Abstract:Abstract Background: To describe the effectiveness of Ixekizumab on a patient with ERA who was refractory to conventional treatment and TNF inhibitors. Case presentation: A 15-year-old boy who manifested as swollen knees, bilateral ankle arthritis, sausage toes and low-grade fever was diagnosed as Enthesitis related arthritis. There was one episode of uveitis three years before the onset of joint symptoms. The patient had a high level of inflammation (ESR to 107 mm/hour, CRP to 248.77 mg/L) with increased level of serum IL-6 and VEGF. Combination therapy of NSAIDs with sulfasalazine, Adalimumab, Infliximab, low dose steroid, Golimumab and Tofacitinib were applied correspondingly. The systemic inflammation of this patient (not including swollen toes and enthesitis) got a little better with oral steroids, but intensified again when steroid was weaned. Ixekizumab (80mg, Q2W twice, then 80mg Q4W) combined with oral methotrexate were then applied for this patient. ESR and CRP decreased into normal level with quick alleviation of arthritis within the first month of treatment. A persistent clinical remission was achieved by using Ixekizumab in the following two years of therapy. Apart from slight redness and swelling at the injection site, no significant side effects were observed during follow up. Conclusion: Ixekizumab can be a substitutive therapy for ERA patients who were unresponsive to traditional therapy or anti-TNF treatment.
Temperature Gradients Between Arterial Blood and Brain in the Monkey.∗

Abstract:Summary Temperature measurements were made simultaneously in the arterial blood and various brain structures in 5 chronically prepared rhesus monkeys at rest in a lighted, sound-attenuated, thermoregulated chamber. The level of alertness of the animals was also noted by observing changes in overt behavior together with EEG and EMG recordings. Spontaneous and induced shifts in the level of arousal were followed in seconds by warming or cooling of the arterial blood and seconds to minutes later by parallel shifts in the temperature of the majority of the brain sites. Arterial blood temperature was thus found to yield a sensitive index of the behavioral state of the monkey in relation to sleep and arousal. Changes in the mean temperature of the arterial blood going to the brain appeared primarily responsible for temperature fluctuations in the brain. A number of steadily maintained but differing thermal gradients were found to exist between regional brain sites and the arterial blood.

More recently, abrocitinib, an oral JAK 1 inhibitor, has shown promising clinical efficacy in the treatment of various skin disorders beyond moderate to severe atopic dermatitis (AD). We firstly presented three cases, each with diagnosis of pyoderma gangrenosum (PG), livedoid vasculopathy (LV), or hidradenitis suppurativa (HS), and conducted a comprehensive scoping review of the available literature on the use of abrocitinib in the treatment of diverse skin disorders. We summarized a total of 16 skin disorders, including our cases. The results indicated that abrocitinib, whether used as monotherapy or in combination with other treatments, was effective and well-tolerated in these disorders. These findings expanded the range of diseases for which abrocitinib may serve as an alternative therapeutic choice. Keywords: JAK inhibitors, abrocitinib, atopic dermatitis, pyoderma gangrenosum, livedoid vasculopathy, hidradenitis suppurativa The Janus kinase (JAK) family is composed of four types of cytoplasmic tyrosine kinases: JAK1, JAK2, JAK3, and tyrosine kinase 2 (TYK2), which mediate signal transduction from cell surface receptors to the nucleus. 1 The JAK pathways play a crucial role in response to over 50 inflammatory cytokines and are also involved in the differentiation of naive T cells into various types of immune cells, such as Th1, Th2, and Th17 cells. 2 These processes are closely associated with the pathogenesis of a wide range of inflammatory immune-mediated diseases, 3 making these conditions theoretically treatable by JAK inhibitors. Abrocitinib, inhibitor of JAK1, has recently shown promising results in the off-label treatment of some skin diseases associated with JAK activity, including alopecia areata (AA), and others. 4,5 Herein, we present a case series comprising three patients, each diagnosed with pyoderma gangrenosum (PG), livedoid vasculopathy (LV), or hidradenitis suppurativa (HS). These patients exhibited a favorable response to abrocitinib following inadequate outcomes with conventional therapies. Furthermore, we conducted a scoping review to provide a comprehensive summary of the current evidence concerning the off-label use of abrocitinib in dermatologic disorders. Patient A, a 16-year-old male, was admitted with growing, painful, ulcerative lesions that started as erythematous papules in perianal area for over 1 year. Physical examination showed multiple, painful swollen, centrally located deep ulcerations with massive purulent discharges (Figure 1Aa). A skin biopsy revealed extensive infiltration of lymphocytes and neutrophils, increased vascular proliferation and dilation (Figure 1Ab), which was consistent with the diagnosis of PG. The patient was prescribed doxycycline, isotretinoin, glucocorticoids, and cyclosporine A, but his symptoms continued to worsen. We initiated treatment with oral abrocitinib (100 mg daily) in combination with cyclosporine A (50 mg twice daily). The patient showed gradual improvement of his symptoms within one week (Figure 1Ac), and almost complete resolution of painful swelling and purulent discharges, significant reduction of the ulcer area and depth after four weeks (Figure 1Ad). Cyclosporine A was discontinued due to the good clinical response, and the patient continued to improve without any adverse events (AEs) over the 16-week follow-up. Abstract:Autoimmune bullous diseases, connective tissue diseases, and vasculitis represent a group of severe rare skin diseases. While glucocorticoids and immunosuppressive agents serve as standard treatments for these diseases, their efficacy is limited due to adverse side effects, indicating the need for alternative approaches. Biologics have been used in the management of some rare skin diseases. However, the use of biologics is associated with concerns, such as infection risk and high costs, prompting the quest for efficacious and cost-effective alternatives. This study discusses the safety issues associated with tofacitinib and its potential in treating rare skin diseases.

