Anatomic Exposure and Surgical Freedom.

Abstract:OBJECTIVE:The purpose of this study was to evaluate the advantages of endoscopy and navigational assistance in the intradural subtemporal keyhole approach and the intradural Kawase approach by studying anatomic exposure and surgical freedom in the posterior cranial fossa.METHODS:Twenty endoscopic intradural subtemporal keyhole approaches (EISKA) were performed on 10 cadaveric heads. An intradural Kawase approach and a navigation-assisted intradural Kawase approach were then carried out on a random side of each specimen. Related anatomic structures were observed through a 0° endoscope and a microscope. Anatomic exposure and surgical freedom were measured by transparent graph paper and analyzed.RESULTS:Compared with the microsurgical technique, the superior, inferior, and medial limits of the endoscopic anatomic exposure increased by 2.9, 15.65, and 10.2 mm, whereas the surgical freedom increased by 2.9, 7.55, and 6 mm (P < 0.05) in the intradural subtemporal keyhole approach. In the intradural Kawase approach and the navigation-assisted intradural Kawase approach, the endoscopic anatomic exposure and surgical freedom also increased (P < 0.05). Using a frameless navigational device, the inferior limit of the anatomic exposure increased 3.8 mm by endoscopy and 3.5 mm by microscopy, whereas the surgical freedom increased by 2.7 and 2.2 mm, respectively (P < 0.05).CONCLUSIONS:The EISKA provides greater anatomic exposure and surgical freedom primarily in the superior, inferior, and medial directions of the brainstem region. Maximum anatomic exposure and surgical freedom of the posterior cranial fossa was obtained by navigational assistance with fewer complications.
Approaching Intradural Lesions of the Anterior Foramen Magnum and Craniocervical Junction: Anatomical Comparison of the Open Posterolateral and Anterior Extended Endonasal Endoscopic Approaches

Abstract:Background: Lesions of the foramen magnum (FM) and craniocervical junction area are traditionally managed surgically through anterior, anterolateral, and posterolateral skull-base approaches. This anatomical study aimed to compare the usefulness of a modified extended endoscopic approach, the so-called far-medial endonasal approach (FMEA), versus the traditional posterolateral far-lateral approach (FLA). Methods: Ten fixed silicon-injected heads specimens were used in the Skull Base ENT-Neurosurgery Laboratory of the University Hospital of Strasbourg, France. A total of 20 FLAs and 10 FMEAs were realized. A high-resolution computed tomography scan was performed for quantitative analysis of the different approaches. The analysis aimed to estimate the extent of surgical exposure and freedom of movement (maneuverability) through the operating channel using a polygonal surface model to obtain a morphometric estimation of the area of interest (surface and volume) on postdissection computed tomography scans using Slicer 3D software. Results: FMEA allows for a more direct route to the anterior FM, with wider brainstem exposure compared with the FLA and an excellent visualization of all anterior midline structures. The limitations of the FMEA include the deep and narrow surgical corridor and difficulty in reaching lesions located laterally over the jugular foramen and hypoglossal canal. Conclusions: The FMEA and FLA are both effective surgical routes to reach FM and craniocervical junction lesions. Modern skull base surgeons should have a good command of both because they appear complementary. This anatomical study provides the tools for comprehensive preoperative evaluations and selection of the most appropriate surgical approach.
Endoscopically Assisted Targeted Keyhole Retrosigmoid Approaches for Microvascular Decompression: Quantitative Anatomic Study

Abstract:Objective: We describe and quantitatively assess minimally invasive keyhole retrosigmoid approaches targeted to the upper, middle, and lower cranial nerve (CN) complexes of the cerebellopontine angle (CPA). Methods: Anatomic dissections were performed on 10 sides of 5 fixed, silicone-injected cadaver heads. Surgical views through various trajectories were assessed in endoscopic videos and 3-dimensional (3D) interactive virtual reality microscope views. Surgical freedom and angles of attack to the proximal and distal areas of CN complexes of the CPA were compared among upper and lower keyholes and conventional retrosigmoid craniotomy using neuronavigation. Results: Compared with keyholes, the conventional approach had superior surgical freedom to most areas except for the distal CN V, the root of CN VII, and the root of CN IX, where differences were not significant. The conventional retrosigmoid approach provided a larger horizontal angle of attack than either the upper or lower keyholes for all selected areas; however, the vertical angles of attack were not different. Splitting the petrosal fissure resulted in a significant increase in the vertical angle of attack to the root zones of CNs V and VII but not to the distal areas of these nerves or CN IX. Illustrative cases of endoscope-assisted keyhole retrosigmoid approaches for the treatment of trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia are presented. Conclusions: Targeted keyhole retrosigmoid approaches require detailed understanding of the 3D anatomy of the CPA to create appropriate locations of corridors, including skin incisions and keyholes. Endoscope assistance complements the standard microsurgical technique by maximizing the visualization and identification of the delicate neurovascular structures.

Abstract:Background: The suprageniculate fossa (SGF) is located between the geniculate ganglion, the middle cranial fossa (MCF) and the anterior semicircular canal (ASCC). An endoscopic transcanal approach has been recently proposed to treat the different lesions in this area. The aim of the study is to describe the anatomical pathway of this approach by measuring the dimensions of its boundaries while checking their correlation with the pneumatization of the SGF area. Methods: This is a retrospective anatomical analysis of Cone Beam CT scans of 80 patients, for a total of 160 temporal bones analyzed. Two checkpoints were measured for the SGF route, as an internal and an external window. These are triangles between the MCF dura, the geniculate ganglion and the ASCC on parasagittal and axial planes. The pneumatization of the SGF was also assessed, classified and correlated with the measured dimensions. Results: The depth of the SGF was 7.5 ± 1.8 mm. The width of the external window was 7.5 ± 1.9, 5.6 ± 2.4 and 1.6 ± 1.6 mm for the posterior, middle and anterior points of measurement, respectively. The height of the internal window was 7.6 ± 1.2, 4.5 ± 1.5 and 1.7 ± 1.7 mm for the posterior, middle and anterior points of measurement, respectively. Type A pneumatization was found in 87 cases, type B in 34 and type C in 39. The degree of pneumatization directly correlated to the depth and height of the fossa. Conclusions: The suprageniculate approach route is defined by the internal and external windows which should be evaluated during a pre-surgery imaging assessment. The detailed anatomy of the approach and the novel classification of the pneumatization of the SGF are here described which may be useful to plan a safer procedure with minimal complications.

Abstract:Objective: To test whether there are significant differences in the morphologic features of the enlarged vestibular aqueduct (EVA) and its anatomic relationships with adjacent structures between EVA patients with and without other inner ear malformations.Methods: The morphology of vestibular aqueduct, volume of vestibule, and anatomic location of facial nerve (FN) were compared among 3 groups. Group A, 18 control subjects; group B, 32 EVA patients without other inner ear malformations; group C, 14 EVA patients with other inner ear malformations.Results: The isthmus of the aqueduct and vestibular volume in group C were larger than those in group B (P < 0.05). Positive correlations were identified between the length of the aqueduct and the vestibular volume in groups B and C. The vertical FN segment in group C located more anteriorly than that in groups A and B (P < 0.05).Conclusions: Compared with EVA patients without other inner ear malformations, the aqueduct, and vestibule in EVA patients with other inner ear malformations are larger, the vertical FN segment locates more anteriorly.