Abstract:Abstract Autosomal recessive congenital ichthyosis (ARCI) is a genetically heterogeneous disorder manifesting aberrant skin scaling and increased transepidermal water loss (TEWL). Current treatments for ARCI are limited and sub-optimal. We studied a 27-year-old man with ARCI resulting from a homozygous missense variant in TGM1 (transglutaminase 1). RNA-sequencing of lesional skin revealed aberrant JAK-STAT signalling, providing a rationale for innovative treatment with a Janus kinase inhibitor. We prescribed oral tofacitinib (11 mg daily) for 26 weeks. Rapid improvements in erythema and fissuring manifested within the first month. Sustained reductions in 5-D itch scale and Dermatology Life Quality Index (DLQI) scores were also observed. TEWL decreased for the first 10 weeks but increased thereafter. Tofacitinib down-regulated inflammatory genes and pathways, while enhancing skin barrier markers. Moreover, TGM1 distribution was normalized although enzymatic activity remained deficient. This study suggests that oral tofacitinib may be a useful therapy to consider in patients with ARCI.

Abstract:Lichen amyloidosis (LA) is a predominant type of primary cutaneous amyloidosis that is characterized by persistent and intense skin itching. Although multiple therapeutics strategies are available for its treatment, there is no standard treatment so far. Abrocitinib, an oral small-molecule Janus kinase 1 inhibitor, has been authorized for the treatment of severe atopic dermatitis (AD) and can also provide rapid relief from pruritus. Here, we discuss the case of a 32-year-old man who was diagnosed with LA with severe AD based on the presence of multiple, discrete, and hyperpigmented papules and pruritic, erythematous macules with lichenification of the limbs, trunk, and buttocks. Given the inefficacy of conventional therapy, abrocitinib treatment was recommended in this patient. After 1 month of treatment, the patient's Eczema Area And Severity Index score decreased significantly from 48 to 15 points, accompanied by a notable reduction in pruritus symptoms. Furthermore, significant improvements were observed in the thickness and pigmentation of the hyperkeratotic papules. Thus, abrocitinib exhibited excellent effectiveness and safety in the treatment of severe AD with LA and warrants further investigation for its potential therapeutic benefits.

Abstract:In clinical practice, some cases of chronic spontaneous urticaria (CSU) remain difficult to treat, with up to 40% of patients showing no response to even high (4-fold) daily doses of antihistamines. Approximately 30% of CSU patients fail to achieve complete control and relief through treatment with omalizumab and may require alternative therapies. Abrocitinib is a small-molecule oral JAK1 inhibitor that suppresses intracellular signaling of multiple key cytokines involved in inflammation cascades, and has shown beneficial effects in patients with mast cell activation disorders. We conducted a retrospective analysis of the diagnosis and treatment records of adult patients with refractory CSU who were treated with abrocitinib after inadequate response to omalizumab (defined as no response to 300 mg/4 weeks of omalizumab treatment for 6 months, followed by adjustment to 300 mg/2 weeks for a further 3 months without controlling symptoms). We also collected data on relevant treatment modalities, clinical outcomes, and adverse events. Among these patients, various treatment modalities failed to adequately control symptoms, but switching to abrocitinib significantly improved clinical outcomes. Therefore, abrocitinib may represent a new treatment option for patients with refractory CSU.

Abstract:Purpose: At present, we have entered the era of using biological agents and small molecule targeted drugs to treat diseases. Although there have been many reports of biological agents treating pityriasis rubra pilaris recently, the clinical application of the JAK inhibitors in the treatment of pityriasis rubra pilaris has been rarely reported, and there is a lack of evidence on the safety and efficacy of these drugs. We explore the use of the JAK inhibitor tofacitinib in the treatment of pityriasis rubra pilaris with significant efficacy and no significant side effects, providing new ideas for the clinical treatment of pityriasis rubra pilaris. Methods: We cover a case of pityriasis rubra pilaris treated with the JAK inhibitor tofacitinib, which showed significant efficacy without any adverse effects. Results: This case report showed that the JAK inhibitor tofacitinib had significant clinical efficacy in the treatment of pityriasis rubra pilaris. We speculated that the treatment of pityriasis rubra pilaris with the JAK inhibitors may be related to blocking the activation of the JAK/STAT pathway, thereby blocking the high expression of cytokines IL-17, IL-12/IL-23, IL-23, TNF-α. Conclusion: The JAK inhibitor tofacitinib can become a new option for treating pityriasis rubra pilaris.