Abstract:The upper end of the central canal of the human spinal cord has been repeatedly implicated in the pathogenesis of various diseases, yet its precise normal position in the medulla oblongata and upper cervical spinal cord remains unclear. The purpose of this study is to describe the anatomy of the upper end of the central canal with quantitative measurements and a three‐dimensional (3D) model. Seven formalin‐embalmed human brainstems were included, and the central canal was identified in serial axial histological sections using epithelial membrane antigen antibody staining. Measurements included the distances between the central canal (CC) and the anterior medullary fissure (AMF) and the posterior medullary sulcus (PMS). The surface and perimeter of the CC and the spinal cord were calculated, and its anterior–posterior and maximum lateral lengths were measured for 3D modeling. The upper end of the CC was identified in six specimens, extending from the apertura canalis centralis (ACC) to its final position in the cervical cord. Positioned on the midline, it reaches its final location approximately 15 mm below the obex. No specimen showed canal dilatation, focal stenosis, or evidence of syringomyelia. At 21 mm under the ACC in the cervical cord, the median distance from the CC to the AMF was 3.14 (2.54–3.15) mm and from the CC to the PMS was 5.19 (4.52–5.43) mm, with a progressive shift from the posterior limit to the anterior third of the cervical spinal cord. The median area of the CC was consistently less than 0.1 mm2. The upper end of the CC originates at the ACC, in the posterior part of the MO, and reaches its normal position in the anterior third of the cervical spinal cord less than 2 cm below the obex. Establishing the normal position of the upper end of this canal is crucial for understanding its possible involvement in cranio‐cervical junction pathologies.

[Application of uncertainty assessment in NIR quantitative analysis of traditional Chinese medicine].

Abstract:The near infrared (NIR) spectra of Liuyi San samples were collected during the mixing process and the quantitative models by PLS (partial least squares) method were generated for the quantification of the concentration of glycyrrhizin. The PLS quantitative model had good calibration and prediction performances (r(cal) 0.998 5, RMSEC = 0.044 mg · g(-1); r(val) = 0.947 4, RMSEP = 0.124 mg · g(-1)), indicating that NIR spectroscopy can be used as a rapid determination method of the concentration of glycyrrhizin in Liuyi San powder. After the validation tests were designed, the Liao-Lin-Iyer approach based on Monte Carlo simulation was used to estimate β-content-γ-confidence tolerance intervals. Then the uncertainty was calculated, and the uncer- tainty profile was drawn. The NIR analytical method was considered valid when the concentration of glycyrrhizin is above 1.56 mg · g(-1) since the uncertainty fell within the acceptable limits (λ = ± 20%). The results showed that uncertainty assessment can be used in NIR quantitative models of glycyrrhizin for different concentrations and provided references for other traditional Chinese medicine to finish the uncertainty assessment using NIR quantitative analysis.
A model for the oscillatory flow in the cerebral aqueduct

Abstract:This paper addresses the pulsating motion of cerebrospinal fluid in the aqueduct of Sylvius, a slender canal connecting the third and fourth ventricles of the brain. Specific attention is given to the relation between the instantaneous values of the flow rate and the interventricular pressure difference, needed in clinical applications to enable indirect evaluations of the latter from direct magnetic-resonance measurements of the former. An order-of-magnitude analysis accounting for the slenderness of the canal is used in simplifying the flow description. The boundary-layer approximation is found to be applicable in the slender canal, where the oscillating flow is characterized by stroke lengths comparable to the canal length and periods comparable to the transverse diffusion time. By way of contrast, the flow in the non-slender opening regions connecting the aqueduct with the two ventricles is found to be inviscid and quasi-steady in the first approximation. The resulting simplified description is validated by comparison with results of direct numerical simulations. The model is used to investigate the relation between the interventricular pressure and the stroke length, in parametric ranges of interest in clinical applications.

Endoscopic Lateral and Superior Cerebellar Keyhole Approach to the Anterior and Middle Incisural Space and Meckel Cave: an Anatomic Study.

Abstract:OBJECTIVE:We sought to assess the feasibility of endoscopic lateral and superior cerebellar keyhole approach for exposure of the anterior and middle incisural space and Meckel cave. METHODS:The endoscopic lateral and superior cerebellar keyhole approach was performed in 6 cadaveric heads (12 sides) using 0- and 30-degree endoscopes, respectively. The anatomic structures for this approach to the anterior and middle incisural space and Meckel cave were observed. RESULTS:By grinding out the suprameatal tubercle and petrous apex and incising the tentorium, the anatomic structures in the anterior incisural space were visualized. The mean area exposed with a 0-degree endoscope in the anterior incisural space was 212.50 ± 6.04 mm2, significantly less than that exposed with a 30-degree endoscope (233.83 ± 8.72 mm2) (P < 0.05). The anatomic distance of the Meckel cave in the depth was the same between a 0-degree endoscope and a 30-degree endoscope; however, the distance in the width was 9.48 and 12.32 mm, respectively (P < 0.01). The area of petrous window grinded by a 30-degree endoscope was only increased by 5.83 mm2, compared with a 0-degree endoscope (P > 0.05). CONCLUSIONS:This approach provides access to the anterior and middle incisural space and Meckel cave, which is feasible to clearly expose the anatomic structures in those regions with minimal invasiveness. Additionally, better visualization and surgical space can be achieved under a 30-degree endoscope.
Navigating a Carotico-Clinoid Foramen and an Interclinoidal Bridge in the Endonasal Endoscopic Approach: An Anatomical and Technical Note

Abstract:Objective The carotico-clinoid foramen and interclinoid bridge are two anatomical variants of the sellar region. If these anatomical variants go unrecognized and are not managed safely by the surgeon during expanded endoscopic endonasal surgery for a posterior clinoidectomy, a carotid artery injury may occur. We summarize a method to safely navigate in the presence of the carotico-clinoid foramen and interclinoid bridge in an endoscopic endonasal approach. Study Design The study involves cadaveric dissection and management of the anatomical variants. Setting The study took place in a cadaveric dissection laboratory. Participants The object of study is one cadaveric head. Main Outcome Measures After discovering the two variants in both cavernous sinuses of a cadaveric head, we established a stepwise coping strategy to avoid carotid artery injury while performing an endoscopic endonasal bilateral interdural pituitary transposition, and we report the final view after endoscopic management. Results Debulking of the middle clinoid process can decrease the obstacle effect, and the pituitary transposition can be performed medial to the ossified carotico-clinoid ligament. Disconnection of the interclinoid bridge is the prerequisite to an effective posterior clinoidectomy, and distinguishing the transition between the sellar diaphragm and the interclinoid bridge is essential. Conclusion In the presence of both the carotico-clinoid foramen and the interclinoid bridge, a bilateral interdural pituitary transposition can still be performed, although preoperative strategic preparation, full inspection, and stepwise disconnections are of paramount importance in such a scenario to avoid cavernous carotid artery injury.
Endoscopic Intradural Subtemporal Keyhole Approach with Neuronavigational Assistance to the Suprasellar, Petroclival, and Ventrolateral Brainstem Regions: an Anatomic Study.

An intradural Kawase approach and a navigation-assisted intradural Kawase approach were then carried out on a random side of each specimen. Related anatomic structures were observed through 4-mm 0° and 30° endoscopes; data were measured by a frameless navigational device. In addition, 2 types of the milled Kawase rhombus ranges and the thicknesses of the residual bones were compared.RESULTS:This keyhole endoscopic technique clearly visualized the anatomic structures in the suprasellar, petroclival, and ventrolateral brainstem regions via an intradural subtemporal route. Most of the structures in the suprasellar region were observed by the oculomotor nerve-posterior communicating artery space. Using a frameless navigational device, the mean area of the milled Kawase rhombus increased by 32.04 mm2 (P < 0.05), and the thicknesses of the residual bones including the superior wall of the internal acoustic meatus, the medial superior wall of the cochlea, and the superior wall of the petrous segment of the internal carotid artery reduced by 1.55, 2.03, and 2.19 mm on average, respectively (P < 0.05).CONCLUSIONS:The endoscopic intradural subtemporal keyhole approach could facilitate excellent observation of the suprasellar, petroclival, and ventrolateral brainstem regions with less invasiveness. More anatomic exposure and surgical freedom could be achieved via neuronavigational assistance with fewer complications.

Computer Methods in Biomechanics and Biomedical Engineering: Imaging and Visualization

Abstract:The surgical treatment of complex intracranial aneurysms is still one of the main challenges for neurosurgeons worldwide. Computational fluid dynamics (CFD) could be used to predict different postoperative flow scenarios prior to surgery, in order to expand neurosurgeons' evaluations and guide surgical decision-making. However, this possibility has been marginally reported in the literature so far. The aim of this study was then to assess the feasibility of using CFD simulations to characterise blood flow rate, pressure and wall shear stress scenarios resulting from alternative surgical options for the treatment of complex intracranial aneurysms. In particular, three alternative surgical solutions were studied and compared to the preoperative condition on a relevant case study of saccular aneurysm. In this case, CFD results suggested the clipping of the aneurysm as the preferred choice, which is also the most common procedure for these types of lesion and coincided with neurosurgeon's choice. This case report suggests that CFD modelling can be used to predict postoperative flow in a complex aneurysm and can actually provide valuable additional information for surgical planning.