Abstract:This study presents a report on the first clinical experiences gained in 68 hospitalized HIV antibody-positive patients from Austria, covering a period of twelve months. 36 patients (52.9%) belonged to risk group I or Ib (homo- or bisexual), whereas 26 (38.2%) patients were i.v. drug abusers (risk group II). 5 (7.4%) patients fulfilled the criteria of stage II of the CDC classification of HIV-associated clinical symptoms, 10 (14.7%) were classified as stage III and the remaining 53 patients (78%) as stage IV. The most frequent and also the most serious problem was the development of opportunistic infections. Multiple infections were found in 45.7% of all cases. Kaposi's sarcoma was found in 9 patients who all belonged to risk group I. During the entire observation period 10 patients died as a consequence of HIV-1-induced immunodeficiency and the resulting opportunistic infections and/or neoplasms.

Abstract:Rheumatoid arthritis (RA) is a heterogeneous chronic disease. RA patients should start disease modifying anti-rheumatic drugs (DMARDs) therapy immediately after diagnosis. If first-line treatment with conventional synthetic DMARDs does not relieve the disease, biology and targeted synthetic DMARDs are options for patients. Patients can switch to different types of biological and targeted synthetic DMARDs if remission is not achieved. However, for patients with difficult-to-treat RA, achieving disease stabilization after the failure of multiple biological and targeted synthetic DMARDs is a clinical challenge that needs to be addressed. As distinct cytokine pathways, the benefits and challenges of dual therapy are worth discussing. As the most extensively used biologic DMARDs, adalimumab is an anti-tumor necrosis factor monoclonal antibody used to treat RA. Tofacitinib, as a Janus Kinase inhibitor, is an orally administered targeted synthetic DMARDs that involved in the regulation of immune responses by directly or indirectly inhibiting cytokine pathways. This report describes a successful case of a 48-year-old woman with difficult-to-treat RA who treated with Tofacitinib combined with adalimumab. She had been on glucocorticosteroid for a long time, but had persistent joint pain and fatigue. At more than one year of follow-up, her Disease Activity Score for 28-joint counts based on the erythrocyte sedimentation rate (DAS28-ESR) remained in complete remission, and she discontinued her glucocorticosteroid medications. Also, she did not develop a mycobacterial tuberculosis infection, herpes zoster, and new-onset cardiovascular events.

Abstract:Tocilizumab is a humanised monoclonal antibody to the interleukin-6 (IL-6) receptor which was first approved for use in refractory rheumatoid arthritis almost a decade ago. Since then, its use has expanded to a number of rheumatological and inflammatory conditions. In dermatology, off-label use of tocilizumab has been reported to be efficacious in morphoea, systemic sclerosis, psoriasis, atopic dermatitis, vitiligo, graft-versus-host disease, pyoderma gangrenosum, Behcet's disease, Schnitzler's syndrome, sarcoidosis, and cutaneous adverse reactions. That being said, the evidence demonstrating tocilizumab's efficacy in dermatology is predominantly low-level case-based evidence, and one must consider the potential for publication bias.In this review we will discuss the reported clinical applications of tocilizumab in dermatology, mechanisms of action, and the range of associated adverse effects (both cutaneous and non-cutaneous) that can occur. Abstract:The polarization plane of a laser beam is made to rotate at high frequency with the help of a special setup containing wave plates and an acoustooptic modulator. The application of this beam for ellipsometric measurements is investigated and a number of applications are proposed. We describe some novel approaches for the analysis of thin films and optical surfaces and for measurements on static and time-varying anisotropic phenomena such as the electrooptic effect, optical activity, and strain analysis using the photoelastic effect.

Abstract:Abstract Nickel hyperaccumulator plants contain at least 1,000 µg Ni/g dry mass. Their high-Ni tissues present a chemically unique resource for herbivorous insects. A prior survey of insects associated with the Ni hyperaccumulator Streptanthus polygaloides in California yielded a new species, Melanotrichus boydi Schwartz and Wall (Hemiptera: Miridae). Here we document the host preference of M. boydi. Surveys of 10 S. polygaloides populations across its geographical and elevational ranges documented the presence of M. boydi upon S. polygaloides at all sites. A host-choice experiment contrasting M. boydi with the polyphagous mirid Lygus hesperus (both collected from S. polygaloides) showed M. boydi preferred S. polygaloides but L. hesperus did not. Other host-choice studies showed M. boydi preferred S. polygaloides to 2 other ultramafic soil plant species in the Brassicaceae, including another species of Streptanthus. No preference was observed when M. boydi was offered samples from several populations of S. polygaloides or given a choice between high-Ni and low-Ni S. polygaloides plants. These results show that M. boydi feeds primarily on S. polygaloides but lacks population-level specificity. This is the first report of a North American insect species monophagous on a Ni hyperaccumulator species.