Abstract:Natural organic matter (NOM) was concentrated from Songhua River using a reverse osmosis system. The hydrophobic fraction (HyO), hydrophilic fraction (HyI), fulvic acid (FA) and humic acid (HA) were fractionated by the XAD resin, and a commercial humic acid (CHA) was used for comparison. The FA fraction predominant in natural water and account for the large part of DOC, and the aromaticity of these fractions followed the order HA> HyO> FA> HyI. A weak relationship exists between the E4/ E6 and aromaticity. FTIR spectra revealed that the degree of saturation among these fractions was in the order of HyO = FA > HyI > HAM=CHA. Gel chromatograph results showed that the weaker ultraviolet-adsorption fraction of NOM contributed to the most part of molecular weight. Potentiometric titration indicated that the total organic acidity was in the range of 9.84 - 12.15 meq/g, and carboxylic acidity decrease in the order of CHA> HyI > HyO > NOM > FA > HA. 1H NMR spectra demonstrated that NOM and the isolated fractions were predominated by aliphatic functional groups.

Abstract:Advances in computational fluid dynamics continuously extend the comprehension of aneurysm growth and rupture, intending to assist physicians in devising effective treatment strategies. While most studies have first modelled intracranial aneurysm walls as fully rigid with a focus on understanding blood flow characteristics, some researchers further introduced Fluid-Structure Interaction (FSI) and reported notable haemodynamic alterations for a few aneurysm cases when considering wall compliance. In this work, we explore further this research direction by studying 101 intracranial sidewall aneurysms, emphasizing the differences between rigid and deformable-wall simulations. The proposed dataset along with simulation parameters are shared for the sake of reproducibility. A wide range of haemodynamic patterns has been statistically analyzed with a particular focus on the impact of the wall modelling choice. Notable deviations in flow characteristics and commonly employed risk indicators are reported, particularly with near-dome blood recirculations being significantly impacted by the pulsating dynamics of the walls. This leads to substantial fluctuations in the sac-averaged oscillatory shear index, ranging from −36% to +674% of the standard rigid-wall value. Going a step further, haemodynamics obtained when simulating a flow-diverter stent modelled in conjunction with FSI are showcased for the first time, revealing a 73% increase in systolic sac-average velocity for the compliant-wall setting compared to its rigid counterpart. This last finding demonstrates the decisive impact that FSI modelling can have in predicting treatment outcomes.

Endoscopic management of a cavernous malformation on the floor of third ventricle and aqueduct of Sylvius: Technical case report and review of the literature

Abstract:Background: Intraventricular cavernous malformations are unusual intracranial vascular malformations; their deep anatomical location complicates their surgical management. Microsurgical approaches are the gold standard approaches for the resection of ventricular lesions, however, they imply considerable neurovascular risks. Case description: A 51-year-old patient presented with acute headache, diplopia, vertigo, blurred vision, and a depressed level of consciousness. A ventricular hemorrhage was treated with a ventriculostomy and the patient was discharged without hydrocephalus. After 11 days, he developed ataxia, diplopia, and a depressed level of consciousness. The patient was diagnosed with hydrocephalus secondary to the previous third ventricle hemorrhage. An endoscopic exploration using a 30° rigid ventricular endoscope was performed; after the third ventriculostomy, an intraventricular cavernous malformation located on the floor of the third ventricle and the aqueduct of Sylvius was resected. Conclusions: Three days after the surgery, magnetic resonance imaging demonstrated a gross total resection and adequate third ventriculostomy flow. One year after the surgery, the patient was asymptomatic. Neuroendoscopy has evolved towards minimally invasiveness, and in selected cases is an equally effective surgical approach to ventricular lesions. It provides minimal cerebral cortex disruption and vascular manipulation.
